Background: Anxiety disorders are mental disorders that cause somatic symptoms for which patients may seek care from general medicine departments. We focused on anxiety disorders in middle-aged patients and examined the effect of a psychotherapeutic intervention. Materials and Methods: The participants were 14 middle-aged patients diagnosed with an anxiety disorder. Patients received pretreatment assessments and were randomly assigned to a pharmacotherapy group (n = 8) or a pharmacotherapy and psychotherapy group (n = 6). The duration of the study was three months. Pre-and post-treatment, the Medical Outcomes Study 36-Item Short-Form Health Survey (SF-36), State-Trait Anxiety Inventory (STAI), and a visual analog scale (VAS) were administered. In the pharmacotherapy and psychotherapy group, salivary cortisol was collected pre- and post-psychotherapy at the first and final psychotherapy sessions. Result: Four patients in the pharmacotherapy group withdrew from the study. There were no significant differences in the total scores of the SF-36 or STAI between groups. Improvement was seen in the pharmacotherapy and psychotherapy group pre- and post- treatment. SF-36 subscales of bodily pain (p = 0.02) and mental health (p = 0.04) were significantly higher than posttreatment. The state anxiety score on the STAI improved post-treatment (p = 0.03). On the VAS, the pharmacotherapy and psychotherapy group’s symptoms were significantly improved (p = 0.02). Conclusion: This suggests that psychotherapy for middle-aged individuals contributes to the improvement of anxiety states and HRQoL in general medicine departments. A 26-year-old woman was admitted to our hospital with fever, dry cough and elevated serum CRP. Chest CT scans revealed mediastinal and bihilar lymphadenopathy, and sarcoidosis was clinically diagnosed based on additional findings. The walls of the aorta and other arteries were thickened and HLA typing was positive for HLA-B52, suggesting Takayasu's arteritis. Corticosteroid clearly improved the symptoms and the patient has remained free of recurrence since therapy was discontinued. Background: A novel index, the cardio-ankle vascular index (CAVI), which reflects the stiffness of the aorta, femoral artery, and tibial artery, was recently developed by measuring brachial-ankle pulse wave velocity and blood pressure. Methods and Results: In the present study 1,014 Japanese adults from the general population were screened to clarify the correlation between CAVI and other existing markers related to atherosclerosis, including carotid intima-media thickness (CIMT) and homocysteine (HCY). CAVI was strongly associated with age in both men and women. After adjustment for age and sex, CAVI was correlated with systolic and diastolic blood pressures. In addition, CAVI was significantly correlated with total cholesterol hemoglobin A1c and total HCY, as well as CIMT. Conclusion: CAVI is an appropriate screening tool for atherosclerosis, but further studies are needed to establish a convenient and effective screening system using it.

An abnormal shadow was detected on chest radiograph in a 28-year-old male on a routine medical check. The chest radiograph showed a solitary mass-like opacity in the right upper lung field. The patient attended our hospital for further examination on May 22, 2002. High-resolution computed tomography showed an aggregate of micronodular opacities consistent with the "pseudo-alveolar" pattern described in recent reports. The final diagnosis was pulmonary sarcoidosis as confirmed by the presence of epithelioid granulomas in specimens from transbronchial lung biopsy. Pulmonary sarcoidosis with a pseudo-alveolar pattern is unusual, particularly in the absence of bilateral hilar lymphadenopathy. Pneumothorax and pneumomediastinum sometimes occur in patients with idiopathic interstitial pneumonias (IIPs). Although steroid treatment is commonly used for IIPs, some reports indicated that this treatment could cause pneumothorax (PT) or pneumomediastinum (PM) in patients with IIPs. The aim of the present study was to evaluate the clinical features of PT and PM in patients with IIPs, and to assess their association with steroid treatment. We enrolled 77 patients with IIPs and divided them into four groups: (1) idiopathic pulmonary fibrosis (IPF) patients without PT and PM [IPF PT/PM(-) group, n = 38]; (2) IPF patients with PT and/or PM [IPF PT/PM(+) group, n = 6]; (3) non-IPF patients without these complications [non-IPF PT/PM(-) group, n = 29]; and (4) non-IPF patients with PT and/or PM [non-IPF PT/PM(+) group, n = 4]. We reviewed and compared the clinical, radiological and laboratory findings among the four groups. We also focused on the above 10 IIPs patients with PT and/or PM to describe the details of their clinical features. Resultingly, IPF PT/PM(+) and non-IPF PT/PM(+) groups showed low percentage of vital capacity (%VC) compared with IPF PT/PM(-) and non-IPF PT/PM(-) groups, respectively. Six of the 10 cases with PT/PM were treated with steroids about 2 weeks before the development of PT and/or PM. We concluded that PT and PM could arise in patients with IIPs, especially in cases with severe restrictive ventilatory impairment. - Abstract
南極周回長時間宇宙線観測計画「BESS-Polar」において,電力源の確保は最も重要な開発課題の一つである.測定器の消費電力600Wを20日間のフライト期間中連続して供給することが求めらるこのシステムは,同時に重量制限300kgなど気球実験特有の制約も満たさなければならない.これまでのBESS実験で用いられてきた一次電池に代わり太陽電池を使った新しい電力システムの開発が進められており,そのシステムの検証およびエアロダイナミクスの測定を目的として気球実験が三陸にて実施された.本稿では,太陽電池システムの概要を紹介する共に,三陸実験について報告する., Establishment of a reliable power supply is one of our new challenges to acheive "BESS-Polar experiment", a long-duration balloon flight in Antarctica. The power system is required to enable the detector system operation with a capacity of 600W during a flight period of 20 days, as well as to meet balloon experimental constraints including weight restriction of 300kg. solar battery is newly adapted as a power source, instead of primary battery utilized in the present BESS system. In order to verify performances of the system being developed, a technical balloon flight at Sanriku Balloon Centre was carried out. BESS実験は1993,1994,1995,1997,1998,1999,2000年の計7回飛翔実験に成功し,宇宙線反陽子スペクトラムの測定を継続的に行ってきた.合計2000例を越える反陽子事象が0.18-4.20GeVの運動エネルギー領域で明確に識別されている.本稿では,これまでのBESSによる反陽子流束測定の成果をまとめる., The energy spectrum of cosmic-ray antiprotons has been measured by BESS successively in 1993, 1995, 1997, 1998, 1999 and 2000. In total more than 2 x10^3 antiprotons were clearly identified in an energy range of 0.18 to 4.20 GeV. The energy spectrum of cosmic-ray antiprotons has been measured in the range 0.18 to 3.56 GeV, based on 458 antiprotons collected by BESS in recent solar-minimum period. We have detected for the first time a distinctive peak at 2 GeV of antiprotons originating from cosmic-ray interactions with the interstellar gas. The peak spectrum is reproduced by theoretical calculations, implying that the propagation models are basically correct and that different cosmic-ray species undergo a universal propagation. Future BESS flights toward the solar maximum will help us to study the solar modulation and the propagation in detail and to search for primary antiproton components., REVTeX, 4 pages including 4 eps figures

Jejunoileal fistula is an extremely rare complication in patients with intestinal lymphoma. Here, we report a Japanese male patient with enteropathy-type intestinal T-cell lymphoma presenting abdominal pain and weight loss. A jejunoileal fistula was discovered during colonoscopy and pathological diagnosis was performed preoperatively by forceps biopsy. After elective surgery for partial resections of jejunum, ileum, and sigmoid colon, eight cycles of cyclophosphamide, doxorubicin, vincristine and prednisolone chemotherapy led complete remission of the disease., Acta medica Nagasakiensia, 54(1), pp.25-27 ; 2009

39. Effects of psychotherapy for middle-aged individuals with anxiety disorders in a general medicine practice

- Abstract
Background: Anxiety disorders are mental disorders that cause somatic symptoms for which patients may seek care from general medicine departments. We focused on anxiety disorders in middle-aged patients and examined the effect of a psychotherapeutic intervention. Materials and Methods: The participants were 14 middle-aged patients diagnosed with an anxiety disorder. Patients received pretreatment assessments and were randomly assigned to a pharmacotherapy group (n = 8) or a pharmacotherapy and psychotherapy group (n = 6). The duration of the study was three months. Pre-and post-treatment, the Medical Outcomes Study 36-Item Short-Form Health Survey (SF-36), State-Trait Anxiety Inventory (STAI), and a visual analog scale (VAS) were administered. In the pharmacotherapy and psychotherapy group, salivary cortisol was collected pre- and post-psychotherapy at the first and final psychotherapy sessions. Result: Four patients in the pharmacotherapy group withdrew from the study. There were no significant differences in the total scores of the SF-36 or STAI between groups. Improvement was seen in the pharmacotherapy and psychotherapy group pre- and post- treatment. SF-36 subscales of bodily pain (p = 0.02) and mental health (p = 0.04) were significantly higher than posttreatment. The state anxiety score on the STAI improved post-treatment (p = 0.03). On the VAS, the pharmacotherapy and psychotherapy group’s symptoms were significantly improved (p = 0.02). Conclusion: This suggests that psychotherapy for middle-aged individuals contributes to the improvement of anxiety states and HRQoL in general medicine departments. 41. Interleukin 5 Levels in Bronchoalveolar Lavage Fluid from Patients with Interstitial Lung Disease

Bronchiolitis obliterans organizing pneumonia (BOOP) is clinically and histologically difficult to differentiate from chronic eosinophilic pneumonia (CEP). The common histological feature of these diseases is an infiltration of mononuclear cells into the small airway, interstitial tissue and alveoli. Activation of T cells and secretion of lymphokines play a crucial role in the regulation and coordination of immune responses and inflammatory processes. In this study, we demonstrated the distribution of lymphocyte subpopulations and their stage of activation and T cellderived cytokine, IL-5 level in bronchoalveolar lavage (BAL) fluid obtained from patients with BOOP and CEP by comparison with sarcoidosis and diffuse panbronchiolitis (DPB). Flow cytometric analysis of T cell activation markers revealed that BOOP and sarcoidosis are characterized by increased absolute numbers of HLA-DR-bearing T cell subsets, while the relative numbers were increased in CEP and DPB. IL-5 in BAL fluids from BOOP and CEP was significantly high levels, and CEP patients with the high level of IL-5 showed the marked elevation of BAL eosinophils. Furthermor, it is of interest that the absolute numbers of HLA-DR T cells correlated with IL-5 levels in BAL fluid. These results suggest that activated T cells and secretion of IL-5 may be important factors in the pathogenetic processes of BOOP and CEP., Acta medica Nagasakiensia. 1994, 39(4), p.95-100

- Abstract
南極周回気球による宇宙線反粒子探査を目的に「BESS-Polar実験」が計画されている.中でも,低エネルギー反陽子の精密測定は主要な目標であり,宇宙線の伝播や太陽活動から受ける影響の正確な理解に加え,一次起源反陽子の探索を目指している.また,反ヘリウム等の反粒子の探索により,現在の宇宙における物質/反物質の非対称性に関する直接的な知見を得ることができる.本稿では,BESS-Polar計画の概要を紹介すると共に,開発の状況について報告する., A long-duration balloon flight in Antarctica, BESS-Polar experiment, is being prepared. It aims at extremely sensitive measurement of low energy antiprotons to search for any novel primary origin and to study the cosmic-ray propagation and the solar modulation. The search for cosmic antimatter such as anti-helium is a fundamental objective to study baryon asymmetry / symmetry in the Universe. This report describes the progress and future prospect of BESS-Polar experiment.

- Abstract
BESS実験は1993,1994,1995,1997,1998,1999,2000年の計7回飛翔実験に成功し,宇宙線反陽子スペクトラムの測定を継続的に行ってきた.合計2000例を越える反陽子事象が0.18-4.20GeVの運動エネルギー領域で明確に識別されている.本稿では,これまでのBESSによる反陽子流束測定の成果をまとめる., The energy spectrum of cosmic-ray antiprotons has been measured by BESS successively in 1993, 1995, 1997, 1998, 1999 and 2000. In total more than 2 x10^3 antiprotons were clearly identified in an energy range of 0.18 to 4.20 GeV. From these successive measurements of the antiproton spectrum at various solar activity, the effect of the solar modulation and the origin of cosmic-ray antiprotons are discussed.
