Abstract Background In Morocco, much progress has been made in breast cancer treatment. However, there is limited information on survival outcomes of breast cancer patients according to their therapeutic management. Methods A pattern-of-care study was conducted in Morocco’s two main oncology centres: Rabat and Casablanca and has shown that major progress has been made in the quality of care with survival rates comparable to those in developed countries. The present study focuses on the different therapeutic strategies used in breast cancer and their impact on prognosis. Patients were classified into two categories: those considered as appropriately managed and those who were not. Results A total of 1901 women with stage I to III breast cancer were included in this study, the majority (53%) were adequately managed and had better disease-free survival (DFS) rates than those who were not: DFS at 3 years (88% versus 62%) and at 5 years (80% versus 50%). Potential significant determinants of better management were: treatment in Rabat’s oncology centre, treatment between 2008 and 2012, being aged younger than 60 years, and early TN stage. Conclusion This study demonstrated the value of proper integrated and coordinated management in a comprehensive cancer centre, to improve breast cancer survival.

Precision Medicine is being increasingly used in the developed world to improve health care. While several Precision Medicine (PM) initiatives have been launched worldwide, their implementations have proven to be more challenging particularly in low- and middle-income countries. To address this issue, the “Personalized Medicine in North Africa” initiative (PerMediNA) was launched in three North African countries namely Tunisia, Algeria and Morocco. PerMediNA is coordinated by Institut Pasteur de Tunis together with the French Ministry for Europe and Foreign Affairs, with the support of Institut Pasteur in France. The project is carried out along with Institut Pasteur d’Algérie and Institut Pasteur du Maroc in collaboration with national and international leading institutions in the field of PM including Institut Gustave Roussy in Paris. PerMediNA aims to assess the readiness level of PM implementation in North Africa, to strengthen PM infrastructure, to provide workforce training, to generate genomic data on North African populations, to implement cost effective, affordable and sustainable genetic testing for cancer patients and to inform policy makers on how to translate research knowledge into health products and services. Gender equity and involvement of young scientists in this implementation process are other key goals of the PerMediNA project.In this paper, we are describing PerMediNA as the first PM implementation initiative in North Africa. Such initiatives contribute significantly in shortening existing health disparities and inequities between developed and developing countries and accelerate access to innovative treatments for global health.

Abstract Background We conducted a Pattern-of-care (POC) study at two premier-most public-funded oncology centers in Morocco to evaluate delays in care continuum and adherence to internationally accepted treatment guidelines of cervical cancer. Method Following a systematic sampling method, cervical cancer patients registered at Centre Mohammed VI (Casablanca) and Institut National d’Oncologie (Rabat) during 2 months of every year from 2008 to 2017, were included in this retrospective study. Relevant information was abstracted from the medical records. Results A total of 886 patients was included in the analysis; 59.5% were at stage I/II. No appreciable change in stage distribution was observed over time. Median access and treatment delays were 5.0 months and 2.3 months, respectively without any significant temporal change. Concurrent chemotherapy was administered to 57.7% of the patients receiving radiotherapy. Surgery was performed on 81.2 and 34.8% of stage I and II patients, respectively. A very high proportion (85.7%) of operated patients received post-operative radiation therapy. Median interval between surgery and initiation of radiotherapy was 3.1 months. Only 45.3% of the patients treated with external beam radiation received brachytherapy. Radiotherapy was completed within 10 weeks in 77.4% patients. An overall 5-year disease-free survival (DFS) was observed in 57.5% of the patients – ranging from 66.1% for stage I to 31.1% for stage IV. Addition of brachytherapy to radiation significantly improved survival at all stages. The study has the usual limitations of retrospective record-based studies, which is data incompleteness. Conclusion Delays in care continuum need to be further reduced. Increased use of chemoradiation and brachytherapy will improve survival further.

Guided by a national cancer plan (2010–19), Morocco made significant investments in improving breast cancer detection and treatment. A breast cancer pattern-of-care study was conducted to document the socio-demographic profiles of patients and tumour characteristics, measure delays in care, and assess the status of dissemination and impact of state-of-the-art management. The retrospective study conducted among 2120 breast cancer patients registered during 2008–17 at the two premier-most oncology centres (Centre Mohammed VI or CM-VI and Institut National d’Oncologie or INO) also measured temporal trends of the different variables.Median age (49 years) and other socio-demographic characteristics of the patients remained constant over time. A significant improvement in coverage of the state-financed health insurance scheme for indigent populations was observed over time. Median interval between onset of symptoms and first medical consultation was 6 months with a significant reduction over time. Information on staging and molecular profile were available for more than 90% and 80% of the patients respectively. Approximately 55% of the patients presented at stage I/II and proportion of triple-negative cancers was 16%; neither showing any appreciable temporal variation. Treatment information was available for more than 90% of the patients; 69% received surgery with chemotherapy and/or radiation. Treatment was tailored to stage and molecular profiles, though breast conservation therapy was offered to less than one-fifth. When compared using the EUSOMA quality indicators for breast cancer management, INO performed better than CM-VI. This was reflected in nearly 25% difference in 5-year disease-free survival for early-stage cancers between the centres.

Abstract Background Sleep quality is among the indicators associated with the quality of life of patients with cancer. A multitude of factors may affect patient sleep quality and are considered as associated predictive factors. The aim of this study was to examine the predictors of poor sleep quality in Moroccan women with gynecological cancer after radical surgery. Methods A cross-sectional study was carried out at the Oncology Department of the Ibn Rochd University Hospital, Casablanca (Morocco), on women who had undergone radical surgery for gynecological cancer (n = 100; mean age: 50.94 years). To assess sleep quality, symptoms of depression and anxiety, self-esteem and body image, the following translated and validated Arabic versions of the tools were used: Pittsburgh Sleep Quality Index (PSQI), Hospital Anxiety and Depression Scale, Rosenberg’s Self-Esteem Scale and Body Image Scale. To determine predictors of sleep quality, multiple linear and hierarchical regressions were used. Results 78% of participants were considered poor sleepers, most of them exhibited very poor subjective quality (53%), longer sleep onset latency (55%), short period of sleep (42%) and low rate of usual sleep efficiency (47%). 79% of these patients did not use sleep medication and 28% were in poor shape during the day. Waking up in the middle of the night or early in the morning and getting up to use the bathroom were the main reasons for poor sleep quality. Abstract Background The identification of effective prognosis biomarkers for nasopharyngeal carcinoma (NPC) is crucial to improve treatment and patient outcomes. In the present study, we have attempted to evaluate the correlation between pre-treatment plasmatic Epstein-Barr virus (EBV) DNA load and the conventional prognostic factors in Moroccan patients with NPC. Methods The present study was conducted on 121 histologically confirmed NPC patients, recruited from January 2017 to December 2018. Circulating levels of EBV DNA were measured before therapy initiation using real-time quantitative PCR. Results Overall, undifferentiated non-keratinizingcarcinoma type was the most common histological type (90.1 %), and 61.8 % of patients were diagnosed at an advanced disease stage (IV). Results of pre-treatment plasma EBV load showed that 90.9 % of patients had detectable EBV DNA, with a median plasmatic viral load of 7710 IU/ml. The correlation between pre-treatment EBV DNA load and the conventional prognostic factors showed a significant association with patients’ age (p = 0.01), tumor classification (p = 0.01), lymph node status (p = 0.003), metastasis status (p = 0.00) and overall cancer stage (p = 0.01). Unexpectedly, a significant higher level of pre-treatment EBV DNA was also found in plasma of NPC patients with a family history of cancer (p = 0.04). Abstract Background Neuroendocrine carcinomas mainly affect the bronchopulmonary and the gastrointestinal systems. Breast localizations are very rare. They represent less than 0.1% of all breast cancers. A definitive diagnosis relies on histological and immunohistochemical examinations. Case presentation Case 1 We report a case of primary neuroendocrine carcinoma of the breast in a 71-year-old Arabic woman who presented with a 3 cm palpable and mobile tumor of the right breast. Clinical and radiological assessment excluded any other primary tumor. Radical mastectomy and axillary lymph node resection were performed. A histopathological examination disclosed the diagnosis of primary breast neuroendocrine tumors, with negative surgical margins and lymph nodes (18 N-/18 N). The tumor cells were positive for neuroendocrine markers, a weak Ki-67 proliferation index and negative Her2/neu. Our patient received adjuvant hormonal treatment with anti-aromatase for 21 months. She is on regular follow-up, and she remains free of disease to date. Case 2 A 48-year-old Arabic woman consulted for a right breast nodule. She underwent lumpectomy with right axillary lymphadenectomy. The diagnosis was breast neuroendocrine tumor. Systemic treatment was proposed, but she was lost to follow-up. She consulted 1 year later for a mass in the same breast. A histological and immunohistochemical examination of a mammary biopsy was consistent with a recurrence of the previous neuroendocrine tumor. A radiological assessment showed a large mass in her right breast, ipsilateral axillary lymphadenopathies, and hepatic and pulmonary metastases. She received first-line metastatic chemotherapy, with good clinical and radiological improvement. She refused the mastectomy and was given hormone therapy. One year later, the tumor expanded clinically and radiologically, and she underwent second-line metastatic chemotherapy, with good clinical progress and radiological stability, and she then underwent maintenance hormonal therapy. Conclusion Due to the rarity of primary breast neuroendocrine tumors, no standard therapy exists and the prognosis remains difficult to determine. Studies, including larger series, are needed in order to understand the biological behavior of these tumors.

The current study was designed to investigate the changes in the circulating Epstein–Barr virus DNA load (EBV DNA) at various time points before and after treatment and its clinical significance in nasopharyngeal carcinoma (NPC). A total of 142 patients with NPC were prospectively enrolled in this study. The plasma EBV DNA concentration was measured before and after treatment using qPCR. The prognostic values of the EBV DNA load were analyzed using the Kaplan–Meier and Cox regression tests. Following multivariate analysis, our data showed that high pre-EBV DNA loads were associated with significantly poorer distant metastasis free survival (DMFS) and progression free survival (PFS); detectable end-EBV DNA loads were associated with significantly worse loco-regional recurrence free survival (LRRFS) and PFS, and the detecTable 6 months-post-EBV DNA loads were associated with significantly poorer overall survival (OS), DMFS and PFS (p < 0.05). Additionally, combining the pre-EBV DNA load and the stage of the disease, our results showed that patients at stage III-IVA with a low pre-EBV DNA load had similar survival rates as patients at stage II with a low or high pre-EBV DNA load, but had better survival rates than those at stage III-IVA with a high pre-EBV DNA load. Taken together, we showed that the change of the EBV DNA load measured at several time points was more valuable than at any single time point for predicting patients’ survival for NPC. Furthermore, combining the pre-EBV DNA load and the TNM classification could help to formulate an improved prognostic model for this cancer.

Background: Breast cancer is the leading cause of cancer death in women worldwide with more than 1. 7 million new cases annually. Major advances have been made in the therapeutic management of this condition in many countries. However, neuropsychiatric disorders in patients with breast cancer constitute a significant concern due to their negative impact on patient's life and on the success of therapy itself.Methodology: In this study we aimed to evaluate psychological disorders in a population of 212 Moroccan women treated for breast cancer within the Mohammed VI Center for the Treatment of Cancers of Casablanca. A questionnaire was designed to this end on the basis of different validated scales, including the Hospital Anxiety Depression Scale (HADS), the Pittsburgh Sleep Quality Index (PSQI), and the Body Image Scale (BIS).Results: Data analysis has shown that 69.3% of participants had significant sleep disorders; 87% suffer from an anxiety-depressive syndrome (ADS), and 83.0% had significant body image dissatisfaction. A positive correlation was shown between ADS and both patients' national health insurance coverage and patients or husbands' education. Analysis further revealed that anxiety and depression were negatively correlated with different types of treatment. Similarly, both BIS and PSQI scores were positively associated with anxiety and depression disorders.Conclusion: The present investigation highlights the need to generalize and strengthen the psychological approach of patients treated for breast cancer in Morocco. We anticipate that such a strategy will alleviate suffering and promote therapy success in these patients and will diminish or prevent conjugal and familial impacts of the illness.

Male breast cancer is a rare disease accounting for less than 1% of all breast cancer diagnoses worldwide to our knowledge. The aim of this retrospective study is to analyse the epidemiologic, clinical, therapeutic and evolutive profiles of this disease and to compare some cancer aspects between male and female in 25 cases collected at Mohamed VI Oncology Center at the University Hospital of Casablanca between 2012 and 2018. Of all breast cancers, men with breast cancer make up a minority. Male compared to female breast cancers occurred later in life with higher stage and more estrogen receptor-positive tumors. The median age was 67.7 years. The average diagnosis delay was 15.7 month. Cancer was discovered through self examination in 76.1% of cases. The mean diameter was 3.5 cm and range from 1-6 cm. According to the TNM classification, tumors were classified as T1-T2 (40%) and T3-T4 (60%). Infiltrating ductal carcinoma was the most frequent (92%) and 1 case of lobular carcinoma. Axillary nodal involvement was present in 82.4% of cases. Hormonal receptors were positive in 83% of cases. 86.6% of our cases present metastasis, bone was the most representative site. Surgery was usually mastectomy with axillary clearance. It was possible to follow 21 of the patients, the median of follow-up was 12 months. The evolution has been characterized by local recurrence in 6 cases. There was 9 cases of death, death was usually due to comorbid disease and to the advanced age. The 5 years overall survival rates were 57%.

Réaliser une revue récente comportant les particularités épidémiologiques, histologiques des patients nouvellement diagnostiqués d´un cancer de sein à Casablanca durant l´année 2018. Nous avons colligé 668 cas pendant l´année 2018, L´âge moyen était 51,6 ans, le sexe féminin était le plus représenté avec 662 cas (99,1%) et les hommes avec 6 cas (0,9%), soit un sexe ratio (H/F) de 0,009. L´âge moyen de la ménopause était 49,8 ans et l´âge moyen de la ménarche était 13,5 ans, 31,7% avait un antécédent de cancer (le sein 14,1%, l´estomac 9% et le foie 7%). Le délai moyen de consultation était 10 mois, la pathologie la plus représentée était la pathologie thyroïdienne, Selon la localisation, le sein gauche était atteint dans 50,2% et le sein droit dans 44,7% et une localisation bilatérale 1,3%, le siège le plus fréquent était le quadrant supérieur externe avec 28,7%, les stades T1 et T2 représentaient 73,6% et les stades T3 et T4 représentaient 26,4%, les tumeurs épithéliales infiltrantes CCI étaient les plus fréquentes (73,2%). La présence des emboles vasculaires et lymphatiques a été notée chez 42,2%, les ganglions axillaires étaient atteints chez 71,1% des patients. Le grade histo-pronostic de SBR avait révélé une prédominance du grade II dans 55,9% des cas. Le LUMINAL B continue à constituer le phénotype le plus fréquent (46%) suivie du TRIPLE NEGATIF (15,3%) et du LUMINAL A (14,2%) et enfin le HER2 (7,4%). Le pronostic immédiat demeure inquiétant du fait du retard de diagnostic. Il nous paraît urgent de mettre en place une politique sanitaire d´information et d´éducation.

Le syndrome du testicule féminisant est une maladie génétique très rare, de transmission récessive liée au chromosome X, appelé actuellement syndrome d´insensibilité aux androgènes (SIA). Les sujets atteints sont des femmes apparemment normales, mais porteuses d´un caryotype mâle 46 XY avec des testicules ectopiques. A travers un cas d´un SIA révélé par une tumeur germinale (TG) sur testicule ectopique chez une patiente âgée de 29 ans, nous allons illustrer les particularités cliniques, anatomopathologiques et thérapeutiques de cette pathologie. Le risque de dégénérescence, lié à la position ectopique des testicules, augmente avec l´âge. Les cancers les plus fréquents sont les tumeurs germinales et les tumeurs à cellules de Sertoli. Une gonadectomie bilatérale prophylactique est donc recommandée après la puberté.

INTRODUCTION: Le cancer colorectal demeure un véritable fléau et un problème majeur de santé Publique. L´objectif principal de cette étude est d´identifier l´impact des facteurs socio-économiques et du niveau d´instruction sur la survenue du cancer colorectal et stade de diagnostic chez la population marocaine. METHODES: nous avons réalisé une étude cas témoin de janvier 2015 à janvier 2017 au Centre Mohammed VI pour le traitement des cancers. 225 patients et 225 témoins éligibles et consentants sont interviewés de façon prospective en utilisant un questionnaire structuré et pré-testé. RESULTATS: l´âge moyen de nos patients était de 55,49 ± 14,06 ans. 53% ont été diagnostiqués à un stade précoce du cancer du colorectal et 47% à un stade avancé. Par ailleurs, l´analyse détaillée de la population étudiée selon le statut socio-économique (SSE), montre que chez le groupe SSE faible, on note une proportion de 25,33% de patients vs 17,33% de témoins, tandis que dans le groupe SSE moyen, on retrouve des taux équivalents (45,33%) chez les patients et les témoins. Quant au groupe SSE élevé, le taux des patients ne représente que 16,89% contre 37,34% des témoins (P = 0,0001). Le niveau intellectuel et le niveau SSE étaient fortement corrélés au stade du diagnostic avec une différence significative. Ainsi 36,44% des analphabètes ont été diagnostiqués à un stade avancé contre 5,33 des patients ayant un niveau d´étude secondaire ou universitaire avec p = 0,02. De même 20,45% des patients de la catégorie SSE la plus basse étaient diagnostiqués à un stade tardif contre 5,33% des patients avec un SSE le plus élevé avec p=0,03. CONCLUSION: nos résultats soulignent que le risque du cancer colorectal est fortement dépendant du niveau intellectuel et du statut socio-économique des patients. Une enquête plus approfondie est nécessaire pour clarifier les raisons de cette disparité.

Introduction: la constipation est considérée comme rare en Afrique subsaharienne, en raison de la forte teneur en fibre de l´alimentation locale. Le but de ce travail était de décrire les différents aspects cliniques de la constipation fonctionnelle en population générale à Cotonou et de déterminer l´influence de l´alimentation dans sa survenue.

Background: The ATM gene encoding a large protein kinase is mutated in ataxia-telangiectasia (AT), an autosomale recessive disease characterized by neurological and immunological symptoms, and cancer predisposition. Previous studies suggest that heterozygous carriers of ATM mutations have an increased risk of breast cancer compared with non carriers, but the contribution of specific variants has been difficult to estimate. However, two functional ATM variants, c.7271T > G and c.1066–6T > G (IVS10–6T > G), are associated with increased risk for the development of breast cancer. Methods: To investigate the role of ATM in breast cancer susceptibility, we genotyped 163 case patients with breast cancer and 150 healthy control individuals for the c.7271T > G and c.1066–6T > G (IVS10–6T > G) ATM variants using polymerase chain reaction (PCR)-restriction fragment length polymorphism (RFLP) analysis. Results: We did not detect the ATM c.7271T > G and c.1066–6T > G (IVS10–6T > G) mutations in any of 150 healthy control individuals and 163 breast cancer patients, including 59 women diagnosed with breast cancer at an early age ( G (IVS10–6T > G) mutation and the rare c.7271T > G variant are not a risk factor for developing breast cancer in the Moroccan population. Mycosis fungoid (MF) is a non-Hodgkin's T-cell lymphoma determined by primary cutaneous involvement. It is a slow-progressing chronic indolent disease characterized by atypical T cells with a cerebral nucleus. Management of this disease depends on the stage and is based essentially on the systemic treatment. Radiotherapy intervenes in case of localized or extended tumor, indeed, the radiosensibility of this tumor, like any other hematological affection, makes it possible to obtain a high rate of response. Clinical case: we report the observation of a 46-year-old patient followed since 2012 for mycosis fungoid revealed by a papullo-squamous lesion located at the level of the right lumbar fossa. The diagnosis was confirmed by cutaneous biopsy, showing the presence of T lymphocytes expressing CD2, CD3, CD4, CCR4, CD45RO markers. Initial assessment included a thoraco-abdominal pelvic CT, which was normal, an accelerated sedimentation rate at the 1st hour, a high C reactive protein (CRP), the electrolytic, renal, hepatic status and the hemogram were normal. Patient received 6 courses of chemotherapy according to the COPP protocol with a decrease in the size of the lesion estimated at 40%. A norm fractionated radiation therapy was delivered at the dose of 36Gy. The evolution was marked by a complete remission, maintained after 6 months of the treatment. Mycosis fungoid is a rare disease, whose management must be discussed in a multidisciplinary team. Radiotherapy remains an interesting option for all stages, but has to be validated in largest studies.

INTRODUCTION : le cancer colorectal constitue un problème majeur de santé publique. L’objectif de notre étude est d’analyser le profil épidémiologique, nutritionnel, clinique et anatomo-pathologique des cancers colorectaux recrutés au CHU de Casablanca.Méthodes: notre étude cas-témoins a porté sur les patients pris en charge pour un cancer colorectal durant l’année 2015, comparés à des témoins non suivi pour un cancer.Résultats: l’âge moyen des patients était de 56,65 ans avec un écart type de 14,64. Le type histologique le plus fréquent chez nos patients était représenté par l’adénocarcinome Lieberkhünien avec une proportion de 82 %. L’analyse de l’indice de masse corporelle a permis de retrouver une obésité chez 50% des patients contre 20% des témoins et un diabète chez 19% des patients versus 8% des témoins (p = 0,019). Par ailleurs, l’étude du régime alimentaire des patients comparé à celui des témoins semble montrer que la moyenne de la fréquence de consommation hebdomadaire de viandes rouges est plus élevée chez les patients que chez les témoins (4,24 vs 3,26; p = 0,009) et inversement pour la consommation du poissons (0,97 contre 1,76; p = 0,0001). En revanche, la moyenne de consommation des légumes et des fruits est plus faible chez les patients que chez les témoins (5,00 vs 9,50; p = 0,0001). Concernant les habitudes toxiques de nos patients, 32% des patients étaient fumeurs vs 13 % des témoins.Conclusion: nos résultats montrent que la prise de conscience à propos du régime alimentaire et des changements dans nos habitudes de vie pourrait réduire l’incidence du cancer colorectal et par conséquent la mortalité et la morbidité.

Le cancer du sein chez l'homme est une affection rare représentant environ 1% de tous les cancers du sein et moins de 1% de l'ensemble des néoplasies masculines. L'objectif de notre étude est de décrire les différents aspects cliniques, histologique, pronostic et thérapeutiques de cette tumeur rare afin de contribuer à l'amélioration de la prise en charge de cette affection. Il s'agit d'une étude rétrospective portant sur 40 patients colligés au Centre Mohamed VI pour le traitement des cancers de Casablanca de Janvier 2000 à Décembre 2012. L'âge médian a été de 62 ans, le délai moyen de consultation a été de 12 mois, l'autopalpation d'un nodule péri aréolaire a été le principal motif de consultation dans 90% des cas. Le carcinome canalaire infiltrant a été le type histologique prédominant dans 90% des cas. La prise en charge thérapeutique multimodale a consisté en une mastectomie suivie d'un traitement adjuvant à type de chimiothérapie, radiothérapie et/ou hormonothérapie en fonction du stade de la tumeur et des caractéristiques histologiques. Le suivi moyen a été de 38 mois, l'évolution a été caractérisée par une rémission complète chez 16 patients (soit 40%), rechute locale chez 3 patients (soit 7.5%) et par une rechute métastatique chez 5 patients (soit 12.5%). Le site métastatique le plus fréquent a été l'os (62%), suivi par le poumon et le foie. Le décès était survenu chez 10 patients (25%). Le cancer du sein chez l'homme présente des similitudes avec le cancer mammaire chez la femme. Mais il présente aussi beaucoup de particularités, d'où l'intérêt de mener des études prospectives randomisés à plus large échelle afin d'améliorer la prise en charge et le pronostic de cette affection dont l'impact psychosociale est considérable.

Granulosa cell tumors is classified into juvenile and adult types and comprise less than 5% of ovarian tumors in women and are much rarer in men which only 45 have been previously reported. We report here a 40-year young man with a left testicular adult type granulosa cell tumor. The tumor measured 5.5X5X4cm; Immunohistochemical stains showed the tumor diffusely positive for inhibin and vimentin. Post operative CT scans shows a lomboaortic lymphnodes treated by four cycles of chemotherapy type BEP (bleomycin, etoposide, cisplatin). The thoraco abdominal CT scans post chemotherapy shows the disappearance of the right testicular nodule and the lomboaortic lymphnodes. 2 years after treatment, the patient is alive and well with no signs of recurrence. Our report highlights one more case of this very rare tumor of the testis, which is quite problematic In terms of prognosis and management, and for this reason seems to have attracted the interest of many researchers recently.

Les métastases osseuses sont généralement d'origine pulmonaire, prostatique, rénale, vésicale ou thyroïdienne chez les hommes. Une origine colorectale est rare. Peu de publications ont décrit ce type de métastases, la localisation la plus fréquente est le rachis axial ou le bassin. La localisation crânio-faciale est exceptionnelle. Nous relatons ainsi le cas d'une observation d'un homme suivi pour un cancer du rectum métastatique avec une métastase osseuse temporale. Notre observation se rapportait à l'histoire d'un homme âgé de 38 ans suivi pour un cancer du rectum initialement opéré par résection antérieur classant la tumeur pT3N0M0. 24 mois après, le patient présentait une exophtalmie gauche révélant un processus tumoral temporal. L'évolution et le contexte étaient en faveur d'une métastase. En conclusion, nous rapportons dans cette observation, un cas exceptionnel d'une métastase osseuse crânio-faciale d'un cancer colo-rectal multi-métastatique qui pourra enrichir les rares données rapportées dans la littérature se rapportant aux métastases osseuses de primitif colo-rectal.

Le rhabdomyosarcome du col utérin fait partie des types histologiques rares des cancers du col, qu'on observe généralement chez la jeune fille ou la femme en période d'activité générale. La stratégie thérapeutique se base sur l'association des trois modalités thérapeutiques (chimiothérapie-radiothérapie-chirurgie) vu l'agressivité de la maladie. Nous rapporterons ainsi un cas de rhabdomyosarcome du col utérin chez une fille de 20 ans. Il s'agit d'une patiente âgée de 20 ans, rapportant comme antécédents pathologique particulier la notion d'infection génitale à répétition. Le premier signe rapporté était des métrorragies abondantes avec à l'examen la présence d'une masse cervicale en grappe de raisin. La biopsie était en faveur d'un rhabdomyosarcome du col. Le bilan d'extension montrait une masse localement avancée sans métastase. La patiente a reçu 5 cures de VAC avec une régression du processus tumoral de 90%. Patiente fut opérée et a bénéficié d'une hystérectomie sans conservation des annexes. Puis elle a bénéficié d'une radiothérapie postopératoire sur le pelvis. Le recul était de 13 mois, la patiente est toujours en rémission complète. Le RMS du col utérin est une tumeur rare qui survit essentiellement chez la jeune fille. L'extension est surtout locorégionale. Le traitement consiste en un geste chirurgical allant d'un éventuel traitement conservateur jusqu'au traitement radical associé à une chimiothérapie péri-opératoire. La place de la radiothérapie demeure imprécise.

Le carcinome neuroendocrine à grandes cellules primitif du sein est une entité extrêmement rare. Moins de dix cas ont été rapportés dans la littérature. Nous présentons un nouveau cas survenu chez un homme de 61 ans qui s'est présenté pour une tumeur localement avancée du sein droit d'emblée métastatique aux poumons et à la plèvre, classée cT4bN1M1. Le patient avait reçu huit cycles de chimiothérapie à base de Docetaxel tous les 21 jours, avec une bonne réponse clinique et radiologique (=50%), puis mis sous hormonothérapie à base de tamoxifène avec une stabilisation pendant 18 mois. L'étude immuni-histochimique reste indispensable pour déterminer la nature neuroendocrine de cette tumeur. Le traitement n'est pas bien codifié vu la rareté de ce type de cancer.

Les cancers du sein bilatéraux synchrones (CSBS) sont des maladies qui se caractérisent par une importante hétérogénéité clinique et morphologique avec une incidence entre 1,5 et 3,2%. Les femmes traitées pour un cancer du sein unilatéral sont à haut risque de développer un cancer au niveau controlatéral. Le dépistage et les progrès de l'imagerie mammaire ont permis une augmentation de découverte des CSBS. L'objectif de notre travail est d'étudier les caractéristiques épidémiologiques, cliniques, histologiques, et thérapeutiques du cancer du sein bilatéral. Il s'agit d'une étude transversale étalée sur deux années des patientes prise en charge au centre Mohammed VI pour le traitement des cancers. L'analyse statistique des résultats a été réalisée par le logiciel R. 31 patientes ont présenté un CSBS, représentant 2,4% des cas du cancer du sein dans notre Centre. L'âge moyen était 47,8 , 8,4 ans, 22,6% utilisaient des contraceptifs oraux. Une histoire familiale de cancer du sein était observée dans 22,6% des cas. Le type histologique le plus fréquent était le carcinome canalaire infiltrant (58,1%), Le grade SBR II et III étaient fréquents (38,7%). Les récepteurs hormonaux entaient positifs, aux progestérones (38,7%) et aux estrogènes (41,9%). Le récepteur HER2 était surexprimé dans 20,0% des cas. 29,0% des cas ont bénéficié d'une hormonothérapie et 3,2% de thérapies ciblées. Notre étude a montré que le cancer du sein bilatéral représente une proportion faible, mais avec certaines particularités cliniques, différant du cancer du sein unilatéral.

Le cancer du sein est le plus fréquent chez la femme et figure parmi les principales causes de mortalité liée au cancer. La curabilité de ce type tumoral est en augmentation, grâce aux programmes de dépistage et aux progrès thérapeutiques, qui ont certes augmenté la survie des patients. Mais des défis restent à relever en rapport avec l'instabilité phénotypique des cellules cancéreuses. L'objectif de ce travail est d'étudier le profil phénotypique du cancer du sein chez les patients pris en charge au Centre Mohammed VI pour le traitement des Cancer, durant les années 2013-2014. Il s'agit d'une étude transversale sur deux années, incluant les cas du cancer du sein pris en charge au Centre. Le recueil des données était fait à partir des dossiers des patients et analysés par le logiciel Epi Info. 1277 patients ont été pris en charge au sein de notre centre. 99,5% des cas de sexe féminin, l'âge moyen était 50,20 , 11,34 ans. Le type histologique le plus fréquent était le carcinome canalaire infiltrant (80,7% des cas). Le stade diagnostic était précoce (56,9%). Le phénotype moléculaire le plus fréquent était le luminal A (41,4% des cas). Le luminal B, le HER2 et Les triples négatifs étaient dans respectivement 10,4%, 6,3%, 11,2% des cas. L'étude du phénotype tumorale des patients atteints du cancer du sein permet l'orientation du clinicien dans le choix du traitement, et des décideurs dans la planification de programmes de lutte contre cette pathologie.

Les tumeurs phyllodes du sein sont des tumeurs très rares et restent exceptionnelles chez les enfants et les adolescents, leurs traitement est basé sur la chirurgie et la radiothérapie, avec un bon pronostic. Nous rapportons le cas d'une adolescente âgée de 12 ans, qui s'est présentée pour une masse du sein gauche. Le diagnoctic d'une tumeur phyllode a été retenu après bilan et histologie. Le traitement a consisté en une tumorectomie large sans traitement adjuvant ; avec une bonne évolution à un recul de deux ans.

Langiosarcome de loreille est une tumeur maligne très rare, et localement très agressive. Elle représente 4-5% des sarcomes cutanés et moins de 1% de tous les sarcomes. Notre objectif est de décrire et de discuter les modalités diagnostiques et thérapeutiques de cette tumeur maligne. A travers le cas clinique d'une patiente âgée de 31 ans reçue pour une masse bourgeonnante du pavillon de l'oreille gauche. Le scanner était en faveur d'une tumeur du pavillon d'oreille avec envahissement de la parotide homolatérale. Le diagnostic histologique était en faveur d'un angiosarcome. Le traitement a consisté en une chirurgie d'exérèse complète suivie d'une radiothérapie adjuvante. Elle est en rémission compléte avec un recul d'une année.

The cutaneous apocrine carcinomas are malignant tumors rare adnexal, slowly and preferably located in the axillary growth. A girl aged 24 with no particular medical history was addressed to a right axillary mass slowly evolving for two years, associated with pain in the right upper limb. The echo mammogram had shown a mass in the axilla and supraclavicular and axillary MRI had objectified mass axillary measuring 171mm. Pathological examination with immunostaining was in favor of apocrine carcinoma. The cutaneous apocrine carcinomas are well-known in the mammary glands, but it is difficult to morphologically distinguish between breast cancer and apocrine carcinoma.

Le cancer broncho-pulmonaire (CBP) de la femme enceinte est une entité rare, d'évolution péjorative. Cette situation devient de plus en plus fréquente, du fait de l'augmentation du tabagisme chez la femme. La transmission tumorale trans-placentaire avec atteinte fétale est décrite surtout chez les femmes non traitées. Le traitement est multidisciplinaire et n'est pas bien codifié. Nous rapportons le cas d'une patiente de 23 ans chez qui le diagnostic d'un carcinome bronchique à petites cellules a été fait au cours de sa grossesse. Elle avait bénéficié d'une chimiothérapie pendant la grossesse, bien tolérée. L'évaluation radiologique a objectivé une stabilisation du processus pulmonaire. Le traitement a été complété par une association radio-chimiothérapie concomitante après l'accouchement.

Nous proposons une étude rétrospective réalisée au sein du département de gastro-entérologie et d'Oncologie du CHU Casablanca colligeant tous les cas des mélanomes ano-rectaux primitifs, sur une période de 15 ans (du 1997 au 2012). Notre série comportait 14 patients, 8 hommes et 6 femmes avec une moyenne d'âge de 60,5 ans. Les signes cliniques étaient dominés par les rectorragies et le syndrome rectal (plus de 80% des malades). L'aspect tumoral noirâtre à été noté chez la moitié des malades. L'examen endoscopique a révélé une prédominance des lésions ulcéro-bourgeonnantes. Dans 5 cas la tumeur était plus haut située entre 5 à 8 cm de la marge anale. Le bilan d'extension avait décelé des métastases ganglionnaires, osseuses ou viscérales chez 7 malades. Un traitement chirurgical a été pratiqué chez 50% des malades (7 cas). Il a consisté en une exérèse locale isolée (2 cas) ou associée à une radiothérapie (2 cas) et une amputation abdomino-périnéale dans 3 cas. Quatre malades ont reçu une chimiothérapie et/ou radiothérapie palliative et dans deux cas on s'est contenté d'un traitement symptomatique. L'évolution a été marquée par une récidive chez les 2 patients traités par exérèse locale, dont un a été bénéficié d'une amputation abdomino-pelvienne de rattrapage et un des trois patients traités par chirurgie radicale. Deux patients sont en rémission complète après 36 mois de recul.

Le syndrome de Stewart-Treves (SST) est une entité rare, correspondant à un angiosarcome cutané compliquant un lymphoedème chronique. Il est de mauvais pronostic. Stewart et Treves ont rapportés en 1948, les premiers cas d'angiosarcome secondaire à un traitement du cancer du sein. Ce terme s'est généralisé pour regrouper l'ensemble des cas de lymphangiosarcome sur lymphoedème d'origine congénital héréditaire ou non héréditaire, post-traumatique ou post-infectieux. Le SST sur un lymphoedème idiopathique reste exceptionnel. Nous rapportons le cas rare d'une patiente présentant un lymphoedème chronique primitif idiopathique des quatre membres évoluant depuis l'adolescence et qui a développé un SST du membre supérieur droit. Elle a subi une amputation à mi- bras vu le caractère très évolué de la tumeur.

INTRODUCTION: Few population-based cancer registries are in place in developing countries. METHODS: In order to know the burden of cancer in Moroccan population, cancer registry initiative was put in place in the Casablanca district, the biggest city of Morocco. RESULTS: The data collected covers 3.6 millions inhabitant and included Casablanca city and the administrative region. The data collected in the years 2005-07 show that the top 5 forms of cancers in women were breast (ASR: 36.4 per 100,000), cervical (15.0), thyroid (6.7), colon-rectum (5.8), and ovarian (5.3); the top 5 cancers in men were lung (25.9), prostate (13.5), bladder (8.7), colon-rectum (8.1) and non-Hodgkin lymphoma (7.2). Tumours of haematopoietic and lymphoid tissues represented 11% of all cancers (skin excluded); some presented unusual sex ratios. For breast, cervical, colorectal and thyroid cancer, respectively 57%, 42%, 28% and 60% of the cases were under 50 years of age. This was attributable to particularly low numbers of cases recorded among old people, and the young age of the general population; the observed age-specific incidences under age 50 were not higher than in western countries. Cancers at young ages were particularly common in women: 67% of the cases were under 50. Stage at diagnosis could be obtained for 82% of the breast cancer cases and was as follows: 28% local, 63% regional and 9% distant, in the absence of screening. CONCLUSION: These first population-based data have provided an invaluable resource for the national cancer control plan of Morocco, and will be useful tool to its future evaluation.

Les métastases osseuses isolées des cancers colorectaux sont très rares. Le squelette axial est habituellement le plus atteint. La localisation au niveau du radius est exceptionnelle. Nous rapportons l'observation d'une femme âgée de 60 ans avec une métastase du radius distal isolée métachrone d'un cancer du haut rectum opéré un an auparavant. La métastase a été découverte sur les examens d'imagerie et confirmée par biopsie. Une amputation a été réalisée suite à une progression de la maladie après une radiothérapie palliative. L'évolution était marquée par l'apparition de métastases pulmonaires et le décès est survenu dans un tableau de défaillance respiratoire.

Les métastases cérébrales des cancers du col de l'utérus sont extrêmement rares. Elles sont généralement supra-tentorielles, survenant à un stade avancé de la maladie et dans un cadre de néoplasie polymétastatique. La tumeur primitive est le plus souvent un carcinome épidermoïde peu différencié. Leur pronostic reste sombre malgré toutes les options thérapeutiques. Vu la rareté de cet événement et le peu de cas publiés dans la littérature, nous rapportons l'observation clinique d'une jeune patiente de 44 ans, opérée pour un carcinome du col utérin et qui présente 14 mois plus tard des métastases cérébrales sus et sous tentorielles associées à des métastases ganglionnaires lombo-aortique, médiastinale et sus-claviculaire. Elle a bénéficié d'un traitement palliatif associant une chimiothérapie et une radiothérapie pan encéphalique. Devant l'altération rapide de l'état général, la patiente a été mise sous un traitement symptomatique et des soins de support.

INTRODUCTION: In Morocco, cervical cancer is the second most common cancer in women. The cases of cervical cancer are diagnosed at a late stage: 43.7% presented at stage II of diagnosis (FIGO) and 38.1% in advanced stage (stage III and IV). The main objective of this study is to investigate factors associated to late the diagnosis of cervical cancer in Morocco as measured by the stage at diagnosis and delays between first symptoms and diagnosis of cancer. METHODS: Cross-sectional studies, conducted from June-2008 to June-2010 at two main oncological centers. Two-hundred cases were recruited. Stages I and II were identified as early-stage. The dates of first-symptoms, first-consultation and first-diagnosis were used to define Patient, Medical and Total delays. RESULTS: Elevated risks for late stage was observed for women unmarried (OR=5.0; 95%CI: 1.43-16.66); living less than 100 km from center of diagnosis (OR=4.51; 95%CI: 1.35-15.11); without a familial history of cancer (OR=14.28; 95%CI: 2.22-100) and whose was the first symptom not bleeding (OR=25; 95%CI: 1.62-300). Frequency of housewives was significantly higher for women with a patient-delayless than1 month. Frequency of patients who had symptoms of bleeding was significantly higher for women with a patient-delay 1 month. Frequency of patients from urban area was significantly higher for women with a ,Medical-delay less than 1 month. Elevated risks for a long -Total-delay, was observed for women aged 50years (OR=2.44; 95%CI: 1.24-4.76); illiterate (OR=3.85; 95%CI: 1.45-10.00) and from rural-area (OR=2.56; 95%CI: 1.25-5.26).
