4. A diversity of patterns: 10-year trajectories of men and women diagnosed with psychosis for the first time. A time-geographic approach

Abstract

People with severe mental illness face a different ‘interventional’ landscape compared to some decades ago, when mental hospitals were dominant, in Sweden as well as in the rest of the Western world. The aim of the research reported in this article was to follow men and women diagnosed with psychosis for the first time over a 10-year period, and to explore what interventions they experienced. The interventions, here defined as “spheres”, were either community-based or institutional. A third sphere represents no interventions. Based on data from registers and using a time-geographic approach, the individuals were visualised as 10- year trajectories where their transitions between the different spheres were highlighted. The results show a great diversity of trajectories. Two main categories were detected: two-spheres (community-based and no interventions) and three-spheres (adding institutional interventions). One third of the population experienced only community-based interventions, with a higher proportion of men than women. Consequently, more women had institutional experience. Two sub-categories reveal trajectories not being in the interventional sphere in a stepwise manner before the 10th year, and long-term trajectories with interventions in the 10th year. The most common pattern was long-term trajectories, embracing about half of the population, while one-fifth left the institutional sphere before the 5th year.

Abstract

Hur ser socialtjänstens förmåga ut när det gäller att stödja barn med funktionsnedsättning som lever i socialt utsatta familjer? Den här studien handlar om samverkan mellan Individ- och familjeomsorgens barn- och ungdomsenheter och enheter för personer med funktionsnedsättning. En fungerande samverkan av olika samhälleliga aktörer runt barn med funktionsnedsättning i socialt utsatta familjer kan ses som extra viktig. Dels för att det handlar om barn, dels för att det berör barn med funktionsnedsättning. Barn med funktionsnedsättning som också lever i socialt utsatta familjer utgör en mycket svag grupp med begränsningar att föra sin egen talan. Forskning visar dessutom att barn med funktionsnedsättning i högre grad än andra barn riskerar att utsättas för våld och övergrepp. Tyvärr visar forskningen också att många professionella saknar kunskap om barn med funktionsnedsättningars speciella behov och ökade utsatthet för våld och andra övergrepp. Utifrån fiktiva fall avseende två barn med funktionsnedsättningar som lever i familjer med social problematik har socialsekreterare och LSS-handläggare i åtta kommuner diskuterat hinder och förutsättningar för samverkan. Får barnen i dessa vinjetter den hjälp och det stöd de behöver? Var hamnar barnen i utredningsprocesserna? Rapporten ringar in ett antal utvecklingsområden och dilemman. I rapporten framkommer också några lösningar och goda exempel. Rapporten avslutas med ett arbetsmaterial med korta sammanfattningar och diskussionsfrågor som kan användas i personalgrupper för att utveckla den egna verksamheten.

Abstract

Sexual violence has become endemic in the Democratic Republic of Congo (DRC), but the perspectives of rural communities of the scourge remain poorly researched. This study aims to describe the attitudes and knowledge of rural communities in regard to sexual violence, its occurrence and associated problems in rural communities in the Itebero/Walikale district in the DRC. A descriptive cross-sectional design was adopted, and a structured questionnaire used. Four hundred respondents participated, representing a group of ten villages populated by a total of 10,000 inhabitants. The respondents stated that perpetrators were often men from their own village. The fields were cited as being the place where most of the assaults occurred. A substantial proportion of the respondents lacked sufficient knowledge of the health outcomes of sexual violence. HIV infection and unwanted pregnancies were the most feared consequences. The victims of violence either experienced compassion or suffered rejection, depending on the community groups. Victims were mostly supported by women from their community, followed by husbands, relatives and authorities. Health facilities were the primary sources of support for victims. In the wake of deinstitutionalization, people with psychiatric disabilities are, to a great extent, living in the community. In this thesis everyday life of people with psychiatric disabilities, living in independent housing with community-based social support to manage their daily life, is investigated. Special attention is paid to the characteristics and meaning of community-based social support. Through participating observational studies, interviews and time-geographic diaries, data have been gathered about everyday life of seventeen men and women. It is the interpersonal social conditions that have been the main target of the study, addressed as social networks and social support. Everyday life conditions are dependent on social as well as physical and material circumstances. Although the social conditions are emphasized in the study they are not looked upon as isolated dimensions but as part of everyday life circumstances. The everyday life perspective offers an opportunity to reveal the structures within which everyday life takes place. The time-geographic concepts “community-organized projects” and “individual-organized projects” were used to investigate the structures of everyday life, resulting in four substructures. Four types of everyday life and four patterns of networks connected to the different substructures were identified, showing great variations. The characteristics of community-based social support can be summarized as “the doing”, “the talking” and “the being with” in a reciprocal type of relationship. The meaning of support is shortly described as solution of and relieving problems, social companionship, security, protection and control. Community-based social support show similar importance regardless of the type of everyday life when it comes to solution of problems and different importance when it comes to aspects of support such as social companionship.

Lökflugan (Delia antiqua) kan lokalt vara en svår skadegörare på gul lök, men också på rödlök, silverlök, salladslök, schalottenlök, purjolök och gräslök. Särskilt känslig är den sådda löken, vars tidiga utvecklingsstadier sammanfaller med äggläggningen hos lökflugan. Lökflugan kan i vissa länder förekomma i flera generationer menhos oss är detvanligastmed en, kanske två generationer, varav den första gör störst skada.

Abstract

ABSTRACT:In recent decades the possibility that people diagnosed with severe mental illness might live a satisfying life outside psychiatric institutions and even independently of psychiatric services has won some acceptance. During the same period the concept of recovery has been given different, contradictory, and/or complementary definitions. Often users, staff, and scientists have taken a stand for a specific definition, clinical, or personal recovery, but do not problematize the other main concept: What is the person in recovery from? In this article we review the different definitions of recovery and the diagnostic categories, and articulate their pros and cons. We propose a critical viewpoint by arguing for transparency and critical reflexivity, putting one's main concepts and choices under scrutiny. This should not be seen as an obstacle, but rather as an obvious part of scientific research, as such concepts and choices will influence the results of the research.

Abstract

People with severe mental illness (SMI) are often described as lonely and socially incapable – an inability resulting from the mental illness. The aim of this article is to explore experiences of being alone among persons with SMI. The article is based on interviews with 19 persons diagnosed with psychosis who were interviewed between four and nine times over a period of three years. The findings show that experiences of being alone can be identified bytwoconcepts: aloneness and loneliness. The persons in the study appeared as socially able and active in relation to their social lives. However, a social agent does not operate in a void but in interaction with specific living conditions; the experiences of aloneness and loneliness may be viewed as the result of the interplay between the individual and the social and material environment.
