Abstract

Background and aims: Proprotein Convertase Subtilisin/Kexin type 9 inhibitors (PCSK9i) are recommended in patients at high and very-high cardiovascular (CV) risk, with documented atherosclerotic CV disease (ASCVD), and for very-high risk patients with familial hypercholesterolaemia not achieving LDL-cholesterol (LDL-C) goal while receiving maximally tolerated dose of lipid-lowering therapy (LLT). However, single country real-life data, reporting the use of PCSK9i in clinical practice, are limited. Therefore, we designed AT-TARGET-IT, an Italian, multicenter, observational registry on the use of PCSK9i in clinical practice. Methods: All data were recorded at the time of the first prescription and at the latest observation preceding inclusion in the study. Results: 798 patients were enrolled. The median reduction in LDL-C levels was 64.9%. After stratification for CV risk, 63.8% achieved LDL-C target; of them, 83.3% took LLTs at PCSK9i initiation and 16.7% did not. 760 patients (95.2%) showed high adherence to therapy, 13 (1.6%) partial adherence, and 25 (3.1%) poor adherence. At 6 months, 99.7% of patients enrolled in the study remained on therapy; there were 519 and 423 patients in the study with a follow-up of at least 12 and 18 months, respectively. Persistence in these groups was 98.1% and 97.5%, respectively. Overall, 3.5% of patients discontinued therapy. No differences in efficacy, adherence, and persistence were found between alirocumab and evolocumab. Conclusions: PCSK9i are safe and effective in clinical practice, leading to very high adherence and persistence to therapy, and achievement of recommended LDL-C target in most patients, especially when used as combination therapy.

Abstract

Background: The severe acute respiratory syndrome coronavirus 2 (SARS‐CoV‐2) has a tropism for the gastrointestinal tract and several studies have shown an alteration of the gut microbiota in hospitalized infected patients. However, long-term data on microbiota changes after recovery are lacking. Methods: We enrolled 30 patients hospitalized for SARS‐CoV‐2-related pneumonia. Their gut microbiota was analyzed within 48 h from the admission and compared with (1) that of other patients admitted for suspected bacterial pneumonia (control group) (2) that obtained from the same subject 6 months after nasopharyngeal swab negativization. Results: Gut microbiota alpha-diversity increased 6 months after the resolution of SARS-CoV-2 infection. Bacteroidetes relative abundance was higher (≈ 36.8%) in patients with SARS-CoV-2, and declined to 18.7% when SARS-CoV-2 infection resolved (p = 0.004). Conversely, Firmicutes were prevalent (≈ 75%) in controls and in samples collected after SARS-CoV-2 infection resolution (p = 0.001). Ruminococcaceae, Lachnospiraceae and Blautia increased after SARS-CoV-2 infection resolution, rebalancing the gut microbiota composition. Conclusion: SARS-CoV-2 infection is associated with changes in the gut microbiome, which tend to be reversed in long-term period.

Abstract

Background and purpose: The objective of this study was to assess the neurological manifestations in a series of consecutive severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2)-positive patients, comparing their frequency with a population hospitalized in the same period for flu/respiratory symptoms, finally not related to SARS-CoV-2. Methods: Patients with flu/respiratory symptoms admitted to Fondazione Policlinico Gemelli hospital from 14 March 2020 to 20 April 2020 were retrospectively enrolled. The frequency of neurological manifestations of patients with SARS-CoV-2 infection was compared with a control group. Results: In all, 213 patients were found to be positive for SARS-CoV-2, after reverse transcriptase polymerase chain reaction on nasal or throat swabs, whilst 218 patients were found to be negative and were used as a control group. Regarding central nervous system manifestations, in SARS-CoV-2-positive patients a higher frequency of headache, hyposmia and encephalopathy always related to systemic conditions (fever or hypoxia) was observed. Furthermore, muscular involvement was more frequent in SARS-CoV-2 infection. Conclusions: Patients with COVID-19 commonly have neurological manifestations but only hyposmia and muscle involvement seem more frequent compared with other flu diseases.

Abstract

Le fratture mandibolari costituiscono una quota che va dal 40% al 62% di tutte le fratture facciali. In circa la meta dei casi le fratture mandibolari sono multiple e tra queste una quota consistente presenta due rime di frattura. L' alta frequenza di fratture doppie è da attribuire alla particolare forma della mandibola. Da un punto di vista biomeccanico la mandibola può essere assimilata per grandi linee ad un arco le cui estremita sono costituite da condili. tali strutture sono libere di ruotare all' interno delle cavità glenoidi assicurando in tal modo un certo grado di ammortizzazione in risposta ai traumi. Nonostante non vi siano dubbi circa le indicazioni chirurgiche delle fratture mandibolari doppie non è ancora chiaro quale frattura dovrebbe essere trattata prima. Lo scopo di questo studio è quello di chiarire, in base alla nostra esperienza, la corretta sequenza chirugica di riduzione da seguire nel trattamento delle fratture mandibolari doppie. Abbiamo condotto uno studio retrospettivo su un campione di pazienti operati presso il nostro reparto nel periodo compreso tra Gennaio 2007 e Gennaio 2010. Abbiamo incluso nello studio solo i pazienti trattati per doppia frattura mandibolare e li abbiamo suddivisi in due gruppi basandoci sulla diversa sequenza di riduzione delle fratture. I pazienti sono stati seguiti con uno stretto follow-up clinico e radiologico per valutare le variabili outcome di confronto tra i gruppi. L' analisi dei risultati ha evidenziato che i pazienti (gruppo A) in cui è stata ripristinata prima la dimensione orizzontale (frattura toothbearing) e poi quella verticale (non- toothbearing) hanno ottenuto minori complicanze postoperatorie e ridotti tempi operatori rispetto al gruppo di confronto (gruppo B). Il ripristino della dimensione orizzontale fornisce una guida per ottenere la riduzione efficace delle fratture non-toothbearing e il ripristino della dimensione verticale. In conclusione, sulla base della nostra esperienza e dei risultati ottenuti, in caso di fratture mandibolari doppie, gli autori suggeriscono la riduzione in prima istanza delle fratture toothbearing e successivamente qualle in area non-toothbearing. AIM: This study aims to clan, according to our experience, the correct surgical sequence which should be followed in order to treat double mandibular fractures.MATERIAL OF STUDY: From January 2007 to January 2010, we have conducted a retrospective study on a sample of patients operated on in our department. We include only those cases in which the jaw was fractured in 2 places, in particular patients who suffer a fracture in tooth-bearing areas (symphysis, parasymphysis, and anterior body) and also contralaterally in non tooth-bearing areas (posterior body, angle, ramus, and condyle). The sample was divided into 2 groups based on the fracture sequence of reduction.RESULTS: At 1-year follow-up, the group of patients who received first the tooth-bearing fractured areas treatment, followed by treatment of non tooth-bearing fractured area on bifocal mandibular fracture (Group A), showed less postoperative complications and reduced surgical time and costs.DISCUSSION: In patients of group B, the non-execution of rigid IMF for the non tooth-bearing fractures made bone segments more free to move. Thus, reduction and fixation of non tooth-bearing fractures is facilitated, but poses a greater risk of complications. The surgeon in this case does not have the occlusal help guide; thus, the tooth-bearing fracture reduction and the subsequent fixation may be imperfect.CONCLUSION: It is recommended from this study that reduction of the tooth-bearing fragment be prior to that of the tooth-free fragment for the double mandibular fracture.

Abstract

Orbital floor fractures are the most common facial fractures. The goals of orbital floor fracture repair are to free incarcerated or prolapsed orbital tissue from the fracture defect and to span the defect with an implant to restore the correct anatomy of the orbital floor and the pretrauma orbital volume. No consensus exists on the choice of implants to be used for orbital floor reconstruction, and several implant materials are available.Our study intended to evaluate, for the first time, the effectiveness and complications related to the use of a resorbable collagen membrane in the reconstruction of small pure blow-out fractures. From October 2008 to November 2010, 23 patients who underwent reconstruction of the orbital floor using a resorbable collagen membrane following fracture were included in this study. At the 6-month follow-up, only 2 patients (9%) reported postoperative complications secondary to the operative procedure (surgical approach, orbital floor dissection), but these were not directly related to the use of the membrane. Abstract

Orbital approaches provide significant trajectory to the skull base and are used with differently designed pathways. The aim of this study is to investigate the feasibility of a combined transorbital and transnasal approach to the anterior and middle cranial fossa. Cadaveric dissection of five silicon-injected heads was used. A total of 10 bilateral transorbital approaches and 5 extended endonasal approaches were performed. Identification of surgical landmarks, main anatomical structures, feasibility of a combined approach and reconstruction of the superior orbital defect were examined. Rod lens endoscope (with 0° and 45° lenses) and endoscopic instruments were used to complete the dissection. The transorbital approach showed good versatility and provides the surgeon with a direct route to the anterior and middle cranial fossa. The transorbital avascular plane showed no conflict with major nerves or vessels. Large exposure area from crista galli to the third ventricle was demonstrated with significant control of different neurovascular structures. A combined transorbital transnasal approach provides considerable value in terms of extent of exposure and free hand movement of the two surgeons, and allows better visualisation and control of the ventral skull base, thus overcoming the current surgical limits of a single approach. Combination of these two minimally invasive approaches should reduce overall morbidity. Clinical trials are needed to evaluate the virtual applications of this approach.Gli approcci transorbitari permettono di ottenere un'ampia via d'accesso al basicranio e sono utilizzati in diverse procedure chirurgiche. Lo scopo di questo studio è valutare la fattibilità di un approccio endoscopico combinato transobitario e transnasale alla fossa cranica anteriore e media. Su cinque teste iniettate con silicone, sono stati eseguiti un totale di 10 approcci endoscopici transorbitari e 5 "extended endonasal approach" (EEA). Durante le procedure sono stati identificati i punti di repere chirurgici e le strutture anatomiche principali, valutando la fattibilità dell'approccio combinato e della successiva ricostruzione del tetto orbitario. Per eseguire la dissezione sono stati utilizzati endoscopi rigidi con sistema a lente cilindrica 0° e 45° e una strumentazione endoscopica dedicata. L'approccio transorbitario ha mostrato grande versatilità ed ha permesso al chirurgo di ottenere un corridoio diretto alla fossa cranica anteriore e media. Il piano transorbitario avascolare non era in conflitto con vasi o nervi maggiori. La dissezione ha mostrato una vasta esposizione del basicranio, dalla crista galli fino al terzo ventricolo, assicurando un controllo efficace sulle strutture neurovascolari circostanti. L'approccio combinato transorbitario e transnasale ha portato dei vantaggi significativi sia nell'ampiezza dell'esposizione che nella manovrabilità della strumentazione chirurgica. L'approccio endoscopico combinato transnasale e transorbitario ha dimostrato una migliore visualizzazione del basicranio ventrale e di poter superare alcune attuali limitazioni dei due singoli approcci. Inoltre la combinazione di due approcci mini-invasivi dovrebbe contribuire a ridurre la morbilità chirurgica. Sono tuttavia necessari studi clinici per validare questo approccio..

Abstract

Castleman's disease (giant lymph node hyperplasia, angiofollicular hyperplasia, lymphoid hamartoma, benign giant lymphoma), is a quite rare and complex disease of lymphoid tissue that causes progressive lymph node enlargement, typically presenting as a solitary mediastinal mass. It was first described by Castleman et al in 1956. The head and neck regions are the second, less common site for this lesion. The preoperative diagnosis is very often extremely difficult and the routine investigations are often inconclusive. A multicentric extension of this disease shows poorer prognosis compared to the unifocal Histologically, three types do exist: the hyaline-vascular type, the plasma cell type and the mixed type. The etiology and pathogenesis is still unclear. In the literature, to our knowledge, only 112 cases have been reported involving head and neck, and only 22, including our own, interesting the parotid gland. In this report we describe a rare singular of Castleman's disease presenting as a tumor of the inferior pole of the parotid gland extending in the submandibular region, in a 35-year-old woman. The patient undergone a surgical treatment and, therefore, the mass was successfully totally removed.

Abstract

The palatovaginal or pharyngeal artery is a small branch of the internal maxillary artery supplying the nasopharynx. Bleeding from this artery is exceptional and only one case of traumatic epistaxis from this artery has been reported previously. We report a case of a 66-year-old male presenting with right recurrent posterior epistaxis. Endoscopic dissection of the pterygopalatine fossa and direct visualization of the palatosphenoidal canal permitted to identify the origin of bleeding, and coagulation of the pharyngeal artery solved the epistaxis. Although rare, intractable posterior epistaxis may arise from the pharyngeal artery. The anatomical knowledge of this artery and of the palatosphenoidal canal is of outmost importance in endoscopic transpterygoid and nasopharyngeal procedures, to identify the vidian canal, evaluate nasopharyngeal cancer spread in the pterygopalatine fossa, reduce bleeding during surgery of the nasopharynx, and harvest adequately the pedicle of the nasoseptal flap.

Abstract

Nowadays agro-industrial waste induces increasing problems due to the high economic cost and heavy environmental impact of disposal. By contrast, its potential re-use as organic fertilizer could represent a sustainable approach to recycling nutrients and reintegrating organic matter into soil. Such recycling should be particularly beneficial in Mediterranean areas because there is a progressive loss of soil fertility. To assess the possible re-use of industrial citrus waste as organic fertilizer, a two-year research project was carried out to study the effects of dried orange waste on the growth and production of durum wheat (Triticum durum Desf.). Two dried orange waste doses of 4 and 8 kg/m2 were compared with conventional mineral fertilization, of 80 kg/ha2 of nitrogen, and with a control without fertilization in the first year. During the second year, the residual effects of the past year's fertilization and two-year application of the orange waste doses and mineral fertilization on duration of biological cycle, grain yield, leaf area index, above-ground biomass and crop growth rate were studied. Our results show that organic fertilization gave similar wheat yields to the mineral fertilization, averaging at 3.63 t/ha. Organic fertilization promoted crop growth much more than mineral fertilization, by up to +400%. However, at the highest dose repeated organic fertilization induced a severe depressive effect on crop establishment. It indeed gave the lowest values for leaf area index of 0.6, biomass of 222 g/m2 dry weight and crop growth rate of 2.5 gm-2 d-1 at the heading stage, and a 50% decrease in wheat grain yield. In both years, the lowest dose of orange waste produced maximum agronomic efficiency. We conclude that an appropriate use of dried orange waste as fertilizer can partially solve environmental problems related to the citrus fruit processing industry, and represents a low cost organic matter source for Mediterranean soil with poor fertility.

Abstract

Scorpiurus subvillosus L., wide spread in pastures of Mediterranean basin, is disappearing in the native pastures of the Hyblean plateau (Sicily, southern Italy), because of overgrazing and intensive management techniques. Moreover, it exhibits seed coat dormancy, which delays and reduces germination preventing its diffusion. This paper represents a first attempt in order to investigate changing in germination determined by storage time and temperature on seeds of two populations of S. subvillosus. Germination of S. subvillosus seeds was tested in relation to four storage time (30, 130, 200 and 360 days after harvest (DAH)), eight constant temperatures (5, 10, 15, 20, 25, 30, 35 and 40°C) and two populations of different provenience (30 and 600 m above mean sea level). The experiments were conducted either on scarified and unscarified seeds. In S. subvillosus the failure of germination under favourable conditions must be attributed only to seed coat, since seed scarification enhanced germination percentage with values up to 100% at almost all tested temperatures. In both treatments, but with a grater incidence in unscarified, seed germination increased gradually as temperature raised, peaking at 20-25°C, then declined with further increases of temperatures. At 40°C no germination occurred. Storage time induced a softening effect, which is somewhat limited by the natural ageing of seeds occurring from about 6 months after harvest.
