BACKGROUND AND OBJECTIVE: The role of cytoreductive nephrectomy (CN) in the treatment of metastatic renal cell carcinoma (mRCC) has been called into question on the basis of clinical trial data from the tyrosine kinase inhibitor (TKI) era. Comparative analyses of CN for patients treated with immuno-oncology (IO) versus TKI agents are sparse. Our objective was to compare CN timing and outcomes among patients who received TKI versus IO therapy. METHODS: This was a multicenter retrospective analysis of patients who underwent CN using data from the REMARCC (Registry of Metastatic RCC) database. The cohort was divided into TKI versus IO first-line therapy groups. The primary outcome was all-cause mortality (ACM). Secondary outcomes included cancer-specific mortality (CSM). Multivariable analysis was used to identify factors predictive for ACM and CSM. The Kaplan-Meier method was used to analyze 5-yr overall survival (OS) and cancer-specific survival (CSS) with stratification by primary systemic therapy and timing in relation to CN. KEY FINDINGS AND LIMITATIONS: We analyzed data for 189 patients (148 TKI + CN, 41 IO +CN; median follow-up 23.2 mo). Multivariable analysis revealed that a greater number of metastases (hazard ratio [HR] 1.06; p = 0.015), greater primary tumor size (HR 1.10; p = 0.043), TKI receipt (HR 2.36; p = 0.015), and initiation of systemic therapy after CN (HR 1.49; p = 0.039) were associated with worse ACM. Heat stress impacts ruminant livestock production on varied levels in this alarming climate breakdown scenario. The drastic effects of the global climate change-associated heat stress in ruminant livestock demands constructive evaluation of animal performance bordering on effective monitoring systems. In this climate-smart digital age, adoption of advanced and developing Artificial Intelligence (AI) technologies is gaining traction for efficient heat stress management. AI has widely penetrated the climate sensitive ruminant livestock sector due to its promising and plausible scope in assessing production risks and the climate resilience of ruminant livestock. Significant improvement has been achieved alongside the adoption of novel AI algorithms to evaluate the performance of ruminant livestock. These AI-powered tools have the robustness and competence to expand the evaluation of animal performance and help in minimising the production losses associated with heat stress in ruminant livestock. Advanced heat stress management through automated monitoring of heat stress in ruminant livestock based on behaviour, physiology and animal health responses have been widely accepted due to the evolution of technologies like machine learning (ML), neural networks and deep learning (DL). The AI-enabled tools involving automated data collection, pre-processing, data wrangling, development of appropriate algorithms, and deployment of models assist the livestock producers in decision-making based on real-time monitoring and act as early-stage warning systems to forecast disease dynamics based on prediction models. Due to the convincing performance, precision, and accuracy of AI models, the climate-smart livestock production imbibes AI technologies for scaled use in the successful reducing of heat stress in ruminant livestock, thereby ensuring sustainable livestock production and safeguarding the global economy.

This review explores the technological advancements, optimal application conditions, and benefits of targeted prostate biopsies facilitated by multiparametric magnetic resonance imaging (mpMRI). Methods: A systematic literature review was conducted to compare traditional 12-core systematic biopsies guided by transrectal ultrasound with targeted biopsy techniques using mpMRI. We searched electronic databases including PubMed, Scopus, and Web of Science from January 2015 to December 2024 using keywords such as “targeted prostate biopsy”, “fusion prostate biopsy”, “cognitive prostate biopsy”, “MRI-guided biopsy”, and “transrectal ultrasound prostate biopsy”. Studies comparing various biopsy methods were included, and data extraction focused on study characteristics, patient demographics, biopsy techniques, diagnostic outcomes, and complications. Conclusion: mpMRI-guided targeted biopsies enhance the detection of clinically significant prostate cancer while reducing unnecessary biopsies and the detection of insignificant cancers. These targeted approaches preserve or improve diagnostic accuracy and patient outcomes, minimizing the risks associated with overdiagnosis and overtreatment. By utilizing mpMRI, targeted biopsies allow for precise targeting of suspicious regions within the prostate, providing a cost-effective method that reduces the number of biopsies performed. This review highlights the importance of integrating advanced imaging techniques into prostate cancer diagnosis to improve patient outcomes and quality of life.

The assessment of the completeness of historical earthquake data (such as, for instance, parametric earthquake catalogues) has usually been approached in seismology - and mainly in Probabilistic Seismic Hazard Assessment(PSHA) - by means of statistical procedures. Such procedures look «inside» the data set under investigation and compare it to seismicity models, which often require more or less explicitly that seismicity is stationary. They usually end up determining times (Ti), from which on the data set is considered as complete above a given magnitude (Mi); the part of the data set before Ti is considered as incomplete and, for that reason, not suitable for statistical analysis. As a consequence, significant portions of historical data sets are not used for PSHA. Dealing with historical data sets - which are incomplete by nature, although this does not mean that they are of low value - it seems more appropriate to estimate «how much incomplete» the data sets can be and to use them together with such estimates. In other words, it seems more appropriate to assess the completeness looking «outside » the data sets; that is, investigating the way historical records have been produced, preserved and retrieved. This paper presents the results of investigation carried out in Italy, according to historical methods. First, the completeness of eighteen site seismic histories has been investigated; then, from those results, the completeness of areal portions of the catalogue has been assessed and compared with similar results obtained by statistical methods. Finally, the impact of these results on PSHA is described.

The Italian "Gruppo Nazionale per la Difesa dai Terremoti" has conducted a project in recent years for assessing seismic hazard in the national territory to be used as a basis for the revision of the current seismic zonation. In this project the data on the major earthquakes were reassessed and a new earthquake data file prepared. Definition of a seismotectonic model for the whole territory, based on a structural-kinematic analysis of Italy and the surrounding regions, led to the definition of 80 seismogenic zones, for which the geological and seismic characteristics were determined. Horizontal PGA and macroseismic intensity were used as seismicity parameters in the application of the Cornell probabilistic approach. The main aspects of the seismic hazard assessment are here described and the results obtained are presented and discussed. The maps prepared show the various aspects of seismic hazard which need to be considered for a global view of the problem. In particular, those with a 475-year return period, in agreement with the specifications of the new seismic Eurocode EC8, can be considered basic products for a revision of the present national seismic zonation.

Introduction, The aim of this systematic review was to evaluate the current evidence for the effectiveness of ginseng, Rutin and Moringa for treating erectile dysfunction. Methods, A broad search of the Scopus, PubMed, Cochrane and Web of Science databases was performed in complicance with the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) guidelines. The following criteria were required for articles to be included in the review: English language; observational studies (cohort studies, case control/comparative studies, single-arm studies); randomized controlled trials; non-randomized comparative studies; case series; number of participants: ≥5 for case series or ≥5 patients per group for comparative studies. The Cochrane risk of bias (RoB) assessment tool for RCTs was used to assess the risk of bias of included studies. Results, Seven full-text articles were included in this review. All studies were randomized controlled trials. No studies on Rutin and Moringa alone matched the inclusion criteria. The methodological quality of the RCTs was variable. In all studies, the group treated with ginseng reported an improvement of erectile function (EF) compared to the control groups. IIEF and IIEF-5 were used to evaluate erectile function in six studies and in four of them, the improvement of the scores in the group treated with ginseng over the control group reached the statistical significance. Conclusion, This review suggests a positive effect of ginseng on EF in men. The association of ginseng along with other nutritional components with potential beneficial effects on ED appears promising and deserves further investigation in large randomized controlled trials.

Abstract Background Recently, renal angioembolization (RAE) has gained an important role in the non-operative management (NOM) of moderate to high-grade blunt renal injuries (BRI), but its use remains heterogeneous. The aim of this review is to examine the current literature on indications and outcomes of angioembolization in BRI. Methods We conducted a search of MEDLINE, EMBASE, SCOPUS and Web of Science Databases up to February 2021 in accordance with PRISMA guidelines for studies on BRI treated with RAE. The methodological quality of eligible studies and their risk of bias was assessed using the Newcastle–Ottawa scale Results A total of 16 articles that investigated angioembolization of blunt renal injury were included in the study. Overall, 412 patients were included: 8 presented with grade II renal trauma (2%), 97 with grade III renal trauma (23%); 225 with grade IV (55%); and 82 with grade V (20%). RAE was successful in 92% of grade III–IV (294/322) and 76% of grade V (63/82). Regarding haemodynamic status, success rate was achieved in 90% (312/346) of stable patients, but only in 63% (42/66) of unstable patients. The most common indication for RAE was active contrast extravasation in hemodynamic stable patients with grade III or IV BRI. Conclusions This is the first review assessing outcomes and indication of angioembolization in blunt renal injuries. The results suggest that outcomes are excellent in hemodynamic stable, moderate to high-grade renal trauma.

Regular monitoring of the seismicity in north-eastern Italy began in 1977, one year after the strong earthquake that caused about 1000 victims in Friuli. Over the years, the monitoring network operated by OGS has increased, today reaching a consolidated level in terms of geometry and number of stations. Furthermore, the entire data acquisition and processing system, which has changed over the years, is currently also highly robust. After the huge number of studies on the 1976 seismic sequence and on the regional seismicity developed in the 1970s and 1980s, the present paper illustrates a comprehensive work on the subject since the beginning of the 21st century. With this work, we take up the thread and analyse the evolution of seismicity in the first two decades of the 21st century (2000-2021). As part of the study, we took a retrospective look at the qualitative development of the data provided by OGS over time, basically focusing on the earthquake catalogue obtained by the network recordings and developing a new relation for calculating the duration magnitude.
