OBJECTIVE: The study reports the efficacy of sequential instillations of intralesional percutaneous polidocanol in the treatment of skeletal ABC. The study also analyses the pain relief, recurrence, radiological outcome and complications after percutaneous sclerotherapy. MATERIALS AND METHODS: A total of 43 patients with ABC were managed by fluoroscopy assisted percutaneous intralesional sclerotherapy with 3% Injection Polidocanol under suitable anaesthesia. The pain relief was assessed by Visual Analog Scale (VAS) and the progressive healing of the lesion by Rastogi Classification. Patients were evaluated clinico-radiologically, pre-operatively and at monthly interval for the first three months and then at 6 months, one year and two years follow up. RESULT: The VAS score decreased significantly at six months follow up and was zero (No pain) at one year follow up. Out of 43 patients, 37 patients had Degree I (excellent) residual lesion and 6 patients had degree II (good) lesion at one year follow-up as per Rastogi classification. The lesions resolved completely by two-year follow-up with all 43 patients having Degree (excellent) I resolution. CONCLUSION: Our study highlights the efficacy of percutaneous intralesional polidocanol in the treatment of ABC's. The procedure is safe, simple and effective with excellent outcome. Sclerotherapy is highly beneficial for deep lesions which are difficult to assess and for comorbid patients who are medically unfit for surgery. Our study strongly advocates the use of sclerotherapy as the first line treatment for ABC, considering its clinical and radiological efficacy.

Chickpea is the fourth most important legume crop contributing 15.42% to the total legume production and a rich source of proteins, minerals, and vitamins. Determination of genetic diversity of wild and elite cultivars coupled with early flowering and higher seed germination lines are quintessential for variety improvement.In the present study, we have analyzed the genetic diversity, population structure, cross-species transferability, and allelic richness in 50 chickpea collections using 23 Inter simple sequence repeats (ISSR) markers. The observed parameters such as allele number varied from 3 to 16, range of allele size varied from 150 to 1600 bp and polymorphic information content (PIC) range lies in between 0.15 and 0.49. Dendrogram was constructed with ISSR marker genotypic data and classified 50 chickpea germplasms into groups I and II, where the accession P 74 - 1 is in group I and the rest are in group II. Dendrogram, Principal component analysis (PCA), dissimilarity matrix, and Bayesian model-based genetic clustering of 50 chickpea germplasms revealed that P 74 - 1 and P 1883 are very diverse chickpea accessions.Based on genetic diversity analysis, 15 chickpea germplasm having been screened for early flowering and higher seed germination and found that the P 1857-1 and P 3971 have early flowering and higher seed germination percentage in comparison to P 1883 and other germplasm. These agronomic traits are essential for crop improvement and imply the potential of ISSR markers in crop improvement.

We performed a retrospective observational study to analyze the neurological recovery pattern in patients with a sub-laminar retro-thecal epidural abscess managed at our tertiary apex center from 2014 to 2020. We evaluated the Maximal Spinal Cord Compression (MSCC) ratio on Magnetic Resonance Imaging (MRI), the time interval between the appearance of neurological deficit and the initiation of management, spasticity as per Modified Ashworth Scale, presence of drug resistance, and the Lower Extremity Motor Score (LEMS). All patients were given anti-tubercular chemotherapy. We surgically managed 8 patients of which 6 required decompression alone, while 2 patients required additional instrumentation. 2 patients were managed conservatively of which 1 responded favorably to conservative treatment while the other patient showed a worsening of neurology following the detection of drug resistance and abrupt discontinuation of chemotherapy. The mean LEMS on admission was 20.2, which improved to 38.5 at the end of 1 year (p-value 0.05).

Introduction: Larsen syndrome (LS) is a rare genetic disorder affecting mainly the connective tissues. It is characterized by characteristic facial anomalies, cervical kyphosis, cardiorespiratory disorders, and multiple joint dislocations. We present a case of a 15-year-old male with unstable neuropathic knee joint instability in a known case of LS. The paucity of literature on the management of this rare condition puts an orthopedician in dilemma regarding the optimal treatment. Case Report: A 15-year-old male, known case of LS, presented to our outpatient department with pain and instability in the right knee for 2 years. Clinically, the patient was having syndromic facies. The diagnosis of LS was confirmed on gene mapping. The right knee was swollen with medial joint line tenderness and restricted flexion. The patient had coronal plane valgus instability. The hypertrophied synovium eroded the articular surface. The radiograph of knee was suggestive of neuropathic arthropathy in fragmentation stage. Conclusion: Orthopedician should be aware of such rare entity with its bony and soft-tissue manifestations. Neuropathic knee is not an absolute contraindication to total knee replacement, especially with advanced prosthesis. Primary arthrodesis to be considered in young adults with instability.

Introduction: Synovial chondromatosis of the knee is an unusual, rare, benign metaplastic neoplasm of the synovium, characterized by the formation of focal cartilaginous nodules in the synovium. It predominantly involves the anterior compartment of knee and extensive, disseminated involvement is rarely reported. Diagnosis is usually established with clinicoradiological imaging and confirmed by histology. The paucity of literature on the management of synovial chondromatosis puts an orthopedician in dilemma regarding the optimal surgical intervention. Case Report: We report a case of 50-year-old male who presented with severe pain and swelling in his left knee associated with functional incapacitation. Magnetic resonance imaging demonstrated diffuse proliferation of synovium with mild effusion displaying multiple, disseminated calcific nodules. We managed the case by open removal of loose bodies and radical synovectomy through combined anterior and posterior approach. At 4 years follow-up, there are no clinical or radiological signs of recurrence. Conclusion: Extensive involvement of the knee with calcific nodules is extremely rare. Treatment is controversial which usually includes removal of loose bodies combined with an open or arthroscopic synovectomy. Our case report recommends single stage, open retrieval of loose bodies combined with radical synovectomy to decrease the incidence of recurrence in extensive synovial chondromatosis. To the best of our knowledge, disseminated synovial chondromatosis of the knee, managed by open, combined approach, has been reported only once in the literature.

Introduction: Hibernoma is an uncommon benign tumor composed of multivacuolated brown adipocytes described in the literature a decade back. Intraosseous hibernomas are extremely rare with propensity to affect axial skeleton. Involvement of appendicular skeleton has been reported only twice in the literature till date. Case Report: We present a case of solitary, painless mass in the left proximal tibia mimicking neoplasm in a 35-year-old male. The plain radiographs revealed a lytic lesion with well-defined margin at the junction of metaphyseal-diaphyseal region of the left proximal tibia. The lesion was hyperintense on T2 and hypointense on T1 with heterogenous enhancement on contrast imaging. FDG-PET showed high uptake of fluorodeoxyglucose. Histopathological examination revealed the presence of multivacuolated cytoplasm with abundant mitochondria suggestive of hibernoma. On immunohistochemistry, the cells were positive for S-100. Conclusion: The findings of imaging modalities are non-specific and histopathological examination is required to confirm the diagnosis of hibernoma. Intraosseous hibernoma needs to be considered as a differential diagnosis of sclerotic bone lesion. Further, evaluation with regular follow-up with imaging will be required if the mass undergoes any clinical changes. Both radiologists and pathologists, need to be aware of this rare entity to avoid misdiagnosis of the uncommon lesion.

Introduction: Gaucher’s disease is a congenital lysosomal storage disorder caused by an autosomal recessive mutation in B-glucocerebrosidase. It is a multi-system disease, wherein patients present with hematological abnormalities, joint pain, osteonecrosis, and developmental delay. We present a case of 38-year-old male with a painless solitary soft-tissue swelling over the left proximal tibia, eventually diagnosed it to be a case of Gaucher’s disease. This case is unique in the literature, because this subcutaneous Gaucher mass was not associated with a significant past history and was being evaluated as a standard approach to a bone tumor. Case Presentation: A 38-year-old man presented to our outpatient department with a solitary, painless soft-tissue swelling of the left proximal tibia noticed 6 months back, which has gradually progressed to a present size of 9x5 cm over a period of 6 months. General physical examination revealed moderate splenomegaly. Radiographs showed an osteolytic lesion in the left proximal tibia without cortical erosion. Radiographs for skeletal survey revealed similar osteolytic lesions elsewhere. Hematological investigations revealed thrombocytopenia. A serum protein electrophoresis was found to be normal and the urine was negative for myeloma proteins. Blood workup for endocrine abnormalities was within normal limits. MRI of the lesion suggests bone infarct. Biopsy from the lesion showed a giant binucleate storage cell filled with glucocerebrosides suggestive of Gaucher’s disease. The diagnosis was confirmed by elevated plasma levels of glucocerebrosidases. Conclusion: Gaucher’s disease is a rare metabolic disease of the bone which may mimic a primary bone tumor or metastasis. A step-wise meticulous approach with biopsy and elevated plasma levels of glucocerebrosidase helps establish the diagnosis. Once must have a high index of suspicion for Gaucher’s disease in an adult with multiple osteolytic lesions without any significant past medical and surgical history.

INTRODUCTION: There exists sparse literature on Gunshot injuries (GSI) to the pediatric spine, its natural course, management protocols and outcomes following surgical intervention. Here, we present a projectile injury to the paediatric lumbar spine without any osseous disruption in a 5-year-old child requiring surgical management. CASE PRESENTATION: A 5-year-old child presented with a gunshot injury to the pediatric lumbar spine. On radiological imaging, there was no osseous disruption and an intraspinal canal projectile was embedded in an organised infected complex, compressing onto the left-sided L5 traversing nerve root with resultant grade 4 motor power of the left Extensor Hallucis Longus (EHL). Under fluoroscopic guidance using two separate mini-open incisions, the pellets were extracted. One from the spinal canal through left-sided L4 laminotomy and the other from the right paraspinal region. Post-surgical decompression and pellet retrieval there was gradual recovery to grade 5 motor power of Left EHL and improvement in the general condition of the child. CONCLUSION: The absence of osseous disruption can be attributed to the size of the projectile, widely spaced interlaminar spaces in the lumbar spine due to the absence of lumbar lordosis in the pediatric group and flexibility of the pediatric spine owing to its ligamentous laxity.

Globally, floods as dynamic hydraulic hazard have caused widespread damages to both socioeconomic conditions and environment at various scales. Managing flood and management of water resource is a global challenge under the changing climatic condition. This study assessed flood susceptibility in the Bhagirathi sub-basin, India using entropy information theory and geospatial technology. Twelve flood susceptibility parameters such as land use/land cover, normalized difference vegetation index (NDVI), slope, elevation, geology, geomorphology, normalized difference water index (NDWI), soil, drainage density, average rainfall, maximum temperature, and humidity during monsoon season were utilized to examine flood susceptibility. Receiver operating characteristics (ROC) curve and Leave-One-Out Cross-Validation (LOOCV) techniques were carried out to validate flood susceptibility map. Kappa statistics was also used to check the reliability of the flood susceptibility model. Findings of the study revealed that nearly 45% area of the sub-basin was highly susceptible to flood followed by moderate (29.3%), very high (19%), low (6.9%), and very low (0.2%). These findings also revealed that nearly 92% area in the eastern, north-eastern, and deltaic sub-basin was susceptible to floods. The north Indian Ganga basin is one of the densely populated basins of the world. Most agricultural and industrial contaminants drained throughout the river length are likely to be accumulated in the lower part of the Ganga basin. We used ten derived irrigation suitability parameters, which are obtained from 495 sampling points locations, besides using long term climate data (GLDAS_NOAH025_M) using “Technique for Order of Preference by Similarity to Ideal Solution” (TOPSIS) model to get the irrigation suitability map. Multi-Criteria Decision Making (MCDM) using TOPSIS helps make the best choices from the available finite number of alternatives based on their ranking. The obtained entropy weight for irrigation suitability parameters such as Electrical Conductivity (Ec), Sodium Adsorption Ratio (SAR), Magnesium Hardness (MH), Sodium Percent (Na%), Total Hardness (TH), Kelly’s ratio (KR), Permeability Index (PI), Chloride concentration (Cl−), Groundwater Level Fluctuation (GWLF), and lang factor (Df) are found to be 0.08, 0.14, 0.02, 0.02, 0.04, 0.08, 0.01, 0.32, 0.29 and 0.01 respectively. We find that SAR, Cl−, and GWLF controls the water quality for irrigation in the Lower Ganga basin since these parameters have relatively higher entropy weights (more than 0.10). The results obtained from the computed performance index or the closeness coefficient show that the area percent having very good, good, and very poor groundwater quality in the Lower Ganga basin is 34.67%,42.36%, and 22.97%, respectively. The LULC change pattern indicates that the percentage change of water and agricultural land was -11.96 and -0.86%, whereas an increase in the settlement area of 131.42% for the period between 2000 and 2015.

A 14-year-old boy with proximal thoracic kyphoscoliosis associated with the bilateral absence of thoracic pedicles presented with progressive deformity, paraparesis, and difficulty in ambulation. The case was managed by preoperative halo traction, single-stage 2-level vertebral column resection, decompression, and arthrodesis of thoracic vertebrae. Two years postoperatively, the patient showed neurological improvement, leading to unassisted ambulation and fusion at the corpectomy site.Preoperative halo-gravity traction restores the sagittal and coronal balance, improves the functional status of the patient, and corrects the deformity to some extent. 3D printed models help in better understanding of osseous anatomy and minimizing intraoperative time.

Further selected 15 diverse chickpea screened for early flowering and high seed germination. Among 15 diversechickpea germplasms P 1857-1 and P 3971 has early flowering and high seed germination compared to P 1883 and other germplasm. Characterization of these diverse chickpea for early flowering and high seed germinationwould help in reducing crop duration and enhancing seed qualities. Utilization of these ISSRs markers in diversity analysis and population structure characterization of 50 chickpea germplasm suggests their wider efficacy for molecular breeding ofearly flowering and high seed germination.

On a local and regional level, climate change has had a significant impact on precipitation in the global climatic state. The purpose of this research is to examine the trend and character of urban precipitation in the world’s most densely inhabited metropolis. From 1981 to 2020, 40 years of monthly and annual precipitation data from 50 major metropolitan cities throughout the world, based on population statistics, were analysed. The monthly and annual precipitation analysis was done using a homogeneity test, shifting point test, non-parametric Modified Mann Kendall test, and also through computing the magnitude of the trend using Sen’s slope estimate. According to the findings of the study, the most homogeneous data was obtained in May (90 %) and the least in September (74%). In 2002, the highest number of breakpoints were found in July (9 cities) and August (8 cities). The month of January has the largest significant positive trend (10 cities) whereas annually it has 20 cities. The monthly maximum of the significant negative trend was discovered in February (4 cities) and annually in 2 main cities. In November, the maximum positive and minimum positive Sen’s slope values were found to be 82% and 56%, respectively. The findings of this study are important for future water resource projections, flood or drought predictions, and engineering, scientific, industrial, agricultural, and social studies. The goal of this research is to come up with a good plan for dealing with urban flash floods and droughts as precipitation acts as the key parameter of the hydrological cycle.

This work is a climatological evaluation of terrestrial water storage anomalies (TWSAs), which act as driving forces for sustainable development, in one of the most populous countries of the world. The objective of this work is to evaluate RL06 mascon data from the GRACE and GRACE-FO satellite missions over India to explore seasonal and interannual changes in terrestrial water storage, encompassing an area of ~3.29 million km2 with 285 grid points, from 2002 through to 2020. Several statistical tests are performed to check the homogeneity (i.e., Pettitt’s test, the BRT, the SNHT, and the VNRT). Most of the homogeneous data are found in winter, pre-monsoon, and post-monsoon, approximately above 42% to 47%, and the least are found in monsoons and annual with only 33%, at a 95% significance level. According to Pettitt’s test, the majority of the breakpoints are present in 2014 for winter, 2012 for pre-monsoon, 2011 for monsoons and post-monsoon, and 2008 as well as 2011 for annual. Furthermore, to detect trends and magnitudes we employed the nonparametric MK test, the MMK test, Sen’s slope estimator, and the parametric SLR test. According to the MK and MMK tests, the most significant negative and positive trends indicate the chances of droughts and floods, respectively. The Indo–Gangetic region shows the highest declination. According to Sen’s slope and the SLR test, the most declining magnitude is found in Delhi, Panjab, Uttrakhand, the northern part of Rajasthan, and Uttar Pradesh. Based on our findings, the average declining rate of yearly terrestrial water storage data from the MK, MMK, and SLR tests is −0.0075 m (−0.75 cm/year) from 2002 to 2020. Koppen–Geiger climate zones are also used to depict the seasonal and interannual descriptive statistics of TWSA trends. Interestingly, the annual means of arid desert cold (−0.1788 cm/year) and tropical savanna (−0.1936 cm/year) have the smallest declining trends when compared to other climatic zones. Northern Indian regions’ temperate dry winter, hot/warm summer, and dry arid steppe hot regions show the maximum declining future trend. This study could be useful in planning and managing water resources, agriculture, and the long-term growth of the country by using an intelligent water delivery system.

Quorum sensing modulatory effect of linalool was tested for the first time against Pseudomonas syringae pv. tomato DC 3000, a bacterial plant pathogen responsible for causing huge losses worldwide. DC 3000 extensively regulates its virulence traits through quorum sensing mechanism involving expression of psyI, an acyl-homoserine lactone (AHL) synthase gene and psyR, a luxR-type regulator. In this study, exposure of DC 3000 to 80 ppm linalool concentration resulted in reduced biofilm formation, hampered motility, decreased AHL production, and reduced secretion of plant cell wall-degrading enzymes followed by negligible effect on the bacterial count and its metabolic activity. Extracellular polymeric substances (EPS), which play a crucial role in the development of biofilm and subsequent infection was significantly reduced which was further confirmed by Fourier Transform Infrared Spectroscopy. The qRT PCR analysis of the gene expression analysis of virulence genes (syringafactin production gene syfA, type III secretion system gene hrpA, flagellar genes fleQ and fliC, and coronatine production gene cfl unveiled significant downregulation of the same under Lin 80 ppm concentration. To further confirm the aforementioned possibilities, docking simulations run between PsyR and linalool suggested a strong interaction with the developed protein model. Overall, the treatment of DC 3000 with Lin 80 ppm affected the phenotypic and genotypic expressions associated with quorum sensing, thereby significantly reducing the infection rate as observed in in-vivo plant assay.

Restoration of proper joint line position after primary total knee arthroplasty (TKA) is important for improved knee function and kinematics. We reviewed the magnitude of joint line alteration and the resulting effect on post-operative knee outcomes scores at one year follow-up.120 patients who underwent TKA for primary osteoarthritis knee were included. Assessment of joint line position before and after surgery was performed with the help of bony landmarks (excluding osteophytes): Medial Epicondyle Joint Line distance (MEJL), Lateral Epicondyle Joint Line distance (LEJL) and Fibula Head Joint Line distance (FHJL) were calculated. Post-operative knee function was assessed using 'The Western Ontario and McMaster Universities Arthritis Index' (WOMAC) and 'Knee Society Score' (KSS) at one year follow-up.Joint line elevation was observed in 104/120 (86.7%) patients and 16/120 (13.3%) patients had no joint line elevation. The mean joint line elevation was 3.00 (±2.13) mm. The patients were sub-classified into two study groups: Group A- Joint line elevation5 mm and Group B- Joint line elevation ≥ 5 mm. The mean post-op one-year KSS score was significantly higher in patients in Group A compared to Group B (52.82 ± 7.564 vs. 40.73 ± 7.146; p 0.001). The mean post-op one-year WOMAC score was lower in patients in Group A compared to Group B (65.51 ± 14.762 vs.75.64 ± 8.203) and the difference was statistically significant (p = 0.002).Elevation of joint line ≥5 mm from the pre-operative value has a negative impact on post-operative functional outcome in primary TKA.

Background The genetic base of soybean cultivars in India has been reported to be extremely narrow, due to repeated use of few selected and elite genotypes as parents in the breeding programmes. This ultimately led to the reduction of genetic variability among existing soybean cultivars and stagnation in crop yield. Thus in order to enhance production and productivity of soybean, broadening of genetic base and exploring untapped valuable genetic diversity has become quite indispensable. This could be successfully accomplished through molecular characterization of soybean genotypes using various DNA based markers. Hence, an attempt was made to study the molecular divergence and relatedness among 29 genotypes of soybean using SSR markers. Methods and results A total of 35 SSR primers were deployed to study the genetic divergence among 29 genotypes of soybean. Among them, 14 primer pairs were found to be polymorphic producing a total of 34 polymorphic alleles; and the allele number for each locus ranged from two to four with an average of 2.43 alleles per primer pair. Polymorphic information content (PIC) values of SSRs ranged from 0.064 to 0.689 with an average of 0.331. The dendrogram constructed based on dissimilarity indices clustered the 29 genotypes into two major groups and four sub-groups. Similarly, principal coordinate analysis grouped the genotypes into four major groups that exactly corresponded to the clustering of genotypes among four sub-groups of dendrogram. Besides, the study has reported eight unique and two rare alleles that could be potentially utilized for genetic purity analysis and cultivar identification in soybean. Conclusion In the present investigation, two major clusters were reported and grouping of large number of genotypes in each cluster indicated high degree of genetic resemblance and narrow genetic base among the genotypes used in the study. With respect to the primers used in the study, the values of PIC and other related parameters revealed that the selected SSR markers are moderately informative and could be potentially utilized for diversity analysis of soybean. The clustering pattern of dendrogram constructed based on SSR loci profile displayed good agreement with the cultivar’s pedigree information. High level of genetic similarity observed among the genotypes from the present study necessitates the inclusion of wild relatives, land races and traditional cultivars in future soybean breeding programmes to widen the crop gene pool. Thus, hybridization among diverse gene pool could result in more heterotic combinations ultimately enhancing genetic gain, crop yield and resistance to various stress factors. The enthalpy increments of UF4 compound were determined along with enthalpy of mixing of UF4 in (Li0.59K0.41)Cl melt. Enthalpy of mixing of lithium chloride and potassium chloride binary melt measured at 933 K, was used for determining ternary enthalpy of mixing. Gibbs energy of mixing of UF4(s) in lithium potassium chloride eutectic melt (LKE) was calculated from enthalpy of mixing data. These values were used for calculation of solubility limit of UF4 in LKE, at 933 K. DTA analysis of partial phase diagram along the UF4-LKE isopleth was carried out to determine the solubility limits of UF4 in LKE at different temperatures. The solubility limit of UF4 in LKE, ˜5 mole % UF4 at 933 K, calculated from thermodynamic parameters showed reasonable agreement with DTA results.

Worldwide Interoperability for Microwave Access applications and QoS specifications are based upon class based schemes. These service classes effect to QoS for user fairness and system utilization. Improved MAX C/I algorithm considers user waiting time and channel to interference ratio but not class based scheduling schemes, after including this neglecting condition algorithm solves the problem of long distance priorities. In Prioritized C/I, if two users having same priority then scheduler allocates execution time according to Channel Quality Indicator (CQI) values. Identification of plant species is a crucial process in natural products. Ocimum, often referred to as the queen of herbs, is one of the most versatile and globally used medicinal herbs for various health benefits due to it having a wide variety of pharmacological activities. Despite there being significant global demand for this medicinal herb, rapid and comprehensive metabolomic fingerprinting approaches for species- and variety-specific classification are limited. In this study, metabolomic fingerprinting of five Ocimum species (Ocimum basilicum L., Ocimum sanctum L., Ocimum africanum Lour., Ocimum kilimandscharicum Gurke., and Hybrid Tulsi) and their varieties was performed using LC-MS, GC-MS, and the rapid fingerprinting approach FT-NIR combined with chemometrics. The aim was to distinguish the species- and variety-specific variation with a view toward developing a quality assessment of Ocimum species. Discrimination of species and varieties was achieved using principal component analysis (PCA), partial least squares discriminate analysis (PLS-DA), data-driven soft independent modelling of class analogy (DD-SIMCA), random forest, and K-nearest neighbours with specificity of 98% and sensitivity of 99%. Phenolics and flavonoids were found to be major contributing markers for species-specific variation. The present study established comprehensive metabolomic fingerprinting consisting of rapid screening and confirmatory approaches as a highly efficient means to identify the species and variety of Ocimum, being able to be applied for the quality assessment of other natural medicinal herbs.
