Aneurysmal bone cyst (ABC) is a benign bone tumor affecting mainly children and young adults. It occurs in the metaphysis of the long bones. The scapula is a very rare location. Imaging may be highly suggestive of ABC in cases of an osteolytic, expansive, and hemorrhagic lesion with fluid-fluid levels and thin septa. The diagnosis must systematically be confirmed by performing a biopsy, in order to adopt the best therapeutic strategy. There are several therapeutic means, but wide resection remains the gold standard. The evolution is very variable and can go from spontaneous healing to recurrence with the destruction of the bone. We report a rare case of aneurysmal bone cyst of the scapula in a young patient.

Abstract Background and study aims The success of conversion total hip arthroplasty (THA) among primary THA and revision THA remains unclear. We hypothesized that most conversion THA’s can be performed using primary implants and will have an uncomplicated post-operative course. Materials and methods Thirty-six patients (23 females, mean age 68,0y) who underwent conversion THA for failed interventions for proximal femur fractures in the period 2008–2018 were matched sequentially against patients of the same sex and age who underwent primary THA or revision THA. Data were collected on implants used, major complications, and mortality. PROMs used included the Western Ontario and McMaster Osteoarthritis Index, Harris Hip Score, Visual Analogue Scale and the EQ-5D Health Questionnaire. Results Seventy-two percent of patients who underwent conversion THA were treated with primary implants and never suffered from a major complication. PROMs were excellent for this group of patients. The distinction primary/conversion/revision THA could not explain differences in outcomes; however, the necessity of using revision implants and the development of major complications could. Conclusions The majority of conversion total hip arthroplasties can be considered a primary replacement. Predicting outcomes for THA should focus on patient frailty and technical difficulties dealing with infection, stability and loss of bone stock and should discard the conversion versus revision terminology.

La rareté actuelle des manifestations ostéoarticulaires de la syphilis tend à faire oublier et méconnaître certains aspects autrefois banals de l´affection. L´arthropathie tabétique, affection neurogène destructrice, est due à une perte de l´innervation douloureuse et proprioceptive. Elle est devenue exceptionnelle grâce au traitement précoce de la syphilis par la pénicilline. Nous rapportons le cas d´un patient de 58 ans, consultant pour une monoarthrite du genou révélant d´une arthopathie tabétique.

Giant squamous cell carcinomas (SCC) larger than 5 cm in diameter are uncommon; there is no guideline on the size of an SCC that is considered giant. Treatment may be difficult with the need for large tissue resections and complex surgical reconstruction. We report a rare case of giant squamous cell carcinoma of the shoulder attached to deep anatomic planes. The entire mass was removed, resulting in a large defect that was repaired with myocutaneous flap rotation of the latissimus dors. Three courses of radiotherapy were performed after surgery. Fifteen months after the operation, the patient is well and working without any local recurrence and metastasis.

Un patient de 45 ans a subi un traumatisme du genou droit à la suite d'un accident de la voie publique occasionnant chez lui une fracture distale supra condylienne comminutive du fémur droit et une rupture du tendon rotulien ipsilatérale. Cette association est exceptionnelle et aucun cas n'a été retrouvé dans la littérature. Un diagnostic précis clinique et radiologique suivi d'une prise en charge précoce et adaptée par une ostéosynthèse interne et une rééducation fonctionnelle précoce et adaptée ont permis d'obtenir de bons résultats à long terme.

Nous rapportons un cas d'un jeune patient ayant présenté à la suite d'un accident de sport une luxation astragalo-scapho-calcanéenne interne où il a bénéficié d'un traitement orthopédique avec un bon résultat clinique et radiologique.

La maladie de Freiberg est une ostéochondrose des têtes métatarsiennes, touchant le plus fréquemment la deuxième tête. Les auteurs rapportent à travers une étude rétrospective étalée sur trois ans, une série de 06 patientes, dont l'âge moyen est de 19,8 ans, opérées par une ostéotomie de soustraction dorsale selon Gauthier avec une fixation par des ostéosutures. Les résultats de cette technique sont satisfaisants vue que la majorité de nos patientes ont retrouvé l'indolence et ont récupéré une activité quotidienne habituelle. Les auteurs ont conclu que la fixation par des ostéosutures est une méthode fiable et peu coûteuse.

La rupture bicroisée est rare, des deux LCA simultanément est exceptionnelle, alors que la rupture bicroisée bilatérale simultanée et post-traumatique n'a été jamais décrite dans la littérature, ce qui rend notre cas très intéressant pour étude, suivi et discussion thérapeutique puisqu'on a privilégié la prise en charge thérapeutique en deux temps opératoires espacés dans le temps et nos résultats étaient très satisfaisantes aussi bien pour nous que pour le malade.

This study presents a comprehensive computational fluid dynamics (CFD) analysis of Trombe walls in laminar flow conditions, focusing on the impact of thermal fins. The investigation explores configurations with 3 and 5 fins, considering varying fin heights, numbers, and rotation angles. The Nusselt number, friction factor, heat transfer rate, and thermal efficiency are meticulously examined for Reynolds numbers ranging from 600 to 1600. Results reveal that for configurations with three fins, a rotation angle of 70° yields the highest thermal efficiency, showcasing a notable increase from 32.3 % to 44 % across all Reynolds numbers. In contrast, configurations with five fins exhibit enhanced thermal efficiency when fins are maintained horizontally (90°) at low Reynolds numbers but experience a decline as Reynolds numbers increase. Specifically, the thermal efficiency drops from 62.48 % to 55.5 % for Re = 1400 and from 64.7 % to 58 % for Re = 1600. The study concludes that Trombe walls with three fins should be designed with a 70° rotation angle, while those with five fins should feature horizontally oriented fins for optimal thermal performance. These findings offer valuable insights for the design and optimization of Trombe walls, providing guidance for achieving enhanced energy efficiency in passive heating systems.

This research aims to showcase the imperative of integrating passive cooling and heating techniques within buildings to effectively decrease energy consumption. To accomplish this objective, numerical simulations were conducted on a representative double-walled brick building in two cold desert climates using TRNSYS software. The simulations were performed to compare and evaluate the thermal and energy performance of two distinct passive techniques, one for cooling the reflective roof and the other for heating the Trombe wall (TW). The findings demonstrate that employing a reflective roof building with an air gap significantly reduces indoor temperatures by 3 °C during the hottest weekdays of summer, as compared to the reference building, across the two studied Moroccan climates. Moreover, this approach decreases the demand for cooling energy by an impressive 69–78 %. Similarly, the passive heating technique proves highly effective during winter, reducing energy requirements by 26–30 % while increasing indoor temperatures by over 2.9 °C. Combining both techniques proves to be the most efficient solution, resulting in a 37.87 % reduction in total annual energy consumption for cooling and heating in Er-Rachidia and a 30 % reduction in Ifran. These results strongly support the suggestion that Moroccan thermal regulations should incentivize the adoption of passive cooling and heating systems, particularly in cold desert climates.

In this paper, a thermal performance assessment of traditional building constructions and configurations in various Moroccan climates is carried out. This research focuses on enhancing conventional energy efficiency of buildings by investigating the thermal behavior of single clay walls, double clay walls, simple brick walls, and double brick walls based on long-term meteorological data using TRNSYS software. The roof pitch, thermal comfort and discomfort index, and building energy consumption are considered while selecting buildings for each temperature zone. It is noticed that the double brick construction is more efficient in all climatic zones of Morocco. Thus, this material reduces the yearly energy requirement of building by 22% to 25% compared to the usual brick building. On the coldest winter days, a 30° south-facing roof raises the internal temperature by 2.5 °C compared to a horizontal roof. Temperature increases could reduce the air conditioning energy usage by 7% to 32% and overcooling hours by 12% to 21% in cities above 30° N. In addition, the findings are interesting enough to inspire individuals to implement the recommended construction design and study energy efficiency and thermal comfort in Moroccan buildings. Consequently, reducing energy consumption, mitigating climate change, and lowering greenhouse gas emissions.
