Faced with lockdowns, it was mandatory the development of supervised home-based RT protocols to keep patients with chronic kidney disease engaged in programs. Nonetheless, there is a lack of scientific literature regarding its effects on patients.To investigate the effects of a supervised home-based progressive resistance training program on functional performance, bone mineral density, renal function, endothelial health, inflammation, glycemic homeostasis, metabolism, redox balance, and the modulation of exerkines in patients with CKD in stage 2.Patients (n = 31) were randomized and allocated into the control group (CTL; n = 15; 58.07 ± 5.22 yrs) or resistance training group (RT; n = 16; 57.94 ± 2.74 yrs). RT group performed 22 weeks of supervised progressive home-based resistance exercises. Bone mineral density, anthropometric measurements, and functional performance were assessed. Venous blood samples were collected at baseline and after the intervention for the analysis of markers of renal function, endothelial health, inflammation, glycemic homeostasis, metabolism, and redox balance.Twenty-two weeks of home-based RT were effective in improving (P 0.05) functional performance, bone mineral density, uremic profile, ADMA, inflammatory markers, the Klotho-FGF23 axis, glycemic homeostasis markers, and exerkines. These improvements were accompanied by higher concentrations of exerkines and anti-inflammatory cytokines. RT group displayed a decrease in cases of osteopenia after the intervention (RT: 50 % vs. CTL: 86.7 %; XResults provide new evidence that supervised home-based progressive RT may be a relevant intervention to attenuate the progression of CKD and improve functional capacity, bone mineral density, and the immunometabolic profile. These improvements are associated with positive modulation of several exerkines.

The objectives of this study were to evaluate the use of a qualitative on-farm milk progesterone test to predict non-pregnancy in dairy cows. Lactating Jersey cows (n = 752) were subjected to the 5-d Cosynch-72 protocol for timed artificial insemination (AI; d -8 GnRH, d -3 and -2 PGF2α, d 0 GnRH and timed AI). Milk was sampled on d -3, 0, 7, and 28 relative to timed AI, and progesterone concentrations were assessed using a lateral flow immunochromatographic test. Samples were classified into 3 groups indicative of high (hP4; test line not visible or lighter than reference), intermediate (iP4; test line similar to reference), and low (lP4; test line darker than reference) progesterone concentrations. Blood was sampled from a subset of cows (n = 50) on d -3, 0, 7, and 28 relative to timed AI, and plasma progesterone concentrations were determined by RIA. Cows were observed daily for signs of estrus based on removal of tail paint. Pregnancy was diagnosed by ultrasonography on d 34 and 62 after AI. Plasma progesterone concentrations across all time points were greater for hP4 (3.13 ± 0.20 ng/mL) followed by iP4 (1.12 ± 0.27 ng/mL) and lP4 (0.38 ± 0.23 ng/mL). Cows in lP4 on d -3 had lesser pregnancy per AI (P/AI) compared with iP4 and hP4 (17.4, 38.3, and 37.2%, respectively). For measurements performed on the day of AI (d 0), lP4 cows had greater P/AI compared with hP4 and iP4 (34.8, 0.0, and 15.6%, respectively), and the risk of pregnancy loss tended to be greater for iP4 compared with lP4. Cows in lP4 on d 7 after AI had lesser P/AI than those in iP4 and hP4 (12.0, 34.0, and 37.7%, respectively). Cows classified as lP4 on d 28 had the least P/AI on d 62 followed by iP4 and then hP4 (0.8, 9.2, and 59.4%, respectively) and were at the greatest risk for pregnancy loss (lP4 = 74.6%, iP4 = 8.4%, hP4 = 7.1%). Sensitivity and specificity to predict non-pregnancy on d 62 were 0.86 and 0.32 (d -3), 0.95 and 0.15 (d 0), 0.93 and 0.23 (d 7), and 0.99 and 0.53 (d 28), respectively. On-farm milk progesterone profiling using a lateral flow immunochromatographic test was able to identify cows without functional corpus luteum and to predict fertility outcomes following timed AI.

[EN] The implementation of expansion projects of water networks supplying growing cities is deemed to be a complex decision-making problem involving both technical aspects and expert knowledge. Management and control processes must rely on experts in the field whose know-how must be coupled with techniques able to deal with the natural subjectivity that affects input evaluations. Given the presence of many decision-making elements, the choice of proper hydraulic technical parameters may be linked to the main aspects of analysis requiring formal expert evaluation. In this contribution, the simulation of hydraulic indicators is integrated with a multi-criteria approach able to eventually determine those areas of a water network through which organising the expansion may be more beneficial. The software EPAnet 2.0 is first used for hydraulic simulations, whereas the Technique for Order of Preference by Similarity to Ideal Solution (TOPSIS) will eventually rank network¿s nodes. Introduction Distinguishing between non-Type 1 Brugada pattern (non-T1BrP) and an athlete's ECG remains challenging and may have important prognostic implications. We aimed to study prevalence and the diagnostic yield of experts and non-experts for the electrocardiographic non-T1BrP criteria in the young adults from the Sudden Cardiac Death-Screening Of risk factorS (SCD-SOS) cohort. Methods We performed a cross-sectional study in which we reviewed 14662 ECGs of SCD-SOS survey participants and selected 2494 that presented a rSr'-pattern in V1-V2. Among these, 98 were classified by experts in hereditary arrhythmic syndromes for the presence of non-T1BrP and by non-experts who performed manual measurements of the diagnostic criteria based on triangle formed by r'-wave. We estimated intra and interobserver concordance for each criterion, and used logistic regression and receiver operating characteristics (ROC) analysis and C-statistics for diagnostic accuracy and definition of the most appropriate cut-off values. Results We detected a rSr'-pattern in V1-V2 in 17% of the individuals and found that it was associated with higher PQ and QTc intervals, male gender and lower BMI. The manual measurements of non-T1BrP criteria were reproducible: we had high intraobserver concordance coefficients (CC) ranging from 0.90 to 0.94 (except for d(B) that had 0.66), but interobserver CC were lower (0.45–0.68). The measurements performed were highly correlated with non-T1BrP diagnosis and the criteria with higher discriminatory capacity were the distance d(B) (AUC 0.77; 95% CI0.69–0.84) and the degree of ST-ascent (AUC 0.79; 95% CI 0.72–0.86). The cut-offs defined by other authors had very low sensitivity (8–12%), despite high specificity (98%), so we defined new cut-offs: d(A) ≥2mm, d(B) ≥1.25mm, d(B)/h ≥0.38, β-angle ≥19° and ST-ascent ≥1mm. The addition of the degree of ST-ascent to a model with these 4 parameters presented an increase in C-statistics from 0.77 (95% CI: 0.68–0.86) to 0.83 (95% CI: 0.75–0.91) for the diagnosis of non-T1BrP by an expert in Sudden Arrhythmic Death and Channelopathies. Conclusion A rSr'-pattern in precordial leads V1-V2 is a frequent finding and the detection of non-T1BrP by using the aforementioned 5 measurements is reproducible and accurate. In this study, we describe new cut-off values that may help untrained clinicians to identify young individuals who should be referred for provocative drug testing for Brugada Syndrome. A novel, inexpensive strategy was used to valorize fish canning industry by-products, aiming at ω -3 compounds and biodiesel production. Winterization and urea complexation methods were used to fraction the oil extracted from fish canning industry wastes. Urea complexation was the most efficient method, producing a liquid fraction with 99.99% ( w ∕ w ) of polyunsaturated fatty acid methyl esters (FAME) when methanol was used as solvent, and a solid fraction with 79.94% (w/w) of saturated and monosaturated FAME when ethanol was used as solvent, which was considered a potential feedstock for biodiesel production. The liquid fraction was further analysed by preparative HPLC analysis, producing an oil fraction with 99.44% ( w ∕ w ) ω -3 compounds, containing 89.25% eicosapentaenoic acid (EPA) ( w ∕ w ). The method here described has an enormous potential for fish canning industry waste valorization as it is easily scale-up, foreseeing an environmentally sustainable society.

Ivermectin is a human and veterinary antiparasitic drug which is one of the most widely used in the world. Studies from our group have revealed several behavioral and neurochemical impairments induced by therapeutic doses of ivermectin in adult rats. However, the effects on juveniles remain unknown. Ivermectin has been prescribed for juvenile humans, pets and farm animals, which still show remarkable development and postnatal maturation and may be more susceptible to drug interventions. Hence, we studied the behavioral and neurochemical effects of two therapeutical doses (0.2 and 1.0 mg/kg) of ivermectin in juvenile rats. As it is underestimated in prescriptions, the stress factor was also studied. Ivermectin 1.0 mg/kg induced hyperlocomotion in juvenile rats. Association of 1.0 mg/kg ivermectin with stress induced hypolocomotion in rats. Ivermectin 1.0 mg/kg whether or not associated with stress exacerbated socialization of rats. Ivermectin did not induce anxiety-like behavior neither affected corticosterone levels of juvenile rats. The motor/exploratory behavioral findings induced by association of ivermectin and stress seem to be triggered after the increase in the striatal serotonergic system activity. Association of ivermectin with stress increased striatal dopamine levels, which increased (excessive) social play behavior. Our results suggest a review of the prescribed dose of ivermectin for juvenile humans and pets. Moreover, the stress factor should be considered for ivermectin medical prescriptions, since it may exacerbate behavioral and neurochemical disturbances.

Since the first assembled genomes, gene sequences alone have not been sufficient to understand complex metabolic processes involving several genes, each playing distinct roles. To identify their roles, a network of interactions, wherein each gene is a node, should be created. Edges connecting nodes are evidence of interaction, for instance, of gene products coexisting in the same cellular component. Such interaction networks are called protein-protein interactions (PPIs). After genome assembling, PPI mapping is used to predict the possibility of proteins interacting with other proteins based on literature evidence and several databases, thus enriching genome annotations. Identifying PPIs involves analyzing each possible protein pair for a set of features, for instance, participation in the same biological process and having the same function and status in a cellular component. Here, we investigated using the three categories of the Gene Ontology (GO) database for efficient PPI prediction, because it provides data about the three features exemplified here. For a broader conclusion, we investigated the genomes of ten different human pathogens, looking for commonality regarding the GO hierarchical relationship-denominated IS_A. The ambitious plan to develop and produce the prolific Pre-Salt reservoirs in the Brazilian Santos Basin was kicked off by the discovery of the gigantic Lula Field in 2006 (previously known as "Tupi"). For Petrobras, the first decade of this century was therefore marked by the significant challenges imposed by these complex carbonate reservoirs, requiring the accelerated introduction of new technologies and innovative approaches to well construction. This article describes the history of well construction in the offshore Brazilian Pre Salt area. It summarizes the evolution in well design that resulted from the implementation of innumerable "lessons learned" that were acquired since drilling of the first wildcat well in the Pre-Salt Cluster. The commulative experience of more than 100 wells drilled and 29 wells completed (to date), has resulted in a progressive reductions of time and CAPEX required for the construction of the development wells in the Santos Basin.

Resumen La paralisis facial bilateral simultanea (PFBS) es una rara entidad clinica que generalmente surge como una manifestacion de enfermedades de caracter sistemico, que se diagnostica como paralisis idiopatica de Bell en una pequena proporcion de casos. Entre las causas mas comunes de PFBS estan los traumatismos craneoencefalicos, paralisis de Bell, enfermedad de Lyme, sindrome de Guillain-Barre, sarcoidosis y meningitis bacteriana. Presentamos el caso de una paciente con cuadro de paralisis de Bell bilateral y simultanea.

Simultaneous bilateral facial palsy (SBFP) is an uncommon disorder that usually results from a systemic disease, with only a few cases diagnosed as Bell's Palsy. The most common causes of SBFP are head injuries, Bell's Palsy, Lyme disease, Guillain-Barre syndrome, sarcoidosis, and meningitis. We present a case of SBFP.

RESUMO O cancro do pulmao e a neoplasia mais frequente no sexo masculino nos paises da U.E., mostrando actualmente tendencia crescente em todos os paises. O cancro do pulmao e a maior causa de morte por neoplasia, tanto na E.U. como nos E.U.A. A sobrevida e geralmente curta, sendo a maioria dos doentes diagnosticados em fases avancadas da doenca, dificultando a decisao terapeutica. Para estabelecer a opcao terapeutica ha necessidade de conhecimento mais aprofundado dos factores que podem influenciar a sobrevida, nao so relativos ao doente, mas tambem em relacao ao proprio tumor. O presente estudo insere-se numa avaliacao prognostica global, incluindo caracteristicas do doente e do tumor. Na avaliacao das caracteristicas do tumor utilizamos citometria de fluxo (FCM) que permite avaliar o conteudo e o indice do ADN e a fraccao de calulas em Fase-S. As amostras a processar sao colhidas por tecnicas diagnosticas (biopsia, escovagem e lavagem bronquicas, biopsia aspirativa transtoracica - BAT e aspirado ganglionar), sendo utilizado no processamento o material restante, apos ter sido obtido o diagnostico. Pretendemos nesta investigacao preliminar definir um controlo de qualidade dos especimes a processar por FCM em relacao a presenca ou nao de celulas neoplasicas, atravas de um rastreio citologico. Por outro lado, verificar se e exequivel utilizando amostras tao exiguas obter resultados de FCM flaveis. Estudamos 78 doentes suspeitos de terem neoplasia maligna do pulmao. So nos 43 em que se provou existir neoplasia maligna se procedeu a realizacao de FCM. Obtivemos resultados avaliaveis de FCM em 62% (27/43). As tecnicas de colheita mais reotaveis, apos rastreio citologico, foram a biopsia bronquica (76%) e a escovagem bronquica (100%). Encontramos uma deficiente rentabilidade utilizando a BAT (31%). A citologia e um bom metodo de seleccao para especimes colhidos por biopsia bronquica e escovagem bronquica. A utilizacao desta metodologia parece-nos um bom metodo de controlo de qualidade para seleccionar amostras para realizacao de FCM.

The authors studied the influence on survival of 21 clinical, anatomical, haematological and biochemical factors evaluated, at diagnosis, of 411 patients (pts) with advanced Non Small Cell Lung Cancer (NSCLC) followed in our department between 1984 and 1990. Most of the patients were male (347-84.4%) and only 64 (15.6%) were females. Median age was 62 years, but was slightly higher in females. Only 34 patients were aged under 45 years. Squamous cell carcinoma (215 pts - 52%) and adenocarcinoma (152 pts - 37%) were the most frequent histologic types. Performance status was poor—only 103 (25%) continued active; 120 (29%) spent at least half of the time in bed; 188 (46%) were severely limited. After staging, 179 (44%) presented locally advanced disease (stage IIIB) and 232 (56%) metastatic dissemination (stage IV). Therapy was defined by the oncologic group according to individual characteristics and based on clinical grounds. Anti-neoplasic therapy was performed in 225 (55%), chemotherapy alone in 121 (30%), radiation therapy alone in 67 (16%), and sequential combined treatment (chemotherapy and thoracic radiation) in 37 (9%). Until 1987, the main chemotherapy regimen was MACC (Metrotrexate + Adriamycine + Cyclophosphamide + Lomustin), after-wards VP(M) (Cisplatin + Vimblastin + Mitomycine). Radiation therapy was performed using Co 60 , 2 Gy/day, 5 days a week, for 4 weeks (approximately 45 Gy total). The response rate was poor—four complete responses (2%), 42 (19%) partial responses. The overall median survival was 4.3 months and only 5% of patients were alive after 18 months of follow up. Prognostic importance of each characteristic studied was initially done by unifactorial analysis, followed by multifactorial analysis according to two methods: Cox proportional hazards model and recursive partitioning amalgamation — RECPAM. Regardless of the method used, the main determinants of survival were found to be performance status (Zubrod), weight loss and serum albumin. Other factors such as the staging (presence or absence of metastasis), lymphocytes, lactic dehydrogenase, and hoarseness were also significant. It is noteworthy that age and histological type were irrelevant; sex and hoarseness only proved important when integrated within a multifactorial model. The overall prognostic evaluation and therapeutic decision of advanced NSCLC patients could be improved by combining the prognostic value of TNM with that of performance status, weight loss and serum albumin. These prognostic guidelines must be taken into account when designing new clinical trials.

São apresentadas as principais informações de ordem geográfica do Estado, que é dividido em 23 micro-regiões e 141 municípios. Desses, em 129 foram encontrados triatomíneos, de acordo com informações obtidas em diversos organismos de pesquisa, de 1957 a 1974. A espécie principal no Estado é o Triatoma brasiliensis, semi-doméstico, encontrado nas casas de 91,5% dos municípios (129), suas taxas de infecção variam de 1,0 a 40,4% (média de 8,2%), o P. megistus é a terceira espécie e é encontrado em 61,7% dos municípios (87), com uma taxa de infecção muito variável, com média de 3,9% e máxima de 25,9%. Em áreas restritas como a do Cariri essa taxa subirá para 24,5%. A segunda espécie, menos doméstica que as duas primeiras é o T. pseudomaculata, encontrado em 68,8% dos municípios e taxas de infecção variando de 0,3% a 7,1% e uma média de 4,2%. A quarta espécie é o Rhodnius nasutus, encontrado em 17,7% dos municípios e infectado em 1,0%. Penetrando na casa (não foram encontradas ninfas) há uma quinta espécie, o Panstrongylus lutzi capturado em algumas casas de 26 municípios (18,4%) e com elevadas taxas de infecção: 17,9%. Com taxas globais de triatomíneos infectados por T. cruzi acima de 10% estão 12 municípios, dos quais mais da metade constitui as 4 micro-regiões centrais do Estado: Sertão de Quixeramobim, Sertão de Senador Pompeu, Sertão dos Inhamuns e Médio Jaguaribe.The paper shows some geographical informations about the State of Ceará, Brasil, that is divided in 23 micro-regions and 141 counties. In 129 of these counties were collected triatomines (91,5%), according all the informations published from 1957 to 1974. The main species is the Triatoma brasiliensis, semi-domestic, 1,0 to 40,4% of them infected. The second species is the Triatoma pseudomaculata (68,8% of the counties); the infection rates are between 0,3 and 7,1%. The third species is the Panstrongylus megistus (61,7% of the counties) with rates of intection between 3,9% and 25,9%. The fourth species is the Rhodnius nasutus (17,7% of the counties) infected 1,0%. The last species, Panstrongylus lutzi, a sylvatic, is seen inside houses in 18,4% of the counties, but only adult forms and showing high rates of intection (17,9%). The most infected micro-regions of the State are: "sertão" of Senador Pompeu, "sertão" of Inhamuns and "baixo " Jaguaribe.

Do Ceará, estado com problema de Doença de Chagas, Russas tem recebido a maior soma de investigações, verificando-se 11,6% de infecção humana, em 1959. Recentemente foram examinados cães, gatos e outros animais encontrando-se altas taxas de infecção. Selecionou-se, na presente pesquisa, uma micro-área (Fazenda Boa Vista) na região, onde haviam sido antes examinados 52 cães e 21 gatos, com 11 cães e 10 gatos positivos. Foram examinados mais 12 cães e 10 gatos, com 1 e 5 positivos, respectivamente, elevando-se a taxa de positividade global para 29,5%. Nessa micro-área foram examinados 125 pessoas pela RFC e pelo xenodiagnóstico, encontrando-se uma positividade de 7,2%. Desses positivos, 55% residem em casas com animal positivo, sendo que duas casas apresentaram duas pessoas positivas cada. Do total de xenos somente dois foram positivos. Na micro-área foram capturados 405 triatomíneos, com 27,7% positivos, dentro e fora das casas (galinheiros, currais, paredes externas, telheiros e alpendres). A espécie prevalente é o Triatoma brasiliensis (93,5%), encontrada dentro das casas (52,8%) com taxa de infecção de 30,2% e fora das casas (47,2%) com 24,6% de infecção. No total da região examinada na pesquisa anterior, as taxas de infecção foram 19,6% dentro das casas e 21,8% fora. A segunda espécie é o T. pseudomaculata (5,4%), encontrado mais fora das casas (72,8%) e infectado 25%. Foram capturados 8 exemplares de P. megistus sempre fora das casas, sendo 4 na micro-área; 5 exemplares estavam infectados. Na micro-área, tal como em 2 outras áreas da região, foram encontrados roedores domésticos e marsupiais com T. cruzi: 4 em 10 Rattus rattus e 1 em 3 Didelphis azarae, tal como já havia sido verificado por um dos AA. em outras áreas do Estado. Conclui-se pela existência de um dinâmico ciclo doméstico e paradoméstico de circulação do T. cruzi na micro-área, com abundantes oportunidades de infecção do homem e dos animais domésticos.Russas, as some others regions of the State of Ceará, has receivedsome attention and a great deal of investigation about the Chagas' disease problem. In 1959 the rate of human infection was 11.6%. Recently a survey was made and high rates of infection in dogs and cats were found. Now we selected in the previousiy region a micro-area (Boa Vista Farm) where we had examined 52 dogs and 21 cats and 11 dogs and 10 cats were infected by T. cruzi. In the same area we have examined another 12 dogs and 10 cats and the global rate of Infection in these animais appeared higher than before: 29.5%. In the same micro-area we performed complement fixation tests and xenodiagnosis in 125 pefsons (50% of the population) and found 7.2% of positivy. AH the persons with positive results, except for one person, have in their homes 1 or 2 animais with positive examinations. 405 triatomines were captured in and outside the houses of the area and 27.7% of them were found to be infected. Triatoma brasiliensis was the most prevalent (93.5%) species and it was found inside the houses in a rate of 52.5% with 30.2% rate of infection. T. maculata was the second species in prevaience (5.4%): 72.8% were captured outside of the houses, 25% of them were infected. 8 specimes of P. megistus were captured outside of the houses and 5 of them were infected. 4 rodents out of 10 captured (Rattus rattus) were found to be infected and also 1 out 3 opossum (Didelphis azarae). The Authors have got the conclusion that domestic and para-domestic cycles of transmission are being developed in the micro-area surveyed.
