Aim of the work was to analyze the content of factors with neuroprotective, neurotoxic and apoptotic eff ects in the blood of patients with prolonged disorders of consciousness (pDOC), depending on the level of consciousness disorder and neuroprotective therapy.Material and methods. Research included 53 patients with pDOC, who were divided into 2 groups. Group 1 included 19 vegetative state/unresponsive wakefulness syndrome (VS/UWS) patients, group 2–34 minimally conscious state (MCS “minus” and “plus”) patients. Firstly at admission and then at the end of treatment course (after 1 month on average), plasma concentrations of brain neurotrophic factor (BDNF), apoptosis antigen (APO-1), apoptosisinducing ligand (Fas-L) and glutamate were studied. Control group consisted of 16 patients without measured once.Results. Decrease of BDNF serum level was revealed in patients with pDOC,which was less pronounced those who had TBI as a etiological factor of pDOC. BDNF level signifi cantly increased after a month against the background of neuroprotective therapy. Glutamate level was higher in the fi rst group (VS/UWS). No signifi cant diff erence in the content of apoptosis factors was observed.Conclusion. In patients with pDOC, decrease serum BDNF level was observed, less pronounced in TBI as etiology of pDOC. In patients who matched MCS criteria at the admission, there was a signifi cant increase of BDNF serum level during treatment course, which could indicate that patients with a higher initial level of consciousness have better potential for realizing the eff ect of neuroprotective therapy. Levels of apoptosis factors did not correspond to the consciousness level.

BACKGROUND: The period of coma in some patients after severe brain damage ends with a transition to one of the forms of chronic disorders of consciousness ― a vegetative state/unresponsive wakefulness syndrome or a minimally conscious state. Almost all patients with chronic disorders of consciousness have dysphagia of varying severity, and therefore nutrition of this category of patients is carried out initially through a nasogastric tube, and then through a gastrostomy. Early tracheostomy cannula removal may lead to the development of aspiration and pneumonia. Dysphagia is often not diagnosed in chronic disorders of consciousness patients. AIMS: Analysis of the results of fibrooptic endoscopic assessment of swallowing in chronic disorders of consciousness patients to identify the relationship between the presence and severity of dysphagia with the level of consciousness, data on the coma recovery scale, as well as the duration of consciousness disorders and dynamics of recovery of consciousness. MATERIALS AND METHODS: The study was of a prospective type, conducted in the period from 2019 to 2021. 39 chronic disorders of consciousness patients (18 ― vegetative state/unresponsive wakefulness syndrome, 18 ― minimally conscious state "minus" and minimally conscious state "plus" and 3 patients with a level of consciousness corresponding to the emergence from the minimally conscious state). The average duration of chronic disorders of consciousness was 7.79.4 months. All patients underwent a neurological examination using the CRS-R upon admission to the hospital and a month later, an endoscopic examination of the swallowing function was performed with scores calculated according to the Federal Endoscopic Dysphagia Severity Assessment Scale (FEDSS) and aspiration assessment scale in accordance with the Rosenbek criteria. RESULTS: Dysphagia of varying severity was detected in 36 patients (92.3%). The correlation of the total CRS-R score with the degree of dysphagia (Ro=-0.481, p=0.002) was found. The degree of dysphagia did not depend on the chronic disorders of consciousness duration. CONCLUSION: Regardless of the chronic disorders of consciousness severity (vegetative state/unresponsive wakefulness syndrome, minimally conscious state "minus", minimally conscious state "plus") before tracheostomy removing and switching to the oral feeding, it is necessary to perform fibrooptic endoscopic studies of swallowing to detect dysphagia, determine its degree, which is a method of preventing complications of the decanulation consequences.

The neutrino dynamics in hot and dense magnetized matter, which corresponds with protoneutron star envelopes in the core collapse supernova explosions, is considered. The kinetic equation for a neutrino phase space distribution function is obtained, taking into account inelastic scattering by nuclear particles. The transfer component in a momentum space using transport properties is studied. The energy transfer coefficient is shown to change from positive to negative values when the neutrino energy exceeds four times the matter temperature. In the vicinity of a neutrino sphere, such effects are illustrated to lead to the energy strengthening in the neutrino spectra. As this paper demonstrates, such a property is favorable for the possibility of observing supernova neutrino fluxes using Large Volume Neutrino Telescopes.

The article presents the results of research and implementation of adaptive calibration systems used in setting up mass-produced products. The proposed method allows you to speed up the tuning of the HF transceiver. In the process of tuning, with the help of a neural network, an adaptive correction of the approximating function of the incident wave sensor is made according to the accumulated data, which makes it possible to reduce the time for tuning the transceiver. The introduction of the mathematical apparatus of neural networks can be applied in the process of mass production for other products.

Medical vocabulary is a part of the Russian language used by the professional community. Most of the terms are borrowed from other languages and poorly understood by most native speakers. The development of medicine, in particular, anesthesiology and resuscitation, leads to the emergence of many new conditions, symptoms, syndromes and their defnitions. The latter often cause controversy among doctors. The article is devoted to some aspects of the appearance of controversial terms in anesthesiology and resuscitation to discuss them.

BACKGROUND: Consciousness is the state of being awake and aware of oneself and the environment. The disorders of consciousness result from pathologies that impair awareness. The development of effective comprehensive personalized interventions contributing to the recovery of consciousness in patients with chronic disorders of consciousness is one of the most pressing and challenging tasks in modern rehabilitation. AIM: The aim of this study was to understand structural problems of the pituitary gland, blood levels of gonadotropins and melatonin as well as brain damage markers in the blood and cerebrospinal fluid in patients with chronic disorders of consciousness and to analyze the levels of the above markers among different groups of patients depending on the level of impaired consciousness. MATERIALS AND METHODS: We examined 61 chronic disorders of consciousness patients and identified three groups depending on the level of consciousness including 24 patients with unresponsive wakefulness syndrome, 24 patients with a minus minimally conscious state, and 13 patients with minimally conscious state plus. We performed magnetic resonance imaging of chiasmatic-sellar region and determined blood serum levels of follicle-stimulating and luteinizing hormones and melatonin, as well as urinary level of 6-sulfatoxymelatonin and the content of brain derived neurotrophic factor (BDNF), apoptosis antigen (APO-1), FasL, glutamate, and S100 protein in the blood serum and cerebrospinal fluid. RESULTS: The patients were examined in the age ranging from 15 to 61 years old. Patient groups were homogeneous by the level of consciousness in terms of age and duration of chronic disorders of consciousness by the time of examination. The patients did not differ in the pituitary volume regardless of the level of consciousness. No significant differences were found between the groups with different levels of consciousness when studying the levels of melatonin in the blood serum and its metabolite in the urine. A peak in melatonin secretion was detected at 3 a.m. in 54.5 % of the patients, which can be considered as a favorable prognostic marker for further recovery of consciousness. Hypogonadotropic ovarian failure was found in 34 % of the patients, with normogonadotropic ovarian failure in the remaining patients. Serum APO-1 and BDNF levels were significantly higher in patients with minimally conscious state relative to those with unresponsive wakefulness syndrome. Significantly lower levels of glutamate in the cerebrospinal fluid were detected in women with unresponsive wakefulness syndrome compared to patients with minimally conscious state. CONCLUSIONS: Further in-depth examination and accumulation of data on patients with chronic disorders of consciousness may provide an opportunity to identify highly informative markers for predicting outcomes and to develop new effective approaches to rehabilitation of consciousness in this category of patients.

An overview of the clinical observation of a patient with the consequences of severe traumatic brain injury, with a tracheoesophageal fistula is presented. The article discusses a staged multidisciplinary approach to treatment.

Chronic disorders of consciousness (DOC) leads to the development of skeletal muscles dysfunction (weakness, pareses)—critical illness polyneuropathy and myopathy. It is of interest to study the mechanisms underlying the development of this pathology and to assess the rehabilitation potential of DOC patients. Using positron emission tomography and computed tomography (PET/CT) with 18F-fluorodeoxyglucose (18F-FDG), as well as pathomorphological and immunohistochemical methods, we obtained data on structural and metabolic changes in the skeletal muscles of the upper extremities in 22 DOC patients. The study showed that structural changes in skeletal muscles have a nonspecific degenerative-atrophic nature and manifest severely on the side of paresis. In DOC patients, metabolic disorders of the shoulder girdle muscles, as revealed by a decrease in the level of 18F-FDG metabolism, developed symmetrically, regardless of muscle tone and deep reflexes levels. Along with degenerative changes, including damage to the contractile elements of the sarcomeres, the loss of desmin and dystrophin, and a decrease in the level of 18F-FDH metabolism, skeletal muscles of DOC patients revealed the signs of adaptive structural and functional rearrangements, such as a slow-to-fast transformation of the muscle fiber phenotype and activation of the autophagic pathway.

We study the properties and mass distributions of ultramagnetized nuclei produced as a result of heavy ion collisions, inside magnetar crust, explosions of type II supernovas, and the merging of neutron stars. For magnetic field strengths ranging from 0.1 to 10 TT, the Zeeman effect results in a linear nuclear magnetic response that can be described using magnetic susceptibility. Binding energies rise correspondingly for nuclei with open shells and fall for nuclei with closed shells. A noticeable increase in the generation of corresponding explosion nucleosynthetic products with antimagic numbers is predicted for nuclei belonging to the iron group and the r-process. The magnetic enhancement of 44Ti isotopes is consistent with results from observations and testifying to the substantial increase in the share of the main titanium isotope (48Ti) in the Galaxy’s chemical composition. An increased number of nuclides with low mass numbers is predicted in the peak of the r-process as a result of magnetic effects.

Parkinson disease (PD) is one of the most common neurodegenerative diseases. Its main clinical manifestation is movement disorders. The study of innovative objective methods for the diagnosis and rehabilitation of movement disorders in PD is relevant and in demand taking into account the slowly progressive course of the disease and the complex set of symptoms that subsequently forms a characteristic movement pattern. This article provides an example of a personalized rehabilitation assessment of biomechanical manifestations of the gait function of a patient with exacted diagnosis, stage 3.5 PD according to Hoehn and Yahr, by means of the method of three-dimensional motion video analysis using the Vicon Motion Capture Systems soft-hardware complex. The patient has postural and gait disorders. This method was applied after a rehabilitation course based on the activation of lifting the foot from the support surface (“back push”). Changes in the tempo and rhythm parameters of gait in a patient with PD in comparison to a healthy person were revealed: acceleration of walking pace with shortening of the length of single and double step, a decrease in the time of limb advancement, acceleration of the moment of heel rise and a decrease in walking pace. Analysis of the locomotion also revealed a decrease in the amplitude of flexion-extension in the coxofemoral joint, knee joint and tibio-tarsic on the side with more pronounced Parkinsonism. Insufficient knee flexion and hip extension, excessive dorsal flexion of the foot with insufficient plantar flexion were noted. Revealing the last features of locomotion in a particular case allows one to make a plan for a targeted personalized rehabilitation program for a given patient. Thus, the method of three-dimensional video analysis is a valuable diagnostic tool that makes it possible to objectively assess the existing violations of locomotion and identify the targets of rehabilitation.

Tectonic fracturing of the Mesozoic and Cenozoic structures was studied in the Northern Priokhotie (Magadan region). The cataclastic analysis method and the statistical method of fracture density analysis were used to reconstruct their state of stress. It is revealed that the folded structures of the Arman’-Viliga synclinorium are subjected to horizontal shearing; the axis of maximum compression is sublatitudinal (azimuth 67°, angle 12°); extension is submeridional (azimuth 161°, angle 19°). In the Uda-Murgal volcanic arc, horizontal extension with shear takes place; the compression axis is directed to NW (azimuth 259°, angle 29°), and the extension axis to NE (azimuth 152°, angle 26°). In the Okhotsk-Chukotka volcanogenic belt, volcanic structures are in the field of varying tectonic stresses, from predominant horizontal extension to horizontal shear. The Cenozoic intermontane depressions of the Miocene – Pliocene ages are subjected to horizontal shear; the compression axis is directed to NE (azimuth 214°, angle 29°), and the extension axis to NW (azimuth 121°, angle 4°). The results of the comparative analysis of the stress states in the above-mentioned areas reliably show that the diversity of the stress state types is statistically related to the structural positions of the studies sites. Such diversity cannot be explained by an influence of active faults, or by any consecutive superposition of deformations at different stages, despite the fact that the deformations have complicated the observed pattern of the stress states. We conclude that each subsequent geodynamic stage only introduced additional elements into the previous structure, but did not completely transform it.

According to the modern classification system, chronic disorders of consciousness are defined as an altered state of consciousness that develops after coma and is accompanied by the restoration of wakefulness without the complete recovery of conscious activity for more than 28 days after traumatic brain injury. Clinical cases of chronic chronic disorders of consciousness development in women after surgical interventions related to various obstetrical and gynecological pathologies, as well as after routine surgical interventions for comorbid somatic pathology, are of particular interest to obstetricians and gynecologists. In the presented chronic chronic disorders of consciousness related clinical cases of operations for ectopic pregnancy, as well as consequences of elective surgery for gallstone disease, the development of hypo- and normogonadotropic ovarian insufficiency associated with chronic disorders of consciousness is described. The characteristics of the thyroid status, adrenocorticotropic function, and vitamin D levels are given. Further in-depth examination and accumulation of data on patients with chronic disorders of consciousness may provide an opportunity to determine informative markers for prognostication of outcomes, as well as to develop new effective approaches to consciousness rehabilitation in this category of patients.

Objective: investigate epidemiology of infectious complications, intensity of antibacterial therapy, and features of the systemic inflammatory response in patients with prolonged disorders of consciousness.Materials and methods: retrospective case histories analysis of 102 patients with prolonged disorders of consciousness who were treated in the department of anesthesiology and intensive care of Polenov Neurosurgical Institute from 2010 to 2019.Results: during hospitalization all patients (100%) had signs of tracheobronchitis, 86 patients (84,3%) had urinary tract infection, 34 (33,3%) pneumonia, 10 patients (9,8%) meningoventriculitis, and in 9 (8,8%) cases sepsis was diagnosed. Identified bacterial complications required systemic antibacterial therapy. With an average length of hospital stay of 51±42 days, duration of antibiotic therapy was 37±41 days. One-component therapy was applied in half of the cases (54%), slightly less often (43,2%) treatment consisted of two antibiotics, in some cases patient’s condition required prescription of three antibiotics (2,8%). Even in the absence of a focus of infection, temperature and laboratory markers of inflammation in blood samples of patients with prolonged disorders of consciousness were higher than the reference values. CRP was increased with addition of any bacterial complications. Count of white blood cells significantly increased only in case of pneumonia and sepsis. Procalcitonin test was positive in 16,6% observations of urinary tract infection, 30,4% of pneumonia, and 28% of meningoventriculitis.Conclusion: most informative proinflammatory marker of bacterial complications in patients with prolonged disorders of consciousness was CRP. Mild leukocyte response can be explained by immunosuppression in long-term ill patients. Limited value of procalcitonin test may be due to the insufficiency of the semi-quantitative method sensitivity with a threshold level of 0,5 ng/ml, as well as reduced proinflammatory response to local infection in patients with prolonged disorders of consciousness as a result of massive antibacterial therapy, colonization of opportunistic microflora and presence of sympathicotonia (non-infectious systemic inflammatory reaction).

Objectives. To study the predictive significance of proton magnetic resonance spectroscopy (MRS) in patients in a vegetative state. Materials and methods. Multivoxel MRS was carried out in 34 patients in a vegetative state with voxels in the thalamus, globus pallidus, putamen, internal capsule, fornix, brainstem, and the temporal and frontal areas of the cortex. Patients were divided into groups depending on the etiology of vegetative state: 22 patients had the sequelae of traumatic brain injury (TBI) and 12 patients had the sequelae of hypoxic brain damage. The groups were comparable in terms of age and the duration of the vegetative state (mean 2.3 months). Neurological status was assessed both during the hospitalization period and at 6–12 months using the Coma Recovery Scale (CRS-R). MRS results obtained at 6–12 months were compared with baseline data. The outcome of patients with the sequelae of TBI were as follows: persistent vegetative state in six patients, minimally conscious state plus in nine patients, recovery from the state of minimal consciousness in seven. Outcomes of patients with the sequelae of hypoxia were: persistent vegetative state in 10 patients and minimally conscious state plus in two. Results. Decreases in the N-acetylaspartate/creatinine ratio in the thalamus, internal capsule, and temporal cortex correlated with poor outcomes in both groups, while higher values correlated with further recovery of consciousness. Decreases in the N-acetylaspartate/creatinine ratio and the N-acetylaspartate/N-acetylaspartate + choline + creatine ratio in the midbrain correlated with poor outcomes only in patients with the sequelae of hypoxia. Conclusions. The results obtained here suggest that MRS provides a more accurate determination of the outcome in patients with hypoxic brain damage and patients with TBI, who showed recovery of consciousness to the level of coming out of the vegetative state.

Neutrino scattering on nuclei embedded in the hot and dense matter typical of the core collapse of supernovae, neutron star mergers, and proto-neutron stars, is studied. At finite temperatures, neutrinos experience exo- and endo-energy scattering on nuclei due to the Gamow–Teller component of a neutral current. It is shown that the average transfer of energy due to neutrino–nucleus scattering changes from positive to negative values at neutrino energies around four times higher than the temperature of the matter.

Bleeding from the uppergastrointestinal tract is one of the most dangerous complications in patients with acute CVA (ischemic and/or hemorrhagic type). The combination of these conditions requires special attention in the diagnosis and choice of treatment methods. Central Nervous System pathology makes diagnosis, choosing methods oftreatment and the use of drug more complicated to surgeons in the postoperative period. With onset of hemorrhage, antithrombotic treatment should be canceled, that can negatively affect the results of treatment of patients with ischemic type of cerebral vascular accident. The use of anti-ulcer drugs, namely proton pump inhibitors, may increase the risk of hospital-acquired pneumonia,Clostridium difficileinfections, and cardiovascular diseases. These factors force us to consider issues related to diagnosis and treatment plans in this group of patients. The most promising direction in the treatment of patients with ischemic or hemorrhagic stroke in combination with gastroduodenal bleeding is the identification of risk factors for upper gastrointestinal bleeding, early endoscopic diagnosis and treatment, performing preventive treatment. The article presents a brief review of domestic and foreign literature, some issues of etiology and pathogenesis of gastroduodenal bleeding in patients with cancer, the results of our own research, capabilities of modern endoscopy in the diagnosis and treatment of gastrointestinal bleeding in this group of patients. The algorithm of management of patients with a combination of bleeding from the upper gastrointestinal tract and CNS pathologies.

Nucleosynthesis at latge magnetic induction levels relevant to core-collapse supernovae and neutron star mergers is considered. For respective magnetic fields of a strength up to ten teratesla, atomic nuclei exhibit a linear magnetic response due to the Zeeman effect. Such nuclear reactivity can be described in terms of magnetic susceptibility. Susceptibility maxima correspond to half-filled shells. The neutron component rises linearly with increasing shell angular momentum, while the contribution of protons grows quadratically due to considerable income from orbital magnetization. For a case j = l + 1/2, the proton contribution makes tens of nuclear magnetons and significantly exceeds the neutron values which give several units. In a case j = l − 1/2, the proton component is almost zero up to the g shell. A noticeable increase in the generation of corresponding explosive nucleosynthetic products with antimagic numbers is predicted for nuclei at charge freezing conditions. In the iron group region, new seeds are also created for the r-process. In particular, the magnetic enhancement of the volume of 44Ti isotopes is consistent with results from observations and indicates the substantial increase in the abundance of the main titanium isotope (48Ti) in the Galaxy’s chemical composition. Magnetic effects are proven to result in a shift of the r-process path towards smaller mass numbers, as well as an increase in the volume of low-mass nuclides in peaks of the r-process nuclei.

Aim of the study. To evaluate the risk factors for the occurrence of intramuscular hematomas in patients with severe coronavirus infection receiving anticoagulant therapy.Materials and methods. Intramuscular hematomas in five patients with severe COVID-19 disease are reported in the paper. The criteria for selecting patients for the study included respiratory distress requiring oxygen, radiographic signs of severe pneumonia, anticoagulant therapy using low molecular weight heparin (LMWH), and spontaneous intramuscular hematoma. Clinical manifestations, blood coagulation results, conservative and surgical management were analyzed.Results. Standard regimen anticoagulation therapy in patients with coronavirus infection requires vigilance because of a risk of development of hemorrhagic complications.Сonclusion. When assessing a patient with hematomas, an emphasis should be given to examination of patients and changes in hemoglobin and hematocrit levels. Best strategy of anticoagulant therapy for patients with coronavirus infection and high risk of VTE, as well as optimal laboratory monitoring during LMWH administration are yet to be explored.

Introduction Stress caused by childhood adversity (CA) is known to contribute to schizophrenia risk and symptoms. Its effects might be mediated by epigenetic mechanisms, specifically DNA methylation (meDNA) within relevant genes, and predominantly influence the hippocampus and prefrontal cortex (PFC). CYP17A1 is a candidate, as it situates within a schizophrenia risk locus and is involved in glucocorticoid synthesis. Objectives To explore meDNA within CYP17A and its relations to hippocampus- and PFC-dependent schizophrenia symptoms: depression and deficits of declarative memory and executive functions. Methods We assessed meDNA at each CpG within a CYP17A fragment (chr10:104594471-104595887, hg19) in blood of 66 schizophrenia patients using the third-generation sequencing. Immediate memory, depression, cognitive shifting and cognitive inhibition (CI) were assessed with the RAVLT, PANSS, TMT-B and Stroop word-color test, respectively. ANCOVA and regression models adjusted for sex and age were applied to explore the relations between the phenotypes, local haplotype, meDNA and CA, defined as the presence of parental alcoholism or psychiatric illness. Results MeDNA at CpG-SNP rs3781286 correlated with CI (corrected p=0.01). However, there were no main or interaction effects of CA either on meDNA at this site or on CI. Both CI and meDNA associated with haplotype, but subsequent analysis showed that meDNA did not mediate the relation between haplotype and CI. Conclusions Our findings suggest that CYP17A associates with PFC-dependent cognitive deficits in schizophrenia but did not support the hypothesis that CA plays a role in this association via meDNA or any other mechanism. The structure of a large paleoseismodislocation located on the southeastern flank of the Cherskii seismic belt, in the upper reaches of the Ola River, 125 km NNE of Magadan, is studied. Detailed morphologic, morphometric, and structural descriptions of this paleoseismodislocation are made. The main parameters of the landslide body and the dammed lake formed by it are determined. The correlating deposits formed in the dammed lake and upon the surface of rupture are studied, and their radiocarbon age is determined.

A Coulomb-excitation experiment to study electromagnetic properties of Ca42 was performed using a 170-MeV calcium beam from the TANDEM XPU facility at INFN Laboratori Nazionali di Legnaro. γ rays from excited states in Ca42 were measured with the AGATA spectrometer. The magnitudes and relative signs of ten E2 matrix elements coupling six low-lying states in Ca42, including the diagonal E2 matrix elements of 21+ and 22+ states, were determined using the least-squares code gosia. The obtained set of reduced E2 matrix elements was analyzed using the quadrupole sum rule method and yielded overall quadrupole deformation for 01,2+ and 21,2+ states, as well as triaxiality for 01,2+ states, establishing the coexistence of a weakly deformed ground-state band and highly deformed slightly triaxial sideband in Ca42. The experimental results were compared with the state-of-the-art large-scale shell-model and beyond-mean-field calculations, which reproduce well the general picture of shape coexistence in Ca42.

Article consists of literature review, authors experience of the application of neurovisualization and neurophysiological research methods to predict the recovery of consciousness in patients in vegetative state (VS). According to the literature data PET with FDG has higher sensitivity in the detection of signs of consciousness, then functional MRI (fMRI). The method fMRI allows assessing the functional activity of the brain in a state of rest and in response to stimulation with different modalities ― visual, auditory, etc (with the application of active and passive paradigm). A higher specificity in the detection of signs of consciousness have the methodology of fMRI with the active paradigm, at the same time, the absence of signs of consciousness according to the fMRI can not be charged as a basis for the conclusion of a poor prognosis in a particular patient. Neurophysiological tests (EEG, TMS, EP, etc) are more readily available and quite effective. Based on the literature analysis, the authors comes to the conclusion that neurovisualization and neurophysiological tests used in the prediction of the outcome of VS reflects the residual functional activity of different brain areas, in a context of diffuse brain damage, and the recovery of consciousness is usually combined with the restoring of the functional activity off the thalamocortical tracts, which activity, indirectly, is evaluated using these methods. In the authors' opinions, the main disadvantage in the interpretation of the is the lack of a common pathophysiological concept of the organization of brain functions in VS patients. The authors offer for the discussion their concept of stable pathological states of the brain, which is based on the works of Russian pathophysiologists.

The report addresses the actual problem of photon Doppler velocimetry of ejecta from shock-loaded metal samples. It is shown that, for modeling the Doppler spectrum from an expanding cloud of ejected particles, the correlation transport equation can be applied.

The Okhotsk deep focus earthquake (M w = 8.3), the largest in the history of instrumental seismology, occurred on May 24, 2013, at 05:45 UTC in the Sea of Okhotsk near the western coast of the Kamchatka Peninsula. For the first time we have succeeded in catching the field of horizontal and vertical coseismic offsets generated by a strong deep seismic event, and investigating its characteristics using continuous GPS measurements. Based on these data and taking into account the seismological information, we have developed a dislocation model of the Okhotsk deep focus earthquake.

We analyze the first ever GPS observations of static surface deformation from a deep earthquake: the 24 May 2013 Mw 8.3 Sea of Okhotsk, 611 km-deep, event. Previous studies of deep earthquake sources relied on seismology and might have missed evidence for slow slip in the rupture. We observed coseismic static offsets on a GPS network of 20 stations over the Sea of Okhotsk region. The offsets were inverted for the best fitting double-couple source model assuming a layered spherical Earth. The seismic moment calculated from static offsets is only 7% larger than the seismological estimate from Global Centroid Moment Tensor (GCMT). Thus, GPS observations confirm shear faulting as the source model, with no significant slow-slip component. The relative locations of the U.S. Geological Survey hypocenter, GCMT centroid, and the fault from GPS indicate slip extending for tens of kilometers across most of the slab thickness.

To study the prognostic value of magnetic resonance spectroscopy (MRS) in patients with vegetative state/unresponsive wakefulness syndrome (VS/UWS).Thirty-four patients with VS/UWS underwent multi-voxel MRS (thalamus, globus pallidus, putamen, internal capsules, fornix, brainstem, temporal and frontal cortex). Subjects were grouped according to etiology: 22 patients with traumatic brain injury (TBI) (group 1) and 12 patients with a hypoxia (group 2). The groups were matched by age and duration of UWS (mean 2, 3 months). The CRS-R was used to identify the first signs of consciousness during hospitalization and 6-12 months later. Outcomes of the patients with TBI were as follows: chronic VS/UWS (n=6), minimally conscious state (MCS) plus (n=9), emergence from MCS (EMCS) (n=7). Outcomes of the patients with hypoxia were: chronic vegetative state (n=10), minimally conscious state (MCS) (n=2).The decrease in the NAA/Cr ratio in thalamus, capsula interna, temporal cortex are correlated with poor outcome in both groups. Higher rates of NAA/Cr in these structures are correlated with further recovery of consciousness. The decrease in the ratio of NAA Cr and NAA/NAA+Cho+Cr in the midbrain is correlated with poor outcome only in UWS with hypoxia.The results suggest that the MRS allows to more accurately predicting the outcome in VS/UWS patients with hypoxic brain damage, as well as in UWS patients with TBI, who have recovered consciousness to the level of EMCS.Цель исследования. Изучение прогностической значимости метода водородной магнитно-резонансной спектроскопии (МРС) у пациентов с вегетативным состоянием. Материал и методы. Многовоксельная МРС выполнена 34 пациентам в вегетативном состоянии, с постановкой вокселей на таламус, бледный шар, скорлупу, внутреннюю капсулу, свод, ствол мозга, височную и лобную кору. В зависимости от этиологии вегетативного состояния пациенты были разделены на группы: 22 пациента с последствием черепно-мозговой травмы (ЧМТ) и 12 пациентов с последствием гипоксического поражения головного мозга. Группы были сопоставимы по возрасту и продолжительности вегетативного состояния (в среднем 2,3 мес). Неврологический статус оценивали как в период госпитализации, так и спустя 6-12 мес с применением шкалы восстановления после комы (CRS-R). Сопоставление результатов с исходными данными МРС проведено через 6-12 мес. Исходы пациентов с последствием ЧМТ были следующими: хроническое вегетативное состояние - 6 пациентов, состояние минимального сознания плюс - 9, выход из состояния минимального сознания - 7. Исходы пациентов с последствием гипоксии: хроническое вегетативное состояние - 10 пациентов, состояние минимального сознания плюс - 2. Результаты. Снижение соотношения N-ацетиласпартат/креатина в таламусе, внутренней капсуле, височной коре коррелировало с плохим исходом в обеих группах, а более высокие показатели - с дальнейшим восстановлением сознания. Снижение соотношения N-ацетиласпартат/креатина и N-ацетиласпартат/N-ацетиласпартата + холина + креатина в среднем мозге коррелировало с плохим исходом только у пациентов с последствием гипоксии. Заключение. Полученные результаты позволяют предположить, что МРС более точно определяет исход у пациентов с гипоксическим поражением мозга, а также у пациентов с ЧМТ, у которых наблюдается восстановление сознания до уровня выхода из состояния минимального сознания.
