A 42-year-old male presented with a 4-week history of a mass in the right inferior palpebral conjunctiva close to the punctum. An excisional biopsy of the lesion and histopathological examination revealed that the mass was composed of Schwann cells with thin conical nuclei, fine chromatin, and unnoticeable nucleoli. Immunohistochemically, the spindle cells were diffusely and strongly positive for S100 protein. Neurofilament immunostaining was also positive, which highlighted axons. In light of these findings, the tumor was diagnosed as solitary circumscribed neuroma. A comprehensive evaluation for multiple endocrine neoplasia type 2b was performed. However, no multiple endocrine neoplasia type 2b stigmata and no family history were detected. The diagnosis was therefore finalized as solitary circumscribed neuroma, which is considered as a rare condition. The differential diagnosis is based on the histopathological examination and immunohistochemical evaluation. As the tumor can be related with multiple endocrine neoplasia type 2b, it is essential to systematically investigate for multiple endocrine neoplasia type 2b in such cases. RESUMO Um homem de 42 anos apresentou uma massa na conjuntiva palpebral inferior direita, próxima ao punctum, com evolução de 4 semanas. Uma biópsia excisional da lesão e o subsequente exame anatomopatológico revelaram que a massa era composta de células de Schwann com núcleos cônicos, cromatina fina e nucléolos não visíveis. Ao exame imuno-histoquímico, as células fusiformes mostraram-se difusa e fortemente positivas para a proteína S100. A imunocoloração também foi positiva para neurofilamentos e evidenciou os axônios. Considerando esses achados, o tumor foi diagnosticado como um neuroma circunscrito solitário. Procedeu-se uma investigação completa para neoplasia endócrina múltipla tipo 2b, entretanto, estigmas característicos e história familiar não foram detectados. Assim, o diagnóstico foi firmado como neuroma circunscrito solitário, condição rara cujo diagnóstico diferencial baseia-se no exame anatomopatológico e na avaliação imuno-histoquímica. Já que esse tumor pode estar relacionado à neoplasia endócrina múltipla tipo 2b, torna-se essencial, nesses casos, a investigação da neoplasia de forma sistemática.

Aim: We aimed to evaluate the epidemiological and survival characteristics of patients diagnosed as melanoma in the database of Ege University Hospital between 1992-2017. Materials and Methods: The data of 1530 patients diagnosed with melanoma enrolled at Ege University between 1992-2017, recorded by the CANREG 4 program by specially trained and certified cancer registry staff of the Ege University Cancer Control Research and Application Center, were collected. While the existence of the relationship between categorical variables was evaluated with the Chi-Square method, the numerical variables between the groups were compared with the One Way Variance Analysis and the Turkey HSD Test. While the effects of categorical variables on survival were examined by Kaplan Meier and Log Rank (Mantel-Cox) methods, Cox-Regression analysis was used for numerical variables. All Hypothesis tests were appliedat 0.05 significance level. Results: Of the 1530 melanoma cases, 53.1% were male and 46.9% were female. The average age was 54.14 ± 16.537. Superficial spreading melanoma and lentigo malignant melanoma were the most common subtypes.11.6% of the melanoma lesions were in situ, 43.6% were localized, 6.7% were locally spread, 16.9% were regionally spread, 21.1% were advanced. The follow-up period of the patients ranged from 0 to 313 months. The mean survival time was 147.4 ± 5.3 months. Purpose: To investigate vitamin D receptor polymorphisms in ocular surface squamous cell neoplasm and to evaluate the relationship between the identified polymorphisms and susceptibility to ocular surface squamous cell neoplasm and the clinical course. Materials and Methods: A totala of 70 patients with ocular surface squamous cell neoplasm (study group) and 75 healthy age and gender-matched individuals (control group) were included in the study. Vitamin D receptor FokI and BsmI polymorphisms were examined. The relationships between histopathological diagnosis, recurrence rates, tumor stage, and identified polymorphisms were investigated. Results: Histopathologically, 43 of the cases were squamous cell carcinoma and 27 of the cases were conjunctival intraepithelial neoplasia. The frequency of FokI (FF, Ff, ff) and BsmI (BB, Bb, bb) polymorphism genotype of vitamin D receptor gene were similar in the groups. The frequency of polymorphism (heterozygous or homozygous) for BsmI (Bb and bb) was significantly higher (p = 0.046) in the study group, while no difference was found between the groups in terms of polymorphic carriers (heterozygous or homozygous) for FokI. There was no correlation between tumor stage, recurrence-polymorphism frequency, and patient age-polymorphism frequency. Conclusion: It is known that active vitamin D inhibits the growth of cancer cells by binding to vitamin D receptor with regulation of genes responsible for cell proliferation. The presence of BsmI polymorphism in vitamin D receptor, in particular bb genotype and b allele, appears to be associated with the susceptibility of ocular surface squamous cell neoplasm. Extramammary Paget’s disease (EMPD) is a rare clinical entity which can be associated with other malignancies. The vulva is the mostly seen presentation in women. Lesions frequently presents in adnexal structures and clinically demonstrate as erythematous, well-demarcated plaques. Early detection is mandatory because the diagnosis is frequently delayed and there is a high incidence of associated invasive disease. With this case, we discuss the management of vulvar Paget's disease which is rarely seen and importance of investigating the accompanying secondary malignancies.

WOS: 000369629100011, Leiomyosarcoma (LMS) commonly arises from smooth muscles of visceral organs, such as uterus, gastrointestinal system and has a poor prognosis due to its metastatic potential. LMS derived from smooth muscles of the skin, named superficial LMS, usually indolent and has a better prognosis. Especially, LMS derived from pilar muscles is usually restricted to the dermis and rarely metastasizes. However, LMS originating from smooth muscles of subcutis behaves more aggressively and metastasizes more commonly. Here, we report a patient with giant superficial LMS on the lower extremity which invaded the underlying tissues like bone, tendons and joint and metastasized to the lung. The patient received chemotherapy following a below-knee amputation.

WOS: 000298720200040, Rheumatoid nodules are the most common extra-articular lesions. They usually occur in rheumatoid factor positive patients and indicate activity and severity of the disease. These nodules characteristically occur subcutaneously on extensor surfaces over the elbows or other bony prominences. They may be overlooked if they occur in sites other than well-known areas. We present a case with rheumatoid arthritis (RA) who developed multiple subcutaneous, histologically-proven rheumatoid nodules in both buttocks as well as on palmar aspects of phalanges after 3.5 years of very low-dose weekly methotrexate (MTX) treatment. These nodules occurred abruptly while there was not active arthritis in any joint. MIX-induced large nodules may occur even with very low doses of MTX, in atypical localizations such as both buttocks.

Background: Eccrine poroma (EP) belongs to the group of benign adnexal tumors, but cases of eccrine porocarcinoma (EPC) arising on long-standing and untreated EP suggest a certain risk of malignant transformation. Objectives: To describe the clinical, dermoscopic and histopathologic features associated with different extremes in the spectrum of eccrine poroid neoplasms and to review the according literature. Methods: A retrospective analysis of patient characteristics and morphologic features associated with EP, EPC in situ and invasive porocarcinoma patients who attended two skin lesion clinics in Italy and Turkey between 2010 and 2011. Results: A total of 4 cases including 1 EP, 1 EPC in situ and 2 cases of invasive EPC in 4 patients were analyzed. Recent changes including bleeding, ulceration and sudden enlargement of a pre-existing lesion were associated with malignant transformation. Dermoscopically, polymorphous vessels consisting of coiled, hairpin and linear vessels were seen at all stages of progression. Histopathological findings of EPC in situ were limited to the epidermal component and were evident only at higher magnification. Conclusions: Clinicians and pathologists should carefully evaluate EPs with a recent history of change and erosion in order to avoid overlooking the potential development of EPC. (C) 2013 S. PubMedID: 22798108 Background/aims: The aim of this multicenter study was to determine the histopathological features and immunohistochemical profiles of gastrointestinal stromal tumors diagnosed in Turkish patients. Material and Methods: Twenty-eight participating centers registered their gastrointestinal stromal tumor cases on a nationwide database. The diagnosis of gastrointestinal stromal tumor relied upon hematoxylin & eosin features and the results of antibody panel including CD117, CD34, desmin, smooth muscle actin, S-100 protein, and Ki67. The database consisted of parameters including age, gender, location, and all other histopathological and immunohistochemical findings. Statistical analysis was performed using Pearson, Kruskal-Wallis, Mann-Whitney U, and Spearman tests. Results: From all of the gastrointestinal stromal tumors in the database, 1160 cases with a male to female ratio of 1.22 and a mean age of 56.75 years were included in the study. The most common location was the stomach (45.0%), followed by the small intestine, omentum-peritoneum, large intestine, and esophagus (32.0%, 12.6%, 9.3%, 1.1%, respectively). The risk groups were distributed as: 6.1% very low, 21.7% low, 19.3% intermediate, and 53% high-risk cases. Many histopathologic findings were correlated with risk groups. CD117 was positive in 95.3% of gastrointestinal stromal tumors, whereas CD34 was positive in 74.9%, smooth muscle actin in 45.9%, desmin in 9.2%, and S-100 in 19.1.%. Though no significant relation was found between CD117 expression and tumor location, CD34, smooth muscle actin and Ki67 expressions significantly varied in different locations (p=0.001) and risk groups. Conclusions: The results of this multicenter study demonstrated that features other than tumor size and mitosis and immune markers other than CD117 and Ki67 included in the antibody panel seem to be useful as predictive risk factors.
