Abstract

Objective: To study the clinico-radiological paradox in multiple sclerosis (MS) relapse by analyzing the number and location of gadolinium-enhanced (Gd+) lesions on brain MRI before methylprednisolone (MP) treatment. Methods: We analyzed brain MRI from 90 relapsed MS patients in two Phase IV multicenter double-blind randomized clinical trials that showed the noninferiority of different routes and doses of MP administration. A 1.5- or 3-T brain MRI was performed at baseline before MP treatment and within 15 days of symptom onset. The number and location of Gd+ lesions were analyzed. Associations were studied using univariate analysis. Results: Sixty-two percent of patients had at least 1 Gd+ brain lesion; the median number was 1 (interquartile range 0–4), and 41% of patients had 2 or more lesions. The most frequent location of Gd+ lesions was subcortical (41.4%). Gd+ brain lesions were found in 71.4% of patients with brainstem-cerebellum symptoms, 57.1% with spinal cord symptoms and 55.5% with optic neuritis (ON). Thirty percent of patients with brain symptoms did not have Gd+ lesions, and only 43.6% of patients had symptomatic Gd+ lesions. The univariate analysis showed a negative correlation between age and the number of Gd+ lesions (p = 0.002). Conclusion: Most patients with relapse showed several Gd+ lesions on brain MRI, even when the clinical manifestation was outside of the brain. Our findings illustrate the clinico-radiological paradox in MS relapse and support the value of brain MRI in this scenario. Resumen: Objetivo: Estudiar la paradoja clínico-radiológica en el brote de la esclerosis múltiple (EM) mediante el análisis de lesiones captantes de gadolinio (Gd+) en la RM cerebral antes del tratamiento con metilprednisolona (MP). Métodos: Analizamos la RM cerebral basal de 90 pacientes con EM en brote de 2 ensayos clínicos aleatorizados multicéntricos fase IV que demostraron la no inferioridad de diferentes vías y dosis de MP, realizadas antes del tratamiento con MP y en los 15 días siguientes a la aparición de los síntomas. Se analizaron el número y la localización de las lesiones Gd+. Se estudiaron las asociaciones mediante análisis univariado. Resultados: El 62% de los pacientes tenía al menos una lesión Gd+ cerebral y el 41% de los pacientes tenía 2 o más lesiones. La localización más frecuente fue la subcortical (41,4%). Se encontraron lesiones Gd+ cerebrales en el 71,4% de los pacientes con síntomas de tronco cerebral o cerebelo, en el 57,1% con síntomas medulares y en el 55,5% con neuritis óptica. El 30% de los pacientes con síntomas cerebrales no tenían lesiones Gd+ y sólo el 4,.6% de los pacientes tenían lesiones Gd+ sintomáticas. El análisis univariante mostró una correlación negativa entre la edad y el número de lesiones Gd+ (p = 0,002). Conclusiones: La mayoría de los pacientes en brote mostraron varias lesiones Gd+ en la RM cerebral, incluso cuando la manifestación clínica fue medular u óptica. Nuestros hallazgos ilustran la paradoja clínico-radiológica en el brote de la EM y apoyan el valor de la RM cerebral en este escenario.

Abstract

Background and purpose: An impaired neurovascular coupling has been described as a possible player in neurodegeneration and cognitive decline. Migraine is a recurrent and incapacitating disorder that starts early in life and has shown neurovascular coupling abnormalities. Despite its high prevalence, the physiology and underlying mechanisms are poorly understood. In this context, new biomarkers from magnetic resonance imaging (MRI) are needed to bring new knowledge into the field. The aim of this study was to determine the vein density from Susceptibility-Weighted Imaging (SWI) MRI, in subjects with migraine and healthy controls; and to assess whether it relates to Resting-State functional MRI (RS-fMRI). Materials and methods: The cohort included 30 healthy controls and 70 subjects with migraine (26 episodic, 44 chronic) who underwent a brain 3.0 T MRI. Clinical characteristics were also collected. Maps of density of veins were generated based on a Mamdani Fuzzy-Type Rule-Based System from the SWI MRI. Mean values of vein density were obtained in grey (GM) and white matter (WM) Freesurfer lobar parcellations. 9. Trust-Aware Assistance Seeking in Human-Supervised Autonomy

Abstract

Our goal is to model and experimentally assess trust evolution to predict future beliefs and behaviors of human-robot teams in dynamic environments. Research suggests that maintaining trust among team members in a human-robot team is vital for successful team performance. Research suggests that trust is a multi-dimensional and latent entity that relates to past experiences and future actions in a complex manner. Employing a human-robot collaborative task, we design an optimal assistance-seeking strategy for the robot using a POMDP framework. In the task, the human supervises an autonomous mobile manipulator collecting objects in an environment. The supervisor's task is to ensure that the robot safely executes its task. The robot can either choose to attempt to collect the object or seek human assistance. The human supervisor actively monitors the robot's activities, offering assistance upon request, and intervening if they perceive the robot may fail. In this setting, human trust is the hidden state, and the primary objective is to optimize team performance. We execute two sets of human-robot interaction experiments. The data from the first experiment are used to estimate POMDP parameters, which are used to compute an optimal assistance-seeking policy evaluated in the second experiment. The estimated POMDP parameters reveal that, for most participants, human intervention is more probable when trust is low, particularly in high-complexity tasks. Our estimates suggest that the robot's action of asking for assistance in high-complexity tasks can positively impact human trust. Our experimental results show that the proposed trust-aware policy is better than an optimal trust-agnostic policy. By comparing model estimates of human trust, obtained using only behavioral data, with the collected self-reported trust values, we show that model estimates are isomorphic to self-reported responses.

In strongly coupled field theories, perturbation theory cannot be employed to study the low-energy spectrum. Thus, non-perturbative techniques are required. We employ the variational method, a rigorous, non-perturbative approach which provides variational upper bounds on the energy eigenstates of the theory. An essential step in the variational method is the choice of trial wave function. In this work, we study the viability of employing a neural network as our variational ansatz. Abstract

Occlusal contacts are the locations at which the occluding surfaces of the maxilla and the mandible posterior teeth meet. Occlusal contact detection is a vital tool for restoring the loss of masticatory function and is a mandatory assessment in the field of dentistry, with particular importance in prosthodontics and restorative dentistry. The most common method for occlusal contact detection is articulating paper. However, this method can indicate significant medically false positive and medically false negative contact areas, leaving the identification of true occlusal indications to clinicians. To address this, we propose a multiclass Vision Transformer and Fully Convolutional Network ensemble semantic segmentation model with a combination hierarchical loss function, which we name as Hierarchical Fully Convolutional Branch Transformer (H-FCBFormer). We also propose a method of generating medically true positive semantic segmentation masks derived from expert annotated articulating paper masks and gold standard masks. The proposed model outperforms other machine learning methods evaluated at detecting medically true positive contacts and performs better than dentists in terms of accurately identifying object-wise occlusal contact areas while taking significantly less time to identify them. Abstract

Measurements of circular polarization of $\gamma$-ray emitted from neutron capture reactions provide valuable information for nuclear physics studies. The spin and parity of excited states can be determined by measuring the circular polarization from polarized neutron capture reactions. Furthermore, the $\gamma$-ray circular polarization in a neutron capture resonance is crucial for studying the enhancement effect of parity nonconservation in compound nuclei. The $\gamma$-ray circular polarization can be measured using a polarimeter based on magnetic Compton scattering. A polarimeter was constructed, and its performance indicators were evaluated using a circularly polarized $\gamma$-ray beam. Abstract

We present an expanded and improved deep-learning (DL) methodology for determining centers of star images on HST/WFPC2 exposures. Previously, we demonstrated that our DL model can eliminate the pixel-phase bias otherwise present in these undersampled images; however that analysis was limited to the central portion of each detector. In the current work we introduce the inclusion of global positions to account for the PSF variation across the entire chip and instrumental magnitudes to account for nonlinear effects such as charge transfer efficiency. The DL model is trained using a unique series of WFPC2 observations of globular cluster 47 Tuc, data sets comprising over 600 dithered exposures taken in each of two filters, F555W and F814W. It is found that the PSF variations across each chip correspond to corrections of the order of 100 mpix, while magnitude effects are at a level of about 10 mpix. Importantly, pixel-phase bias is eliminated with the DL model; whereas, with a classic centering algorithm, the amplitude of this bias can be up to 40 mpix. Our improved DL model yields star-image centers with uncertainties of 8-10 mpix across the full field of view of WFPC2., Comment: accepted for publication by PASP

Abstract

Background: The development of endovascular techniques has put Interventional Neuroradiology (INR) as the first-option treatment in the majority of vascular diseases of the central nervous system. Scientific societies in developed countries have created standard procedures for training and accreditation for a safe practice in these procedures. Discussion: In Spain, we are waiting for the development of the legislation on the accreditation for specialists which will establish the official formative model to achieve an accreditation in INR. Until this moment comes, it is necessary to establish standards that define desirable minimums for the formative period in INR. Radiology specialists as well as neurologists and neurosurgeons will have access to INR accreditation. Specific requirements for the hospitals that wish to offer this technique and training should also be defined. Conclusion: The Spanish Group of Interventional Neuroradiology, the Spanish Society of Neuroradiology, the Spanish Group of Cerebrovascular Diseases, the Spanish Society of Neurology and the Spanish Society of Neurosurgery have approved the content of this document and will create a committee in order to put into practice the accreditation of formative centres and INR specialists. Abstract

Body tides reveal information about planetary interiors and affect their evolution. Most models to compute body tides rely on the assumption of a spherically-symmetric interior. However, several processes can lead to lateral variations of interior properties. We present a new spectral method to compute the tidal response of laterally-heterogeneous bodies. Compared to previous spectral methods, our approach is not limited to small-amplitude lateral variations; compared to finite element codes, the approach is more computationally-efficient. While the tidal response of a spherically-symmetric body has the same wave-length as the tidal force; lateral heterogeneities produce an additional tidal response with an spectra that depends on the spatial pattern of such variations. For Mercury, the Moon and Io the amplitude of this signal is as high as $1\%-10\%$ the main tidal response for long-wavelength shear modulus variations higher than $\sim 10\%$ the mean shear modulus. For Europa, Ganymede and Enceladus, shell-thickness variations of $50\%$ the mean shell thickness can cause an additional signal of $\sim 1\%$ and $\sim 10\%$ for the Jovian moons and Encelaudus, respectively. Future missions, such as $ \textit{BepiColombo}$ and $\textit{JUICE}$, might measure these signals. Lateral variations of viscosity affect the distribution of tidal heating. Abstract

Routine sampling of pregnant women at first antenatal care (ANC) visits could make Plasmodium falciparum genomic surveillance more cost-efficient and convenient in sub-Saharan Africa. We compare the genetic structure of parasite populations sampled from 289 first ANC users and 93 children from the community in Mozambique between 2015 and 2019. Samples are amplicon sequenced targeting 165 microhaplotypes and 15 drug resistance genes. Metrics of genetic diversity and relatedness, as well as the prevalence of drug resistance markers, are consistent between the two populations. In an area targeted for elimination, intra-host genetic diversity declines in both populations (p = 0.002-0.007), while for the ANC population, population genetic diversity is also lower (p = 0.0004), and genetic relatedness between infections is higher (p = 0.002) than control areas, indicating a recent reduction in the parasite population size. These results highlight the added value of genomic surveillance at ANC clinics to inform about changes in transmission beyond epidemiological data.
