BACKGROUND: Post-stroke arm impairment at rehabilitation admission as predictor of discharge arm impairment was consistently reported as extremely useful. Several models for acute prediction exist (e.g. the Scandinavian), though lacking external validation and larger time-window admission assessments. OBJECTIVES: (1) use the 33 Fugl-Meyer Assessment-Upper Extremity (FMA-UE) individual items to predict total FMA-UE score at discharge of patients with ischemic stroke admitted to rehabilitation within 90 days post-injury, (2) use eight individual items (seven from the Scandinavian study plus the top predictor item from objective 1) to predict mild impairment (FMA-UE≥48) at discharge and (3) adjust the top three models from objective 2 with known confounders. METHODS: This was an observational study including 287 patients (from eight settings) admitted to rehabilitation (2009-2020). We applied regression models to candidate predictors, reporting adjusted R2, odds ratios and ROC-AUC using 10-fold cross-validation. RESULTS: We achieved good predictive power for the eight item-level models (AUC: 0.70-0.82) and for the three adjusted models (AUC: 0.85-0.88). We identified finger mass flexion as new item-level top predictor (AUC:0.88) and time to admission (OR = 0.9(0.9;1.0)) as only common significant confounder. CONCLUSION: Scandinavian item-level predictors are valid in a different context, finger mass flexion outperformed known predictors, days-to-admission predict discharge mild arm impairment. Background: Bone marrow edema syndrome (BMES), previously referred to as transient osteoporosis, is an uncommon and underdiagnosed self-limiting condition typically affecting the weight-bearing joints of the hip and lower limb. Its occurrence in upper limb or non-weight bearing joints is particularly rare. Case presentation: We report the case of an otherwise healthy 42-year-old man who gradually developed severe and disabling left shoulder pain over the course of 6 months. Nine months after onset, he presented clinically with bilateral involvement and radiologically (magnetic resonance imaging, MRI) with diffuse BME associated with a subchondral insufficiency fracture, suggestive of proximal humeral avascular necrosis. Clinical evidence of subacromial shoulder impingement and MRI findings of bilateral subchondral bursitis and tenosynovitis with effusion of the biceps tendons likely resulted from primary BME, as musculoskeletal sonography confirmed the absence of rotator cuff tears. Repeated MRI findings at 2, 12, and 18 months documented near complete resolution of both edema and fracture, consistent with BMES of the proximal humerus. During this time, the patient reported a gradual improvement in both pain symptoms and range of motion. The clinical picture of insidious shoulder pain, exacerbated by activity and improved by load relief, in the absence of predisposing factors for osteonecrosis or antecedent trauma in patients of middle age, should indicate the possibility of the diagnosis of BMES. Conclusions: This unprecedented report documents a rare case of bilateral BMES of the humeral head mimicking avascular necrosis. The correct diagnosis of BMES within an atypical anatomical location avoids invasive measures in the affected bone. The misdiagnosis of secondary BME and idiopathic osteonecrosis can be avoided by recognizing the characteristics of BME and subchondral fractures of the humeral head in the absence of rotator cuff tears, as well as their evolution on serial MRI. Objectives: Helical plates are preferably used for proximal humeral shaft fracture fixation and potentially avoid radial nerve irritation. Aims: Safety of applying four different long plate designs (straight, 45°-, 90°-helical and ALPS) with MIPO technique as well as assessment and comparison of their distances to adjacent anatomical structures. Methods: MIPO was performed in 16 human cadaveric humeri using either a straight (group 1), a 45°-helical (group 2), a 90°-helical (group 3) plate, or an ALPS (group 4). Applying CT angiography, distances between brachial arteries and plates were evaluated. All specimens were dissected and distances to the axillary, radial and musculocutaneous nerve were evaluated. Results: No specimens demonstrated injuries of the anatomical structures at risk after MIPO with all investigated plate designs. Closest overall distance (mean (range); mm) between each plate and the radial nerve was 1 (1–3) in group 1, 7 (2–11) in group 2, 14 (7–25) in group 3 and 6 (3–8) in group 4. It was significantly bigger in group 3 and significantly smaller in group 1 compared to all other groups, p < 0.001. Closest overall distance between each plate and the musculocutaneous nerve was 16 (8–28) in group 1, 11 (7–18) in group 2, 3 (2–4) in group 3 and 6 (3–8) in group 4. It was significantly bigger in group 1 and significantly smaller in group 3 compared to all other groups, p < 0.001. Conclusions: MIPO with 45°- and 90°-helical plates as well as with ALPS is safely feasible and demonstrates significantly bigger distances to the radial nerve compared to straight plates. However, the distances remain small and attention must be paid to the musculocutaneous nerve and the brachial artery when MIPO is applied using ALPS, 45°- and 90°-helical implants. OBJECTIVES:Studies of osteoarthritis (OA) in human skeletal remains can come with scalar problems. If OA measurement is noted as present or absent in one joint, like the elbow, results may not identify specific articular pathology data and the sample size may be insufficient to address research questions. If calculated on a per data point basis (i.e., each articular surface within a joint), results may prove too data heavy to comprehensively understand arthritic changes, or one individual with multiple positive scores may skew results and violate the data independence required for statistical tests. The objective of this article is to show that the statistical methodology Generalized Estimating Equations (GEE) can solve scalar issues in bioarchaeological studies. MATERIALS AND METHODS:Using GEE, a population-averaged statistical model, 1,195 adults from the core and one colony of the prehistoric Tiwanaku state (AD 500-1,100) were evaluated bilaterally for OA on the seven articular surfaces of the elbow joint. RESULTS:GEE linked the articular surfaces within each individual specimen, permitting the largest possible unbiased dataset, and showed significant differences between core and colony Tiwanaku peoples in the overall elbow joint, while also pinpointing specific articular surfaces with OA. Data groupings by sex and age at death also demonstrated significant variation. A pattern of elbow rotation noted for core Tiwanaku people may indicate a specific pattern of movement. DISCUSSION:GEE is effective and should be encouraged in bioarchaeological studies as a way to address scalar issues and to retain all pathology information.

Physiotherapy improves the movement range after the onset of post-traumatic elbow stiffness and reduces the pain, which is a factor limiting elbow range of motion. However, no results have been reported for motor-cognitive intervention programs in post-traumatic elbow stiffness management. The objective was to investigate the efficacy of graded motor imagery (GMI) in post-traumatic elbow stiffness. Fifty patients with post-traumatic elbow stiffness (18 female; mean age, 41.9 ± 10.9 years) were divided into 2 groups. The GMI group (n = 25) received a program consisting of left-right discrimination, motor imagery, and mirror therapy (twice a week for 6 weeks); the structured exercise (SE) group (n = 25) received a program consisting of range-of-motion, stretching, and strengthening exercises (twice a week for 6 weeks). Both groups received a 6-week home exercise program. The primary outcome was the Disabilities of the Arm, Shoulder, and Hand (DASH) questionnaire. The secondary outcomes were the active range of motion (AROM), visual analog scale (VAS), Tampa Scale for Kinesiophobia (TSK), muscle strength of elbow flexors and extensors, grip strength, left-right discrimination, and Global Rating of Change. Patients were assessed at baseline, at the end of treatment (12 sessions), and a 6-week follow-up. The results indicated that both GMI and SE interventions significantly improved outcomes (P <.05). After a 6-week intervention, the DASH score was significantly improved with a medium effect size in the GMI group compared with the SE group, and improvement continued at the 6-week follow-up (F 1,45 = 3.10, P =.01). The results with a medium to large effect size were also significant for elbow flexion AROM (P =.02), elbow extension AROM (P =.03), VAS-activity (P =.001), TSK (P =.01), and muscle strength of elbow flexors and elbow extensors (P =.03) in favor of the GMI group. The GMI is an effective motor-cognitive intervention program that might be applied to the rehabilitation of post-traumatic elbow stiffness to improve function, elbow AROM, pain, fear of movement-related pain, and muscle strength. Introduction: Anthropometry is one of the traditional and conceivably the basic tool of biological anthropology. Height is an important identifying feature of an individual. Estimation of height can be done by measurement of ulna and various regression formulae have been derived by different authors for different regions. Aim: To define correlation and to derive the linear regression equation from the length of ulna in male tribal population of Udaipur region, Rajasthan, India. Materials and Methods: The present cross-sectional study was conducted at Geetanjali Medical College and Hospital, Udaipur, Rajasthan, India, from February 2019 to January 2020. Male tribal population between the age of 20-40 years, in and around Udaipur district of Rajasthan, were included in this study. Measurement of ulnar length of both right and left sides were recorded along with their height. ANOVA test was done to find out the correlation between height and ulnar length. Regression equation was formulated for the study population. Results: Total 245 males, belonging to Bhil and Meena tribes of Rajasthan, of mean age 31.87 years, were included in this study. The mean length of right ulna was 25.52 cm, of left ulna was 25.82 cm, and mean height was 159.34 cm. The study showed a positive correlation between the length of ulna and total height of male population of Udaipur region, in which the correlation coefficient of right ulna with height was 0.61, and that of left ulna with height was 0.64. Conclusion: A positive correlation exists between the length of ulna and height of an individual.

Some of the Sima de los Huesos (SH) humeri have been previously studied and described elsewhere. Here we present an updated inventory and a review of the specimens recovered to the present day. The morphological key traits of the adult and subadult specimens are described, discussed, and illustrated. The SH humeri share with Neandertals many traits usually considered to be Neandertal specializations, thus, most of this morphological pattern is not exclusive to them. The variation found within fossil samples stresses the frequential nature of all these traits and in the specific case of the SH humeri, most of the traits considered as phylogenetically relevant are retained by their descendants, the Neandertals. Some traits are plesiomorphic for the entire genus Homo or are present in European hominins since the early Pleistocene. Finally, some other traits display high variability within the SH sample or different hominin samples and are of uncertain phylogenetic value. Altogether, this evidence is consistent with the hypothesis based on the overall cranial and postcranial morphology that the SH hominins are a sister group to the later Neandertals., (© 2023 The Authors. Introduction: The Kocher approach is the workhorse approach to the lateral elbow. However, the exposure is often limited, particularly for open reduction. The purpose of this study is to quantitatively compare the articular exposure of the anconeus and Kocher approaches to the lateral elbow.Methods: Eight surgical approaches (four Kocher and four Anconeus) were performed on four fresh cadavers. The right elbows of the first two specimens were dissected via the Kocher approach, and the left elbows via the anconeus approach. For the remaining two specimens, the laterality of the approaches was reversed. Access to key articular landmarks were assessed, including the capitellum, humeral trochlea, radial head, olecranon, coronoid process, and greater and lesser sigmoid notches of the ulna. A calibrated digital image was taken from the optimum surgeon's viewing angle of each approach, and these images were analyzed with ImageJ software (NIH, Bethesda, MD, USA) to calculate the area of exposed articular surfaces.Results: The average surface area exposed was 2.9 times greater with the anconeus approach compared with the standard Kocher approach (8.3 vs 3.1 cm2, p value 0.001). All key anatomic landmarks were directly visualized with the anconeus approach in each specimen. Visualization of the humeral trochlea, olecranon, coronoid process, and greater and lesser sigmoid notches of the ulna was not obtained in any of the Kocher approaches.Discussion: The Anconeus approach provides superior exposure of the lateral elbow joint compared with the Kocher approach. We recommend consideration of the anconeus approach for treatment of select traumatic injuries of the lateral elbow requiring increased access to the ulnohumeral and radiocapitellar joints. BackgroundThe effects of dietary protein on bone health are controversial.ObjectiveWe examined the relation between protein intake with fracture and bone mineral density (BMD) within the Women's Health Initiative (WHI).DesignThis prospective analysis included 144,580 women aged 50-79 y at baseline in the WHI clinical trials (CTs) and observational study (OS) that recruited participants in 1993-1998 with follow-up through 2011. Self-reported clinical fractures were collected semiannually through the original end of the trials (WHI CTs) and annually (WHI OS) by questionnaires. Hip fracture was adjudicated by a central review of radiology reports. BMDs for total body, hip, and spine were measured at baseline and 3 and 6 y in 9062 women at 3 WHI clinics by using dual-energy X-ray absorptiometry. Protein intake was assessed via food-frequency questionnaire and calibrated by using biomarkers of energy and protein intakes. Associations between protein intake and fracture were estimated by using Cox proportional hazards regression, and the relation between protein intake and BMD was estimated by using linear regression.ResultsMedian biomarker-calibrated protein intake was 15% of energy intake. Per 20% increase in calibrated protein intake (percentage of energy), there was no significant association with total fracture (HR: 0.99; 95% CI: 0.97, 1.02) or hip fracture (HR: 0.91; 95% CI: 0.84, 1.00), but there was an inverse association with forearm fracture (HR: 0.93; 95% CI: 0.88, 0.98). Each 20% increase in calibrated protein intake was associated with a significantly higher BMD for total body (mean 3-y change: 0.003 g/cm²; 95% CI: 0.001, 0.005 g/cm²) and hip (mean 3-y change: 0.002 g/cm²; 95% CI: 0.001, 0.004 g/cm²).ConclusionsHigher biomarker-calibrated protein intake within the range of usual intake was inversely associated with forearm fracture and was associated with better maintenance of total and hip BMDs. These data suggest higher protein intake is not detrimental to bone health in postmenopausal women.

Accurate placement of a coordinate system on the radius is important to quantitatively report 3D surgical planning parameters or joint kinematics using 4D imaging techniques. In clinical practice, the scanned length of the radial shaft varies among scanning protocols and scientific studies. The error in positioning a radial coordinate system using a partially scanned radius is unknown. This study investigates whether the imaged length of the radius significantly affects the positioning of the coordinate system. For different lengths of the radius, the error of positioning a coordinate system was determined when placed automatically or manually. A total of 85 healthy radii were systematically shortened until 10% of the distal radius remained. Coordinate systems were placed automatically and manually at each shortening step. A linear mixed model was used to associate the positioning error with the length of the radial shaft. The accuracy and precision of radial coordinate system placement were compared between automatic and manual placement. For automatic placement of the radial coordinate system, an increasing positioning error was associated with an increased shortening of the radius (P = < 0.001). Background: The accurate restoration of premorbid anatomy is key for the success of reconstructive surgeries of the proximal part of the humerus. The bicipital groove has been proposed as a landmark for the prediction of humeral head retrotorsion. We hypothesized that a novel method based on bilateral registration of the bicipital groove yields an accurate approximation of the premorbid anatomy of the proximal part of the humerus.Methods: Three-dimensional (3D) triangular surface models were created from computed tomographic data of 100 paired humeri (50 cadavers). Segments of the distal part of the humerus and the humeral shaft of prespecified lengths were defined. A surface registration algorithm was applied to superimpose the models onto the mirrored contralateral humeral model based on the defined segments. We evaluated the 3D proximal humeral contralateral registration (p-HCR) errors, defined as the difference in 3D rotation of the humeral head between the models when superimposed. For comparison, we quantified the landmark-based retrotorsion (LBR) error, defined as the intra-individual difference in retrotorsion, measured with a landmark-based 3D method.Results: The mean 3D p-HCR error using the most proximal humeral shaft (bicipital groove) segment for the registration was 2.8° (standard deviation [SD], 1.5°; range, 0.6° to 7.4°). The mean LBR error of the reference method was 6.4° (SD, 5.9°; range, 0.5° to 24.0°).Conclusions: Bilateral 3D registration of the bicipital groove is a reliable method for approximating the premorbid anatomy of the proximal part of the humerus.Clinical Relevance: The accurate approximation of the premorbid anatomy is a key for the successful restoration of the premorbid anatomy of the proximal part of the humerus. Summary: In the large UK Biobank population-based cohort, we found that amongst men, but not women, prior fragility fracture was associated with increased risk of admission with ischaemic heart disease.Introduction: We aimed to investigate the relationship between prior fracture and risk of incident ischaemic cardiovascular events in a UK population-based cohort.Methods: UK Biobank is a large prospective cohort comprising 502,637 men and women aged 40-69 years, with detailed baseline assessment. History of fracture was self-reported, and details of hospital admissions for ischaemic heart disease (IHD) (ICD-10:I20-I25) were obtained through linkage to UK Hospital Episode Statistics. Cox proportional hazards models were used to investigate the prospective relationships between prior fracture and hospital admission for men and women, controlling for age, BMI, smoking, alcohol, educational level, physical activity, systolic blood pressure, calcium and vitamin D use, ankle spacing-width, heel BUA and HRT use (women).Results: Amongst men, a fragility fracture (hip, spine, wrist or arm fracture resulting from a simple fall) within the previous 5 years was associated with a 35% increased risk of IHD admission (fully adjusted HR 1.35; 95%CI 1.00, 1.82; p = 0.047), with the relationship predominantly driven by wrist fractures. Associations with hospitalisation for angina in men were similar in age-adjusted models [HR1.54; 95%CI: 1.03, 2.30), p = 0.037], but did not remain statistical significant after full adjustment [HR 1.64; 95%CI: 0.88, 3.07); p = 0.121]. HRs for admission with angina were lower in women, and neither age- nor fully adjusted relationships attained statistical significance.Conclusions: Prior fragility fracture is an independent risk factor for incident ischaemic cardiovascular events in men. Further work may clarify whether this association is causal or represents shared risk factors, but these findings are likely to be of value in risk assessment of both osteoporosis and cardiovascular disease. Anterior elbow dislocations in the pediatric population represent rare and sometimes difficult injuries to manage. Associated olecranon fractures are even more uncommon with limited literature existing on the topic. We present the case of a six-year-old male with a traumatic transolecranon anterior elbow fracture dislocation in whom closed reduction was prevented by buttonholing of the proximal ulna through the anterior joint capsule. Although there are numerous reliable multivariate models for sex estimation, many scenarios, both forensic and archaeological, result in the recovery of fragmentary remains, which prevents the collection of various standard measurements. The purpose of this research was to establish metric applications for sex estimation from the distal humerus and distal radius of European/American Whites and American Blacks. Data for this research were retrieved from a sample consisting of a larger database from multiple skeletal collections. Measurements include epicondylar breadth, breadth of the capitulum-trochlea, and distal breadth of the radius. Student’s t-test and descriptive statistics were utilized to analyze the data. Classification rates ranged from 75% to 96.4%, with the capitulum-trochlea breadth performing best. Overall, significant metric differences exist between the sexes in the distal humerus and radius and can be utilized to estimate sex. This study offers a reliable alternative sex estimation technique applicable to fragmentary or damaged remains. [ABSTRACT FROM AUTHOR]

Vitamin D-dependent rickets type II is a rare hereditary disorder. It occurs due to mutations in the gene chr. 12q12-q14, which codes for vitamin D receptor. End-organ resistance to 1,25-(OH)2 vitamin D3 and alopecia in severe cases are the characteristic features. We report a case of a 4-year-old boy with loss of hair over the scalp and body - first observed after 1 month of birth. The boy also developed difficulty in walking at the age of 2 year. On analysis, reduced serum calcium level (7.5 mg/dL) and elevated alkaline phosphatase level (625 IU/L) were reported. Initially, the treatment included intramuscularly administered single dose of 600,000 IU vitamin D, followed by 400 IU of vitamin D along with 1 g of supplemental calcium every day. Periodic follow-up was conducted for 2 months. Improvement was observed in the biochemical parameters and X-rays of the distal radial and ulnar metaphyses, although no improvement was observed in alopecia. [ABSTRACT FROM AUTHOR]

Background: This study investigated the differences and correlations of bone health status, isokinetic muscular strength and power, and body composition of Malay adolescent female silat and taekwondo practitioners. Materials and methods: There were thirty six participants (mean age: 17.31 ± 1.6 years old) with three groups, i.e. sedentary control, silat and taekwondo groups (12 participants per group) in this study. Participant's percent body fat and fat-free mass were measured. Quantitative ultrasound measurement of bone speed of sound (SOS) (an indicator of bone mineral density) of participants' legs and arms were measured. BIODEX isokinetic dynamometer was used to measure participant's knee and shoulder extension and flexion muscular peak torque (strength) and power at 3 angular velocities, i.e. 60°.s-1, 180°.s-1 and 300°.s-1. Results: There were no statistically significant differences in percent body fat and fat free mass among sedentary control, silat and taekwondo groups. Silat group exhibited statistically higher knee extension and shoulder extension peak torque and power values compared to taekwondo group. However, taekwondo group showed statistically significant higher shoulder flexion peak torque and power values than silat group. Silat and taekwondo groups showed statistically significant strong positive correlations of fat free mass with isokinetic muscular peak torque of arms and legs generally. Conclusion: Muscular knee and shoulder extension strength and power are important isokinetic variables in silat. Meanwhile muscular shoulder flexion strength and power are important isokinetic variables in taekwondo. These results can be applied to facilitate the development of training programmes for optimal performance of silat and taekwondo practitioners. [ABSTRACT FROM AUTHOR]

The skeletal study of archaeological material is the only detailed source of demographic information on ancient populations and investigating sex differences is crucial for the reconstruction of the social structure of past societies. Determination of skeletal sex can be achieved using visual (non-metric) or metric methods. Metric methods are considered more appropriate for assessing sex in archaeological skeletal remains since the accuracy of non-metric methods decreases in cases of fragmented bones. Additionally, it is well established that the expression of sexual dimorphism is population specific. Therefore, sex prediction equations should be used only when the sample is known to come from the same population from which the functions were derived. The aim of this study is to test the application of sex prediction equations, which were produced using measurements from the arm bones of a modern Greek population, in ancient Greek skeletal remains. For the purpose of this research five ancient populations were examined; Ancient Corinth, Corfu, Agia Triada Thebes, Edessa and Thebes. According to our results, ulnar sex prediction equations cannot be considered adequate for sex determination of ancient skeletal remains. On the contrary, humeral as well as radial sex prediction equations can be considered adequately reliable for sex determination of ancient skeletal remains. More specifically, sex prediction equations containing the humeral vertical head diameter, its combination with the humeral epicondylar width as well as the maximum radial distal width, achieve a classification accuracy over 72%. Modeling and remodeling are two key determinants of human skeletal growth though little is known about the histomorphometry of cortical bone during ontogeny. In this study, we examined the density and geometric properties of primary and secondary osteons (osteon area and diameter, vascular canal area and diameter) in subperiosteal cortical bone from the human humerus (n = 84) between birth and age 18 years. Sections were removed from the anterior midshaft aspect of humeri from skeletons. Age-at-death was reconstructed using standard osteological techniques. Analyses revealed significant correlation between the histomorphometric variables and age. Higher densities of primary osteons occurred between infancy and 7 years of age but were almost completely replaced by secondary osteons after 14 years of age. The geometry of primary osteons was less clearly related to age. Secondary osteons were visible after 2 years of age and reached their greatest densities in the oldest individuals. Osteon size was positively but weakly influenced by age. Our data imply that modeling and remodeling are age-dependent processes that vary markedly from birth to adulthood in the human humerus. Background: Long bone fractures are common injuries caused by trauma. Some studies have demonstrated that ultrasound has a high sensitivity and specificity in the diagnosis of upper and lower extremity long bone fractures. Objective: The aim of this study was to determine the accuracy of ultrasound compared with plain radiography in diagnosis of upper and lower extremity long bone fractures in traumatic patients. Methods: This cross-sectional study assessed 100 patients admitted to the emergency department of Imam Khomeini Hospital, Ahvaz, Iran with trauma to the upper and lower extremities, from September 2014 through October 2015. In all patients, first ultrasound and then standard plain radiography for the upper and lower limb was performed. Data were analyzed by SPSS version 21 to determine the specificity and sensitivity. Results: The mean age of patients with upper and lower limb trauma were 31.43±12.32 years and 29.63±5.89 years, respectively. Radius fracture was the most frequent compared to other fractures (27%). Sensitivity, specificity, positive predicted value, and negative predicted value of ultrasound compared with plain radiography in the diagnosis of upper extremity long bones were 95.3%, 87.7%, 87.2% and 96.2%, respectively, and the highest accuracy was observed in left arm fractures (100%). Tibia and fibula fractures were the most frequent types compared to other fractures (89.2%). Sensitivity, specificity, PPV and NPV of ultrasound compared with plain radiography in the diagnosis of upper extremity long bone fractures were 98.6%, 83%, 65.4% and 87.1%, respectively, and the highest accuracy was observed in men, lower ages and femoral fractures. Conclusion: The results of this study showed that ultrasound compared with plain radiography has a high accuracy in the diagnosis of upper and lower extremity long bone fractures. This document is a description of the macroscopic functional anatomy of the ulna bony prominences in the Whitefooted Tamarin (Saguinus leucopus), a neotropical endemic, and monotypic primate of Colombia. Few studies have been conducted on its anatomy, and thus its ulnar morphology, an essential element for this animal's quadrupedal arboreal locomotion, remains unknown. This study is based on eight specimens, fixed in formalin at 10 % whose ulnas were extracted in order to describe the main bony prominences along with muscular and ligamentous fixation functions. These elements of the locomotor apparatus and the multiple anatomical contours of the ulna of the White-footed Tamarin, exhibit characteristics for positional and articular adjustment with other bones of the forelimb in harmony with the attachments of the muscles, essential for quadrupedal arboreal locomotion. OBJETIVO: Comparação dos resultados funcionais nos pacientes submetidos à tenotomia com ou sem tenodese da CLB associada ao reparo da lesão do manguito rotador por visão artroscópica, com seguimento superior a dois anos. MÉTODO: Estudo retrospectivo não randomizado, com nível de evidência III, em que foram revisados os prontuários e realizada reavaliação clínica de 77 pacientes com lesão da cabeça longa do bíceps, sendo que 55 foram submetidos à tenotomia sem tenodese e 22 à tenotomia com a tenodese, com seguimento ambulatorial maior que dois anos. Foram avaliados idade, dominância, lado operado, tamanho das lesões classificadas por Gartsman, arco do movimento pré e pós-operatório, presença ou ausência do sinal do Popeye, dor na corredeira bicipital, avaliação segundo a escala University of California at Los Angeles e Elbow Strength Index. RESULTADOS: O UCLA médio total da amostra foi de 16,92 (8 a 25) para 31,45 (13 a 35) (p < 0,001). Comparando a variação do UCLA pré com o pós-operatório entre os dois grupos, nos pacientes submetidos à tenotomia com a tenodese esta variação foi de 15,95 e nos pacientes submetidos somente à tenotomia a variação foi de 14,62 (p = 0,023). No entanto, não houve significância estatística na comparação entre os grupos quanto à dor na corredeira bicipital, sinal do Popeye e Elbow Strength Index. CONCLUSÃO: O estudo apresentou diferença estatística na variação do UCLA. O grupo em que foi realizada a tenotomia com a tenodese da CLB apresentou melhores resultados funcionais.OBJECTIVE: To compare the functional results among patients un dergoing tenotomy with or without tenodesis of the long head of the biceps associated with arthroscopic repair of rotator cuff in juries, with a minimum two-year follow-up. METHOD: This was a retrospective non-randomized trial with evidence level III, in which the medical files of 77 patients with lesions of the long head of the biceps were reviewed and clinically reassessed. Among these, 55 patients underwent tenotomy without tenodesis and 22 underwent tenotomy with tenodesis, with outpatient follow-up for at least two years. The age, dominant side, operated side, lesion size using the Gartsman classification, pre and postoperative range of mo tion, presence or absence of the Popeye sign, pain in the bicipital groove and assessments using the University of California at Los Angeles (UCLA) score and the elbow strength index. RESULTS: The mean UCLA score of the sample went from 16.92 (range: 8 to 25) to 31.45 (range: 13 to 35) (p < 0.001). Comparison of the pre and postoperative UCLA scores in the two groups showed that the dif ference in the group with tenotomy and tenodesis was 15.95 and in the group with tenotomy alone, 14.62 (p = 0.023). However, there was no statistical significance in comparing the groups regarding pain in the bicipital groove, Popeye sign or elbow strength index. CONCLUSION: This study showed that the difference in UCLA scores was statistically significant. Background: It is not clear whether the anterior capsule should be repaired to the coracoid process or to the native glenoid during the modified Latarjet procedure. We investigated joint stability and range of motion of the shoulder after the modified Latarjet procedure with both of these methods of capsular repair.Methods: Eighteen fresh-frozen cadaveric shoulders were used. After a Bankart lesion and 6-mm glenoid defect were created, the coracoid process was transferred to the glenoid and fixed with screws. The anterior capsule was repaired either to the coracoid process (coracoid group) or to the native glenoid (glenoid group). The ranges of internal and external axial rotation were measured with the arm at 0° and 60° of glenohumeral abduction. The range of motion was measured with a constant torque of 200 N-mm. Joint stability was measured using a custom stability testing device. The stability ratio in the anterior-posterior direction was measured with the arm at maximal external rotation and neutral rotation.Results: The range of external rotation was greater at both 0° and 60° of abduction in the coracoid group compared with the glenoid group (p < 0.05). The range of internal rotation was not significantly different between groups. The end-range stability ratio was not significantly different between groups, but the mid-range stability ratio was significantly greater in the glenoid group.Conclusions: Because the difference in the mid-range stability may not be clinically relevant, we recommend repairing the capsule to the coracoid, as that preserves the range of motion in external rotation.Clinical Relevance: Repairing the capsule to the transferred coracoid during the modified Latarjet procedure appears to be beneficial to avoid the limited range of motion in external rotation, but the direct contact of the humeral head and the transferred coracoid might confer a risk of osteoarthritis. Unlabelled: In this cross-sectional study the psychometric properties are examined of the adapted Dutch translation of the Short Musculoskeletal Function Assessment (SMFA) questionnaire in severely injured patients (ISS>15).Patients and Methods: Patients (N=173) completed the SMFA, the World Health Organization Quality of Life assessment instrument-BREF (WHOQOL-BREF), the Dutch Impact of Event Scale (IES), the Hospital Anxiety and Depression Scale (HADS) and the Cognitive Failure Questionnaire (CFQ). The Abbreviated Injury Score and the Injury Severity Score were established to determine the injured body area and the severity of the injuries. Exploratory factor analysis (method: PAF) was performed. Correlations were calculated between our SMFA factors and scores on the WHOQOL-BREF, IES, HADS and CFQ. The SMFA scores of the factors Upper extremity dysfunction and Lower extremity dysfunction were compared between subgroups of patients with and without injuries in respectively the upper extremities and the lower extremities. For responsiveness analysis, data were compared with the baseline SMFA measurement of a reference group.Results: A three-factor structure was found: Lower extremity dysfunction, Upper extremity dysfunction, and Emotion. Strong correlations between the SMFA and the other questionnaires were found. Patients with injury of the lower extremities had significantly higher scores on the factor Lower extremity dysfunction than patients without injury of the lower extremities (p=0.017). In none of the factors, a significant difference in mean scores was found between patients with and without injury of the upper extremities. Severely injured patients had significantly higher SMFA scores than the reference group (p<0.001).Conclusion: The adapted Dutch translation of the SMFA showed good psychometric properties in severely injured patients. Bones of forelimb were studied from a prepared skeleton of an adult female Asian elephant (Elephas maximus) in Anatomy Museum of Chittagong Veterinary and Animal Sciences University to understand the morphological form and structure of Asian elephant forelimb. The angle was approximately 123° between caudal border of scapula and caudal border of humerus. The scapula, humerus and bones of the antebrachium (particularly the ulna) were massive bones. The bones of manus were the short and relatively small. The dorsal border of scapula extended from the level of proximal extremity of first rib to the middle of the 6th rib. Ventral angle of scapula articulated with humerus by elongated shaped glenoid cavity (cavitas glenoidalis) of scapula and head of humerus (caput humeri). The major tubercle (tuberculum majus) of humerus was situated laterally to the head, which had smaller cranial part with large caudal part and extended cranially to the head. The crest of minor tubercle (tuberculum minus) was present as the rough line on the mediocaudal surface of humerus that ends in a slight depressed or elevated area, known as teres major tuberosity (tuberositas teres major). The lateral supracondyler crest (crista supra condylaris lateralis) at the caudal surface of the shaft limit the musculo-spiral groove in body of humerus. The radius and ulna are twin bones of forearm and the attachment between ulna and radius occurs in such a way, the radius articulates craniomedially with the ulna in the proximal part. But the shaft spirals laterally over the cranial surface of the ulna to articulate distally with the medial aspect of the ulna. There were 8 carpal bones, 5 metacarpal bones and 5 digits. The comparative size of the proximal and distal raw of carpal bones were ulnar carpal > radial > intermediate > accessory carpal and IV > III > II > I respectively. Axillary nerve injury is the most common complication in humerus fracture. The symptoms that are caused by affecting axillary nerve vary according to the structures involved such as sensory disturbance and weakness of muscles, e.g., three parts of deltoid and teres muscles in an affected limb. The severity of injury is classified in demyelinating and axonal lesions, which usually occurs at the onset of fracture. Purpose: Radiographs are usually taken on day of pin removal for children treated with closed reduction and percutaneous pinning (CRPP) of type 2 supracondylar humerus fractures. The purpose of this study was to determine whether radiographs taken at time of pin removal for patients recovering uneventfully alter management. Methods: After IRB approval, billing records identified 1213 patients aged 1-10 years who underwent elbow surgery between 2007 and 2013 at our institution for a supracondylar humerus fracture. Of these patients, 389 met inclusion criteria. Clinical charts were reviewed for demographics, operative details, and clinical follow-up, focusing on clinical symptoms present at pin removal. Radiographs taken at time of pin removal and subsequent visits were assessed for healing and fracture alignment. Results: In no case was pin removal delayed based on radiographs. One hundred and nineteen (31 %) patients had radiographs taken following pin removal; in no case was loss of reduction found among these patients. No cases of neurologic or vascular injury, re-fracture, or loss of reduction occurred. Infection occurred in 12 patients (3 %). Pins were kept in place for 23.8 ± 4.4 days. Eighty-six patients (22 %) had additional intervention after pin removal (cast application in all cases). Of 389 patients, 75 (19 %) had no documented reason for extended casting, four (1 %) were extended based on physician evaluation of radiographs, and seven (2 %) were extended for other reasons. Conclusions: Elimination of radiographs at time of pin removal should be considered. If continuing to obtain radiographs at pin removal, we recommend removing pins before taking radiographs to reduce patient fear and anxiety from visualizing percutaneous pins. [ABSTRACT FROM AUTHOR]

Purpose: This paper analyzed outcomes of the osteosynthesis with a locking plate system for the fractures of the humerus in throwers using the anterior humeral approach.Methods: Retrospective case series including 31 patients. Bone union was assessed through follow-up radiographs. Results of visual analogue scale (VAS) for pain, range of motion in the elbow joint, time of return to work, and the Mayo Elbow Performance Score (MEPS) were evaluated to determine functional outcomes. Direction and length of the fracture, the distal cortical length, the humeral diameter, and the total humeral length were measured as part of fracture configuration analysis.Results: Mean patient age was 25.8 (range, 18-34) years. The follow-up average was 16.0 months (range, 12-23). Delayed union was observed in one (3.1 %) patient. Mean final VAS was 0.4 (range, 0-2), mean time of return to work was 18.2 weeks (range, 13-36), and mean MEPS was 96.3 (range, 88-100) points. All fractures showed a spiral configuration. Mean fracture length was 79.7 (95 % CI, 72.6-86.7) mm, and mean distal cortical length was 48.3 (95 % CI, 37.8-58.8) mm.Conclusions: The results of the current study indicates that plate osteosynthesis using a locking plate system combined with interfragmentary lag screws through anterior humeral approach may be a favorable option for the surgical treatment of humeral shaft fractures in throwers. Background: Repetitive valgus stress applied during a throwing motion can lead to various elbow disturbances, including ulnar collateral ligament (UCL) injury. Subchondral bone density reportedly reflects the cumulative force on a joint surface under actual loading conditions. Purpose: (1) To evaluate the distribution of subchondral bone density across the elbow joint in asymptomatic baseball pitchers and symptomatic valgus instability pitchers and (2) to clarify the alterations in stress distribution pattern associated with symptomatic UCL insufficiency pitching activities. Study Design: Controlled laboratory study. Methods: Computed tomography (CT) imaging data were collected from the dominant-side elbow of 7 nonathletic volunteers (controls), 12 asymptomatic pitchers (asymptomatic group), and 12 symptomatic valgus instability pitchers with UCL insufficiency (symptomatic group). Bone mineral density across the elbow joint was measured with CT osteoabsorptiometry. A 2-dimensional mapping model was divided into 4 areas of the distal end of the humerus and 5 areas of the ulna with the radial head. The locations and percentages of high-density areas on the articular surface were quantitatively analyzed. Results: High-density areas in the asymptomatic and symptomatic groups were found in the anterolateral and posteromedial parts of the humerus and in the radial head, posteromedial to the ulna. The high-density areas in the anterior and posteromedial of the humerus, the radial head, and the posteromedial part of the ulna in the controls were smaller than those in the baseball group. In the symptomatic group, the percentages of high-density areas in the anterolateral part of the humerus (mean, 36.3%; 95% CI, 31.9%-40.7%) and the anterolateral part of the ulna (mean, 31.7%; 95% CI, 24.3%-39.1) were significantly greater than those in the asymptomatic group (P = .047 and P < .0001, respectively). Conclusion: Symptomatic UCL insufficiency was associated with characteristic high-stress distribution patterns on the anterolateral part of the capitellum and the anterolateral part of the ulna. The current results indicate that symptomatic UCL insufficiency produces excessive and cumulative stress in the elbow joint. An accurate and precise estimate of stature can be very useful in the analysis of human remains in forensic cases. A problem with many stature estimation methods is that an unknown individual must first be assigned to a specific group before a method can be applied. Group membership has been defined by sex, age, year of birth, race, ancestry, continental origin, nationality or a combination of these criteria. Univariate and multivariate sex-specific and generic equations are presented here that do not require an unknown individual to be assigned to a group before stature is estimated. The equations were developed using linear regression with a sample (n=244) from the Terry Collection and tested using independent samples from the Forensic Anthropology Databank (n=136) and the Lisbon Collection (n=85). Tests with these independent samples show that (1) the femur provides the best univariate results; (2) the best multivariate equation includes the humerus, femur and tibia lengths; (3) a generic equation that does not require an unknown to first be assigned to a given category provides the best results most often; (4) a population-specific equation does not provide better results for estimating stature; (5) sex-specific equations can provide slightly better results in some cases; however, estimating the wrong sex can have a negative impact on precision and accuracy. To our knowledge, no prior report focused on the risk factors for proximal humerus fractures in patients with rheumatoid arthritis. The purpose of this study was to evaluate the association between potential risk factors and the occurrence of proximal humerus fractures in patients with rheumatoid arthritis. A total of 11,907 patients with rheumatoid arthritis were enrolled in our observational cohort rheumatoid arthritis study between 2000 and 2012. Self-reported proximal humerus fractures were verified using the patients' medical records. Cox proportional hazard models were used to analyze the independent contribution of risk factors to the occurrence of proximal humerus fractures. During follow-up (mean 5.6 years), 92 proximal humerus fractures were verified in 91 patients. Multivariate Cox regression analyses estimated that the hazard ratios of sustaining a proximal humerus fracture were 1.37 for every 10-year increase in age [95 % confidence interval (CI) 1.10-1.70; P < 0.01], 1.95 for increases in serum C-reactive protein levels (mg/100 mL; 95 % CI 1.15-3.34; P < 0.05), 2.13 for a history of fractures (95 % CI 1.34-3.40; P < 0.01), 1.07 for the daily prednisolone dose (per mg; 95 % CI 1.01-1.13; P < 0.05), and 1.97 for oral bisphosphonate use (95 % CI 1.20-3.23; P < 0.01). Better control of rheumatoid arthritis with a smaller daily prednisolone dose in elderly patients with a history of fractures may be important for preventing proximal humerus fractures. Purpose: We report a very rare case of a fast-growing benign fibrous histiocytoma at the temporal bone. Methods: A 1-year-old girl was referred for investigation of a right temporal mass that increased during 2-week observation. Imaging studies showed a lytic, loculated skull tumor at the left temporal bone. On magnetic resonance imaging scans, the tumor was isointense on T1- and relatively high-intense on T2-weighted images. The tumor was mostly homogeneously enhanced by gadolinium. On diffusion-weighted images, it was iso- to relatively low-intense; perfusion-weighted images revealed low perfusion. Results: Complete macroscopic resection of the tumor was performed. Immunohistochemical analysis showed that the tumor was positive for CD68 and α-smooth muscle actin and negative for CD1a and CD34; the MIB-1 labeling index was 4 %. A diagnosis of primary benign fibrous histiocytoma of the skull was made. At 6-month follow-up, there were no clinical or radiological signs of tumor recurrence and/or metastasis. Background: Autograft from iliac crest is considered as gold standard for augmentation of bone healing in delayed and nonunion of fractures. Demineralized bone grafts have been used to stimulate fracture union in experimental studies. Bone demineralized with 0.6N hydrochloric acid has shown to retain its osteoinductive capacity. We report the outcome of partially decalcified bone allograft (decal bone) in the treatment of delayed union and atrophic nonunions of bones. Materials and Methods: Twenty patients with clinicoradiological diagnosis of delayed union or atrophic nonunion of long bone fractures were included in this retrospective study. Patients at extreme of ages (<18 years and >60 years), pathological fractures, metabolic bone diseases, infected nonunion, hypertrophic nonunion and those having systemic illness like diabetes mellitus and on drugs that impair fracture healing were excluded from the study. Decal bone was prepared in the bone bank and maintained in Department of Orthopedics. Allografting was done in 20 patients of delayed union (9/20) and atrophic nonunion (11/20) of long bone fractures with mean age of 34 years (range 18-55 years). The bones involved were humerus (8/20), tibia (7/20) and femur (5/20). Fourteen patients underwent treatment in the form of internal fixation and allografting and six patients were operated with osteoperiosteal allografting. Results: Nineteen patients achieved union in mean time of 14.9 weeks range (range 8-20 weeks). Eight patients had serous discharge from the operative site that subsided in 11 days (range 4-21 days). One patient had pus discharge that required repeat debridement and antibiotics for 6 weeks. The fracture healed in 16 weeks. Conclusion: The partially decalcified bone allograft is an effective modality for augmentation of bone healing without complication associated with autograft like donor site morbidity, increased blood loss and increase in the surgical time. OBJECTIVES: To evaluate and compare the functional outcome in humeral shaft fractures after treatment with U cast and functional brace. STUDY DESIGN: A Randomized Controlled Trial PLACE AND DURATION: The study was conducted over a period of 6 months in Benazir Bhutto Hospital Rawalpindi, Orthopaedics Department from 1st Nov 2012 to 31st May 2013. METHODOLOGY: 280 patients of either gender from 20-60 years of age with closed Humeral Shaft Fractures were included in the study. All patients were treated in OPD by trainee researcher under supervision. Patients were divided in 2 groups on the basis of lottery method. In group A functional brace was applied and in group B conventional U-cast was applied. Functional outcome was assessed in terms of subjective evaluation by using Hunter's criteria at 06 weeks by the researcher trainee. RESULTS: A total of 280 Patients were included in both groups. In Group A Hunter's score was G-1 in 4 patients (1.4%), G-2 in 6 patients (2.1%), G-3 in 7 patients (2.5%), G-4 in 39 patients (13.9%) and G-5 in 84 patients (30%). In Group B Hunter's score was G-1 in 8 patients (2.9%), G-2 in 14 patients (5%), G-3 in 41 patients 14.6(%), G-4 in 45 patients (16.1%) and G-5 in 32 patients (11.4%). CONCLUSION: Functional Outcome after the treatment of Humerus in patients treated with Humerus brace is better than functional outcome in patients treated with conventional U cast. Background: An opening into the bone shaft for passage of blood vessels to the medullary cavity of a bone for its nourishment and growth is called as nutrient foramen. There is always a need for a greater understanding of nutrient foramina in upper limb bones such as the humerus, radius and ulna. So the aim of present study was to record the location, number and direction of nutrient foramina m long bones of the upper limb. Material and methods: A total number of 120 long bones (40 humeri, 40 radii and 40 ulnae) were used for the study. Results: In the results, 66% of the humeri had a single foramen, 18% had double foramina and 26% had no foramen. For the radii, 68% had a single nutrient foramen and 32% had no nutrient foramen. 78% of the ulnae ad a single nutrient Had no nutrient foramen. All the foramina except one (in the radius) were directed away from the growing end, that is, they were directed towards the elbow. In the present study, the case of absence of the foramen supratrochleare in humerus bones which were in both legs of 10-year-old German Shepherd dog was identified. Morphometric measurements were taken with digital callipers on the bones which were duly carried out a maceration. While calcification areas were not seen in the proximal and corpus parts of right and left humerus; a large calcification of 9,17 mm width and 17,75 mm length was determined in length on the edge of fossa olecrani with the caudal edge of epicondylus lateralis of the right humerus, especially in the distal part. Also, a large calcification area of 5,51 mm width and 14,69 mm length which was overflowed towards the lateral was determined just above the epicondylus medialis of the same humerus. In addition, a wide range of small calcification areas were determined inside the fossa radialis and fossa olecrani. In left humerus, only a few small calcification areas were observed in fossa radialis. In addition to this, these calcification areas may be formed depending on the age of animal. Background: Fixation failure is a relatively common sequela of surgical management of proximal humerus fractures (PHF). The purpose of this study is to understand the current state of the literature with regard to the biomechanical testing of proximal humerus fracture implants. Methods: A scoping review of the proximal humerus fracture literature was performed, and studies testing the mechanical properties of a PHF treatment were included in this review. Descriptive statistics were used to summarize the characteristics and methods of the included studies. Results: 1,051 proximal humerus fracture studies were reviewed; 67 studies met our inclusion criteria. The most common specimen used was cadaver bone (87 %), followed by sawbones (7 %) and animal bones (4 %). A twopart fracture pattern was tested most frequently (68 %), followed by three-part (23 %), and four-part (8 %). Implants tested included locking plates (52 %), intramedullary devices (25 %), and non-locking plates (25 %). Hemiarthroplasty was tested in 5 studies (7 %), with no studies using reverse total shoulder arthroplasty (RTSA) implants. Torque was the most common mode of force applied (51 %), followed by axial loading (45 %), and cantilever bending (34 %). Substantial testing diversity was observed across all studies. Conclusions: The biomechanical literature was found to be both diverse and heterogeneous. More complex fracture patterns and RTSA implants have not been adequately tested. These gaps in the current literature will need to be addressed to ensure that future biomechanical research is clinically relevant and capable of improving the outcomes of challenging proximal humerus fracture patterns. Objective: To describe the results of surgical treatment of post-traumatic segmental bone defects (PSBD) through the induction membrane technique. Methods: Observational study in patients attended at Hospital Nacional Cayetano Heredia in Lima from January 2009 to June 2014, in whom a two-step reconstruction procedure for PSBD was performed using the induction membrane technique. In the first step, the formation of a membrane was induced with an antibiotic spacer block followed by replacement of the spacer with bone grafting. Time to bone consolidation, degree of shortening of the extremity, functional status and complications were recorded. Results: Twenty patients were included in the study (15 males and 5 females); mean age was 29 year (range: 13-54). Eleven cases had PSBD in the tibia, 7 in the femur, 1 in the radius and 1 in the ulna; in 90% of cases the PSBD had a length of at least 50mm (mean of 73 mm); 60% of cases had a positive culture at the beginning of treatment. Background: Proximal humerus fractures are a common fragility fracture that significantly affects the independence of older adults. The outcomes of these fractures are frequently disappointing and previous systematic reviews are unable to guide clinical practice. Through an integrated knowledge user collaboration, we sought to map the breadth of literature available to guide the management of proximal humerus fractures. Methods: We utilized a scoping review technique because of its novel ability to map research activity and identify knowledge gaps in fields with diverse treatments. Through multiple electronic database searches, we identified a comprehensive body of proximal humerus fracture literature that was classified into eight research themes. Meta-data from each study were abstracted and descriptive statistics were used to summarize the results. Results: 1,051 studies met our inclusion criteria with the majority of research being performed in Europe (64%). The included literature consists primarily of surgical treatment studies (67%) and biomechanical fracture models (10%). Nearly half of all clinical studies are uncontrolled case series of a single treatment (48%). Non-randomized comparative studies represented 12% of the literature and only 3% of the studies were randomized controlled trials. Finally, studies with a primary outcome examining the effectiveness of non-operative treatment or using a prognostic study design were also uncommon (4% and 6%, respectively). Conclusions: The current study provides a comprehensive summary of the existing proximal humerus fracture literature using a thematic framework developed by a multi-disciplinary collaboration. Iatrogenic injury during the posterior approach to the humerus during operative fixation is not an uncommon occurrence. A comprehensive understanding of the normal anatomy and its variants is of paramount importance in order to avoid such injury. Typically, the inferior lateral cutaneous branch of the radial nerve originates towards the distal end of the humerus at the inferior portion of the spiral groove. Here, we report an important variant of this nerve, which originated significantly more proximal than expected, further emphasizing the importance of identification, dissection and protection of the radial nerve and its major branches. Purpose: The purpose of the study is to identify the safe zone in which the surgeon can place the screws for fixation of the coracoid graft during the Latarjet procedure to avoid injuries of the suprascapular nerve with the shoulder in internal and external rotation. Methods: The dissection on twelve fresh-frozen shoulders was performed according to a standard posterior approach to the gleno-humeral joint. The suprascapular nerve and its branches for the infraspinatus muscle were identified at the spinoglenoid notch region. Then, the distance between the glenoid and the suprascapular nerve at the spinoglenoid notch region was measured by using a ruler with the shoulder at 90° internal rotation and at 90° of external rotation. Results: The median distance between the glenoid and the suprascapular nerve was 12 mm (range 6-15 mm) with the shoulder at 90° of internal rotation and 19 mm (range 11-23 mm) with the shoulder at 90° of external rotation. The distance between the glenoid and the suprascapular nerve was statistically significantly greater with the shoulder at 90° of external rotation than 90° of internal rotation ( P = 0.002). Conclusions: The suprascapular nerve is furthest away from the glenoid with the shoulder at 90° of external rotation. Therefore, the placement of screws in external rotation of the humerus during glenoid bone block procedures likely reduces the risk of iatrogenic injuries of the suprascapular nerve. Background: Elbow injuries are common lesions with an incidence of approximately 8 % of all injuries. In childhood, the elbow region tends to show high complication rates and it is noteworthy that the majority of complications are of iatrogenic origin. Treatment of fractures of the growing elbow requires knowledge of the characteristic features. Attributable to the low growing potential of the distal humerus in children older than 7 years, malposition in the sagittal plane should not be acceptable. Therapy: The fracture treatment has to be adapted to age and fracture dislocation. Supracondylar fractures should be treated according to the type of fracture and malpositions should be corrected in all three planes. In fractures of the lateral condyle, a differentiation between types I and II is crucial. Fractures of the radial neck have a good prognosis if treated by repositioning and subsequent stabilization with intramedullary wire.
