Background. This study aimed to improve the clinical effect of dental orthopedic treatment for patients who need a complete dental reconstruction, by making use of the temporomandibular joint characteristics. Materials and Methods. A total of 647 patients with adentia who needed a complete dental reconstruction were exposed to a comprehensive examination and treatment. Patients were divided into 3 groups: orthopedic rehabilitation group (218 patients), interdisciplinary rehabilitation group (195 patients), and multidisciplinary rehabilitation group (234 patients). The follow-up period was 7 years. Results. After the proposed rehabilitation approach, patients in the main group demonstrated an improvement in the VAS (Visual Analogue Scale) score, which was significantly smaller with respect to that in the comparison groups. The frequency of clicking and pain during chewing as well as soreness sensations in this group decreased at a significantly lower pace compared to that in the comparison groups. Conclusions. Dental reconstruction procedures aimed at restoring the shape of the teeth should focus on pain relief and temporomandibular joint function correction. The most important stage in implementing this approach is a complete and consistent examination of patients who need dental reconstruction.

Objective. To study the dynamics of esthetic indicators during orthopedic dental treatment of patients requiring full-mouth rehabilitation. Materials and methods. Within the study, an examination and treatment of 647 patients with adentia requiring full-mouth rehabilitation were carried out. Patients were divided into three groups. In the first group (218 people), the treatment with standard orthopedic dentistry techniques was performed. The medical care of the second group (195 people) included only several elements of the multidisciplinary approach, while for the third group (234 people), only the interdisciplinary approach was applied. The monitoring of involved patients was carried out for seven years. Results. The application of the developed integrated multidisciplinary approach to orthopedic rehabilitation contributes to the fact that patients with adentia have a more favorable dynamics of esthetic indicators in relation to those who underwent a standard complex of treatment and rehabilitation measures. Thus, a statistically significant decrease was noted in the number of patients with the vertical shift of the smile line; smile asymmetry; gingival displacement; the impaired position of the incisors and fangs; mispronunciation of F and S sounds; inaccurate buccal tube placement; negative changes in the visualization of central incisors under the red border of the lips; differences in the size of the upper and lower central incisors; inadequate proximal contacts; and the inclination of the lower incisors. Conclusions. The use of an interdisciplinary approach to the treatment of patients with adentia requiring full-mouth rehabilitation contributes to the constant improvement of aesthetic characteristics of the maxillofacial area.

This study aimed to test the algorithm of complex clinical, functional, and instrumental analysis in the context of treatment for patients with a need of complete dental reconstruction. Materials and methods. A rehabilitation program for patients with adentia was proposed, which was based on the objective examination data obtained comprehensively with multiple diagnostic methods. Particular attention was paid to the temporomandibular joint function and symptoms of maxillofacial muscle abnormalities. Results. The clinical case report covers the results of the examination and treatment of a patient who previously received implants in one of the clinics without an appropriate examination and an operating template. A set of diagnostic and therapeutic procedures was defined that included clinical, functional, and instrumental analysis of condylographic and cephalometric data. The outcomes from treatment and rehabilitation allowed the optimal occlusal load distribution, and the oral hygiene improved. The approach that was used here enabled a timely correction of functional and esthetic disorders. Conclusions. The algorithm to treat patients who need complete dental reconstruction should include asking the medical history, clinical instrumental analysis (condylography), recording and evaluating the static and dynamic occlusal characteristics, and assessing cephalometric and esthetic characteristics. The algorithm is anatomically and pathogenetically justified, since it takes into account all alterations and interconnections between dentofacial structures and other body systems that underlie the clinical manifestations in patients with adentia who have a need for complete dental reconstruction.

Introduction. Aim of the study is testing the algorithm of complex clinical, functional and instrumental analysis in the treatment of patients with the need for total restoration of the dentition with manifestations of myofascial pain syndrome. Material and methods. A system for the rehabilitation of patients with adentia is proposed, in which, when planning occlusion correction, objective examination data for patients obtained using a set of diagnostic methods should be taken into account. Particular attention is paid to assessing the function of the temporomandibular joint and the presence of pathological signs of disorders of maxillofacial muscles. Results. A clinical case is described a patient who has been diagnosed with a set of indicators of clinical, functional and instrumental analysis obtained using methods of condylography and cephalometry. The results of treatment and rehabilitation measures allowed achieving the optimal distribution of loads on the dentition, while reducing the risk of ceramic chipping and improving oral hygiene. The approach used allowed timely correction of functional and aesthetic disorders. Conclusions. The algorithm for working with patients who need total restoration of the dentition should include a thorough history taking, clinical functional analysis using condylography methods, model analysis to register and evaluate the static and dynamic ratios of the dentition. As well as performing cephalometric analysis and other manipulations, in accordance with standard criteria for clinical examination. The developed algorithm is anatomically and pathogenetically justified, since it takes into account the entirety of changes and interconnections of the structures of the dentofacial system and other body systems that underlie the clinical manifestations in this category of patients.

This case report involved the development of a rehabilitation system for patient with edentulism and an algorithm for diagnostic and therapeutic measures to be planned based on the patient’s individual anatomical and physiological features. The results of treatment and rehabilitation made it possible to achieve the optimal distribution of loads on the dentition. It was shown that it is necessary to analyze casts in an articulator to reproduce a three-dimensional model of mandibular movements taking into account the individual features of the patient to obtain a satisfactory assessment and treatment planning of patients with edentulism requiring full mouth reconstruction. This case confirms that the developed algorithm is anatomically and pathogenetically justified, as it considers all changes and interrelationships of structures of the dentition.

Objectives Planning, adequate preparation, and further prosthodontic management are the most important steps in the treatment of this population of patients with dental and mandibular anomalies. The purpose of the article is the improvement of safety and clinical effectiveness of prosthodontic rehabilitation of patients after full-mouth reconstruction. Materials and Methods A single-center open prospective nonrandomized study, examination, and treatment of 198 patients with partial or total absence of teeth were performed. Higher safety and clinical effectiveness of the treatment were found in the patients of the main group who showed a significant reduction in the rate of complications relative to the comparison group. In the main group, the number of aesthetic defects and cases of repeated prosthetics decreased in comparison with the corresponding occurrence rate of such cases in patient groups. Results It showed statistically significantly reduction in the rates of occurrence of the complications when using the proposed algorithm: the rate of inflammation of the marginal periodontium and peri-implant tissues in the main group was 2.9 times less frequent and loosening of the supporting implants was 3.9 times less frequent. The rate of polymer chipping or abrasion in the area of the incisal edge or masticatory surface was two times lower in the main group of patients, short crowns were found 1.9 times less often, sores in the area of the dental pontic in the main group were 1.8 times less frequent than in the comparison group, and no fractures of bridgework were found in the main group. The frequency of neuralgic facial pain in the patients of the main group was 5.2 times less than in the comparison group, complaints about pain in the temporomandibular joint area were 8.4 times less frequent, increased abrasion of antagonist teeth was 3.9 times less frequent, and phonetics disorders were 8.4 times less frequent.The effectiveness of the algorithm developed by us is also confirmed by the fact that the number of aesthetic defects and cases of the need for repeated prosthetic treatment was reduced in comparison with the corresponding frequency of such cases in groups of patients where standard approaches to prosthetics were applied. In particular, aesthetic defects found in the main group of patients were 2.5 times less frequent than in the comparison group, and repeated prosthetics was performed 2.7 times less frequently. Conclusion Application of the developed comprehensive approach to planning of prosthodontic rehabilitation provides for significant reduction in the frequency of complications and aesthetic defects, as well as cases of repeated need for prosthetics.

Aim: based on occlusion and temporomandibular health indicators, to improve clinical effectiveness of dental orthopedic treatment of patients with a need for a full mouth reconstruction. Materials and methods. A total of 647 patients with adentia and a need for a full mouth reconstruction received comprehensive examination and treatment. Patients were divided into 3 groups. Group 1 (n=218) was exposed to standard treatment procedures. Group 2 (n=195) received certain elements of the interdisciplinary approach to dental orthopedic rehabilitation. Group 3 (n=234) was exposed to interdisciplinary approach towards dental orthopedic rehabilitation. The follow-up period was 7 years. A key step in the treatment of group 3 patients was to conduct full patient examinations sequentially. The corresponding procedures were supplemented with tests for temporomandibular disorder and occlusal relationships. In treatment planning, the cause-and-effect relationships between this disorder and malocclusion were identified. Results. It was found that the use of the present proposal contributes to the normalization of occlusion. For instance, the occlusal index dropped 2.1-fold (relative to that in the comparison groups). The frequency of ICP manifestations decreased by 2.1 to 4.3 times, the protrusion frequency decreased by 12.2 to 22.3 times, and the bruxism frequency became 4.8 to 5.2 times lower. Furthermore, the frequency of left and right mediotrusion detection dropped by 3.2—4.3 and 3.8—4.4 times, respectively. The revealed changes were persistent, and a positive effect maintained throughout the follow-up. Conclusion. The interdisciplinary approach to the restoration of the anatomical shape of teeth should involve the assessment and correction of occlusal disturbances.

Untimely diagnosis of the oral cavity leads to an underlying periodontal inflammation. Therefore, it was raised an issue of psychological correction in patients with inflammatory periodontal diseases where it was observed the role of mental state and the importance of the addition of this therapeutic component to a comprehensive program for the treatment of patients in dental clinics is emphasized. The study consisted of two stages: diagnostic and therapeutic. The first stage included 76 patients who needed an examination to detect periodontal inflammation; these patients were examined using the Fluorit-4S therapeutic and diagnostic device (TDD). The second stage of the study included 28 patients with inflammatory periodontal diseases. Patients of the study group (14 people) received treatment using a mesodiencephalic modulation method (MDM-therapy). It was proved the possibility of earlier detection of periodontal inflammation (even at the level of a "latent" process) using the Fluorit-4S device. Higher therapeutic efficacy of comprehensive dental treatment in patients with periodontal inflammation using the Fluorit-4S TDD and MDM-therapy was proved compared to standard therapeutic strategy in terms of more evident reduction in local discomfort (by 0.5 points compared to the control group), more significant decrease in the PMA index (an index that measures the presence or absence of gingival inflammation as occurring on the papillae or the marginal or attached gingivae) (by 0.9% compared to the control group), fuller elimination of the pathological focus, as well as a complete normalization of the emotional state. The use of the Fluorit-4S TDD in dental practice allows dentists to study the affected tissues more closely, resulting in objective conclusions on the pathological processes in the oral mucosa and, accordingly, the selection of adequate therapeutic strategies for individual clinical cases.

The paper studies the therapeutic and prophylactic efficacy of mesodiencephalic modulation in patients with type II diabetes mild-to-moderate and undergoing dental prosthetics using dental implantation. The influence of mesodiencephalic modulation on the concentration of glucose in the blood, the cardiac rate and the emotional state of patients in the pre-implantation and post-implantation period is considered. The influence of mesodiencephalic modulation on the restoration of quality of life in the early postoperative period of dental implantation, including due to its local therapeutic effect, is estimated.

In this article considered the current vision of the problem of the influence of dental status in patients on the occurrence and/or development of squamous cell carcinoma (SCC) of the mucous membrane of the mouth (MMOC). On the basis of the some studies have noted a relationship between low level of the oral hygiene and the sharp edges of the teeth or dentures and the risk of developing this disease. Also revealed a correlation between periodontal disease and the possible development of SCC MMOC that corresponds to the main prediction about the risk of the development of this pathology on the background of the effects of chronical infection in the oral cavity. Significant correlation between the presence of carious lesions and the risk of developing SCC MNOC in contrast, is not detected. But clearly expressed role in the development of the cancers are galvanism and precancerous diseases of the oral mucous membrane and the red border of the lips.

Increase in academic performance of medical universities' students is one of the most significant tasks of the modern education and healthcare systems as a factor contributing to high quality of the future doctors' qualification. Increased level of anxiety and depression negatively correlate with the level of the students' academic performance. Such a method of physiotherapy as mesodiencephalic modulation takes a stabilising effect on psychoemotional state of a person. The paper contains the analysis of emotional state of the students from the Faculty of Dentistry, defines the level of anxiety and depression, and notes their correlational interconnection with the level of academic performance in their study. The authors have practically stated positive influence of the mesodiencephalic modulation sessions on psychoemotional state of the students and indirectly on the level of their educational academic performance.
