9. Portal vein thrombosis in patients with liver cirrhosis: Risk factors and clinical presentation

Abstract

Background:Portal vein thrombosis (PVT) is a common complication of liver cirrhosis. PVT impact on disease progression is not clarified as yet. Anticoagulation therapy is considered effective in this setting, but is associated with potentially bleeding episodes. Aim : to assess the risk factors and clinical impact of non-neoplastic PVT complicating cirrhosis, as well as the treatment profile and its efficacy in clinical practice.Methods:A retrospective monocentric study over a period of 19 years including all patients diagnosed with cirrhosis and non-neoplastic PVT was conducted.Results:A total of 49 patients were enrolled in the present study.The mean age was 60.86±11.61 years old. Chronic viral hepatitis was the most frequent cause of cirrhosis (63.2%). Most of our cases had advanced liver disease (89.9% Child class B/C) with a mean MELD score of 19.27. The risk factors for thrombophilia, inherited or acquired, were: a deficiency in coagulation inhibitors, either isolated or combined (protein S, protein C and antithrombin III) in 19 patients, a heterozygous Factor V Leiden mutation in 2 patients, a heterozygous MTHFR mutation in one patient, an antiphospholipid antibodies syndrome in 2 patients, an essential thrombocythemia in one patient. Anticoagulant therapy was indicated in half of the cases. Multivariate analyses demonstrated that thrombus extension was the only independent predictive factor of portal vein recanalization (p=0.009). During follow-up, progression was observed in 8% of treated patients with anticoagulants versus 12.5% of untreated patients (p=0.12). Our study has shown that anticoagulant treatment is not associated with elevated risk of bleeding or developing other complications. The mean survival was higher in patients treated successfully (38.31 months Vs 23.41 months, p=0.204). Conclusions:Our outcomes confirm that anticoagulation therapy in cirrhotic patients with non-neoplastic PVT is not associated with increased risk of liver disease decompensation, including bleeding.

Abstract

There is a high interest on gut health in poultry with special focus on consequences of the intestinal diseases, such as coccidiosis and C. perfringens-induced necrotic enteritis (NE). We developed a custom gene expression panel, which could provide a snapshot of gene expression variation under challenging conditions. Ileum gene expression studies were performed through high throughput reverse transcription quantitative real-time polymerase chain reaction. A deep review on the bibliography was done and genes related to intestinal health were selected for barrier function, immune response, oxidation, digestive hormones, nutrient transport, and metabolism. The panel was firstly tested by using a nutritional/Clostridium perfringens model of intestinal barrier failure (induced using commercial reused litter and wheat-based diets without exogenous supplementation of enzymes) and the consistency of results was evaluated by another experiment under a coccidiosis challenge (orally gavaged with a commercial coccidiosis vaccine, 90× vaccine dose). Growth traits and intestinal morphological analysis were performed to check the gut barrier failure occurrence. Results of ileum gene expression showed a higher expression in genes involved in barrier function and nutrient transport in chickens raised in healthy conditions, while genes involved in immune response presented higher expression in C.perfringens-challenged birds. On the other hand, the Eimeria challenge also altered the expression of genes related to barrier function and metabolism, and increased the expression of genes related to immune response and oxidative stress. The panel developed in the current study gives us an overview of genes and pathways involved in broiler response to pathogen challenge. It also allows us to deep into the study of differences in gene expression pattern and magnitude of responses under either a coccidial vaccine or a NE.

Abstract

MDPI accés obert UTP, An experiment was performed to evaluate the effect of four different microencapsulated blends of organic acids (OA) and nature-identical aromatic compounds (AC) on growth performance and gut health of broilers challenged with a recycled NE litter. A total of 600 one-day-old male Ross 308 broilers were randomly assigned to five treatments consisting of a basal diet (as negative control) supplemented with each of the tested microencapsulated blends: OA1 (malic and fumaric acid) + AC; 2.5 g/kg; OA2 (calcium butyrate+fumaric acid) + AC; 1.7 g/kg; MCFA (capric-caprylic; caproic and lauric acid) + AC; 2 g/kg; and MCFA + OA3(calcium butyrate + fumaric and citric acid) + AC; 1.5 g/kg. The AC used was the same for all treatments; including cinnamaldehyde, carvacrol, and thymol (8:1:1), as major compounds. Three tested blends enhanced growth performance by improving intestinal histomorphology (p < 0.001). The tested blends enhanced the abundance of some beneficial families such as Ruminococcaceae and Lachnospiraceae; while reducing that of harmful ones such as Enterobacteriaceae and Helicobacteraceae. A further dose-response experiment showed that 0.5 g/kg of the blend 2 and 2 g/kg of the blend 4 improved growth performance and intestinal histomorphology of chickens on d 42 and decreased fecal Enterobacteriaceae and C. perfringens counts. Similar effects to the previous experiment were observed for cecum microbiota.

28. Maladie cœliaque et hypertension portale intra-hépatique non cirrhotique : à propos de 3 cas

Abstract

Introduction La maladie cœliaque est une affection systemique resultant d’une reaction immunitaire inappropriee au gluten chez des individus genetiquement predisposes. L’atteinte hepatique est frequente au cours de la maladie cœliaque et elle est dominee par l’hypertransaminasemie regressive sous regime sans gluten ainsi que par l’association a des authentiques hepatopathies auto-immunes. L’association a une hypertension portale (HTP) intra-hepatique non cirrhotique est beaucoup plus rare. Nous en rapportons trois observations. Materiels et methodes Nous avons collige tous les cas de maladie cœliaque associee a une hypertension portale intra-hepatique non cirrhotique recenses au service de gastro-enterologie de l’hopital militaire de Tunis entre 2005 et 2015. Le diagnostic de maladie cœliaque reposait sur la presence d’une atrophie villositaire associee a la positivite des anticorps anti-transglutaminase et/ou anti-endomysium. Resultats Trois patients ont ete colliges dont 2 femmes et 1 homme. L’âge moyen etait de 31 ans (24–38 ans). La decouverte d’une splenomegalie a revele l’hypertension portale dans les 3 cas. Des signes d’hypersplenisme etaient presents chez les 3 patients (pancytopenie dans 2 cas et thrombopenie isolee dans un cas). L’endoscopie digestive haute trouvait des varices œsophagiennes dans 2 cas. L’activite des enzymes hepatiques etait normale et la fonction hepatique etait conservee dans les 3 cas. L’imagerie montrait un foie a contours lisses et avait exclu une obstruction des veines hepatiques ou de la veine porte. Les causes communes de cirrhose en particulier virales et auto-immunes ont ete exclues chez les 3 malades. L’histologie hepatique a montre l’absence de fibrose significative ainsi que la presence de remaniements microcirculatoires compatibles avec une veinopathie portale obliterante chez les 3 malades. La serologie VIH, l’enquete medicamenteuse et la recherche d’une cause constitutionnelle (mutation facteur II, facteur V Leiden, deficit en proteines C, S et anti-thrombine III) ou acquise (anticorps anti-phospholipides) de thrombophilie etaient negatives chez les trois malades. La mutation V617F du JAK2 recherchee chez un seul malade etait negative. L’electrophorese des protides n’a pas montre d’hypogammaglobulinemie chez les 3 malades. Une hypergammaglobulinemie etait presente chez un malade. L’HTP a revele la maladie cœliaque chez 2 patients et elle a ete decouverte, 5 ans apres le diagnostic de la maladie cœliaque, chez une patiente en remission complete sous regime sans gluten bien suivi. Chez tous les malades, la mise ou la poursuite du regime sans gluten etait la regle associee a la prevention des complications de l’hypertension portale. Il n’est survenu aucun cas d’ascite ni d’encephalopathie au cours du suivi. Conclusion La maladie cœliaque peut etre associee ou meme revelee par une HTP non cirrhotique. C’est une association rarement rapportee dans la litterature. Son etiopathogenie n’est pas claire, neanmoins, le role de l’hyperpermeabilite intestinale a ete evoque. Le benefice du regime sans gluten sur la severite de l’HTP reste a prouver.

Abstract

Ecballium elaterium is an herbaceous plant belonging to the Cucurbitaceae family. This plant is fairly common in the Mediterranean regions. It is frequently consumed in infusion, mixture of fruit or even in aerosol in cases of fever or flu. This plant is known for its respiratory and ocular toxicity. Hepatotoxicity has never been described in the literature. We report a case of acute cholestatic hepatitis due to Ecballium elaterium in a 39 years old patient, with no past medical history.

Abstract

High rates of early colorectal cancers (CRC) are observed in Tunisia suggesting genetic susceptibility. Nevertheless, up to now, no molecular study has been performed in the Tunisian population. In our research, we evaluated the clinical characteristics of Tunisian families suspected of Lynch syndrome and the contribution of DNA mismatch repair (MMR) genes.Thirty-one unrelated families suspected of Lynch syndrome were studied. Probands were tested for the presence of germline mutations in the MMR genes MLH1, MSH2, MSH6 and in MUTYH. Available tumours were analysed for microsatellite instability and expression of MMR proteins. Detailed family and medical histories were collected.A total of 134 cancers were noted in the 31 families, the most frequent type of cancer corresponding to CRC (69%), followed by uterine cancer (7.5%). Germline mutations were identified in 11 (35.5%) families (six MSH2, five MLH1, including seven novel mutations), seven of which fulfilled the Amsterdam criteria (sensitivity, 63.6%; positive predictive value, 58.3%). Noteworthy, germline mutations were detected in 52.6% of male patients tested, but in only 8.3% of females (p = 0.02). Moreover, CRC were essentially left sided in families without detected mutation (p = 0.017). Ages of onset of cancers and tumour spectrum were very similar in families with or without MMR germline mutation, contrasting with previous studies performed in other populations.MMR genes contribute significantly to CRC susceptibility in the Tunisian population. However, the cause of early CRC susceptibility remains unknown in most cases, especially in women and in patients with early left colon or rectal cancer.

Abstract

Background/Aim: Variceal bleeding is a life-threatening complication of portal hypertension with a high probability of recurrence. Treatment to prevent first bleeding or rebleeding is mandatory. The study has been aimed at investigating the effectiveness of endoscopic band ligation in preventing upper gastrointestinal bleeding in patients with portal hypertension and to establish the clinical outcome of patients. Patients and Methods: We analyzed in a multicenter trial, the efficacy and side effects of endoscopic band ligation for the primary and secondary prophylaxis of esophageal variceal bleeding. We assigned 603 patients with portal hypertension who were hospitalized to receive treatment with endoscopic ligation. Sessions of ligation were repeated every two to three weeks until the varices were eradicated. The primary end point was recurrent bleeding. Results: The median follow-up period was 32 months. A total of 126 patients had recurrent bleeding. All episodes were related to portal hypertension and 79 to recurrent variceal bleeding. There were major complications in 51 patients (30 had bleeding esophageal ulcers). Seventy-eight patients died, 26 deaths were related to variceal bleeding and 1 to bleeding esophageal ulcers. Conclusions: A great improvement in the prevention of variceal bleeding has emerged over the last years. However, further therapeutic options that combine higher efficacy, better tolerance and fewer side effects are needed.

Abstract

Objectif Evaluer l’innocuite et l’efficacite a long terme de la dilatation hydrostatique des stenoses de localisation digestive haute et basse de la maladie de Crohn. Patients et Methodes Tous les patients qui avaient beneficie d’une dilatation hydrostatique pour une stenose symptomatique compliquant une maladie de Crohn, dans notre unite d’endoscopie, entre janvier 1997 et juin 2008, ont ete colliges. Le diagnostic de stenose symptomatique etait retenu devant des signes cliniques d’obstruction et un retrecissement de la lumiere intestinale objective par une opacification digestive et confirme par un examen endoscopique. Ont ete exclus les patients avec une stenose longue (plus de 6 cm) et/ou associee a une fistule ou une collection. Le succes immediat etait defini par la possibilite de franchir la stenose en fin de seance et le succes a long terme par la disparition des symptomes cliniques d’obstruction et l’absence de recours a des dilatations complementaires ou a la chirurgie. Resultats 11 hommes et 8 femmes d’un âge moyen de 40,8 ans (extremes : 29-65 ans) ont ete colliges. La maladie etait de localisation ileale dans 8 cas, ileo-colique dans 9 cas et duodenale dans 2 cas. 15 patients avaient des antecedents de resection intestinale : 4 avaient ete operes deux fois et un patient trois fois. Le delai moyen entre la derniere intervention chirurgicale et la dilatation hydrostatique etait de 52 mois (extremes : 36-72 mois). Chez la majorite des patients (84 %), la maladie etait quiescente au moment de la dilatation. La stenose etait primitive dans 9 cas, siegeant au niveau du duodenum (n = 2), de l’ileon (n = 4), et du colon (n = 3). Elle etait anastomotique ileo-colique dans 11 cas et colo-colique dans un cas (deux patients avaient deux stenoses chacun). La longueur moyenne de la stenose etait de 2,8 cm (extremes : 1,5-6 cm). La stenose etait ulceree dans 85,7 % des cas. La dilatation etait possible chez 18 malades et un total de 22 seances a ete realise soit une moyenne de 1,2 seance par malade (extremes : 1-2), 14 patients (77 %) ont ete dilates avec succes. Aucune complication severe n’a ete constatee. Le suivi moyen etait de 25,2 mois (extremes : 3-114 mois). Le taux de recidive clinique etait de 27 % a 6 mois et de 76 % a 3 ans. Le taux de recours a la chirurgie etait de 0, 20 et 47 % respectivement a 6, 12 et 36 mois. En analyse univariee, aucun des facteurs etudies n’est ressorti comme predictif d’echec de la dilatation hydrostatique. Conclusion La dilatation hydrostatique est une methode therapeutique sure des stenoses symptomatiques de la maladie de Crohn permettant d’eviter ou de retarder le traitement chirurgical.

Abstract

This paper presents our contribution in terms of synchronization processor to a SoC design methodology based on the theory of the latency insensitive systems (LIS) of Carloni et al.. This methodology 1) promotes pre-developed IPs intensive reuse, 2) segments inter-IPs interconnects with relay stations to break critical paths and 3) brings robustness to data stream irregularities to IPs by encapsulation into a synchronization wrapper. Our contribution consists in IP encapsulation into a new wrapper model containing a synchronization processor which speed and area are optimized and synthetizability guarantied. The main benefit of our approach is to preserve the local IP performances when encapsulating them. This approach is part of the RNRT ALIPTA project which targets design automation of intensive digital signal processing systems with GAUT [1], a high-level synthesis tool. [ABSTRACT FROM AUTHOR]
