- Abstract
Objectives:This study aimed to evaluate how Rhinapi nasal spray affects symptoms of allergic rhinitis. Methods:In this prospective, multicenter, observational study, 10,000 patients (comprising 5028 males and 4972 females) exhibiting symptoms of allergic rhinitis (namely, nasal discharge, sneezing, nasal itching, and nasal obstruction) from different centers in different regions of Turkey were enrolled in the study between March 2022 and March 2023. All the patients wanted to participate in the study and were administered Rhinapi one puff to each nostril three times a day, for a period of 3 weeks. Total symptom scores, quality of life (QoL) scores, and otolaryngological examination scores were evaluated before and 3 weeks after treatment. Results:The scores for discharge from the nose, sneezing, nasal pruritus, and blockage of the nose all indicated improvement when compared to pre-medication and post-medication. This difference achieved statistical significance (P< .001). The mean total symptom score fell following treatment (P< .001): whilst the score was 11.09 ± 3.41 before administering Rhinapi; after administration, the average score was 6.23 ± 2.41. The mean QoL scores also altered after medication (P< .001), improving from a mean value of 6.44 ± 1.55 to a mean of 7.31 ± 1.24. Significant improvement was also noted in the scores for conchal color and degree of edema after the treatment had been administered (P< .001). Conclusion:The study demonstrates that Rhinapi nasal spray decreases total symptom scores, and results in improved QoL and otolaryngological examination scores. Propolis spray may be recommended for patients with allergic rhinitis alongside other treatments.

6. Determination of Vaccination Rates for Influenza and Pneumococcal Vaccines in Patients with Chronic Obstructive Pulmonary Disease and Factors Affecting Vaccination.

Objective: Influenza and pneumococcal vaccines are recommended for chronic obstructive pulmonary disease (COPD) cases in national and international guidelines. In this study, it was aimed to determine the vaccination rates for influenza and pneumococcal diseases in COPD patients and to determine the demographic and clinical, characteristics that affect the vaccination of the patients. Methods: Our study included 297 COPD patients aged 18 years and older who were diagnosed with COPD for at least 1 year according to the Global Initiative for Chronic Obstructive Lung Disease criteria. Pulmonary function tests of the patients, staging of the disease, and the Modified Medical Research Council scale were recorded. Results: When the 297 patients included in the study were evaluated according to the inclusion and exclusion criteria. In the study, the rate of influenza vaccination in COPD patients in the last year was 29.4% and the rate of pneumococcal vaccination at least once in their lifetime was 34.5%. Vaccination rates of patients aged 65 and over were significantly higher in influenza vaccination (p=0.036). In pneumococcal vaccination, the vaccination rate was statistically high in those with a high education level (p=0.001). It was observed that the vaccination rates were significantly lower in patients with low-income levels in both vaccine groups (p=0.044, p=0.034). When asked about the reason for the unvaccinated patients, they were told that they were not aware of the vaccine (41.3%, 76.0%) in the first place and that their doctor did not recommend it (28.2%, 27.6%) in the second place for influenza and pneumococcal vaccines. Conclusion: Influenza and pneumococcal vaccine application rates in patients with COPD were found to be low in our country, in line with the literature. The lack of doctor's advice and lack of knowledge about the vaccine was important factors in unvaccinated individuals. [ABSTRACT FROM AUTHOR]

Objective: In this study, it was aimed to show that the place of death of patients who died due to lung cancer, shifted to the palliative service, after the establishment of the palliative care service in our hospital.Materials and Methods: The study was conducted as a 4-year retrospective cross-sectional study at the Chest Diseases and Thoracic Surgery Training and Research Hospital between 2017 and 2020. Patients who were diagnosed with lung cancer and died in the hospital during the study period were included in the study. Demographic data of the patients, comorbidities, laboratory values, changes in hospital location, respiratory support (invasive and noninvasive mechanical ventilation, oxygen), medical treatments, places of death and at most four causes of death were recorded from hospital mortality data. Data were summarized by descriptive analysis. Results: During the study period, 15,552 patients were hospitalized due to lung cancer and the number of patients who died from these patients was determined as 1896 (12%). Of the 1896 patients who died, 44% (n=834) had their first hospitalization in palliative care, and only 19 of these patients were referred back to the intensive care unit. 8. Value of prognostic nutritional index in patients with non-small cell lung cancer.

BACKGROUND AND AIM: The systemic inflammatory response plays a crucial role in the development and progression of many cancer types. In our study, we investigated the association of prognostic nutritional index (PNI) with progression-free survival (PFS) and overall survival (OS) in patients with non-small cell lung cancer (NSCLC). METHODS: This was a retrospective cohort study. The data of patients who were followed up in the oncology clinic of our hospital between October 2011 and June 2014 were obtained from the hospital automation system records and patient files. A total of 240 patients with NSCLC diagnosis were included in the study, and their demographic and clinicopathological characteristics were recorded. PNI was calculated at the time of diagnosis based on albumin levels and lymphocyte counts. RESULTS: In total, 231 patients were included in the study (205 [88.7%] men and 26 [11.3%] women), with a mean age of 59.97±9.44 years. We divided the patients into two groups, namely low (≤42.2) and high (>42.2) PNI groups, based on their median PNI values. The median OS of the low and high PNI groups were 380.00 (95% CI: 347.00–412.96) and 568.00 (95% CI: 515.52–620.48) days, respectively. The difference was statistically significant (p=0.009). Low PNI was associated with a poor OS, and the mortality rate of the low PNI group was 1.5 times higher than that of the high PNI group (hazard ratio: 1.50; 95% CI: 1.08–2.08). No statistically significant difference was observed between PNI values and PFS (p=0.328). CONCLUSIONS: This study showed that PNI (≤42.2) at diagnosis is an independent biomarker of poor prognosis in patients with NSCLC. Therefore, PNI can be used as a biomarker of NSCLC prognosis because it is simple, inexpensive, and easily available. [ABSTRACT FROM AUTHOR]

BACKGROUND AND AIM: The use of tobacco and tobacco products is one of the biggest public health problems that continue to threaten human health worldwide. Thirdhand smoke (THS) is a new concept that emerges from tobacco use, and there are not enough studies on its effects on human health. Therefore, we aimed to determine the level of awareness of THS and the parameters affecting it in individuals who applied to a chest diseases hospital in Türkiye. METHODS: A total of 400 volunteers who admitted to our hospital between February and March 2022 were literate and accepted to participate in the study were included. Volunteers were given a questionnaire including the demographic characteristics and Baths-T survey, which was used to determine the awareness of THS. RESULTS: Of the 400 participants included in the study, 162 (40.5%) were females, 238 (59.5%) were males, and their mean age was 44.09±12.75 years. In our study, the mean Baths-T score was 35.1±6.74. The mean score of persistence and health beliefs was 15.51±3.54 and 19.58±3.74, respectively. It was determined that the total and two subdimension scores were statistically significantly higher in women than in men (p=0.019, 0.046, and 0.011). Awareness was statistically significantly lower in smokers and those with low education levels (p=0.001 and 0.014). CONCLUSIONS: We think that the male gender, low education level, and smokers, whose awareness level is low, should be the primary target group in informing about THS. [ABSTRACT FROM AUTHOR]

Introduction: COPD is a disease characterized by chronic inflammation in the airways and increased systemic inflammation. Interleukin-6 is a major immune and inflammatory mediator.Objective: To examine the level of IL-6, an inflammatory mediator, in induced sputum and serum of patients with COPD in the acute attack and stable period.Materials and Methods: Thirty consecutive patients diagnosed with COPD were included in the study. In the attack and stable period, IL-6 levels were studied in the induced sputum and blood taken simultaneously. Twenty-one patients who did not have any pathological findings in terms of chest diseases were selected as the control group. ELISA kit (Bender MedSystems, Vienna, Austria) was used for IL-6 measurements and values were measured as pg/ml. Mann-Whitney U test was used for intergroup comparisons and Wilcoxon Signed Ranks test was used for intragroup comparisons.Results: The serum IL-6 level in the patient group was 6.66±7.49 pg/ml in the acute attack, while it was 3.08±4.07 pg/ml in the stable period. Amaç: Plevral sıvılar sık karşılaştığımız tanısal ve klinik bir problemdir. Tanı aşamasında ilk basamak eksuda transüda ayırımının yapılmasıdır.Bu çalışmada amacımız transüda eksüda ayırımında sadece plevral mayide bakılan biokimyasal parametrelerin kullanılabilirliğini değerlendirmektir. Yöntem ve Gereç: Hastane kayıtlarından 1 Mayıs 2013-1 Mart 2015 tarihleri arasında plevral sıvı analizi yapılan hastaların verilerine ulaşıldı. İleri tetkik ve invaziv işlemler sonucu son tanısı konanlar çalışmaya dahil edildi. Son tanılar esas alınarak eksuda transüda ayırımında plevral LDH total protein ve ADA için cut-off değerler belirlendi, sensitivite ve spesifiteleri hesaplandı.Light kriterleri ile karşılaştırıldı. Bulgular: Toplamda hastane kayıtlarından dahil edilme kriterlerine uyan 1433 hasta çalışmaya dahil edildi. Son tanıları esasa alındığında hastaların %75.1'i eksuda, %24.9'u ise transuda vasfında sıvıya sahipti. Plevral sıvı LDH için 171 IU/L ve protein için 3.2 g/L cut-off değer olarak ele alındığında eksuda vasfındaki sıvıları ayırt etmede sensitivite, spesifite, PPD ve NPD sırası ile 95.0%, 64.4%,88.9% and 81.0% olarak saptandı. Öte yandan Light kriterleri için eksuda tarnsüda ayırımında sensitivite, spesifite, PPD ve NPD sırası ile %96.0, %75.0, %92.0 and %85.9 olarak tespit edildi. Sonuç: Bu çalışmada eksuda transuda ayırımında belirli cut-off değerlerinde plevral LDH ve protein kombinasyonunun sensitivite ve spesifite değerlerinin Light kriterleri ile karşılaştırılabilir olduğu görülmüştür. Klinik pratikte eksuda transüda ayırımında laboratuar imkanlarının kısıtlı olduğu durumlarda daha kolay ve ucuz olması nedeniyle sadece plevral sıvı parametrelerinin kullanımı akılda tutulabilir., Aim: The pleural fluid often presents diagnostic and clinical problems.The first step in the examination of pleural effusion is to differentiate between transudate and exudate.The aim of this study was to evaluate the clinical utility of standard pleural fluid parameters in the differentiation of transudative and exudative pleural effusion in tuberculosis endemic country. Material and methods: The records of patients who had pleural fluid analysis between 1 May 2013 and 1 March 2015 were electronically obtained.On the basis of the classification according to certain diagnosis after the careful evaluation of clinical data and diagnostic procedures, the cut-off values of pleural lactate dehydrogenease(LDH), total protein(TP) and adenosine deaminase(ADA) were determined. The sensitivity and spesifity of these parameters used in exudate-transudate differentiation were calculated and compared with Lights criteria. Results: Totally 1433 precisely diagnosed patients with appropriate criteria were included in the study. On the basis of certain diagnosis 75.1% of them were exudate and 24.9% of them were transudate. When 171 IU/L was taken as the cut-off value of pleural LDH, the rates of sensitivity, specificity were 85% and over, while 3.2 g/L was taken as the cut-off value of the pleural total protein, the rates of sensitivity and spesifity were between 72 % - 85 %. The most appropriate combination parameters for clinical use were those of LDH at the level of 171 IU/L and total protein at the level of 3.2 g/L. In this combination; the sensitivity, specificity, PPV, NPV in determining the exudative fluids were determined as 95.0%, 64.4%, 88.9% and 81.0% respectively. On the basis of certain diagnosis, the sensitivity, specificity, PPV, NPV of Lights critera in determining the exudative fluids were determined as 96.0%, 75.0%, 92.0 %, and 85.9 % respectively. Conclusion: In this study, the sensitivity and spesifity of pleural LDH and TP at certain cut-off values was found to be comparable with Lights criteria in the exudate-transudate differentiation.In clinical practice, to use only pleural fluid parameters may be considered as easier and simple approach for the differentiation of exudate and transudate when the laboratory facilities are limited.
