We present the Massive and Distant Clusters of WISE Survey (MaDCoWS), a search for galaxy clusters at 0.7 ≲ z ≲ 1.5 based upon data from the Wide-field Infrared Survey Explorer (WISE) mission. MaDCoWS is the first cluster survey capable of discovering massive clusters at these redshifts over the full extragalactic sky. The search is divided into two regions - the region of the extragalactic sky covered by Pan-STARRS (δ > -30°) and the remainder of the southern extragalactic sky at δ < -30° for which shallower optical data from the SuperCOSMOS Sky Survey is available. In this paper, we describe the search algorithm, characterize the sample, and present the first MaDCoWS data release - catalogs of the 2433 highest amplitude detections in the WISE-Pan-STARRS region and the 250 highest amplitude detections in the WISE-SuperCOSMOS region. A total of 1723 of the detections from the WISE-Pan-STARRS sample have also been observed with the Spitzer Space Telescope, providing photometric redshifts and richnesses, and an additional 64 detections within the WISE-SuperCOSMOS region also have photometric redshifts and richnesses. Spectroscopic redshifts for 38 MaDCoWS clusters with IRAC photometry demonstrate that the photometric redshifts have an uncertainty of σ z /(1 + z) ≃ 0.036. Combining the richness measurements with Sunyaev-Zel'dovich observations of MaDCoWS clusters, we also present a preliminary mass-richness relation that can be used to infer the approximate mass distribution of the full sample. The estimated median mass for the WISE-Pan-STARRS catalog is , with the Sunyaev-Zel'dovich data confirming that we detect clusters with masses up to M 500 ∼ 5 ×10 14 M (M 200 ∼ 10 15 M ).

This experiment was carried out at College of Veterinary Medicine, University of Sulaimani. Microscopically, no adverse histopathological changes were seen in the kidneys, liver, gills and intestines of Cyprinus carpio for T2(replacing fishmeal with 5% lentil) and T3 (replacing fishmeal with 10% lentil) in comparison with the control (T1). However, various levels of adverse histopathological changes were seen in the T4(replacing fish meal with 15% lentil) and T5 (replacing fishmeal with 20% lentil). In the kidneys, dilation of the Bowman’s spaces was evidently associated with a decrease in the mesangial cellularity of glomerular tuft. In the liver, congestion of the central vein was apparent together with centrilobular infiltration of inflammatory cells. In the gills, hyperplasia of primary and secondary lamellae was evident. In the intestine, vacuolar degeneration of the lining epithelial cells was apparent together with extravasation of RBCs associated with marked infiltration of inflammatory cells in the lamina propria. In conclusion, the partial replacement of the fishmeal in diets of C. carpio by crude lentil seeds was safe at the 5% and 10% levels as indicated by various levels of adverse histopathological effects on the kidneys, liver, gills, and intestine. This article sets forth the proposition that liquidity risk may be optimized in an attempt to forestall or minimize the impact of a liquidity crisis. For a generic (but typical) endowment asset allocation, the authors find that liquidity levels between 6% and 14% are optimal, all other things equal, because 95% of the time, an allocation in this range would obviate situations in which a portfolio's pay out rate exceeds its liquidity pool. The framework also provides insights for tail-risk events involving a particularly severe liquidity crisis. For a generic endowment portfolio, the analysis indicates that in order to reduce the severity of a liquidity crisis to zero (i.e., eliminate risk completely), the allocation to fixed income would have to be around 35% (close to seven times the pay out rate of 5%). Such an allocation would entail a very significant opportunity cost in terms of forgone returns based solely oil a desire to mitigate extreme liquidity events (the proverbial "100-year flood"). In the authors' view, reducing the likelihood of a liquidity crisis to below 5%, may be undesirable for all but the most risk-averse and least return-sensitive endowments. [ABSTRACT FROM AUTHOR]

Orbital hemangiomas are benign vascular tumors commonly affecting infants and young children, often manifesting with proptosis, eyelid swelling, or discoloration. Propranolol has emerged as the primary therapy due to its efficacy in promoting regression and minimizing complications. Here, we present a case of a previously healthy six-month-old male infant with progressive right eyelid swelling and discoloration. Magnetic resonance imaging confirmed a large orbital hemangioma. The patient was referred to an ophthalmology center where treatment with propranolol resulted in substantial improvement. Early recognition and initiation of propranolol therapy are crucial in managing orbital hemangiomas in pediatric patients. Background: Long-term outcome of patients with locally advanced gastric cancer (LAGC) who achieved a pathological complete response (pCR) was scarcely discussed, and never had the factors affecting the prognosis of pCR patients been investigated., Methods: We retrospectively reviewed all patients who achieved a pCR to neoadjuvant chemotherapy (NAC) in Jinling Hospital. The 3- and 5-year overall survival (OS) and progression-free survival (PFS) were calculated with the Kaplan-Meier method. Meanwhile, univariate and multivariate COX regression analysis was applied to identify prognostic factors affecting patients' survival., Results: A total of 37 consecutive LAGC patients with pCR were included. The 3- and 5-year OS rates were 88.8% and 78.6%, and the 3- and 5-year PFS rates were 86.5% and 75.8%. In the multivariate COX model, NAC duration of more than 3 cycles (HR 0.11 [0.02-0.62], P = 0.013) and poorly differentiated tumor at diagnosis (HR 0.17 [0.03-0.95], P = 0.043) were detected as protective factors for patients OS. Whereas for PFS, NAC duration (HR 0.12 [0.02-0.67], P = 0.015) was the only protective factor confirmed, with tumor differentiation at diagnosis exhibiting marginal significance (HR 0.21 [0.04-1.09], P = 0.063)., Conclusions: Patients with LAGC who achieved a pCR displayed favorable long-term survival outcome, especially those with adequate cycles (≥ 3) of NAC. Background: The prognosis of unresectable gastric cancer is poor, while the efficacy of anti-PD antibodies has not been evaluated., Methods: Patients with unresectable gastric cancer who received intra-arterial chemotherapy (IAC) with sequential anti-PD-1 antibody as induction therapy in Jinling Hospital were retrospectively analyzed. The primary outcome is R0 resection rate. The secondary outcomes include safety, conversion surgery rate, overall survival (OS) and progression free survival (PFS) after postoperative IAC and anti-PD-1 treatments. Meanwhile, Tumor immunity in the microenvironment (TIME) before and after IAC was comprehensively dissected with multiplex immunofluorescence in order to detect possible mechanisms favoring anti-PD-1 treatment response., Results: Between May 2019 and October 2020, 36 patients received at least one cycle of IAC with sequential anti-PD-1 antibody in our institution. The objective response was achieved in 28 patients (77.8%). Thirty patients (83.3%) successfully underwent conversion surgery, among which R0 resection was managed in 25/30 patients, and 23.3% (7/30) was assessed as pathological complete remission. During the median follow-up period of 19.7 months, patients who underwent R0 resection displayed superior OS (HR 0.14 [95% CI 0.04-0.50], P < 0.0001) and PFS (HR 0.11 [0.03-0.44], P < 0.0001) than those who did not. Grade 3 adverse events (AEs) were only encountered in 19.4% patients, no grade 4 AEs observed. In TIME analysis, the number of tertiary lymphoid structures (TLSs) (P = 0.004) were greatly induced by IAC, as well as CD8 + T cells (P = 0.011) and PD-1 + cells (P = 0.025). Meanwhile, Tumor associated macrophages shifted towards anti-tumor M1-like subtypes, with CD68 + CD163 + M2-like subpopulation significantly decreased (P = 0.04)., Conclusion: Preoperative IAC with sequential anti-PD-1 antibody exhibited promising clinical benefit for unresectable gastric cancer with remarkable conversion rate and R0 resection rate, and also prolonged survival as postoperative regimen. The aim of this study was to investigate the effects of oral branched-chain amino acids (BCAAs) intake on muscular (creatine kinase and myoglobin) and central (serotonin) fatigue during an incremental exercise protocol and to determine the time to exhaustion. Sixteen male long-distance runners (25.7 ± 2.0 yrs) performed two trials, 14 days apart. Using a double-blind, placebo-controlled, randomised crossover design, participants ingested either 20 g of BCAAs (BCAA trial) or a placebo 1 hour prior to performing an incremental exercise session on a treadmill. The starting speed was 8 km/h and this was increased by 1 km/h every 5 minutes until volitional exhaustion. Blood analysis indicated that plasma levels of serotonin were lower in the BCAA trial (259.3 ± 13.5 ng/ml) than the placebo trial (289.1 ± 14.5 ng/ml) (p < 0.05). There was a similar pattern of results for free fatty acid (p < 0.05). The creatine kinase level was higher in the BCAA trial (346.1 ± 33.7 U/L) than the placebo trial (307.3 ± 30.2 U/L). No significant difference between trials was observed regarding the level of myoglobin (p = 0.139). Time to exhaustion was longer in the BCAA trial (50.4 ± 2.3 min) than the placebo trial (46.6 ± 3.2 min). In conclusion, oral intake of 20 g of BCAAs 1 hour prior to an incremental treadmill exercise session increased time to exhaustion, probably due to the reduction in serotonin concentration. Background: Intensive community treatment to reduce dependency on adolescent psychiatric inpatient care is recommended in guidelines but has not been assessed in a randomised controlled trial in the UK. We designed a supported discharge service (SDS) provided by an intensive community treatment team and compared outcomes with usual care., Methods: Eligible patients for this randomised controlled trial were younger than 18 years and had been admitted for psychiatric inpatient care in the South London and Maudsley NHS Foundation Trust. Patients were assigned 1:1 to either the SDS or to usual care by use of a computer-generated pseudorandom code with random permuted blocks of varying sizes. The primary outcome was number of inpatient bed-days, change in Strengths and Difficulties Questionnaire (SDQ) scores, and change in Children's Global Assessment Scale (CGAS) scores at 6 months, assessed by intention to treat. Cost-effectiveness was explored with acceptability curves based on CGAS scores and quality-adjusted life-years (QALYs) calculated from the three-level EuroQol measure of health-related quality of life (EQ-5D-3L), taking a health and social care perspective. This study is registered with the ISRCTN Registry, number ISRCTN82129964., Findings: Hospital use at 6 months was significantly lower in the SDS group than in the usual care group (unadjusted median 34 IQR 17-63 vs 50 days, 19-125, p=0·04). The ratio of mean total inpatient days for usual care to SDS was 1·67 (95% CI 1·02-2·81, p=0·04), which decreased to 1·65 (0·99-2·77, p=0·057) when adjusted for differences in hospital use before randomisation. Scores for SDQ and CGAS did not differ between groups. The cost-effectiveness acceptability curve based on QALYs showed that the probability of SDS being cost-effective compared with usual care was around 60% with a willingness-to-pay threshold of £20 000-30 000 per QALY, and that based on CGAS showed at least 58% probability of SDS being cost-effective compared with usual care irrespective of willingness to pay. We recorded no adverse events attributable to SDS or usual care., Interpretation: SDS provided by an intensive community treatment team reduced bed usage at 6 months' follow-up but had no effect on functional status and symptoms of mental health disorders compared with usual care. Cyclin D1 immunostaining of non-neoplastic cells has been a source of diagnostic confusion especially in lymphoproliferative lesions. This study has reviewed these in two hundred and thirty-one haematopathological samples stained for cyclin D1. Most cases were formalin-fixed except for a few bone marrow trephines, which were B-5 fixed, and EDTA decalcified. Overall, 94% (216/231) of cases showed one or more types of non-neoplastic cells expressing Cyclin D1 of variable intensity. Endothelial cells and histiocytes were the most commonly identified Cyclin D1 positive cells being positive in 92% (214/231) of cases. Other normal cell types identified included fat cells, stromal fibroblasts, glial cells, spermatocytes, smooth muscle cells, osteoblasts and where present epithelial cells. All 250 children presenting with diarrhoea at 2 teaching hospitals in Mosul, Iraq over a 9-month period were studied for the presence of Yersinia spp. in stools by cold-enrichment culture at 4 degrees C for 21 days. Pathogenicity of the isolated Yersinia was determined. Antibodies to Y. enterocolitica were raised for rapid Yersinia detection in the stool. Yersinia spp. were isolated from the stools of only 4 patients; 3 isolates were identified as Y. enterocolitica and 1 was Y. pseudotuberculosis. The blood culture was also positive for Y. enterocolitica in 1 case. The antibiogram test for the isolated Yersinia was determined. Cross-reaction between Y. pseudotuberculosis and Salmonella typhi or S. paratyphi B, and between Y. enterocolitica and Brucella was detected serologically.
