Abstract

Background: The Italian Society of Echography and Cardiovascular Imaging (SIECVI) conducted a national survey to understand the volumes of activity, modalities and stressors used during stress echocardiography (SE) in Italy. Methods: We analyzed echocardiography laboratory activities over a month (November 2022). Data were retrieved through an electronic survey based on a structured questionnaire, uploaded on the SIECVI website. Results: Data were obtained from 228 echocardiographic laboratories, and SE examinations were performed in 179 centers (80.6%): 87 centers (47.5%) were in the northern regions of Italy, 33 centers (18.4%) were in the central regions, and 61 (34.1%) in the southern regions. We annotated a total of 4057 SE. We divided the SE centers into three groups, according to the numbers of SE performed: <10 SE (low-volume activity, 40 centers), between 10 and 39 SE (moderate volume activity, 102 centers) and ≥40 SE (high volume activity, 37 centers). Dipyridamole was used in 139 centers (77.6%); exercise in 120 centers (67.0%); dobutamine in 153 centers (85.4%); pacing in 37 centers (21.1%); and adenosine in 7 centers (4.0%). We found a significant difference between the stressors used and volume of activity of the centers, with a progressive increase in the prevalence of number of stressors from low to high volume activity (P = 0.033). The traditional evaluation of regional wall motion of the left ventricle was performed in all centers, with combined assessment of coronary flow velocity reserve (CFVR) in 90 centers (50.3%): there was a significant difference in the centers with different volume of SE activity: the incidence of analysis of CFVR was significantly higher in high volume centers compared to low - moderate - volume (32.5%, 41.0% and 73.0%, respectively, P < 0.001). Abstract

Background: The Italian Society of Echocardiography and Cardiovascular Imaging (SIECVI) conducted a national survey to understand better how different echocardiographic modalities are used and accessed in Italy. Methods: We analyzed echocardiography laboratory activities over a month (November 2022). Data were retrieved via an electronic survey based on a structured questionnaire, uploaded on the SIECVI website. Results: Data were obtained from 228 echocardiographic laboratories: 112 centers (49%) in the northern, 43 centers (19%) in the central, and 73 (32%) in the southern regions. During the month of observation, we collected 101,050 transthoracic echocardiography (TTE) examinations performed in all centers. As concern other modalities there were performed 5497 transesophageal echocardiography (TEE) examinations in 161/228 centers (71%); 4057 stress echocardiography (SE) examinations in 179/228 centers (79%); and examinations with ultrasound contrast agents (UCAs) in 151/228 centers (66%). We did not find significant regional variations between the different modalities. The usage of picture archiving and communication system (PACS) was significantly higher in the northern (84%) versus central (49%) and southern (45%) centers (P < 0.001). Lung ultrasound (LUS) was performed in 154 centers (66%), without difference between cardiology and noncardiology centers. The evaluation of left ventricular (LV) ejection fraction was evaluated mainly using the qualitative method in 223 centers (94%), occasionally with the Simpson method in 193 centers (85%), and with selective use of the three-dimensional (3D) method in only 23 centers (10%). 3D TTE was present in 137 centers (70%), and 3D TEE in all centers where TEE was done (71%). The assessment of LV diastolic function was done routinely in 80% of the centers. Abstract

This chapter aims at presenting the basic definitions and classifications for polygeneration systems by especially focusing on the type of layout, fuel, type of by-product, and energy vectors. The chapter provides a comprehensive and detailed discussion on the state of the art of the polygeneration system concept, fueled by fossil fuels and/or renewable energy sources, by presenting the innovative layouts designed to maximize the utilization of the input fuels. Polygeneration systems allow one to simultaneously generate energy vectors (power, heating, and cooling) as well as other valuable products (hydrogen, syngas, biodiesel, fertilizers, drinking water, etc.) by converting one or multiple energy sources. In this framework, different energy fuels can be considered as the supply source of polygeneration systems: fossil fuels (natural gas, coal, hydrogen, etc.) and renewable sources (geothermal, solar, biomass, wind, hydro). The contribution of polygeneration to the development of both centralized and decentralized systems is presented. The pros and cons of these plants are discussed in detail.

Abstract

Background: Malan syndrome (MALNS) is a recently described ultrarare syndrome lacking guidelines for diagnosis, management and monitoring of evolutive complications. Less than 90 patients are reported in the literature and limited clinical information are available to assure a proper health surveillance. Results: A multidisciplinary team with high expertise in MALNS has been launched at the “Ospedale Pediatrico Bambino Gesù”, Rome, Italy. Sixteen Italian MALNS individuals with molecular confirmed clinical diagnosis of MALNS were enrolled in the program. For all patients, 1-year surveillance in a dedicated outpatient Clinic was attained. The expert panel group enrolled 16 patients and performed a deep phenotyping analysis directed to clinically profiling the disorder and performing critical revision of previously reported individuals. Some evolutive complications were also assessed. Previously unappreciated features (e.g., high risk of bone fractures in childhood, neurological/neurovegetative symptoms, noise sensitivity and Chiari malformation type 1) requiring active surveillance were identified. A second case of neoplasm was recorded. No major cardiovascular anomalies were noticed. An accurate clinical description of 9 new MALNS cases was provided. Conclusions: Deep phenotyping has provided a more accurate characterization of the main clinical features of MALNS and allows broadening the spectrum of disease. A minimal dataset of clinical evaluations and follow-up timeline has been proposed for proper management of patients affected by this ultrarare disorder.

Abstract

Malan Syndrome (MS) is an ultra-rare overgrowth genetic syndrome due to heterozygous variants or deletions in the Nuclear Factor I X (NFIX) gene. It is characterized by an unusual facial phenotype, generalized overgrowth, intellectual disability (ID) and behavioral problems. Even though limitations in cognitive and adaptive functioning have been previously described, systematic studies on MS cohorts are still lacking. Here, we aim to define the cognitive and adaptive behavior profile of MS children and adolescents, providing quantitative data from standardized evaluations. Subjects included in this study were evaluated from October 2020 to January 2022 and the study is based on a retrospective data archive: fifteen MS individuals were recruited and underwent evaluation with Wechsler Intelligence Scales, Leiter International Performance Scales and Griffith Mental Development Scales for cognitive profiles and with Vineland Adaptive Behavior Scales-II Edition (VABS-II) for adaptive functioning. Language skills and visuomotor integration abilities were assessed too. Comparisons and correlations between scales and subtests were performed. All the assessed MS individuals showed both low cognitive and adaptive functioning. One subject presented with mild ID, five had moderate ID and eight showed severe ID. One female toddler received a diagnosis of psychomotor delay. Linguistic skills were impaired in all individuals, with language comprehension relatively more preserved. Results revealed significant differences between VABS-II subdomains and a strong relationship between cognitive and adaptive functioning. All subjects exhibited mild to moderate ID and adaptive behavior lower than normal, with communication skills being the most affected. Regarding the daily living skills domain, personal and community subscale scores were dramatically lower than for the domestic subdomain, highlighting the importance of considering behavior within developmental and environmental contexts

The paper deals with the technical and economic analysis of a project aimed at transforming an existing plant, used for the anaerobic digestion of zoo-technical and agroforest biomass and including a CHP system of about 1 MW of electric capacity, into a facility producing bio-methane for automotive and/or stationary power applications. A comparison of different biogas upgrading technologies is performed, aimed at selecting the technology most appropriate to the size and typology of application under evaluation. Similarly, an analysis is performed to evaluate the opportunity of installing a bio-methane liquefaction facility, to simplify the management and transportation of the fuel, to be used in vehicles. The economic analysis is performed by considering the incentives presently available in Italy for bio-methane producers. Abstract

Human chromosome 15q25 is involved in several disease-associated structural rearrangements, including microdeletions and chromosomal markers with inverted duplications. Using comparative fluorescence in situ hybridization, strand-sequencing, single-molecule, real-time sequencing and Bionano optical mapping analyses, we investigated the organization of the 15q25 region in human and nonhuman primates. We found that two independent inversions occurred in this region after the fission event that gave rise to phylogenetic chromosomes XIV and XV in humans and great apes. One of these inversions is still polymorphic in the human population today and may confer differential susceptibility to 15q25 microdeletions and inverted duplications. The inversion breakpoints map within segmental duplications containing core duplicons of the GOLGA gene family and correspond to the site of an ancestral centromere, which became inactivated about 25 million years ago. The inactivation of this centromere likely released segmental duplications from recombination repression typical of centromeric regions. We hypothesize that this increased the frequency of ectopic recombination creating a hotspot of hominid inversions where dispersed GOLGA core elements now predispose this region to recurrent genomic rearrangements associated with disease.

Abstract

The detailed synthesis/design optimization of a hybrid solid oxide fuel cell–gas turbine (SOFC–GT) power plant is presented in this paper. In the first part of the paper, the bulk-flow model used to simulate the plant is discussed. The performance of the centrifugal compressors and radial turbine is determined using maps, properly scaled in order to match the values required for mass flow rate and pressure ratio. Compact heat exchangers are simulated using Colburn and friction factor correlations. For the SOFC, the cell voltage versus current density curves (i.e. polarization curves) are generated on the basis of the Nernst potential and overvoltages. Validation of the SOFC polarization curves is accomplished with data available from Siemens Westinghouse. Both the steam–methane pre-reforming and internal reforming processes are modeled assuming the water–gas shift reaction to be equilibrium-controlled and the demethanization reactions to be kinetically controlled. Finally, a thermoeconomic model is developed by introducing capital cost functions for each plant component. The whole plant is first simulated for a fixed configuration. Then, a synthesis/design optimization of the plant is carried out using a traditional single-level approach. The results of the optimization are presented and discussed.

Abstract

This paper investigates the integration of renewable energy sources and water systems, presenting a novel solar system producing simultaneously: electrical energy, thermal energy, cooling energy and domestic water. The polygeneration system under analysis includes photovoltaic/thermal solar collectors (PVT), a multi-effect distillation (MED) system for seawater desalination, a single-stage LiBr-H2O absorption chiller and additional components, such as storage tanks, auxiliary heaters and balance of plant devices. The PVT produces simultaneously electrical energy and thermal energy, at a maximum temperature of about 100 °C. The electrical energy is delivered to the grid, whereas the thermal energy may be used for different scopes. First, the thermal energy may be used for heating purposes and/or domestic hot water production. As an alternative, the solar thermal energy can be used to drive an absorption chiller, producing chilled water for space cooling. Finally, the solar energy, in combination with the thermal energy produced by an auxiliary biomass-fired heater, may be used by the MED system to convert seawater into domestic water. The system is dynamically simulated by means of a zero-dimensional transient simulation model. The simulation model also includes detailed control strategies, for the management of the solar thermal energy and for the control of the whole system. Results show an excellent energetic performance of the system under investigation whereas the economic profitability is good only in case of public funding.

Abstract

Recently a method to evaluate deformation imaging, 2D speckle strain echocardiography(2DSE), has been developed. We evaluated a new phase analysis technique to quantify left ventricular asynchrony from speckle strain curves. In conclusion our study shows that this new phase analysis technique is a powerful tool to quantify left ventricular asynchrony and to select responders to CRT.

Abstract

In this paper, the optimization of a hybrid solid oxide fuel cell–gas turbine (SOFC–GT) power plant is presented. The plant layout is based on an internal reforming SOFC stack; it also consists of a radial gas turbine, centrifugal compressors and plate-fin heat exchangers. In the first part of the paper, the bulk-flow model used to simulate the plant is presented. In the second part, a thermoeconomic model is developed by introducing capital cost functions. The whole plant is first simulated for a fixed configuration of the most important synthesis/design (S/D) parameters in order to establish a reference design configuration. Next a S/D optimization of the plant is carried out using a traditional single-level approach, based on a genetic algorithm. The optimization determined a set of S/D decision variable values with a capital cost significantly lower than that of the reference design, even though the net electrical efficiency for the optimal configuration was very close to that of the initial one. Furthermore, the optimization procedure dramatically reduced the SOFC active area and the compact heat exchanger areas.

Abstract

Summary. Background: Low ankle–brachial Index (ABI) identifies patients with symptomatic and asymptomatic peripheral arterial disease. The aim of this study was to correlate ABI value (normal or low) with 1-year clinical outcome in patients hospitalized for acute coronary syndromes or cerebrovascular diseases (CVD). Methods: ABI was measured in consecutive patients hospitalized because of acute myocardial infarction, unstable angina, stroke or transient ischemic attack (TIA). An ABI lower than or equal to 0.90 was considered abnormal. The primary outcome of the study was the composite of non-fatal acute myocardial infarction, non-fatal ischemic stroke, and death from any cause during the year following the index event. Results: An abnormal ABI was found in 27.2% of 1003 patients with acute coronary syndromes, and in 33.5% of 755 patients with acute CVD. After a median follow-up of 372 days, the frequency of the primary outcome was 10.8% (57/526) in patients with abnormal ABI and 5.9% (73/1232) in patients with normal ABI [odds ratio (OR) 1.96; 95% CI 1.36–2.81]. Death was more common in patients with abnormal ABI (OR 2.05; 95% CI 1.31–3.22). Cardiovascular mortality accounted for 81.7% of overall mortality. ABI was predictive of adverse outcome after adjustment for vascular risk factors in the logistic regression analysis (OR 1.93; 95% CI 1.24–3.01). The predictive value of ABI was mainly accounted for by patients hospitalized for acute coronary syndromes (adverse outcome: 12.8% in patients with abnormal ABI and 5.9% in patients with normal ABI, OR 2.35; 95% CI 1.47–3.76). Conclusions: An abnormal ABI can be found in one-third of patients hospitalized for acute coronary or cerebrovascular events and is a predictor of an adverse 1-year outcome.
