OBJECTIVE Glioblastoma (GBM) is the most aggressive primary brain tumor. Despite all treatments, very few have long-term survival. This retrospective study aimed to investigate the clinicopathological features, treatment modalities, and factors affecting survival in GBM patients with short- and long-term survival. METHODS Data from 217 GBM patients who received radiotherapy (RT) between 2010 and 2021 were analyzed. The patients were divided into two groups: short (<6 months) and long (>2 years) living groups. Treatment, patient, and tumor characteristics were evaluated. RESULTS While 37 (17.1%) of 217 patients included in the group lived <6 months, 49 (22.6%) were in the group that lived longer than 2 years. In the long-living group, being under 65 years of age, having better performance, performing total excision, applying conventional RT, and receiving adjuvant chemotherapy (CT) detected more frequently. The regression test showed that young age, good performance, and receiving conventional RT and adjuvant chemotherapy (CT) were independently associated with survival. CONCLUSION It was observed that patients who lived longer were frequently young and well-performing ones who underwent wide excision and received conventional RT and adjuvant CT. By estimating the pre-treatment survival, treatment and support plans can be made accordingly. [ABSTRACT FROM AUTHOR]

Background: Triple negative breast cancer (TNBC) is the most aggressive and worst prognosis group among breast cancer molecular subtypes. This retrospective study aimed at determining the prognostic factors affecting survival in the TNBC group. Methods: Between 2010 and 2020, patients with invasive ductal carcinoma who received curative radiotherapy were included in the study. The patients were divided into two groups as TNBC and non-TNBC. Clinicopathological and treatment parameters of both groups were compared. Survival rates were evaluated using the Kaplan Meirer method. Results: One hundred ten (11.1%) of 992 patients were triple negative. The TNBC group showed more grade 2-3 tumors (95.8% vs 87.8%, P=0.019), a higher ki-67 value (72.7% vs 44.9%, P<0.001), more metastasis presence (28.2% vs 16.2%, P=0.002) and more exitus (28.2% vs 14.5%, P<0.001) than the non-TNBC group. Brain metastasis was observed more frequently in the TNBC group. In the TNBC group, being =70 years of age (P=0.05), having the T3-4 disease (P=0.040), the presence of perineural invasion (P=0.022), the presence of metastasis (P<0.001), and the presence of brain metastasis (P=0.049) had a negative effect on OS in univariate analysis. Having TNBC was determined as an independent variable that negatively affected both overall and disease-free survival in multivariate analysis. Conclusion: This single-center study showed that having TNBC had a negative impact on survival. The treatment in TNBC patients should be determined by considering all factors affecting recurrence and survival and should be individualized. [ABSTRACT FROM AUTHOR]

Amaç: Tüm dünyada yaşlı nüfüsun giderek artmasıyla birlikte rektum kanserinin de yaşlı nüfusta görülme sıklığı artmaktadır. Raktum kanserinin standart tedavisi olan neoadjuvan krt ile sağkalım sonuçlarının arttığı bilinmekle birlikte yaşlı hastalarda prognozu ve sağkalımı etkileyen faktörler ile ilgili veriler sınırlıdır. Gereç ve Yöntem: Aralık 2009-Haziran 2021 tarihleri arasında, 65 yaş ve üstü olan ve neoadjuvan krt uygulanmış 171 hastanın verileri retrospektif olarak incelendi. Tek değişkenli analizler Kaplan Meier, çok değişkenli analizlar ise Cox Regresyon analizi ile değerlendirildi. Bulgular: Çalışmaya dahil edilen 171 hastanın 117 si erkek (%68), 54 ü kadındı ve median yaş 72 idi (65-90 yaş). Median izlem süresi 40,5 aydı (3-156 ay). Hastaların %94.7 si planlanan eşzmanlı kRT yi alabilmişken sadece 9 hastaya KT verilemedi. Hastaların %63 ü KRT sonrası opere edilirken 62 hastaya çeşitli nedenlerle operasyon uygulanmadı. Opere edilen 109 hastadan 83 ü Low anterior rezeksiyon, 26 sı Abdominoperineal rezeksiyon yapıldı. Ortalama genel sağkalım süresi opere olanlarda 109 ay (74-144 ay) iken opere olmayanlarda 29 ay idi (p<0,001). Kaplan Meier tek değişkenli analizlerde; yaş (<75 vs >75, p<0.00), Karnofsky Performans skalası(KPS, <70 vs>70, p=001), tanı anında lenf nodu durumu (LN+ vs LN-, p<0,001), opere olanlarda lenfovasküler invazyon durumu (LVI+ vs -, p<0,001) ve tümör regresyon derecesi (TRG tam/tamayakın ve diğerleri, p=0,01) genel sağkalımı (GS) etkileyen faktörler olarak bulundu. Cox Regresyon çok değişkenli analizinde de KPS dışındaki tüm değişkenler istatistiksel olarak anlamlı bulundu. Opere edilen grupta GS istatistiksel olarak anlamlı olarak uzun olmasına karşı, yaş gruplarına göre bakıldığında <75 yaşta operasyonun sağkalıma anlamlı katkısı varken (p<0,001), 75 yaş üstü hastalarda opere olup olmamanın sağkalıma katkısı gösterilemedi (p=0,3). Benzer şekilde <75 yaşta KPS nin GS a olumlu katkısı varken (p<0,001)>75 yaşta KPS nin anlamlı katkısı gösterilemedi (p=0,1). Sonuç: Çalışmamızda 65 yaş üstü rektum kanserli hastalarda yaş, KPS, operasyon durumu, tanı anındaki LN durumu, operasyon sonrası LVI ve TRG durumu nun sağlkalımı etkileyen önemli faktörler olduğu görüldü. >75 yaş hasta grubunda ise opere olup olmamanın ve KPS nin sağkalıma bir katkısı gösterilemedi. Yaş ilerledikçe komorbid hastalıkların daha sık ve ağır seyretmesi ve genel performansı etkileyeceğinden dolayı opersyon durumunun sağkalıma etkisinin saptanamamış olabileceği düşünüldü. Among young women breast cancer exhibits quite a heterogenous, quite agressive and complex biology. The purpose of this study is to describe clinicopathological features that affect survival ratio among breast cancer women below age-40. 803 patients having received adjuvant radiotherapy were retrospectively analyzed. Patients were categorized under two groups: age ≤ 40 and age > 40. Treatments and clinicopathological features were analyzed. 19.4% (156) of 803 patients were below age 40. In patient group below age 40, more neoadjuvant chemotherapy (NAC) was administered compared to the patient group above age 40 (p= 0.007) and it was detected that there were higher incidences of stage 3 disease (p= 0.011), more advanced nodal disease (N2-3) (p= 0.046) and more metastasis (p< 0.001). In conducted multivariate analysis for all age groups presence of N2-3 disease (p= 0.011) and in below age-40 group being grad 3 (p= 0.025) was found to affect overall survival (OS) negatively. In disease free survival (DFS)-focused analysis; for all age groups, receiving NAC (p= 0.001), presence of N2-3 disease (p= 0.002) and being below age 40 were found to have a negative effect; in below age-40 group presence of NAC (p= 0.013) and perineural invasion (p= 0.035); in above age-40 group receiving NAC (p= 0.023) and presence of N2-3 disease (p= 0.035) had a negative effect. Being below age 40 is an independent prognostic factor for DFS. It is suggested to conduct further studies on specific tumor biology to analyze the same group with respect to the characteristics of more aggressive tumors. [ABSTRACT FROM AUTHOR]

Objective: This study investigated pre- and post-treatment tumor and lymph node dimension response rates and differences between side-effect profiles in patients with locally advanced inoperable nonsmall-cell lung cancer (NSCLC) receiving radiotherapy (RT) and concurrent chemotherapy (CT). Materials and Methods: A total of 30 inoperable patients who had not previously received RT and having a mean age of 58.73±8.65 years with sufficient hematological reserves and normal hepatic and renal functions were included in the study. Those with pleural effusion, supraventricular lymph node metastasis, and N3 lymph node involvement were excluded. Group I (n=15) received a 21-day 75 mg/m2 cisplatin (D1) and 15 mg/m2 vinorelbine (D1, D8), whereas Group II (n=15) received 45 mg/m2 paclitaxel and AUC2 carboplatin weekly. RT was administered using a linear accelerator device with the 3D conformal RT technique at 6-18 MV energy with a 1.8-2 Gy fraction for 6-7 weeks. Results: Patients were randomized into Group I receiving RT and concurrent cisplatin--vinorelbine and Group II receiving weekly paclitaxel--carboplatin CT. Pre- and post-treatment tumor and lymph node dimensions significantly differed in both groups (p<0.001 and p<0.01, respectively). No significant change was observed in post-RT tumor and lymph node dimensions in terms of applied CT regimens (p>0.05). Conclusion: The significant response achieved with concurrent RT and CT in groups I and II in the local advanced stage of NSCLC is important for local tumor control. Objective: The clinical feature of breast cancer is very heterogeneous because of the variable prognostic factors impact its behaviour. The aim of study is to find the prognostic importance of Ki-67 and to analyse the correlation between Ki-67 index and the other conventional prognostic factors in breast cancer patients. Materials and Methods: Between 2010 and 2017, patients with invasive ductal carcinoma who received radiotherapy after surgery were included in study. A single pathologist re-defined of all cases retrospectively. Ki-67 were established three categories based on Ki-67 levels: low (<10%), intermediate (10-25%) and high (>25%). Results: A total of 258 patients were included. 46 of 258 (18%) patients were in low, 82 of 258 (32%) patients were in intermediate and 130 of 258 (50%) patients were in high Ki-67 group. There were no correlations between menopausal status, age, and Ki-67 level. Low-pT stages tended to have low Ki-67 expression (p=0.07). Low-pN stages correlated with low Ki-67 values (p=0.007). Patients with ECE (+) were prone to have higher Ki-67 values (p=0.02). The significant correlation was seen between Ki-67 and tumour grading (p=<0.0001). Patients with LVI (+) had higher Ki-67 expression (p=0.007). Luminal A tumours were correlated with low Ki-67 group (p=<0.0001). Ki-67 values had significant effect on DFS (p=0.03) but not OS (p=0.09). Conclusion: This study showed that high Ki-67 expression is associated with higher pT-stage, higher pN-stage, higher grade, ER/PR negativity, HER2/neu positivity, ECE and LVI positivity. Amaç: Rektum kanseri tanısıyla neoadjuvan KRT uygulanan 50 yaş ve altı genç hastalarda sağkalımı etkileyen faktörleri değerlendirmek. Gereç ve Yöntem: Ocak 2010-Haziran 2021 tarihleri arasında, 50 yaş ve altı olan ve neoadjuvan krt uygulanmış 49 hastanın verileri retrospektif olarak incelendi. Tek değişkenli analizler Kaplan Meier, çok değişkenli analizlar ise Cox Regresyon analizi ile değerlendirildi. Bulgular: Çalışmaya dahil edilen 49 hastanın 31i erkek (%63), 54 ü kadındı ve median yaş 45 idi (25-50 yaş). Median izlem süresi 51 aydı (7-150 ay). Hastaların hepsinin Karnofsky performans durumu 90 ve üzerindeydi. Tüm hastalara eşzamanlı oral kapasitabin kemoterapisi verildi ve hastaların hepsi planlanan eşzmanlı KRT yi alabildi. Tanı anında tümör yerleşim yerlerine göre gruplandırıldığında 21 hastanın tümörü distal (0-4.cm), 24 hastanın orta (5-9. Cm) ve 4 hastanın proksimal (>10.cm) rektum yerleşimliydi. Hastaların %85 i KRT sonrası opere edilirken 7 hastaya çeşitli nedenlerle operasyon uygulanmadı. Opere edilen 42 hastadan 32 sine Low anterior rezeksiyon, 10 una Abdominoperineal rezeksiyon yapıldı. Ortanca genel sağkalım (GS) süresi opere olanlarda 112 ay (94-131 ay) iken opere olmayanlarda 51 ay idi (p=0,004). Hastaları KRT sonrası patolojik klinik yanıt durumlarına göre değerlendirdiğimizde 5 hastada patolojik tam yanıt, 2 hastada klinik tam yanıt, 30 hastada belirgin patolojik regresyonla tama yakın yanıt elde edilirken17 hastada daha düşük düzeyde patolojik yanıt elde edildi, opere olmayan 5 hastanın ise yanıt durumu değerlendirilemedi. Kaplan Meier tek değişkenli analizlerde; operasyon durumu (opere + vs -, p= 0,004), tümör yerleşim yeri (proksimal vs distal, p<0,001) ve tümör regresyon gradi (TRG tam/tama yakın grup ve dve tümör regresyon derecesi (TRG tam/tama yakın grup vs diğerleri, p=0,01) genel sağkalımı (GS) etkileyen faktörler olarak bulundu. Cox Regresyon çok değişkenli analizinde de tüm değişkenler istatistiksel olarak anlamlı bulundu. Sonuç: Çalışmamızda 50 yaş altı rektum kanserli hastalarda operasyon durumunun, tanı anındaki tümörün rektumdaki yerleşim yerinin ve TRG durumunun sağkalımı etkileyen önemli faktörler olduğu görüldü. Rektım kanserinin standart tedavisi olan neodjuvan KRT sonrası cerrahi uygulamanın tüm yaş gruplarında olduğu gibi genç yaş hasta grubunda da sağkalıma etkisi bulunmaktadır. [ABSTRACT FROM AUTHOR]
