The review presents data on the use of hypnotic techniques in anesthesiology and neurosurgery. The authors describe the historical data, methodology of hypnotic communication, modern use of hypnosis, and methods of hypnotic communication in clinical practice. Hypnotherapy techniques have shown excellent results in pain management and can be effective in reducing preoperative anxiety and intraoperative surgical stress.

After discharge from the Intensive Care Unit (ICU), more than 50 % of patients experience pathological symptoms that are not related to the primary emergency but reduce the quality of life and require rehabilitation. Post Intensive Care Syndrome (PIСS) include only those conditions that do not have a direct causal relationship with the emergency condition that gave rise to hospitalization in the ICU. The pathophysiological basis of the PICS is the phenomenon of “learned non-use”: a state of artificial limitation of the patient’s motor and cognitive activity as a result of the use of analgosedation, bed rest and immobilization. The clinical picture of PICS is determined by the severity of its individual components, detailed using a package of clinimetric scales. Based on the results of dynamic testing, the PICS severity index is calculated. The sum of the scores in the range from 0 to 10 reflects the severity and effectiveness of rehabilitation measures. For the prevention of PICS, the Union of Rehabilitologists of Russia together with the Federation of Anesthesiologists and Resuscitators of Russia has developed the rehabilitation complex “ReabICU”. In the English-language literature, such a complex is called “Awakening and Breathing Coordination, Delirium monitoring/management, and Early exercise/mobility” bundle. ReabICU is a complex of technologically therapeutic and diagnostic modules “positioning and mobilization”, “prevention of dysphagia and nutritional deficiency”, “prevention of emotional and cognitive impairment and delirium”, “prevention of loss of self-care skills”. The basis of the multidisciplinary rehabilitation team, which includes a doctor for medical rehabilitation, 2 specialists in physical rehabilitation, a specialist in occupation therapy, a medical psychologist, a medical speech therapist, a nurse for medical rehabilitation. The activity of multidisciplinary rehabilitation team is evaluated on the basis of quality criteria and the achievement of the main goal of ReabICU — the preservation of the premorbid status of the patient’s socialization.

Introduction. Patients must be fully informed about their disease, about different variants of the treatment of their disease, complications and prognosis to make decisions to accept the treatment. The most important they should be able to remember this information.The aim of the study was studying the level of learned information by patients about the upcoming surgery “removal of vestibular schwannoma”. Materials and methods. 44 patients with vestibular schwannoma were included in this prospective study. Average age was 39,8 + 7,2 years, gender distribution was 16 males and 28 females. Education: secondary special – 56,82, and higher – 43,18 %. We used scales for evaluation “Checklist. Surgery to removal of vestibular schwannoma” developed by authors, State-Trait Anxiety Inventory (STAI) in the adaptation by Y.L. Hanin, assessment of depression – The Hospital Anxiety and Depression Scale by Zigmond A.S., Snaith R.P. (HADS). R-Studio (Version 1.0.1532009–2017 RStudio) was used for statistical analisis.Results. Patients were able to remember correctly 24,8 % of medical information on the next day after consultation by a neurosurgeon on average. We got negative correlation between anxiety, depression to the level of information assimilated by patients (r = –0,52; r = –0,47; r = –0,85, respectively). The higher level of anxiety and depression before surgery led to decrease remembering of medical information by the patients.Conclusion. The level of remembering of the medical information by the patient about their disease was low. Quantity of the remembering information depends on the level of anxiety and depression of patients. It is necessary to increase remembering of medical information by patients because of it is allowing to improve the results of treatment.

This paper presents the key data on the effectiveness and safety of the regional anesthesia of the scalp, also known as the scalp block, for providing perioperative analgesia for supratentorial craniotomy. The authors describe the technique and its limitations and also trace the implementation of the scalp block method into the routine practice of the largest Russian neurosurgical clinic based on personal experience, results of the own research, and analysis of literature data.

BACKGROUND: Postoperative pain is a significant problem in patients that causes various complications. AIM: To evaluate the efficacy and safety of prolonged epidural analgesia using disposable elastomeric pumps in elderly patients undergoing spinal neurosurgery and its comparison with the traditional method of postoperative analgesia in 80 patients. MATERIALS AND METHODS: Hemodynamic parameters were recorded and the efficiency of pain relief and nature of postoperative complications were assessed in comparable groups. The concentration of interleukin-6 (IL-6) and IL-2 were determined. RESULTS: In the group with prolonged epidural analgesia with 0.2% ropivacaine solution, an adequate level of analgesia was achieved in 92% of the patients. The revealed changes in the blood serum levels of cytokines IL-6 and IL-2 may indicate a balanced response of the immune system in the group with prolonged epidural analgesia. CONCLUSIONS: Prolonged epidural analgesia using disposable elastomeric pumps provides complete pain relief for patients and correction of individual links of the surgical stress response.

The review presents the methodological recommendations of the Federation of Anesthesiologists and Resuscitators for the management of patients receiving long-term antithrombotic therapy, revised in 2021. The classification of complications and the degree of hemorrhagic risk, stratification of perioperative thromboembolic risk are presented. Taking into account modern concepts, the principles of interruption and resumption of antithrombotic, anticoagulant and antiplatelet therapy are described. Approaches to the emergency interruption of antithrombotic therapy during urgent surgical interventions, including emergency reversion of the effects of drugs received by the patient, are presented. The features of the appointment and changes in the tactics of antithrombotic therapy depending on the thromboembolic and hemorrhagic risk in such concomitant diseases and conditions as the presence of artificial heart valves, atrial fibrillation, ischemic stroke, myocardial infarction, coronary stenting are described. A separate section is devoted to thrombohemorrhagic safety when performing regional anesthesia against the background of antithrombotic therapy. Indications, contraindications and limitations for regional anesthesia in the context of antithrombotic therapy are given. Criteria for the quality of the provided medical care are given.

Chronic disorders of consciousness (DOC) develop after severe traumatic and non-traumatic brain damage and are characterized by the restoration of wakefulness in a patient after a coma without the recovery of consciousness. To optimize the diagnosis and treatment of patients with chronic DOC, a Russian working group on the problems of chronic DOC was organized, which included specialists in various areas, primarily anesthesiologists, critical care physicians and neurologists. While discussing the terminology of chronic DOC, the group identified that currently there is no definition for the state that falls into the period from the recovery of wakefulness and until 28 days after the brain damage when vegetative state/unresponsive wakefulness syndrome (VS/UWS) or minimally conscious state (MCS) may be diagnosed. In the intensive care unit (ICU) setting, there is often no consultant to provide critical care physicians with the correct diagnosis of the latter clinical syndromes, and neurophysiological tests are not feasible either. Therefore, there is a need to create a set of simple, understandable and easily reproducible strategies for managing this category of patients in the ICU. Thus, the working group proposed the term “prolonged disorders of consciousness” to be used for the patients with the signs of VS/UWS or MCS syndromes during their stay in the ICU until 28 days after initial brain damage and/or until the correct differential diagnosis of a type of chronic DOC is made. With the introduction of prolonged disorders of consciousness definition, the regular ICU staff will better understand how to provide an optimal set of supportive therapy and early rehabilitation activities in the lack of specific diagnostics techniques and dedicated specialists. Allocation of this category of patients allows us to create an algorithm for their better diagnosis and management and ensures consistent and effective interdisciplinary care at various levels. On the one hand, this approach will help us allow to free up ICU beds that are in high demand, while on the other, it will maximize the opportunity to realize the rehabilitation potential of DOC patients due to timely transfer to specialized centers. Conclusion. If the term “prolonged disorder of consciousness” is accepted by professional communities of specialists (critical care physicians, neurologists, neurosurgeons, etc.), it will be used in guidelines for the management of DOC patients.

This article is analysis of state of the art «blood reinfusion» in surgical patients, based on literature analysis and self-experience. Main topics are indication and contraindication, modern cell-savers, possible complications and utilization of blood reinfusion in different fields of surgery.

The review presents the clinical guidelines of the Federation of Anaesthesiology and Resuscitation specialists of Russia, revised in 2018. The recommendations are based on a review of publications and modern international guidelines of the British Hematology Standards Committee (BSH, 2016), the European Society of Cardiology in partnership with the European Society of Cardiothoracic Surgeons (ESC and EACTS, 2018), the American Society of Regional Anesthesia and Pain (ASRA, 2018), The European Society of Anesthesiologists (ESA, 2010), as well as a consensus document of an interdisciplinary group of experts of the Russian Federation. Perioperative management of patients receiving long-term antithrombotic (antiplatelet and / or anticoagulant) therapy may be problematic, as they often need to interrupt antithrombotic therapy before surgery. In this, it is necessary to maintain a balance between the effectiveness of antithrombotic therapy and its safety by minimizing the frequency of both hemorrhagic and thrombotic complications. The recommendations presented in the review are aimed at achieving the strategic goal of perioperative antithrombotic therapy — minimizing the risk of bleeding subject to the minimum necessary antithrombotic potential.

The paper is devoted to comparison of two drugs from the group of alpha-adrenoagonist: clonidine and dexmedetomidine, in the context of their impact on the quality of awakening in patients with neurosurgical profile. In the study of two groups of patients, the authors did not reveal a clear advantage or disadvantage of each drug, both of them were appropriate for the successful solution of the tasks.

Central neurocytomas of the brain are rare benign tumors of the cerebral lateral ventricles. The main treatment for them is surgical resection. Resection provides a long-term relapse-free period, but surgical intervention is associated with a number of significant difficulties due to the location, size, and blood supply features of these tumors. The postoperative period is often accompanied by hemorrhagic complications, impaired cerebrospinal fluid circulation, and worsening of neurological symptoms.The study purpose was to evaluate the effectiveness of surgical treatment in neurocytoma patients, assess the risk of complications after neurocytoma resection, and develop techniques for their prevention.The paper presents surgical treatment outcomes in 115 patients with central neurocytomas for the period from 2008 to 2017. The choice of a surgical approach and the surgical features are described in detail, and the radicality is assessed with allowance for the location and size of tumors. The immediate clinical outcomes of treatment are evaluated, complications are analyzed, and methods for prevention and treatment of complications are described.Analysis of the data revealed that the tumor was resected totally in 41 (36%) patients, subtotally - in 37 (32%), and partially in 37 (32%). The most common and dangerous complications were as follows: CSF circulation occlusion with the development of hydrocephalus in 23 (20%) patients; hemorrhage into the residual tumor, which required immediate revision of the surgical wound in most cases, in 25 (22%) patients. In the early postoperative period after tumor resection, almost all patients developed deterioration of condition with transient worsening of cerebral and focal symptoms. The degree and duration of this worsening were different. In 50% of cases, the postoperative condition was characterized by reduced voluntary activity, drowsiness or jitteriness, and motor or speech anxiety.We consider resection of neurocytomas as the main treatment option, despite the potential complications and effects of the operation.Центральные нейроцитомы головного мозга относятся к редко встречающимся доброкачественным опухолям боковых желудочков головного мозга. Основным методом их лечения является хирургическая резекция. Удаление позволяет добиться длительного безрецидивного периода, однако хирургическое вмешательство сопряжено с рядом значительных трудностей, что определяется локализацией, размерами и особенностями кровоснабжения этих опухолей. Послеоперационный период часто осложняется развитием геморрагических осложнений, нарушением ликвороциркуляции и нарастанием неврологической симптоматики. Цель исследования - оценка эффективности хирургического лечения пациентов с нейроцитомами головного мозга; оценка риска развития осложнений после удаления нейроцитом головного мозга и разработка методов их профилактики. Материал и методы. В работе представлены результаты хирургического лечения 115 пациентов с центральными нейроцитомами головного мозга в период с 2008 по 2017 г. Детально описаны выбор хирургического доступа, особенности операций, проведена оценка радикальности с учетом локализации и размеров опухолей. Оценены ближайшие клинические результаты лечения, проанализированы осложнения и показаны методы их профилактики и лечения. Результаты. Анализ данных показал, что радикальное удаление выполнено у 41 (36%) пациента, субтотальная резекция - у 37 (32%) и частичное удаление - у 37 (32%). Самыми частыми и опасными осложнениями были нарушение ликвородинамики по окклюзионному типу с развитием гидроцефалии у 23 (20%) пациентов, кровоизлияния в остатки опухоли, в большинстве случаев явившиеся показанием к немедленной ревизии операционной раны, у 25 (22%) пациентов. После удаления опухоли в раннем послеоперационном периоде практически у всех больных наблюдалось ухудшение состояния с преходящим нарастанием общемозговой и очаговой симптоматики. Степень и продолжительность этого нарастания различались. В 50% случаев послеоперационное состояние характеризовалось снижением произвольной активности, сонливостью либо тревожным состоянием, двигательным или речевым беспокойством. Выводы. Мы предлагаем рассматривать удаление нейроцитом головного мозга как основной метод лечения, несмотря на возможные осложнения и последствия операции.

Dexmedetomidine is a high-selective α2-agonist, which is used as an anesthetic component, and it is expected to induce polyuria. Initially, it was mainly used for sedation in emergency wards, and further this medicine got widespread use in anesthesiology. The major positive effect of dexmedetomidine is related to its central sympatholytic action and suppression of the activity of sympathetic nervous system. This medicine also provides a favorable hemodynamic profile. However, it has a number of certain disadvantages. The present clinical study reports development of acute polyuria syndrome due to the administration of dexmedetomidine during spinal neurosurgery.

This clinical case demonstrates specific features of anesthetic management during radiosurgery in pediatric patients considering the limited access to them during continuous manipulations with pharmacological sedation. Considering the age of the child, dexmedetomidine, the drug from the group of agonists of α 2 -adrenergic receptors, was used as the main agent. It was used since it could be administred by the noninvasive way for premedication and intravenously in a form of infusion to provide the necessary depth of sedation with preservation of spontaneous breathing and minimization of uncontrolled motions of the patient.

The article presents the results of a prospective randomized double-blind study which was investigating the efficacy of intra-operative transcutaneous stimulation of median nerve on the wrist aimed to prevent post-operative nausea and vomiting after planned resection of brain tumors located in posterior fossa. Methods. 40 patients were included into the study: 20 patients were enrolled into the stimulation group and 20 of them were enrolled into the control group, where the prevention of emetogenic reactions was limited by the use of dexamethasone and ondansetron. Results. The demographic and clinical parameters were compatible in the both groups. The review presents the most recent data on the causes, frequency, risk factors, prevention methods, and principles of management of post-operative nausea and vomiting. The special attention is paid to specific aspects of this type of adverse reactions to anesthesia in the neurosurgical practice. The review concludes that it is still necessary to search for better methods of prevention and management of post-operative nausea and vomiting in the group of neurosurgical patients considering the specific risk factors.

Nowadays, in the age of rapid introduction of digital and fiber-optic technologies in neurosurgery, a technique for removal of tumors in the chiasmatic-sellar area via an endoscopic transsphenoidal endonasal approach is actively developed; this technique is considered less invasive and is well tolerated by patients, thus permitting to operate patients with somatic complications, as well as the elderly. Taking into account these facts, as well as world statistic data indicating a continuous trend of population aging in developed countries, including Russia, optimization of the perioperative care of elderly patients with a tumor of the chiasmatic-sellar area becomes the problem of highest priority. In order to solve it, this review discusses the basic peculiarities of the perioperative management of elderly patients (characteristics of their somatic status and possible variants of the pre-operational state) with a pathology in the chiasmatic-sellar region; it also presents the modern and most acceptable alternative solutions of this difficult problem (introduction of modern methods of anesthesia, management of the postoperative cognitive dysfunction, postoperative pain syndrome, and postoperative nausea and vomiting).

Background: Surgery on posterior cranial fossa (PCF) and pineal region (PR) carries the risks of intraoperative trauma to the brainstem structures, blood loss, venous air embolism (VAE), cardiovascular instability, and other complications. Success in surgery, among other factors, depends on selecting the optimal patient position. Our objective was to find associations between patient positioning, incidence of intraoperative complications, neurological recovery, and the extent of surgery.Methods: This observational study was conducted in two medical centers: The Ohio State University Wexner Medical Center (USA) and The Burdenko Neurosurgical Institute (Russian Federation). Patients were distributed in two groups based on the surgical position: sitting position (SP) or horizontal position (HP). The inclusion criteria were adult patients with space-occupying or vascular lesions requiring an open PCF or PR surgery. Perioperative variables were recorded and summarized using descriptive statistics. The post-treatment survival, functional outcome, and patient satisfaction were assessed at 3 months.Results: A total of 109 patients were included in the study: 53 in SP and 56 in HP. A higher proportion of patients in the HP patients had >300 mL intraoperative blood loss compared to the SP group (32 vs. 13%; p = 0.0250). Intraoperative VAE was diagnosed in 40% of SP patients vs. 0% in the HP group (p < 0.0001). However, trans-esophageal echocardiographic (TEE) monitoring was more common in the SP group. Intraoperative hypotension was documented in 28% of SP patients compared to 9% in HP group (p = 0.0126). A higher proportion of SP patients experienced a new neurological symptom compared to the HP group (49 vs. 29%; p = 0.0281). The extent of tumor resection, postoperative 3-months survival, functional outcome, and patient satisfaction were not different in the groups.Conclusions: The SP was associated with, less intraoperative bleeding, increased intraoperative hypotension, VAE, and postoperative neurological deficit. More HP patients experienced macroglossia and increased blood loss. The authors report resection of anaplastic convexital meningioma in a middle-aged woman complicated by expected massive blood loss. The most intense bleeding occurred at the final stage of resection and it was impossible to stop it with traditional approaches. The surgeon pressed a standard tachocomb plate moistened with a diluted solution of recombinant activated factor VII (coagil, Russia) to the most bleeding area for 5 minutes. Subsequently, surgeon replaced finger pressure with a permanent napkin. Hemostatic effect of recombinant activated factor VII following its systemic administration is well known and convincingly proven in many surgical areas including neurosurgery. However, we do not know any descriptions of its local application in neurosurgical patients.В статье приведено описание клинического наблюдения удаления анапластической конвекситальной менингиомы у женщины средних лет, которое осложнилось ожидаемой массивной кровопотерей (около одного должного объема циркулирующей крови). Наиболее интенсивным операционное кровотечение было на финальном этапе удаления опухоли, и остановить его традиционными подходами не удавалось. Тогда к наиболее кровоточащему участку операционной раны оперирующий хирург прижал на пять минут стандартную плоскую губку препарата тахокомб, смоченную разведенным раствором препарата рекомбинантного активированного фактора свертывания крови VII (коагил-VII, АО «Генериум», Россия), в последующем заменив пальцевое прижатие постоянным с помощью салфетки. Гемостатический эффект активированного фактора VII при его системном введении хорошо известен и убедительно доказан во многих областях хирургии, в том числе и в нейрохирургии. Однако описания его локального применения у нейрохирургических больных нам неизвестны.

Awake craniotomy (AC) has gained fantastic popularity over the past years. This approach is no longer the destiny of only highly specialized neurosurgical centers. Technical features of AC are completely developed. However, certain aspects of patients' sensations and their satisfaction are still unclear. The review is devoted to these issues. It was shown that AC is positively evaluated by the vast majority of patients. Many patients would choose this technique for redo surgery. However, there are certain important details that can adversely affect satisfaction of patients. Thus, these features should be considered in AC.Методика краниотомии в сознании (КС) за последние годы приобрела фантастическую распространенность и теперь уже не является уделом только высокоспециализированных нейрохирургических центров. Методологическая сторона КС разработана достаточно полно, чего нельзя сказать о таком аспекте, как исследование ощущений оперируемых пациентов и их общей удовлетворенности. Анализу этой стороны проблемы и посвящен приводимый обзор литературы, в котором показано, что процедура КС положительно оценивается подавляющим большинством пациентов. Многие из них выбрали бы эту методику при повторных вмешательствах. Однако есть ряд клинически важных деталей, которые могут негативно влиять на общую удовлетворенность пациента КС, и их следует учитывать.

Mapping of effective speech connections between the frontal and temporal lobes with cortico-cortical evoked potentials.There were 3 patients with brain tumors in the left frontoparietal region. The neoplasms were localized in the dominant hemisphere near cortical speech centers and pathways. Cortico-cortical evoked potentials were intraoperatively recorded in response to bipolar stimulation with a direct current delivered through the subdural electrodes (single rectangular biphasic impulses with duration of 300 μs and frequency of 1 Hz). Stimulation intensity was gradually increased from 2 mA within 3-4 mA. Registration was carried out by averaging ECoG (30-50 stimuli in each session) in the 300-ms epoch after stimulus. Direct cortical stimulation was used to validate the results of cortico-cortical speech mapping with cortico-cortical evoked potentials.In our cases, we obtained cortico-cortical evoked potentials from inferior frontal gyrus after stimulation of superior temporal gyrus. In one case, this effective relationship was unidirectional, in the other two patients reciprocal. Mean latency of N1 peak was 65 ms (range 49.6-90 ms), mean amplitude 71 µV (range 50-100 µV). Cortico-cortical mapping data were confirmed by detection of Broca's area in 2 out of 3 cases out during direct cortical stimulation with maximum amplitude of N1 wave. «Awake craniotomy» protocol was applied. In one case, Broca's area was not detected during direct stimulation. No postoperative speech impairment was noted.Initial results of cortical mapping with cortico-cortical evoked potentials in a small sample confirmed its practical significance for analysis of cortical projections of effective speech communications between the frontal and temporal lobes. Further study of this method in large samples is required.Картирование эффективных речевых связей между лобной и височной долями головного мозга методом кортико-кортикальных вызванных потенциалов.В исследование включены 3 пациента с внутримозговыми опухолями левой лобно-теменной области. Опухоли располагались в доминантном по речи полушарии, в непосредственной близости к корковым речевым центрам и речевым проводящим путям. Интраоперационно проводилась регистрация кортико-кортикальных вызванных потенциалов (ККВП), возникающих в ответ на биполярную электростимуляцию постоянным током с двух прилежащих контактов субдуральных электродов: одиночные прямоугольные бифазные импульсы длительностью 300 мкс и частотой 1 Гц. Интенсивность стимуляции поднималась постепенно с 2 мА, в пределах 3—4 мА. Регистрацию ККВП осуществляли путем усреднения вызванных ответов (30—50 стимулов в каждой сессии) с эпохой анализа электрокортикограммы 300 мс, начиная с момента стимула. Для валидации результатов кортико-кортикального картирования речевых зон по методу ККВП использована прямая электрическая стимуляция коры головного мозга.В представленных клинических случаях удалось получить ККВП с нижней лобной извилины при стимуляции верхней височной извилины. В одном случае такая эффективная связь была однонаправленной, в двух других — реципрокной. Средняя латентность пика N1 составила 65 мс (49,6—90 мс), средняя амплитуда — 71 мкв (50—100 мкв). Результаты кортико-кортикального картирования подтверждены нахождением зоны Брока методом прямой стимуляции коры по протоколу «краниотомия в сознании» под электродами с максимальной амплитудой волны N1 в двух случаях из трех. В одном случае зону Брока методом прямой стимуляции обнаружить не удалось. Нарушения речевой функции в послеоперационном периоде по результатам нейропсихологического обследования не было.Первые результаты использования метода регистрации кортико-кортикальных вызванных потенциалов у небольшого числа больных показали его практическую значимость в определении корковых проекций эффективных речевых связей между лобной и височной долями головного мозга. Необходимо дальнейшее изучение представленного метода на большей серии клинических случаев.

We present an analytical review of various neurosurgical interventions in conscious patients. An analysis of the literature indicates growing interest in this problem. Craniotomy in conscious patients has been extensively used in resection of space-occupying cerebral lesions in the eloquent hemispheric areas and in epilepsy surgery. In recent years, there have been a number of reports on interventions in conscious patients with other neurosurgical pathologies, which may be regarded as a new emerging tendency in neurosurgery and neuroanesthesiology. Neurosurgery in conscious patients provides a special advantage because it enables highly functional neuromonitoring without use of complex devices.Представлен аналитический обзор различных нейрохирургических вмешательств в условиях сохраненного во время операции сознания у оперируемого больного. Анализ литературы свидетельствует о все возрастающем интересе к этой проблеме. Методика краниотомии в сознании широко используется при удалении объемных образований головного мозга из функционально важных зон больших полушарий, хирургии эпилепсии, а за последнее время появился ряд сообщений об операциях, проводимых с сохраненным сознанием и при другой нейрохирургической патологии, что может быть расценено как формирующаяся новая тенденция в нейрохирургии и нейроанестезиологии. Особое преимущество дает проведение нейрохирургических вмешательств у больных в сознании с позиции нейромониторинга, позволяя проводить его без привлечения сложных технических устройств и на наиболее высоком функциональном уровне.

A neurosurgical intervention in a patient on dual antiplatelet therapy is a serious challenge for both the neurosurgeon and anesthesiologist..The article describes a clinical case of a successful urgent neurosurgical intervention (ventriculoperitoneostomy for obstructive hydrocephalus caused by a large meningioma of the posterior surface of the petrous pyramid) in a patient on dual antiplatelet therapy (DAT) due to a recently placed coronary stent..Given a high risk of coronary stent thrombosis, the surgery was performed in the presence of ongoing DAT. There were no intracranial hemorrhagic complications, but subcutaneous hemorrhagic complications developed. The article discusses the features of managing similar patients whose number is growing.Нейрохирургическое вмешательство у пациента, получающего двойную дезагрегантную терапию (ДАТ), представляет серьезную проблему как для нейрохирурга, так и для анестезиолога. Материал и методы. Описано клиническое наблюдение, в котором достаточно срочное нейрохирургическое вмешательство (вентрикулоперитонеостомия по поводу окклюзионной гидроцефалии, обусловленной большой менингиомой задней поверхности пирамиды височной кости) было успешно произведено больному, получающему ДАТ в связи с недавно установленным коронарным стентом. Заключение. Учитывая высокий риск тромбоза коронарного стента, операция была произведена на фоне продолжающейся ДАТ. Интракраниальных геморрагических осложнений удалось избежать, но подкожных - нет. Обсуждаются особенности ведения таких пациентов, которых становится все больше.

An extended endoscopic endonasal approach is increasingly used in surgical treatment of space-occupying skull base lesions. The international literature reports only 20 cases of surgical treatment for fibrous dysplasia (PD) of the skull base using the endoscopic endonasal approach. We present our experience with the endoscopic endonasal approach in surgical treatment for giant fibrous dysplasia of the skull base, spreading to the right orbital cavity and nasopharynx.A 26-year-old male patient presented with cranial pain, Vth nerve dysfunction on the right, right keratopathy. OD=0.2 (near acuity - 0.3), OS=1.0, OD - incomplete eyelid closure of 2 mm, conjunctival injection, mucous discharge, corneal opacity in the lower pole and paracentrally, OS - normal appearance. Severe right-sided exophthalmos (more than 15 mm), impaired nasal breathing on the right, nasal (hemorrhagic) discharge. Magnetic resonance imaging and spiral computed tomography scans revealed a bone density lesion located in the area of the orbit, nasal cavity, maxillary sinus on the right, and labyrinth of the ethmoid bone. The patient underwent endonasal endoscopic resection of the lesion.The lesion was resected totally, which was confirmed by control SCT. Right-sided exophthalmos partially regressed (on the right: exophthalmos of 8 mm; protrusion: OD=23 mm, OS=15 mm; the eyeball was displaced downward and outward). The visual and oculomotor functions did not change. The neurological status remained at the preoperative level.Fibrous dysplasia of the skull base is an extremely rare disease. Modern techniques expand the indications for surgery of giant tumors of the skull base using minimally invasive approaches, in particular the endoscopic endonasal approach.Расширенный эндоскопический эндоназальный доступ находит все большее применение в хирургическом лечении объемных образований основания черепа. В мире описано всего 20 случаев оперативного лечения фиброзной дисплазии основания черепа (ФД) с использованием эндоскопического эндоназального доступа. Представляем наш опыт оперативного лечения эндоскопическим эндоназальным доступом гигантского патологического очага фиброзной дисплазии основания черепа, распространяющегося в полость правой орбиты и носоглотку. Клинический случай. Мужчина, 26 лет, с краниальным болевым синдромом, недостаточностью V нерва справа, кератопатией справа. OD=0,2 (вблизи — 0,3), OS=1,0, OD — несмыкание век 2 мм, конъюнктивальная инъекция, слизистое отделяемое, в нижнем полюсе роговицы и парацентрально помутнение роговицы, OS — спокоен. Выраженный правосторонний экзофтальм (более 15 мм), нарушение носового дыхания справа, выделения (геморрагического характера) из носа. На магнитно-резонансных томограммах и спиральных компьютерных томограммах выявлено образование костной плотности в области орбиты, полости носа, верхнечелюстной пазухи справа, лабиринта решетчатой кости. Произведено эндоскопическое эндоназальное удаление патологического очага. Результаты. Удаление патологического очага тотальное, что подтверждено контрольной СКТ. Частичный регресс правостороннего экзофтальма (справа — экзофтальм 8 мм, выстояние OD=23 мм, OS=15 мм, глазное яблоко смещено книзу и кнаружи). Зрительные и глазодвигательные функции без динамики. Неврологический статус на дооперационном уровне. Заключение. Фиброзная дисплазия основания черепа является крайне редким заболеванием. Современные технологии позволяют расширять показания к оперативному лечению гигантских новообразований основания черепа с использованием минимально инвазивных доступов, а именно эндоскопического эндоназального доступа.

The study purpose was to present a clinical case of spinal stroke in a pregnant female, which was caused by an endodermal cyst of the cervical spinal cord, and to analyze treatment tactics.A 20 week pregnant female presented with acute transverse spinal cord injury at the of C3-C5 spinal segment level. CT revealed an extramedullary space-occupying lesion in the ventrolateral position, with compression of the spinal cord at this level. The patient in the state of progressive deterioration with respiratory failure was transferred to the Neurosurgical Institute on the 5th day after disease onset. The patient underwent surgery on the 7th day after disease onset. Doctors of various specialties participated in preparation for surgery. During surgery, total resection of the space-occupying lesion and spinal cord decompression were performed. An obstetrician-gynecologist conducted intraoperative fetal monitoring by ultrasound. The histological diagnosis was an endodermal cyst. There was no improvement of neurological symptoms in the early postoperative period. After stabilization of the condition, the patient was discharged for follow-up care at the place of residence. According to the follow-up report, the patient underwent the cesarean section because of exacerbation of lung infection and a significant delay in the fetal development. After a few days, the patient died due to multiple organ failure. The child was alive, in serious condition, under mechanical ventilation.In the case of spinal stroke, the decision on treatment tactics should be made no later than 12 hours after its onset; otherwise, the outcome is usually unfavorable, and a neurological deficit is irreversible. The decision about continuing pregnancy should be made individually in each case, and an approach to the choice of appropriate treatment tactics should be multi-disciplinary.Острое нарушение спинального кровообращения у беременной - редко встречающаяся патология несет серьезную опасность для жизни и здоровья пациентки и плода. Цель исследования - продемонстрировать клинический случай острого нарушения спинального кровообращения у беременной, причиной которого послужила эндодермальная киста шейного отдела спинного мозга, с разбором тактики лечения. Результаты. У беременной со сроком гестации 20 нед остро развилась клиническая картина поперечного поражения спинного мозга на уровне сегментов позвоночника С3-С5. При КТ выявлено экстрамедуллярное объемное образование вентролатерального расположения, с компрессией спинного мозга на указанном уровне. Переведена в НИИ нейрохирургии на 3-и сутки от начала болезни в состоянии прогрессирующего ухудшения, с нарушением дыхания. Операцию удалось выполнить на 7-е сутки от начала болезни. В подготовке к операции принимали участие врачи разных специальностей. В ходе операции удалось тотально удалить объемное образование и декомпримировать спинной мозг. Интраоперационный мониторинг состояния плода осуществлялся акушером-гинекологом с помощью ультразвукового исследования. Гистологический диагноз: эндодермальная киста. В раннем послеоперационном периоде улучшения неврологической симптоматики не было. После стабилизации состояния переведена для долечивания по месту жительства. По катамнестическим сведениям в связи с обострением инфекционного процесса в легких и существенной задержкой внутриутробного развития плода выполнено кесарево сечение. Спустя несколько суток пациентка скончалась вследствие полиорганной недостаточности. Ребенок жив, находится в тяжелом состоянии, на ИВЛ. Заключение. При развитии острого нарушения спинального кровообращения решение о тактике следует принимать не позднее первых 12 ч после его начала, в противном случае исход, как правило, неблагоприятен, а неврологический дефицит - необратим. The paper presents the author's analysis of the main trends in the development of modern neuroanesthesiology over the past five to ten years. These, in the author\s opinion, include the introduction and elaboration of blood-sparing techniques, monitoring the depth of anesthesia, fast track concept, applying regional (conduction) anesthesia techniques, xenon anesthesia, development of effective and safe protocols for DVT and PTE prophylaxis for neurosurgical patients, study of the hemostatic system using bedside methods for assessing hemostasis (thromboelastogram) and correcting hypocoagulation by activated recombinant VII factor.В работе приведен авторский анализ основных тенденций развития современной нейроанестезиологии за последние 5-10 лет. К числу последних, по мнению автора, относятся внедрение и развитие современных кровесберегающих технологий, мониторинг глубины анестезии, концепция фаст-трека, применение регионарных методик анестезии, ксеноновая анестезия, разработка эффективных и безопасных протоколов профилактики тромбоза глубоких вен и тромбоэмболии легочной артерии для нейрохирургических больных, исследование системы гемостаза с помощью прикроватных методов и коррекция гипокоагуляции с помощью препаратов активированного рекомбинантного VII фактора.

Intraoperative relaxation of brain tissue is one of the important factors affecting the quality and successful outcome of neurosurgical interventions. Despite many clinical studies on the problem of brain bulging, many issues remain not fully resolved. First of all, these are safety aspects of the preventive and therapeutic use of various strategies to fight this phenomenon, development of indications for introduction of hyperosmotic solutions, and use of hyperventilation or lifting of the head end of the operating table.Интраоперационная релаксация мозговой ткани - один из важных факторов, влияющих на качество и успешный исход нейрохирургических операций. Несмотря на множество клинических исследований, посвященных проблеме набухания головного мозга, многие вопросы остаются решенными не до конца. Прежде всего это аспекты безопасности профилактического и терапевтического использования тех или иных стратегий борьбы с этим феноменом, формирование показаний к началу введения гиперосмотических растворов, использование гипервентиляции или подъема головного конца операционного стола.

Postoperative nausea and vomiting (PONV) can induce brain displacement and herniation, especially in patients with cerebral edema.To evaluate the urgency of the problem associated with postoperative nausea and vomiting in current clinical practice (with modern approaches being used for its prevention) and to reveal the risk factors of PONV that are typically encountered in neurosurgical patients.A prospective observational study involved 240 patients who had undergone elective surgeries at the N.N. Burdenko National Scientific and Practical Center for Neurosurgery between July and November 2017. The data were collected from the questionnaires filled out by the patients during the first 48 h after the surgery and from patients' medical records.The overall rate of PONV was 39.6%. Thirty-six out of 53 (68%) patients developed PONV after the posterior fossa surgeries. The risk of PONV in this group was significantly higher (p0.05) compared to the rate of PONV after interventions at a different location. The rate of PONV after treatment of extracranial pathology was ~10.5%; for a different location, it was as high as 32-37%. Intraoperative dexamethasone was used in 156 (65%) patients; in this group, the rate of PONV was 39.9%. Patients received ondansetron at a dose of 8 mg for a preventive purpose at the end of the surgery. A total of 162 patients were given the drug; 59 (36.4%) of them developed POTV during 48 h post-administration. Seventy-eight patients did not receive ondansetron. Thirty-six of them (46.2%) (p0.05) developed POTV. The rate of POTV assessed during the first 8 h after surgery was 22.8% in patients who had received ondansetron and 37.2% in those who had not received it (p0.05). Patients who had not intraoperatively received a combination of these drugs developed POTV in 55 (45%) cases (p0.05).The problem associated in POTV remains topical in neurosurgery. The current approaches are not absolutely effective for prevention of POTV, whose rate ranges between 10.5 and 68% depending on surgery location. Further studies focused on administration of NK-1 receptor antagonists and electrical stimulation of the median nerve are needed to enhance the effectiveness of POTV prevention.Послеоперационная тошнота и рвота (ПОТР) могут провоцировать дислокацию и вклинение мозга, особенно в условиях отека. Цель исследования - оценить актуальность проблемы ПОТР в условиях использования современных подходов к ее профилактике, выявить факторы риска ПОТР, характерные для пациентов нейрохирургического профиля. Материал и методы. В обсервационное проспективное исследование включены 240 больных, перенесших плановые вмешательства в Центре нейрохирургии в июле - ноябре 2017 г. Материалом для исследования послужили результаты анкетирования больных в течение первых 48 ч после операции, а также данные историй болезни. Результаты. Частота развития ПОТР в общей группе составила 39,6%. После операций на задней черепной ямке ПОТР развилась у 36 (68%) из 53 пациентов. В сравнении с частотой после вмешательств при другой локализации риск развития ПОТР после операций на задней черепной ямке достоверно выше (p0,05). При лечении экстракраниальной патологии частота ПОТР достигает 10,5%, при патологии иных локализаций - 32-37%. Дексаметазон в операционной использовали у 156 (65%) пациентов, в этой группе частота ПОТР составила 39,9%. Ондансетрон с профилактической целью вводили в конце операции в дозе 8 мг. Препарат получили 162 больных, ПОТР в первые 48 ч возникла у 59 (36,4%) из них. В 78 случаях ондансетрон не вводился, и ПОТР отмечена в 36 (46,2%) случаях (p0,05). При применении ондансетрона частота ПОТР в первые 8 ч после операции составила 22,8%, без применения ондансетрона - 37,2% (p0,05). Среди тех, кому при операции комбинация вышеназванных препаратов не назначалась, ПОТР развилась у 55 (45%) пациентов (p0,05). Заключение. Проблема ПОТР в нейрохирургии остается актуальной. ПОТР встречается с частотой от 10,5 до 68% в зависимости от области операции. Эффективность современных подходов в ее предотвращении неабсолютна. Необходимы дальнейшие исследования, направленные на повышение эффективности профилактики ПОТР, с применением антагонистов NK-1 рецепторов и электростимуляции срединного нерва.

Juvenile nasal angiofibroma (JNA) is a benign, fibro-vascular, locally aggressive tumor with invasive growth patterns and high probability of recurrence. JNA present exclusively in adolescent boys between 9 and 19 years of age, total about 0.05% of head and neck tumors. 143 patients with JNA had been treated surgically from 2002 to August 2018 at N.N. Burdenko National Medical Research Center for Neurosurgery. All patients (primary and recurrences) were males of the age 7-38 years (16.1 [11.3; 19] years). The age of patients with primary JNA was less than 18-year old, of the exception of presenting case. There are more and more publications about case reports and successful treatment series of JNA. However, after detailed review the literature we revealed some kind of confusion: there are case reports of JNA in women and elderly people, JNA in patients with some systemic diseases, JNA with different places of origin and different morphological structure. In this paper we present a very rare case of 38 year-old male with pure primary JNA, accidentally diagnosed on CT and pathologically proven after endoscopic removal. This case confirms the possibility of detecting the JNA in adult men and explains why proper diagnosis of angiofibromas is necessary. The unique case and the literature review are presented.Юношеская ангиофиброма основания черепа (ЮАОЧ) - это редкая быстрорастущая доброкачественная сосудисто-фиброзная опухоль, характеризующаяся локальным деструирующим ростом и высоким процентом рецидивов. Она встречается в 0,05% случаев всех опухолей головы и шеи, у пациентов подросткового возраста (9-19 лет) [1-3]. В НМИЦ нейрохирургии им. акад. Н.Н. Бурденко накоплен большой опыт лечения пациентов с ЮАОЧ: с 2002 г. по август 2018 г. были прооперированы 143 пациента. Все пациенты как с первичной опухолью, так и с ее рецидивом были мужского пола, в возрасте от 7 до 38 (16,1 [11,3; 19]) лет. Возраст первичных пациентов не превышал 18 лет, за исключением одного пациента, у которого ЮАОЧ впервые была выявлена в возрасте 38 лет. В последние годы появляется все больше публикаций успешного лечения пациентов с данной патологией. Однако при ближайшем рассмотрении оказалось, что в литературе отмечается некая путаница: описания случаев юношеских ангиофибром у женщин, а также у мужчин зрелого и пожилого возраста, ангиофибром различной локализации, при различных системных заболеваниях и с разным гистологическим строением. В данной статье мы приводим редкое клиническое наблюдение пациента 38 лет с истинной первичной ЮАОЧ, случайно обнаруженной при плановом обследовании по данным КТ. После эмболизации сосудов опухоли пациент был успешно прооперирован эндоскопически. Диагноз был подтвержден гистологически и иммуногистохимически. Приведенное наблюдение подтверждает возможность обнаружения истинной ЮАОЧ у взрослых мужчин и объясняет, почему необходима правильная диагностика ангиофибром.

The paper discusses the problem ofpredicting, prevention and therapy of massive intraoperative blood loss in patients with metastasis in spine and spinal cord. We analyze 60 surgical cases in last 14 years in our clinic. Amount of blood loss was more that 80% of total blood volume in each case (from 2.5 to 17 liters). Preoperative selective angiography data on intensity of tumor blood supply were essential for blood loss prediction. Simultaneous embolization oftumor during angiography dramatically reduced intraoperative blood loss. Combination of blood saving techniques (preoperative autodonation, acute normovolemic hemodilution and intraoperative cell salvage) led to effective compensation of blood volume deficit and minimizing of allogenic blood transfusion. Plasma-derived and recombinant factors were effective in management of hemostatic disorders associated with massive blood loss.

The autonomic nervous system (ANS) provides homeostasis due to the innervation of the secretory glands, smooth muscle and cardiac muscle. Higher centers of the ANS (primarily the hypothalamus, some centers of the brain stem and limbic system) form a integrative network, which plays a key role in coordinating the functioning of the endocrine, immune system and other parts of the central nervous system. Intracranial centers of the ANS are responsible for the consciousness, behavioral, emotional, and other components of the higher nervous activity. Thus, the significance of the ANS can't be overestimated. At the same time today in neurointensive care there are no clear criteria for ANS dysfunction, we don't have universally recognized monitoring facilities for ANS and approaches to targeted therapy of its disorders. This paradox is even more important as in the pathogenesis of some critical conditions such as neurogenic pulmonary edema, stunned myocardium, cardiomyopathy Takotsubo lies precisely ANS imbalance. This review devoted to the ANS and some problems associated with its imbalance.

The concept of fast-track is one of the most discussed trends in the development of modern surgery. According to supporters of this ideology, the use of fast-track leads to the reduction ofhospital stay without increasing in readmission, reducing the frequency of postoperative complications, which is reflected in the decrease in the cost of the treatment and social expenses, due to earlier return of patients to normal life. The authors of this review discuss different anesthetic issues of fast-track approach (early awakening, postoperative analgesia, prevention of PONV; neuromuscular block reversion). The paper contains also valuable reasons for using fast-track in neurosurgery, as well as contraindications to the use of this approach.

Awake craniotomy is a "gold standard"for intraoperative brain language mapping. One of the main anesthetic challenge of awake craniotomy is providing of optimal sedation for initial stages of intervention. The goal of this study was comparison of different technics of anesthesia for awake craniotomy. Materials and methods: 162 operations were divided in 4 groups: 76 cases with propofol sedation (2-4mg/kg/h) without airway protection; 11 cases with propofol sedation (4-5 mg/kg/h) with MV via LMA; 36 cases of xenon anesthesia; and 39 cases with dexmedetomidine sedation without airway protection. Results and discussion: brain language mapping was successful in 90% of cases. There was no difference between groups in successfulness of brain mapping. However in the first group respiratory complications were more frequent. Three other technics were more safer Xenon anesthesia was associated with ultrafast awakening for mapping (5±1 min). Dexmedetomidine sedation provided high hemodynamic and respiratory stability during the procedure.

The article describes a rare clinical case of a patient with previously undiagnosed von Willebrand disease and basal meningioma; an intracranial neurosurgical intervention was complicated by delayed intracranial hematomas, both at the resected tumor site and distantly. The diagnosis of von Willebrand disease was established only after special hematology tests and only after surgery. Despite the use of specific therapy, the patient died due to intracranial hemorrhagic complications in the postoperative period. The paper discusses the problem of preoperative diagnosis of asymptomatic hemostasis disorders in neurosurgical patients and potential ways of its solution.Дано описание редкого клинического наблюдения: у пациента с ранее не диагностированной болезнью Виллебранда и базальной менингеомой проведенное интракраниальное нейрохирургическое вмешательство осложнилось формированием отсроченных интракраниальных гематом как на месте удаленной опухоли, так и дистантно. Диагноз болезни Виллебранда был установлен только в результате специального исследования в гематологической лаборатории и только после операции. Несмотря на применение специфической терапии, пациент погиб от интракраниальных геморрагических осложнений в послеоперационном периоде. В работе обсуждаются проблема предоперационной диагностики скрытых нарушений гемостаза у нейрохирургических больных и возможные пути ее решения.
