The discrepancy of echocardiographic signs of right ventricular overload with the clinical picture and the level of plasma troponin concentration may underestimate the severity of the condition and lead to an inadequate choice of therapy, since all these parameters are currently determining the tactics of treatment of patients. Objective: to assess the prevalence and clinical characteristics of patients with pulmonary embolism with inconsistency of clinical, laboratory and instrumental picture.Material and methods. The course of the disease of 456 patients with pulmonary embolism was analyzed. 21 patients had a fatal outcome, the rest recovered. All patients underwent general clinical and biochemical blood tests, determination of the level of D-dimer and troponin, electrocardiography, echocardiography, ultrasound of the veins of the lower extremities, CT angiopulmonography, analysis of the results obtained.Results. It was found that among patients with high- and moderate-risk pulmonary embolism, women more often than men have an increase in the plasma concentration of troponin levels by 2.4 times, as well as a 3-fold discrepancy in the clinical and laboratory-instrumental picture with the presence of signs of right ventricular overload according to echocardiography data, which appear in dynamics. The phenomenon of "delayed echocardiography signs of overload of the right heart" is recorded in 0.8% of all patients with pulmonary embolism. At the same time, in patients with a high risk of early pulmonary embolism – associated death, its occurrence is 2.1%, and in patients with a moderately high risk – 4.4%. When comparing the prevalence of clinical and instrumental symptoms of pulmonary embolism in these patients with the data of patients with high and moderate risks of early death from pulmonary embolism, no statistically significant differences were found, including in terms of echocardiography in dynamics. The delay of еchocardiography signs of overload of the right parts of the heart" by 12–36 hours from the clinical picture, including in patients with hemodynamically significant pulmonary embolism, can be detected only with a series of еchocardiography, which allows, if necessary, timely review scope of treatment. In patients with pulmonary embolism with a pronounced lesion of the pulmonary artery, due to the rapidly progressing course of the disease, it is possible that the еchocardiography criteria of the disease do not have time to develop. In this case, an assessment of the stroke volume may play an important role, hover this parameter is non-specific for this disease.

Introduction. Serum chemokines are inflammatory mediators, which role is shown in the occurrence and progression of a number of malignant tumors. Produced by white blood cells, stem cells, tumor and endothelial cells, chemokines control their movement and positioning. Chronic inflammation underlies the progression of ovarian cancer (OC ). This increases the likelihood of chemokines stimulating or blocking tumor progression. The aim of the study was to evaluate the relationship between the blood levels of inflammatory cytokines in blood and the number of circulating tumor cells (CTC s) with the response to standard chemotherapy (CT ) in patients with cancer.Material and Methods. In patients with primary OC before and after 2–4 courses of chemotherapy and in patients with benign ovarian tumors (as a control), serum levels of CCL 2, CCL 3, CCL 4, CXCL 8 and CX3CL 1 were evaluated by multiplex xMAP analysis. The amount of CTC s (population CD 45-/ Epcam+/CK+) was determined using a flow cytometer. Patients with ovarian cancer were divided into 3 groups according to the platinum sensitivity criterion of GC JG 4th, and progression-free interval (PFI) was determined. Results. It was found that the levels of CCL 2, CCL 3, CCL 4, CXCL 8, and CX3CL 1 in case of OC did not significantly differ from that in the control, strongly negatively correlated with age (except for the CCL 2 level). CT significantly increased the level of CCL 2 in the group of refractory OC ; of CCL 3 – in the group of sensitive OC , of CCL 4 – in the groups of resistant and sensitive OC , and C XCL 8 level increased in the groups with resistant and sensitive OC and decreased in the group of refractory OC . The number of CTC s in patients with OC was significantly higher than in the control. After CT , a decrease in the amount of CTC s strongly and significantly correlated with a decrease in the level of CX3CL 1 in the groups of refractory andsensitive OC . The maximum PFI occurred with an increase in serum levels of CCL 3, CXCL 8, a decrease in CCL 4 and a constant level of CX3CL 1.Conclusion. Thus, no significant differences in the levels of CCL 2, CCL 3, CCL 4, and IL -8 between patients with OC and control groups were found. The levels of chemokines studied and the amount of CTC s differed in the groups divided by the tumor sensitivity to CT . We observed significant correlations between the amount of CTC s and the level of CX3CL 1 in the group of platinumsensitive OC .

Green spaces of the city are an integral part of its ecological framework. When planning urban space, information is needed on their actual distribution, condition and dynamics. To do this, it is advisable to use satellite images. The purpose of the work is to conduct remote monitoring of green spaces of Ufa using Landsat satellite images over the past 30 years. Image processing, including radiometric calibration, reducing the amount of data from 6 channels to 3 main components, uncontrolled classification of images, grouping classes to 5 types of underlying surface (open soil, no vegetation, grassy vegetation, tree and shrub vegetation, underwater), matrix construction changes were made using the tools of the SAGA GIS software product. As a result, a map was constructed showing spatial changes of the tree-shrub vegetation type over 30 years. The plots on which this type is preserved make up 16 073 ha, on which it was replaced by another type of underlying surface - 3700 ha. Plots characterized in 1988 by other types, and in 2018 converted to the tree-shrub vegetation type, occupy 8665 ha. On the one hand, there is a positive trend in the conservation and increase in the area of tree-shrub plantings. But this is not characteristic of the entire urban district, but mainly of the forests of the Ufa urban forestry, which are located mainly on the outskirts, around the central part of the city. For intracity tree-shrub plantings that are not related to the forest fund and located in the built-up part of the city, stable negative dynamics and a change in other types of underlying surface are characteristic.

The article discusses the results of a study aimed at determining the frequency of different genetic variants of U. parvum in women with inflammatory diseases of the urogenital system and clinically healthy subjects. Molecular typing of 40 samples of U. parvum taken from women with different forms of inflammatory diseases of the urogenital system was performed for the mba gene. The authors revealed a relationship between clinical manifestations of inflammatory diseases of the urogenital system caused by U. parvum and different U. parvum serovars. An infection with 6 U. parvum serovar results in the development of inflammatory processes in the form of cervicitis or vaginitis accompanied by subjective manifestations more frequently than an infection with other serovars under examination (1 and 3) (p < 0.05).

General Medicine - Abstract
Liver cancer remains a global challenge of healthcare, with the incidence growing worldwide. According to various authors, over 1 million patients will be diagnosed with liver cancer each year by 2025. The molecular pathogenesis of HCC varies with respect to genotoxic lesions and aetiologies. Although our understanding of the HCC pathophysiology and drivers tends to improve, it is still distant from translation into clinical practice. About 25 % of HCC cases are associated with variant mutations. HCC pathophysiology is a complex multi-step process. The interaction of various factors underlies the early stages of malignant hepatocyte transformation towards the development of HCC. Overall, about 20–25 % of HCC patients have at least one potential driver mutation. Obesity should also be noted as being associated with a higher risk of HCC and various other cancers. Despite many issues in the HCC pathogenesis being already known, the unresolved questions remain. Modern molecular genetic diagnostics and animal modelling of malignant tumours are expanding our horizons of knowledge in this field.

Introduction. Over the past two decades, a large amount of data has been accumulated that show the significant impact of social factors on the health of the population. The Republic of Bashkortostan is a large industrial centre and one of the most promising subjects of the Russian Federation. Purpose of the study. Ranking the territories of the Republic of Bashkortostan by priority socioeconomic indices, as well as determining their impact on the health of the child population. Material and methods. As the initial data, the materials of the socioeconomic state of the Republic of Bashkortostan, data on the number and morbidity of the child population for the period 2014-2018 were used. Correlation-regression analysis was carried out, and qualitative assessments of the results obtained were given. The principle of dividing the territory into seven socio-economic zones, taking into account climatic and geographical features, the development of industrial potential and the existing socio-economic ties, was chosen as the basis for the study. Results. The ranking of territories by socio-economic indices in the Republic of Bashkortostan showed that most of the municipalities (over 60%) have a low level of socio-economic development. The most favourable conditions in terms of social comfort for children were found in the southern, central and northwestern economic zones. So, as socio-economic indices improve by 2018, the incidence of the population tends to decrease. Conclusion. Thanks to the data obtained, a number of the most disadvantaged areas in socio-economic development and morbidity in the child population have been identified. In these territories, it is recommended to develop a set of measures to improve and stabilise socio-economic indices.

Introduction. The activities of enterprises engaged in the production, preparation, transportation and primary processing of oil are associated with the formation and accumulation of a large amount of waste (spent drilling fluids, drilling mud, oil sludge, spent catalysts, etc.), which leads to pollution of environmental objects and can contribute to the formation of adverse effects on public health. Materials and methods. The sanitary and hygienic state of atmospheric air, soil, the water of surface (rivers, lakes) and underground (wells, wells, springs) water sources, the water of centralized water supply systems in the territories of large oil fields the Republic of Bashkortostan is carried out. The origins of information were data from many years of in-house research, the regional information fund of the Sanitary Hygienic Monitoring, and departmental laboratories. Results. Studies have found that in areas of oil production, the content of chemicals in concentrations exceeding hygienic standards is detected: in the atmospheric air - dihydrosulfide, hydrocarbons, nitrogen dioxide, sulfur dioxide; in underground waters - chlorides, sulfates, nitrates, iron, strontium, increased mineralization and general hardness; in the soil - sulfates, chlorides, nitrates, petroleum products. Contamination of water from underground water sources and atmospheric air can contribute to the risk of adverse effects from individual organs and systems, as well as carcinogenic hazards. Based on the results of the study, ecological and hygienic recommendations were developed. Conclusion. As a result of the study, the level of public health risk associated with environmental factors in the oil-producing region was assessed, and a set of hygienic measures was justified.

Currently, polypropylene is one of the most common polymers in the world. Due to its resistance to high temperatures, strength and density, it has found very wide application in the field of medicine. Polypropylene produces medical devices, pharmaceutical packaging, containers, suture threads. But a special place is occupied by non-woven materials based on polypropylene: surgical underwear, surgical gowns. As you know, to sterilize these products using ionizing radiation, which is the most effective of all methods. However, it leads to the destruction of polypropylene, as a result of which its physical and mechanical properties deteriorate. It is also known that the dose of radiation can vary depending on the location of the product in the box. Thus, materials can be degraded to varying degrees depending on the location in the box from the electronic emitter. As the object of study was selected: PP 1562R polypropylene produced by OAO Nizhnekamskneftekhim. It has been established that the location of polypropylene samples upon irradiation has little effect on the degree of their destruction.

Introduction. Wastewater contaminated with oil, demulsifiers, hydrogen sulfide, ammonium sulfide, phenol, sulfates, aromatic hydrocarbons, alkali, fatty acids, various solutes, etc. is formed at oil refining and petrochemical enterprises. Runoff entering surface and underground water sources can harm water quality, sanitary conditions of life, and water use of the population. Taking this into account, further improvement of scientific approaches to ensure the safety of water bodies that are sources of drinking and recreational use, assessment, and prevention of public health risks caused by water factors is of particular importance. Material and methods. To assess the degree of influence of industrial enterprises on the quality of water sources, we studied the sanitary condition of the Belaya River and its tributaries, as well as underground water lying in the area of oil refining and petrochemical industries. The analysis of the water quality of surface and underground water sources according to the data of laboratory studies of departmental laboratories; the results of scientific and practical research and publications on the studied problem for 1999-2019. Results. Materials of observation data show that wastewater from changes of industrial enterprises the sanitary condition of surface reservoirs: there is a decrease in the content of ammonium nitrogen and biological oxygen consumption indices and a significant increase in the content of nitrates, chlorides, sulfates, and petroleum products. Surface water sources are characterized by unfavorable organoleptic characteristics, high organic pollution, and the presence of specific compounds (petroleum products, surfactants, alpha-methylstyrol, benzene, toluene, isopropylbenzene, and hydrogen sulfide) in concentrations higher than their hygiene regulations. Underground waters located on the territories of the main industrial complexes are characterized by high mineralization, extremely high hardness, high content of iron, nitrates, and petroleum products. Conclusion. Refineries and petrochemicals affect the pollution of surface and underground water sources. The water quality of the surveyed water bodies does not meet sanitary and hygienic standards. The priority indices of the influence of the enterprises of branch on water bodies include organoleptic indices of organic pollution indices of total salts composition of water, the content of specific ingredients: oil, benzene, toluene, isopropyl, alpha-methyl styrene, xylene, cresol, kerosene, gasoline, fuel oil, ethylene, propylene, 3,4-benzo(a)pyrene, phenols (volatile), methyl mercaptan, nonionic and anion-active surfactants, biological oxygen consumption indices and others. The research made it possible to assess the level of pollution of water bodies located in the zone of industrial enterprises and develop measures to reduce the man-made impact on the environment and public health.

The production of nonwovens over the past few years remains one of the most promising areas in the textile industry. This is due to the short cycle of production of such materials from raw materials to finished products and the ability to produce at a low cost a wide range of materials for both household and technical use. In particular, non-woven materials are widely used for the manufacture of disposable medical clothing and underwear. An important characteristic for surgical clothing is an indicator of bursting strength. This study simulates material deformation similar to elbow pressure when bending the arms or knee when bending the legs. Since the lower part of the sleeve of the product refers to the critical zone of the product, i.e. to the zone most likely to be involved in the transfer of infectious pathogens to or from the wound, the sterile product must have a significant margin of bursting strength. The effect of the absorbed dose of electronic radiation on the strength during the bursting strength of nonwoven medical material of SMS design with a surface density of 35 and 50 g/m2 obtained using the spanmelt technology was studied. It is shown that electronic radiation significantly reduces the resistance of the nonwoven to bursting strength. With an absorbed dose of 40 kGy, the material no longer meets the requirements for surgical clothing and linen by regulatory documents. For non-woven materials obtained using spanmelt technology, the bursting strength rate is even more sensitive to the action of ionizing radiation than the tensile tensile load, therefore, this indicator is recommended to be used as a characteristic indicator of radiation resistance.

Non-woven materials are widely used for the manufacture of disposable medical clothing and underwear. Radiation is widely used to sterilize single-use medical devices. The paper analyzes the effect of ionizing radiation at absorbed doses of 0-60 kGy on the stress-strain properties of medical non-woven spanmelt material based on polypropylene obtained by blow-molding technology. It has been established that ionizing radiation significantly reduces the breaking load and elongation in the machine and cross directions of the web. For this type of material, the most critical is the decrease in strength in the cross direction of the web, primarily because the level of strength in the cross direction of spanmelt materials is generally low. Sterilization by ionizing radiation further reduces strength and leads to the fact that non-woven materials irradiated with an absorbed dose of 50-60 kGy are close to unacceptable values in accordance with the requirements of EN 13975-2011.

The health of the population is closely linked to the social gradient: improving the quality of health is associated with improving the socio-economic situation of the country. The level and quality of life is one of the fundamental indicators of the country’s economic development. The Republic of Bashkortostan is a leading agrarian and industrial region and makes a significant contribution to the development of the country. There are no studies on a comprehensive assessment of socio-economic factors in the Republic of Bashkortostan. To carry out the study, the following data were used: total morbidity and number of children, materials of the Federal Information Fund for Social and Hygienic Monitoring data for a block of socio-economic indicators for 2014–2018 The relationship of socioeconomic indicators and the incidence of the child population was carried out using correlation and regression analysis. General analysis of the socio-economic situation of the Republic of Bashkortostan for the period 2014–2018. indicates a stable economic situation and positive dynamics indicators. The results of the influence of priority socio-economic factors on the health of the child population living in urban and rural settlements are obtained. For children living in cities of the Republic of Bashkortostan, the leading socio-economic indicators affecting the overall incidence were 7 indicators: educational expenses; percentage of apartments that do not have running water; the proportion of living space equipped with central heating; monthly average nominal accrued wages of workers in the economy; investment in fixed assets per capita; number of doctors of all specialties; the number of doctors in outpatient medical institutions. The incidence of children living in rural areas was revealed by the influence of 2 indicators: the average monthly nominal accrued wages of workers in the economy and investment in fixed assets per capita. The countryside of the region is lagging behind in most indicators compared to cities. The results of the study must be used to improve the regional development strategy, as well as minimize the impact of these indicators on the health of both children and the population as a whole.

Introduction. Operation of enterprises of petrochemical and oil refining industries is accompanied by significant pollution of air, soil, underground and surface water sources with a complex of toxic chemical compounds, which in turn has an adverse impact on the health and living conditions of the population. In order to carry out a correct assessment of the ecological and hygienic situation and reduce the risk to public health in regions with developed petrochemistry and oil refining, there is a need to optimize the existing mechanism of sanitary and epidemiological supervision of the quality of environmental facilities, to develop effective hygienic recommendations and targeted measures to reduce the level of anthropogenic impact and to prevent poor living conditions. Material and methods. While producing polymers as well as during their processing, a certain amount of stabilizers is introduced into the product, which should ensure polymer properties saving during processing and those of polymer products during storage and operation. However, in cases where medical products based on polypropylene are subjected to radiation sterilization, there are not enough stabilizers in it to save their characteristics during operation. In this regard, we made an assessment of the influence of processing conditions on the properties of polypropylene with a different set of stabilizers in the manufacture of products based on it, in order to assess the degree of influence of each technological operation, including the effects of ionizing radiation during sterilization. Processing and radiation exposure are shown to affect the properties of polypropylene. Nevertheless, the effect of ionizing radiation with an absorbed dose of 40-60 kGy exceeds the negative effect of thermo-oxidative breakdown greatly during the extrusion of PP. Polypropylene containing organophosphorus stabilizers (brand PP 1562R) is more susceptible to breakdown. This is indicated by low values of oxidation induction time, breakdown initial temperature, as well as high values of MFI after exposure to electron radiation. PP brand PP H350FF/1 whose stabilizing complex contains phenol-phosphite antioxidants is more resistant to breakdown during processing and sterilization. For both brands under study, it is apparently necessary to increase the content of stabilizing additives in order to save the properties at the level of the original unexposed material.

Different cytokines are believed to play an important role in the pathogenesis of juvenile idiopathic arthritis (JIA). Some of them may serve as the prognostic markers for the disease severity, the eye lesion, and the effectiveness of therapy in JIA patients. To evaluate the serum concentration of TNFα, IL1β, IL6 and IL10, their spontaneous and mitogen-induced production levels in the whole blood cell culture of JIA patients and to correlate these indicators with the disease features and with the effectiveness of methotrexate therapy. Twentyeight Russian patients with JIA, naive for disease-modifying antirheumatic drugs, were enrolled in the study. The serum levels of cytokines (TNFα, IL1β, IL6, and IL10); the levels of their spontaneous and mitogen-induced production in the whole blood cell culture were studied by solid-phase enzyme immunoassay. The serum levels and spontaneous production levels of TNFα, IL1β, IL10 were low in JIA patients. The levels of TNFα, IL1β, IL6, IL10 in mitogen-induced production were significantly higher than those in spontaneous production (pcor0,05). The association of some clinical features (disease duration, uveitis, elevated ESR, hyperimmunoglobulinemia M with the cytokines (TNFα, IL6, and IL10) levels was established in JIA. The most promising is the association between the uveitis in JIA patients and an increase of spontaneous IL6 production (pcor=0,004), as well as a decrease of mitogen-induced TNFα production (pcor=0,029) in the whole blood cell culture. Statistically significant associations with a response to methotrexate therapy and disease severity in JIA patients were not observed. As a result of studying levels of cytokines (TNFα, IL1β, IL6, and IL10) in JIA patients, the relationship of TNFα, IL6 and IL10 was established with the number of clinical manifestations of the disease. If confirmed in larger samples, the study of the spontaneous IL6 production in the whole blood cell culture might be used for the early diagnosis of the uveitis in JIA patients.

The authors reviewed the functions of strategic planning and identified problems in the development of urban passenger transport services. They identified the impact of land use on public transport development. Scientific studies of regions and cities have determined a number of conditions for their economic growth in the post-industrial economy, which include the importance of agglomerations and clusters, the differentiation and specialization of economic activities, the development of services and infrastructure. Increasing employment, the development of regional and municipal innovation systems, the ability of cities and regions to be attractive for living and creative fruitful work are increasingly important for the socio-economic development of cities. One important consequence of the research is that economic transformation towards regional and urban growth and development is becoming increasingly dependent on dense physical space. All these conditions can be realized with the availability of efficient and high-quality transportation services, vehicles and infrastructure, increased mobility of the population and accessibility of the services provided. This, in turn, means that public transport is the driving force for the economic development of regions and municipalities in terms of mobility and accessibility in general, as well as in comparison with other modes of transport.

History ,Computer Science Applications ,Education - Abstract
The effect of low-pressure high-frequency (HF) plasma treatment on the structure of the materials formed by bundles of natural protein (collagen) fibers is considered. The outer surface of the material is being affected by a constant flow of ions and a pulse-periodic flow of electrons. During this procedure the electrons flow at opposite sides of the sample in antiphase. This leads to that an alternating electric field is created, which intensity is estimated approximately at 105 V/m, inside the material. As a result, in the capillaries and pores, a breakdown occurs and charged particles are forming during this process. The recombination of these particles releases an energy of 15.76 eV on the surface of pores and capillaries, which is transferring to the surface molecules of the protein, which leads to the bulk modification of the material based on protein fibers. Such volumetric processing provides an effective modification of structural elements, allowing you to significantly improve the quality of liquid treatments in the production of collagen-containing materials and their properties in general.

The globalization taking place in modern conditions permeates all the structural elements of the world economy, leading them to competitive confrontation. Globalization overcomes economic barriers, promotes the growth of trade associations, and previously closed sectors of the economy makes it accessible for international competition. Nowadays, it is difficult to overestimate the importance of the services market in the economy not only of a single state, but also in conditions of international socio-economic relations. In today's world, services are becoming increasingly popular. Many people are willing to pay a lot to have windows installed, the stove repaired, and even screwed in a light bulb. There are several views on the development of services. One of them fully supports the fact that services have replaced work in many areas. Thus, unemployment can be reduced. Since there is someone to perform each service who will perform it for a certain amount, thereby ensuring wages for himself. Services have their pros and cons, while their number is growing every year. According to the statistics of the Russian Federation, in recent years, the share of intangible production in the structure of GDP has been steadily increasing, the employment rate in the provision of services is also actively growing every year, which indicates an increase in population demand for the consumption of various services. The article presents a study of the impact of globalization on the service sector. The factors determining the development of the service economy are determined.

- Abstract
As a result of the evaluation of the indicators of a large fishing enterprise of the Volga-Caspian basin (carrying out extraction and processing of biological resources), proposals were presented to reduce the value of the property and sources of financing of the organization, and this decrease was generally balanced with changes in revenue and profit of the enterprise. Considering the results of the analysis, as well as external and internal factors of the organization’s functioning, it is recommended to create a planning system to ensure financial stability in subsequent periods. Implementation is possible with the organization of the budgeting process. A stock management strategy was proposed to optimize their volume and structure. ABC analysis was used for this purpose, which made it possible to strengthen control over the main types of stocks and improve management efficiency. The proposed measures will facilitate the release of funds and contribute to the stabilization of the financial condition of the enterprise.

The analysis in the centralized laboratory is the main model of laboratory diagnosis and monitoring in the conditions of intensive care. However, alternative models of “point-of-care testing” technology are being actively introduced nowadays. This review article presents the analysis of “Point-of-care testing” technologies from the perspective of the intensive care doctor. The main advantages of this monitoring idea include the speed of analysis and the cost of components. Among the disadvantages we can identify ambiguous assessment of the accuracy and objectivity of analyses, additional burden on the staff of resuscitation and intensive care units. Possible ways of resolving disputable issues aimed at improving quality and reproducibility of the analyses are outlined in this work. Despite the fact that currently “Point-of-care testing” technologies cannot replace centralized laboratories, they are steadily occupying a certain niche and are going to play a more important role in diagnosing and monitoring patients in critical condition.

Free radicals have taken a firm place among the explanatory principles of the onset and development of tumors. Unambiguous ideas about the role of these highly active agents in carcinogenesis have not yet been obtained. The experimental results are contradictory, often the opposite. The field of study of the value of the activity of antioxidant systems based on the results obtained is no less controversial. In this regard, the urgent problem is the need to prescribe antioxidants in oncology. Antitumor effects of antioxidants are being questioned in many works, which brings to the forefront the appropriateness and adequacy of their appointment. The ambiguous role of free radicals in carcinogenesis determines the development of therapeutic strategies based on the precise modulation of oxidative processes.
