In this case-report we describe the first reported case of early-onset fungal interface keratitis (IK) after Descemet Membrane Endothelial Keratoplasty (DMEK) successfully treated with penetrating keratoplasty (PK) during the active stage of infection. A patient with graft failure after Descemet Stripping Automated Endothelial Keratoplasty (DSAEK) was operated on with DMEK. Donor rim culture and broth were positive for Candida albicans. Several interface infiltrates were confirmed and localized using anterior segment optical coherence tomography. Three days after diagnosis, observing clear signs of intraocular infection, the graft was removed with simultaneous washed-up of anterior chamber with fluconazole 1% followed by a PK and intrastromal corneal injections of fluconazole. A best-corrected visual acuity of 20/20 was achieved. This case highlights the importance of analysing every donor rim and broth, despite the patient doesn't show any symptoms or signs during the post-operative period. PK is a viable treatment option in early-onset interface keratitis.

Every year, about ten glaciers in the Western Pamirs are in the active stage of movement. The time from the beginning of the movement of kinematic waves along the glacier to the full completion of the pulsation takes, as a rule, 1– 2 years, and in some cases lasts up to 5 years. Activity of the Pamir glaciers in 2020 is discussed in this paper, and we suppose that some pulsations are still in progress in the following years. Data from a number of automatic satellite instruments were used for the analysis, but mainly these were obtained from the International Space Station. In 2020, 10 glaciers in the basins of the Surkhob, Muksu, Seldara, Kyzylsu and Vanch rivers became more active, and in some cases surged. Similar dynamic instability of glaciers was also characteristic for the preceding four years. At present, several major surges are taking place in the Western Pamirs; the Byrs, Vali, Lenin, and Medvezhy glaciers started the active phase of their developments. Therefore, it is necessary to study them by field methods and continue permanent monitoring of them from automatic satellites and the International Space Station.

Background: Treatment options in patients with an acute Charcot foot is not well standardized and still challenging.Aims: To evaluate the results of the non-operative treatment of patients with active stage of diabetic charcot arthropathy in outpatient foot clinic and to identify factors influenced on treatment results.Materials and methods: Medical files of 141 patients with unilateral ulcer-free active charcot arthropathy were reviewed. 78 patients agreed for treatment (total contact cast or walker), 63 patients refused of treatment (follow-up only). The time of resolution of the acute stage, severity of final deformities and foot-related complications were evaluated. Refusers were asked about the cause of their decision.Results: Mean delay of the diagnosis was 3,2±2,8 months. Main causes of the refuse were: mistrust to doctor - 38%, problems with employment - 33%, home/family problems - 18%, medical reasons - 11%. Median healing times (months) in the treatment group vs controls: 9 (6 - 19) 15 (13 - 25) (p=0,001) and progression of the initial deformity: 14% and 35%, respectively (p=0,01). Foot-related complications: 17,5% in the treatment group and in 55,6% of controls (including 6 amputations) (p=0,001). Frequency of breaking the cast/walker - 32%/16,6%. Foot lesions due to cast/walker: 13,6%/41,7% (pConclusions: The significant delay of diagnosis was revealed. The refuse rate in our cohort was high and main causes of the refuse were social and psychological. Neglect of treatment leads to high frequency and severity of foot-related complications. We noticed high rate of breaking of walkers and casts and device-related foot lesions. Total contact casts were more effective and safe compared with walkers.

BACKGROUND Pregnancy in the setting of systemic lupus erythematosus can worsen the condition from the stable to active stage, with quality of life and fertility desire being particular concerns. Pregnancy in the active stage of systemic lupus erythematosus (ASLE), although rare and complicated to manage, can be treated favorably with immunotherapies ifs used properly. Here we report such a success case. CASE SUMMARY A 31-year-old primigravida patient, diagnosed with SLE seven years ago, was induced ASLE after a cold at 21 + weeks. The patient’s vital signs on presentation were normal. Her laboratory exam was remarkable for significant proteinuria, liver and renal dysfunction, and low C3 and C4 levels. Infectious work-up was negative. The patient was diagnosed with ASLE. She was given immunosuppressive agents (methylprednisolone, gamma globulin and azathioprine etc.) and plasma adsorption therapy, monitoring blood pressure every 8 h, fetal heart rate twice a day, and liver and renal function at least twice a week. Successful maternal and fetal outcomes are presented here. CONCLUSION Child-bearing in ASLE has become more promising, even for this difficult case of ASLE with multiple organ damage. Thorough antepartum counseling, cautious maternal-fetal monitoring, and multi-organ function monitoring by multidisciplinary specialties are keys to favorable pregnancy outcomes.

Background This clinical study investigates the role of 5-methoxytryptophan (5-MTP) in pediatric systemic lupus erythematosus (SLE), with a particular interest in lupus nephritis (LN). Patients and methods One hundred ten children with SLE were enrolled in the cohort study. Among the patients, seventy-seven (70%) had active LN and thirty-three (30%) were not present with LN during their first visit to the clinic. The diagnoses of LN were biopsy-proven. Serum samples were collected before and after administration of immunosuppressive medications to evaluate 5-MTP levels and regular laboratory data. Data were analyzed longitudinally. Results Before any treatment started, patients with active LN had significantly higher 5-MTP levels as compared to patients with no LN (1.021 ± 0.709 vs. 0.719 ± 0.606, P = 0.0456). Also, in patient with active LN, 5-MTP level was significant decreased after treatment, compared with the levels before treatment (1.021 ± 0.709 vs. 0.802 ± 0.597, P = 0.0484). Patients who reached complete remission also had significantly higher initial serum 5-MTP levels than that in patients with no remission (1.244 ± 0.784 vs. 0.846 ± 0.556, P = 0.0488). There was an overall reduction in 5-MTP levels after six months of immunosuppressive treatment, regardless of the disease outcome. Subgroup analysis further revealed a significantly higher 5-MTP level during the active stage of LN (1.127 ± 0.149 vs. 0.742 ± 0.092, P = 0.0384). Conclusion We demonstrated that serum 5-MTP level is positively correlated to the disease activity, prognosis, and remission status of pediatric LN in vivo.

Hydrographic observations, carried out in March-May, 2019 during “Transarktika-2019” expedition onboard R/V “Akademik Tryoshnikov” allowed studying mechanisms of Atlantic Water (AW) transformation in the Barents Sea. Although this research topic is rather traditional for oceanographic studies, there are still a number of questions, which require clarification. Among these is a deeper understanding of the AW transformation in specific regions in cold season, when the coverage by observations is scarce. In this study we performed temperature and salinity (TS) analysis of conductivity — temperature — depth (CTD) data, collected in the north-eastern “corner” of the Barents Sea — this is the area with difficult access in winter due to high concentration of pack ice. The results allowed identification of areas along the pathways of AW branches, where various types of open sea convection and cascading acted as dominant processes of AW properties change. We distinguish several driving mechanisms controlling modification of the waters of Atlantic origin. An advantage of winter measurements is that the active stage of AW transformation mechanisms is explicitly observed at the consecutive CTD sections.

Purpose.To present a clinical case of leprosy exacerbation on the background of ongoing therapy. Materials and methods.A 52-year-old patient with a diagnosis of "lepromatous (cutaneous) leprosy, leprosy LL" (multi-bacterial leprosy, lepromatous form, active stage), has an exacerbation in the form of nodular erythema at 3.5 years after the start of treatment. Due to the exacerbation of the leprosy process, dexamethasone therapy was performed intravenously in a dose of 4 mg/ml 3.0 ml + 0.9% NaCl 200.0 ml daily No. 10. Results. An exacerbation was diagnosed leprosy nodular erythema. The prescription of adequate therapy led to a complete regression of clinical manifestations. Conclusion.The described case is presented in connection with the rarity of this dermatosis.

Introduction: Vitiligo is a common acquired pigmentary disorder characterized by well defined hypopigmented or depigmented macules due to absence or decrease in melanocytes. Aim: To study the clinico epidemiological pattern of vitiligo in children. Materials and Methods: A group of 50 consecutive children attending out patient in the department of DVL at Shadan Institute of Medical Sciences will be included. All the patients meeting the inclusion criteria were taken into the study. A designed, coded proforma was used for recording all the findings. Results: Almost half of the children taken for study belongs to age group of 9-11 years. Majority of the study population were females. The most common pattern of vitiligo was vitiligo vulgaris. Common site of involvement was lower limbs. The most common associated feature was leukotrichia. The most common associated condition was photosensitivity. Family history was positive in only 4% of cases. Most of them were in the active stage of disease. Conclusion: The female predominance was high in prevalence of vitiligo in children and an earlier age of onset seen among children with family history of vitiligo or autoimmune disorders. Vitiligo vulgaris is the most common presentation, and lower limbs being the most common site of involvement in children. Keywords: Childhood, Vitiligo.

Purpose Ulcerative colitis (UC) is a chronic inflammatory immune-related disease. Imbalance between pathogenic cells and immunosuppressive cells is associated with disease activity of UC. Regulatory T cells (Tregs) are critical for this immune homeostasis. However, the clinical significance of CD226 and TIGIT expressions on FoxP3+Tregs in UC remains unclear. Methods Comprehensive analyses of CD226 and TIGIT expressions on FoxP3+Tregs were performed by flow cytometry in 72 UC patients and 35 healthy controls, and ten active UC patients achieving remission after treatment with 5-aminosalicylic acid were followed up. Expressions of β7, α4 and αE on FoxP3+Tregs were analyzed. Clinical indicators were retrospectively acquired and serum cytokines were detected using ELISA, and their correlations with FoxP3+Treg subsets were conducted. Results In active UC, levels of FoxP3+Tregs and CD226-FoxP3+Tregs regardless of TIGIT expression were significantly decreased while percentages of CD226+TIGIT-FoxP3+Tregs and CD226+TIGIT+FoxP3+Tregs were obviously increased. The expressions of β7, α4β7 and αEβ7 in FoxP3+Tregs, CD226-TIGIT+FoxP3+Tregs and CD226-TIGIT-FoxP3+Tregs were significantly elevated in active UC. Furthermore, inverse correlations were found between FoxP3+Tregs, CD226-TIGIT+FoxP3+Tregs, CD226-TIGIT-FoxP3+Tregs and serum CRP, as well as Mayo scores. IL-10 was down-regulated and positively correlated with CD226-TIGIT+FoxP3+Tregs and CD226-TIGIT-FoxP3+Tregs while IL-12 was up-regulated and negatively correlated with CD226-TIGIT+FoxP3+Tregs and CD226-TIGIT-FoxP3+Tregs in active UC. In follow-up patients, the levels of FoxP3+Tregs, CD226-TIGIT+FoxP3+Tregs and CD226-TIGIT-FoxP3+Tregs and serum IL-10 levels were significantly recovered when achieving remission after treatment. Conclusions Lack of CD226 expression on FoxP3+Tregs regardless of TIGIT expression may play an important role in exhibiting their suppressive function and preventing from disease activity in UC.

Combination of active stage of diabetic Charcot neuroosteoarthropathy and diabetic lower limb macroangiopathy is the rare condition. In the present paper we describe two cases of development of acute Charcot foot in the non-critically ischemic foot. The first case is the patient with previously diagnosed intermitted claudication and the second case is patient who developed the Charcot foot 5 months later after successful endovascular treatment of arterial occlusions of his left lower limb. In both cases the absence of redness in the early stage, the mild-to-moderate pain and mild temperature gradient between affected and non-affected feet were noticed. The clinical course of the Charcot disease in the first patient was favourable. He used walker for 9 months and his foot shape was preserved and deformity was considered as mild. The second patient had more active and profound destructions due to delay of the treatment. He was casted, however his deformity progressed and the treatment continues up to date. In both patients the MRI revealed more affected bones compared with X-ray. These cases emphasize the importance of keeping in mind the Charcot disease even in patients with diabetic peripheral vascular disease.

Sleep is a conserved behavior across the evolutionary timescale. Almost all known animal species demonstrate sleep or sleep like states. Despite extensive study, the mechanistic aspects of sleep need are not very well characterized. Sleep appears to be needed to generate resources that are utilized during the active stage/wakefulness as well as clearance of waste products that accumulate during wakefulness. From a metabolic perspective, this means sleep is crucial for anabolic activities. Decrease in anabolism and build-up of harmful catabolic waste products is also a hallmark of aging processes. Through this lens, sleep and aging processes are remarkably parallel— for example behavioral studies demonstrate an interaction between sleep and aging. Changes in sleep behavior affect neurocognitive phenotypes important in aging such as learning and memory, although the underlying connections are largely unknown. Here we draw inspiration from the similar metabolic effects of sleep and aging and posit that large scale metabolic phenotyping, commonly known as metabolomics, can shed light to interleaving effects of sleep, aging and progression of diseases related to aging. In this review, data from recent sleep and aging literature using metabolomics as principal molecular phenotyping methods is collated and compared. The present data suggests that metabolic effects of aging and sleep also demonstrate similarities, particularly in lipid metabolism and amino acid metabolism. Some of these changes also overlap with metabolomic data available from clinical studies of Alzheimer’s disease. Together, metabolomic technologies show promise in elucidating interleaving effects of sleep, aging and progression of aging disorders at a molecular level.

Porous materials have attracted great attention in energy and environment applications, such as metal organic frameworks (MOFs), metal aerogels, carbon aerogels, porous metal oxides. These materials could be also hybridized with other materials into functional composites with superior properties. The high specific area of porous materials offer them the advantage as hosts to conduct catalytic and electrochemical reactions. On one hand, catalytic reactions include photocatalytic, photoelectrocatalytic and electrocatalytic reactions over some gases. On the other hand, they can be used as electrodes in various batteries, such as alkaline metal ion batteries and electrochemical capacitors. So far, both catalysis and batteries are extremely attractive topics. There are also many obstacles to overcome in the exploration of these porous materials. The research related to porous materials for energy and environment applications is at extremely active stage, and this has motivated us to contribute with a roadmap on ‘porous materials for energy and environment applications’.

Background: Cost effective and benefit equations are important in health care. When discussing alternative options of care nipple stimulation can be a standardised methodology for induction of labour. In accordance to it, a quasi-experimental post-test only control group design, was carried out in selected hospitals in Jabalpur city to assess the effect of nipple stimulation. The basic purpose was to use nipple stimulation as a means of labour induction allowing greater control by mothers and has an advantage of being natural, inexpensive and non-invasive. Methods: Purposive sampling technique was used for study. 60 subjects were divided into two groups: control and experimental, each group 30 sample (primipara women). The experimental group was given nipple stimulation for 15 min alternatively during active phase. The result was determined by modified BISHOPS SCORE and DURATION of active stage of labour (4-7cm dilatation). Results: The principle result findings stated that, modified BISHOP score was satisfactory is 23(76.6%) in control group and 27 (90%) in experimental group. Partially satisfactory were 7 (23.4%) in control group and 3 (10%) in experimental group and none not satisfactory. The duration of labour in control group came to be 12(40%) subjects had frequency less than 4 hrs and 18 (60%) had had duration more than 4hrs.whereas in experimental group 19(64.3%) had duration less than 4hrs and only 11(36.6%) had duration more 4hrs. The effectiveness of nipple stimulation in experimental group was statistically proved by t test. Conclusion: Hence it could be concluded than, nipple stimulation was effective in early effacement and dilatation of cervix during first stage of labour in active phase among primipara women.

The clinical CD activity index (CDAI) was used as the disease activity grouping standard, 29 patients with CD were grouped into activity groups, 18 patients in the active group (CDAI ≥ 150) with 36 intestinal segments, and 11 patients in the remission group (CDAI < 150) with 20 intestinal segments.The virtual single energy CT value, slope of energy spectrum curve and iodine content were analyzed to evaluate the evaluation of intestinal CD activity by DECTE.Results: There were statistically significant differences in virtual single energy CT value (except 90 keV and 100 keV virtual single energy CT value), curve slope and iodine content between remission group and active group (P < 0.05), and has more diagnostic value for active phase (AUC > 0.5). ①Virtual single energy CT value: the AUC of 60 keV in arterial phase was the highest (0.924). The specificity of diagnosing CD in active stage was high (95%). ② Curve slope: the AUC of portal vein phase was the largest (0.731). The specificity of diagnosing CD in active stage was higher (85%). ③ Iodine content: the AUC of arterial phase was the highest (0.885). The review presents data on damage to the organ of vision in patients recovered from the new coronavirus infection reflecting on the results obtained by various researchers from examining patients with varying severity of the disease, both during the active stage and after recovery. Possible ways of transmission of the infectious agent into the organ of vision were analyzed. The most common ocular manifestation of COVID-19 is conjunctivitis, while lesions of the retina and optic nerve were noted less often. The article also outlines the strategy for treatment and describes infection prevention measures for doctors and patients.В обзоре представлены данные о поражении органа зрения при новой коронавирусной инфекции. Отражены результаты, полученные разными авторами при обследовании пациентов с заболеванием различной степени тяжести, выполненном во время и после перенесенной болезни. Проанализированы возможные пути передачи инфекционного агента в орган зрения, рассмотрено наиболее частое проявление конъюнктивита, обусловленного COVID-19, обращено внимание на реже отмечаемые поражения сетчатки и зрительного нерва. Обозначена стратегия лечения, описаны методы профилактики заражения врача и пациента.

Purpose To evaluate the accuracy of single-photon emission computed tomography (SPECT) in diagnosing unilateral condylar hyperplasia (UCH) and to describe the condylar growth of patients with UCH. Materials and Methods Using a retrospective study design, patients with UCH who had undergone SPECT and cone-beam computed tomography (CBCT) examinations at the same time were included in the study. We used 3D medical models based on CBCT data as the gold standard. The SPECT results were compared with the model data, and the sensitivity and specificity were calculated. To further describe the condylar growth activity, statistical analysis was performed, and the P value was set at 0.05. Results The sample was composed of 75 patients. The sensitivity of SPECT was 55.3%, the specificity was 48.6%, and the area under the receiver operating characteristic curve was 0.53. There was no significant difference in sex between patients with and without active growth. Conclusion The sensitivity and specificity of SPECT are poor, and SPECT alone is not suitable for evaluating the active stage of condylar growth. 3D medical modeling has good prospects for application in the diagnosis of condylar hyperplasia.

To evaluate the new ultrasound-based signs for the diagnosis of post-cesarean section uterine niche in nonpregnant women.We investigated prospectively a cohort of 160 consecutive women with one previous term cesarean delivery (CD) between December 2019 and 2020. All women were separated into two subgroups according to different stages of labor at the time of their CD: subgroup A (n = 109; 68.1%) for elective CD and CD performed in latent labor at a cervical dilatation (≤4 cm) and subgroup B (n = 51; 31.9%); for CD performed during the active stage of labor (4 cm).Overall, the incidence of a uterine niche was significantly (P .001) higher in women who had an elective (20/45; 44.4%) compared with those who had an emergent (21/115; 18.3%) CD. Compared with subgroup B, subgroup A presented with a significantly (P = .012) higher incidence of uterine niche located above the vesicovaginal fold and with a significantly (P = .0002) lower proportion of cesarean scar positioned below the vesicovaginal fold. There was a significantly (P .001) higher proportion of women with a residual myometrial thickness (RMT) 3 mm in subgroup A than in subgroup B and a significant negative relationship was found between the RMT and the cervical dilatation at CD (r = -0.22; P = .008).Sonographic cesarean section scar assessment indicates that the type of CD and the stage of labor at which the hysterotomy is performed have an impact on the location of the scar and the scarification process including the niche formation and RMT.

Objective Quantitatively staging TAO using MRI remains limited. Our study aims to identify the cut-off signal intensity value for staging TAO using STIR sequence scan. Methods Between June 2018 and July 2020, a number of 51 patients with TAO (102 eyes) and 19 volunteer controls (38 eyes) were recruited. The clinical and biochemical parameters were measured in each patient. Disease activity was diagnosed based on the Clinical Activity Score (CAS). The signal intensities of extraocular muscles were scanned using short-tau inversion recovery (STIR) sequences from MRI. Results Compared to the inactive TAO patients and the controls, the signal intensity ratios (SIRs) of the superior rectus, inferior rectus, medial rectus, lateral rectus on STIR images were significantly increased in the active TAO patients. After adjustment for age and smokers, the SIRs of four extraocular muscles showed strong associations with CAS. By receiver operator characteristic (ROC) curve analysis, all four muscle SIRs demonstrated good efficiency for predicting disease activity [area under curve (AUC) 0.75–0.83, all P 2.9 in the inferior rectus showed optimal diagnostic value for staging the active TAO. Conclusions the signal intensity of extraocular muscles on STIR sequence was a good predictor for TAO activity. A cut-off SIR value of > 2.9 in the inferior rectus could be applied to evaluate the active stage of TAO.

This is a case report of successful endoscopic resection (ER) of a bleeding duodenal lipoma. An 85-year-old woman who was diagnosed with asymptomatic subepithelial tumor of the duodenum 3 years ago visited the emergency room with hematemesis and was admitted to our hospital. Emergent esophagogastroduodenoscopy revealed bleeding from an ulcer on the superior aspect of a subepithelial tumor measuring about 20 mm in diameter, at the superior duodenal angle. The ulcer was in the active stage (A1), with a visible vessel. The bleeding was controlled by ER of the tumor using a snare. The final pathological diagnosis was duodenal lipoma with mucosal ulceration. The patient showed no signs of bleeding for 10 days after the procedure; subsequently, she was discharged and followed up for regular checkups.

Background To investigate the concentrations of oxidative stress markers, 8-hydroxy-2′-deoxyquanosine (8-OHdG) and malondialdehyde (MDA), in tears and their correlation with the clinical activity score (CAS) in patients with Graves’ orbitopathy (GO) according to disease activity. Methods We recruited 27 participants with inactive stage GO, 35 participants with active stage GO, and 25 healthy controls without GO. The tear concentrations of 8-OHdG and MDA were determined by enzyme-linked immunosorbent assay. The correlation between CAS and the concentrations of tear 8-OHdG and MDA were analyzed according to the disease activity in the GO patients. Results The levels of 8-OHdG and MDA were 56.30 ± 16.81 ng/mL and 5.39 ± 1.31 pmol/mg, respectively, in the control subjects, and 123.46 ± 22.67 ng/mL and 13.59 ± 3.93 pmol/mg, respectively, in patients with inactive stage GO, and 215.14 ± 35.61 ng/mL and 22.52 ± 4.63 pmol/mg, in patients with active stage GO. BACKGROUND Term “active management of labour” means codified approach to manage women in labour. It aims at reducing discomfort and pain of mother, shortening duration of labour, and reducing cesarean section rate. Recently, much focus is around shortening duration of labour. It is the single factor which can reduce pain and discomfort of parturient lady. Role of Buscopan (hyoscineN-butylbromide) as labour accelerant has not been evaluated despite the fact that same pathways which mediate pain also mediate cervical dilation. 1 Factors responsible for cervical dilatation, other than parasympathomimetic mediators, like spasmolytic effect and relaxation of pelvic muscles, smooth muscle relaxation of cervix, softening and ripening of cervix, etc. are also affected by Buscopan. 2 Aims and Objectives: 1) Efficacy of Buscopan suppository in reducing duration of active stage of labour. 2) Safety of Buscopan suppository. MATERIALS AND METHODS Randomized controlled prospective study in primiparous women with singleton intra-uterine full-term pregnancy, who go into labour spontaneously, in tertiary care institute between 1 st Jan 2009 to 31st Dec 2009. In the study group, 25 women were given single Buscopan suppository 10 mg, at 3 cm cervical dilatation, post-amniotomy. 25 women who were not given any medication were taken as controls. RESULTS Mean duration of active stage of labour in study group was ~104 minutes less than the same in control group. Background To study the related factors of hypercoagulability in patients with inflammatory bowel disease (IBD) and their relationship with CRP, ESR and D-D, and to screen out the population with the highest risk of hypercoagulation, so as to prevent the formation of thrombosis in the early stage. Methods We directly extract data from the MIMIC-III and Philips eICU collaboration databases. 739 cases were met; the diagnostic standard was included and divided into UC group and CD group. The patients were divided into groups of 0–20 years old, 20–40 years old, 40–60 years old and 60–80 years old, and divided into male and female according to gender. The patients with UC were divided into mild group, moderate group and severe group according to the improved Truelove and Witts severity. The patients hospitalized with polyps of digestive diseases were chosen as normal control group. Record data about blood routine examination, coagulation function and D-D were also collected. Results There were statistical differences in TT and FIB between patients with IBD and those with polyps (P Conclusions IBD patients have higher coagulation status than normal people. The hypercoagulability of IBD patients was correlated with ESR, but not with CRP and D–D. The higher the age of CD patients, the higher the risk of coagulation. The risk of hypercoagulability in UC patients was independent of gender and age; The more serious the condition was in the active stage, the more significant the hypercoagulability was. The hypercoagulability of UC patients has little correlation with ESR and D–D, but no correlation with CRP.

Crohn's disease is a chronic disorder that typically affects the gastrointestinal tract. The increased incidence in the recent years, especially in Asian countries, prompts for performing studies and gain newer insights into the etiology and pathogenesis of the disease. Among other causative factors, gut microbiome and its cross-talk with the salivary microbiome is a known factor that has a plausible role in the pathogenesis of Crohn's disease. The gut microbiome has been extensively studied, however, the salivary microbiome and its dynamics during different phases of this disease remain understudied. In this study, we obtained saliva samples from the patients during active and remission phases of the disease and compared them with control samples and highlighted the differences in taxonomic as well as predicted functional pathways among them. Our results indicated that the α and β diversities were significantly lower during the active phase in contrast with remission phase and healthy samples. In general, Firmicutes were most abundant among the three sample groups, followed by Bacteroidetes and Proteobacteria. Genus level distribution highlighted Streptococcus, Neisseria, Prevotella, Haemophilus, and Veillonella as the five most abundant taxa. Differential abundance analysis of the three sample groups identified significant enrichment of 30 bacterial taxa in the active phase that included g_Prevotella, f_Prevotellaceae, and p_Bacteroidetes. Furthermore, remission phase and control also exhibited significant enrichment of 24 and 22 bacterial taxa, respectively. Eleven differentially abundant pathways were also identified, four were significantly enriched in healthy controls whereas other seven were significantly enriched in active phase of the disease. Several important pathways, such as ribosome biogenesis and Energy metabolism were depleted in the active phase. Our study has highlighted several taxa and functional categories that could be implicated with the onset of Crohn's disease and thus have the potential to serve as biomarkers of the active disease. However, these findings require further validation through functional studies in the future.

Background The purpose of this study was to evaluate patients' and orthodontists' perspectives on knowledge of techniques for reducing orthodontic treatment time and acceptance of these techniques. Methods A total of 200 individuals were interviewed and equally divided into two groups: orthodontist group (62 female and 38 male; mean age, 38.07 years) and patient group (52 female and 48 male; mean age, 22.61 years; in the active stage of fixed orthodontic treatment). One questionnaire for each group was administered, including questions about the duration of orthodontic treatment and techniques used for treatment optimization, such as corticotomy, distraction osteogenesis, vibration, and laser therapy. The associations between variables were analyzed by the χ2 test at a significance level of 5%. Results Among orthodontists, 76% knew at least one technique to reduce the treatment duration, with corticotomy being the most frequently cited (66%); however, only 12% used one or more of these techniques. Laser therapy was the most frequently implemented technique (7%). Regarding the duration of orthodontic treatment, the mean time reported by orthodontists was 19 to 24 months, regardless of the technique or the experience of the orthodontist. Furthermore, 39% of patients expected their treatment to last for more than 24 months, with 50% accepting to undergo further procedures to reduce this duration. Conclusions Patients are willing to undergo additional procedures to reduce the treatment duration and to bear additional costs. However, despite their knowledge, orthodontists do not apply or offer these techniques to the patients.

Management of a pregnant woman with complete heart block presenting during pregnancy and without pacing remains debatable. To bear up against any hemodynamic variations in peripartum period, temporary pacemakers have been advocated by some authors. Herein, we report a case of successful management of a 24 year old, pregnant woman with CHB who had an uneventful emergency caesarean delivery under spinal anesthesia after temporary pacing. She was an unbooked patient detected with CHB first time during active stage of labour. She delivered a healthy male baby and was discharged from the hospital in a stable and satisfactory condition on seventh postoperative day.

Aneurysmal bone cysts are benign, rare bony tumours frequently observed among children and young adults principally located in the long bones, pelvis, and spine and rarely in other anatomical district such as the hand. We report the case of a 12-year-old girl with an aneurysmal bone cyst, in active stage, involving the still-open epiphysis of the fourth metacarpal of the right hand, which was in a first time treated by curettage, and 3 months later, occurring a recurrence, by a radically excision of the bone and reconstruction with a graft from the iliac crest. At 10-year follow-up the patient had good cosmetic results and a functioning hand. We also performed a systematic Literature review in order to retrieve the key information regarding: the diagnosis, the clinical features and the treatment.

Symbiotic stars are strongly interacting binaries, consisting of a white dwarf and a cool giant, mainly of spectral type M. AG Draconis belongs to a less numerous group of the yellow symbiotic systems, as the cool component in this binary is of a spectral type earlier than K4. Recently, after seven years of quiescence, this symbiotic star exhibited a very unusual active stage with the four minor outbursts observed. Thanks to the excellent involvement of amateur astronomers and professional observatories, we can study the activity of AG Draconis in unprecedented details. The objective of this study was to assess whether an antibody ELISA applied to bulk tank milk (BTM) could be used to accurately estimate within-herd prevalence of digital dermatitis (DD). The ELISA was designed for the detection of antibodies against Treponema phagedenis-like strain V1 (PrrA antigen). The hind feet of all lactating cows from 40 commercial French dairy herds with a history of DD were scored by an observer in the milking parlor, using the 4 M-stage system. After milking, a BTM sample was collected and tested for anti-Treponema phagedenis-like antibodies using the antibody ELISA. Within-herd DD prevalence at the cow level was determined using 2 different approaches: (1) having DD lesion on at least 1 hind foot (Prev; prevalence of affected cows), and (2) having an M1 or M2 lesion on at least 1 hind foot (PrevA; prevalence of cows affected by DD in an active stage). Receiver operating characteristic analysis was used to determine both optimal within-herd DD prevalence and BTM sample to positive (S/P) ratio cut-off values. Two optimal cut-off values were identified. Herds with an S/P ratio of BTM ≤0.2 had a Prev ≤10% (sensitivity = 0.97, specificity = 1), whereas herds with an S/P ratio of BTM >0.38 had a Prev >40% (sensitivity = 0.94, specificity = 0.86). In the same way but with a slightly lower specificity, an S/P ratio >0.38 corresponds also to a PrevA >18% (sensitivity = 0.92, specificity = 0.70). The BTM antibody ELISA shows great promise for screening purposes during DD management programs.

Calcification is a clinical marker of atherosclerosis. This review focuses on recent findings on the association between calcification and plaque vulnerability. Calcified plaques have traditionally been regarded as stable atheromas, those causing stenosis may be more stable than non-calcified plaques. With the advances in intravascular imaging technology, the detection of the calcification and its surrounding plaque components have evolved. Microcalcifications and spotty calcifications represent an active stage of vascular calcification correlated with inflammation, whereas the degree of plaque calcification is strongly inversely related to macrophage infiltration. Asymptomatic patients have a higher content of plaque calcification than that in symptomatic patients. The effect of calcification might be biphasic. Plaque rupture has been shown to correlate positively with the number of spotty calcifications, and inversely with the number of large calcifications. There may be certain stages of calcium deposition that may be more atherogenic. Moreover, superficial calcifications are independently associated with plaque rupture and intraplaque hemorrhage, which may be due to the concentrated and asymmetrical distribution of biological stress in plaques. Conclusively, calcification of differential amounts, sizes, shapes, and positions may play differential roles in plaque homeostasis. The surrounding environments around the calcification within plaques also have impacts on plaque homeostasis. The interactive effects of these important factors of calcifications and plaques still await further study.

The diagnosis of cystic echinococcosis (CE) is based on imaging, while serology is a complementary test of particular use when imaging is inconclusive. Serology has several limitations. Among them, false‐positive results are often obtained in subjects with alveolar echinococcosis (AE), rendering difficult the differential diagnosis. We set up an immune assay based on IL‐4‐specific production after stimulating whole blood with an antigen B (AgB)‐enriched fraction from E granulosus that associates with CE and CE cysts in active stage. We aimed to evaluate potential cross‐reactivity of this test using samples from patients with AE. Twelve patients with AE were recruited; IL‐4 levels ranged from 0 to 0.07 pg/mL. Based on the previously identified cut‐off of 0.39 pg/mL using samples from patients with CE, none of samples from AE patients scored positive. In contrast, almost 80% of samples from AE patients scored positive in serology tests based on different E granulosus‐derived antigenic preparations. Our preliminary data show that this experimental whole‐blood assay has no cross‐reactivity in our cohort of patients with AE, in turn indicating a high specificity of the assay for CE diagnosis. This result supports further work towards the development of improved diagnostic tests for CE.

Türkmen,Hülya (Balikesir Author), Aim This study aims to determine the effect of acupressure (SP6) on first stage of labor pain and duration. Methods A single blind, randomized controlled experimental trial was conducted with 60 pregnant women: 30 in the experimental group and 30 in the control group. At the active and transition stages of labor, acupressure was applied to the SP6 point of the experimental group, and the SP6 point of the control group was touched each contraction. The data were collected using the Pregnant Information Form. The Partograph Form was used to determine the duration of active stage and transition stage. The labor pain severity was measured using a numeric rating scale (NRS) immediately after each acupressure/touch SP6 in active and transition stages. A participant questionnaire on satisfaction was administered in the immediate postnatal period. The group difference was tested for statistical significance using the Chi-square test, Fisher's exact test, and the Mann-Whitney U test. Results As the primary outcome, labor pain was less and duration of labor was shorter for the experimental group. As the secondary outcome, satisfaction was increased that could be as a result of reduced pain in labor and/or shorter duration of labor. The perceived pain level in the active stage in the experimental group (n = 30) was less than the control group (n = 30) (labor pain: 7.17 + 0.89 vs. 7.66 + 0.71, p = .002). The mean duration of the first stage of labor in the experimental group was shorter than the control group (4.88 + 0.85 hr vs. 5.56 +/- 0.66, p = .001). It was also found that pregnant women in the experimental group would recommend this method to other pregnant women (Chi-square = 5.711, p = .017). Conclusions Application of acupressure on SP6 was found to have a positive effect on pregnant women's labor experience and reducing the labor pain and shortening the duration of first stage of labor compared to touch on SP6. The women receiving acupressure had higher levels of satisfaction in acupressure application than women receiving only touch on SP6, which could be as a result of reduced pain in labor and/or shorter duration of labor.

Current review provides data of etiology, pathogenesis and clinical evidences of urinary tract infection (UTI) in adolescents in connection with physiology of pubertal period. Data of congenital and acquired factors leading to origin and chronicity of the inflammation are presented. Treatment and rehabilitation approaches for adolescents with UTI are provided. Authors emphasize that metabolic disorders appeared in active stage of inflammation do not disappear when clinical symptoms of UTI, bacteriuria and leukocyturia are gone.

This communication reports the current progress in thermal analysis studies on oil sludge pyrolysis. The pyrolysis behaviors of three different oil sludge samples were studied and compared using the results of thermogravimetry (TG) and differential thermogravimetry (DTG) analysis. Then, the evaporation stage of the pyrolysis process was analyzed by comparing the peak distribution in the DTG curves of the oil sludge samples with that obtained for artificial oil sludge. The relationship between the DTG peak distribution and the surface properties of the oil sludge particles was determined. As a complement to our previous study, the active stage of oil sludge ash additive was determined based on the results obtained from the pyrolysis of oil sludge with and without the addition of oil sludge ash. The kinetic parameters were calculated using the iso-conversional differential method, the integral method, and the Kissinger method. The results of this study provide reference values for practical engineering applications.
