BackgroundUnderstanding the predictors of metabolic disorders in persons with HIV/HCV coinfection post-HCV therapy is crucial for improving patient outcomes. Since immune checkpoint proteins are usually upregulated in these persons with HIV/HCV coinfection, we aimed to evaluate the association between plasma immune checkpoint proteins at baseline (before HCV therapy) and metabolic disturbances during the follow-up (about 5 years after successful HCV treatment) in persons with HIV/HCV coinfection.MethodsWe performed a retrospective study on 80 persons with HIV/HCV coinfection with advanced fibrosis or cirrhosis who cleared HCV infection after successful HCV therapy and were followed for about 5 years after completion of HCV treatment. Plasma samples were collected at baseline. Immune checkpoint proteins were analyzed using a Luminex 200™ analyzer. Outcomes were the development of a metabolic event (type 2 diabetes mellitus and/or dyslipidemia) and the change in Triglycerides and glucose (TyG) index.ResultsDuring follow-up, 21 (26%) patients developed metabolic events (type 2 diabetes mellitus/dyslipidemia), and 29 (46.0%) patients had an increase in TyG during the follow-up. Low baseline values of BTLA and LAG-3, two immune checkpoint proteins, were associated with the development of metabolic events (aAMR = 0.69 and aAMR = 0.71, respectively) and with increases in TyG values (aAMR = 0.72 and aAMR = 0.70, respectively). In addition, other immune checkpoint proteins were also inversely associated with increases in TyG.ConclusionWe discovered that low plasma levels of BTLA and LAG-3 before HCV therapy significantly correlate with an increased risk of developing metabolic disorders after treatment.

Abstract Drought affects crops directly, and indirectly by affecting the activity of insect pests and the transmitted pathogens. Here, we established an experiment with well-watered or water-stressed melon plants, later single infected with either cucumber mosaic virus (CMV: non-persistent), or cucurbit aphid-borne yellow virus (CABYV: persistent), or both CMV and CABYV, and mock-inoculated control. We tested whether i) the relation between CMV and CABYV is additive, and ii) the relationship between water stress and virus infection is antagonistic, i.e., water stress primes plants for enhanced tolerance to virus infection. Water stress increased leaf greenness and temperature, and reduced leaf water potential, shoot biomass, stem dimensions, rate of flowering, CABYV symptom severity, and marketable fruit yield. Virus infection reduced leaf water potential transiently in single infected plants and persistently until harvest in double-infected plants. Double-virus infection caused the largest and synergistic reduction of marketable fruit yield. The relationship between water regime and virus treatment was additive in 12 out of 15 traits at harvest, with interactions for leaf water content, leaf:stem ratio, and fruit set. We conclude that both virus-virus relations in double infection and virus-drought relations cannot be generalized because they vary with virus, trait, and plant ontogeny.

Timely and accurate crop acreage information is essential for food security and the informed decision-making by governmental bodies and stakeholders in the agro-economic system. Surveys and fieldwork are expensive and time consuming, and the information is usually only released after the cropping season. Remote sensing technology is inexpensive, scales well for large areas, and offers the opportunity to gather within-season information. However, environmental and technical constraints complicate model applications to larger scales. For example, clouds and snow introduce data gaps, and the weather-driven crop development creates highly variable spectral-temporal signatures. This study determines the earliest possible month that permits the accurate classification of the major crops in the CONUS using gap-filled data, transfer learning, and cloud computing technology. The resulting within-season, annual crop classifications are generated with precision rates of up to 80%, presented as annual crop maps and county-level area statistics. The crop classification products presented in this work are unique, in that they were created with shorter records for the first time, using data within a concise time-range (i.e. a few months) and at a 30-meter spatial resolution, while covering continental scale. This novel prospect allows for potential new applications in forecasting regional crop productions.

Reconstructing of past hydroclimates at regional scales during the Common Era (CE) is necessary to place the current warming in the context of natural climate variability. Here we present a composite record of oxygen isotope variations during last 2500 years based on eight stalagmites from four caves in the central Pyrenees (NE Spain) dominated by temperature variations, with the amount of precipitation playing a minor role. The dataset is compared with other Iberian reconstructions that show a high degree of internal coherence with respect to variability at the centennial scale. The Roman Period (RP) (especially 0–200 CE), the Medieval Climate Anomaly (MCA), and part of the Little Ice Age (LIA) represent the warmest periods, while the coldest decades occurred during the Dark Ages (DA) and most of the LIA intervals (e.g., 520–550 CE and 1800–1850 CE). Importantly, the LIA cooling or the MCA warming were not continuous or uniform and exhibited high decadal variability. The Industrial Era (IE) shows an overall warming trend although with marked cycles and partial stabilization during the last 2 decades (1990–2010). The strong coherence between the speleothem data, European temperature reconstructions and global tree-ring data informs about the regional representativeness of this new record as Pyrenean past climate variations. Solar variability, likely through its impact on the North Atlantic Oscillation, and major volcanic eruptions appear to be the two main drivers of climate in southwestern Europe during the past 2.5 millennia.

Introduction Infants born preterm usually show a Preterm Behavioural Phenotype, which includes mixed symptomatology characterized by lack of attention, anxiety and social difficulties, with a 3-4 times greater risk of disorders in further childhood. Critically, this behavioural pattern is also observed in infants born after a threated preterm labour (TPL), regardless of the presence of prematurity. It is known that the course of this Preterm Behavioural Phenotype shows high variability. Nevertheless, the predictors of this Preterm Behavioural Phenotype prognosis remain unknown. Objectives This study aimed to explore the predictors of change of Preterm Behavioural Phenotype symptomatology during preschool ages in order to improve prognosis. Methods In this prospective cohort study, 117 mother–child pairs who experienced TPL were recruited. Preterm Behavioural Phenotype symptoms were assessed at age 2 and 6 using Child Behaviour Checklist. Gestational age at birth, maternal anxiety trait, maternal history of psychological traumas, prenatal and postnatal maternal depression, anxiety, and cortisol as well as parenting stress were included as predictors in a regression model. Results Whereas increased internalizing problems were associated with a previous trauma history (p = .003), increased externalizing symptoms were linked to prenatal and postnatal maternal anxiety (p = .004 and p = .018, respectively). Conclusions Identifying modifiable risk factors, such as the history of maternal traumas and anxiety at TPL diagnosis and postpartum is recommendable to enhance better prognosis of Preterm Behavioural Phenotype in the offspring. Introduction Emotional dysregulation are considered early manifestations of neuropsychiatric disorders. Recent research has shown that a threatened preterm labour (TPL) represents an adverse prenatal event that involves temperament disturbances, even in absence of prematurity. Thus, full-term TPL infants at 6 months of age are characterized by lower positive affect, higher negative affect, and worse emotional regulation relative to a full-term non-TPL control group. Objectives The aim of this study is to explore the predictors of change of emotional infant competences. Methods This prospective cohort study recruited mothers who suffered from a TPL. Infants’ temperament assessment was performed at 6 and 30 months of age using the Rothbart Behaviour Questionnaires, examining positive affectivity/surgency, negative emotionality, and orienting and emotional regulatory capacity. A regression model was carried out, including gestational age at birth, maternal anxiety trait, maternal history of psychological traumas, prenatal and postnatal maternal depression, anxiety, and cortisol as well as parenting stress as predictors. Results Increased positive affectivity was related with lower paternal stress (p = .044). Maternal history of trauma and parenting stress was associated with increased negative emotionality (p = .037 and p = .045, respectively). Increased emotional regulation disturbance was linked to low gestational age at birth (p < .001), higher postnatal depression (p = .002), higher prenatal anxiety at TPL diagnosis (p = .039) and higher postnatal anxiety (p = .008). Conclusions Therefore, maternal previous traumas, maternal psychopathology from pregnancy to postpartum as well as parenting stress should be considered in psychological treatment to improve infant’s emotional competences and prevent subsequent neuropsychiatric disorders. Introduction Due to the global humanitarian crisis, there has been a significant increase in global immigration.(1) The migration process typically involves multiple trauma exposures that are sustained over time(2), which may result in an impact on the mental health of these individuals(3), such as posttraumatic stress disorder(3). A recent meta-analysis estimated that 25% of migrants had PTSD(15), which is significantly higher than the 0.2% to 3.8 percent prevalence data found for the general population(4). In addition, a number of meta-analyses indicate an increased risk of psychosis among immigrants(5). Despite this rise, there is a gap in trauma research in non-refugee immigrants, particularly those with psychotic disorders. Objectives To describe and compare PTSD diagnosis between immigrants and locals recruited from mental health services in Barcelona. Methods Patients who have presented, according to DSM-V criteria, one or more non-affective psychotic episodes, were recruited in Acute and Chronic inpatients units at Hospital del Mar (Barcelona) from November 2019 to June 2021, leading to a total sample of 199 patients. Demographic characteristics of patients, clinical data and main pharmacological treatment were recorded through a questionnaire. Database information was completed with electronic medical records. Global Assessment of Posttraumatic Stress Questionnaire (EGEP-5) was used as an instrument to assess PTSD diagnosis, main trauma nature and PTSD symptoms. Comparative analysis was performed with IBM SPSS Statistics (Chicago INC) using Chi-Square Test for qualitative variables and t-Student test for continuous variables. Covariate adjustment with demographic and clinical variables was performed by ANOVA test. Study received local ethics committee approval “CEIC” (No. 2019/8398/I). Results From the total sample of 199 individuals, 98 were immigrants and 98 locals. From the total sample 39 individuals (19.69%) presented PTSD. 32.3% of the immigrants with psychotic disorders presented PTSD compared to 7.1% of the locals with psychotic disorders (F1=19.9, p=0.00). Most traumatic events related to PTSD in immigrants were: “murder of relatives” (33.1%), Physical violence (21.9%) and Terrorism (15.6%) in locals were: “physical violence” (28.6%). Immigrants and locals with psychotic disorders showed similar averages of symptoms, except for avoidance symptoms where locals showed a mean of 5.1 compared to a mean of 3.5 in the immigrant group. Finally, immigrants showed one more functionality affected area by PTSD (5.1) when compared to locals (4) (F7=3.9, p=0.05). Conclusions According to our results there are important differences in PTSD prevalence between immigrants and locals with psychotic disorders. These findings ought to be taken into consideration for programs that are both clinically and sociopolitically tailored to improve assessment and treatment for this population. The Aneto glacier, although it may be considered a very small glacier ( km2), is the largest glacier in the Pyrenees. Its surface and thickness loss have been continuous in recent decades, and there have been signs of accelerated melting in recent years. In this study, thickness and surface losses of the Aneto glacier from 1981 to 2022 are investigated using historical aerial imagery, airborne lidar point clouds and unoccupied aerial vehicle (UAV) imagery. A ground-penetrating radar (GPR) survey conducted in 2020, combined with data from photogrammetric analyses, allowed us to reconstruct the current ice thickness and also the existing ice distribution in 1981 and 2011. Over the last 41 years, the total glacierised area has decreased by 64.7 %, and the ice thickness has decreased, on average, by 30.5 m. The mean remaining ice thickness in autumn 2022 was 11.9 m, as against the mean thickness of 32.9, 19.2 and 15.0 m reconstructed for 1981 and 2011 and observed in 2020, respectively. The results demonstrate the critical situation of the glacier, with an imminent segmentation into two smaller ice bodies and no evidence of an accumulation zone. We also found that the occurrence of an extremely hot and dry year, as observed in the 2021–2022 season, leads to a drastic degradation of the glacier, posing a high risk to the persistence of the Aneto glacier, a situation that could extend to the rest of the Pyrenean glaciers in a relatively short time.

Purpose: To analyse cases with suspected autoimmune-associated epilepsy (AAEp) and to compare them to patients with acute symptomatic seizures secondary to autoimmune encephalitis (ASS-AEn). Methods: Single-centre retrospective analysis of patients with suspected AAEp seen in an outpatient epilepsy clinic between 2014 and 2021. Differences according to autoimmune testing results and their responsiveness to immunotherapy were assessed and compared with our cohort of patients with ASS-AEn. Results: A total of 30 patients were included: 18 women (60%); mean age 28.2 years at seizure-onset. AAEp was diagnosed in 14 (46.6%), on the basis of antineuronal antibodies, CSF pleocytosis/OCB (oligoclonal bands), MRI with neuroinflammation, and/or PET hypermetabolism. Thirteen patients (43.3%) received immunotherapy, of whom 5 responded (38.4%). Abstract Background The increasing demand for childbirth care based on physiological principles has led official bodies to encourage health centers to provide evidence-based care aimed at promoting women’s participation in informed decision-making and avoiding excessive medical intervention during childbirth. One of the goals is to reduce pain and find alternative measures to epidural anesthesia to enhance women’s autonomy and well-being during childbirth. Currently, water immersion is used as a non-pharmacological method for pain relief. This review aimed to identify and synthesize evidence on women’s and midwives’ experiences, values, and preferences regarding water immersion during childbirth. Methods A systematic review and thematic synthesis of qualitative evidence were conducted. Databases were searched and references were checked according to specific criteria. Studies that used qualitative data collection and analysis methods to examine the opinions of women or midwives in the hospital setting were included. Non-qualitative studies, mixed-methods studies that did not separately report qualitative results, and studies in languages other than English or Spanish were excluded. The Critical Appraisal Skills Program Qualitative Research Checklist was used to assess study quality, and results were synthesized using thematic synthesis. Results Thirteen studies met the inclusion criteria and were included in this review. The qualitative studies yielded three key themes: 1) reasons identified by women and midwives for choosing a water birth, 2) benefits experienced in water births, and 3) barriers and facilitators of water immersion during childbirth. Conclusions The evidence from qualitative studies indicates that women report benefits associated with water birth. From the perspective of midwives, ensuring safe water births requires adequate resources, midwives training, and rigorous standardized protocols to ensure that all pregnant women can safely opt for water immersion during childbirth with satisfactory results.

Ice caves are one of the least studied parts of the cryosphere, particularly those located in inaccessible permafrost areas at high altitudes or high latitudes. We characterize the climate dynamics and the geomorphological features of Devaux cave, an outstanding ice cave in the Central Pyrenees on the French–Spanish border. Two distinct cave sectors were identified based on air temperature and geomorphological observations. The first one comprises well-ventilated galleries with large temperature oscillations likely influenced by a cave river. The second sector corresponds to more isolated chambers, where air and rock temperatures stay below 0 ∘C throughout the year. Seasonal layered ice and hoarfrost occupy the first sector, while transparent, massive perennial ice is present in the isolated chambers. Cryogenic calcite and gypsum are mainly present within the perennial ice. During winter, the cave river freezes at the outlet, resulting in a damming and backflooding of the cave. We suggest that relict ice formations record past damming events with the subsequent formation of congelation ice. δ34S values of gypsum indicate that the sulfate originated from the oxidation of pyrite present in the bedrock. Several features including air and rock temperatures, the absence of drips, the small loss of ice in the past 7 decades, and the location of ice bodies in the cave indicate that the cave permafrost is the result of a combination of undercooling by ventilation and diffusive heat transfer from the surrounding permafrost, reaching a thickness of ∼ 200 m.

Grit, the passion for achieving long-term goals, has been conceived as a two-dimensional construct (Consistency of interest and Perseverance of effort). The construct is well known for its easy measurement and its relationship with performance, including academic performance. However, there have been different criticisms, such as the overlap of grit with other personality characteristics, the variability in the psychometric structure of the Grit-O and Grit-S tests, and the fact that some work reports a weak influence of grit on academic performance. Within this framework, this study contrasts different psychometric structures of the Grit-S scale and its relationship with the academic performance of higher education students. So, with this purpose, a dichotomous model of high and low grit, a K-medias clustering model, and three structural equation models have been tested. The results indicate that (a) there is a statistically significant relationship between grit--mainly determined by the consistency dimension--and academic performance, although it decreases when controlling for contextual variables, and (b) Consistency mediates the relationship between Perseverance and academic performance. The instability of grit to predict performance is discussed, and the thesis of a two-sub-dimensional structure is supported.

Architectural glazing has several advantages in terms of aesthetics and user well-being perspectives. However, they can have large impacts on heating and cooling demands and artificial lighting requirements. Ethylene tetrafluoroethylene (ETFE) cushion systems present adequate insulating and transparency characteristics, and, combined with organic photovoltaic (OPV) modules, become a glazing element leading towards energy efficient buildings. The present manuscript reports on a detailed optical, thermal and electrical analysis of a 3-layer ETFE/ OPV cushion that helps the design of these glazing-type systems by providing a better understanding of their performance. Spectrophotometric optical measurements up to 50μmallow proper estimations of an individual layer radiative behavior. As a matter of fact, these measurements feed the radiative and thermal models that aim at determining the behavior of the organic photovoltaic module as a function of its position in the cushion. Based on the organic photovoltaic module spectral response, electrical power production is estimated. Results reveal that the best configuration is the one placing the organic photovoltaic module in the outer layer, since it represents the case where the OPV module performs better due to the higher incident irradiance and the low dependency on temperature of the generated power.

Introduction: Spinal muscular atrophy (SMA) is a neurodegenerative disease caused by a biallelic mutation of the SMN1 gene, located on the long arm of chromosome 5, and predominantly affects the motor neurons of the anterior horn of the spinal cord, causing progressive muscle weakness and atrophy. The development of disease-modifying treatments is significantly changing the natural history of SMA, but uncertainty remains about which patients can benefit from these treatments and how that benefit should be measured. Methodology: A group of experts specialised in neurology, neuropediatrics, and rehabilitation and representatives of the Spanish association of patients with SMA followed the Delphi method to reach a consensus on 5 issues related to the use of these new treatments: general aspects, treatment objectives, outcome assessment tools, requirements of the treating centres, and regulation of their use. Consensus was considered to be achieved when a response received at least 80% of votes. Results: Treatment protocols are useful for regulating the use of high-impact medications and should guide treatment, but should be updated regularly to take into account the most recent evidence available, and their implementation should be assessed on an individual basis. Age, baseline functional status, and, in the case of children, the type of SMA and the number of copies of SMN2 are characteristics that should be considered when establishing therapeutic objectives, assessment tools, and the use of such treatments. The cost-effectiveness of these treatments in paediatric patients is mainly influenced by early treatment onset; therefore, the implementation of neonatal screening is recommended. Conclusions: The RET-AME consensus recommendations provide a frame of reference for the appropriate use of disease-modifying treatments in patients with SMA. Resumen: Introducción: La atrofia muscular espinal (AME) es una enfermedad neurodegenerativa, causada por una mutación bialélica del gen 5q SMN1, que afecta predominantemente a las neuronas motoras de la asta anterior medular causando una progresiva debilidad y atrofia muscular. La aparición de tratamientos modificadores del curso de la enfermedad está cambiando considerablemente la historia natural de la AME, pero existe todavía incertidumbre sobre qué pacientes se pueden beneficiar de estos tratamientos y cómo se debería medir ese beneficio. Metodología: Un grupo de expertos especialistas en neurología, neuropediatría, rehabilitación y de la asociación de pacientes FundAME analizaron, siguiendo la metodología Delphi, 5 apartados relacionados con el uso de los nuevos tratamientos: aspectos generales; objetivos del tratamiento; herramientas de medición de resultados; requisitos de los centros tratantes; y regulación de su uso. Se definió como consenso cuando una respuesta recibió al menos el 80% de los votos. Resultados: Los protocolos de tratamiento son útiles para regular el uso de medicamentos de alto impacto y deben constituir una guía para el tratamiento, pero se deben actualizar regularmente para recoger la evidencia más reciente disponible y su implementación se debe valorar de forma individualizada. La edad, la funcionalidad basal y, en el caso de los niños, el tipo de AME y el número de copias de SMN2 son características que se deben tener en cuenta a la hora de establecer los objetivos terapéuticos, las herramientas de medición y el uso de dichos tratamientos. El aspecto más determinante del coste-efectividad de estos tratamientos en la edad pediátrica es su inicio precoz, por lo que se recomienda la instauración de un cribado neonatal. Conclusiones: Las recomendaciones del consenso RET-AME proporcionan un marco de referencia para el uso adecuado de tratamientos modificadores de la enfermedad en pacientes con AME.

Intro: Coronaviruses infect humans and a wide range of wild and domestic animals. Some CoVs could be zoonotic, being able to mutate, crossing the species barrier and infecting humans (e.g. SARS-CoV and MERS-CoV). Since the emergence of SARS-CoV-2, several studies were carried out to ascertain the susceptibility of both domestic and wild animals to SARS-CoV-2. However, information on some species is lacking, and for others only RDB-ACE receptor affinity studies have been carried out. Considering the high densities of Marmota marmota in the alpine environment, where livestock and recreational activities are commonly present, this study aims to investigate the presence and characterization of CoVs in this species. Methods: During provincial relocation plan carried out in 2021 and 2022, 170 alpine marmots were captured in municipality of Livigno in Sondrio province (North-Italy) for decreasing animal density and, after a quarantine period, they were released in other alpine places. Faecal samples were collected from each animal and then subjected to RNA extraction and nested RT-PCR pan-Coronavirus and real time RT-PCR for SARS-CoV-2. PCR positive samples for pan-CoV were then sequenced. Findings: The pan-Coronavirus RT-PCR detected CoVs in seven marmots. The CoV sequence originating from one marmot sampled in 2021 had 97% affinity to strains isolated in lagomorphs. The other six sequences from 2022 were highly correlate with Bovine Beta-CoVs. This could be explained by the fact that marmots share alpine pastures with these species; in fact, the trapping area in 2022 represented grazing and forage production areas. All samples tested for SARS-CoV-2 resulted negative. Conclusion: Despite the absence of zoonotic coronaviruses, marmots show high plasticity in harbouring CoVs of sympatric species. For this reason, and considering the affinity of their ACE-receptor demonstrated for SARS-CoV, it would be worthwhile to increase surveillance for CoVs in this species.

Intro: Several rodents, including mice and the brown rat, are synanthropic animals usually found in rural and urban environments in contact with other animals and humans. Rodents are natural reservoirs of infectious agents and could harbour a plethora of zoonotic pathogens of public health importance. Taking advantage of a parallel study on presence and distribution of Hantaviruses, we aimed to investigate the occurrence in mice of other viruses with zoonotic or economic impact. Methods: From May to July 2022, 41 mice (Mus domesticus) were captured and killed by using baited snap traps in 13 selected cattle, goat and poultry farms located in the Piedmont region. Gut and lung samples were homogenised and tested by PCR methods for pan-Coronavirus (CoV) and SARS-CoV-2, pan-Pestivirus, Mammalian orthoreoviruses, Canine Distemper virus (CDV), Flaviviruses, Influenza A (IAV) and D (IDV) viruses. Findings: All captured animals did not present at necropsy lesions related to infectious diseases. Virological investigations detected the presence of CoV in six mice. By sequencing Rodent CoVs was identified in two samples (four more pending). Mammalian orthoreovirus was detected in nine animals and typing and characterization are in progress. One mouse, captured in a bovine farm, tested slightly positive for IDV and confirmation of positivity is in progress by complete sequencing with NGS approach. All samples were negative for Flaviviruses, IAV, CDV, pan-Pestivirus and SARS-CoV-2. Conclusion: Rodents are well adapted to a wide range of habitats, including peri-urban and rural environments, where they benefit from human activities. These results, although preliminary, underline the importance of enhancing surveillance in rodents in anthropized areas to better assess the presence of zoonotic agents and the potential risk of transmission.

Intro: Methicillin susceptibility prevails within clinically relevant Staphylococcus aureus strains. This study aimed to describe proportions of Methicillin- Susceptible S. aureus (MSSA), prescription behavior for its treatment, and impact of a post-prescription antimicrobial stewardship (AMS) strategy within two private tertiary care Mexican hospitals over 10 years. Methods: Patients older than 14 years with an S. aureus culture-proven infection between 2011 to 2020 were included. Disease was classified as monomicrobial or polymicrobial; strains were classified as MSSA if Cefoxitin susceptible. Spectrum Score was calculated according to the participants’ antibiotic regimen at: admission, 72 hours after culture results, and end-of- therapy. During the last two years analyzed, post-prescription review with feedback to prescribers as an AMS strategy. MSSA rates were compared within both time periods. Findings: We identified 1322 clinically relevant S. aureus strains isolated from 2011 to 2020. A total of 817 specimens were MSSA (61.8%), while only 505 demonstrated methicillin resistance(38.2%). During the antimicrobial stewardship period, MSSA proportion increased by 21.6% compared to the pre- intervention period(78.9% vs 57.3%). In patients with monomicrobial MSSA infections, mean decrease Spectrum Score was lower at end of therapy in comparison to that calculated 72 hours after culture results(1.3 vs 0.2 points). The latter didn't seem to differ when contrasting the Spectrum score between the period before(0.3 points) and after(0.2 points) the AMS intervention. The three most commonly used antimicrobials at admission, 72 hours after culture results, and end of therapy were respectively: Ceftriaxone, Meropenem, Vancomycin; Clindamycin, Vancomycin, Linezolid; and Trimethoprim/Sulfamethoxazole, Clindamycin, Ciprofloxacin. Conclusion: MSSA caused more infections in comparison to methicillin-resistant strains in our studied population. Despite the poor antimicrobial de-escalation in MSSA infections, evidenced by the high Spectrum scores, methicillin resistance has decreased over time. Post prescription interventions have not proven to be effective at these private hospitals and AMS strategies should be expanded to continue avoiding antibiotic resistance.

The adoption of learning analytics (LA) in higher education institutions (HEIs) in Mexico is still at an early stage despite increasing global interest and advances in the field. The use of educational data remains a challenging puzzle for many universities, which strive to provide students, teachers, and institutional administrators with information and insights to better understand their performance. The objective of this study was to identify the perspectives of teachers, students, and administrators about the use of educational data to explore opportunities for the adoption and integration of LA in three different Mexican universities. A qualitative approach was used, adopting instruments and guidelines previously developed in the framework of Learning Analytics for Latin America (LALA) project, adapting them to the Mexican context. Methods included (1) structured interviews with high-level institutional administrators and (2) focus groups with students, teachers, and other educational administrators. Results showed that perceptions are oriented toward improving school performance through data-based feedback, with ethical responsibility. Emergent categories were physical and mental health, development of healthy relationships and well-being, feedback style, and governance in a bureaucratic setting. The specific modern construct of LA still needs to be internalized and disseminated to Mexican universities' educational stakeholders to increase the likelihood of successful adoption.

Introduction This umbrella review is the frst to systematically examine psychological trauma as a transdiagnostic risk factor across psychiatric conditions. Objectives This review aimed to be the frst to evaluate whether psychological trauma fulflilled criteria as a transdiagnostic risk factor cutting across various diagnostic categories and spectra. Transdiagnosticity will be assessed against the framework of the TRANSD criteria (Fusar-Poli, World Psychiatry 2019; 18 361-362). The paper additionally aimed to analyse the association of psychopathology with specifc trauma type. Methods We searched Pubmed, Scopus, and PsycNET databases from inception until 01/05/2021 for systematic reviews/meta-analyses evaluating the association between psychological trauma and at least one diagnosed mental disorder. We re-calculated the odds ratio (OR), then classifed the association as convincing, highly suggestive, suggestive, or weak, based on the number of cases and controls with and without psychological trauma, random-efects p value, the 95% conf- dence interval of the largest study, heterogeneity between studies, 95% prediction interval, small-study efect, and excess significance bias. Additional outcomes were the association between specifc trauma types and specific mental disorders, and a sensitivity analysis for childhood trauma. Transdiagnosticity was assessed using TRANSD criteria. The review was pre-registered in Prospero CRD42020157308 and followed PRISMA/MOOSE guidelines. Results Fourteen reviews met inclusion criteria, comprising 16,277 cases and 77,586 controls. Psychological trauma met TRANSD criteria as a transdiagnostic factor across diferent diagnostic criteria and spectra. There was highly suggestive evidence of an association between psychological trauma at any time-point and any mental disorder (OR=2.92) and between childhood trauma and any mental disorder (OR=2.90). Regarding specifc trauma types, convincing evidence linked physical abuse (OR=2.36) and highly suggestive evidence linked sexual abuse (OR=3.47) with a range of mental disorders, and convincing evidence linked emotional abuse to anxiety disorders (OR=3.05); there were no data for emotional abuse with other disorders. Introduction A significant global increase in immigration has been reported due to humanitarian crisis around the world. Trauma exposure related to migration process is usually multiple and maintained in long-term which could provoke a cumulative effect. Moreover, several meta-analysis describe increased risk for psychosis in immigrant population. Despite this increase, there is a lack of research in non-refugee immigrants specially within those with psychotic disorder. Objectives The aim of the study is to describe and compare cumulative lifetime trauma between immigrants and locals with psychotic disorder. Methods Patients who have presented, according to DSM-V criteria, one or more non-affective psychotic episodes, were recruited in Acute and Chronic inpatients units at Hospital del Mar (Barcelona) from November 2019 to June 2021, leading to a total sample of 199 patients. Demographic characteristics of patients, clinical data and main pharmacological treatment were recorded through a questionnaire. Database information was completed with electronic medical records. Cumulative trauma Scale was used as instrument to assess lifetime trauma exposure frequency and distress. Comparative analysis was performed with IBM SPSS Statistics (Chicago INC) using Chi-Square Test for qualitative variables and t-Student test for continuous variables. Covariate adjustment with demographic and clinical variables was performed by ANOVA test. Study received local ethics committee approval “CEIC” (No. 2019/8398/I). Results From a total of 198 patients, 99 (50%) were immigrants and 99 (50%) locals. Immigrants were exposed on average 3 times more to lifetime traumatic events (16.12) when compared to locals (5.39). Likewise, distress intensity caused by trauma exposure had a mean of 97.13 in immigrants compared to 27.24 in locals. Traumatic events more present in immigrants’ group were “uprooting” (82.8%), “physical abuse” (76.8%), racial discrimination (74.7%), threat of death (74.7%) and life-threatening to close friend (72.2%) and in local group was school failure (42.4%), serious disease (38.4%), accidents (36.4%), physical abuse (36.4%) and interpersonal relationship rejection (36.4%). Conclusions According to our results there are important differences in cumulative traumatic events between immigrants and locals with psychotic disorder. Immigrants showed three times more lifetime traumatic events than locals. Likewise, immigrants presented significant higher level of distress caused by lifetime trauma and the nature of traumatic events was more severe. These results should be considered in order to offer better assessment and treatment to this population considering this comorbidity. Disclosure of InterestNone Declared

Objectives: The aim of this study was to evaluate the impact on 30-day mortality of early use of corticosteroids in COVID-19 patients with supplementary oxygen requirements and without invasive mechanical ventilation at the initiation of therapy. Methods: All patients hospitalized with COVID-19 between April 15 and July 15, 2020, and requiring supplementary oxygen, were prospectively included in a database. Patients who died or required intubation within the first 48 hours were excluded. Patients who received corticosteroids within the first 5 days of hospitalization and at least 24 hours prior to intubation were allocated to the ‘early corticosteroids’ group. To compare both populations and adjust for non-random treatment assignment bias, a weight-adjusted propensity score model was used. Results: In total, 571 patients met the inclusion criteria, 520 had sufficient information for the analysis. Of these, 233 received early corticosteroids and 287 did not. Analysis showed a reduction of 8.5% (p = 0.038) in 30-day mortality in the early corticosteroid group. The reduction in mortality was not significant when patients with corticosteroid initiation between day 5 and day 8 of hospitalization were included. Conclusion: Early corticosteroid use reduced mortality in patients with pneumonia due to COVID-19, who required supplementary oxygen but not initial invasive mechanical ventilation.
