Neurodegenerative diseases can occur so early as to affect neurodevelopment. From a cohort of more than 2,000 consanguineous families with childhood neurological disease, we identified a founder mutation in four independent pedigrees in cleavage and polyadenylation factor I subunit 1 (CLP1). CLP1 is a multifunctional kinase implicated in tRNA, mRNA, and siRNA maturation. Kinase activity of the CLP1 mutant protein was defective, and the tRNA endonuclease complex (TSEN) was destabilized, resulting in impaired pre-tRNA cleavage. Germline clp1 null zebrafish showed cerebellar neurodegeneration that was rescued by wild-type, but not mutant, human CLP1 expression. Patient-derived induced neurons displayed both depletion of mature tRNAs and accumulation of unspliced pre-tRNAs. Transfection of partially processed tRNA fragments into patient cells exacerbated an oxidative stress-induced reduction in cell survival. Our data link tRNA maturation to neuronal development and neurodegeneration through defective CLP1 function in humans.

Objective: The World Health Organization labelled the COVID-19 outbreak a pandemic on March 11, 2020, the first day COVID-19 cases were diagnosed in Turkey. Since then, people's main concerns have been whether their lives or the lives of their relatives would be at risk if they became infected. The objective of this study was to determine the risk factors that increase mortality in COVID-19 patients treated in ICUs and to facilitate the appropriate precautions. Methods: In this study, patients who were diagnosed with COVID-19 [positive on a polymerase chain reaction (PCR) test] and monitored in the Pandemic Hospital at Diyarbakır Dicle University between 15 March 15 and 15 May, 2020, were evaluated for risk factors for mortality. Results: A total of 50 ICU patients were included in the study. Of them, 52% were males, and the mean age was 63.8±17.6 years. A comparison of deceased and surviving patients showed that being female, older than 62, and a smoker and having diabetes mellitus, hypertension, and/or coronary artery disease significantly increased mortality. An investigation of the findings from the patients' lung tomography examinations revealed that relatively prominent pulmonary involvement and localization did not affect mortality. Conclusions: Because patients who are advanced in age, female and smokers and have comorbidities are in the high-risk group, measurements related to treating the COVID-19 disease should be improved. Laboratory parameters are useful for the evaluation of mortality and morbidity, while mechanical ventilation increased mortality. Giriş: Dünya Sağlık Örgütü tarafından COVID-19 hastalığı pandemi olarak ilan edildiği 11 Mart 2020 tarihinde Türkiye'de de ilk vakalar görülmeye başlandı. O tarihten beri insanlarda paniğe neden olan düşüncelerin başında hastalığa yakalanmasıyla beraber kendi ve yakınlarının hayatının tehlikeye girip girmeyeceği oldu. Bu çalışmanın amacı COVID-19 hastalığına bağlı yoğun bakımda takip edilen hastalarda mortaliteyi arttıran risk faktörlerin tespit edilip bu yönde önlem alınmasını sağlamaktır Yöntemler: Bu çalışmada, Diyarbakır Dicle Üniversitesi Pandemi Hastanesine 15 Mart 2020 ve 15 Mayıs 2020 tarihleri arasında COVID-19 Yoğun bakımlarında yatan ve polymerase chain reaction (PCR) ile COVID-19 tanısı konan yoğun bakım olguları değerlendirildi. Sonuçlar: Çalışmaya alınan 50 yoğun bakım hastasının %54'ü erkekti. Hastaların yaş ortalaması 63.8 ± 17.6 yıl olarak hesaplandı. Ölen hastalarda sağ kalanlara göre kadın cinsiyet, 62 yaş üzeri hastalar, sigara kullanımı, diyabetes mellitus, hipertansiyon ve koroner arter hastalığının varlığının mortaliteyi istatistiksel olarak arttırdığı bulundu. Tomografi bulguları karşılaştırıldığında, akçiğer tutulumun fazla olması ile mortalite arasında doğru orantı olmadığı bulundu. Tartışma: İleri yaş, kadın cinsiyet, sigara kullanımı ve komorbid hastalıkları olanlar yüksek risk grubunda olduğundan dolayı COVID-19 hastalığından korunmak için önlemleri arttırmak gerekiyor. Akciğer tomografi bulguları tanısal olarak anlamlı olsa da hastalığın sürveyansında anlamlı olmayabilir. Laboratuvar bulguları mortalite ve morbidite üzerinde etkili parametreler olup, olup mekanik ventilatöre bağlananlarda mortalite oranı yüksek olarak rapor edildi.

Background and Aim: In chronic hepatitis B infection, antiviral therapy significantly reduces the incidence of complications. This study aimed to present real-life 12-month effectiveness and safety data for TAF. Materials and Methods: This Pythagoras Retrospective Cohort Study included patients from 14 centers in Turkiye. The study presents 12-month results of 480 patients treated with TAF as initial therapy or after switching from another antiviral drug. Results: The study shows treatment of about 78.1% patients with at least one antiviral agent (90.6% tenofovir disoproxil [TDF]). The rate of undetectable HBV DNA increased in both treatment-experienced and naive patients. In TDF-experienced patients, the rate of alanine transaminase (ALT) normalization increased slightly (1.6%) within 12 months, but the change was not statistically significant (p=0.766). Younger age, low albumin, and high body mass index and cholesterol were identified as risk factors for abnormal ALT after 12 months, but no linear relationship was detected. In TDF-experienced patients, renal and bone function indicators showed significant improvement three months after the transition to TAF and remained stable for 12 months. Conclusion: Real-life data demonstrated effective virological and biochemical responses with TAF therapy. After switching to TAF treatment, gains in kidney and bone functions were achieved in the early period. [ABSTRACT FROM AUTHOR]

11. Clinical features and prognosis with Guillain-Barré syndrome

Background: Guillain-Barré syndrome (GBS) is an acute inflammatory polyneuropathy commonly characterized by rapidly progressive, symmetric weakness and areflexia. Materials and Methods: We retrospectively assessed the clinical manifestations, results of electrodiagnostic tests, functional status and prognosis of 36 children diagnosed with GBS. Results: Based on clinical and electrophysiological findings, the patients were classified as having acute inflammatory demyelinating polyradiculoneuropathy (AIDP) (n = 25), acute motor axonal neuropathy (AMAN) (n = 10) and acute motor-sensory axonal neuropathy (AMSAN) (n = 1). Twenty (55.5%) patients were males and 16 (44.5%) patients were females. The mean age of the 36 patients was 68.1 ± 45.01 months (range, 6-180 months). Five (13.8%) patients were younger than 2 years. The most common initial symptoms were limb weakness, which was documented in 34 (94.4%) patients. In our study, 18 patients (51.4%) showed albuminocytological dissociation (raised protein concentration without pleocytosis) on cerebrospinal fluid (CSF) examination. Three patients (8.3%) required mechanical ventilation therapy during hospitalization. Unfortunately, three (8.3%) patients died; one patient had AIDP and two patients had axonal involvement (one case was AMAN and another case was AMSAN). When we compared the cases of residual sequel/dead and cases of complete recovery for neural involvement type including AIDP, AMAN and AMSAN, we did not find a statistically significant difference between the groups (P > 0.05). Conclusion: Our findings showed that cases of GBS was not uncommon in children younger than 2 years of age, and CSF protein level might be found high in the first week of the disease in about one half of the patients, with a higher rate of morbidity and mortality in patients with axonal involvement than in those with AIDP.

Pnömoni çocukluk çağı yaş gruplarında ciddi mortalite nedeni olup, bu yazıda SARS-CoV-2 enfeksiyonuna ait gelişen klinik bulguları ve pandemi süresince üniversitemiz hastanesinde çocukluk çağı pandemi yönetimini sistemik olarak derlemeyi amaçladık. Studies on the clinical and demographic characteristics of SARS-CoV-2 (Severe Acute Respiratory Syndrome causing Coronavirus) infection in the pediatric population are limited. Pneumonia is a serious cause of mortality in childhood age groups, and we aimed to systematically review the clinical findings of SARS-CoV-2 infection and the management of childhood pandemics in our university hospital during the pandemic.

PMID: 35110115 Objective: Although many pediatric studies on children infected with coronavirus disease 2019 (COVID-19) have been published, the diagnosis, clinical symptoms, laboratory findings, and treatment of COVID-19 in children are still unclear. Materials and Methods: This study was conducted with an aim to examine the hematological findings of symptomatic pediatric patients diagnosed with COVID-19 in May 2020 at the Pandemic Hospital in Dicle University. Patient records were evaluated retrospectively. This study involved 59 symptomatic pediatric patients with a definite diagnosis of COVID-19 who had positive SARS-CoV-2 RT-PCR test results on nasopharyngeal swab between March 15, 2020 and May 31, 2020. Results: The records of a total of 10 (16.9%) patients under the age of 1; 21 (35.6%) patients aged 1-10 years, and, 28 (47.5%) patients aged 10-18 years, who had been diagnosed with COVID-19 were evaluated. Based on severity, 35 (59.3%) patients were in the mild group (group 1) and 24 (40.7%) patients were in the moderate-severe group (group 2). The blood parameters of WBC, neutrophil, lymphocyte, monocyte, and thrombocyte counts, the hemoglobin (Hgb) level, and NLR, PLR, MPV, fibrinogen, ferritin, and D-dimer levels were compared between groups, the difference was not statistically significant (P > .05). LDH was higher in group 2 (P = .014). Conclusion: Since children infected with COVID-19 show mild clinical symptoms or are asymptomatic, fewer pediatric patients may be detected than adults. Therefore, it should be known that the laboratory findings typical for adults may not accompany the disease in pediatric cases. More studies are needed to determine the most appropriate COVID-19 treatment approach for children, as hospitalization history and testing rates are less reported among children.

Bu yazıda, fiziksel ve duygusal istismarın bir arada görüldüğü 25 aylık kız çocuğu sunularak, ailede en küçük çocuk olmanın diğer faktörlerle birlikte değerlendirilerek, çocuk istismarı üzerinde etkisi tartışılmıştır. Hastaneye başvuran veya adli makamlarca gönderilen olgularda, çocuk istismarının ortaya çıkarılabilmesi, engellenebilmesi ve mağdur çocukların rehabilitasyonlarının yapılabilmesi için hastane aşamasında yapılması gerekenler, ilgili klinik branşlardan oluşan multidisipliner ekiplerin kurulması ve risk altındaki çocukların iyi tanımlanmasıdır. Child abuse continues to be an important public health problem. Child abuse, which causes serious social problems in the long term period is often done by the child's closest relatives and is therefore difficult to detect and treat. In this article, a 25-month-old girl with a combination of physical and emotional abuse and neglect is presented and the impact of being the youngest child in the family with other factors on child abuse discussed. In cases applied to or reported by the hospital or judicial authorities, multidisciplinary team works consisting of related clinical branches in hospitals and children at risk need to be well identified at the hospital level in order to detect and prevent child abuse and rehabilitate child abuse victims.

Amaç: Aşı tereddüdü hem Türkiye hem de dünya için toplumsal tehdit oluşturan bir tutumdur. Bu konuda Türkçeye validasyonu yapılmış bir ölçek bulunmamaktadır. Bu çalışmanın amacı Opel ve ark. tarafından 2011 yılında geliştirilip 2013 yılında revize edilen ve ‘Parent Attitudes About Childhood Vaccines Survey ‘isimli ölçeğin Türkçeye dil validasyonunun yapılmasıdır.Gereç ve Yöntem : Çalışma bir dil validasyonu çalışması olarak gerçekleştirilmiştir. Opel DJ ve arkadaşları tarafından geliştirilen ölçek için ‘Seri yöntem’ kullanılarak dil validasyonu çalışması yapılmıştır. Ölçeğin çeviri, gözden geçirme, geri çeviri, açıklığın ve aynılığın değerlendirmesi ile çevirisi sağlanmış ve Modifiye Lawshe yöntemiyle kapsam geçerlilik analizi yapılarak son hali verilmiştir. Uzmanların ölçek maddelerine yönelik içerik uygunluğu yönünden belirtmiş oldukları görüşlere göre her madde için kapsam geçerlilik oranı ve kapsam geçerlilik indeksi hesaplanmıştır.Bulgular. Tüm ölçek soruları için kapsam geçerlilik oranı kritik değer olan 0,455’in üzerinde saptanmıştır. Ölçeğin kapsam geçerlilik indeksi değeri ise 0,630 olup ölçek sorularının kapsam geçerliliğinin istatistiksel olarak anlamlı olduğu tespit edilmiştir.Çıkarımlar: Çocukluk çağı aşılarına yönelik ebeveyn tutumları ölçeğinin dil validasyonunun tamamlanması ve kapsam geçerlilik testlerinden istatistik olarak anlamlı sonuçlar alınmış olması bu ölçeğin Türkçe anlaşılırlığının sağlandığını göstermektedir. Sonrasında yapılacak geçerlilik ve güvenilirlik çalışmaları ile ölçeğin Türkiye’de aşı tereddüdü konusunda ebeveyn tutumlarını ölçebilir hale gelmesi hedeflenmektedir. Böylece daha geniş kitlelerde çalışılarak aşı tereddüdü ve mücadelesi konusunda yapılacak çalışmalara katkı sağlayacağı düşünülmektedir.

Amaç: Çocukluk çağı ishalleri sıklıkla virüslerden kaynaklanır. Enterik virüsler (özellikle rotavirüs ve adenovirüs) bakteriyel olmayan akut gastroenteritlerin en sık nedeni olarak bildirilmiştir. Gereç ve Yöntem: Çalışmaya 129 hasta ve 130 kontrol grubu olmak üzere toplam 259 çocuk dâhil edildi. Çalışma grubundaki vakalar rotavirüs pozitif, adenovirüs pozitif ve hem rotavirüs hem de adenovirüs pozitif olmak üzere üç gruba ayrıldı. Bulgular: Çalışma grubundaki olguların yaş ortalamaları 21,7 ± 30,0 ay ve %60,5’i erkek idi. Kontrol grubundaki vakalarda ishal varlığı (p = 0,023) ve 0-10 arasındaki ishal sayısı (p = 0,001) istatistiksel olarak daha yüksek iken, çalışma grubunda 11-15 arasındaki ishal sayısı (p = 0,001) çalışma grubunda daha fazla idi. Çalışma grubundaki olguların %71,3’ünde rotavirüs, %20,2’sinde adenovirüs ve %8,5’inde hem rotavirüs hem de adenovirüs antijen pozitifliği vardı. Rotavirüs pozitif gruptaki hastaların üre, albümin ve CRP değerleri arasında istatistiksel yönden kuvvetli ve anlamlı bir ilişki varken, Adenovirüs ve hem Rotavirüs hem de Adenovirüs pozitif gruptaki hastaların beyaz küre sayısı, üre, albümin ve CRP değerleri arasında istatistiksel yönden kuvvetli ve anlamlı bir ilişki vardı (p < 0,05). Rotavirüs pozitif, adenovirüs pozitif ve hem rotavirüs hem de adenovirüs pozitif olan üç gruptaki vakalarda ishal, kusma, karın ağrısı ve iştahsızlık ile aralarında istatistiksel yönden kuvvetli ve anlamlı bir ilişki vardı (p < 0,05). Sonuç: Özellikle viral gastroenteritlerden rotavirüs ve/veya adenovirüs enfeksiyonunda üre ve albumin ile sekonder enfeksiyon açısından beyaz küre sayısı ve CRP düzeylerinin de yakın takip edilmesi gerektiği kanısındayız. Objective: Childhood diarrheas are mostly caused by viruses. Enteric viruses (particularly rotavirus and adenovirus) are the most common culprits of acute gastroenteritis. Materials and Methods: A total of 259 children were included in the study, and the patients were divided into two groups: patient group (n = 129) and control group (n = 130). The study group was also sub-divided in three as rotavirus positivity, adenovirus positivity, and both rotavirus + adenovirus positivity. Results: The mean age of the patient group was 21.7 ± 30.0 months, and 60.5% were males. While the presence of diarrhea (p = 0.023) and the number of diarrhea between 0 and 10 (p = 0.001) were statistically higher in the control group, the number of diarrhea between 11 and 15 was statistically highe r in the patient group. Rotavirus was positive for 71.3%, adenovirus was positive for 20.2%, both rotavirus + adenovirus were positive for 8.5% in the patient group. There was a statistically strong and significant relation with white blood cell, urea, albumin and C-reactive protein (CRP) values for the patients with rotavirus positivity (p < 0.05). There was a statistically strong and significant relation with diarrhea, vomiting, abdominal pain, and lack of appetite for both three subgroups with rotavirus positivity, adenovirus positivity, and both rotavirus + adenovirus positivity (p < 0.05). Conclusion: Based on our study results, we believe that patients with viral gastroenteritis caused by rotavirus and/or adenovirus should be monitored for the development of secondary infections, considering urea, albumin levels, white blood cells, and CRP values.

Amaç: Bu çalışmada, obez çocuk ve adölesanlarda tiroid fonksiyonları ile metabolik ve antropometrik parametreler arasındaki ilişkinin değerlendirilmesi amaçlanmıştır. Yöntemler: Çalışmaya pediatrik endokrinoloji polikliniğinde eksojen obezite tanısı konulan 147 hasta ile benzer yaş ve cinsiyette 46 sağlıklı birey alındı. Obez hastalar Grup 1 (karaciğerde yağlanma olmayan) ve Grup 2 (karaciğerde yağlanma olan) olarak iki gruba ayrıldı. Hasta ve kontrol grubundaki (Grup 3) tüm vakalardan açlık glukoz, insülin, total kolesterol, trigliserid, aspartat aminotransferaz, alanin aminotransferaz, tiroid uyarıcı hormon (TSH), serbest triiyodotironin (fT3) ve serbest tiroksin (fT4) için örnek alındı. İnsülin direnci HOMA-IR (Homeostatic Model Assesment for Insulin Resistance) ile değerlendirildi. Bulgular: Grup 2’de bel çevresi (BÇ), beden kitle indeksi - standart deviasyon skorları (SDS), kalça çevresi (KÇ), BÇ/KÇ oranı ile trigliserid, açlık insülin ve HOMA-IR değerleri Grup 1 ve Grup 3’e göre anlamlı derecede yüksek iken yüksek dansiteli lipoprotein kolesterol düzeyi daha düşük idi. Obezitesi olan gruplarda (karaciğerde yağlanma olan ve olmayan) fT4 düzeyi kontrol grubuna göre anlamlı düzeyde düşük iken fT3/fT4 oranı daha yüksek idi. Ortalama TSH düzeyi Grup 2’de Grup 3’e göre daha yüksek saptandı. fT3/fT4 oranı; BÇ, BÇ/KÇ oranı, açlık insülin ve HOMA-IR ile pozitif korelasyon göstermekteydi. Sonuç: Obez çocuklarda fT4 düzeyinin düşük olduğu, fT3/fT4 oranının ise daha yüksek olduğu tespit edilmiştir. Obez çocuklarda yüksek fT3/fT4 oranının saptanması durumunda bu oranın BÇ ve insülin direnci ile pozitif korelasyon gösterdiği düşünülerek visseral yağ dokusuyla ilişkili komplikasyonlar konusunda klinisyen için uyarıcı olmalıdır. Background: The aim of this study was to evaluate the relationship between thyroid functions and metabolic and anthropometric parameters in obese children and adolescents. Methods: The study included 147 patients diagnosed with exogenous obesity in the pediatric endocrinology outpatient clinic and 46 healthy subjects with similar age and sex. Obese patients were divided into two groups as Group 1 (non-fatty liver) and Group 2 (with fatty liver). Fasting glucose, insulin, total cholesterol, triglyceride, aspartate aminotransferase, alanine aminotransferase, thyroid stimulating hormone (TSH), free triiodothyronine (fT3) and free thyroxine (fT4) samples were performed from all cases in patient groups and control group (Group 3). Insulin resistance was assessed by HOMA-IR (Homeostatic Model Assessment of Insulin Resistance). Results: In Group 2, waist circumference (WC), body mass index-standard deviation scores (SDS), hip circumference (HC), WC/ HC ratio and triglyceride, fasting insulin and HOMA-IR values were significantly higher than Group 1 and Group 3 while high density lipoprotein cholesterol level was lower. In Group 1 and Group 2, fT4 level was significantly lower than Group 3, whereas fT3 / fT4 ratio was higher. The mean TSH level was higher in Group 2 than Group 3. fT3 / fT4 ratio; WC, WC / HC ratio was positively correlated with fasting insulin and HOMA-IR. Conclusion: We found that fT4 level was low and fT3 / fT4 ratio was higher in obese children. Considering the high fT3 / fT4 ratio in obese children, this ratio should be stimulating for the clinician regarding the complications associated with visceral adipose tissue as it is positively correlated with WC and insulin resistance.

Objective: Preterm neonates were approximately 70% of total low birth weight infants in developed countries. The aim of this study is to evaluate the low birth weight neonates who were hospitalized in the neonatal unit of our hospital between the years 2010-2011, retrospectively. Methods: 626 infants that born with under 2500 gram were enrolled to the study. Family history, natal, prenatal, postnatal, maternal and demographic features and risk factors were recorded retrospectively. Results: Mother giving birth age was 21-35 years, gestational age was at 33 weeks and birth weight was found 2001-2500 gram mostly and the mortality rate was found 29.7% in our study. Maternal age, multipl pregnancy ratio, place of birth, birth presentation, duration of hospitalization, causes of premature birth and mortality rate is found statistically significant difference between low birth weight infants when comparing with birth weight (p, Amaç: Gelişmiş ülkelerde düşük doğum ağırlıklı yenidoğanların yaklaşık %70’ini preterm yenidoğanlar oluşturmaktadır. Çalışmamızda 2010-2011 yılları arasında hastanemiz yenidoğan servisinde yatan düşük doğum ağırlıklı bebeklerin geriye dönük olarak değerlendirilmesi amaçlandı.Yöntemler: Bu çalışmaya, 2500 gram altı doğan 626 yenidoğan alındı. Retrospektif olarak, aile öyküsü, natal, prenatal, postnatal, anne ve demografik özellikleri ile risk faktörleri kaydedildi.Bulgular: Çalışmamızda anne yaşı en sık 21-35 yaş arası, gebelik haftası 33 hafta üstü, doğum ağırlığı 2001-2500 gram arası ve mortalite oranı %29,7 olarak bulundu. Sjögren-Larsson syndrome (SLS) is a rare neurocutaneous disease showing an autosomal recessive transmission due to a lack of fatty acid aldehyde dehydrogenase. Spastic diplegia or triplegia, mental retardation and congenital lamellar ichthyosis are the major findings of the disease. The syndrome may be accompanied by various eye and teeth features, skeletal system anomaly, speaking defects, hypertelorism and epilepsy. A 9-month male patient has been hospitalized for convulsion and flaking on body. The patient history showed that flaking skin thickening and peeling was started at the birth, and he suffered a right-side focused seizure when he was three month-old and he was treated with phenobarbital and carbamazepine upon the epilepsy diagnosis. Wide ichthyosis, hypertelorism and bilateral simian line were observed in the physical examination. Bilateral punctuate lesions in cornea, pigment epithelial atrophy in the right eye and esotropia in the left eye have been determined during the eye examination. An epiteliform anomaly has been observed in the left hemisphere by electroencephalography. In brain magnetic resonance imaging (MRI), an increase in cerebral-cerebellar brain parenchyma and T1-T2 relaxation time and in the signal in corpus callosum (delayed myelination) have been determined. With the observation of the white matter in centrum semi oval using brain MRI spectroscopy, signs of a sphingolipid peak at 1.3 ppm have been observed. An SLS diagnosis has been proposed upon clinical and laboratory observations. Öz: Amaç: Pseudomonas aeruginosa infeksiyonları en sık görülen hastane infeksiyonlarıdır. Bu çalışmada, Dicle Üniversitesi Tıp Fakültesi Hastanesi'nde izole edilen P.aeruginosa suşlarının direnç oranlarının belirlenmesi amaçlandı. Gereç ve Yöntem: Çalışmamıza Haziran 2010 ve Haziran 2011 tarihleri arasında Dicle Üniversitesi Tıp Fakültesi Hastanesi'nde çeşitli materyallerden izole edilen 171 hastaya ait P.aeruginosa kültür sonuçları alındı. P.aeruginosa suşlarının direnç oranları klasik yöntemlerle veya BD Phoenix (BD Diagnostic Systems, Sparks, MD) otomatik sistem kullanılarak belirlendi. Bulgular: Çalışmamız 64 (%37.4) kız ve 107 (%62.6) erkek olgudan oluşuyordu. P. aeruginosa suşlarında direnç oranları; siprofloksosin %50 (104/210), sefepime %43 (66/154), seftazidime %40 (63/158), gentamisin %32 (51/157), imipenem %25 (40/159), meropenem %25 (40/159), piperasillin/tazobaktam %23 (36/157), amikasin %14 (22/161), kolimisin %5 (2/37) olarak bulundu. Sonuç: Kültür materyallerinden izole ettiğimiz P.aeruginosa suşlarında yüksek oranda antimikrobiyal direnç ve çoğul direnç bulundu. Bu sonuçlar direnç gelişimini engelleme amacıyla uygun, dar spektrumlu antibiyoterapi seçimi ve hijyen kurallarına uyumun önemini göstermektedir. Objectives: Pseudomonas aeruginosa infections are the most common nosocomial infections. This study, was aimed to determine resistance rates of P.aeruginosa strains isolated from Dicle University Medical Faculty Hospital. Materials and Methods: The study was included P.aeruginosa culture results of 171 patients that were isolated from a variety of materials at Dicle University Medical Faculty Hospital between June 2010 and June 2011. The resistance rates of P.aeruginosa strains were measured by conventional methods or Phoenix (BD Diagnostic Systems, Sparks, MD) automatic system. Results: Our study consisted of 64 (37.4%) female and 107 (62.6%) male cases. P. aeruginosa resistance rates were as ciprofloxacin %50 (104/210), cefepime %43 (66/154), ceftazidime %40 (63/158), gentamicin %32 (51/157), imipenem %25 (40/159), meropenem %25 (40/159), piperacillin / tazobactam %23 (36/157), amikacin %14 (22/161), colistin %5 (2/37). Conclusion: P. aeruginosa strains isolated from our cultures had high antimicrobial resistance and multiple resistance. For the preventing the development of resistance, the importance of choosing the appropriate narrow-spectrum antibiotics and compliance with the rules of hygiene are emerging.

Amaç: Mekonyum aspirasyon sendromu (MAS) term infantlarda solunum sıkıntısı nedenlerinden olup değişken morbidite ve mortalite ile seyredebilmektedir. Bu çalışmanın amacı MAS tanısı konan yenidoğanlarda demografik verileri, muhtemel risk faktörlerini ayrıca morbidite ve mortalite ile ilgili durumları değerlendirmektir. Gereç ve Yöntem: Bu çalışmamızda Dicle Üniversitesi Tıp Fakültesi Çocuk Sağlığı ve Hastalıkları Yenidoğan Yoğun Bakım Ünitesi’nde (YYBÜ) Ocak 2011- Eylül 2013 tarihleri arasında MAS tanısı alan 106 yenidoğan retrospektif olarak değerlendirildi. Bulgular: MAS YYBÜ yatışlarının %1.05’inden sorumlu bulundu. Olgularımızın 51’i (%48.1) erkek, 55’i (%51.9) kız, 63’ü (%59.4) term, 25’i (%23.6) post-term idi. Gestasyon haftaları 38.6 ± 2.4 (34-43) hafta, doğum ağırlıkları 3136 ± 655 (1700-4300) gr bulundu. Ortalama yatış süreleri 8.7 ± 4.4 (3-27) gündü. 54 (% 50.9) olgunun 1. dakika Apgar skoru ≤ 3 ve 10 (%9.43) olgunun 5. dakika Apgar skoru ≤ 3 idi. Mortalite oranı %21.7 idi. Sonuç: 1. dakika Apgar skoru ≤ 3 olması MAS’lı olgularda artmış morbidite ve mortalite ile ilişkili olarak bulundu. MAS ile ilişkili risk faktörlerinin tanımlanması hastalığın teşhisinde ve erken tedavi stratejilerinin geliştirilmesinde önemlidir. Post-term gebeliklerden korunmakla ve fetal distres durumunda sezeryan doğumların uygun zamanda yapılması ile morbidite ve mortalite oranlarının iyileştirilmesinin mümkün olacağı düşünüldü. Aim: Meconium aspiration syndrome (MAS) is one of the common causes of respiratory distress in term infants presenting highly variable morbidity and mortality. The aim of this study was to evaluate the demographic data, possible risk factors, factors associated with morbidity and mortality in MAS newborns. Materials and Methods: The study was conducted at Neonatal Intensive Care Unit (NICU) of Dicle University Medical School Department of Pediatrics. A total of 106 newborns diagnosed as MAS between January 2011 and September 2013 were retrospectively evaluated. Results: MAS found to be responsible from 1.05% of NICU admissions. The study included 51 (48.1%) male and 55 (51.9%) female newborns. Of them 63 (59.4%) were term infants and 25 (23.6%) were postterm. The mean gestational age was 38.6 ± 2.4 (34- 43) weeks and the mean birth weight was 3136 ± 655 (1700-4300) gram. The mean hospitalization duration was 8.7±4.4 (range 3-27) days. The first minute APGAR score ≤ 3 was found in 54 (50.9%) and the fifth minute APGAR score ≤ 3 was found in 10 (9.43%) of newborns with MAS. The overall mortality was 21.7%. Conclusion: The first minute APGAR score ≤ 3 was found to be related to increased morbidity and mortality. Determination of risk factors associated with MAS is important regarding diagnosis and implementing early treatment strategies. It was tought that morbidity and mortality rates could be ameliorated by prevention of post term gestation and performing cesarian section at appropriate time.

Henoch-Schonlein purpura are many and varied symptoms, also it was followed in children policlinics and hospitalized diseases for an important role. We aimed to determination of the patients with Henoch-Schonlein purpura by examination of their clinical analyses. Material and Method: The medical records of 212 patients with Henoch-Schonlein purpura in our polyclinics have been retrospectively studied. These patients were analyzed age groups, diagnosed seasons, held systems, laboratory findings, the treatments, complications and prognosis. SPSS 13 statistical software package was used for data analysis. Results: The mean age of the patients was 9.33 +/- 3.20 years. The disease was more frequently demonstrated in fall and winter seasons. There was no statistically assosiation between age groups with season, joint involvement, central neryeous system involvement, gastrointestinal system involvement and scrotal involvement but there was a positive correlation between age groups and renal involvement Conclusion: The treatment duration increases with the age of the patient and the necessity to use a combined treatment (antihistaminic, non-steroid anti-inflammatory treatment and steroids) increases.
