Multiple-beam holography has been widely used for the realization of photonic quasicrystals with high rotational symmetries not achievable by the conventional periodic crystals. Accurate control of the properties of the interfering beams is necessary to provide photonic band-gap structures. Here we show, by FDTD simulations of the transmission spectra of 8-fold quasiperiodic structures, how the geometric tiling of the structure affects the presence and properties of the photonic band-gap for low refractive index contrasts. Hence, we show an interesting approach to the fabrication of photonic quasicrystals based on the use of a programmable Spatial Light Modulator encoding Computer-Generated Holograms, that permits an accurate control of the writing pattern with almost no limitations in the pattern design. Using this single-beam technique we fabricated quasiperiodic structures with high rotational symmetries and different geometries of the tiling, demonstrating the great versatility of our technique., Comment: 8 pages, 6 figures

3. Isotropic properties of the photonic band gap in quasicrystals with low-index contrast

We report on the formation and development of the photonic band gap in two-dimensional 8-, 10- and 12-fold symmetry quasicrystalline lattices of low index contrast. Finite size structures made of dielectric cylindrical rods were studied and measured in the microwave region, and their properties compared with a conventional hexagonal crystal. Band gap characteristics were investigated by changing the direction of propagation of the incident beam inside the crystal. Various angles of incidence from 0 \degree to 30\degree were used in order to investigate the isotropic nature of the band gap. The arbitrarily high rotational symmetry of aperiodically ordered structures could be practically exploited to manufacture isotropic band gap materials, which are perfectly suitable for hosting waveguides or cavities., Comment: 16 pages, 7 figures, submitted to PRB

- Abstract
Background: The role of neuromuscular blocking agents (NMBAs) in coronavirus disease 2019 (COVID-19) acute respiratory distress syndrome (ARDS) is not fully elucidated. Therefore, we aimed to investigate in COVID-19 patients with moderate-to-severe ARDS the impact of early use of NMBAs on 90-day mortality, through propensity score (PS) matching analysis. Methods: We analyzed a convenience sample of patients with COVID-19 and moderate-to-severe ARDS, admitted to 244 intensive care units within the COVID-19 Critical Care Consortium, from February 1, 2020, through October 31, 2021. Patients undergoing at least 2 days and up to 3 consecutive days of NMBAs (NMBA treatment), within 48 h from commencement of IMV were compared with subjects who did not receive NMBAs or only upon commencement of IMV (control). The primary objective in the PS-matched cohort was comparison between groups in 90-day in-hospital mortality, assessed through Cox proportional hazard modeling. Secondary objectives were comparisons in the numbers of ventilator-free days (VFD) between day 1 and day 28 and between day 1 and 90 through competing risk regression. Results: Data from 1953 patients were included. After propensity score matching, 210 cases from each group were well matched. In the PS-matched cohort, mean (± SD) age was 60.3 ± 13.2 years and 296 (70.5%) were male and the most common comorbidities were hypertension (56.9%), obesity (41.1%), and diabetes (30.0%). The unadjusted hazard ratio (HR) for death at 90 days in the NMBA treatment vs control group was 1.12 (95% CI 0.79, 1.59, p = 0.534). After adjustment for smoking habit and critical therapeutic covariates, the HR was 1.07 (95% CI 0.72, 1.61, p = 0.729). At 28 days, VFD were 16 (IQR 0–25) and 25 (IQR 7–26) in the NMBA treatment and control groups, respectively (sub-hazard ratio 0.82, 95% CI 0.67, 1.00, p = 0.055). Il contributo, ripercorrendo la produzione legislativa emanata sul tema dell’intervento nei centri storici dalla Regione Siciliana dagli anni Settanta ad oggi, si sofferma in particolare sulla LR 13/2015 “Norme per favorire il recupero del patrimonio edilizio di base dei centri storici”, un provvedimento legislativo molto contestato in ambito accademico che, di fatto, costituisce un grave arretramento culturale e tecnico non tenendo in alcun conto gli avanzamenti metodologici sul tema dei centri storici a partire dalla Carta di Gubbio del 1960. Nell’intento di favorire il recupero dei centri storici, la legge si fa portatrice di una serie di semplificazioni, a partire dal ricorso a classificazioni tipologiche approssimative e predefinite per tutti i centri storici siciliani (non certo riconducibili agli studi della cosiddetta scuola muratoriana), che consentono di omettere le analisi storiche e urbanistiche delle diverse realtà territoriali, dimenticando che i centri storici sono strutture urbane di antica formazione, che costituiscono la parte più pregiata della città contemporanea e il palinsesto della memoria collettiva.

Le misure relative al contenimento del Covid-19 hanno cambiato profondamente i nostri stili di vita nei mesi scorsi e ancora oggi, soprattutto per quanto concerne l’uso dello spazio pubblico. Improvvisamente lo spazio privato è diventato predominante a causa delle impellenti necessità dettate dal distanziamento sociale, talvolta con effetti drammatici. Tale situazione ha fatto emergere con tutta evidenza la necessità di ripensare le politiche pubbliche sulla questione abitativa e la dotazione dei servizi, suggerendo allo stesso tempo, come possibile strategia di prevenzione del contagio, il riequilibrio territoriale tra sistemi urbani densamente abitati e aree interne del Paese in declino demografico. Particolare attenzione va a nostro avviso rivolta a quelle del Sud d’Italia, caratterizzate da basse densità di popolazione, disponibilità di patrimonio edilizio non utilizzato e standard ambientali più elevati. Obiettivo del contributo è una proposta metodologica rivolta alle amministrazioni pubbliche locali per delineare politiche di ripopolamento dei comuni delle aree interne, come precondizione per la prevenzione del contagio. Un ragionamento intorno a forme integrate di housing sociale diffuso nel tessuto storico e l’adeguata dotazione di servizi possono fornire una soluzione efficace, con riguardo in particolare alle categorie sociali più fragili ed esposte ai rischi connessi alle pandemie. The measures relating to Covid-19 containment changed deeply our lifestyles in the past months and still now, especially for what regards the use of public space. Private space suddenly became predominant for the impellent needs of the social distancing, sometimes with dramatic effects. This situation highlighted the importance of rethinking public policies on housing and services provision and at the same time suggested as a possible strategy for contagion prevention the rebalancing between urban systems densely inhabited and internal areas of the country in demographic decline. Particularly, attention will be paid to a southern internal area where population density is low, high-unused housing stock is available and environmental standards are higher. The objective is to build a methodological proposal for supporting municipalities’ repopulation policies in inland areas, taking into account contagion prevention. Reasoning about integrated forms of social housing spread throughout the historical fabric and the adequate services provision can provide an effective solution, with particular regard to the most vulnerable social categories, which are most exposed to pandemics risks.

- Abstract
Initial stages of Best vitelliform macular dystrophy (BVMD) and adult vitelliform macular dystrophy (AVMD) harbor similar blue autofluorescence (BAF) and optical coherence tomography (OCT) features. Nevertheless, BVMD is characterized by a worse final stage visual acuity (VA) and an earlier onset of critical VA loss. Currently, differential diagnosis requires an invasive and time-consuming process including genetic testing, electrooculography (EOG), full field electroretinogram (ERG), and visual field testing. The aim of our study was to automatically classify OCT and BAF images from stage II BVMD and AVMD eyes using a deep learning algorithm and to identify an image processing method to facilitate human-based clinical diagnosis based on non-invasive tests like BAF and OCT without the use of machine-learning technology. After the application of a customized image processing method, OCT images were characterized by a dark appearance of the vitelliform deposit in the case of BVMD and a lighter inhomogeneous appearance in the case of AVMD. By contrast, a customized method for processing of BAF images revealed that BVMD and AVMD were characterized respectively by the presence or absence of a hypo-autofluorescent region of retina encircling the central hyperautofluorescent foveal lesion. The human-based evaluation of both BAF and OCT images showed significantly higher correspondence to ground truth reference when performed on processed images. The deep learning classifiers based on BAF and OCT images showed around 90% accuracy of classification with both processed and unprocessed images, which was significantly higher than human performance on both processed and unprocessed images. The ability to differentiate between the two entities without recurring to invasive and expensive tests may offer a valuable clinical tool in the management of the two diseases.

Background The role of neuromuscular blocking agents (NMBAs) in coronavirus disease 2019 (COVID-19) acute respiratory distress syndrome (ARDS) is not fully elucidated. Therefore, we aimed to investigate in COVID-19 patients with moderate-to-severe ARDS the impact of early use of NMBAs on 90-day mortality, through propensity score (PS) matching analysis. Results Data from 1953 patients were included. After propensity score matching, 210 cases from each group were well matched. In the PS-matched cohort, mean (± SD) age was 60.3 ± 13.2 years and 296 (70.5%) were male and the most common comorbidities were hypertension (56.9%), obesity (41.1%), and diabetes (30.0%). The unadjusted hazard ratio (HR) for death at 90 days in the NMBA treatment vs control group was 1.12 (95% CI 0.79, 1.59, p = 0.534). After adjustment for smoking habit and critical therapeutic covariates, the HR was 1.07 (95% CI 0.72, 1.61, p = 0.729). At 28 days, VFD were 16 (IQR 0–25) and 25 (IQR 7–26) in the NMBA treatment and control groups, respectively (sub-hazard ratio 0.82, 95% CI 0.67, 1.00, p = 0.055). At 90 days, VFD were 77 (IQR 0–87) and 87 (IQR 0–88) (sub-hazard ratio 0.86 (95% CI 0.69, 1.07; p = 0.177). Conclusions In patients with COVID-19 and moderate-to-severe ARDS, short course of NMBA treatment, applied early, did not significantly improve 90-day mortality and VFD. In the absence of definitive data from clinical trials, NMBAs should be indicated cautiously in this setting.

The contraction of relational public space in favor of private one was one of the consequences due to the Covid-19 containment, which generated deep changes on lifestyles. This situation leads not only to reformulate public policies on housing and services provision, but also to reconsider the territorial rebalancing between urban systems densely inhabited and internal areas of the country in demographic decline as a precondition of prevention contagion. Particularly, attention will be paid to a southern internal area where population density is low, high-unused housing stock is available and environmental standards are higher. The objective is to build a methodological proposal for supporting municipalities’ repopulation policies in inland areas, taking into account contagion prevention. Reasoning about integrated forms of social housing spread throughout the historical fabric and the adequate services provision can provide an effective solution, with particular regard to the most vulnerable social categories, which are most exposed to pandemics risks.

- Abstract
PURPOSE: Brain death (BD) causes metabolic and energetic imbalances leading to cardiac dysfunction, and predisposes the donor heart to further injury following heart transplantation (HTx). The metabolic mechanisms required for myocardial energy production during BD and subsequent HTx are poorly understood. Our aim was to determine the myocardial metabolic profile and mitochondrial function following donor BD and HTx. METHODS: Donor BD in sheep was induced by inflation of a catheter placed through the skull (catheter placement, but no inflation for SHAM), followed by 24 hrs monitoring, and heart procurement (n=6/group, BD vs. SHAM). Additional donor hearts exposed to BD/SHAM were flushed with cold St Thomas cardioplegia, and stored via cold static storage (CSS) for ∼2 hrs. Following standard orthotopic HTx, recipients were weaned off bypass and monitored for ≤6 hrs prior to heart procurement (n=4/group, BD-Tx vs. Sh-Tx). Cardiac mitochondrial function was assessed using high resolution respirometry. Metabolic profiles were determined in hearts using metabolomics. Cardiac mitochondrial function was also determined in two sheep that underwent HTx following BD and 8 hr hypothermic ex vivo perfusion (HEVP) preservation. RESULTS: BD caused significant right ventricular (RV) mitochondrial uncoupling (vs. SHAM). HTx following CSS also impaired RV mitochondrial function, with these effects more pronounced in hearts exposed to both donor BD and HTx. Early findings show that HEVP improved cardiac mitochondrial function post-HTx (vs. CSS). Metabolically, BD increased myocardial amino-acid utilisation and accumulation of glucose metabolites. Post-HTx, particularly in those exposed to donor BD, there was a significant decrease in metabolites involved in mitochondrial respiration (eg. NAD, Acetyl-CoA) and accumulation of fatty acids and xanthine (purine breakdown). CONCLUSION: BD appears to trigger cardiac mitochon
