Abstract: Background The long-term outcomes of COVID-19 hospitalisation in individuals with pre-existing airway diseases are unknown. Methods Adult participants hospitalised for confirmed or clinically suspected COVID-19 and discharged between 5 March 2020 and 31 March 2021 were recruited to the Post-hospitalisation COVID-19 (PHOSP-COVID) study. Participants attended research visits at 5 months and 1 year post discharge. Clinical characteristics, perceived recovery, burden of symptoms and health-related quality of life (HRQoL) of individuals with pre-existing airway disease (i.e., asthma, COPD or bronchiectasis) were compared to the non-airways group. Results A total of 615 out of 2697 (22.8%) participants had a history of pre-existing airway diseases (72.0% diagnosed with asthma, 22.9% COPD and 5.1% bronchiectasis). Abstract: Summary: Background: The scale of COVID-19 and its well documented long-term sequelae support a need to understand long-term outcomes including frailty. Methods: This prospective cohort study recruited adults who had survived hospitalisation with clinically diagnosed COVID-19 across 35 sites in the UK (PHOSP-COVID). The burden of frailty was objectively measured using Fried's Frailty Phenotype (FFP). The primary outcome was the prevalence of each FFP group—robust (no FFP criteria), pre-frail (one or two FFP criteria) and frail (three or more FFP criteria)—at 5 months and 1 year after discharge from hospital. For inclusion in the primary analysis, participants required complete outcome data for three of the five FFP criteria. Longitudinal changes across frailty domains are reported at 5 months and 1 year post-hospitalisation, along with risk factors for frailty status. Patient-perceived recovery and health-related quality of life (HRQoL) were retrospectively rated for pre-COVID-19 and prospectively rated at the 5 month and 1 year visits. This study is registered with ISRCTN, number ISRCTN10980107. Findings: Between March 5, 2020, and March 31, 2021, 2419 participants were enrolled with FFP data. Mean age was 57.9 (SD 12.6) years, 933 (38.6%) were female, and 429 (17.7%) had received invasive mechanical ventilation. 1785 had measures at both timepoints, of which 240 (13.4%), 1138 (63.8%) and 407 (22.8%) were frail, pre-frail and robust, respectively, at 5 months compared with 123 (6.9%), 1046 (58.6%) and 616 (34.5%) at 1 year. Factors associated with pre-frailty or frailty were invasive mechanical ventilation, older age, female sex, and greater social deprivation. Frail participants had a larger reduction in HRQoL compared with before their COVID-19 illness and were less likely to describe themselves as recovered. Interpretation: Physical frailty and pre-frailty are common following hospitalisation with COVID-19. Improvement in frailty was seen between 5 and 12 months although two-thirds of the population remained pre-frail or frail. This suggests comprehensive assessment and interventions targeting pre-frailty and frailty beyond the initial illness are required. Abstract: Background: In adults central blood pressure has stronger associations with cardiovascular outcomes than peripheral blood pressure, with some evidence that this difference is greater in mid- than older-aged adults. The relationship of central blood pressure to cardiovascular structure and function in adolescence is unclear. Aim: To compare associations of central and peripheral pulse pressure (PP) with cardiac structure and left ventricular function in a general population of adolescents. Methods: 1,421 (17y; 45% males) participants in the Avon Longitudinal Study of Parents and Children had measurements of peripheral and central blood pressure using Omron705 IT and Sphygmocor devices and echocardiography using a HDI 5000 ultrasound system. Results: Central to peripheral amplification was 21 mmHg (95% CI: 20, 21). Central and peripheral PP were positively associated with left ventricular mass index (LVMI), mitral E/A ratio, left atrial size and inversely associated with s′ even when adjusting for age, sex, DEXA determined fat mass and physical activity (Table 1); with consistently greater associations noted for cPP. Neither central nor peripheral PP were associated with relative wall thickness, midwall fractional shortening, ejection fraction e’ or E/e′. Conclusions: Central PP is more strongly associated with measures of cardiac structure and function than peripheral PP in adolescence. Previous studies are likely to have underestimated the effect of PP on cardiac structure and function in children and adolescents based on peripheral measurements. P3.14 Withdrawn by author

Abstract: Background: Central (aortic) blood pressure (BP) indices independently predict cardiovascular events and all-cause mortality, but the physiological mechanisms underlying aortic BP waveform morphology are subject to debate. The ‘aortic reservoir’ and ‘excess pressure’ are proposed as determinants of aortic BP, but this relationship has only been assessed using a mathematically-derived aortic reservoir-excess pressure model (ARderived and XPderived). This study aimed to directly measure the aortic reservoir (ARdirect; by cyclic change in aortic volume) and determine the relationship with ARderived and aortic BP. Methods: Ascending aortic BP and Doppler flow velocity were recorded via intra-arterial wire in 10 males (aged 62±12 years) during coronary artery bypass graft surgery. Simultaneous ascending aortic transesophageal echocardiography was used to measure ARdirect. Published mathematical formulae were used to determine ARderived and XPderived. A direct excess pressure (XPdirect) was calculated by subtracting ARdirect from aortic BP. Results: When normalised to the same scale (Figure A), ARdirect (solid line) was strongly and linearly related to ARderived (broken line) during systole (r=0.980, P0.05) similar to the cyclic relationship between aortic BP and ARderived. Abstract: Background: Dietary inorganic nitrate is reduced to (nitric oxide) NO in the body. Beetroot is a rich source of dietary inorganic nitrate and has been shown to lower brachial blood pressure (BP) by this mechanism. The effect on aortic (central) BP, which may be a better predictor of cardiovascular disease, has not been studied. Objectives: To measure the effect of dietary nitrate in beetroot juice on central BP at both 30 minutes and over a 6 hour period post-beetroot juice consumption. Method: A double-blind, randomised, placebo-controlled crossover trial was performed in nine healthy, normotensive men and women aged between 22 and 45 years. Participants were randomised to receive beetroot juice (6.5–7.3 mmol nitrate) or placebo juice (0.04–0.06 mmol nitrate). Brachial and central BP were measured at baseline, 30 and 60 minutes post-ingestion, and at least hourly for the following 24 hours. Following a washout period, the procedure was repeated within seven days with crossover to the opposite arm of the trial. There were no dietary restrictions during the study. Results: Compared with placebo, beetroot juice lowered central systolic BP at 30 minutes (change in beetroot juice: −2.6±3.4 mmHg vs. change in placebo: 1.8±5.9 mmHg, P=0.045). Beetroot juice also lowered central systolic BP averaged over 6 hours post-ingestion (beetroot: 106±8 mmHg vs. placebo: 111±11 mmHg, P=0.029). Conclusion: Consumption of beetroot juice lowered central BP. Beetroot juice could have a role in CV risk management in the general population, but further research is required to establish its long-term benefits, safety and tolerability.

Abstract: Background: Left ventricular systolic dysfunction, an independent predictor of heart failure (HF) and cardiovascular mortality, may be worse in South Asians and African-Caribbeans (than Europeans) as they have more adverse cardiovascular risk factor profiles. Objectives: We sought to investigate ethnic differences in speckle-tracking echocardiography (STE)-derived strain (ε) and strain rate (SR) in a community-based cohort of European, South Asian and African-Caribbean older men, and the role of CVD risk factors in accounting for these differences. Methods and results: We recruited 339 men (113 from each ethnic group) with echocardiographic data from the Southall and Brent REvisited (SABRE) cohort, and measured peak systolic longitudinal ε and SR. Anthropometric, haemodynamic, fasting blood and conventional and tissue Doppler echocardiographic data were also collected. There were no significant differences in peak systolic longitudinal ε between Europeans (mean±SD, −17.7±3.1%), South Asians (−16.9±2.9%) and African-Caribbeans (−17.1±3.0%) (p=0.200). Arterial stiffness is associated with an increased risk of cardiovascular disease (CVD) and carotid-femoral pulse wave velocity (cfPWV) is considered as the ‘gold standard’ measurement method. However cfPWV does not assess the stiffness of the proximal aorta and its sensitivity at higher levels of arterial stiffness, such as in the elderly, has been questioned. We compared associations between CVD and cfPWV and total arterial elastance (TAE) in a cohort of elderly individuals. Methods. 961 individuals (69±6yrs; 76% male) underwent echocardiography (Philips iE33), cerebral MRI, cfPWV (Pulse Trace) and central blood pressure (Pulsecor) measurements. Central pulse pressure and stroke volume were used to calculate TAE. Coronary heart disease (CHD) was defined as a coronary event or revascularisation identified by medical record review, and adjudicated by an independent committee. Cerebrovascular disease was defined as those with infarcts (2+) on MRI or adjudicated history of stroke. Associations are presented as odds ratios (OR) [95% confidence intervals] by logistic regression. Results. A modest correlation was observed between cfPWV and TAE (spearman’s rho = 0.14). Abstract: Introduction: Intra-aortic balloon pumping is widely used to augment diastolic coronary perfusion. However, far less is known about its effects on other large arteries. In this study we assessed the effects of intra-arterial balloon pump pressure support on pressure and flow in the proximal aorta, carotid, coronary and renal arteries. Methods: Recordings of simultaneous pressure and flow velocity were made using intra-arterial sensor tipped wires. Velocity time integral (VTI) was calculated at each location. Results: With balloon pump support VTI increased in the coronary arteries (458 to 540cm, 15%). In contrast, in the carotid arteries VTI markedly decreased (278 to 228cm, −21%). VTI remained constant in the renal arteries (247cm) and the aorta (225cm). The detrimental reduction in carotid VTI was due to the sudden fall in pressure occurring with balloon deflation, which results in carotid velocity transiently becoming negative (Fig. 1). Conclusion: Intra-arterial balloon pumping augments coronary blood flow, at the expense of diminishing carotid blood flow. Abstract: Background: It is widely believed that changes in the magnitude and timing of reflected waves reaching the heart result in deleterious cardiovascular effects. Inherent in this is the assumption that waves propagate well in the backward direction. There is limited information regarding retrograde travel of the reflected wave from an imposed occlusion. We investigated this. Methods: In this study, 20 subjects (age 31–83 years), underwent invasive measurement of pressure & Doppler velocity with sensor-tipped intra-arterial wires placed in the aorta, iliac artery and femoral artery. An external cuff was inflated to occlude one femoral artery, creating a site of total reflection and the effect of this assessed. Separated wave intensity analysis was used to identify and quantify incident and reflected waves. Wave reflection index (WRI) expresses the backward compression wave as a percentage of the incident forward compression wave. Results: The additional reflection (evident by statistically significant increase in WRI and earlier arrival of the reflected wave) generated in the femoral artery by femoral occlusion was not visible in the proximal aorta. Furthermore, the reflected wave in the proximal aorta occurred in systole in all subjects, much earlier than is widely proposed. Conclusion: A reflection arising from an occlusion is wholly attenuated by the time it reaches the proximal aorta. This lack of retrograde travel could account for the limited reflection in the proximal aorta and prompts review of our understanding of the mechanisms underlying blood pressure augmentation.

Wave intensity analysis can describe the energy transported by wavefronts in the arterial system in the direction of wave propagation. A non-invasive ultrasound-based technique [1] was used to compare the effects of nebivolol and atenolol on carotid arterial wave intensity in patients participating in a hypertension treatment study. Methods 38 hypertensive patients (age 63±11yrs, 21 female) took part in a double-blind randomised cross-over study. All received bendroflumethazide 2.5mg for 6 weeks in advance, and throughout the study. Abstract: Introduction: Anti-hypertensive agents differ in their ability to slow progression of the increase in carotid artery intima-media thickness (IMT) with disease and have different effects on the lumen diameter (LD) of the carotid artery. We hypothesised that changes in flow and shear stress in the common carotid artery may contribute to these differences in remodelling. Methods: 10 treatment naïve hypertensive subjects were recruited into a double blind, placebo controlled, randomised 3-way cross-over study comparing the effects of 7 days treatment with amlodipine or lisinopril. Brachial and carotid blood pressure (BP), common carotid artery (CCA) flow rate, IMT and LD were measured at the end of each treatment period. Magnetic resonance imaging-based computational fluid dynamics was used to calculate time averaged wall shear stress (WSS) in the carotid artery bifurcation. There were significant reductions in brachial and carotid BP with both active treatments compared to placebo, but brachial and carotid BP did not differ significantly between the antihypertensive agents. CCA flow rate was significantly lower and distal vascular resistance was higher following lisinopril treatment compared with amlodipine. WSS on the inner wall of the CCA was significantly lower after lisinopril treatment compared with amlodipine. Conclusion: Amlodipine causes increased carotid blood flow and increased WSS compared with lisinopril, probably as a result of greater cerebrovascular vasodilatation. These effects could account for differences in arterial remodelling caused by these agents.

Background: The scale of COVID-19 and its well documented long-term sequelae support a need to understand long-term outcomes including frailty. Methods: This prospective cohort study recruited adults who had survived hospitalisation with clinically diagnosed COVID-19 across 35 sites in the UK (PHOSP-COVID). The burden of frailty was objectively measured using Fried's Frailty Phenotype (FFP). The primary outcome was the prevalence of each FFP group—robust (no FFP criteria), pre-frail (one or two FFP criteria) and frail (three or more FFP criteria)—at 5 months and 1 year after discharge from hospital. For inclusion in the primary analysis, participants required complete outcome data for three of the five FFP criteria. Longitudinal changes across frailty domains are reported at 5 months and 1 year post-hospitalisation, along with risk factors for frailty status. Patient-perceived recovery and health-related quality of life (HRQoL) were retrospectively rated for pre-COVID-19 and prospectively rated at the 5 month and 1 year visits. This study is registered with ISRCTN, number ISRCTN10980107. Findings: Between March 5, 2020, and March 31, 2021, 2419 participants were enrolled with FFP data. Mean age was 57.9 (SD 12.6) years, 933 (38.6%) were female, and 429 (17.7%) had received invasive mechanical ventilation. 1785 had measures at both timepoints, of which 240 (13.4%), 1138 (63.8%) and 407 (22.8%) were frail, pre-frail and robust, respectively, at 5 months compared with 123 (6.9%), 1046 (58.6%) and 616 (34.5%) at 1 year. Factors associated with pre-frailty or frailty were invasive mechanical ventilation, older age, female sex, and greater social deprivation. Frail participants had a larger reduction in HRQoL compared with before their COVID-19 illness and were less likely to describe themselves as recovered. Interpretation: Physical frailty and pre-frailty are common following hospitalisation with COVID-19. Improvement in frailty was seen between 5 and 12 months although two-thirds of the population remained pre-frail or frail. This suggests comprehensive assessment and interventions targeting pre-frailty and frailty beyond the initial illness are required. Background: Most studies of immunity to SARS-CoV-2 focus on circulating antibody, giving limited insights into mucosal defences that prevent viral replication and onward transmission. We studied nasal and plasma antibody responses one year after hospitalisation for COVID-19, including a period when SARS-CoV-2 vaccination was introduced. Methods: In this follow up study, plasma and nasosorption samples were prospectively collected from 446 adults hospitalised for COVID-19 between February 2020 and March 2021 via the ISARIC4C and PHOSP-COVID consortia. IgA and IgG responses to NP and S of ancestral SARS-CoV-2, Delta and Omicron (BA.1) variants were measured by electrochemiluminescence and compared with plasma neutralisation data. Findings: Strong and consistent nasal anti-NP and anti-S IgA responses were demonstrated, which remained elevated for nine months (p < 0.0001). Nasal and plasma anti-S IgG remained elevated for at least 12 months (p < 0.0001) with plasma neutralising titres that were raised against all variants compared to controls (p < 0.0001). Of 323 with complete data, 307 were vaccinated between 6 and 12 months; coinciding with rises in nasal and plasma IgA and IgG anti-S titres for all SARS-CoV-2 variants, although the change in nasal IgA was minimal (1.46-fold change after 10 months, p = 0.011) and the median remained below the positive threshold determined by pre-pandemic controls. Samples 12 months after admission showed no association between nasal IgA and plasma IgG anti-S responses (R = 0.05, p = 0.18), indicating that nasal IgA responses are distinct from those in plasma and minimally boosted by vaccination. Interpretation: The decline in nasal IgA responses 9 months after infection and minimal impact of subsequent vaccination may explain the lack of long-lasting nasal defence against reinfection and the limited effects of vaccination on transmission. These findings highlight the need to develop vaccines that enhance nasal immunity. Funding: This study has been supported by ISARIC4C and PHOSP-COVID consortia. Background: The risk factors for recovery from COVID-19 dyspnoea are poorly understood. We investigated determinants of recovery from dyspnoea in adults with COVID-19 and compared these to determinants of recovery from non-COVID-19 dyspnoea. Methods: We used data from two prospective cohort studies: PHOSP-COVID (patients hospitalised between March 2020 and April 2021 with COVID-19) and COVIDENCE UK (community cohort studied over the same time period). PHOSP-COVID data were collected during hospitalisation and at 5-month and 1-year follow-up visits. COVIDENCE UK data were obtained through baseline and monthly online questionnaires. Dyspnoea was measured in both cohorts with the Medical Research Council Dyspnoea Scale. We used multivariable logistic regression to identify determinants associated with a reduction in dyspnoea between 5-month and 1-year follow-up. Findings: We included 990 PHOSP-COVID and 3309 COVIDENCE UK participants. We observed higher odds of improvement between 5-month and 1-year follow-up among PHOSP-COVID participants who were younger (odds ratio 1.02 per year, 95% CI 1.01–1.03), male (1.54, 1.16–2.04), neither obese nor severely obese (1.82, 1.06–3.13 and 4.19, 2.14–8.19, respectively), had no pre-existing anxiety or depression (1.56, 1.09–2.22) or cardiovascular disease (1.33, 1.00–1.79), and shorter hospital admission (1.01 per day, 1.00–1.02). Similar associations were found in those recovering from non-COVID-19 dyspnoea, excluding age (and length of hospital admission). Interpretation: Factors associated with dyspnoea recovery at 1-year post-discharge among patients hospitalised with COVID-19 were similar to those among community controls without COVID-19. Abstract: People of African origin have increased risk of stroke and retinal microvascular disease compared with populations of European origin. We compared quantitative measures of retinal microvasculature in British white Europeans and African Caribbeans.Population-based study of 215 (45% male) British African-Caribbean migrants and 323 (48% male) white Europeans aged 40-69 years. Digitised retinal images were analysed using a validated semi-automated system.Arteriolar optimality deviation, an indicator of endothelial dysfunction, was greater in African Caribbeans (age- and sex-adjusted means [95% CIs]: 0.06 [0.05-0.06] vs 0.04 [0.04-0.05], p = 0.004); this was unexplained by conventional risk factors. Arteriolar diameters were narrower in African Caribbeans (age- and sex-adjusted means [95% CIs]: 18.4 [18.1-18.6] vs 17.9 [17.6-18.2], p = 0.011). These ethnic differences in diameters were attenuated on adjustment for systolic BP (SBP) (adjusted means: 18.2 vs 18.1, p = 0.31). However, there was a significant interaction (p = 0.011) between diabetes and SBP, such that SBP was strongly associated with arteriolar diameter in people without diabetes, but not in those with diabetes (adjusted beta-coefficients for SBP: Europeans: -0.42, p = 0.002 vs 0.17, p = 0.69, African Caribbeans: -0.35, p = 0.023 vs 0.01, p = 0.96). Other measures of retinal vasculature did not differ by ethnicity.British African Caribbeans appear to have poorer retinal arteriolar endothelial function than white Europeans. Higher BPs explained the narrower arterioles in African Caribbeans; however, patterns of association between arteriolar narrowing and BP suggest the possibility that cerebral autoregulation and/or remodelling might be adversely affected by diabetes in both ethnic groups.

47. A regional chemostratigraphically-defined correlation framework for the late Triassic TAG-I Formation in Blocks 402 and 405a, Algeria

Abstract: The Triassic Argilo-Greseux Inferieur Formation (TAG-I) is one of the principal hydrocarbon reservoirs in the Berkine Basin of Algeria. Sedimentological studies have shown that it exhibits marked spatial and temporal facies variations on both a local field scale and a regional basinal scale. This variability, combined with a lack of diagnostic flora and fauna, makes regional correlation within the unit difficult. In turn, the lack of a consistent regional stratigraphic framework hampers the comparison of the various correlation schemes devised by operators in the basin. Contrasting the TAG-I in Blocks 402 and 405a exemplifies the problems encountered when attempting regionally to define a correlation framework for the interval. Between these two blocks, a distance of approximately 200 km, there are marked changes in the style of deposition from sand-dominated, proximal fluvial systems in the SW (Block 405a, MLN, MLC, KMD and MLNW fields) to a more distal, more clay-prone system in the NE (Block 402, ROD/BRSE/BSFN, SFNE and BSF fields). A chemostratigraphic study of the TAG-I in these two blocks has allowed a four-fold correlation framework to be defined, where each chemostratigraphic package has distinctive geochemical features. Chemostratigraphic Package 10, the oldest unit, lies above the Hercynian Unconformity, but beneath a geochemically identifiable hiatal surface. Chemostratigraphic Package 20 lies above the hiatal surface but is separated from the overlying packages by a mineralogical change identifiable in both claystone and sandstone geochemistry. Chemostratigraphic Packages 30 and 40 are chemically somewhat similar, but are separated by a regional event interpreted as a period of dolocrete and lacustrine development. By combining the geochemical differentiation of the units and recognition of their stratal boundaries, it is possible to define a correlation for the TAG-I between Blocks 402 and 405a. The proposed correlation between the two blocks suggests that the northern parts of Block 405a may have been occupied by a spur or subsidiary channel from the main SW–NE-trending fluvial system, resulting in one of the chemically defined packages being demonstrably absent in the MLNW, MLN, KMD and MLC fields when compared with the other areas of the study.

Abstract: Contraction induced by hyposmotic swelling was examined in rat tail arteries mounted on a myograph containing a modified Krebs physiological saline solution (PSS) containing 50 mM mannitol (300 mosm/l). Hyposmotic swelling was induced by removing mannitol. In arteries having basal tone or arteries precontracted with K+ or the thromboxane mimetic U-46619, removal of mannitol caused a concentration dependent contraction of rat tail arteries. Concurrent measurement of tension and intracellular calcium [Ca2+]i in arteries loaded with fura-2 showed that both tension and [Ca2+]i increased on exposure to a hyposmotic solution. Removal of endothelium or inhibition of nitric oxide and cyclooxygenase together did not affect contractile responses. Removal of extracellular Ca2+ abolished the contractile response to hyposmotic solution and NiCl2, a nonspecific inhibitor of Ca2+ influx pathways, blocked the rise in [Ca2+]i and tension in response to a hyposmotic solution. Verapamil and nisoldipine, inhibitors of Cav1.2 (L-type) calcium channels significantly reduced the contractile response to a hyposmotic solution. Addition of NiCl2 to nisoldipine caused an additional inhibition of the response to a hyposmotic solution. Inhibition of calcium release from the sarcoplasmic reticulum by ryanodine or cyclopiazonic acid (CPA) did not cause any change in the tension response to a hyposmotic solution. CPA did not significantly inhibit the response to a hyposmotic solution in the presence of NG-methyl-L-arginine, oxyhaemoglobin and indomethacin. We conclude that contraction induced by a hyposmotic solution is largely due to Cav1.2 calcium channels although other Ca2+ influx pathways also contribute.

Abstract: Ojectives: carotid endoluminal intervention is an alternative to urgery but carries a risk of embolic stroke even with distal rotection devices. We investigated the clinical features and degree of stenosis related to number and size of emboli during carotid angioplasty. Design: an experimental ex vivo study. Materials: an ex vivo pulsatile flow model was used in which temperature, velocity, flow, pressure and viscosity characteristics were designed to simulate the carotid circulation. Methods: carotid endarterectomy specimens excised as intact cylinders ( n = 28) were subjected to a standardised angioplasty procedure using radiological guidance. Emboli collected in filters placed distally were counted and sized using microscopy. Results: median number of emboli during angioplasty was 133 (range 15-1331). Median size of the largest embolus was 700 microns (range 75-2400). Severity of stenosis correlated with increased maximum size ( r = 0.55, p = 0.012). Statin therapy >4 weeks pre-operatively was associated with reduced emboli number and size (54 (range 15-748) vs 247 (range 37-1331) [ p = 0.023] and 400 μm (range 75-2400) vs 1300 μm (range 600-2200) [ p = 0.022]). Conclusions: in this model a wide range of emboli number and size were produced. Number and size of embolic particles were highest in patients with high-grade stenoses not receiving statin therapy.
