Crohn’s disease (CD) is a chronic recurrent inflammatory bowel disease with a wide variety of clinical manifestations – from classical gastrointestinal phenotypes to predominance of extraintestinal symptoms. Atypical course of CD is observed relatively often and is manifested with a damage to almost all body systems, including hepatobiliary, immune, hematopoietic, cardiovascular, nervous, as well as eyes, skin, lungs and kidneys. Atypical manifestation often precedes intestinal, making diagnosis difficult. The presence of various clinical manifestations of CD, often an atypical course, as well as the absence of standardized diagnostic criteria can be an obstacle for physicians in the timely detection of the disease and effective treatment. Aim: to describe a clinical case of a diagnostically difficult patient of our clinic with an atypical course of CD and complications incompatible with life; highlight the problem of monitoring and possible solutions. Results. In the presented clinical case the patient with an atypical course of CD, with the prevailing displays of defeat of nervous and vascular systems is described. During hospitalization, a full range of diagnostic tests was performed, consultations with related specialists were obtained, but no direct data of inflammatory bowel disease were found. At the same time, the impossibility of performing a colonoscopy with a biopsy, due to the severity of the patient’s condition, delayed the timely diagnosis of CD and contributed to the development of complications incompatible with life. The diagnosis was made posthumously. The patient died as a result of intoxication due to the development of complications of CD – perforation of the intestine, intestinal abscess, development of peritonitis, septicemia. Conclusions. In the described clinical case CD is considered as a complex interdisciplinary problem, and emphasizes the importance of doctor’s awareness of the variants of atypical manifestations and course of inflammatory bowel disease, methods for their assessment and ways of effective management. This case also presents the latest epidemiological data on the prevalence of atypical manifestations of CD, identifies key areas of diagnostic search in case of a difficult patient.

Spelt wheat has great prospects in the technology of manufacture of products of the highest biological value. In the technology of rolled cereal, water-heat treatment is energy-consuming. Therefore, the study of technological parameters of ultrahigh-frequency electromagnetic field irradiation in the technology of spelt wheat cereal is relevant. The purpose of the study is to examine the yield and quality of rolled spelt wheat depending on the duration of exposure to an ultrahigh-frequency electromagnetic field. The study used laboratory (determination of croup yield) and statistical methods. It was identified that the total yield of cereal with the use of the grain of the Holikovs`ka variety substantially increased to 97.3% with the use of irradiation for 60-80 seconds. The yield of premium rolled cereal is the highest with irradiation for 80-100 seconds. Water-heat treatment of cereal from the Holikovs`ka variety grain did not substantially increase the yield of cereal. The highest total yield of rolled cereal was obtained in the LP 1152 line with a duration of irradiation of 100-120 seconds – 93.2-94.9%. The highest yield of rolled cereal of the highest grade was obtained with an irradiation duration of 120-140 seconds. Notably, water-heat treatment substantially affected the yield of cereal (96.0-96.1%). In the technology of production of rolled cereal from spelt wheat of the Holikovs`ka variety using an ultra-high frequency electromagnetic field, the duration of irradiation is 80-100 seconds without moistening. When using cereal No. 1 from spelt wheat of the LP 1152 line, it is necessary to moisten it by 0.5% with an irradiation duration of 120-140 seconds. The duration of cooking of premium rolled cereal is 10.7-11.3 minutes. The materials of the study are of practical value for low-productivity cereal enterprises in the production of spelt wheat cereal products. У статті на основі аналізу наукових поглядів вчених та норм чинного законодавства України розглянуто сучасний стан правового регулювання вирішення трудових спорів у судовому порядку, оскільки виникає багато питань, пов’язаних із визначенням юрисдикції суду під час розгляду таких спорів. Оскільки сучасний стан правового регулювання порядку розгляду та вирішення трудових спорів у судовому порядку не повною мірою відповідає вимогам реального захисту учасників трудових правовідносин, і насамперед працівника. Відтак необхідні ефективні правові механізми, щоб трудові спори вирішувалися повноважним юрисдикційним органом у суворій відповідності до закону, виходячи зі встановлених фактичних обставин справи. Звернено увагу на важливість правових позицій Верховного Суду у вирішенні трудових спорів, оскільки чинний Кодекс законів про працю України, як і загалом стан всього трудового законодавства в Україні, є застарілим, таким, що не в повній мірі відповідає умовам сучасності та динамічності розвитку трудових відносин. Завдяки правовим позиціям Верховного Суду заповнюються прогалини трудового законодавства, формується єдина національна судова практика, оскільки важливо захистити обидві сторони трудових відносин – роботодавця та працівника, дотриматися рівного балансу захисту їх інтересів в умовах ринкової економіки. Зауважено, що питань стосовно вирішення трудових спорів, пов’язаних із визначенням юрисдикції суду, можна було б уникнути, якщо б діюче трудове законодавство України не було позбавлене спеціальної норми, якою б урегульовувалися питання щодо судової юрисдикції. Тому в новому Трудовому кодексі України необхідно з урахуванням практики Європейського Суду з прав людини та спеціалізованих (трудових) судів Європейського Союзу врегулювати питання щодо судової юрисдикції, передбачивши правову норму, в якій були б закріплені чіткі критерії визначення та розмежування судової юрисдикції під час вирішення спорів щодо захисту трудових прав.

The peculiarities of obtaining planting material of rare representatives Drosera spatulate L. and Drosera aliciae L. using microclonal propagation in order to preserve and cultivate them in ex vitro conditions were studied. The method of sterilization of D. spatulate and D. aliciae explants with 80-90% obtaining aseptic material has been developed. The influence of different sterilization options on the development of microshoots has been studied. The best mode of sterilization is 0,1% solution of AgNO3 and 12.5% solution of H2O2. The features of organogenesis and regeneration of the whole organism from cultivated tissues and organs of Drosera L. was investigated. The effect of exogenous growth regulators at different stages of plant morphogenesis in vitro is shown. Improved conditions of rhizogenesis in vitro. It was found experimentally that MS nutrient media with the addition of 2 g∙l-1 PVP is optimal at the stage of introduction into culture in vitro D. spatulate and D. aliciae. The regeneration of microshoots of D. spatulate and D. aliciae depending on the type of explant and the composition of nutrient media was studied. Morphogenesis was most effective on nutrient media with the addition of 0.25 mg∙l-1 kinetin and on the hormone free MS media. Such cultivation conditions provided 100% regeneration of plants with a reproduction rate of 1:8. Studying the effect of cytokinins on the microclonal reproduction of D. spatulate and D. aliciae, it was found that the development and induction of multiple shoot formation in vitro is best performed on hormone free MS media. To induce the formation of the root system, it is necessary to add into MS nutrient media 0.5 mg∙l-1 IBA. According to the results of the research, a method of microclonal propagation was developed by cutting stem culture, which made it possible to obtain genetically stable, disease-free regenerating plants of D. spatulate and D. aliciae with an optimally formed root system and vegetative mass. The obtained homogeneous planting material can be used in floriculture, creation of terrariums, for pharmacological purposes and for the purpose of introduction

Context. The solution of the classification problem (including graphical data) based on the use of modular neural networks and modified machine learning methods with reinforcement for the synthesis of neuromodels that are characterized by a high level of accuracy is considered. The object of research is the process of synthesizing modular neural networks based on machine learning methods with reinforcement. Objective is to develop a method for synthesizing modular neural networks based on machine learning methods with reinforcement, for constructing high-precision neuromodels for solving classification problems. Method. A method for synthesizing modular neural networks based on a reinforcement machine learning approach is proposed. At the beginning, after initializing a system of modular neural networks built on the bottom-up principle, input data is provided – a training set of data from the sample and a hyperparameter to select the size of each module. The result of this method is a trained system of modular neural networks. The process starts with a single supergroup that contains all the categories of the data set. Then the network size is selected. The output matrix is softmax, similar to the trained network. After that, the average probability of softmax is used as a similarity indicator for group categories. If new child supergroups are formed, the module learns to classify between new supergroups. The training cycle of modular neural network modules is repeated until the training modules of all supergroups are completed. This method allows you to improve the accuracy of the resulting model. Results. The developed method is implemented and investigated on the example of neuromodel synthesis based on a modular neural network for image classification, which can later be used as a model for technical diagnostics. Using the developed method significantly reduces the resource intensity of setting up hyperparameters. Conclusions. The conducted experiments confirmed the operability of the proposed method of neuromodel synthesis for image classification and allow us to recommend it for use in practice in the synthesis of modular neural networks as a basis for classification models for further automation of tasks of technical diagnostics and image recognition using big data. Prospects for further research may lie in using the parallel capacities of GPU-based computing systems to organize directly modular neural networks based on them. [ABSTRACT FROM AUTHOR]

The incidence and prevalence of chronic inflammatory diseases of the gastrointestinal tract has been steadily increasing recently in all regions of the world. Celiac disease, Crohn's disease and non-specific ulcerative colitis, among others, are diagnosed during the most productive period of life, which significantly limits its quality, as well as violate the patient's psycho-emotional state and social status. Among the intestinal manifestations of chronic diseases of the small and large intestine, the osteoporosis is most relevant, and associated with an increased risk of fractures. Aim. To describe the clinical case of secondary osteoporosis in patient with a long course of nonspecific ulcerative colitis, to cover the problem of monitoring, treatment and possible solutions. Results. The clinical case of development of severe secondary osteoporosis in a patient with non-specific ulcerative colitis is presented. Determinants of bone loss in this category of patients are chronic inflammation of the mucous membrane, impaired enteral absorption and glucocorticoid therapy. Current risk monitoring tools for osteoporosis in risk groups are the determination of individual risk of fractures FRAX®, regular clinical laboratory monitoring and densitometry. Current treatment of secondary osteoporosis should be personalized with proper control over the course of the underlying disease. Conclusion. The described clinical case demonstrates a need for further population-based prospective studies to create diagnostic and therapeutic algorithms. These algorithms could help provide personalized therapy to patients with gastrointestinal diseases in group of osteoporosis and fractures risk at a young age.

The purpose of the work is to investigate the effectiveness of the use of physical condition control of students of special medical groups for the development of physical therapy programs. Materials and methods. 40 students of 1st and 2nd year students of National University “Lviv Polytechnic” and Zaporizhzhya State Medical University participated in the experimental studies, which on the basis of the medical examinations were assigned to a special medical group for the presence of diseases of the musculoskeletal system (scoliosis I, II and III degrees). The following methods were used in the research process: analysis and generalization of scientific literature and Internet data; analysis of medical records of students; instrumental methods of morphometric and psychophysiological studies; pedagogical observations; pedagogical experiment of ascertaining orientation; methods of mathematical statistics. The results of the study show that the application of theoretical and methodological provisions and recommendations of corrective measures in the system of test control of physical conditions in students of a special medical group in the practical work, on the basis of integration of specially selected means of physical therapy, enables to activate compensatory mechanisms, mobilize reserve mechanisms ability of the body to increase the competence to perform motor actions, helps to accelerate the formation of knowledge, skills, and elimination of defects motive sphere, eliminating the difference in indicators of psychophysical development between students of the main, preparatory and special medical group. Conclusions. The control of morphofunctional and psychophysiological indicators is an effective mechanism of correction of the physical condition of students of a special medical group with functional disorders of the musculoskeletal system and a necessary prerequisite for the development of complex programs of rehabilitation based on the use of therapeutic exercises.

The purpose of the work is to evaluate the degree of dynamic recovery of the movements of the operated limb during the second period of rehabilitation by means of physical therapy in patients who underwent hip replacement and strengthening of the capsule-ligament apparatus with a polypropylene mesh. Materials and methods. A retrospective study of case histories, radiographs, and laboratory data of 47 patients requiring restoration or strengthening of the hip capsule was performed, of which primary endoprosthesis was performed in 26 (55.3 %) patients, and revision in 21 (44.7 %). Among the studied patients, 27 patients (57.4 %) were male, 20 patients (42.6 %) – female. The average age of the patients was 57 years (from 20 to 86 years). Results. Based on the analysis of the results of the study, we note that the quality of life of patients in both groups, the overall subjective condition of the joint, the volume of movements of the operated limb, as well as the evaluation of clinical and functional outcomes of treatment on the Harris Hip Score (HHS) directly correlate with performance preoperative exercises of a therapeutic nature and adherence to the postoperative rehabilitation program of physical therapy. Such correlation is more significant at 12 months after surgery, at 24 months of observation the indexes of the operated hip joint in the two groups of patients were not significantly different: 65.3 % of the observation group and 52.2 % of the comparison group at 12 months after surgery and 91.3 % and 81.0 % after 24 months, respectively. Conclusions. The use of polypropylene mesh to strengthen the posterior capsule ligaments of the hip joint enhances the functionality of the operated limb and restores patients overall, accelerates the physical rehabilitation process after discharge from the hospital. The expediency of careful preoperative preparation of patients and active management of the postoperative period (complexes of therapeutic exercises, informing of patients) is proved.

The purpose of the work is to investigate and to test the effectiveness of therapeutic exercises in the process of physical rehabilitation of players in futsal after traumatic injuries of the musculoskeletal system. Materials and research methods. 36 athletes (18–27 years old), who during 2016–2019 were members of the ZSMU futsal team, took part in the experimental studies. Quantitative indicators of traumatic injuries of the musculoskeletal system are determined in the training and competition processes in annual training cycles based on the processing of official competition protocols and athletes' medical cards. Analysis and generalized data from scientific literature and the Internet; pedagogical observations; analysis of official competition protocols; analysis of medical records of athletes; pedagogical experiment stating orientation; methods of mathematical statistics were used in this experiment. Results. Analysis and synthesis of the obtained data allows us to state that the main feature using to therapeutic exercises in the generalized physical rehabilitation program is that they have restorative, training orientation and comprehensively affect the restoration of stability and mobility in injured joints, muscle tone and coordination of movements, proprioceptive function, special motor skills, skills, physical qualities, general and special physical performance. Conclusions. It was determined that the main therapeutic exercises which are used in the physical rehabilitation of athletes in futsal after injuries of the musculoskeletal system are massage exercises, stretching exercises, low-intensity cyclic locomotion, tension and relaxation exercises for healthy limbs, therapeutic exercises, conditioning exercises for large muscle groups, breathing exercises, local exercises on simulators and technical rehabilitation equipment.

Vorticity in closed quantum fluid circuits is known to arise in the form of persistent currents. In this work, we develop a method to engineer transport of the quantized vorticity between density-coupled ring-shaped atomic Bose-Einstein condensates in experimentally accessible regimes. Introducing a tunable weak link between the rings, we observe and characterize the controllable periodic transfer of the current and investigate the role of temperature on suppressing these oscillations via a range of complementary state-of-the-art numerical methods. Our setup paves the way for precision measurements of local acceleration and rotation.

The article defines the place and components of IT solutions in the knowledge management system. The author substantiates that the use of modern IT solutions in the knowledge management system is an essential component of creation, search, transfer, exchange, development and effective use of knowledge. A large number of different technologies are used to integrate into a single complex of knowledge management system, including traditional automation systems and information retrieval systems, e-mail, corporate networks and Internet services, databases and data warehouses, electronic document management systems, specialized data processing programs, expert systems and databases knowledge In this case, a computer remains as a main tool of information management knowledge management in Ukraine. The scholar analysed the dynamics of the major indicators of computer devices’ use of by enterprises. The paper determined that not all enterprises in Ukraine use computers in their business activities. The proportion of enterprises, which used computers in 2017, amounted for 95.4%, which is by 0.2% more in comparison with 2015. The best situation in this aspect is in such types of economic activities as professional, scientific and technical activities; information and telecommunications; water supply; sewage, waste management. However, on average only every third employee in Ukraine used a computer to perform his/her professional duties. The author examined the main Internet-based jobs, where traditional activities are the most popular ones, namely: sending or receiving various e-mails, receiving information about goods and services. At the same time, videoconferencing is less popular, which is a negative factor in creating effective communication in the transmission, dissemination and exchange of knowledge. The paper explored the degree of using the possibilities of a corporate website for personnel training. The author determined that only every tenth enterprise in Ukraine uses its own website for the professional development of the employees. The scholar substantiated the features of social media, which promote the efficiency of their use as a tool for knowledge management. Exchanging views, opinions or knowledge within the enterprise and engaging customers in the development or innovation of goods and services remain the least popular objectives of social media work.

Context. Problem of traffic prediction in a city is closely connected to the tasks of transportations in a city as well as air pollution detection in a city. Modern prediction models have redundant complexity when used for separate stations, require large number of measuring stations, long measurement period when predictions are made hourly. Therefore, there is a lack of method to overcome these constraints. The object of the study is a city traffic. Objective. The objective of the study is to develop a method for traffic prediction, providing models for traffic quantification at measuring stations in the future under data and resource constraints. Method. The method for agent-oriented traffic prediction under data and resource constraints was proposed in the paper. This method uses biLSTM models with input features, including traffic data obtained from agent, representing target station, and other agents, representing informative city stations. These agents are selected by ensembles of decision trees using Random Forest method. Input time period length is proposed to set using autocorrelation data. Results. Experimental investigation was conducted on traffic data taken in Madrid from 59 measuring stations. Models created by the proposed method had higher prediction accuracy with lower values of MSE, MAE, RMSE and higher informativeness compared to base LSTM models. Conclusions. Obtained models as study results have optimal number of input features compared to the known models, do not require complete system of city stations for all roads. It enables to apply these models under city traffic data and resource constraints. The proposed solutions provide high informativeness of obtained models with practically applicable accuracy level. The article proves the necessity of internal (formative) evaluation, the purpose of which is to intensify and optimize the process of the formation of digital citizenship of future specialists. Implementation of competence-based approach and informatization of education allows increasing efficiency of existing evaluation models, requires deep changes in all components of the education system. The attention is focused on a new generation of educational standards through improvement of innovative technologies of evaluation, providing open educational process (transparency of monitoring procedures and equal opportunities of students to achieve educational goals), help the student as a personality of learning activities to solve the problem of self-determination, self-control, self-realization.
