Abstract

BACKGROUND: Solid pseudopapillary neoplasm (SPN) is an uncommon pathology of the pancreas with unpredictable malignant potential. Endoscopic ultrasound (EUS) assessment plays a vital role in lesion characterization and confirmation of the tissue diagnosis. However, there is a paucity of data regarding the imaging assessment of these lesions. AIM: To determine the characteristic EUS features of SPN and define its role in preoperative assessment. METHODS: This was an international, multicenter, retrospective, observational study of prospective cohorts from 7 large hepatopancreaticobiliary centers. All cases with postoperative histology of SPN were included in the study. Data collected included clinical, biochemical, histological and EUS characteristics. RESULTS: One hundred and six patients with the diagnosis of SPN were included. The mean age was 26 years (range 9 to 70 years), with female predominance (89.6%). The most frequent clinical presentation was abdominal pain (80/106; 75.5%). The mean diameter of the lesion was 53.7 mm (range 15 to 130 mm), with the slight predominant location in the head of the pancreas (44/106; 41.5%). The majority of lesions presented with solid imaging features (59/106; 55.7%) although 33.0% (35/106) had mixed solid/cystic characteristics and 11.3% (12/106) had cystic morphology. Calcification was observed in only 4 (3.8%) cases. Main pancreatic duct dilation was uncommon, evident in only 2 cases (1.9%), whilst common bile duct dilation was observed in 5 (11.3%) cases. One patient demonstrated a double duct sign at presentation. Elastography and Doppler evaluation demonstrated inconsistent appearances with no emergence of a predictable pattern. EUS guided biopsy was performed using three different types of needles: Fine needle aspiration (67/106; 63.2%), fine needle biopsy (37/106; 34.9%), and Sonar Trucut (2/106; 1.9%). Abstract

Background and purpose After failure of endoscopic retrograde cholangiopancreatography (ERCP), endoscopic ultrasound (EUS)-guided biliary drainage is a feasible and safe alternative to percutaneous transhepatic biliary drainage and surgical bypass for distal malignant biliary strictures (DMBS). The aim of this study was to compare the efficacy, safety, cost, and patency of self-expandable metallic stent insertion by EUS-guided versus percutaneous route for biliary drainage after failure of ERCP. Patients and methods A total of 40 patients with DMBS after failure of ERCP were randomized into two groups: group I (20 patients) underwent EUS-BD and group II (20 patients) underwent percutaneous metallic biliary drainage (PMBD); both groups used SEMS. Results The technical and clinical success rates for EUS-BD and PMBD groups were 100 and 95% (P=0.897). Nonetheless, complications were more common in the PMBD group (10 vs. 25%, P=0.212). Fever, cholangitis, and mild bile leakage were early complications in both groups, whereas stent migration (one patient) in EUS-BD group and stent obstruction (four patients) (P=0.035) in PMBD group were late complications. Between EUS-BD and PMBD groups, the rate of reintervention was 5 and 20% (P=0.171), and cumulative cost of the procedures was 964.32 ± 79.8$ and 1098.61 ± 84.5$, respectively (P=0.035). Conclusion When compared with PMBD for DMBS after failure of ERCP, EUS-BD is a technically successful and safe procedure with a shorter fluoroscopy duration, less minor complications, longer stent patency, and a lower cost with fewer re-interventions. Abstract

Background There is a close interaction between inflammation and coagulation. Hemostatic abnormalities are common in inflammatory bowel disease (IBD) with higher risk for a hypercoagulable state and prothrombotic conditions. In addition, a few cases of acquired coagulopathy with higher risk of bleeding have been reported. The involved pathophysiologic mechanisms are complex and incompletely understood. Objective This is a case–control study that aimed to assess the levels of Von Willebrand factor (VWF) in IBD as a marker of disease activity and its relation to higher risk of bleeding or thrombotic events. Patients and methods A total of 46 patients with IBD aged 18 years or older were enrolled in the study. After consenting, patients were divided into two groups: one group included 23 patients with active IBD and the other group included 23 patients with inactive IBD. Activity of Crohn’s disease was assessed through the Harvey-Bradshaw index and activity in patients with ulcerative colitis was evaluated with the simple clinical colitis activity index. The white blood cell count, hemoglobin level, platelet count, activated partial thromboplastin time, C-reactive protein, erythrocyte sedimentation rate, albumin, fecal calprotectin, VWF antigen level (VWF:Ag), and VWF ristocetin cofactor activity (VWF: RCo) were measured. The VWF:RCo/VWF:Ag ratio was calculated. Results There were significant differences in the mean±SD of vWF antigen and vWF:RCo levels between active IBD group (189.30±62.83 and 101.73±23.42, respectively, P=0.001) and inactive IBD group 177.30±64.90 and 97.08±24.21, respectively, P=0.001). The IBD activity index was correlated with VWF antigen (r=0.78 P=0.001) and VWF:RCo levels (r=0.74 P=0.001). VWF antigen and VWF:RCo were correlated with fecal calprotectin (r=0.65, P=0.001, and r=0.67, P=0.001, respectively). The odds ratio of an elevated vWF antigen greater than 150% was 24 (95% confidence interval: 4.38–131.47) in the group with active IBD compared with the inactive IBD group. The VWF:RCo/VWF:Ag ratio of less than 0.7 which reflects the possibility of acquired Von Willebrand syndrome was detected in five patients (21.7%) with active IBD in comparison with only one patient (4.3%) in the inactive IBD group, with odds ratio of 6.1 (95% confidence interval: 0.65–57.1). Conclusion VWF antigen and activity levels can be used as markers for evaluation of IBD activity. Assessment of VWF in IBD could be significant for better hemostatic control of such patients. Abstract

In international relations literature, there seems to be some confusion caused by the many contradictory theories on alliance formation patterns. For this reason, this thesis surveys why there is not just one theory that explains most of the alliance formations throughout history. Using logistic regression models and statistical analysis for different historical periods from 1816 to 2012, the thesis explores the effects of four state-level variables—regime type, national material capabilities, geographical proximity, and trade exchange—on alliance formation behaviors. The results show that the four state-level variables have different levels of significance in the different periods. The thesis concludes that alliance formation behaviors differ depending on the prevailing system-level conditions in the different historical periods, especially under conditions of war and peace and based on the polarity of the international system.
