Abstract

ABSTRACT: The advent of immunological therapies has revolutionized the treatment of solid and haematological cancers over the last decade. Licensed therapies which activate the immune system to target cancer cells can be broadly divided into two classes. The first class are antibodies that inhibit immune checkpoint signalling, known as immune checkpoint inhibitors (ICIs). The second class are cell‐based immune therapies including chimeric antigen receptor T lymphocyte (CAR‐T) cell therapies, natural killer (NK) cell therapies, and tumour infiltrating lymphocyte (TIL) therapies. The clinical efficacy of all these treatments generally outweighs the risks, but there is a high rate of immune‐related adverse events (irAEs), which are often unpredictable in timing with clinical sequalae ranging from mild (e.g. rash) to severe or even fatal (e.g. myocarditis, cytokine release syndrome) and reversible to permanent (e.g. endocrinopathies).The mechanisms underpinning irAE pathology vary across different irAE complications and syndromes, reflecting the broad clinical phenotypes observed and the variability of different individual immune responses, and are poorly understood overall. Immune‐related cardiovascular toxicities have emerged, and our understanding has evolved from focussing initially on rare but fatal ICI‐related myocarditis with cardiogenic shock to more common complications including less severe ICI‐related myocarditis, pericarditis, arrhythmias, including conduction system disease and heart block, non‐inflammatory heart failure, takotsubo syndrome and coronary artery disease. In this scientific statement on the cardiovascular toxicities of immune therapies for cancer, we summarize the pathophysiology, epidemiology, diagnosis, and management of ICI, CAR‐T, NK, and TIL therapies. We also highlight gaps in the literature and where future research should focus.

2. Incertus.jl -- The Julia Lego Blocks for Randomized Clinical Trial Designs

Abstract

In this paper, we present Insertus.jl, the Julia package that can help the user generate a randomization sequence of a given length for a multi-arm trial with a pre-specified target allocation ratio and assess the operating characteristics of the chosen randomization method through Monte Carlo simulations. The developed package is computationally efficient, and it can be invoked in R. Furthermore, the package is open-ended -- it can flexibly accommodate new randomization procedures and evaluate their statistical properties via simulation. It may be also helpful for validating other randomization methods for which software is not readily available. In summary, Insertus.jl can be used as ``Lego Blocks'' to construct a fit-for-purpose randomization procedure for a given clinical trial design.

3. On the Expressive Power of Sparse Geometric MPNNs

Abstract

Motivated by applications in chemistry and other sciences, we study the expressive power of message-passing neural networks for geometric graphs, whose node features correspond to 3-dimensional positions. Recent work has shown that such models can separate generic pairs of non-equivalent geometric graphs, though they may fail to separate some rare and complicated instances. However, these results assume a fully connected graph, where each node possesses complete knowledge of all other nodes. In contrast, often, in application, every node only possesses knowledge of a small number of nearest neighbors. This paper shows that generic pairs of non-equivalent geometric graphs can be separated by message-passing networks with rotation equivariant features as long as the underlying graph is connected. When only invariant intermediate features are allowed, generic separation is guaranteed for generically globally rigid graphs. We introduce a simple architecture, EGENNET, which achieves our theoretical guarantees and compares favorably with alternative architecture on synthetic and chemical benchmarks.

4. Efficacy and safety of mij821 in patients with treatment-resistant depression: Results from a randomized, placebo-controlled, proof-of-concept study

Abstract

Introduction MIJ821 is a novel N-methyl-D-aspartate (NMDA) receptor antagonist, with a potentially low rate of the psychotomimetic side effects that limit the therapeutic utility of ketamine in treatment-refractory depression (TRD). Objectives To assess efficacy and safety of MIJ821. Methods Adults with TRD (>2 prior treatment failures; Montgomery-Asberg Depression Rating Scale [MADRS], >24) were eligible and were randomized (n=70) to low versus high doses of MIJ821, with two dosing regimens of weekly or biweekly, versus ketamine versus placebo. The primary outcome was change in MADRS total score at 24 hours and final follow up was at 6 weeks. Results At 24 hours, adjusted mean differences (ΔAM) versus placebo were –8.25 (p=0.001), –5.71 (p=0.019) and –5.67 (p=0.046) and at 48 h were –7.06 (p=0.013), –7.37 (p=0.013), –11.02 (p=0.019) in the pooled MIJ821 low dose, high dose, and ketamine groups, respectively. At 6 weeks, ΔAM (80% CI) versus placebo on MADRS were –6.46 (–11.8, –1.15); p=0.059 for low dose MIJ821, –5.42 (–10.8, –0.02); p=0.099) for high dose MIJ821, and –5.24 (–10.4, –0.06); p=0.097 for ketamine. Further details on dosing, efficacy, and safety outcomes will be provided. Conclusions In this proof-of-concept study, MIJ821 was effective and tolerable in TRD. This study was funded by Novartis. 7. Variability and Reliability of Graphene Field-Effect Transistors with CaF2 Insulators

Abstract

Graphene is a promising material for applications as a channel in graphene field-effect transistors (GFETs) which may be used as a building block for optoelectronics, high-frequency devices and sensors. However, these devices require gate insulators which ideally should form atomically flat interfaces with graphene and at the same time contain small densities of traps to maintain high device stability. Previously used amorphous oxides, such as SiO2 and Al2O3, however, typically suffer from oxide dangling bonds at the interface, high surface roughness and numerous border oxide traps. In order to address these challenges, here we use for the first time 2nm thick epitaxial CaF2 as a gate insulator in GFETs. By analyzing device-to-device variability for over 200 devices fabricated in two batches, we find that tens of them show similar gate transfer characteristics. Our statistical analysis of the hysteresis up to 175C has revealed that while an ambient-sensitive counterclockwise hysteresis can be present in some devices, the dominant mechanism is thermally activated charge trapping by border defects in CaF2 which results in the conventional clockwise hysteresis. We demonstrate that both the hysteresis and bias-temperature instabilities in our GFETs with CaF2 are comparable to similar devices with SiO2 and Al2O3. In particular, we achieve a small hysteresis below 0.01 V for equivalent oxide thickness (EOT) of about 1 nm at the electric fields up to 15 MV/cm and sweep times in the kilosecond range. Thus, our results demonstrate that crystalline CaF2 is a promising insulator for highly-stable GFETs.

8. The vascular plant species of different protection levels in the ecosystems of the regional landscape parks of the Ukrainian Polissia

Abstract

The Ukrainian Polissia is a distinctive region in terms of biodiversity, with a significant number of northern and boreal species and communities, as well as Central European species in the Western Polissia. The research region stretches from west to east for 750 km, and from north to south for 120–150 km. In the northeast direction, its territory gradually narrows to 50–80 km and constitutes a significant part of the important ecological corridor of Eastern Europe. Background. The natural conditions of the Ukrainian Polissia are favorable for the formation of coniferous, broadleaf and mixed forests, which are zonal communities here. The widespread distribution of fluvioglacial sediments causes the predominance of sandy sod-podzolic soils and associated pine forests. The high groundwater table and the low surface area contribute to the development of eutrophic peat bogs. All this leads to the formation of a diverse vegetation cover, which contributes to the representativeness and significant preservation of floristic and coenotic diversity. One of the key roles in the protection of rare species of phytodiversity in the Polissia region of Ukraine belongs to multifunctional protected areas, including the category of “regional landscape parks”. They cover more than 200 species of vascular plants of the Polissia region, with diffe­rent conservation statuses – from the international level of protection to the regional one. Materials and Methods. The study was conducted using conventional field (route-based, geobotanical descriptions of key areas) and desk methods. The species names are given according to the International Plant Names Index, and their belonging to the protected categories is based on the conservation criteria and protection levels accor­ding to the regulatory documents. Results and Discussion. The materials of the research of phytodiversity of five regional landscape parks (Prypiat-Stokhid, Nadsluchanskyi, Ptashynyy Rai, Mizhrichenskyi, Yalivshchyna), in particular rare species, are presented, as evidenced by their diversity, including a number of glacial relics, endemics and borderline species located at the southern or eastern border of the distribution area. Conclusions. The study has revealed that the rare vascular plant flora of the Polissia regional landscape parks is represented by 56 species included in the Red Data Book of Ukraine, 145 species under regional protection of three oblasts of the Polissia region, nine species from Appendix I of the Bern Convention, four species from Appendix II of CITES, and three species included in the European Red List. Two RLPs are the most sozologically representative in the study area: Nadsluchansky (six species of international protection statuses, 30 species of the Red Data Book of Ukraine, 107 species of regional protection level) and Mizhrichynsky (14 species with international protection statuses, 28 species of the Red Data Book of Ukraine, 51 species of regional protection level), which is determined by the peculiarity of natural conditions, habitat diversity and area of the territories. regional landscape parks as a category of nature reserve fund of regional status within the Polissia region of Ukraine cover most of the representative habitats, which include a corresponding number of rare species of different levels of protection, namely: forest (87 species), herbaceous (85 species), marsh (36 species) and aquatic (30 species) habitats.

Abstract

Global collaboration in cardio-oncology is needed to understand the prevalence of cancer therapy-related cardiovascular toxicity in different risk groups, practice settings, and geographic locations. There are limited data on the socioeconomic and racial/ethnic disparities that may impact access to care and outcomes. To address these gaps, we established the Global Cardio-Oncology Registry, a multinational, multicenter prospective registry.We assembled cardiologists and oncologists from academic and community settings to collaborate in the first Global Cardio-Oncology Registry. Subsequently, a survey for site resources, demographics, and intention to participate was conducted. We designed an online data platform to facilitate this global initiative.A total of 119 sites responded to an online questionnaire on their practices and main goals of the registry: 49 US sites from 23 states and 70 international sites from 5 continents indicated a willingness to participate in the Global Cardio-Oncology Registry. Sites were more commonly led by cardiologists (85/119; 72%) and were more often university/teaching (81/119; 68%) than community based (38/119; 32%). The average number of cardio-oncology patients treated per month was 80 per site. The top 3 Global Cardio-Oncology Registry priorities in cardio-oncology care were breast cancer, hematologic malignancies, and patients treated with immune checkpoint inhibitors. Executive and scientific committees and specific committees were established. A pilot phase for breast cancer using Research Electronic Data Capture Cloud platform recently started patient enrollment.We present the structure for a global collaboration. Information derived from the Global Cardio-Oncology Registry will help understand the risk factors impacting cancer therapy-related cardiovascular toxicity in different geographic locations and therefore contribute to reduce access gaps in cardio-oncology care. Risk calculators will be prospectively derived and validated.

Abstract

Despite the high prevalence of heart failure in the western world, there are few effective treatments. Fibulin-3 is a protein involved in extracellular matrix (ECM) structural integrity, however its role in the heart is unknown. We have demonstrated, using single cell RNA-seq, that fibulin-3 was highly expressed in quiescent murine cardiac fibroblasts, with expression highest prior to injury and late post-infarct (from ~ day-28 to week-8). In humans, fibulin-3 was upregulated in left ventricular tissue and plasma of heart failure patients. Fibulin-3 knockout (Efemp1-/-) and wildtype mice were subjected to experimental myocardial infarction. Fibulin-3 deletion resulted in significantly higher rate of cardiac rupture days 3-6 post-infarct, indicating a weak and poorly formed scar, with severe ventricular remodelling in surviving mice at day-28 post-infarct. Fibulin-3 knockout mice demonstrated less collagen deposition at day-3 post-infarct, with abnormal collagen fibre-alignment. RNA-seq on day-3 infarct tissue revealed upregulation of ECM degradation and inflammatory genes, but downregulation of ECM assembly/structure/organisation genes in fibulin-3 knockout mice. GSEA pathway analysis showed enrichment of inflammatory pathways and a depletion of ECM organisation pathways. Fibulin-3 originates from cardiac fibroblasts, is upregulated in human heart failure, and is necessary for correct ECM organisation/structural integrity of fibrotic tissue to prevent cardiac rupture post-infarct.

Abstract

In the era of big data, standard analysis tools may be inadequate for making inference and there is a growing need for more efficient and innovative ways to collect, process, analyze and interpret the massive and complex data. We provide an overview of challenges in big data problems and describe how innovative analytical methods, machine learning tools and metaheuristics can tackle general healthcare problems with a focus on the current pandemic. In particular, we give applications of modern digital technology, statistical methods, data platforms and data integration systems to improve diagnosis and treatment of diseases in clinical research and novel epidemiologic tools to tackle infection source problems, such as finding Patient Zero in the spread of epidemics. We make the case that analyzing and interpreting big data is a very challenging task that requires a multi-disciplinary effort to continuously create more effective methodologies and powerful tools to transfer data information into knowledge that enables informed decision making.

Abstract

The Clifford spectrum is an elegant way to define the joint spectrum of several Hermitian operators. While it has been know that for examples as small as three $2$-by-$2$ matrices the Clifford spectrum can be a two-dimensional manifold, few concrete examples have been investigated. Our main goal is to generate examples of the Clifford spectrum of three or four matrices where, with the assistance of a computer algebra package, we can calculate the Clifford spectrum., Comment: The Mathematica and video ancillary files are available at https://math.unm.edu/~loring/CliffordExperiments/. Abstract

This position statement from the Heart Failure Association of the European Society of Cardiology Cardio-Oncology Study Group in collaboration with the International Cardio-Oncology Society presents practical, easy-to-use and evidence-based risk stratification tools for oncologists, haemato-oncologists and cardiologists to use in their clinical practice to risk stratify oncology patients prior to receiving cancer therapies known to cause heart failure or other serious cardiovascular toxicities. Baseline risk stratification proformas are presented for oncology patients prior to receiving the following cancer therapies: anthracycline chemotherapy, HER2-targeted therapies such as trastuzumab, vascular endothelial growth factor inhibitors, second and third generation multi-targeted kinase inhibitors for chronic myeloid leukaemia targeting BCR-ABL, multiple myeloma therapies (proteasome inhibitors and immunomodulatory drugs), RAF and MEK inhibitors or androgen deprivation therapies. Applying these risk stratification proformas will allow clinicians to stratify cancer patients into low, medium, high and very high risk of cardiovascular complications prior to starting treatment, with the aim of improving personalised approaches to minimise the risk of cardiovascular toxicity from cancer therapies.
