The aim of the study was to assess the effect of hemoadsorption with CytoSorb on the inflammatory response, respiratory failure, and mortality in patients with severe novel coronavirus infection.Materials and methods. A retrospective single-center cohort comparative study of hemoadsorbtion using the CytoSorb therapy included data from 124 COVID-19 ICU patients. Patients were divided into two groups: the study arm with hemoadsorption (group 1, N=93) and the control arm without hemoadsorption (group 2, N=31). Patients in group 1 had more severe respiratory failure at baseline, but were otherwise comparable to patients in group 2 in terms of clinical and demographic parameters.Results. After hemoadsorption, group 1 patients showed significant improvement in 9 of 13 monitored clinical, instrumental, and laboratory parameters: fever (P=0.005), lactate dehydrogenase (LDH) (P

Comparative studies of economic systems have been gaining popularity in recent years. The existing methodology for comparing quantitative indicators of the functioning of the economies of different countries has a number of limitations. The presented article discusses methods for comparing the purchasing power of monetary units of the economies of different countries, and also analyzes the possibility of using them to carry out a comparative analysis of economic indicators and indicators of the development of industries in the USSR and modern Russia in order to develop proposals for improving the state economic and industrial policy. The analysis of methods of comparing the purchasing power of monetary units made it possible to substantiate the choice of approaches for comparing production, technological and financial and economic indicators of the development of industries in the USSR and the Russian Federation. Based on the selected approaches of comparing monetary units, the conversion coefficients for various historical periods were calculated: the USSR (from 1940 to 1990) and the Russian Federation (from 2000 to 2020).

We present a case of mechanical hemolysis as a complication of extracorporeal membrane oxygenation (ECMO) occurring in a COVID-19 patient as a result of pump head thrombosis. After emergency extracorporeal circuit replacement, hemoadsorption was initiated to address the negative hemolysis effects and plasma free hemoglobin rise in the setting of rapid clinical deterioration and impaired renal function. During therapy hemolysis severity reduced, the lactate dehydrogenase (LDH) levels decreased, while the P/F ratio increased two-fold. The patient was discharged from hospital on day 54 without the need for either oxygen therapy or dialysis. In the discussion section we addressed frequent issues of choosing therapy for ECMO complications.Conclusion. The timely, properly chosen, and clinically relevant use of hemoadsorption combined with advanced high-technology therapeutic procedures can have a positive impact on the patient's outcome.

The feet have an increased load, they are often injured, especially with diabetes. Because of the violation of innervation, patients do not notice minor injuries: abrasions, bruises, cracks, cuts. Because of the circulatory disorders associated with diabetes, the protective function of tissues decreases, and the smallest injury can lead to the formation of a long-term wound that does not heal, which turns into an ulcer when infected. Despite advances in the study of the pathogenesis of diabetes and its complications, diabetic foot ulcers are difficult to treat, can recur, and lead to amputations of the lower limbs. The main reason for pathological dryness of the foot skin in diabetes is dysfunction of the sweat glands of the skin of feet due to diabetic neuropathy. Regular skin care with specially selected products can help restore and improve the barrier function of the skin, as well as increase the moisture level of the stratum corneum. Adequate skin hydration reduces itching and the risk of infection if the skin is damaged. In most patients with diabetes, conventional cosmetics slowly eliminate dry skin of the feet. In addition, traditional remedies contain a large amount of oils and can cause unpleasant sensations, which often causes rejection of their use. In patients with DM, it is desirable to use drugs that quickly and effectively eliminate dryness and cracks. Treatment of dry skin is indicated as a prevention of diabetic foot syndrome. Taking into account the fact that the skin of patients with diabetes mellitus has a low level of humidity, we recommend daily care using moisturizing or nourishing creams or lotions. Some studies have confirmed that the use of external dry skin care products containing dexpanthenol improves the protective function of the skin, increasing the hydration of the stratum corneum.

The paper considers some existing numerical estimations of controllability and reacability; their disadvantages are stated from the point of view of application of these estimations to solve the control system synthesis tasks. The existing indices do not take into account the compromise character of requirements to the control systems which include not only a good control response attack but weak dynamic response as well. To estimate these features it is necessary to use different quantitative and structural indices which reflect the interaction of external effects and system reactions. The author has proposed to form on the basis of gramians of controllability and reachability two types of «input-state» quantitative indices from the terminal control tasks and the tasks of synthesizing weak-pertubable systems. The similar approach may be implemented as well on the basis of structural characteristic application.
