Abstract

Abstract Background Surfactant is a well-established therapy for preterm neonates affected by respiratory distress syndrome (RDS). The goals of different methods of surfactant administration are to reduce the duration of mechanical ventilation and the severity of bronchopulmonary dysplasia (BPD); however, the optimal administration method remains unknown. This study compares the effectiveness of the INtubate-RECruit-SURfactant-Extubate (IN-REC-SUR-E) technique with the less-invasive surfactant administration (LISA) technique, in increasing BPD-free survival of preterm infants. This is an international unblinded multicenter randomized controlled study in which preterm infants will be randomized into two groups to receive IN-REC-SUR-E or LISA surfactant administration. Methods In this study, 382 infants born at 24+0–27+6 weeks’ gestation, not intubated in the delivery room and failing nasal continuous positive airway pressure (nCPAP) or nasal intermittent positive pressure ventilation (NIPPV) during the first 24 h of life, will be randomized 1:1 to receive IN-REC-SUR-E or LISA surfactant administration. The primary outcome is a composite outcome of death or BPD at 36 weeks’ postmenstrual age. The secondary outcomes are BPD at 36 weeks’ postmenstrual age; death; pulse oximetry/fraction of inspired oxygen; severe intraventricular hemorrhage; pneumothorax; duration of respiratory support and oxygen therapy; pulmonary hemorrhage; patent ductus arteriosus undergoing treatment; percentage of infants receiving more doses of surfactant; periventricular leukomalacia, severe retinopathy of prematurity, necrotizing enterocolitis, sepsis; total in-hospital stay; systemic postnatal steroids; neurodevelopmental outcomes; and respiratory function testing at 24 months of age. Randomization will be centrally provided using both stratification and permuted blocks with random block sizes and block order. Stratification factors will include center and gestational age (24+0 to 25+6 weeks or 26+0 to 27+6 weeks). Analyses will be conducted in both intention-to-treat and per-protocol populations, utilizing a log-binomial regression model that corrects for stratification factors to estimate the adjusted relative risk (RR). Discussion This trial is designed to provide robust data on the best method of surfactant administration in spontaneously breathing preterm infants born at 24+0–27+6 weeks’ gestation affected by RDS and failing nCPAP or NIPPV during the first 24 h of life, comparing IN-REC-SUR-E to LISA technique, in increasing BPD-free survival at 36 weeks’ postmenstrual age of life. Abstract

In Italy, the use of autogenous inactivated vaccines prepared with the bacterial strains isolated from affected animals is authorized by the Ministry of Health in farms where bacterial diseases occur frequently. The autogenous vaccine performed using Pasteurella multocida is frequently used in rabbit farms, but the feedback of its application is not available. Therefore, the aim of this study is to give information about the impact on the clinical signs of a bivalent autogenous vaccine in rabbits of a genetic centre. The vaccine was prepared using two P. multocida strains belonging to serogroups A and F, equipped with virulence genes and responsible for cyclical outbreak of pasteurellosis in the farm. The vaccine was administered with a first injection, followed by another one after 15 days, then another one four months after the first injection, and then continuing with a further injection every six months to all rabbits. Clinical conditions and mortality rates were monitored for two years after the first vaccination. The improvement in clinical condition and the decrease of the mortality rate were significant especially in the first year post-vaccine. In addition, the number of animals removed due to the disease decreased greatly. Based on the finding of P. multocida strains belonging to serogroup D and serogroup A equipped with different virulence-gene patterns from those previously found, we suggest that the vaccine was unable to prevent the introduction and spreading of new strains among the rabbits.

Abstract

When non-trivial local structures are present in a topological space $X$, a common approach to characterizing the isomorphism type of the $n$-th homotopy group $\pi_n(X,x_0)$ is to consider the image of $\pi_n(X,x_0)$ in the $n$-th \v{C}ech homotopy group $\check{\pi}_n(X,x_0)$ under the canonical homomorphism $\Psi_{n}:\pi_n(X,x_0)\to \check{\pi}_n(X,x_0)$. The subgroup $\ker(\Psi_n)$ is the obstruction to this tactic as it consists of precisely those elements of $\pi_n(X,x_0)$, which cannot be detected by polyhedral approximations to $X$. In this paper, we use higher dimensional analogues of Spanier groups to characterize $\ker(\Psi_n)$. In particular, we prove that if $X$ is paracompact, Hausdorff, and $LC^{n-1}$, then $\ker(\Psi_n)$ is equal to the $n$-th Spanier group of $X$. We also use the perspective of higher Spanier groups to generalize a theorem of Kozlowski-Segal, which gives conditions ensuring that $\Psi_{n}$ is an isomorphism., Comment: 21 pages, 2 figures

Abstract

The influence of ultra-short laser modification on the surface morphology and possible chemical alteration of poly-lactic acid (PLA) matrix in respect to the optimization of cellular and antibacterial behavior were investigated in this study. Scanning electron microscopy (SEM) morphological examination of the processed PLA surface showed the formation of diverse hierarchical surface microstructures, generated by irradiation with a range of laser fluences (F) and scanning velocities (V) values. By controlling the laser parameters, diverse surface roughness can be achieved, thus influencing cellular dynamics. This surface feedback can be applied to finely tune and control diverse biomaterial surface properties like wettability, reflectivity, and biomimetics. The triggering of thermal effects, leading to the ejection of material with subsequent solidification and formation of raised rims and 3D-like hollow structures along the processed zones, demonstrated a direct correlation to the wettability of the PLA. A transition from superhydrophobic (thetha > 150 deg.) to super hydrophilic (thetha < 20 deg.) surfaces can be achieved by the creation of grooves with V = 0.6 mm/s, F = 1.7 J/cm2. The achieved hierarchical architecture affected morphology and thickness of the processed samples which were linked to the nature of ultra-short laser-material interaction effects, namely the precipitation of temperature distribution during material processing can be strongly minimized with ultrashort pulses leading to non-thermal and spatially localized effects that can facilitate volume ablation without collateral thermal damage.

Abstract Background Pregnancy is a crucial transition moment exposing women to potential mental health problems, especially depressive disturbances. Sociodemographic, pregnancy-related, and psychological factors have been related to depressive symptoms in the perinatal period. This study aims at (1) exploring personality and individual factors related with perinatal depressive symptoms, and (2) testing the mediating role of personality in the relation between characteristics of the woman’s family of origin and depressive symptoms. Methods Women in the perinatal period admitted to the gynecology unit for motherhood-related routine assessments (n = 241) were included in the study. Abstract

Metalenses offer the ground-breaking opportunity to realize highly performing low-weight, flat and ultrathin, optical elements which substantially reduce size and complexity of imaging systems. Today, a major challenge in metalenses design is still the realization of achromatic optical elements ideally focussing a broad wavelength spectrum at a single focal length. Here we present a fast and effective way to design and fabricate extremely thin all-dielectric metalenses, optimally solving achromaticity issues by means of machine learning codes. The enabling technology for fabrication is a recently developed hyper resolute two-photon direct laser writing lithography equipment. The fabricated metalenses, based on a completely flat and ultrathin design, show intriguing optical features. Overall, achromatic behavior, focal length of 1.14 mm, depth of focus of hundreds of microns and thickness of only few nanometers allow considering the design of novel and efficient imaging systems in a completely new perspective.

Abstract

ObjectiveSepsis and septic shock are major challenges and economic burdens to healthcare, impacting millions of people globally and representing significant causes of mortality. Recently, a large number of quality improvement programs focused on sepsis resuscitation bundles have been instituted worldwide. These educational initiatives have been shown to be associated with improvements in clinical outcomes. We aimed to evaluate the impact of a multi-faceted quality implementing program (QIP) on the compliance of a “simplified 1-h bundle” (Sepsis 6) and hospital mortality of severe sepsis and septic shock patients out of the intensive care unit (ICU).MethodsEmergency departments (EDs) and medical wards (MWs) of 12 academic and non-academic hospitals in the Lombardy region (Northern Italy) were involved in a multi-faceted QIP, which included educational and organizational interventions. Patients with a clinical diagnosis of severe sepsis or septic shock according to the Sepsis-2 criteria were enrolled in two different periods: from May 2011 to November 2011 (before-QIP cohort) and from August 2012 to June 2013 (after-QIP cohort).Measurements and main resultsThe effect of QIP on bundle compliance and hospital mortality was evaluated in a before–after analysis. We enrolled 467 patients in the before-QIP group and 656 in the after-QIP group. At the time of enrollment, septic shock was diagnosed in 50% of patients, similarly between the two periods. Abstract

Infants born preterm are at a high risk of both gut microbiota (GM) dysbiosis and neurodevelopmental impairment. While the link between early dysbiosis and short-term clinical outcomes is well established, the relationship with long-term infant health has only recently gained interest. Notably, there is a significant overlap in the developmental windows of GM and the nervous system in early life. The connection between GM and neurodevelopment was first described in animal models, but over the last decade a growing body of research has also identified GM features as one of the potential mediators for human neurodevelopmental and neuropsychiatric disorders. In this narrative review, we provide an overview of the developing GM in early life and its prospective relationship with neurodevelopment, with a focus on preterm infants. Animal models have provided evidence for emerging pathways linking early-life GM with brain development. Furthermore, a relationship between both dynamic patterns and static features of the GM during preterm infants’ early life and brain maturation, as well as neurodevelopmental outcomes in early childhood, was documented. Future human studies in larger cohorts, integrated with studies on animal models, may provide additional evidence and help to identify predictive biomarkers and potential therapeutic targets for healthy neurodevelopment in preterm infants.

Abstract

Abstract Nutrition in the first 1000 days of life is essential to ensure appropriate growth rates, prevent adverse short- and long-term outcomes, and allow physiologic neurocognitive development. Appropriate management of early nutritional needs is particularly crucial for preterm infants. Although the impact of early nutrition on health outcomes in preterm infants is well established, evidence-based recommendations on complementary feeding for preterm neonates and especially extremely low birth weight and extremely low gestational age neonates are still lacking. In the present position paper we performed a narrative review to summarize current evidence regarding complementary feeding in preterm neonates and draw recommendation shared by joint societies (SIP, SIN and SIGENP) for paediatricians, healthcare providers and families with the final aim to reduce the variability of attitude and timing among professionals.

Abstract

Previous evidence showed abnormal parietal sources of resting-state electroencephalographic (EEG) delta (< 4 Hz) and alpha (8–12 Hz) rhythms in treatment-Naïve HIV (Naïve HIV) subjects, as cortical neural synchronization markers in quiet wakefulness. Here, we tested the hypothesis that these local abnormalities may be related to functional cortical dysconnectivity as an oscillatory brain network disorder.The present EEG database regarded 128 Naïve HIV and 60 Healthy subjects. The eLORETA freeware estimated lagged linear EEG source connectivity (LLC). The area under receiver operating characteristic (AUROC) curve indexed the accuracy in the classification between Healthy and HIV individuals.Parietal intrahemispheric LLC solutions in alpha sources were abnormally lower in the Naïve HIV than in the control group. Furthermore, those abnormalities were greater in the Naïve HIV subgroup with executive and visuospatial deficits than the Naïve HIV subgroup with normal cognition. AUROC curves of those LLC solutions exhibited moderate/good accuracies (0.75–0.88) in the discrimination between the Naïve HIV individuals with executive and visuospatial deficits vs. Naïve HIV individuals with normal cognition and control individuals.In quiet wakefulness, Naïve HIV subjects showed clinically relevant abnormalities in parietal alpha source connectivity. HIV may alter a parietal “hub” oscillating at the alpha frequency in quiet wakefulness as a brain network disorder.

Abstract

This exploratory study illustrates the challenges and opportunities of remote qualitative research on sensitive issues. The analysis is based on the experience of fourteen researchers who have lived the digital switchover linked to the covid-19 pandemic as both a constraint and a resource. Based on exchanges in four focus groups, the participants identified tensions at the heart of remote qualitative research: the data collected lacks proxemic and contextual information, while benefiting from more discursive depth; interaction allows for a certain redistribution of power, but is accompanied by a loss of control on the part of the researcher; virtual distance favors the integrity and autonomy of the respondent, while hindering empathy and the ethic of care. Easy access and cost-effectiveness make remote research attractive, at the risk of generating new inequalities and superficial analyses. These results show that remote investigation on sensitive subjects is a paradoxical scientific practice, capable of both innovating and distorting the research process.

Abstract

Background: Particulate contamination due to infusion therapy (administration of parenteral nutrition and medications) carries a potential health risk for infants in neonatal intensive care units (NICUs). This particulate consists of metals, drug crystals, glass fragments, or cotton fibers and can be generated by drug packaging, incomplete reconstitution, and chemical incompatibilities. In-line filters have been shown to remove micro-organisms, endotoxin, air, and particles in critically ill adults and older infants, but its benefits in newborn remain to be demonstrated. Moreover, 50% of inflammatory episodes in the setting of NICUs are blood culture-negative. These episodes could be partly related to the presence of particles in the infusion lines. Methods: A multicenter randomized single-blind controlled trial was designed. All infants admitted to NICUs for which prolonged infusion therapy is expected will be enrolled in the study and randomized to the Filter or Control arm. All patients will be monitored until discharge, and data will be analyzed according to a "full analysis set." The primary outcome is the frequency of patients with at least one sepsis-like event, defined by any association of suspected sepsis symptoms with a level of c-reactive protein (CRP) > 5 mg/L in a negative-culture contest. The frequency of sepsis, phlebitis, luminal obstruction, and the duration of mechanical ventilation and of catheter days will be evaluated as secondary outcomes. The sample size was calculated at 368 patients per arm. Discussion: This is the first multicenter randomized control trial that compares in-line filtration of parenteral nutrition and other intravenous drugs to infusion without filters. Sepsis-like events are commonly diagnosed in clinical practice and are more frequent than sepsis in a positive culture contest. The risk of these episodes in the target population is estimated at 30–35%, but this data is not confirmed in the literature. If the use of in-line filters results in a significant decrease in sepsis-like events and/or in any other complications, the use of in-line filters in all intravenous administration systems may be recommended in NICUs. 47. Revising the $f_1(1420)$ resonance

Abstract

We have studied the production and decay of the $f_1(1285)$ into $\pi a_0(980)$ and $K^* \bar K$ as a function of the mass of the resonance and find a shoulder around 1400 MeV, tied to a triangle singularity, for the $\pi a_0(980)$ mode, and a peak around 1420 MeV with about 60 MeV width for the $K^* \bar K$ mode. Both these features agree with the experimental information on which the $f_1(1420)$ resonance is based. In addition, we find that if the $f_1(1420)$ is a genuine resonance, coupling mostly to $K^* \bar K$ as seen experimentally, one finds unavoidably about a 20\% fraction for $\pi a_0(980)$ decay of this resonance, in drastic contradiction with all experiments. Altogether, we conclude that the $f_1(1420)$ is not a genuine resonance, but the manifestation of the $\pi a_0(980)$ and $K^* \bar K$ decay modes of the $f_1(1285)$ at higher energies than the nominal one., Comment: 9 pages, 12 figures, new references added

48. An Investigation into the Relationships between Reading Rate, Processing Speed, and Extended Time

Abstract

Through ESSA (2015) and IDEIA (2004), all students must participate in statewide assessments and students with disabilities are entitled to receive reasonable adaptations and accommodations in order to ensure that the assessments accurately measure their achievement within the designated academic areas. Research indicates that the provision of an accommodation should be based upon a student's individual needs; however, studies within this area frequently compare the effects of an accommodation on the performance of students with a disability and students without a disability. Currently, extended time is the most frequently requested and provided accommodation for state assessments across disabilities. This study sought to determine whether there is a relationship between reading rate and the boost from extended time on reading, a relationship between cognitive processing speed and the boost from extended time on reading, and a relationship between reading rate and cognitive processing speed. The Nelson-Denny Reading Test (NDRT) Forms I and J and the Woodcock-Johnson Tests of Cognitive Abilities, 4th Edition (WJ-IV COG) were used to assess 21 students attending a suburban high school in New Jersey. Results indicated there is a relationship between cognitive processing speed and boost from extended time on the Vocabulary subtest (r = -0.38) and a relationship between reading rate and performance on the Letter-Pattern Matching test (r = 0.37). Results indicated there is not a relationship between reading rate or overall cognitive processing speed and boost from extended time on comprehension or general reading ability and there is not a relationship between reading rate and overall cognitive processing speed. Further investigation is needed to determine whether extended time provides a differential boost for students based upon reading fluency or cognitive processing speed.
