Die Kalziumoxalat- (CaOx-)Steinbildung ist eine weltweit haufige Erkrankung. Es uberrascht daher, dass die pathogenetischen Mechanismen, die zur CaOx-Steinbildung fuhren, noch weitgehend unbekannt sind. Die lange akzeptierte Hypothese der Steinentstehung durch ein Uberschreiten des Loslichkeitsprodukts lithogener Substanzen im Urin ist aber sicherlich nicht ausreichend, um die komplexen Vorgange zu beschreiben. Abweichend von der bisherigen Annahme einer primar intratubularen Kristallbildung legen neuere Arbeiten zumindest bei einem Teil der Patienten die primare Kristallisation im Interstitium der Papille nahe. Dabei werden initial Kalziumphosphat- (CaPh-)Kristalle sowie organische Matrix an den Basalmembranen der Henle-Schleifen abgelagert. Diese breiten sich anschliesend bis nach unmittelbar suburothelial aus und imponieren dort als sog. Randall-Plaques, wie sie regelmasig endoskopisch bei CaOx-Steinbildnern nachgewiesen werden konnen. Diese CaPh-Plaques scheinen den Ausgangspunkt der spateren CaOx-Steine darzustellen, die sich durch Anlagern weiterer Matrixmolekule und CaOx aus dem Urin an den Plaque bilden. Die treibenden Krafte, die genauen pathogenetischen Mechanismen und die beteiligten Matrixmolekule sind bisher noch weitgehend unbekannt. Moglicherweise stellt die klinische Diagnose „CaOx-Steinbildner“ nur die gemeinsame Endstrecke unterschiedlicher Pathomechanismen dar.

The effects of a low Na medium on PAH transport, renal morphology, and acetate stimulation of PAH transport have been studied. Low Na inhibits PAH uptake and induces vacuolar formation in tubular cells. Both processes are reversible by reincubating in a high Na medium. Vacuolization appears temperature dependent being much greater at 37 than at 25 degrees C. Acetate uptake consists of a passive and an active component, neither of which is sodium dependent. Acetate stimulation of PAH uptake does, however, require Na. This latter observation suggests that Na may act by altering cell metabolism.

The role of tubular secretion in the excretion of tritiated norepinephrine (3H-NE) released from renal symphathetic nerves was examined. The endogenous pool of NE in renal nerves in the superficial cortex of rabbits was labelled with 3H-NE by surface application of the isotope. Release of the 3H-NE was stimulated by surface application of tyramine (T). T significantly increased the urinary excretion of nonmetabolized 3H-NE. The increase was totally prevented by cyanine 863, but was not significantly affected by probenecid. Electrical stimulation of the renal nerves after intraarterial infusion of 3H-NE produced a 9 ± 3% increase (p< 0.025) in the excretion of nonmetabolized 3H-NE, but did not affect the excretion of 3H-metabolites. The data suggest that 3H-NE released from renal neurons can be excreted largely as nonmetabolized 3H-NE by cation-specific tubular secretion.
