Untreated obstructive sleep apnea-hypopnea syndrome (OSAHS) is associated with significant direct and indirect medical costs. This disorder also has a significant negative impact on work performance and safety, and is implicated in a substantial proportion of motor vehicular crashes. Timely diagnosis and optimal therapy have shown a lower utilization rate related to health care systems and reduced costs, while adverse risks are mitigated at the same time.Prompt diagnosis and optimal therapy have shown to decrease heath care utilizaton and costs, as well as mitigating these adverse risks.Similarly, untreated OSAHS is associated with higher unemployment rates. For health care professionals, having a patient with OSAHS involved in a MVC is of paramount importance for a several reasons, including personal and public damage, as well as the potential physical disability that may be caused by the accident. In Latin America, measuring direct and indirect costs is necessary considering the public health problem associated with OSAHS and the implications mentioned above., El síndrome de apnea-hipopnea obstructiva del sueño (SAHOS) no tratado se asocia con costos directos e indirectos significativos. Este trastorno también tiene un impacto negativo sobre el desempeño y la seguridad laboral y está implicado en una proporción considerable de accidentes automovilísticos. El diagnóstico oportuno y la terapia óptima han mostrado disminución en la utilización de los sistemas de salud y en los costos, al tiempo que atenúan los riesgos adversos. Del mismo modo, el SAHOS no tratado se asocia con incremento en las tasas de desempleo. Para los profesionales de la salud, tener un paciente con SAHOS involucrado en una colisión automovilística es de crucial importancia debido al daño personal y público, así como la potencial discapacidad física por el accidente. En Latinoamérica se requiere de la medición de los costos directos e indirectos dado el problema de salud pública que tiene asociado el SAHOS y las implicaciones mencionadas.

This annual report contains information on the operations of various divisions over the fiscal year: statistical data on workplace incidents, claims, insurance industry premiums and losses, revenue and operating expenditures, as well as legislative, regulatory and case law changes.

Fiscal year 2006 saw a decrease in filed claims by 3.6 percent, which presented a continuing, though slower decrease than the prior year’s 4.4 percent. The majority of the claim drop occurred within the metro jurisdictions. Claims from Baltimore City, Baltimore County and Anne Arundel County decreased while Prince George’s County, Montgomery County and Harford County increased.

Soon after the 1999 Institute of Medicine report, To Err Is Human, was released, the Department of Defense (DoD) began an aggressive examination of medical errors and the strategies for minimizing them. A primary goal was the creation of a standardized medication event reporting system, including a central registry for the compilation of reported data. This paper describes important experiences gleaned from the DoD's transition to a standardized medication error reporting system. MEDMARXSM, an Internet-based commercial reporting application, was selected by the DoD leadership as the standard tool for medication event reporting. MEDMARX was implemented initially at five military hospitals in fall 2000 as part of a patient safety pilot project, and was later made available to all 143 military treatment facilities worldwide. Medication errors represent approximately 50 percent of all patient safety events reported by military health care facilities. Nurses espouse a caring ethic and demonstrate effectiveness in prevention of patient falls but are often observed taking risks with patients’ safety. These actions reflect poor congruence between espoused values and behaviours. Attitudes, values and involvement in decision- making are factors that influence work behaviours. Nurses’ attitudes are held to be a definitive factor in prevention work; however, few studies have focused on adherence with best practice principles of fall prevention. Yet nurses claim no authority to change their work. It was assumed that increased adherence would be achieved by improving nurses’ attitudes through participation in decision- making surrounding fall prevention practice. This study aimed to tes t this assumption by empowering nurses working in two medical wards with high numbers of patient falls to improve their ownership of practice by utilising critical social theory and action research. Nurses’ attitudes, including self-esteem, professional values and work satisfaction were established before and after a practice development project using action research. Mixed methods were employed by praxis groups meeting fortnightly for a year reflecting on, and re-engineering practice. Action research occurred in cycles focusing on assessment, communication, everyday work, and performance. Nurses’ work was re-organised to gain time to spend in prevention work. Patients’ environments were made safer and more patient-centred. New and effective ways of assessing risk to fall, communication of risk and monitoring nurses’ performance of prevention work were created and evaluated. Analysis demonstrated that nurses had good self-esteem and professional values but were not satisfied with their work. Self-esteem and professional values were unaffected by participation in work-related decisions however, nurses expressed increased sense of ownership, more satisfaction and were observed to engage in more prevention work. The Articulated Total Body (ATB) Model and its predecessors have been in use for over twenty-five years to study the effects of various force environments on the response of a multi-body subject. The ATB model has been used by both government and industry to study the effects of automobile crashes, aircraft ejections, and other accident scenarios. The applications of the ATB Model have varied since its inception, and it is now commonly used for accident reconstruction, ergonomic simulations and a myriad of other situations where human body motion is modeled. The ATB Model was originally written in the FORTRAN IV (FORTRAN 66) programming language, but with time the code has gradually been modified to become essentially Fortran 77 compliant code. This modernization of the code has helped to reduce some of the clever coding logic originally employed to circumvent some of the hardware constraints of the earlier computers, such as purposely allowing arrays to overflow in adjacent memory locations to conserve memory space. With the increased use of the ATB Model, it would be beneficial to have the program coded in a more modem modular format that is readily amenable to future enhancements. Such a modular version of the code would also easily interface with other programs, such FEM and aerodynamic codes, other dynamics codes, feedback control packages, graphics packages, and numerous other related codes. In addition, such a modular code would lend itself to faster debugging and reduce the potential for undetected errors. The Fortran 90 programming language was chosen as the upgrade path for the ATB Model code for a number of reasons. Since Fortran 90 is a superset of Fortran 77, any standard Fortran 77 code should run without modification.

In 2001, the NATO RTO established a new Task Group, HFM-089/TG-024, to review how various countries test Personal Protective Equipment (PPE) against Anti-Personnel (AP) mines, and to recommend action for future testing. This was done in response to mounting evidence that anti-personnel mines pose a significant threat to soldiers, which has prompted the development of new PPE in several countries. Hundreds of different AP mine types exist but the majority fall into only two categories: Fragmentation and Blast mines. These category names reflect the primary injury mechanism associated with each. The accident data available to TG-024 demonstrated that the majority of fragmentation mine accidents result in ballistic wounds. Blast mines result in two patterns of wounds depending on whether a mine explodes under the lower extremity or in front of a soldier that is conducting mine clearance tasks. These three scenarious result in distinct patterns of injury, each scenario requiring that the PPE be subjected to a different test that is appropriate for the threat conditions., The original document contains color images. Once again, the Commission has pressed forward during this past year in our continuing effort to improve our service to the community. For the past few years, the Commission has witnessed the wrapping up of a number of important projects that will ensure the efficient and effective completion of our mission for years to come.
