Abstract

The coronary arteries are the major blood vessels supplying the heart. Under abnormal conditions, the coronary arteries may become stenosed, or narrowed. Coronary artery stenosis may cause myocardial ischemia, the insufficient supply of blood to the heart, and possibly lead to infarction, the deterioration of tissue due to cessation of blood supply. Coronary artery stenosis can be treated by percutaneous transluminal coronary angioplasty (PTCA), a method in which a balloon attached to a catheter, a tube-like structure, is inserted into the coronary artery, and inflation of the balloon causes dilation or widening of the narrowed vessel. However, coronary artery stenosis may recur after PTCA. The effectiveness of computer-assisted analysis in assessing restenosis lesions was compared with visual estimates in 932 patients. The effect of these assessments on the success of angioplasty and recurrence of stenosis was also examined. There were significant differences between computer-derived measurements and visual estimates of the severity of stenosis before, immediately after and six months after PTCA. Because of these differences in measurements of the severity of stenosis, it was difficult to consistently determine the rate at which stenosis following PTCA would likely occur again and the rate of success with PTCA. 3. Fate of side branches after intracoronary implantation of the Gianturco-Roubin flex-stent for acute or threatened closure after percutaneous transluminal coronary angioplasty

Abstract

Side branch occlusion may occur in the course of percutaneous transluminal coronary angioplasty (PTCA), particularly if complicated by site dissection. Concern that the additional placement of a stent may further jeopardize side branches is logical. Consequently, this study analyzed pre PTCA, post-PTCA, poststent, and 6-month follow-up angiograms of 100 consecutive patients in whom 103 Gianturco-Roubin stents were implanted for acute or threatened closure after PTCA. Side branches were defined as major (>50% of the stented vessel diameter) and minor (50% stenosis), and 129 minor branches were analyzed. Seven major branches (6%), all of which were diseased before PTCA, and 23 minor branches (18%) were lost after PTCA. Immediately after stent insertion, only 1 additional major and 1 minor branch were lost, whereas 2 of 7 major (29%) and 9 of 23 minor (39%) branches reappeared. At follow up angiography, 7 major branches (6%) were more stenosed and 6 (6%) were improved compared with the angiogram before PTCA. Only 2 major (2%) and 5 minor (4%) branches remained occluded. Additionally, 2 major and 1 minor branch, which were patent after PTCA and stenting, were occluded at follow-up as a result of total occlusion of the stented segment. It is concluded that: (1) stent implantation is rarely associated with side branch loss; (2) more typically, recovery of patency of branches occluded by PTCA occurs immediately after stenting; (3) as a delayed effect, a stent may facilitate later reopening of branches occluded after PTCA; and (4) excellent patency of branches of the stented segment is maintained in the follow-up period.
