In the traditional view, heavy deformed nuclei are like axially-symmetric prolate ellipsoids, rotating about one of the short axes. In the present picture, their shapes may be triaxial. The triaxial shape yields complex rotations, which actually well reproduce experimental data, as confirmed by state-of-the-art Configuration Interaction calculations. Two origins are suggested for the triaxiality: (i) binding-energy gain by the symmetry restoration for triaxial shapes, and (ii) another gain by specific components of the nuclear force, like tensor force and high-multipole (e.g. hexadecupole) central force. While the origin (i) produces basic modest triaxiality for virtually all deformed nuclei, the origin (ii) produces more prominent triaxiality for a certain class of nuclei. An example of the former is 154Sm, a typical showcase of axial symmetry but is now suggested to depict a modest yet finite triaxiality. The latter, prominent triaxiality, is discussed from various viewpoints for some exemplified nuclei including 166Er, and experimental findings. Many-body structures of the gamma band and the double-gamma band are clarified. Regarding the general features of rotational states of deformed many-body systems including triaxial ones, the well-known J(J+1) rule of rotational excitation energies is derived, within the quantum mechanical many-body theory, without resorting to the quantization of a rotating classical rigid body. This derivation is extended to finite K. The present picture of the rotation is robust and can be applied to various shapes or configurations, including clusters and molecules. Thus, two long-standing open problems, (i) occurrence and origins of triaxiality and (ii) quantum many-body derivation of rotational energy, are resolved. Their possible relations to Davydov's rigid-triaxial-rotor model are mentioned., Comment: 45 pages, 30 figures, minor revision from the v5 version

Nuclear Theory - Abstract
We report $J^\pi = 0^+$ ground-state energies and point-proton radii of $^4$He, $^8$Be, $^{12}$C, $^{16}$O and $^{20}$Ne nuclei calculated by the $ab$ $initio$ no-core Monte Carlo shell model with the JISP16 and Daejeon16 nonlocal $NN$ interactions. Ground-state energies are obtained in the basis spaces up to seven oscillator shells ($N_{\rm shell} = 7$) with several oscillator energies ($\hbar \omega$) around the optimal oscillator energy for the convergence of ground-state energies. These energy eigenvalues are extrapolated to obtain estimates of converged ground state energies in each basis space using energy variances of computed energy eigenvalues. We further extrapolate these energy-variance-extrapolated energies obtained in the finite basis spaces to infinite basis-space results with an empirical exponential form. This form features a dependence on the basis-space size but is independent of the value of $\hbar\omega$ used for the harmonic-oscillator basis functions. Point-proton radii for these states of atomic nuclei are also calculated following techniques employed for the energies. From these results, it is found that the Daejeon16 $NN$ interaction provides good agreement with experimental data up to approximately $^{16}$O, while the JISP16 $NN$ interaction provides good agreement with experimental data up to approximately $^{12}$C. Beyond these nuclei, the interactions produce overbinding accompanied by radii that are too small. These findings suggest and encourage further revisions of nonlocal $NN$ interactions towards the investigation of nuclear structure in heavier-mass regions., Comment: 24 pages, 18 figures, 2 tables, version published in Phys. However, its geographic distribution is becoming widespread. Here, we present a Japanese male who resided in the United States for 1 year, where he was diagnosed with pulmonary coccidioidomycosis that was accompanied by cavity formation. He did not tolerate antifungal therapy and consequently underwent partial resection of the upper lobe of his left lung upon his return to Japan. The patient’s symptoms improved after surgery. The trend toward global networking and logistics means that a diagnosis of coccidioidomycosis should be considered in routine practice in nonendemic areas. Due to the rarity of surgical treatment for this disease, prolonged follow-up is necessary. During the last follow-up, the patient was symptom-free.Keywords: coccidioidomycosis, Coccidioides immitis, Coccidioides posadasii, endemic disease

Here, we described pneumothorax in addition to pulmonary opacities with eosinophilia in a man with a positive P. westermani serology. He was misdiagnosed with chronic eosinophilic pneumonia (CEP) during the initial phase. Paragonimiasis can share similar clinical findings with CEP in cases where the worm is confined to the lungs. The findings of the current study suggest that paragonimiasis and CEP can be distinguished from each other by the presence of various symptoms. Notably, eosinophilia with pneumothorax should be an important diagnostic factor for paragonimiasis. Keywords: Paragonimus westermani, chronic eosinophilic pneumonia, paragonimiasis, pneumothorax

Nuclear Theory - Abstract
The ground-state energies and radii for $^{4}$He, $^{16}$O, and $^{40}$Ca are calculated with the unitary-model-operator approach (UMOA). In the present study, we employ the similarity renormalization group (SRG) evolved nucleon-nucleon ($NN$) and three-nucleon ($3N$) interactions based on the chiral effective field theory. This is the first UMOA calculation with both $NN$ and $3N$ interactions. The calculated ground-state energies and radii are consistent with the recent {\it ab initio} results with the same interaction. We evaluate the expectation values with two- and three-body SRG evolved radius operators, in addition to those with the bare radius operator. With the aid of the higher-body evolution of radius operator, it is seen that the calculated radii tend to be SRG resolution-scale independent. We find that the SRG evolution gives minor modifications for the radius operator., Comment: 11 pages, 7 figures

Meteorology & Atmospheric Sciences - Abstract
We investigate the forces and atmosphere-ionosphere coupling that create atmospheric dynamo currents using two rockets launched nearly simultaneously on 4 July 2013 from Wallops Island (USA), during daytime Sq conditions with ΔH of -30 nT. One rocket released a vapor trail observed from an airplane which showed peak velocities of >160 m/s near 108 km and turbulence coincident with strong unstable shear. Electric and magnetic fields and plasma density were measured on a second rocket. The current density peaked near 110 km exhibiting a spiral pattern with altitude that mirrored that of the winds, suggesting the dynamo is driven by tidal forcing. Such stratified currents are obscured in integrated ground measurements. Large electric fields produced a current opposite to that driven by the wind, believed created to minimize the current divergence. Using the observations, we solve the dynamo equation versus altitude, providing a new perspective on the complex nature of the atmospheric dynamo.

Dark matter (DM) simplified models are by now commonly used by the ATLAS and CMS Collaborations to interpret searches for missing transverse energy (ETmiss). The coherent use of these models sharpened the LHC DM search program, especially in the presentation of its results and their comparison to DM direct-detection (DD) and indirect-detection (ID) experiments. However, the community has been aware of the limitations of the DM simplified models, in particular the lack of theoretical consistency of some of them and their restricted phenomenology leading to the relevance of only a small subset of ETmiss signatures. This document from the LHC Dark Matter Working Group identifies an example of a next-generation DM model, called 2HDM+a, that provides the simplest theoretically consistent extension of the DM pseudoscalar simplified model. A comprehensive study of the phenomenology of the 2HDM+a model is presented, including a discussion of the rich and intricate pattern of mono-X signatures and the relevance of other DM as well as non-DM experiments. Based on our discussions, a set of recommended scans are proposed to explore the parameter space of the 2HDM+a model through LHC searches. The exclusion limits obtained from the proposed scans can be consistently compared to the constraints on the 2HDM+a model that derive from DD, ID and the DM relic density.

Physics - Accelerator Physics - Abstract
The Compact Linear Collider (CLIC) is a TeV-scale high-luminosity linear $e^+e^-$ collider under development at CERN. Following the CLIC conceptual design published in 2012, this report provides an overview of the CLIC project, its current status, and future developments. It presents the CLIC physics potential and reports on design, technology, and implementation aspects of the accelerator and the detector. CLIC is foreseen to be built and operated in stages, at centre-of-mass energies of 380 GeV, 1.5 TeV and 3 TeV, respectively. CLIC uses a two-beam acceleration scheme, in which 12 GHz accelerating structures are powered via a high-current drive beam. For the first stage, an alternative with X-band klystron powering is also considered. CLIC accelerator optimisation, technical developments and system tests have resulted in an increased energy efficiency (power around 170 MW) for the 380 GeV stage, together with a reduced cost estimate at the level of 6 billion CHF. The detector concept has been refined using improved software tools. Significant progress has been made on detector technology developments for the tracking and calorimetry systems. A wide range of CLIC physics studies has been conducted, both through full detector simulations and parametric studies, together providing a broad overview of the CLIC physics potential. Each of the three energy stages adds cornerstones of the full CLIC physics programme, such as Higgs width and couplings, top-quark properties, Higgs self-coupling, direct searches, and many precision electroweak measurements. The interpretation of the combined results gives crucial and accurate insight into new physics, largely complementary to LHC and HL-LHC. The construction of the first CLIC energy stage could start by 2026. First beams would be available by 2035, marking the beginning of a broad CLIC physics programme spanning 25-30 years., Comment: 112 pages, 59 figures; published as CERN Yellow Report Monograph Vol. 2/2018; corresponding editors: Philip N. Recently, it has been reported that peripheral blood components, including eosinophils, may be effective ICI biomarkers. This study was designed to evaluate the prognostic value of eosinophils for measuring the effects of nivolumab on recurrent/metastatic head and neck squamous cell carcinoma (R/M HNSCC).Materials and Methods: The study included 47 patients with R/M HNSCC treated with nivolumab. Eosinophil-related biomarkers, such as absolute eosinophil count (AEC), relative eosinophil count (REC), and neutrophil-to-eosinophil ratio (NER), were measured from the peripheral blood of the patients before nivolumab treatment. For each biomarker, the patients were divided into a high- and a low-value group according to their cutoff values, and these groups were compared. Results: Regarding AEC and REC, no significant improvement in the objective response rate (ORR) was observed between patients with AEC > 0.9 × 103/μL and those with AEC < 0.9 × 103/μL (p = 0.147) and between patients with REC > 2.2% and those with REC < 2.2% (p = 0.110). However, patients with NER < 32 had improved ORR compared with those with NER > 32 (P = 0.0361). Additionally, although patients with AEC > 0.9 × 103/μL, REC > 2.2%, and NER < 32 had longer overall survival (OS) than those with AEC < 0.9 × 103/μL, REC < 2.2%, and NER > 32, only patients with NER < 32 showed prolonged progression-free survival (PFS) compared with those with NER > 32 according to the Log rank test (p = 0.046, 0.027, and 0.035, respectively). Furthermore, the multivariate analysis revealed that baseline NER > 32 (p = 0.027) was an independent prognostic factor for worse OS.Conclusion: A pretreatment feature of low NER (NER < 32) may predict better clinical outcomes in patients with R/M HNSCC treated with nivolumab. Keywords: head and neck cancer, nivolumab, eosinophil, neutrophil-to-eosinophil ratio, recurrent/metastatic head and neck squamous cell carcinoma

Nuclear Theory ,Nuclear Experiment - Abstract
The quantum self-organization is introduced as one of the major underlying mechanisms of the quantum many-body systems, for instance, atomic nuclei. It is shown that atomic nuclei are not necessarily like simple rigid vases containing almost free nucleons, in contrast to the naive Fermi liquid picture. Nuclear forces are demonstrated to be rich enough to change single-particle energies for each eigenstate, so as to enhance the relevant collective mode. When the quantum self-organization occurs, single-particle energies can be self-organized (or self-optimized), being enhanced by (i) two quantum liquids, e.g., protons and neutrons, (ii) two major force components, e.g., quadrupole interaction (to drive collective mode) and monopole interaction (to control resistance). Type II shell evolution is considered to be a simple visible case involving excitations across a (sub)magic gap. Actual cases such as shape coexistence, quantum phase transition, octupole vibration/deformation, super deformation, etc. can be studied with this scope. The quantum self-organization becomes more important in heavier nuclei where the number of active orbits and the number of active nucleons are larger. With larger numbers of them, the effects of the organization can be more significant. The quantum self-organization is a general phenomenon, and is expected to be found in other quantum systems., Comment: To be published in Journal of Physics: Conference Series

Astrophysics - High Energy Astrophysical Phenomena ,Astrophysics - Astrophysics of Galaxies - Abstract
We present a multi-wavelength spectral and temporal analysis of the blazar OJ 287 during its recent activity between December 2015 -- May 2016, showing strong variability in the near-infrared (NIR) to X-ray energies with detection at $\gamma$-ray energies as well. Most of the optical flux variations exhibit strong changes in polarization angle and degree. All the inter-band time lags are consistent with simultaneous emissions. Interestingly, on days with excellent data coverage in the NIR--UV bands, the spectral energy distributions (SEDs) show signatures of bumps in the visible--UV bands, never seen before in this source. The optical bump can be explained as accretion-disk emission associated with the primary black hole of mass $\sim \rm 1.8 \times10^{10} M_{\odot}$ while the little bump feature in the optical-UV appears consistent with line emission. Further, the broadband SEDs extracted during the first flare and during a quiescent period during this span show very different $\gamma$-ray spectra compared to previously observed flare or quiescent spectra. The probable thermal bump in the visible seems to have been clearly present since May 2013, as found by examining all available NIR-optical observations, and favors the binary super-massive black hole model. The simultaneous multi-wavelength variability and relatively weak $\gamma$-ray emission that shows a shift in the SED peak is consistent with $\gamma$-ray emission originating from inverse Compton scattering of photons from the line emission that apparently contributes to the little blue bump., Comment: 24 pages, 6 figures, 2 tables, MNRAS in press

We have investigated the crystal structure of LaOBiPbS3 using neutron diffraction and synchrotron X-ray diffraction. From structural refinements, we found that the two metal sites, occupied by Bi and Pb, were differently surrounded by the sulfur atoms. Calculated bond valence sum suggested that one metal site was nearly trivalent and the other was nearly divalent. Neutron diffraction also revealed site selectivity of Bi and Pb in the LaOBiPbS3 structure. These results suggested that the crystal structure of LaOBiPbS3 can be regarded as alternate stacks of the rock-salt-type Pb-rich sulfide layers and the LaOBiS2-type Bi-rich layers. From band calculations for an ideal (LaOBiS2)(PbS) system, we found that the S bands of the PbS layer were hybridized with the Bi bands of the BiS plane at around the Fermi energy, which resulted in the electronic characteristics different from that of LaOBiS2. Stacking the rock-salt type sulfide (chalcogenide) layers and the BiS2-based layered structure could be a new strategy to exploration of new BiS2-based layered compounds, exotic two-dimensional electronic states, or novel functionality., Comment: 11 pages, 5 figures

We present recent optical photometric observations of the blazar OJ 287 taken during September 2015 -- May 2016. Our intense observations of the blazar started in November 2015 and continued until May 2016 and included detection of the large optical outburst in December 2016 that was predicted using the binary black hole model for OJ 287. For our observing campaign, we used a total of 9 ground based optical telescopes of which one is in Japan, one is in India, three are in Bulgaria, one is in Serbia, one is in Georgia, and two are in the USA. These observations were carried out in 102 nights with a total of ~ 1000 image frames in BVRI bands, though the majority were in the R band. We detected a second comparably strong flare in March 2016. In addition, we investigated multi-band flux variations, colour variations, and spectral changes in the blazar on diverse timescales as they are useful in understanding the emission mechanisms. We briefly discuss the possible physical mechanisms most likely responsible for the observed flux, colour and spectral variability., Comment: 11 pages, 6 figures, 4 tables; Accepted for publication in MNRAS

The Compact Linear Collider (CLIC) is a multi-TeV high-luminosity linear e+e- collider under development. For an optimal exploitation of its physics potential, CLIC is foreseen to be built and operated in a staged approach with three centre-of-mass energy stages ranging from a few hundred GeV up to 3 TeV. The first stage will focus on precision Standard Model physics, in particular Higgs and top-quark measurements. Subsequent stages will focus on measurements of rare Higgs processes, as well as searches for new physics processes and precision measurements of new states, e.g. states previously discovered at LHC or at CLIC itself. In the 2012 CLIC Conceptual Design Report, a fully optimised 3 TeV collider was presented, while the proposed lower energy stages were not studied to the same level of detail. This report presents an updated baseline staging scenario for CLIC. The scenario is the result of a comprehensive study addressing the performance, cost and power of the CLIC accelerator complex as a function of centre-of-mass energy and it targets optimal physics output based on the current physics landscape. The optimised staging scenario foresees three main centre-of-mass energy stages at 380 GeV, 1.5 TeV and 3 TeV for a full CLIC programme spanning 22 years. For the first stage, an alternative to the CLIC drive beam scheme is presented in which the main linac power is produced using X-band klystrons., Comment: 57 pages, 27 figures, 12 tables, published as CERN Yellow Report. We present the nuclear matrix element for the neutrinoless double-beta decay of $^{48}$Ca based on large-scale shell-model calculations including two harmonic oscillator shells ($sd$ and $pf$ shells). The excitation spectra of $^{48}$Ca and $^{48}$Ti, and the two-neutrino double-beta decay of $^{48}$Ca are reproduced in good agreement to experiment. We find that the neutrinoless double-beta decay nuclear matrix element is enhanced by about 30\% compared to $pf$-shell calculations. This reduces the decay lifetime by almost a factor of two. The matrix-element increase is mostly due to pairing correlations associated with cross-shell $sd$-$pf$ excitations. We also investigate possible implications for heavier neutrinoless double-beta decay candidates., Comment: Phys. Rev. Lett., accepted

Nuclear Theory ,Nuclear Experiment - Abstract
Large-scale shell model calculations including two major shells are carried out, and the ingredients of nuclear matrix element for two-neutrino double beta decay are investigated. Based on the comparison between the shell model calculations accounting only for one major shell ($pf$-shell) and those for two major shells ($sdpf$-shell), the effect due to the excitation across the two major shells is quantitatively evaluated., Comment: To appear in J. This work is on the Physics of the B Factories. Part A of this book contains a brief description of the SLAC and KEK B Factories as well as their detectors, BaBar and Belle, and data taking related issues. Part B discusses tools and methods used by the experiments in order to obtain results. The results themselves can be found in Part C. Please note that version 3 on the archive is the auxiliary version of the Physics of the B Factories book. This uses the notation alpha, beta, gamma for the angles of the Unitarity Triangle. The nominal version uses the notation phi_1, phi_2 and phi_3. Please cite this work as Eur. Phys. J. C74 (2014) 3026., Comment: 928 pages, version 3 (arXiv:1406.6311v3) corresponds to the alpha, beta, gamma version of the book, the other versions use the phi1, phi2, phi3 notation

Although patients with BKPyVAN usually received a reduction of immunosuppressive agents, the majority of these patients undergo the loss of the graft kidney without any effective treatment afterward. Therefore, development of more effective therapy for BKPyVAN is eagerly expected.Patients and Methods: Among patients who underwent a kidney transplantation between January 2016 and April 2019 at our hospital, there were five cases of BKPyVAN. After the initial diagnosis, all patients discontinued administration of mycophenolate mofetil (MMF), which was not enough to diminish decoy cells in urine cytology test. Therefore, all patients received additional intravenous immunoglobulin (IVIG) (100 mg/kg/day) therapy for five days and were evaluated for the therapeutic effect of IVIG with immunohistochemical examination using re-biopsy samples of the graft kidney. Results: After IVIG therapy, 2 cases showed negative decoy cells in urine and 3 cases showed a drastic decrease of plasma BK virus load. Importantly, simian virus (SV) 40 large T antigens diminished after IVIG administration in all cases, which degraded polyomavirus nephropathy classification. Conclusion: Although it is difficult to treat BKPyVAN after kidney transplant, IVIG therapy was considered to a promising treatment to improve severity of BKPyVAN especially in cases that dose reduction of immunosuppressive agents was ineffective. Keywords: BK virus, BK virus nephropathy, intravenous immunoglobulin; IVIG, kidney transplantation, polyomavirus nephropathy classification

- Abstract
Background: Current management practices and outcomes in weaning from invasive mechanical ventilation are poorly understood. We aimed to describe the epidemiology, management, timings, risk for failure, and outcomes of weaning in patients requiring at least 2 days of invasive mechanical ventilation. Methods: WEAN SAFE was an international, multicentre, prospective, observational cohort study done in 481 intensive care units in 50 countries. Eligible participants were older than 16 years, admitted to a participating intensive care unit, and receiving mechanical ventilation for 2 calendar days or longer. We defined weaning initiation as the first attempt to separate a patient from the ventilator, successful weaning as no reintubation or death within 7 days of extubation, and weaning eligibility criteria based on positive end-expiratory pressure, fractional concentration of oxygen in inspired air, and vasopressors. The primary outcome was the proportion of patients successfully weaned at 90 days. Key secondary outcomes included weaning duration, timing of weaning events, factors associated with weaning delay and weaning failure, and hospital outcomes. This study is registered with ClinicalTrials.gov, NCT03255109. Findings: Between Oct 4, 2017, and June 25, 2018, 10 232 patients were screened for eligibility, of whom 5869 were enrolled. 4523 (77·1%) patients underwent at least one separation attempt and 3817 (65·0%) patients were successfully weaned from ventilation at day 90. 237 (4·0%) patients were transferred before any separation attempt, 153 (2·6%) were transferred after at least one separation attempt and not successfully weaned, and 1662 (28·3%) died while invasively ventilated. The median time from fulfilling weaning eligibility criteria to first separation attempt was 1 day (IQR 0–4), and 1013 (22·4%) patients had a delay in initiating first separation of 5 or more days. Of the 4523 (77·1%) patients with separation attempts, 2927 (64·7%) had a short wean (≤1 d

Nuclear Theory - Abstract
We report no-core solutions for properties of light nuclei with three different approaches in order to assess the accuracy and convergence rates of each method. Full configuration interaction (FCI), Monte Carlo shell model (MCSM) and no core full configuration (NCFC) approaches are solved separately for the ground state energy and other properties of seven light nuclei using the realistic JISP16 nucleon-nucleon interaction. The results are consistent among the different approaches. The methods differ significantly in how the required computational resources scale with increasing particle number for a given accuracy., Comment: 19 pages, 14 figures, 6 tables

We propose the Hyper-Kamiokande (Hyper-K) detector as a next generation underground water Cherenkov detector. It will serve as a far detector of a long baseline neutrino oscillation experiment envisioned for the upgraded J-PARC, and as a detector capable of observing -- far beyond the sensitivity of the Super-Kamiokande (Super-K) detector -- proton decays, atmospheric neutrinos, and neutrinos from astronomical origins. The baseline design of Hyper-K is based on the highly successful Super-K, taking full advantage of a well-proven technology. (to be continued), Comment: 108 pages, 69 figures

Nuclear Theory - Abstract
The Monte Carlo shell model is firstly applied to the calculation of the no-core shell model in light nuclei. The results are compared with those of the full configuration interaction. The agreements between them are within a few % at most., Comment: 4 pages, 1 figure, 1 table, Proceedings of the International Symposium on New Faces of Atomic Nuclei, Okinawa, Japan, Nov. 15-17, 2010
