Introduction. Polymorphic variants of interferon receptor genes determine cell sensitivity to interferons and the antiviral immune response.The study aimed to assess the impact of polymorphic variants of interferon receptor genes on humoral immunity to influenza and frequency of acute respiratory viral infections; taking into account vaccination status.Methods. We conducted a cross-sectional study involving adult residents of Arkhangelsk who do not receive vaccination against influenza; N=156; and those who receive vaccination annually; N=70. The research procedure included a survey; assessment of concentration of immunoglobulin G to influenza A virus and determination of specific polymorphic variants of interferon receptor genes.Results. Among those receiving vaccination annually; the proportion of seropositive individuals was higher (70;0%) compared to the non-vaccinated group (53;8%). In non-vaccinated individuals with the TT genotype of the polymorphic marker rs2229207 in the IFNAR2 gene; specific immunoglobulins G to influenza A virus were determined more frequently. The proportion of participants receiving annual influenza vaccination who experienced acute respiratory viral infections more than twice a year was 21;4%; while this proportion in non-vaccinated individuals was 32;7%. Among those vaccinated against influenza annually; the odds of frequent acute respiratory viral infections were four times higher in individuals with the GGTC haplotype (rs2257167 IFNAR1 + rs2229207 IFNAR2) compared to other haplotypes.Conclusion. The associations between polymorphic variants of interferon receptor genes and humoral immunity to influenza and frequency of acute respiratory viral infections can be used to identify risk groups and for the development of personalized approaches for the specific prevention of viral infections.

Background. Setting of norms for the parameters of P300 cognitive auditory evoked potentials (EP) in elderly people with intact cognitive functions considering their residence in certain climatic and geographical regions is an urgent problem.The aim of the study. To determine age-related parameters of P300 cognitive auditory evoked potentials in elderly people aged 60–69 and 70–74 years, living in the European North of Russia (using the example of Arkhangelsk).Methods. The parameters of P300 auditory EP were determined in randomly selected urban residents in the age groups of 60–69 years (n = 284) and 70–74 years (n = 115) with normal scores on the Montreal Cognitive Assessment Scale (MoCA), without depression (according to Beck Depression Inventory), with preserved ability to work and/or social functions. We calculated the 5th–95th percentile values (P5– P95) of the P300 EP parameters and assessed the relationships of these parameters with socio-demographic characteristics, lifestyle and the results on the MoCA scale and Beck Depression Inventory.Results. Statistically significant differences in latency indicators of P300 EP were determined between groups of 60–69 and 70–74 years (P25–P90) in all studied brain regions (frontal, central). In the group of 60–69 years, the range of P25–P75 values of P300 EP latencies was 342.5–401 ms, in the group of 70–74 years – 358.5–443 ms. Age differences in P300 EP amplitudes were minimal with an interquartile range of 4–13 μV in the total sample. Participants who smoked had higher latency scores and lower amplitude scores; former smokers had higher latency scores compared to never-smokers.Conclusion. Latency above 400 ms at the age of 60–65 years and above 443 ms at 70–74 years can be considered as a criterion for reduced cognitive reserve and an increased risk of developing cognitive disorders in elderly people living in the European North of Russia.

Aim. To evaluate the usage of antihypertensive drugs (AHDs) and their combinations in participants aged 35 to74 years with arterial hypertension (AH) in the population-based study ESSE-RF3.Material and methods. Representative samples of the population aged 35 to 74 years from 15 regions of Russia (n=28731) with a response rate over 70% were examined in the ESSE-RF3 study. Therapy received by 9944 participants with AH (with systolic BP ≥140 mm Hg and/or diastolic BP ≥90 mm Hg, or when the subject was taking AHDs) was analyzed. Information about AHDs intake (brand name of the drug) was recorded by questionnaire and coded according to International Nonproprietary Names by classes. Statistical analysis was performed using the open-source R 4.1 environment. Comparison of discrete indicators between groups was performed using Fisher’s exact test. The significance level for all tested hypotheses was taken as.05. The study was approved by the Ethics Committee of FGBI “NMRC TPM” of the Ministry of Health of the Russian Federation, each participant signed an informed consent.Results. Among the patients receiving therapy for AH, angiotensin-converting enzyme inhibitors (ACEIs) were used by 38.8% of participants, angiotensin receptor blockers (ARBs) — 31.6%, betablockers (BBs) — 29.0%, сalcium channel blockers (CCBs) — 21.5%, diuretics — 18.6%, 1.1% — outdated AHDs; 8.6% — other groups of drugs. Monotherapy was used by 53.1% of patients, 33.1% of participants received two, and 13.9% received three AHDs. Among participants taking two or more AHDs (including single-pill combinations (SPC)), males most often received the combination of BB+ ACEI and females — BB+ARBs. SPC AHDs were used by 10.3% of those receiving therapy (males: 9.8%, females: 10.6%). Among SPCs, the top three combinations were CCBs + ACEIs (28%), diuretics + ACEIs (27.5%), and diuretics + ARBs (24.4%).Conclusion. The population study ESSE-RF3, based on the survey of a representative sample of the Russian population aged 35-74 years, showed that more than a half of participants with AH receiving therapy were used the monotherapy, only every tenth of those treated received SPC. The problem of insufficient patients’ literacy was indicated — about 1% of patients received outdated AGPs. In addition, 8.6% of patients used non-AHDs for the treatment of AH. For improving the control of AH treatment, it is necessary to increase the adherence of patients to the prescribed therapy and more strict adherence of doctors to the published guidelines for AH treatment.

Introduction. Reperfusion (both pharmacological and interventional) is the cornerstone of acute ischemic stroke (AIS) management. Currently, there are only limited data in the scientific literature regarding the influence of systemic hemodynamic parameters on the recanalization status and complications of AIS.The objective of our study was to assess the relationship between the systemic hemodynamic parameters in the early postreperfusion period and the outcomes of AIS.Materials and methods. For retrospective analysis, data of 292 patients who received intravenous thrombolysis (IVT) at the Arkhangelsk Regional Stroke Center between 2010 and 2021 were collected. The main endpoints were functional outcome, mortality, and symptomatic intracranial hemorrhage (sICH). The success of reperfusion was defined as the neurological improvement by 4 or more points of the NIHSS score within the first 24 hours after the start of IVT.Results. The relationship between mean systolic blood pressure (SBP) during the first 24 hours of IVT and functional recovery was obtained: B = 0.016 (β = 0.096) [95 % CI 0.000 – 0.031] (р = 0.49). The relationship between maximal SBP during the first day of IVT and functional recovery was also determined: B = 0.009 (β = 0.102) [95 % CI 0.001 – 0.018] (p = 0.037). The sICH occurred in nine cases, all of them were in the group without achieving successful reperfusion.Conclusion. The baseline levels of SBP ≥ 160 mm Hg at hospital admission in patients with AIS followed by IVT are accompanied by worse functional outcome and greater odds of death. The neurological improvement with significant decline in the NIHSS score during the first 24 hours after IVT is associated with a decreased risk of sICH.

Single nucleotide substitutions in gene sequence associated with conformational changes in protein receptor or in expression of interferon receptors may explain variations in human susceptibility to infection and severity of COVID-19 along with other well-known risk factors. The study aimed to investigate associations between polymorphic variants of interferon receptor genes, COVID-19 severity and prevalence of antibiotic resistance genes in the gut microbiota. Materials and methods. The study was conducted using a random sample of Arkhangelsk population aged 42 to 76 years (n = 305). The research involved gathering COVID-19 data from the Federal Register, conducting blood tests for SARS-CoV-2 antibodies and polymorphic interferon receptor gene variants, and identifying antibiotic resistance genes in stool samples. Results. During the first 12–15 months of the COVID-19 pandemic, 17.4% of the study participants had symptomatic COVID-19, while 32.8% were asymptomatic. By the Autumn of 2022, symptomatic COVID-19 cases rose up to 36.4%, while asymptomatic cases increased to 61.3%. We reveal an association between the CC genotype of the IFNAR1 gene rs2257167 variant, the presence of the T allele of IFNAR2 gene rs2229207 variant, the CCTT haplotype and symptomatic COVID-19. The GCTC haplotype was associated with pneumonia and COVID-19 severity. In November 2022, macrolide resistance genes were observed in 98.4% of cases, whereas those to beta-lactams and glycopeptides — in 26.9% and 13.8% cases, respectively. Resistance to three classes of antibiotics was observed in 4.9% and was more frequently detected in individuals with the ССТТ haplotype. Genes encoding beta-lactamases were more often found in individuals with the GCTC haplotype, those who had COVID-19 with pneumonia and those who received hospital treatment. Glycopeptide resistance genes were associated with the CC genotype of the rs2257167 variant of IFNAR1 gene. Conclusion. We identified genetic determinants of susceptibility, symptomatic infection and COVID-19 severity. The associations between polymorphic variants of interferon receptor genes and COVID-19 severity can be used to identify people with a genetic predisposition to severe infection and to determine priority groups for vaccination, including the prevention of antibiotic resistance in complicated course of viral infections.

Objective: to estimate the prevalence of antibiotic resistance genes in the resistome of adult residents of Arkhangelsk with regard to the severity of the novel coronavirus infection (COVID-19).Materials and methods. A cross-sectional study was conducted between October and November 2022 (2.5 years after the start of the COVID-19 pandemic) on a random sample (N=455) of Arkhangelsk population aged 42-76 years. The data collection involved a questionnaire survey, assessment of immunoglobulins G to S-, S2, N-proteins of SARS-CoV-2 and detection of antibiotic resistance genes in fecal samples by polymerase chain reaction.Results. Almost all participants (98.5%) had at least one antibiotic resistance gene, the resistance determinants to three classes of antibiotics simultaneously were detected in 5.6%. The prevalence of resistance genes to macrolides was 98.5%, to beta-lactams – 29.0%, and to glycopeptides – 16.0%. Antibiotic resistance genes to beta-lactams were more prevalent among participants who had previously been hospitalized for COVID-19 (44.8%) and among those having had frequent acute respiratory infections (50.0%). Individuals vaccinated against SARS-CoV-2 (26.6%) and participants with cardiovascular diseases (17.0%) were less likely to have beta-lactam resistance genes.Conclusion. The high prevalence of antibiotic resistance genes has been revealed in the resistome of adult residents of Arkhangelsk. We determined the association between resistance to beta-lactams and COVID-19 severity. The study results could be used to improve the protocols of antibiotic therapy and to guide a decision-making related to the antibiotic prescription in adults.

BACKGROUND: Influence of obesity on the body at whole and with regard to metabolic changes is still unclear. In Russia there are a few data about prevalence of metabolic phenotypes among population based on epidemiological data.AIM: to assess the prevalence of metabolic phenotypes among citizens of Arctic area of the Russian Federation (in the Arkhangelsk city setting).MATERIALS AND METHODS: a cross-sectional study was conducted using a random sample of Arkhangelsk citizens (n=2380) 35–69 years old, which was obtained within a population study of cardiovascular diseases («Know your heart» (KYH)). The participants were divided into metabolic phenotypes according to the presence of obesity (BMI≥30 kg/m2) and metabolic syndrome (AHA/NHBLI): phenotype 1 — metabolically healthy normal weight, phenotype 2 — metabolically unhealthy normal weight, phenotype 3 — metabolically healthy obesity, phenotype 4 — metabolically unhealthy obesity.RESULTS: 2352 participants of KYH were included in the study, 982 (41,8%) men and 1370 (58,3%) women. Mean age was 53,9 (SD 9,7) years. The distribution of participants by metabolic phenotypes was as follows: 1167 (49,6%) persons had phenotype 1, 489 (20,8%) — phenotype 2, 248 (10,5%) — phenotype 3, 448 (19,1%) — phenotype 4. In men, the second common after the first phenotype was phenotype 2, while in women, the second position was shared by the 2nd and 4th phenotypes, which had approximately the same frequency. «Arterial hypertension» was the most prevalent component of metabolic syndrome and seen in 68–96% men and 38–94% women in the study with different phenotypes. The proportions of phenotypes with metabolic disorders increased with age.CONCLUSION: in a study of a random population sample within the framework of the concept of metabolic phenotypes, a half of the participants had no obesity and metabolic syndrome. Proportions of participants with metabolic disorders with and without obesity was 20% each. Only 10% of participants had «metabolically healthy» obesity. If excluding individuals without obesity and metabolic syndrome, the phenotype characterized by metabolic disorders in the absence of obesity was the most common among men. Phenotypes with metabolic disorders on the background of obesity or without obesity were equally common among women. The most common component of metabolic syndrome was «arterial hypertension». There was a tendency of accumulation of metabolic disturbances with age.

Aim. To study the problem of obesity at the national level.Material and methods. The ESSE-RF3 study examined a representative sample of the population of 15 Russian regions aged 35-74 years with a response >70% (n=28731). Modular design questionnaire was used. The study received approval from the Independent Ethics Committee of the National Medical Research Center for Therapy and Preventive Medicine. Signed informed consent was obtained from each participant. Body mass (BM) was measured once with an accuracy of 100 g. The height was measured with an accuracy of 0,5 cm once in a standing position without shoes. Waist circumference was measured with a standard measuring tape with an accuracy of 0,5 cm. There were following BM categories: insufficient (BM index (BMI) 40). Abdominal obesity (AO) was assessed using the following criteria: waist circumference ≥102 cm and ≥88 cm for men and women, respectively. The changes of obesity were studied with the inclusion of materials from the ESSE-RF (2012-14) and ESSE-RF2 (2017) studies, carried out using the same methodology with ESSE-RF3 in the same age range of 35-64 years. Statistical analysis was carried out using the open-source R 3.6.1 environment. Analysis of associations between factors and a binary variable was carried out using the logistic regression method. The significance level for all tested hypotheses was 0,05.Results. Mean BMI values is 28,2±4,6 kg/m2 among men and 28,9±5,9 kg/m2 among women. The prevalence of overweight is 44,0 and 33,7%, obesity — 30,0 and 39,5%, AO — 30,9 and 55,1% among men and women, respectively. In addition, 30,3% have a combination of obesity and AO; obesity and/or AO — 49,1%. Obesity statistics did not significantly change, but in each section the rates are higher among women. To analyze the prevalence of anxiety and depression in Russians using the Hospital Anxiety and Depression Scale (HADS) during the Epidemiology of Cardiovascular Diseases and their Risk Factors in Regions of Russian Federation (ESSE-RF) study. To study the associations of increased levels of anxiety and depression (HADS-A/ HADS-D ≥8+) with demographic and socio-economic characteristics according to the ESSE-RF3 study.Material and methods. Aim. To analyze the prevalence of smoking according to the Epidemiology of Cardiovascular Diseases and their Risk Factors in Regions of Russian Federation-3 (ESSE-RF3) study, and compare these data with the previous two stages.Material and methods. Data from following three cross sections of the ESSE-RF study were used: 2013-2014, 2017, 2020-2022. At the first stage, the prevalence and probability of smoking was assessed according to ESSE-RF3 data in 2020-2022 (n=28628, 35-74 years). At the second stage, an analysis was carried out over time using data from all three ESSE-RF sections (n=43804, 35-64 years).Results. In the overall ESSE-RF3 sample, the prevalence of smoking was 17,0% (men — 27,7%, women — 7,6%). Smoking probability varies significantly by sex, age, education level and income. Over time, the standardized prevalence of smoking increases from 25,0% in 20132014 to 26,2% in 2017, and then decreases to 21,5% in 2020-2022. Women are characterized by the same direction of trends, while men had consistent decrease in prevalence from 41,3 to 37,0 and 34,3%, respectively. The probability of smoking in 2017, relative to 20132014, decreases only in some socio-demographic categories of the population. In 2020-2022, a decrease in the probability of smoking is typical for both the general sample and all subgroups of the population.Conclusion. The results obtained characterize the current situation and time trends of the last decade and contain important information on smoking epidemiology in Russia.

At the present time, there is a growing trend of vegetarian diets, even in countries with a traditional Nordic diet.Aim. To study the prevalence and characteristics of vegetarian diets in the Russian population at the present stage.Material and methods. The analysis included data from representative samples of the adult population 25-74 years old, examined within three sections of the Epidemiology of Cardiovascular Diseases and their Risk Factors in Regions of Russian Federation (ESSE-RF) study in 20132014, 2017 and 2020-2022. A total of 57351 participants (24862 men and 32489 women) were analyzed. Nutrition assessment was carried out using the frequency method. Restrictive diets include semi-vegetarian, vegetarian and vegan diets. Data are presented as frequencies and odds ratios (OR) with 95% confidence intervals (CI).Results. The prevalence of restrictive diets in the Russian population according to three sections of the ESSE-RF study was 3,1%. There is an increase in the prevalence of these diets from 2,7% in 2013, to 3,6% in 2017 and 3,3% in 2020-2022. To study the proportion of coronavirus disease 2019 (COVID-19) survivors and hospitalization cases of the Russian population for the period 2020-2022, depending on socio-demographic characteristics.Material and methods. The work was performed on random samples of the population aged 35-74 years from 15 regions participating in the Epidemiology of Cardiovascular Diseases and their Risk Factors in Regions of Russian Federation-3 (ESSE-RF3) study. The examination was carried out from February 2020 to March-April 2022 (n=28718, 47% men).Results. According to a survey of sample participants, the proportion of respondents who had COVID-19 by the spring of 2022 among all surveyed was 26,0%. Among 45-54-year-old respondents, this parameter was maximum (28,0%), while among those aged 65-74-year-old — minimal (22,3%). Aim. To establish reference values of handgrip test for a middle-aged and elderly population (35-74 years), as well as study the association of health characteristics and muscle strength.Material and methods. For the analysis, we used data from the Epidemiology of Cardiovascular Diseases and their Risk Factors in Regions of Russian Federation-3 (ESSE-RF3) study, conducted in 2020-2022. The sample was selected similarly to the previous ESSE-RF stages. The sample included 27689 men and women aged 35-74 years from 14 Russian regions. Of the individual variables, the characteristics with the highest evidence level of influence on muscle strength were taken as covariates. Muscle strength was assessed by a handgrip test using a MEGEON 34090 device with an accuracy of 0,1 kg. Three measurements were taken for each arm in a standing position, and the maximum value was used in the analysis. Normative sex-specific handgrip test values were established regardless of age: a participant's handgrip value was considered reduced if it was 2 standard deviations below the average value of participants at the age with the best/highest scores.Results. The average handgrip test values for the entire male sample were 43,4±11,1 kg, while for the female sample — 27,6±6,1 kg. In men under 53 years of age, muscle strength decreases at a rate of 0,12 kg/year, then the decline accelerates to 0,48 kg/year. In women under 45,5 years of age, a slight and insignificant increase in muscle strength was detected by 0,02 kg/year, and after 45,5 years, handgrip value falls at a rate of 0,2 kg/year. Therefore, both men and women lose 1,3% of muscle strength per year to year and the decline rate do not differ. The peak age can be considered 35 years old, since at this age, the average muscle strength in men and women is 47,6±11,5 and 29,2±5,7 kg, respectively, and the calculated norm (M–2SD) for men was 47,6–2´11,5≈24 kg using rounding down, and for women it was 29,2– 2´5,7≈17 kg. With age, as expected, the proportion of people of both sexes with a metabolic syndrome level below the threshold increases, reaching 7,5% after 65 years. Multivariate regression analysis showed that, taking into account adjustments for covariates, for every 10 years, muscle strength decreases by an average of 3,1 kg in men and by 1,7 kg in women. Negative associations of metabolic syndrome with inflammation markers (fibrinogen and C-reactive protein) and noncommunicable diseases are also significant, but only men have significantly lower rates of metabolic syndrome in the presence of anxiety and depression, low income and urban residence.Conclusion. The use of handgrip test in any age range during medical examinations or preventive examinations of the adult population and informing primary care specialists about the results obtained can help improve the general health of the population.

To assess adherence to a healthy lifestyle depending on the individual and socio-economic characteristics of the Russian population (according to the Epidemiology of Cardiovascular Diseases and their Risk Factors in Regions of Russian Federation-3 (ESSE-RF3) study), followed by a comparative analysis with ESSE-RF2 (2017).Material and methods. The analysis was carried out based on research data from ESSE-RF2 in 2017 and ESSE-RF3 in 2020-2022. The healthy lifestyle adherence index included following parameters: no smoking; sufficient consumption of vegetables and fruits; physical activity; no excessive salt intake and alcohol abuse. Three following categories of adherence to a healthy lifestyle were analyzed: high, satisfactory, low. From individual variables, individual socio-economic characteristics with the highest evidence level of influence on healthy lifestyle were taken.Results. In the ESSE-RF3 sample, 47,2% of respondents had low adherence to a healthy lifestyle, 37,2% — satisfactory, and 15,6% — high. To assess the prevalence of psychological stress (PS) using the Perceived Stress Scale-10 (PSS-10) in various Russian regions and to analyze PS associations with socio-demographic characteristics and behavioral risk factors and some non-communicable diseases (NCDs).Material and methods. We studied representative samples of the population aged 35-74 years in the Russian regions, examined as part of the Epidemiology of Cardiovascular Diseases in Regions of Russian Federation-3 (ESSE-RF3) study. The subjects were interviewed using a questionnaire containing a section on the psychological stress — PSS-10. For men, PS levels are ranked as follows: "Low" — 0-11, "Medium" — 12-17, "High" — 18-40. For women, PS levels are ranked as follows: "Low" — 0-13, "Medium" — 14-19, "High" — 20-40. Statistical analysis was carried out using the open-source R 3.6.1 environment.Results. The average PS level was 12,0±7,3 (men: 10,7±7,0; women: 13,1±7,4). Every fifth person (20,3%) in the Russian population has a high PS level, with higher PS rates among women compared to men (high PS level — 21,6 vs 18,9%, respectively). A high PS level was associated with the income level (average or low), no marriage, unemployment, low level of physical activity, depression, anxiety, NCDs (type 2 diabetes, myocardial infarction, stroke), additionally for women — hypertension.Conclusion. The Russian population is characterized by a high PS prevalence, while PS rates are higher among women, which must be taken into account when predicting the NCD risks.

Vaccination is an effective tool to control infectious diseases at the population level. There is an increase in the proportion of the adult population vulnerable to controlled infections, which is confirmed by the emergence of disease outbreaks involving population aged >18 years. Qualitative research methods are used to explore factors influencing vaccination behavior.Aim. To develop and present a qualitative study design to study barriers and drivers for vaccination in adults.Material and methods. The World Health Organization (WHO) approach of Tailoring Immunization Programs (TIP), Capability-Opportunity-Motivation-Behavior (COM-B) tool, developed by the WHO and adapted in several countries to study vaccination issues, and the Rapid Assessment Process method were chosen and adapted for conducting a study in three Russian regions. The qualitative methods selected are focus groups and in-depth personal interviews in two target groups of adults and medical workers. A purposive sampling method will be used to select population participants aged ≥18 years.Results. A qualitative research design was developed, including the following steps: selecting target populations, developing a discussion plan, training the analytical group, testing the methodology and questions in the target groups, collecting and analyzing data, developing recommendations and interventions.Conclusion. Qualitative research methods are preferable for identifying vaccination barriers and drivers. Based on the results obtained, an educational program for health workers will be developed, including communicating approaches to be used with patients when discussing vaccination matters.The qualitative research design developed and presented can be used by researchers, public health professionals, and decision-makers to study vaccination behavior among adults.

Aim. To analyze availability and affordability of alcoholic beverages in Russian cities.Material and methods. We assessed the actual state of urban infrastructure in the Arkhangelsk, Tver and Sverdlovsk regions. Data were collected in three districts of Arkhangelsk, three districts of Tver, three districts of Yekaterinburg and Revda, where participants of the Epidemiology of Cardiovascular Diseases and their Risk Factors in Regions of Russian Federation-3 (ESSE-RF3) study (2021) lived. The study objects were elements of the city infrastructure where alcoholic beverages were sold. To assess the infrastructure, special software was developed to collect and analyze the necessary data using Open Street Maps. Data were collected from January to March 2022.Results. In total, 1180 alcohol-selling outlets were identified in the study areas: in Arkhangelsk — 450, in Yekaterinburg and Revda — 444, and in Tver — 286. Mainly alcoholic beverages in Sverdlovsk and Tver regions can be purchased in the alcohol section of a grocery store (n=260, 58,6% and n=185, 41,4%, respectively), and in the Arkhangelsk region — in a specialized liquor stores (n=263, 58,4%). The median number of alcohol stores located within a radius of 1000 m from the place of residence of the ESSE-RF3 participant was 33, and within a radius of 400 meters — 8. In Arkhangelsk, the alcohol was sold in a grocery store located in the same building as the college branch. In the Sverdlovsk region, the nearest alcohol store was located at a distance of 50 m from the kindergarten, in the Tver region — 44 m from the college. A 0,5 liter bottle of vodka was sold below the price floor in 55,1% of specialized liquor stores in the Sverdlovsk region and in 66,8% of such liquor stores in the Tver region.Conclusion. As a result of the study, availability and affordability of alcoholic beverages in Arkhangelsk, Tver, Yekaterinburg and Revda was assessed. The findings reflect the high availability of alcohol and indicate the need for interventions at both the national and local government levels. The used method of collecting and analyzing data demonstrated sufficient research effectiveness and can be recommended for wider use.

Aim. To study the relationship of the density and proximity of tobacco products (TPs) and electronic nicotine delivery systems (ENDS) with increased levels of depression, anxiety and stress among the population in 3 constituent entities of the Russian Federation.Material and methods. The work used data from the Epidemiology of Cardiovascular Diseases and their Risk Factors in Regions of Russian Federation-3 (ESSE-RF3) study in 3 Russian regions (Arkhangelsk, Sverdlovsk and Tver regions) among the population aged 35-64 years (n=1674), who signed a consent to participate in the study. We analyzed data on the status of smoking and consumption of ENDS, passive smoking, attempts to quit smoking, as well as the presence of depression, anxiety (Hospital Anxiety and Depression Scale (HADS) score ≥8) and stress (Perceived Stress Scale (PSS) >21). Elements of urban infrastructure (data collection: January-March 2022) selling TPs and ENDS were assessed using aт OpenStreetMap-based program. Associations were established using logistic regression analysis.Results. With a twofold increase in the distance of TP and ENDS outlets from the place of residence, the depression probability was reduced by 13% (odds ratio (OR) 0,87; 95% confidence interval (CI): 0,79-0,96) and by 12% (OR 0,88; 95% CI: 0,78-0,99), respectively. With increase in distance from of TP outlets, the anxiety decreased probability by 12% (OR 0,88; 95% CI: 0,81-0,97). With an increase in the number of TP outlets within a radius of 1000 m from the place of residence, there were an increase in the probability of depression (OR 1,18; 95% CI: 1,031,36), anxiety (OR 1,12; 95% CI: 1-1,26) and stress (OR 1,24; 95% CI: 1,05-1,46). A greater number of ENDS outlets within a radius of 1000 m and 400 m was associated with a high likelihood of depression (OR 1,17; 95% CI: 1,02-1,34 and OR 1,25; 95% CI: 1,04-1,47, respectively). An increased depression probability was found in those exposed to tobacco smoke at home (OR 1,88; 95% CI: 1,27-2,73) and at work (OR 1,67; 95% CI: 1,14-2,41). An increase in the number of attempts to quit smoking doubles the odds of depression (OR 2,40; 95% CI: 1,35-4,23) and anxiety (OR 2,00; 95% CI: 1,18-3,38).Conclusion. Unfavorable environmental factors, including the proximity and density of TP and ENDS outlets, can increase the probability of depression, anxiety and stress among the population living in these areas.

Objectives. Two-dimensional transition metal dichalcogenides (TMDs) are utilized for various optical applications due to the presence in these materials of a direct band gap corresponding to the visible and near-infrared spectral regions. However, a limited set of existing TMDs makes the region of the used spectral range discrete. The most effective way to solve this problem is to use two-dimensional TMD films based on multicomponent alloys, including three or more different chemical elements (while TMDs consist of two). By varying their morphological composition, one can control the value of the band gap and thus their optical absorption spectrum. However, since the band gap in such structures is highly nonlinear as far as their chemical composition is concerned, it can be challenging to select the required concentration in order to achieve uniform absorption. In this regard, the purpose of this work is to theoretically determine the dependence of the band gap of four-component two-dimensional MoxW1–xS2ySe2(1–y) alloys on their morphological composition.Methods. The calculations were performed within the framework of the density functional theory using the Quantum Espresso software package. Flakes of two-dimensional TMDs alloys were prepared from bulk TMDs crystals by mechanical exfoliation on a Si/SiO2 substrate. An experimental study of the photoluminescence characteristics was carried out using photoluminescence microscopy-spectroscopy. Results. In this work, the dependence of the band gap on the morphological composition of two-dimensional MoxW1–xS2ySe2(1–y) alloys was determined. Upon varying the composition of TMDs alloys, it was found that the band gap changes from 1.43 to 1.83 eV. The obtained theoretical results are in qualitative agreement with the experimental data.Conclusions. The minimum band gap is observed in alloys close to MoSe2, while alloys close to WS2 have the maximum band gap value.

Background. For the proper diagnosis and optimal prognosis of acute leukemia (AL), a complex of clinical and laboratory methods is used. Among them, chromosomal analysis plays an important role. Cytogenetic markers of leukemic cells are widely used in clinical settings. WHO classification of Tumors of Haematopoietic and Lymphoid Tissues is based on a combined analysis of clinical, cytomorphological, immunophenotypic and cytogenetic findings. Immunophenotyping of blast cells using monoclonal antibodies has significantly improved the diagnosis of leukemia by detecting the grade of tumor differentiation. Aim: based on the results of immunophenotypic and cytogenetic examinations, to describe the structure of AL and to study the effect of cytogenetic abnormalities on the prognosis of acute leukemia in the child population of the Arkhangelsk region.Material and Methods. Archived data were used to analyze the results of cytogenetic and immunophenotypic examinations of all children aged 0–17 years in the Arkhangelsk region, who were diagnosed with AL in the period from January, 2004 to december, 2018. Results. Immunophenotypic examination of bone marrow blast cells in patients with AL revealed that B-linear lymphoblastic leukemia was the most common (74.8 %). Among the quantitative anomalies, hyperploidy was the most common (additional chromosomes 4, 10 and 17 were observed). The t(12; 21) was the most frequent structural chromosomal abnormality. Aim. To analyze the demographic, socio-economic and behavioral factors associated with awareness and passage of medical screening among the hypertension adults from Arkhangelsk and Novosibirsk.Material and methods. The database was formed as part of the Know Your Heart study (2015-2018, Arkhangelsk, Novosibirsk, n=4504). For the present study, 2216 people with hypertension were selected. Binary logistic regression was used to determine the factors associated with awareness of ongoing medical screening and active participation in them.Results. The analysis determined that 82,5% of the hypertensive residents of Arkhangelsk and Novosibirsk aged 35-69 years old are aware of the medical screening, and 51,4% have passed (are going to pass) it. General factors associated both with greater awareness of ongoing preventive measures and with greater participation in them are female sex and contacts with doctors during the year. From the point of view of awareness, such a factor as the average income level turned out to be significant, while the absence of a permanent employment and the pension status were associated with the passage of medical screening.Conclusion. The awareness of the hypertensive residents of Arkhangelsk and Novosibirsk about the medical screening was 82,5%. The coverage level is still low. The identified factors associated with greater awareness of the medical screening and more active participation showed that medical specialists should be more attentive to the men working in urban areas with a low income level. More active work is also needed to involve in preventive contacts those persons who are not in constant contact with the healthcare system. This requires the consolidated work of various institutions and departments, including the media.

In the article, the role of law-enforcement bodies in ensuring economic security of Russia by counteraction of criminalization of financial and economic activity of the commercial organizations is considered. Features of law-enforcement bodies in this direction are considered; their role in comparison with other law enforcement agencies is estimated. The attention to interdependence of the concepts “economic activity” and “financial and economic activity” of the commercial organizations and to a role of law-enforcement bodies in the economic policy pursued by the state is paid.

Application of modern technologies and innovative technical equipment at cultivation of grain-crops, including winter wheat, flax, clover and rape, causes actuality of carrying out the multivariable field test. The planned long-term field experience of determination of dependence of the productivity of the cultivated agricultural cultures on the set factors will allow to justify basic technological methods and confirm optimal parameters and modes of operations of innovative working tools for tillage, sowing and crop protection. By results of planning of field experiment width and length of a plot were 3 and 20 m respectively. At randomized placement for one crop cultivation 81 registration plots were necessary. The general size of the test field at a research of three factors with three levels of variation was 110 m width and 215 m length. Before conducting field experiment the authors fixed an initial condition of a soil profile, agronomical and biological characteristics of the soil. Change of soil properties of the and also crops vegetation was noted at of the end of carrying out studies of each series of tests. The main response of field experiment conducting was the productivity. On the basis of the received results more than 27 hectares will be cultivated to increase crop productivity by 17-19 percent owing to the saved-up soil fertility and introduction of innovative technological and technical means of cultivation. The developed recommendations will be realized at agricultural enterprises in the Tver’ region and in others regions of the of the Non-black earth zone.

Modern agricultural enterprises need universal seeders suitable for sowing various types of seeds in different soil conditions. Seeders should be available for agronomic requirements for planting in various conditions: high humidity, constantly desired depth of planting and other factors. The authors developed and manufactured a pneumatic sowing machine on the basis of modular combined adapter BMKA. The combined unit is designed for secondary tillage, sowing and covering of seeds of small-seeded crops such as flax. BMKА is equipped with pneumatic horizontal disc sowing unit, two units helical rotary harrows, unit of cultivation and coulter group. Created design ensures a quality soil preparation, uniform seeding small-seeded crops and set depth of covering. A rotary harrow has several main parameters: diameter is 180 mm; height of teeth equals 120 mm; helix pitch distance is 80 mm. Soil cultivation is possible at depth down to 140 mm. Optimal parameters and modes of operation of the sowing machine: disc diameter - 600 mm, seed cell diameter is 8 mm, the pressure in the pneumatic system - 0.13 MPa, frequency of rotation of the disc equals 65 rpm. BMKA manufacturing application increases the yield of straw by 13.7-33.1 percent, seeds of flax by 19.6-31.3 percent in comparison with the control variant. The annual economic effect is 2035-3034 rubles per 1 ha, when the payback period is one season. The machine manufacturing application provides high-quality seeds for breeding and seed production.

Scientific researches of tussocks physical and technological properties are necessary for working out of technological means for improvement of the tussock meadows and pastures. The author has defined a type of tussocks, geometrical indicators, a form and density of their location. Investigation of a flood meadow, which was near the river Orsha in Kalinin district of the Tver region was conducted. It was showed that the average nominal diameter of the tussock bottom was equal 410 mm, height - 470 mm. Averaged density of a tussocks location made 1.7 pieces per sq. m. The main indicators of physical and technological properties of tussocks for use of machines for cultivating of the tussock forage lands were conducted. It was given that density of tussocks was equal 1.23-1.49 g per cc, absolute moisture of a tussock was 25.3 percent, earthy hillock - 19.3 percent, hardness - 116, and 5.5 kPa respectively. Installation for determination of shear strength of a hillock is worked out. It was revealed that the greatest energy expenses accrue to the lower layers of tussoks.

The transfer matrix method has been widely used to calculate wave propagation through the layered structures consisting entirely of either linear or nonlinear optical materials. In the present work, we develop the transfer matrix method for structures consisting of alternating layers of linear and nonlinear optical materials. The result is presented in a form that allows one to directly substitute the values of material constants, refractive index and absorption coefficient, into the expressions describing the second harmonic generation (SHG) field. The model is applied to the calculation of second harmonic (SH) field generated in nano-thin layers of transition metal dichalcogenides exfoliated on top of silicon oxide/silicon Fabry-Perot cavity. These structures are intensively studied both in view of their unique properties and perspective applications. A good agreement between experimental and numerical results can be achieved by small modification of optical constants, which may arise in an experiment due to a strong electric field of an incident focused pump laser beam. By considering the SHG effect, this paper completes the series of works describing the role of Fabry-Perot cavity in different optical effects (optical reflection, photoluminescence and Raman scattering) in 2D semiconductors that is extremely important for characterization of these unique materials.
