Abstract

Objective: The first COVID-19 pandemic wave was a period of reduced surgical activity and redistribution of resources to only those with late stage or critical presentations. This Vascular and Endovascular Research Network COVID-19 Vascular Service (COVER) study aimed to describe the six-month outcomes of patients who underwent open surgery and or endovascular interventions for major vascular conditions during this period. Methods: In this international, multicentre, prospective, observational study, centres recruited consecutive patients undergoing vascular procedures over a 12-week period. The study opened in March 2020 and closed to recruitment in August 2020. Patient demographics, procedure details, and post-operative outcomes were collected on a secure online database. The reported outcomes at 30 days and six months were post-operative complications, re-interventions, and all cause in-hospital mortality rate. Multivariable logistic regression was used to assess factors associated with six-month mortality rate. Results: Data were collected on 3 150 vascular procedures, including 1 380 lower limb revascularisations, 609 amputations, 403 aortic, 289 carotid, and 469 other vascular interventions. The median age was 68 years (interquartile range 59, 76), 73.5% were men, and 1.7% had confirmed COVID-19 disease. The cumulative all cause in-hospital, 30-day, and six-month mortality rates were 9.1%, 10.4%, and 12.8%, respectively. The six-month mortality rate was 32.1% (95% CI 24.2–40.8%) in patients with confirmed COVID-19 compared with 12.0% (95% CI 10.8–13.2%) in those without. After adjustment, confirmed COVID-19 was associated with a three times higher odds of six-month death (adjusted OR 3.25, 95% CI 2.18–4.83). Increasing ASA grade (3–5 vs. 1–2), frailty scores 4–9, diabetes mellitus, and urgent and or immediate procedures were also independently associated with increased odds of death by six months, while statin use had a protective effect. Conclusion: During the first wave of the pandemic, the six-month mortality rate after vascular and endovascular procedures was higher compared with historic pre-pandemic studies and associated with COVID-19 disease.

Abstract

Objective: To document the recovery of vascular services in Europe following the first COVID-19 pandemic peak. Methods: An online structured vascular service survey with repeated data entry between 23 March and 9 August 2020 was carried out. Unit level data were collected using repeated questionnaires addressing modifications to vascular services during the first peak (March – May 2020, “period 1”), and then again between May and June (“period 2”) and June and July 2020 (“period 3”). The duration of each period was similar. From 2 June, as reductions in cases began to be reported, centres were first asked if they were in a region still affected by rising cases, or if they had passed the peak of the first wave. These centres were asked additional questions about adaptations made to their standard pathways to permit elective surgery to resume. Results: The impact of the pandemic continued to be felt well after countries’ first peak was thought to have passed in 2020. Aneurysm screening had not returned to normal in 21.7% of centres. Carotid surgery was still offered on a case by case basis in 33.8% of centres, and only 52.9% of centres had returned to their normal aneurysm threshold for surgery. Half of centres (49.4%) believed their management of lower limb ischaemia continued to be negatively affected by the pandemic. Reduced operating theatre capacity continued in 45.5% of centres. Twenty per cent of responding centres documented a backlog of at least 20 aortic repairs. At least one negative swab and 14 days of isolation were the most common strategies used for permitting safe elective surgery to recommence. Conclusion: Centres reported a broad return of services approaching pre-pandemic “normal” by July 2020. Many introduced protocols to manage peri-operative COVID-19 risk. Backlogs in cases were reported for all major vascular surgeries.

ObjectiveThe aim of this review is to summarize the fundamental concepts of AR technologies and conduct a scoping review of the impact of AR and mixed reality in vascular and endovascular surgery. MethodsA systematic search of MEDLINE, Scopus, and Embase was performed in accordance with the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines. All studies written in English from inception until January 8, 2021, were included in the search. Combinations of the following keywords were used in the systematic search string: (“augmented reality” OR “hololens” OR “image overlay” OR “daqri” OR “magic leap” OR “immersive reality” OR “extended reality” OR “mixed reality” OR “head mounted display”) AND (“vascular surgery” OR “endovascular”). Studies were selected through a blinded process between 2 investigators (JE and AS) and assessed using data quality tools. ResultsAR technologies have had a number of applications in vascular and endovascular surgery. Most studies (22/32, 69%) used 3D imaging of computed tomography angiogram–derived images of vascular anatomy to augment clinicians’ anatomical understanding during procedures. A wide range of AR technologies were used, with heads up fusion imaging and AR head-mounted displays being the most commonly applied clinically. AR applications included guiding open, robotic, and endovascular surgery while minimizing dissection, improving procedural times, and reducing radiation and contrast exposure. ConclusionsAR has shown promising developments in the field of vascular and endovascular surgery, with potential benefits to surgeons and patients alike. These include reductions in patient risk and operating times as well as in contrast and radiation exposure for radiological interventions. Further technological advances are required to overcome current limitations, including processing capacity and vascular deformation by instrumentation.

Abstract

BACKGROUND Technological advances have transformed vascular intervention over recent decades. Augmented reality (AR) is a subject of growing interest within surgery, with potential to improve the clinicians’ understanding of 3D anatomy and their processing of real-time information. The aim of this review was to summarise the fundamental concepts of these technologies and to systematically assess the literature currently applying AR to vascular surgery. METHODS A systematic literature review of ‘Medline,’ ‘Scopus’ and ‘Embase’ was performed according to PRISMA guidelines. Studies were selected by a blinded process between two investigators and assessed with data quality tools. RESULTS AR technologies have had a number of applications across vascular and endovascular surgery. The majority of studies use 3D imaging (e.g) CT angiogram derived images of vascular anatomy to augment the clinicians anatomical understanding during procedures. A wide range of AR technologies have been employed with ‘heads up’ fusion imaging and AR head-mounted displays the most commonly clinically applied. AR applications have included guiding open, robotic and endovascular surgery while minimising dissection, improving procedural times and reducing radiation and contrast exposure. Additionally, AR has been successfully applied to surgical training, with scope to improve technical and team communication skills. CONCLUSIONS AR has shown promising developments in the field of vascular and endovascular surgery, with potential benefits to surgeons and patients alike. These include reductions in patient risk and operating times while optimising contrast and radiation exposure for radiological interventions. Abstract

Background Over three-quarters of surgical trainees intend to undertake clinical fellowships to improve competence, confidence and the development of super specialised skills. At present there is no standardised national regulation for fellowships in the UK, leading to variability. The Association of Surgeons in Training (ASiT), an organisation representing surgical trainees in the UK and Ireland, sought to provide recommendations on the structure and quality assurance of fellowships in General Surgery. Methods A consensus session was held by ASiT at the Association of Surgeons of Great Britain and Ireland (ASGBI) 2017 Congress in Glasgow. A variety of perspectives on fellowships were presented by invited speakers, and thereafter a moderated discussion ensued in order to reach consensus. Live-polling using an electronic application facilitated consensus discussions. A writing group thereafter developed an initial consensus document which was then subject to electronic review by all authors and by ASiT council. Results Thirty-four delegates attended the consensus session. Views of the delegates and online discussants were that fellowships should be optional and not mandatory. There was (n = 16) no preference expressed between UK and international fellowships by half the group, while half expressed a preference for international fellowships. Regarding UK fellowships, the majority (n = 24) said that national registration of such fellowships should be established and (n = 23) agreed that a defined curriculum was required. However, national selection into fellowship training was not supported (n = 20). Perceived advantages of UK fellowships included ease of relocating within the same country (which was considered more family-friendly and cost-effective), and familiarity with the NHS system. Delegates considered the advantages of international fellowships to include: access to high-volume centres; diversity of case-mix; and structured, regulated fellowship programmes. Conclusion The consensus session found unanimous support for maintaining the option of both UK and international fellowships. While trainees support quality assurance of fellowships, they do not support a national selection process.
