1. A critical evaluation of the electronic surgical logbook

Abstract

Abstract Background The Association of Surgeons of Great Britain and Ireland (ASGBI) devised the electronic surgical logbook (version 2.4) for higher trainees in General Surgery enabling trainees to compile a uniform data set of their operative and training experience. This is in use by higher surgical trainees (HST) in the United Kingdom. This logbook permits trainees to submit data centrally into a Regional Analysis Database (RAD). With the implementation of the European Working Time Directive (EWTD) there is need for reliable data to assess the effects of the directive on training. In order to draw meaningful conclusions from the database the quality of data needs to be validated. We critically analysed the RAD in the Yorkshire region for a one-year period. Methods The RAD from the ASGBI for the Yorkshire region was analysed. Data for the period 01/10/2002–30/09/2003 was identified and interrogated using Microsoft Excel (2000 version). The RAD was compared with information obtained from the Regional Surgical Advisor for Yorkshire with respect to hospitals, surgical consultants and HST's in the region during the study period. Results There were 13,755 operations entered for the study period. 579 corrections to the data had to be made (4.2%) and a further 1140 entries were deleted (8.2%). Following corrections and deletions 12,615 operative entries were available for analysis. Overall 12.5% of the data required either correction or exclusion from the database prior to analysis. Conclusion The RAD has a large dataset useful to monitor and assess training. However, the quality of the data needs to be verified prior to use. Recommendations have been made to develop the ASGBI logbook, which would eventually translate to improved data reliability of the RAD.

Abstract

Nowadays every possible transaction is occurring online so for all these transactions required security must be provided. It is not easy to produce such security since there will be a huge amount of data involved here as well as stored data must be kept confidential so to achieve these things we need to perform transactions through the cloud. Since technology has been developed the rate of the cloud being attacked is also increased so to prevent this integration two clouds must be used. The existing system does not apply cloud integration instead of that they use only data encryption for secure transactions. In the Proposed system of the project apart from data encryption cloud integration is also gets implemented here to establish transactions more securely. In this proposed system, we implement three levels of security which are low, medium, and high. Here We use only data encryption for low security level and in medium security level splitting of data occur along with data encryption to establish more secure transaction than low security level. Finally in order to produce a higher security level we use both cloud integration and splitting of data along with data encryption to produce transactions more securely than medium security level.

6. Identification of Multiple Attacks in Cloud Environment using Big Data

Abstract

Since the Cloud environment is prone to many attacks we are implementing a Big Data based centralized log analysis system to identify the network traffic occurred by attackers through DDOS, SQL Injection, and Brute Force attacks. The log file is automatically transmitted to the centralized cloud server where big data is initiated and uses a tool called Hadoop to process the huge amount of log files that are being sent to big data. If an attacker attacks any files then it will be compared with the attack dataset that is maintained here to detect the attacks that are being used by the attacker. This system also helps in storing the information of all registered users, and their files which get uploaded to the cloud server and downloaded from the server and IP addresses in order to view any attacks that may occur in the future. All this stored information is maintained securely using SQL which is a backend process. Thus we are implementing a system that delivers a very high performance as well as very efficient results in categorizing the attacks. Since the Hadoop tool is being used in the system, this system is able to increase its scalability and achieves a faster detection of attacks like DDOS, SQL Injection, and Brute Force attacks. This system can play a vital role in various organizations to safeguard their privacy. Data integrity, Data confidentiality, and non-repudiation can be achieved by using this system. Finally, we can say that this system avoids any faults that occurred in the previous system that was invented before this.

Abstract

The accumulation of dust and scrap in the floors of workshops and warehouses creates untidiness and many occupational hazards. To have a dust free and safe working environment inside the workshop, it is necessary to sweep and clean them periodically. Mostly the cleaning is done using hand sweepers, ride on sweepers and walk behind type sweepers. The process of sweeping can be automated in order to minimize the effort of human. The presently available electrically operated sweepers occupy large space and it becomes difficult to operate in congested places. The machine should be compact so that it can move to any place with ease. The robotic sweeper consists of a cylindrical brush and two side brushes. The cylindrical brush is connected to DC motor through pulley and V belt. The miter gears are used at the ends of the cylindrical brush to transmit the power to the side brushes which rotates perpendicular to cylindrical brush. The robotic sweeper is controlled using Arduino, drives and Bluetooth module.

Abstract

While inhomogeneous diffusivity has been identified as a ubiquitous feature of the cellular interior, its implications for particle mobility and concentration at different length scales remain largely unexplored. In this work, we use agent-based simulations of diffusion to investigate how heterogeneous diffusivity affects the movement and concentration of diffusing particles. We propose that a nonequilibrium mode of membrane-less compartmentalization arising from the convergence of diffusive trajectories into low-diffusive sinks, which we call ‘diffusive lensing,’ is relevant for living systems. Our work highlights the phenomenon of diffusive lensing as a potentially key driver of mesoscale dynamics in the cytoplasm, with possible far-reaching implications for biochemical processes.

Abstract

Introduction: The 2008 National Mastectomy and Breast Reconstruction Audit demonstrated marked variation in the practice and outcomes of breast reconstruction in the UK. To standardise practice and improve outcomes for patients, the British professional associations developed best-practice guidelines with specific guidance for newer mesh-assisted implant-based techniques. We explored the degree of uptake of best-practice guidelines within units performing implant-based reconstruction (IBBR) as the first phase of the implant Breast Reconstruction Evaluation (iBRA) study. Method(s): A questionnaire developed by the iBRA Steering Group was completed by trainee and consultant leads at breast and plastic surgical units across the UK. Simple summary statistics were calculated for each survey item to assess compliance with current best-practice guidelines. Result(s): 81 units from 79 NHS Trusts completed the questionnaire. Marked variation was observed in adherence to guidelines, especially those relating to clinical governance and infection prevention strategies. Less than half (n = 28, 47%) of units obtained local clinical governance board approval prior to offering new mesh-based techniques and prospective audit of the clinical, cosmetic and patient-reported outcomes of surgery was infrequent. Most units screened for methicillin-resistant staphylococcus aureus prior to surgery but fewer than 1 in 3 screened for methicillin-sensitive strains. Laminar-flow theatres (recommended for IBBR) were not widely-available with less than 1 in 5 units having regular access. Peri-operative antibiotics were widely-used, but the type and duration were highly-variable. Conclusion(s): The iBRA national practice questionnaire has demonstrated variation in reported practice and adherence to IBBR guidelines. Abstract

Type 2 diabetes mellitus (T2DM) emerged as a major health care concern in modern society, primarily due to lifestyle changes and dietary habits. Obesity-induced insulin resistance is considered as the major pathogenic factor in T2DM. In this study, we investigated the effect of vindoline, an indole alkaloid of Catharanthus roseus on insulin resistance (IR), oxidative stress and inflammatory responses in dexamethasone (IR inducer)-induced dysfunctional 3T3-L1 adipocytes and high-glucose-induced insulin-resistant L6-myoblast cells. Results showed that dexamethasone-induced dysfunctional 3T3-L1 adipocytes treated with different concentrations of vindoline significantly enhanced basal glucose consumption, accompanied by increased expression of GLUT-4, IRS-1 and adiponectin. Similarly, vindoline-treated insulin-resistant L6 myoblasts exhibited significantly enhanced glycogen content accompanied with upregulation of IRS-1 and GLUT-4. Thus, in vitro studies of vindoline in insulin resistant skeleton muscle and dysfunctional adipocytes confirmed that vindoline treatment significantly mitigated insulin resistance in myotubes and improved functional status of adipocytes. These results demonstrated that vindoline has the potential to be used as a therapeutic agent to ameliorate obesity-induced T2DM-associated insulin resistance profile in adipocytes and skeletal muscles.

We recently described the mapping of a recessive spastic CP locus to a 5 cM chromosomal region located at 2q24-31.1, in rare consanguineous families.\ud \ud Methods\ud \ud Here we present data that refine this locus to a 0.5 cM region, flanked by the microsatellite markers D2S2345 and D2S326. The minimal region contains the candidate gene GAD1, which encodes a glutamate decarboxylase isoform (GAD67), involved in conversion of the amino acid and excitatory neurotransmitter glutamate to the inhibitory neurotransmitter γ-aminobutyric acid (GABA).\ud \ud Results\ud \ud A novel amino acid mis-sense mutation in GAD67 was detected, which segregated with CP in affected individuals.\ud \ud Conclusions\ud \ud This result is interesting because auto-antibodies to GAD67 and the more widely studied GAD65 homologue encoded by the GAD2 gene, are described in patients with Stiff-Person Syndrome (SPS), epilepsy, cerebellar ataxia and Batten disease. Further investigation seems merited of the possibility that variation in the GAD1 sequence, potentially affecting glutamate/GABA ratios, may underlie this form of spastic CP, given the presence of anti-GAD antibodies in SPS and the recognised excitotoxicity of glutamate in various contexts.\ud
