Abstract

This article discusses the outcomes of the endoscopic endonasal approach (EEA) for nonfunctional pituitary adenomas (NFPAs) in elderly patients. The study involved a retrospective review of NFPAs in patients aged 65 and older who underwent EEA between 2007 and 2022 at a single medical center. The results showed that EEA was a safe and effective option for NFPA resection in elderly patients, with high rates of visual recovery postoperatively and acceptable rates of gross total resection (GTR). The study also identified risk factors for progression and subtotal resection, but found no associations between postoperative complications and age or frailty scores. [Extracted from the article]

Abstract

This article, published in the Journal of Neurological Surgery, introduces a novel outcome metric called the 30-day hospitalization ratio (HR) for patients with pituitary adenomas. The metric combines the length of stay and readmission rates within a 30-day period to provide a comprehensive measure of the economic burden of treatment. The study analyzed the clinical records of 468 patients who underwent endoscopic endonasal surgery for pituitary adenoma. 29. Effect of Adding Zeolitic Tuff on Geotechnical Properties of Lime-Stabilized Expansive Soil.

Abstract

This study aims to evaluate experimentally the potential of expansive soil stabilization using different additives: zeolitic tuff (ZT), lime, and a combination of lime and ZT. Four different percentages of ZT (10%, 20%, 25%, and 30%), three percentages of lime (2%, 4%, and 6%), and variable percentages of their combinations were used to stabilize the soil for pavement subbase application. Atterberg limits, pH, compaction, linear shrinkage, swelling, unconfined compressive strength (UCS), and California bearing ratio (CBR) tests were performed on treated and untreated soil specimens at different curing times. Results showed that ZT additives effectively reduced the plasticity, linear shrinkage, and swell potential in addition to increase the maximum dry unit weight, UCS, and CBR. The results of this study were supported by a microstructural analysis using scanning electron microscopy (SEM) associated with the energy-dispersive X-ray spectroscopy (SEM/EDX) technique. It was determined that the UCS and CBR values for the 4% lime stabilized soil increased by 22% and 70%, respectively, after the addition of 25% ZT. Based on evaluation of the results, an optimum mixture of 25% ZT and 4% lime stabilized soil can be used in pavement subbase applications as it achieved the minimum strength target. Abstract

To artificially reprogram cell fate, experimentalists manipulate the gene regulatory networks (GRNs) that maintain a cell's phenotype. In practice, reprogramming is often performed by constant overexpression of specific transcription factors (TFs). This process can be unreliable and inefficient. Here, we address this problem by introducing a new approach to reprogramming based on mathematical analysis. We demonstrate that reprogramming GRNs using constant overexpression may not succeed in general. Instead, we propose an alternative reprogramming strategy: a synthetic genetic feedback controller that dynamically steers the concentration of a GRN's key TFs to any desired value. The controller works by adjusting TF expression based on the discrepancy between desired and actual TF concentrations. Theory predicts that this reprogramming strategy is guaranteed to succeed, and its performance is independent of the GRN's structure and parameters, provided that feedback gain is sufficiently high. As a case study, we apply the controller to a model of induced pluripotency in stem cells. Abstract

Disease trajectories following antibody therapy can have a significant impact on the pharmacokinetics of the antibody. Although this phenomenon can often be explained by reduced target‐expressing cells, other mechanisms may play a role. We use a novel minimal physiologically‐based pharmacokinetic model to evaluate an alternative drug–disease interaction mechanism involving competitive inhibition of neonatal Fc receptor (FcRn)‐mediated Immunoglobulin G recycling by paraproteins. The model is validated with clinical data from the anti‐FcRn antibody M281 and is used to conduct a scenario test to quantify the interaction among M‐protein, the characteristic paraprotein of multiple myeloma (MM), and the anti‐CD38 antibody daratumumab indicated for MM treatment. Simulations predict up to a 3.6‐fold increase in daratumumab half‐life following M‐protein reduction, which lends credence to the hypothesis that FcRn competition in MM can manifest as time‐dependent reduction of clearance for daratumumab. This model can inform optimal dosing strategies for antibodies in MM and other pathologies of paraprotein excess. [ABSTRACT FROM AUTHOR]

In the past decade, researchers have been able to obtain pluripotent stem cells directly from an organism's differentiated cells through a process called cell reprogramming. This opens the way to potentially groundbreaking applications in regenerative and personalized medicine, in which ill patients could use self-derived induced pluripotent stem (iPS) cells where needed. While the process of reprogramming has been shown to be possible, its efficiency remains so low after almost ten years since its conception as to render its applicability limited to laboratory research. In this paper, we study a mathematical model of the core transcriptional circuitry among a set of key transcription factors, which is thought to determine the switch among pluripotent and early differentiated cell types. By employing standard tools from dynamical systems theory, we analyze the effects on the system's dynamics of overexpressing the core factors, which is what is performed during the reprogramming process. We demonstrate that the structure of the system is such that it can render the switch from an initial stable steady state (differentiated cell type) to the desired stable steady state (pluripotent cell type) highly unlikely. This finding provides insights into a possible reason for the low efficiency of current reprogramming approaches.
