1. The impact of SARS-Cov-2 pandemic on mental health in medical staff

Abstract: Introduction SARS-Cov-2 pandemic resulted in a great amount of mental health suffering both in patients, families and medical staff. Objectives To assess the personal impact of SARS-Cov-2 management in hospital health care staff. Methods The study included 300 participants, comprising medical staff, 150 of which directly treated patients with SARS-Cov-2 infection, whilst the rest did not. Participants were asked to fill in online self-assessment scales: PSS-10 (Perceived Stress Scale), BAI (Beck Anxiety Inventory), BDI (Beck Depression Inventory), and SSOSH (Self Stigma of Seeking Psychology Help Scale). The data were collected at the end of 2021 during the COVID-19 pandemic with the Omicron variant. Results 238 (79.3%) women and 62 (20.7%) men responded to the online assessment. Participants working in COVID wards had higher scores for anxiety and depression (χ²=12.21, p=0.007). The intensity of depression and anxiety (BDI / BAI) depends on the professional degree (higher in senior specialists and specialists than in nurses and residents), working in shifts (χ²=8.77, p=0.01) and recent contact with patients infected with SARS-Cov-2 (χ²=76.10, p

Abstract: Schizophrenia is one of the most disabling of the psychiatric diseases. The Brief Psychiatric Rating Scale Extended (BRSE) is used to evaluate the severity of psychiatric symptoms. Long-acting injectable (LAI) antipsychotics are commonly used and are preferred over oral antipsychotic medications. A two-center-based cross-sectional study was performed on 130 patients diagnosed with schizophrenia or schizoaffective disorder based on the International Classification of Diseases 10 criteria. We studied the relation between the development of cardiovascular risk factors and the antipsychotic medication that was administered in these patients. Our study demonstrates strong links between several cardiovascular risk factors and the duration of psychosis; the duration of the LAI antipsychotic treatment; the duration between the onset of the disease and the start of LAI antipsychotic treatment; and the use of specific LAI antipsychotic medications. Abstract: Science data gateways are effective in providing complex science data collections to the world-wide user communities. In this paper we describe a gateway for the Advanced Simulation Capability for Environmental Management (ASCEM) framework. Built on top of established web service technologies, the ASCEM data gateway is specifically designed for environmental modeling applications. Its key distinguishing features include (1) handling of complex spatiotemporal data, (2) offering a variety of selective data access mechanisms, (3) providing state-of-the-art plotting and visualization of spatiotemporal data records, and (4) integrating seamlessly with a distributed workflow system using a RESTful interface. ASCEM project scientists have been using this data gateway since 2011.

It has been speculated that ToM impairments could be related to cognitive deficits in PD. However, the current state of literature suggests that there is heterogeneity regarding the involvement of cognitive functioning in the relationship of PD and ToM. The study aimed to measure affective ToM abilities and cognitive performance in a sample of PD patients, to explore the link between affective ToM abilities and cognitive status, and to examine the impact of PD on affective ToM through the mediator effect of cognitive performance.Patients and methods: Sixty-five patients diagnosed with idiopathic PD and 51 healthy controls matched for age, gender and educational level completed a visual affective ToM task (Reading the Mind in the Eyes – RMET), cognitive performance was evaluated with Montreal Cognitive Assessment, and psychiatric symptoms were measured with BPRS-E (Brief Psychiatric Rating Scale).Results: Affective ToM abilities were preserved in early PD patients, declining as the disease progressed. Deficits in cognitive functioning predicted deficiencies in affective ToM. Although attention (AT), executive functions (EF) and visuospatial abilities (VSA) together mediated the relationship between PD and affective ToM, only VSA impairment had a specific negative impact on affective ToM. Moreover, 41% of the total effect of attention and executive functions on affective ToM was mediated by visuospatial skills.Conclusion: Cognitive performance may have an impact on the relationship between PD and affective ToM through the involvement of VSA. In our study, we attempted to quantify the potential causes of cardiovascular damage, assess cardiovascular parameters, and correlate them with the time elapsed from the onset of the psychosis until the initiation of injectable antipsychotic therapy, as well as the duration of long-acting therapy, and finally, to compare two of the most utilized long-acting injectable (LAI) medications (olanzapine vs risperidone).Patients and methods: This cross-sectional study recruited 64 patients of 2 outpatient clinics undergoing treatment with LAI antipsychotics for schizophrenic spectrum disorder. The study reports outpatients’ clinical data, laboratory blood sample findings, routine echocardiography, as well as speckle tracking echocardiography.Results: Among patients with longer durations of pre-long-acting antipsychotic treatment, body mass indices, mitral velocity wave values (E and A waves), and the global longitudinal strain (GLS) measurements significantly correlated with patients’ myocardial contractility. The study also found that GLS was significantly lower in the group in which pre-LAI duration was prolonged, and was not influenced by the duration of LAI treatment. Furthermore, patients receiving olanzapine showed significantly improved myocardial contractility as measured by the aforementioned parameters, in comparison with patients treated with risperidone.Conclusion: The results of our study indicate that patients suffering from schizophrenia and who are left untreated or poorly treated for a longer period of time may develop myocardial impairment. The changes may be both secondary to a high prevalence of cardiovascular risk factors and may also be generated by the disease per se. The group who received olanzapine demonstrated improved results for a longer period of time without proper medication. Abstract: BACKGROUND:Antipsychotic medication, stress, gender, and age are factors that influence prolactin levels in patients with psychosis. The aim of the study was to investigate the level of prolactin response to antipsychotic treatment in acute patients, taking into account the total duration of psychosis. METHODS AND FINDINGS:The study was conducted on 170 acute patients with schizophrenia spectrum disorders and bipolar disorder. Subjects were divided into three subgroups according to the duration of the psychosis (less than 5 years, between 5 and 10 years and more than 10 years of disorder duration). The initial prolactin response under antipsychotic treatment was measured, while the severity of the psychiatric symptoms was assessed with the BPRS (Brief Psychiatric Rating Scale). Hyperprolactinemia was found in 120 (70.6%) patients, amongst which 80 (66.7%) were females and 40 (33.3%) were males. Coping strategies such as “self-blame,” “rumination,” “catastrophizing,” “blaming others,” lack of empathy, decreased emotion recognition abilities, and neuroticism are maladaptive and, thus, less desirable traits in medical professionals. The purpose of the study was to comparatively assess and find potential correlations between personality traits, empathy levels, emotion recognition abilities, and cognitive emotion regulation strategies of three medical student samples: general medicine (GM), dental medicine (DM), and general nursing (GN) students. Patients and methods: This cross-sectional comparative study was conducted throughout the second semester of 2017, during Psychiatry class, on 306 medical undergraduates of the “Victor Babes” University of Medicine and Pharmacy, Timisoara, Romania. Personality was assessed by using Neuroticism–Extraversion–Openness to Experience Five-Factor Inventory (NEO-FFI). Cognitive emotion regulation strategies were identified using the Cognitive Emotion Regulation Questionnaire (CERQ). Empathy quotient (EQ) was used to measure empathy levels. Emotion recognition abilities were evaluated with the Reading the Mind in the Eyes test (RMET). Results: GM students scored significantly higher than both DM and GN students in blaming others (CERQ) and significantly higher than GN students in “neuroticism” (NEO-FFI). GM and DM students obtained significantly lower scores than their GN colleagues in “agreeableness” (NEO-FFI) and empathy (EQ). Compared to DM students, GN students gave significantly more correct answers in RMET. Neuroticism was associated with less efficient coping mechanisms (self-blame, rumination, catastrophizing, blaming others) and lower empathy scores. Empathy correlated negatively with blaming others and was positively associated with agreeableness and emotion recognition abilities. Conclusion: The differences found between the student samples can be consequences of several overlapping factors. Certain personality traits may predispose individuals to maladaptive coping responses, increased vulnerability to stress, and lower empathy levels. The results of this study can be viewed as baseline data for future, more comprehensive, longitudinal analyses. Abstract: Patients with severe COVID-19 experience high-stress levels and thus are at risk for developing acute stress disorder (ASD) and/or post-traumatic stress disorder (PTSD). The present study aims to search for correlations between psychiatric response to stress and coping strategies among individuals with acute vs. remitted COVID-19. Ninety subjects with COVID-19 were included in the study, divided into two samples by disease category. Our focus was analysing the perceived stress intensity according to NSESSS and PCL-C-17 scales, and coping strategies with COPE-60. High NSESSS scores were found in 40% of acute patients, and 15.6% of remitted patients had high PCL-C-17 scores fulfilling the criteria for PTSD. We found a negative correlation between stress level and disease category. Acute patients used significantly more engagement and emotion-focused coping methods, but less disengagement types of coping than patients in the remitted phase. Remitted patients under high stress levels are prone to use disengagement and emotion-focused coping strategies. In conclusion, remitted COVID-19 patients experience lower levels of stress and use less emotion-focused strategies, except among those who developed PTSD post-COVID-19 infection, presenting with high-stress levels and using more disengagement and emotion-focused types of coping strategies.

13. The Impact of Multiplex PCR in Diagnosing and Managing Bacterial Infections in COVID-19 Patients Self-Medicated with Antibiotics

Abstract: The multiplex PCR is a powerful and efficient tool that was widely used during the COVID-19 pandemic to diagnose SARS-CoV-2 infections and that has applications for bacterial identification, as well as determining bacterial resistance to antibiotics. Therefore, this study aimed to determine the usability of multiplex PCR, especially in patients self-medicated with antibiotics, where bacterial cultures often give false-negative results. A cross-sectional study was developed in two COVID-19 units, where 489 eligible patients were included as antibiotic takers and non-antibiotic takers. Antibiotic takers used mostly over-the-counter medication; they suffered significantly more chronic respiratory conditions and were self-medicated most often with cephalosporins (41.4%), macrolide (23.2%), and penicillin (19.7%). The disease severity in these patients was significantly higher than in non-antibiotic takers, and bacterial superinfections were the most common finding in the same group (63.6%). Antibiotic takers had longer hospital and ICU admissions, although the mortality rate was not significantly higher than in non-antibiotic takers. The most common bacteria involved in secondary infections were Staphylococcus aureus (22.2%), Pseudomonas aeruginosa (27.8%), and Klebsiellaspp (25.0%). Patients self-medicating with antibiotics had significantly higher rates of multidrug resistance. The multiplex PCR test was more accurate in identifying multidrug resistance and resulted in a quicker initiation of therapeutic antibiotics compared with instances where a bacterial culture was initially performed, with an average of 26.8 h vs. 40.4 h, respectively. The hospital stay was also significantly shorter by an average of 2.5 days when PCR was used as an initial assessment tool for secondary bacterial infections. When adjusted for age, COVID-19 severity, and pulmonary disease, over-the-counter use of antibiotics represented a significant independent risk factor for a prolonged hospitalization (AOR = 1.21). Similar findings were observed for smoking status (AOR = 1.44), bacterial superinfection (AOR = 1.52), performing only a conventional bacterial culture (AOR = 1.17), and a duration of more than 48 h for bacterial sampling from the time of hospital admission (AOR = 1.36). Multiplex PCR may be a very effective method for diagnosing secondary bacterial infections in COVID-19 individuals self-medicating with antibiotics. Utilizing this strategy as an initial screen in COVID-19 patients who exhibit signs of sepsis and clinical deterioration will result in a faster recovery time and a shorter period of hospitalization.

Abstract: Limits on $\nu_\mu \to \nu_e$ and $\bar\nu_\mu \to \bar\nu_e$ oscillations are extracted using the NuTeV detector with sign-selected $\nu_\mu$ and $\nub_\mu$ beams. In $\nub_\mu$ mode, for the case of $\sin^2 2\alpha = 1$, $\Delta m^2 > 2.6$ ${\rm eV^2}$ is excluded, and for $\Delta m^2 \gg 1000$ ${\rm eV^2}$, $\sin^2 2\alpha > 1.1 \times 10^{-3}$. The NuTeV data exclude the high $\Delta m^2$ end of $\bar\nu_\mu \to \bar\nu_e$ oscillations parameters favored by the LSND experiment without the need to assume that the oscillation parameters for $\nu$ and $\nub$ are the same. Abstract: A search for long-lived neutral particles (N^0) which decay into at least one muon has been performed using an instrumented decay channel at the E815 (NuTeV) experiment at Fermilab. The decay channel was composed of helium bags interspersed with drift chambers, and was used in conjunction with the NuTeV neutrino detector to search for N^0 decays. The data were examined for particles decaying into the muonic final states mu mu, mu e, and mu pi. Three mu mu events were observed over an expected background of 0.040 +/- 0.009 events; no events were observed in the other modes. Although the observed events share some characteristics with neutrino interactions, the observed rate is a factor of 75 greater than expected. Abstract: Analyses of structure functions (SFs) from neutrino and muon deep inelastic scattering data have shown discrepancies in F2 for x < 0.1. A new SF analysis of the CCFR collaboration data examining regions in x down to x=.0015 and 0.4 < Q^2 < 1.0 is presented. Comparison to corrected charged lepton scattering results for F2 from the NMC and E665 experiments are made. Differences between muon and neutrino scattering allow that the behavior of F2 from muon scattering could be different from F2 from neutrino scattering as Q^2 approaches zero. Abstract: The NuTeV experiment at Fermilab presents a determination of the electroweak mixing angle. High purity, large statistics samples of muon-neutrino and muon-antineutrino events allow the use of the Paschos-Wolfenstein relation. Abstract: Data from the CCFR E770 Neutrino Deep Inelastic Scattering (DIS) experiment at Fermilab contain events with large Bjorken x (x>0.7) and high momentum transfer (Q^2>50 (GeV/c)^2). A comparison of the data with a model based on no nuclear effects at large x, shows a significant excess of events in the data. Addition of Fermi gas motion of the nucleons in the nucleus to the model does not explain the excess. Adding a higher momentum tail due to the formation of ``quasi-deuterons'' makes some improvement. An exponentially falling F_2 \propto e^-s(x-x_0) at large x, predicted by ``multi-quark clusters'' and ``few-nucleon correlations'', can describe the data. Abstract: We present new limits on nu_e to nu_tau and nu_e to nu_sterile oscillations by searching for electron neutrino dissappearance in the high-energy wide-band CCFR neutrino beam. Sensitivity to nu_tau appearance comes from tau decay modes in which a large fraction of the energy deposited is electromagnetic. The beam is composed primarily of muon neutrinos but this analysis uses the 2.3% electron neutrino component of the beam. Electron neutrino energies range from 30 to 600 GeV and flight lengths vary from 0.9 to 1.4 km. This limit improves the sensitivity of existing limits and obtains a lowest 90% confidence upper limit in sin**2(2*alpha) of 9.9 x 10**(-2) at delta-m**2 of 125 eV**2., Comment: submitted to Phys. 37. Analysis of the Impact of Comorbidities on Endometrial Lesions Using the Charlson Comorbidity Index in Western Romania

Abstract: Background and Objectives: This retrospective study aimed to identify the main comorbidities found in gynecological patients hospitalized for endometrial lesions and to analyze the relationships between these comorbidities and each type of endometrial lesion. The Charlson comorbidity index (CCI) was calculated, thus assessing the patient’s probability of survival in relation to the underlying disease and the existing comorbidities. Materials and Methods: During 2015–2019, 594 cases hospitalized for vaginal bleeding outside of pregnancy were included in the research. For all cases, the frequency of comorbidities was calculated, applying the Cox proportional hazard model, considering the hospitalizations (from the following year after the first outpatient or hospital assessment) as a dependent variable; age and comorbidities were considered as independent variables. Results: Analysis of variance (ANOVA) for mean age of patients enrolled after diagnosis and multiple comparisons (via the Tukey post-hoc test) indicate significant differences (p < 0.05) between the average age for endometrial cancer (EC) and that for the typical endometrial hyperplasia or other diagnoses. The most common comorbidities were hypertension (62.28%), obesity (35.01%), and diabetes (22.89%), followed by cardiovascular disease. An intensely negative correlation (r = −0.715281634) was obtained between the percentage values of comorbidities present in EC and other endometrial lesions. The lowest chances of survival were calculated for 88 (14.81% of the total) patients over 50 years (the probability of survival in the next 10 years being between 0 and 21%). The chances of survival at 10 years are moderately negatively correlated with age (sample size = 594, r = −0.6706, p < 0.0001, 95% confidence interval (CI) for r having values from −0.7126 to −0.6238) and strongly negatively correlated with the CCI (r = −0.9359, p < 0.0001, 95% CI for r being in the range −0.9452 to −0.9251). Conclusions: Using CCI in endometrial lesions is necessary to compare the estimated risk of EC mortality with other medical conditions.

38. A High Statistics Search for muon-neutrino(anti-muon-neutrino) ---> electron-neutrino(anti-electron-neutrino) Oscillations in the Small Mixing Angle Regime

Abstract: Limits on $\nu_\mu (\overline{\nu}_\mu) \to \nu_e (\overline{\nu}_e)$ oscillations based on a statistical separation of $\nu_e N$ charged current interactions in the CCFR detector at Fermilab are presented. $\nu_e$ interactions are identified by the difference in the longitudinal shower energy deposition pattern of $\nu_e N \rightarrow eX$ versus $\nu_\mu N \to \nu_\mu X$ interactions. Neutrino energies range from 30 to 600 GeV with a mean of 140 GeV, and $\nu_\mu$ flight lengths vary from 0.9 km to 1.4 km. The lowest 90% confidence upper limit in $sin^2 2\alpha$ of $1.1 \times 10^{-3}$ is obtained at $\Delta m^2 \sim 300 eV^2$. For $sin^2 2\alpha = 1$, $\Delta m^2 > 1.6 eV^2$ is excluded, and for $\Delta m^2 \gg 1000 eV^2$, $sin^2 2\alpha > 1.8 \times 10^{-3}$ is excluded. This result is the most stringent limit to date for $\Delta m^2 > 25 eV^2$ and it excludes the high $\Delta m^2$ oscillation region favoured by the LSND experiment. Abstract: We present results of searches for 5-meson decays to K+π -π+ with the BABAR detector. With a data sample of 61.6 × 106 BB̄ pairs, we measure the branching fractions and 90% confidence-level upper limits averaged over charge-conjugate states (the first error is statistical and the second is systematic): B[B+ → K*0(892)π+] = (15.5 ± 1.8 -4.0+1.5) × 10-6, B[B+ → f0(980)K+, f0 → π+π -] = (9.2 ± 1.2-2.6+2.1) × 10 -6, B[B+ → D̄0π+, D̄0 → K+π-] = (184.6 ± 3.2 ± 9.7) × 10-6, B[B+ → ρ0(770)K+] < 6.2 × 10-6 and B[B+ → K+π-π+ nonresonant] < 17 × 10-6.

42. File caching in data intensive scientific applications

Abstract: We present some theoretical and experimental results of an important caching problem that arises frequently in data intensive scientific applications. In such applications, jobs need to process several files simultaneously, i.e., a job can only be serviced if all its needed files are present in the disk cache. The set of files requested by a job is called a file-bundle. This requirement introduces the need for cache replacement algorithms based on file-bundles rather then individual files. We show that traditional caching algorithms such Least Recently Used (LRU), and GreedyDual-Size (GDS), are not optimal in this case since they are not sensitive to file-bundles and may hold in the cache non-relevant combinations of files. In this paper we propose and analyze a new cache replacement algorithm specifically adapted to deal with file-bundles. We tested the new algorithm using a disk cache simulation model under a wide range of parameters such as file request distributions, relative cache size, file size distribution,and queue size. In all these tests, the results show significant improvement over traditional caching algorithms such as GDS.

Abstract: We present a study of B--->D(0)(CP)K- decays, where D(0)(CP) is reconstructed in CP-even channels, based on a sample of 88.8 x 10(6) Upsilon(4S)-->BB decays collected with the BABAR detector at the PEP-II e(+)e(-) storage ring. We measure the ratio of Cabibbo-suppressed to Cabibbo-favored branching fractions B(B--->D(0)(CP)K-)/B(B--->D(0)(CP)pi(-))=[8.8+/-1.6(stat)+/-0.5(syst)]x10(-2) and the CP asymmetry A(CP)=0.07+/-0.17(stat)+/-0.06(syst). We also measure B(B--->D0K-)/B(B--->D0pi(-))=[8.31+/-0.35(stat)+/-0.20(syst)]x10(-2) using a sample of 61.0 x 10(6) BB pairs.

Abstract: We present measurements of branching fractions and CP-violating asymmetries in decays of B mesons to two-body final states containing a K0. The results are based on a data sample of approximately 88 x 10(6) Upsilon(4S)-->BB decays collected with the BABAR detector at the PEP-II asymmetric-energy B Factory at SLAC. We measure B(B+-->K(0)pi(+))=(22.3+/-1.7+/-1.1)x10(-6), B(B0-->K(0)pi(0)=(11.4+/-1.7+/-0.8)x10(-6), B(B+-->K(0)K+)K(0)K(0)K(0)pi(+))=-0.05+/-0.08+/-0.01 and A(CP)(B0-->K(0)pi(0)=0.03+/-0.36+/-0.11.

Abstract: Using events in which one of two neutral B mesons from the decay of an Upsilon(4S) meson is fully reconstructed, we determine parameters governing decay (DeltaGamma(d)/Gamma(d)), CP, and T violation (|q/p|), and CP and CPT violation (Re z,Im z). The results, obtained from an analysis of 88 x 10(6) Upsilon(4S) decays recorded by BABAR, are sgn(Re lambda(CP))DeltaGamma(d)/Gamma(d)=-0.008+/-0.037(stat)+/-0.018(syst)[-0.084,0.068],|q/p|=1.029+/-0.013(stat)+/-0.011(syst)[1.001,1.057],(Re lambda(CP)/|lambda(CP)|) Re z=0.014+/-0.035(stat)+/-0.034(syst)[-0.072,0.101],Im z=0.038+/-0.029(stat)+/-0.025(syst)[-0.028,0.104]. The values inside the square brackets indicate the 90% confidence-level intervals. These results are consistent with standard model expectations.
