1. Preference-CFR$\:$ Beyond Nash Equilibrium for Better Game Strategies

Abstract

Recent advancements in artificial intelligence (AI) have leveraged large-scale games as benchmarks to gauge progress, with AI now frequently outperforming human capabilities. Traditionally, this success has largely relied on solving Nash equilibrium (NE) using variations of the counterfactual regret minimization (CFR) method in games with incomplete information. However, the variety of Nash equilibria has been largely overlooked in previous research, limiting the adaptability of AI to meet diverse human preferences. To address this challenge, where AI is powerful but struggles to meet customization needs, we introduce a novel approach: Preference-CFR, which incorporates two new parameters: preference degree and vulnerability degree. These parameters allow for greater flexibility in AI strategy development without compromising convergence. Our method significantly alters the distribution of final strategies, enabling the creation of customized AI models that better align with individual user needs. Using Texas Hold'em as a case study, our experiments demonstrate how Preference CFR can be adjusted to either emphasize customization, prioritizing user preferences, or to enhance performance, striking a balance between the depth of customization and strategic optimality.

Abstract

Hot Isostatic Pressing elaboration of Norem02, an austenitic-ferritic hypereutectoid stainless steel, leads to the formation of an austenitic matrix with a mixture of acicular M7C3 and globular M23C6 carbides. The sintering tests, carried out by using an AISI 304L container, showed that the final microstructure and the carbides’ distribution of the HIPed Norem02 are strongly influenced by the process parameters (heating and cooling rate, sintering time, holding temperature and pressure) and by the particles’ size, microstructure and phase distribution of the initial powder. The morphological, crystallographic and chemical analysis of the sintered samples were completed by comprehension of the diffusion phenomena at the Norem02/304L interface, enabling the establishment of a correlation between elaboration process and final microstructure.

Abstract

The coupling between mass transfer and hydrodynamic phenomena in two-phase flow is not necessarily straightforward due to the different effects that can be encountered. The treatment of such coupling is complex and requires particular efforts, especially in the modelling of the interface between phases. In this paper, we consider the case of a droplet composed of two components (one miscible and one immiscible in water) released in a 2D rectangular domain filled with water. Mass transfer occurs between the miscible element and the surrounding water, which leads to a density inversion that directly affects the droplet trajectory through buoyancy. We perform simulations using a ternary Cahn-Hilliard model (implemented in the "phase\_field" model of the TrioCFD code) to capture such coupled phenomena. The Boussinesq approximation for a multicomponent system is used to define the density law and an analytical chemical potential is proposed for the thermodynamic landscape. The effect of the mobility parameter on the flow is highlighted and the results found are in good agreement with the dynamics described from an experimental study of the open literature.

Abstract

BINGO is a project aiming to set the grounds for large-scale bolometric neutrinoless double-beta-decay experiments capable of investigating the effective Majorana neutrino mass at a few meV level. It focuses on developing innovative technologies (a detector assembly, cryogenic photodetectors and active veto) to achieve a very low background index, of the order of $10^{-5}$ counts/(keV kg yr) in the region of interest. The BINGO demonstrator, called MINI-BINGO, is designed to investigate the promising double-beta-decay isotopes $^{100}$Mo and $^{130}$Te and it will be composed of Li$_2$MoO$_4$ and TeO$_2$ crystals coupled to bolometric light detectors and surrounded by a Bi$_4$Ge$_3$O$_{12}$-based veto. This will allow us to reject a significant background in bolometers caused by surface contamination from $\alpha$-active radionuclides by means of light yield selection and to mitigate other sources of background, such as surface contamination from $\beta$-active radionuclides, external $\gamma$ radioactivity, and pile-up due to random coincidence of background events. This paper describes an R\&D program towards the BINGO goals, particularly focusing on the development of an innovative assembly designed to reduce the passive materials within the line of sight of the detectors, which is expected to be a dominant source of background in next-generation bolometric experiments. Abstract

Abstract This study evaluates the safety and potential benefits of PBM on pancreatic beta cells and islets. PBM was applied to insulin-secreting cell lines (MIN6) and rat pancreatic islets using a 670 nm light source, continuous output, with a power density of 2.8 mW/cm², from 5 s to several 24 h. Measure of cell viability, insulin secretion, mitochondrial function, ATP content, and cellular respiration were assessed. Additionally, a diabetic rat model is used for islet transplantation (pre-conditioning with PBM or not) experiments. Short and long-term PBM exposure did not affect beta cell islets viability, insulin secretion nor ATP content. While short-term PBM (2 h) increases superoxide ion content, this was not observed for long exposure (24 h). Mitochondrial respirations were slightly decreased after PBM. In the islet transplantation model, both pre-illuminated and non-illuminated islets improved metabolic control in diabetic rats with a safety profile regarding the post-transplantation period. In summary, for the first time, long-term PBM exhibited safety in terms of cell viability, insulin secretion, energetic profiles in vitro, and post-transplantation period in vivo. Further investigation is warranted to explore PBM’s protective effects under conditions of stress, aiding in the development of innovative approaches for cellular therapy.

11. Speech-Language Pathologists' Perceptions When Supporting Students with Dyslexia

Abstract

This study explored the rationale and significance of integrating speech-language pathologists (SLPs) into the assessment and remediation of dyslexia, traditionally the domain of general and special education teachers. Drawing on their unique qualifications in language-related areas, such as phonology, syntax, and semantics, SLPs have emerged as valuable assets in supporting students with dyslexia. The American Speech-Language and Hearing Association (ASHA) has recognized the value of SLPs working with students with dyslexia and has set specific guidelines for working with them. Despite these guidelines, recent data have indicated a gap between policy and practice, with only a fraction of SLPs (36%) actively engaging in supporting students with reading and writing problems in public schools. However, SLPs in the State of New Jersey have been tasked to identify and assist children at risk for dyslexia. Examining the experiences of SLPs who were already supporting students with dyslexia would help to provide a template for other SLPs. The study used a phenomenological case study approach to increase understanding and create a new perspective on the lived experiences of 10 SLPs in New Jersey as they support students with dyslexia. Data from interviews, journals, and focus groups were gathered and analyzed through a six-step process (Peoples, 2020) and were used to answer the following research questions: (Q1) What are the experiences of SLPs in New Jersey as they support students with dyslexia?; (Q2) What are the experiences of SLPs in New Jersey as they collaborate with faculty, administration, and other educational professionals to support students with dyslexia?; and (Q3) What context or situations have typically influenced SLPs' experiences supporting students with dyslexia? Thirteen essential themes and three emerging themes arose from the data. Overall, SLPs saw their experience through the lens of their individual duties, the teamwork that supported students, and specific contexts that impacted their ability to work with students with dyslexia. The data showed they believed their role included identification, assessment, remediation, and support of students with dyslexia. The SLPs also saw their role as supporting both language and literacy and were generally pleased with their training experiences. Abstract

Salmonella infection causes epidemic death in wild songbirds, with potential to spread to humans. In February 2021, public health officials in Oregon and Washington, USA, isolated a strain of Salmonella enterica serovar Typhimurium from humans and a wild songbird. Investigation by public health partners ultimately identified 30 illnesses in 12 states linked to an epidemic of Salmonella Typhimurium in songbirds. We report a multistate outbreak of human salmonellosis associated with songbirds, resulting from direct handling of sick and dead birds or indirect contact with contaminated birdfeeders. Companion animals might have contributed to the spread of Salmonella between songbirds and patients; the outbreak strain was detected in 1 ill dog, and a cat became ill after contact with a wild bird. This outbreak highlights a One Health issue where actions like regular cleaning of birdfeeders might reduce the health risk to wildlife, companion animals, and humans.

Abstract

"γc" cytokines are a family whose receptors share a "common-gamma-chain" signaling moiety, and play central roles in differentiation, homeostasis, and communications of all immunocyte lineages. As a resource to better understand their range and specificity of action, we profiled by RNAseq the immediate-early responses to the main γc cytokines across all immunocyte lineages. The results reveal an unprecedented landscape: broader, with extensive overlap between cytokines (one cytokine doing in one cell what another does elsewhere) and essentially no effects unique to any one cytokine. Responses include a major downregulation component and a broad Myc-controlled resetting of biosynthetic and metabolic pathways. Various mechanisms appear involved: fast transcriptional activation, chromatin remodeling, and mRNA destabilization. Other surprises were uncovered: IL2 effects in mast cells, shifts between follicular and marginal zone B cells, paradoxical and cell-specific cross-talk between interferon and γc signatures, or an NKT-like program induced by IL21 in CD8+ T cells.

Abstract

This is an account that should be heard of an important struggle: the struggle of a large group of experts who came together at the beginning of the COVID-19 pandemic to warn the world about the risk of airborne transmission and the consequences of ignoring it. We alerted the World Health Organization about the potential significance of the airborne transmission of SARS-CoV-2 and the urgent need to control it, but our concerns were dismissed. Here we describe how this happened and the consequences. We hope that by reporting this story we can raise awareness of the importance of interdisciplinary collaboration and the need to be open to new evidence, and to prevent it from happening again. Acknowledgement of an issue, and the emergence of new evidence related to it, is the first necessary step towards finding effective mitigation solutions.

Abstract

Jamestown Canyon virus (JCV) is a mosquitoborne orthobunyavirus in the California serogroup that circulates throughout Canada and the United States. Most JCV exposures result in asymptomatic infection or a mild febrile illness, but JCV can also cause neurologic diseases, such as meningitis and encephalitis. We describe a case series of confirmed JCV-mediated neuroinvasive disease among persons from the provinces of British Columbia, Alberta, Quebec, and Nova Scotia, Canada, during 2011–2016. We highlight the case definitions, epidemiology, unique features and clinical manifestations, disease seasonality, and outcomes for those cases. Two of the patients (from Quebec and Nova Scotia) might have acquired JCV infections during travel to the northeastern region of the United States. This case series collectively demonstrates JCV’s wide distribution and indicates the need for increased awareness of JCV as the underlying cause of meningitis/meningoencephalitis during mosquito season.

Abstract

Vaccination is essential for the management of infectious diseases, many of which continue to pose devastating public health and economic challenges across the world. However, many vaccines are imperfect having only a partial protective effect in decreasing disease transmission and/or favouring recovery of infected individuals, and possibly exhibiting trade-off between these two properties. Furthermore, population turnover, that is the rate at which individuals enter and exit the population, is another key factor determining the epidemiological dynamics. While these factors have yet been studied separately, we investigate the interplay between the efficiency and property of an imperfect vaccine and population turnover. We build a mathematical model with frequency incidence rate, a recovered compartment, and an heterogeneous host population with respect to vaccination. We first compute the basic reproduction number $\mathcal{R}_0$ and study the global stability of the equilibrium points. Using a sensitivity analysis, we then assess the most influential parameters determining the total number of infected and $\mathcal{R}_0$ over time. We derive analytically and numerically conditions for the vaccination coverage and efficiency to achieve disease eradication ($\mathcal{R}_0 < 1$) assuming different intensity of the population turnover (weak and strong), vaccine properties (transmission and/or recovery) and trade-off between the latter. We show that the minimum vaccination coverage increases with lower population turnover, decreases with higher vaccine efficiency (transmission or recovery), and is increased/decreased by up to 15\% depending on the trade-off between the vaccine properties. We conclude that the coverage target for vaccination campaigns should be evaluated based on the interplay between these factors.

22. Improved inference of population histories by integrating genomic and epigenomic data

Abstract

With the availability of high-quality full genome polymorphism (SNPs) data, it becomes feasible to study the past demographic and selective history of populations in exquisite detail. However, such inferences still suffer from a lack of statistical resolution for recent, for example bottlenecks, events, and/or for populations with small nucleotide diversity. Additional heritable (epi)genetic markers, such as indels, transposable elements, microsatellites, or cytosine methylation, may provide further, yet untapped, information on the recent past population history. We extend the Sequential Markovian Coalescent (SMC) framework to jointly use SNPs and other hyper-mutable markers. We are able to (1) improve the accuracy of demographic inference in recent times, (2) uncover past demographic events hidden to SNP-based inference methods, and (3) infer the hyper-mutable marker mutation rates under a finite site model. As a proof of principle, we focus on demographic inference in Arabidopsis thaliana using DNA methylation diversity data from 10 European natural accessions. We demonstrate that segregating single methylated polymorphisms (SMPs) satisfy the modeling assumptions of the SMC framework, while differentially methylated regions (DMRs) are not suitable as their length exceeds that of the genomic distance between two recombination events. Combining SNPs and SMPs while accounting for site- and region-level epimutation processes, we provide new estimates of the glacial age bottleneck and post-glacial population expansion of the European A. thaliana population. Our SMC framework readily accounts for a wide range of heritable genomic markers, thus paving the way for next-generation inference of evolutionary history by combining information from several genetic and epigenetic markers.

Abstract

Summary: Human genetic studies of critical COVID-19 pneumonia have revealed the essential role of type I interferon-dependent innate immunity to SARS-CoV-2 infection. Conversely, an association between the HLA-B∗15:01 allele and asymptomatic SARS-CoV-2 infection in unvaccinated individuals was recently reported, suggesting a contribution of pre-existing T cell-dependent adaptive immunity. We report a lack of association of classical HLA alleles, including HLA-B∗15:01, with pre-omicron asymptomatic SARS-CoV-2 infection in unvaccinated participants in a prospective population-based study in the United States (191 asymptomatic vs. 945 symptomatic COVID-19 cases). Moreover, we found no such association in the international COVID Human Genetic Effort cohort (206 asymptomatic vs. 574 mild or moderate COVID-19 cases and 1,625 severe or critical COVID-19 cases). Finally, in the Human Challenge Characterisation study, the three HLA-B∗15:01 individuals infected with SARS-CoV-2 developed symptoms. As with other acute primary infections studied, no classical HLA alleles favoring an asymptomatic course of SARS-CoV-2 infection were identified.

Abstract

Foodborne outbreak investigations often provide data for public health officials to determine how the environment contributed to the outbreak and on how to prevent future outbreaks. State and local health departments are responsible for investigating foodborne illness outbreaks in their jurisdictions and reporting the data to national-level surveillance systems, including information from the environmental assessment. This assessment is designed to describe how the environment contributed to the outbreak and identifies factors that contributed to the outbreak and environmental antecedents to the outbreak. Environmental antecedents, also referred to as root causes, are specific reasons that allow biological or chemical agents to contaminate, survive, or grow in food. From 2017 to 2019, 24 jurisdictions reported 1,430 antecedents from 393 outbreaks to the National Environmental Assessment Reporting System. The most reported antecedents were lack of oversight of employees/enforcement of policies (89.1%), lack of training of employees on specific processes (74.0%), and lack of a food safety culture/attitude towards food safety (57.5%). These findings highlight the critical role that employees play in restaurant food safety and are heavily influenced by restaurant management, who can exercise active managerial control to manage these antecedents. Identifying antecedents during investigations is essential for understanding the outbreak’s root cause and implementing sustainable corrective actions to stop the immediate outbreak and future outbreaks.

Abstract

Mathematical modelling of epidemiological and coevolutionary dynamics is widely being used to improve disease management strategies of infectious diseases. Many diseases present some form of intra-host quiescent stage, also known as covert infection, while others exhibit dormant stages in the environment. As quiescent/dormant stages can be resistant to drug, antibiotics, fungicide treatments, it is of practical relevance to study the influence of these two life-history traits on the coevolutionary dynamics. We develop first a deterministic coevolutionary model with two parasite types infecting one host type and study analytically the stability of the dynamical system. We specifically derive a stability condition for a five-by-five system of equations with quiescence. Second, we develop a stochastic version of the model to study the influence of quiescence on stochasticity of the system dynamics. We compute the steady state distribution of the parasite types which follows a multivariate normal distribution. Furthermore, we obtain numerical solutions for the covariance matrix of the system under symmetric and asymmetric quiescence rates between parasite types. When parasite strains are identical, quiescence increases the variance of the number of infected individuals at high transmission rate and vice versa when the transmission rate is low. However, when there is competition between parasite strains with different quiescent rates, quiescence generates a moving average behaviour which dampen off stochasticity and decreases the variance of the number of infected hosts. The strain with the highest rate of entering quiescence determines the strength of the moving average and the magnitude of reduction of stochasticity. Thus, it is worth investigating simple models of multi-strain parasite under quiescence/dormancy to improve disease management strategies.

Abstract

Neutrinoless double-beta decay ($0\nu2\beta$) is a hypothetical rare nuclear transition. Its observation would provide an important insight about the nature of neutrinos (Dirac or Majorana particle) demonstrating that the lepton number is not conserved. BINGO (Bi-Isotope $0\nu2\beta$ Next Generation Observatory) aims to set the technological grounds for future bolometric $0\nu2\beta$ experiments. It is based on a dual heat-light readout, i.e. a main scintillating absorber embedding the double-beta decay isotope accompanied by a cryogenic light detector. BINGO will study two of the most promising isotopes: $^{100}$Mo embedded in Li$_2$MoO$_4$ (LMO) crystals and $^{130}$Te embedded in TeO$_2$. BINGO technology will reduce dramatically the background in the region of interest, thus boosting the discovery sensitivity of $0\nu2\beta$. The proposed solutions will have a high impact on next-generation bolometric tonne-scale experiments, like CUPID. Abstract

Reactor design requires safety studies to ensure that the reactors will behave appropriately under incidental or accidental situations. Safety studies often involve multiphysics simulations where several branches of reactor physics are necessary to model a given phenomenon. In those situations, it has been observed that the neutron transport part is still a bottleneck in terms of computational times, with more than 80% of the total time. In the case of hexagonal lattice reactors, transport solvers usually invert the discretised Boltzmann equation by discretising the regular hexagon into lozenges or triangles. In this work, we seek to reduce the computational burden of the neutron transport solver by designing a numerical spatial discretisation scheme that would be more appropriate for honeycomb meshes. In our past research efforts, we have set up interesting discretisation schemes in the finite element setting in 2D, and we wish to extend them to 3D geometries that are prisms with a hexagonal base. In 3D, a rigorous method was derived to shrink the tensor product between 2D and 1D bases to minimum terms. We have applied these functions successfully on a reactor benchmark—Takeda Model 4—to compare and contrast the numerical results in a physical setting.

Abstract

Human parechoviruses (HPeVs) are known to cause meningo-encephalitis among neonates and infants. We aimed to describe the epidemiology of HPeVs causing central nervous system infections in Alberta from 2014 to 2019 with comparison of known HPeV-3 and emerging HPeV-5. Genomic analysis was completed on a subset of HPeV-5 strains to understand genetic relatedness to other known strains. All cerebrospinal fluid (CSF) samples in Alberta with detection of HPeVs were identified and a case review of medical records was conducted, retrospectively, to gather demographic and clinical details. Descriptive and analytic statistics were used to describe and compare the characteristics of cases affected by HPeV-3 with HPeV-5. Genome amplification was completed on six HPeV-5 samples. During the study period, 18,882 CSF samples were tested; 56 were positive for HPeV-3 or HPeV-5, and 52 patients were included in this study (40 HPeV-3 cases and 12 HPeV-5). A total of 40% of cases occurred in 2016, and 64% of infections occurred in the months of August to October. The mean age of cases was 18 days for HPeV-5 compared with 26 days for HPeV-3 (p = 0.045). Phylogenetic comparison showed similarity to a recombinant strain reported in Australia. HPeV meningo-encephalitis affected neonates/infants, mostly in late summer/early fall, and genomic sequencing of new strains can contribute to understanding the epidemiology of HPeV infections globally.

Abstract

The question of whether SARS-CoV-2 is mainly transmitted by droplets or aerosols has been highly controversial. We sought to explain this controversy through a historical analysis of transmission research in other diseases. For most of human history, the dominant paradigm was that many diseases were carried by the air, often over long distances and in a phantasmagorical way. This miasmatic paradigm was challenged in the mid to late 19th century with the rise of germ theory, and as diseases such as cholera, puerperal fever, and malaria were found to actually transmit in other ways. Motivated by his views on the importance of contact/droplet infection, and the resistance he encountered from the remaining influence of miasma theory, prominent public health official Charles Chapin in 1910 helped initiate a successful paradigm shift, deeming airborne transmission most unlikely. This new paradigm became dominant. However, the lack of understanding of aerosols led to systematic errors in the interpretation of research evidence on transmission pathways. For the next five decades, airborne transmission was considered of negligible or minor importance for all major respiratory diseases, until a demonstration of airborne transmission of tuberculosis (which had been mistakenly thought to be transmitted by droplets) in 1962. The contact/droplet paradigm remained dominant, and only a few diseases were widely accepted as airborne before COVID-19: those that were clearly transmitted to people not in the same room. The acceleration of interdisciplinary research inspired by the COVID-19 pandemic has shown that airborne transmission is a major mode of transmission for this disease, and is likely to be significant for many respiratory infectious diseases.

Abstract

Vaccination hesitancy is a major obstacle to achieving and maintaining herd immunity. It is therefore of prime importance for public health authorities to understand the dynamics of an anti-vaccine opinion in the population. We introduce a novel mathematical model of opinion dynamics with spatial reinforcement, which can generate echo chambers, i.e. opinion bubbles in which information that is incompatible with one's entrenched worldview, is likely disregarded. In a first mathematical part, we scale the model both to a deterministic limit and to a weak-effects limit, and obtain bifurcations, phase transitions, and the invariant measure. In a second part, we fit our model to measles and meningococci vaccination coverage across 413 districts in Germany. We reveal that strong echo chambers explain the occurrence and persistence of the anti-vaccination opinion. We predict and compare the effectiveness of different policies aimed at influencing opinion dynamics in order to increase vaccination uptake.

Abstract

Due to the relevance for conservation biology, there is an increasing interest to extend evolutionary genomics models to plant, animal or microbial species. However, this requires to understand the effect of life-history traits absent in humans on genomic evolution. In this context, it is fundamentally of interest to generalize the replicator equation, which is at the heart of most population genomics models. However, as the inclusion of life-history traits generates models with a large state space, the analysis becomes involving. We focus, here, on quiescence and seed banks, two features common to many plant, invertebrate and microbial species. We develop a method to obtain a low-dimensional replicator equation in the context of evolutionary game theory, based on two assumptions: (1) the life-history traits are {\it per se} neutral, and (2) frequency-dependent selection is weak. We use the results to investigate the evolution and maintenance of cooperation based on the Prisoner's dilemma. We first consider the generalized replicator equation, and then refine the investigation using adaptive dynamics. It turns out that, depending on the structure and timing of the quiescence/dormancy life-history trait, cooperation in a homogeneous population can be stabilized. We finally discuss and highlight the relevance of these results for plant, invertebrate and microbial communities.

44. What do we know about Objective Structured Clinical Examination in Sport and Exercise Medicine? A scoping review

Abstract

Background and objectives: Despite the importance of the Objective Structured Clinical Examination (OSCE) in Sport and Exercise Medicine, the literature on the topic is fragmented and has been poorly developed. The goal of this review was to map current knowledge about how the OSCE is used in Sport and Exercise Medicine, and to identify knowledge gaps for future research. Method: The authors conducted a scoping review. They searched PubMed and Scopus for articles using key terms related to 'OSCE' and 'sport medicine' with no limit on search start date and up to July 2022. Retrieved records were imported, abstracts were screened, and full-text articles were reviewed. A forward and backward citation tracking was conducted. Data was extracted and a qualitative meta-summary of the studies was conducted. Results: A total of 469 records were screened, and 22 studies were included. The objectives of the studies included using OSCEs to assess knowledge/skills after a training program (n = 11), to assess an intervention (n = 8), and to assess and improve the OSCE itself (n = 3). Thirteen studies reported validity and/or reliability of the OSCE. Conclusion: Despite the widespread use of OSCEs in the examination of Sport and Exercise Medicine trainees, only a handful of scholarly works have been published. More research is needed to support the use of OSCE in Sport and Exercise Medicine for its initial purpose. We highlight avenues for future research such as assessing the need for a deeper exploration of the relationship between candidate characteristics and OSCE scores.

45. Empirical sample-specific approaches to define HPV16 and HPV18 seropositivity in unvaccinated, young, sexually active women

Abstract

ABSTRACT Given low seroconversion rates following human papillomavirus (HPV) infection, fixed external cutoffs may lead to errors in estimating HPV seroprevalence. We evaluated finite mixture modeling (FMM) and group-based trajectory modeling (GBTM) among unvaccinated, sexually active, HPV-exposed women to determine study-specific HPV16 and HPV18 seropositivity thresholds. We included 399 women (aged 18–24 years) enrolled in the HPV Infection and Transmission Among Couples Through Heterosexual Activity (HITCH) cohort study between 2005 and 2011 in Montreal, Canada. Participants’ blood samples from up to six visits spanning 2 years were tested by multiplex serology for antibodies [median fluorescence intensity (MFI)] specific to bacterially expressed HPV16 and HPV18 L1 glutathione S-transferase fusion proteins. We applied FMM and GBTM to baseline and longitudinal antibody titer measurements, respectively, to define HPV type-specific seronegative and seropositive distributions. Study-specific thresholds were generated as five standard deviations above the mean seronegative antibody titers, mimicking cutoffs (HPV16: 422 MFI; HPV18: 394 MFI) derived from an external population of sexually inactive, HPV DNA-negative Korean women (aged 15–29 years). Agreement (kappa) of study-specific thresholds was evaluated against external cutoffs. Seroprevalence estimates using FMM (HPV16: 27.5%–43.2%; HPV18: 21.7%–49.5%) and GBTM (HPV16: 11.8%–11.8%; HPV18: 9.9%–13.4%) thresholds exceeded those of external cutoffs (HPV: 10.2%; HPV18: 9.7%). FMM thresholds showed slight-to-moderate agreement with external cutoffs (HPV16: 0.26%–0.46%; HPV18: 0.20%–0.56%), while GBTM thresholds exhibited high agreement (HPV16: 0.92%–0.92%; HPV18: 0.82%–0.99%). Kappa values suggest that GBTM, used for longitudinal serological data, and otherwise FMM, for cross-sectional data, are robust methods for determining the HPV serostatus without prior classification rules.IMPORTANCEWhile human papillomavirus (HPV) seropositivity has been employed as an epidemiologic determinant of the natural history of genital HPV infections, only a fraction of women incidentally infected with HPV respond by developing significant antibody levels. HPV seropositivity is often determined by a dichotomous fixed cutoff based on the seroreactivity of an external population of women presumed as seronegative, given the lack of evidence of HPV exposure. However, considering the variable nature of seroreactivity upon HPV infection, which arguably varies across populations, such externally defined cutoffs may lack specificity to the population of interest, causing inappropriate assessment of HPV seroprevalence and related epidemiologic uses of that information. This study demonstrates that finite mixture modeling (FMM) and group-based trajectory modeling (GBTM) can be used to independently estimate seroprevalence or serve as the basis for defining study-specific seropositivity thresholds without requiring prior subjective assumptions, consequently providing a more apt internally valid discrimination of seropositive from seronegative individuals.

Abstract

Background: COVID-19 is primarily known as a respiratory illness; however, many patients present to hospital without respiratory symptoms. The association between non-respiratory presentations of COVID-19 and outcomes remains unclear. We investigated risk factors and clinical outcomes in patients with no respiratory symptoms (NRS) and respiratory symptoms (RS) at hospital admission. Methods: This study describes clinical features, physiological parameters, and outcomes of hospitalised COVID-19 patients, stratified by the presence or absence of respiratory symptoms at hospital admission. RS patients had one or more of: cough, shortness of breath, sore throat, runny nose or wheezing; while NRS patients did not. Results: Of 178,640 patients in the study, 86.4 % presented with RS, while 13.6 % had NRS. NRS patients were older (median age: NRS: 74 vs RS: 65) and less likely to be admitted to the ICU (NRS: 36.7 % vs RS: 37.5 %). NRS patients had a higher crude in-hospital case-fatality ratio (NRS 41.1 % vs. RS 32.0 %), but a lower risk of death after adjusting for confounders (HR 0.88 [0.83–0.93]). Conclusion: Approximately one in seven COVID-19 patients presented at hospital admission without respiratory symptoms. These patients were older, had lower ICU admission rates, and had a lower risk of in-hospital mortality after adjusting for confounders.

Abstract

Salmonella infection causes epidemic death in wild songbirds, with potential to spread to humans. In February 2021, public health officials in Oregon and Washington, USA, isolated a strain of Salmonella enterica serovar Typhimurium from humans and a wild songbird. Investigation by public health partners ultimately identified 30 illnesses in 12 states linked to an epidemic of Salmonella Typhimurium in songbirds. We report a multistate outbreak of human salmonellosis associated with songbirds, resulting from direct handling of sick and dead birds or indirect contact with contaminated birdfeeders. Companion animals might have contributed to the spread of Salmonella between songbirds and patients; the outbreak strain was detected in 1 ill dog, and a cat became ill after contact with a wild bird. This outbreak highlights a One Health issue where actions like regular cleaning of birdfeeders might reduce the health risk to wildlife, companion animals, and humans.
