Abstract Background Cerebral cardiac embolism accounts for an increasing proportion of ischemic strokes and transient ischemic attacks. Calcified cerebral emboli are rare and mostly iatrogenic secondary to heart or aorta catheterization. However, spontaneous cerebral calcified embolism in the case of calcified aortic valve is very rare and there are less than 10 case reports in the literature. And a more interesting fact is that such an event, in the context of calcified mitral valve disease, has never been reported, at least to our knowledge. We are reporting a case of spontaneous calcified cerebral embolism revealing a calcified rheumatic mitral valve stenosis. Case presentation We report a case of a 59 year-old Moroccan patient, with a history of rheumatic fever at the age of 14 and no history of recent cardiac intervention or aortic/carotid manipulation, who was admitted to the emergency department after a transient ischemic attack. Physical examination at admission found normal blood pressure of 124/79 mmHg and heart rate of 90 bpm. A 12-lead electrocardiogram showed an atrial fibrillation, no other anomalies. Unenhanced cerebral computed tomography imaging was performed, revealing calcified material inside both middle cerebral arteries. Transthoracic echocardiography was performed, showing severe mitral leaflets calcification with a severe mitral stenosis, probably due to rheumatic heart disease. Cervical arteries Duplex was normal. A vitamin K antagonist (acenocoumarol) was prescribed, targeting an international normalized ratio of 2–3 and mitral valve replacement surgery was performed using mechanical prosthesis. Short- and long-term health, with a 1-year follow-up, were good and the patient did not experience any stroke. Conclusion Spontaneous calcified cerebral emboli secondary to mitral valve leaflet calcifications is an extremely rare condition. Replacement of the valve is the only option to prevent recurrent emboli and outcomes are still to be determined.

The purposes of this study are (1) to explore the concept of design thinking/human-centred design as expressed across the literature, using a systematic content analysis methodology, and (2) to arrive at empirically-based implications and recommendations for the instructional design field drawn from this search and analysis. Since 2000, design thinking has been highly sought after and is increasingly applied to novel challenges. Since then, it has become a force of innovation in business, and a point of contention in design--having created a reductionist perspective of design which has simultaneously become a buzzword for innovation. In the instructional design field, practitioners incorporate the methods used into new design models, and scholars frame new theories within its bounds. Through this process, the meaning and nature of the concept have become complex and clouded. This paper reports results of a systematic content analysis involving examination of 12 databases, analysing 1075 abstracts for fit and identifying 11 core articles. An extended literature review, including more than 70 non-core articles selected from the study data pool, provides a foundation for analysis of the 11 core articles. Results include analysis of the core articles for themes, industry and common citations; recommendations and implications for the instructional design field are drawn from the literature, as well as recommendations for future research.

Plant-pollinator interactions are crucial for ecosystem diversity and functionality. Although, these interactions are still less studied and known, a growing body of evidence is showing that pollinators populations across the world are declining and that affects severely these necessary interactions. This study investigates flowering plants and pollinators diversity, abundance and interactions in a Mediterranean cedar forest in Morocco using two common methods; pan traps and observation plots. From March to August 2023, 1627 insects were captured. Hymenoptera accounted for 35.34%, Diptera 40.14%, Coleoptera 22.62%, and Lepidoptera 1.90%. Bees comprised 85.57% of Hymenoptera, with Andrena, Lasioglossum, and Panyrgus as predominant genera. Beetles, mainly represented by Tropinota, Anthaxia, and Melanthaxia, showed fluctuating abundances within months. 1274 insect visits to 46 flowering plants were recorded, with solitary bees (24.73%), beetles (18.29%), Muscoid flies (10.20%), and honey bees (8.95%) being the most frequent visitors. The most visited plants were the Asteraceae's Mantisalca salamantica, Cardus nutans, and Bellis selvestris, the Brassicaceae's Brassica napus, Erysimum grandiforum and Isatis tinctoria, the cistaceae's Helianthemum hirtum, and the Fabaceae's Linaria sp. This study highlights the diverse plant and pollinators communities in a Mediterranean preserved area in Morocco and gives insights in the distribution of pollinators among flowering plant species. Partial anomalous pulmonary venous return is a rare congenital cardiopathy that can progress to right heart failure in the absence of treatment. It can be associated with an intra-auricular communication. The APVR remains often asymptomatic and its discovery is in the majority of cases fortuitous. The thoracic CT Angiogram is the examination of choice to make the diagnosis by showing the localization, the number and the path APVR drainage. We report the case of a 26-year-old female patient with congenital heart disease type an atrial septal defect; who was diagnosed with a partial bilateral APVR associated with PAH signs. We describe the history of the patient whose symptomatology was nonspecific and in whom the imaging examinations, in particular the CT angiogram, made it possible to reach the diagnosis.

Amniotic embolism is a serious complication. The symptoms associated with amniotic embolism (EA) result from the passage into the maternal circulation of fetal and amniotic material, most often during labor or in immediate postpartum. His prognosis remains gloomy. His diagnosis is difficult and his treatment is just symptomatic. We reviewed 12 files of patients with probable or autopsy-confirmed amniotic embolism.

Multidrug-resistant TB (MDR TB) strains have become a severe issue, motivating researchers to find new TB drugs effective against these MDR strains. In this work, 3D-QSAR (three-dimensional quantitative structure-activity relationship), molecular docking, ADMET, and molecular dynamics studies were performed to investigate the inhibitory activity of 1,2,3-triazole derivatives against Mycobacterium tuberculosis by acting as H37Rv active site inhibitors. The 3D-QSAR study was conducted based on molecular field comparative analysis (CoMFA) and molecular similarity index benchmarking (CoMSIA), which revealed CoMFA model values (Q2 = 0.62; R2 = 0.95) and CoMSIA model values (Q2 = 0.57; R2 = 0.84) for the best-observed models. These values presented excellent predictability, which was also evaluated by using the criteria established by A. Golbraikh and A. Tropsha. A molecular docking interaction between the Gln164 and Lys160 residues was observed at the mycobacterial active site H37Rv, as verified by the interaction type, total score, and MD molecular dynamics. Furthermore, the ADMET properties and drug similarity of these new inhibitors were analyzed. Investigation of new active therapeutic drugs against the emerging drug‐resistant Mycobacterium tuberculosis is a current medical and health challenge. Based on previously reported isoxazole‐based molecules that have a potent effect against Mycobacterium tuberculosis, computational methods were used in the current study to investigate the effect of isoxazole‐based molecules as a potential active drug‐resistant drug Mycobacterium tuberculosis H37Rv. Molecular models of isoxazole‐based molecules were established by 3D‐QSAR study, and different results were interpreted to propose 6 candidate agents more active than the previously reported compounds in the literature. Also, the candidate structures are more active than the therapeutic drug agent Isoniazid (NIH) against Mycobacterium Tuberculosis. Evaluation of the synthetic accessibility coefficient and the Lipinski properties of newly designed agent candidates indicate that these agents meet the criteria of a drug according to the Lipinski and Veber rules because they can be synthesized. In‐silico evaluation of ADMETox properties shows satisfactory results for most newly designed agent candidates. According to molecular docking, the drug candidates have a high score with a stable docked pose in the receptor (PDB code: 5v3y) compared to Isoniazid (NIH).
