The aim : to evaluate the effect of angiotensin-converting enzyme inhibitor (ACEI) therapy on the course of chronic kidney disease (CKD) in children with urinary system congenital malformations. Patients and methods . 119 patients with urinary system congenital malformations aged 3 to 18 years were examined. A control group of 10 clinically healthy children. There were 3 groups: I - 55 children with congenital vesicoureteral reflex, II - 34 children with congenital hydronephrosis, III - 30 children with other forms of urinary systems dysembryogenesis. ACEI therapy "Enap" ("KRKA", Slovenia), held for 6 months (children up to 14 years at a dose of 0.2 mg/kg/24 h in 1 reception, adolescents 5-10 mg/24 h in 1 reception). Results . were evaluated according to clinical data, according to the daily monitoring of blood pressure (BP), renal function, renin concentration, plasma aldosterone. Renin and aldosterone production was determined in blood plasma (lying down) by enzyme immunoassay. Results . During ACEI treatment in all children there was stabilization of BP, renal function improvement. Patients with hypertension showed a significant decrease in the concentration of renin. ACEI therapy leads to normalization of aldosterone concentration in patients with hypertension and without hypertension. Conclusion. The analysis of the course of CKD in patients with US CM on the background of ACEI therapy demonstrates the main aspects of the anti-fibrotic action of the pharmacological blockade of the renin-aldosterone ACEI system.

THE AIM. To determine the concentration of natriuretic peptide in the blood serum in children with congenital malformations of the urinary system (CM US) and to compare with the activity of renin-angiotensin-aldosterone system (RAAS).MATERIALS AND METHODS.119 patients with CM US aged 3 to 18 years were examined. A control group of 10 clinically healthy children. 3 groups were assigned: group I – 55 children with congenital vesicoureteral reflux, and group II – 34 children with congenital hydronephrosis and ureterohydronephrosis, III group – 30 children with other forms of dysembryogenesis of the US. Following indicators were identified by ELISA in the blood: renin, aldosterone, N – terminal propeptide natriuretic hormone (NT-рroВNР). RESULTS.NT-рroВNР, renin and aldosterone hyperproduction were diagnosed in 59,6%, 69,7%, 54.6 % of sick children relatively. Concentrations were higher in all variants of malformations in comparison with the control group. Significant differences were revealed in obstructive species, where arterial hypertension (AH) was diagnosed more often. Patients with AH recorded significantly higher concentrations of NT-proВNР and renin.CONCLUSION.The key point in pathological processes developmentand progression in the cardiovascular system and kidneys is the activation of RAAS. The system of natriuretic factors is important in maintaining the compensated state of patients due to the blockade of RAAS.

To develop integral assessment of the health status based on the examination of representative samples from different regions of Russia (Veliky Novgorod, Nizhni-Novgorod, Vologda, Omsk and Nalchik) with the use of special questionnaires and simple anthropometric and functional methods in the framework of a prospective 3-year population study of organized groups.The above questionnaires allowed to elucidate the socio-economic status of the patients, their somatic and psychological conditions (lifestyle index or psychological protection mechanisms (PPM), social adaptation, sanogenic reflexia; also used were Eysenck's psychotism scale, hospital anxiety and depression scale, the perceived stress scale (PSS), and moral potential of personality development scale.It was shown that the profile of psychological protection mechanisms is dominated by primitive reactions, such as projection, protection, and negation. Most subjects from the sample formed the pathogenic type of reflexia (inability to resolve a problem situation based on the cognitive components of the patient's mental state). Mathematical treatment of the so-called "simple cross-tabulation" with unification of all negative PPM revealed the highly significant relationship of LSI (not norm) with most previous somatic diseases (p0.001), i.e. the history of myocardial infarction, angina of effort, cardiovascular syndromes, diabetes, hypertension, chronic broncho-pulmonary, hepatic, gastrointestinal, urogynecological, oncological, and psychic diseases. (p0.007).Pyschological changes have greater influence than somatic ones on physiological parameters determining the population health status which suggests the necessity of taking them into account when planning health promotion measures.

Evaluation of independent and combined interrelations of bronchial obstruction and autonomic regulation of cardiac rhythm in elderly patients with bronchial asthma was carried out. Positive correlation of the extent of bronchial obstruction and decrease of absolute indices of cardiac rhythm variability was established.

Цель. Изучить влияние психосоциальных факторов на здоровье организованной популяции и параллельно - синдром эмоционального выгорания (СЭВ).Материал и методы. По специальной шкале были протестированы 2227 рабочих и служащих в 5 городах России. СЭВ - это неминуемое последствие неадекватного разрешения стресса на рабочем месте и профессионального кризиса (самоощущение собственной профессиональной некомпетенции). Непрекращающееся воздействие на человека стрессовых ситуаций приводит к интеллектуальному, душевному и физическому переутомлению. Если стресс имеет негативные последствия для здоровья - это дистресс (СЭВ). Проявления СЭВ - организационные недостатки. К ним относятся такие коллизии, как высокая рабочая нагрузка и при этом дефицит времени для выполнения работы; отсутствие поддержки со стороны коллег и начальства; недостаточное вознаграждение за работу. Сюда же следует отнести противоречивые требования к сотруднику, постоянную угрозу штрафных санкций и т.п. Опасность СЭВ заключается в том, что «выгоранию» свойственно изо дня в день прогрессировать. На его фоне могут обостряться разные хронические заболевания, развиваться новые болезни и меняться физиологические параметры с саногенных на патогенные. Как правило, профессиональная помощь врача не приносит быстрого и стойкого облегчения.Результаты. Низкие значения (0-1) СЭВ (характерно для эмоционально здоровой части популяции) отмечены у 47,6 и 58,7% наиболее молодых (25-34 года) мужчин и женщин. С возрастом этот процент снижается до 23,3 и 32,4% в 35-44 года и до 9,1 и 10,9% - в 65-74 года. Зато возрастает частота менее благоприятных значений СЭВ (2-6 балла), это касается лиц зрелого и особенно пожилого возраста. В 35-44 года такие значения отмечены более чем в 60% наблюдений, а у пожилых - более чем в 80%. Явный же СЭВ диагностирован, конечно, существенно реже, но все же у ~15% мужчин и 6-8% женщин зрелого и пожилоговозраста (65-74 года).Заключение. При проведении профилактики хронических неинфекционных заболеваний в производственных коллективах должны учитываться психологическая обстановка и правильная организация труда.

Twenty-four hour blood pressure monitoring in groups of patients of elderly age with bronchial asthma and hypertension was conducted. The study showed an increased variability of blood pressure and insufficient reduction in night time that leads to development of cardiovascular complications.

was detection of behavioral factors, main risk factors of development of cardiovascular diseases (CVD), assessment of adequacy of their management, as well as assessment of quality of life in a cohort of patients with ischemic heart disease from three republics of the North Caucasian region.A sample of patients with ischemic heart disease (IHD) aged 40-69 years (n=1500) from 3 republics of the North Caucasian Federal Okrug (Ingushetia, Kabardino-Balkaria, and North Ossetia - Alania) were included in a clinico-epidemiological project. In this work we present results obtained in three towns and districts of the Republic Ingushetia (300 men and 230 women with IHD and class II-III effort angina). Socio-demographic factors, family history of CVD, disturbances of nutrition, and physical activity were assessed by standard WHO questionnaires. For assessment of anthropometric deviations we measured height, body mass, and waist circumference. With the aim of detection of hyperlipidemia and disturbances of carbohydrate metabolism we measured levels of total cholesterol and fasting glucose in venous blood. Quality of life was assessed by EQ-5D questionnaire which comprised 5 points: movement, self service, everyday activity, pain/discomfort, and anxiety/depression.Each forth men (24.3%) smoked, and 12% were alcohol abusers. Each forth patient had severe disturbance of nutrition. Disturbances of nutrition of medium degree were detected twice more often. Way of life was sedentary in 63% of women and 46% of men. Abdominal obesity was detected in 46% of men and 72.6% of women. Prevalence of hypercholesterolemia was 96.7% among men and 90.7% among women. Every fifth patient with IHD had diabetes, in 10% of patients diabetes was newly detected. Quality of life changed in every second IHD patient with angina because of limitation in carrying out everyday work, movement and presence of chest discomfort, this was associated with presence of anxiety/depression.Main priority of secondary prevention in the Republic Ingushetia is adequate correction of hypercholesterolemia and arterial hypertension. However every forth patient requires complex therapy of 3-4 risk factors (hypercholesterolemia, hypertension, and/or diabetes, and/or smoking, and/or alcohol abuse).

Twenty-four hour blood pressure monitoring in group of patients of elderly and middle age with hypertension has been carried out. Features of daily average parameters have been studied. The hypertension in elderly patients shows an increase in variability of blood pressure and insufficient decrease in blood pressure at night that leads to development of cardiovascular complications.

The comparative analysis of accompanying diseases depending on the form of hypertension in elderly has been carried out. Prevalence of the isolated systolic or systolo-diastolic essential hypertension in elderly has been studied. The analysis of data has shown that systole-diastolic hypertension in patients is more often accompanied with diseases of heart, and in patients with isolated systolic hypertension is more often accompanied with cerebrovascular diseases. Both forms of a hypertension in elderly persons are adverse by the occurrence of cardiovascular and cerebral complications.

С целью разработки алгоритма интегральной оценки общественного здоровья, основанного на результатах обследования представительных выборок в 2009-2011гг. в разных регионах России в организованных популяциях: Нижний Новгород, Великий Новгород, Вологда, Омск и Нальчик (2227 человек) с использованием специально разработанных вопросников, данных диспансеризации и простейших антропометрических (в том числе индекс массы тела - ИМТ) и функциональных методов, проводится проспективное (через 3 года) популяционное исследование.Методы. Методами для оценки индивидуального и общественного здоровья служили опросники на выявление социально-экономического статуса, соматического и психологического состояния здоровья (индекс жизненного стиля, социальной адаптации, саногенной рефлексии, шкала психотизма Г.Айзенка и нравственный потенциал развития личности, синдром эмоционального выгорания, госпитальная шкала тревоги и депрессии, международная шкала на выявление стресса - PSS). Комплексно эти методы были впервые нами применены для популяционных научных исследований. Математическая, статистическая обработка полученных данных осуществлялась с использованием статистических пакетов SAS.Результаты. В 2012г. был проведен скрининг в городах Нижний Новгород и Нальчик. Скрининг одной и той же организованной популяции проведен у 831 человека (359 мужчин и 472 женщины). Отклик (охват обследованием) составил в Нижнем Новгороде 96,2%, а в Нальчике - 78,2%. Такой отклик является вполне репрезентативным, однако в Нальчике для усиления эффекта значимости исследуемых параметров в 2013 г. будет сделана попытка дополнительно дообследовать 110 человек. Кроме того, в 2013 г. буду проскринированы по 500 человек в Великом Новгороде и Омске. Конечные точки (новые случаи всех заболеваний и смертей по расширенным критериям) составили 13%. Мы проанализировали частную корреляцию по Спирмену, где изучали корреляционную связь новых конечных точек в зависимости от двух типов переменных - физиологических и психологических параметров. Оказалось, даже анализируя данные на 831 человека (первично были обследованы 2227 человек), выявлено, что психологические признаки сильно опережают по значимости влияния на состояние здоровья популяции физиологические параметры ( р< 0,0078 против р

По определению ВОЗ - «здоровье это единство физического, психического и духовного состояний человека». Нарушение любого из указанных компонентов - свидетельство «нездоровья», индивидуального и/или общественного. С целью разработки алгоритма интегральной оценки общественного здоровья нами были обследованы репрезентативные выборки в 5 регионах России. В 2010 году в этих регионах был проведен скрининг на полноценных выборках (всего обследовано 1552 человека, 919 женщин и 633 мужчины). Методами для оценки здоровья служили опросники на выявление социально-экономического статуса, соматического и психологического состояния здоровья (индекс жизненного стиля, социальной адаптации, саногенной рефлексии, шкала психотизма Айзинка и нравственный потенциал развития личности, синдром эмоционального выгорания). Математическая, статистическая обработка полученных данных осуществлялась с использованием статистических пакетов SAS. Результаты. Все обследованные были опрошены в отношении уровня образования, уровня благосостояния (самооценка). Лица с высшим образованием составляли от 82% в Нальчике (работники К-Б Госуниверситета) до 29, 3 в г. Омске (работники одного из заводов). Самооценка уровня благосостояния оказалась низкой у значительной части обследованных: от 10, 4 и 13% (мужчины в Омске и Нальчике) до 40,4% (женщины в В. Новгороде). Еще один социальный фактор риска - это отсутствие брачного союза. Сюда относятся как многие молодые респонденты, так и разведенные и вдовцы среди лиц старших возрастов. Таких оказалось от 24 до 44%. Среди лиц без высшего образования низкая самооценка своего благосостояния была у 20% мужчин и у 37,7% женщин. Но среди лиц с высшим образованием (в/о) 13,2% мужчин и 18,7% женщин оценивали уровень своего благосостояния как низкий. Высоко оценили уровень благосостояния примерно каждый пятый мужчина, при любом уровне образования, а у женщин эта оценка различалась: 30,8% при в/о и лишь 19,6% - без в/о. Ведущий показатель нездоровья конкретной популяции - распространенность общепризнанных факторов риска (ФР) хронических неинфекционных заболеваний (ХНИЗ). Артериальная гипертония, диагностируемая по расширенным критериям (АД >140/90 мм рт.ст. и/или антигипертензивная терапия), была выявлена - от 1,29 и 1,64% у мужчин и женщин в самой молодой группе 25-34 г. до 60 и 75,8% в самой старшей группе. Уровни систолического АД оказались у лиц с высшим образованием (в/о) как у мужчин, так и у женщин в среднем несколько ниже (126,65±0,87 и 124,91±0,64 мм рт.ст.) по сравнению с лицами без в/о (127,42±0,89 и 128,00±0,91 мм рт.ст.). Столь заметных различий для диастолического АД мы не наблюдали. Ожирение (ИМТ > 30) выявлено более чем у половины обследованных во всех социальных группах. Ценными были данные по шкалам синдрома эмоционального выгорания (СЭВ). Низкие значения [0-1] СЭВ (характерно для эмоционально здоровой части популяции) отмечены у 47.6 и 58,7% наиболее молодых (25-34 года) мужчин и женщин. Плутчика выявлено, что шкала «защитное отрицание» свойственна 63.57±4.91% мужчин и 56.55±1.71% женщин, что говорит об их не всегда адекватной реакции, завышенной самооценке. Шкала «защитной проекции» в негативном смысле выявлена у 76.69±1.75% мужчин и у 75.09±1.52 женщин. Эти лица склонны обвинять других для облегчения внутреннего состояния и снятия напряжения. По шкале «защитное замещение» видно, что "внутренняя агрессия" выражена у 45.01± 4.93 мужчин и у 40.46±1.74 женщин. По шкале «защитное подавление», которое приводит к скрытой агрессии или равнодушию, выявлено 44.0±3.24% респондентов. У женщин данные по указанной шкале выглядят хуже (до 73% в старшей возрастной группе). По шкале «защитной регрессии» видно, что у мужчин проявление «инфантильного» поведения выявлено в 55,9±9,5% и 63,9%±10,3 случаев, а у женщин - в 47,3±1,8%. Была проведена оценка теста на социальную адаптацию (поведение в конфликтной ситуации). Мужчины во всех возрастных группах склонны к проявлению агрессивного стиля поведения в сложных ситуациях: от 74.78±5.37% в возрасте 35-44 г. до 90,2±2,8% в возрасте 45-54 г. У женщин это- 92,4%±2,2 в возрасте 35-64 г. Анализ психотизма по Х. Ю. Айзинку также показал, очень высокую склонность к вербальной агрессии (89,9%). Таким образом, при психологическом анализе выявлено высокое напряжение механизмов психологических защит, таких как подавление, проекция, замещение; у респондентов слабо сформирован навык разрешать конфликтную ситуацию с учетом личной модели поведения в конфликте, наблюдается нереализованный нравственный потенциал, замечена тенденция к личной безопасности и эгоцентризму. Для реального оздоровления населения требуется разработка мер, могущих улучшить психологический климат, в первую очередь, среди работающего населения.

The study using multifactor personality questionnaire (shorter variant of MMPI) covered 619 drivers aged 20 to 59. Professional success was assessed according to occupationally important psychophysiologic parameters (OIPP) and number of committed traffic accidents (TA) in 4 groups of the drivers. Individual psychologic traits appeared to correlate with reliability criteria (OIPP and TA). Approaches evaluating the drivers' reliability should therefore consider personal features for better differentiated correction of psychophysiologic parameters. Advances in programs of traffic safety is strongly associated with complex approaches of prophylaxis that should consider significance of psychologic factor.

Conducting a program on prophylaxis of arterial hypertension among automobile transport workers, the researchers revealed higher blood pressure among 22.6% of the examines. Prophylactic measures during 5 years induced decrease in prevalence of arterial hypertension from 23.6% to 13.7% among the exposed group members, whereas the reference group members demonstrated increase of the parameter from 21.5% to 34.3%. Prophylaxis of arterial hypertension among drivers should be based on non-pharmacologic methods, if inevitable, the drug therapy should be selected under control of psychophysiologic tests.

Epidemiologic, electrophysiologic and psychophysiologic studies among urban and rural automobile drivers helped to reveal occurrence of coronary artery disease (CAD), arterial hypertension (AH) and their risk factors. In addition, special studies (monitoring of ECG in natural conditions) among 118 drivers diagnosed the signs of coronary failure and arrhythmias. Psychophysiologic studies among the individuals suffering from cardiovascular diseases (CAD, AH and arrhythmias) proved worse parameters important for occupational activities and for traffic safety.

An epidemiological examination of 4323 motor transport workers made according to the program of the All-Union Cardiology Research Center, USSR AMS, revealed high arterial pressure (AP) in 22.6% of cases. The knowledge of hypertension was found to be low (34%) as was the coverage with hypotensive treatment (7.1%) and the efficacy of the latter one (1.8%). The follow-up studies (for 5 years) demonstrated the efficacy of non-medicamentous and medicamentous AP control exercised in motor transport workers. It was proved that in the principle AP might adequately be decreased and maintained for a long time at the safe level in the overwhelming majority of subjects with AP.

Cross-sectional studies covered 2562 males (a urban population), 580 miners, 1126 agricultural drivers, 682 rural habitants and 4323 urban drivers. The mean blood pressures were basically identical in males of various occupations and age, except for urban and rural drivers aged 20-29 and 40-49 years who had significantly lower mean systolic blood pressures than those from the other groups under study. All the males showed elevation in systolic and diastolic blood pressures with age; the differences in blood pressures were insignificant only in rural drivers aged 30-39 and 40-49 years and miners aged 40-49 and 50-59 years. The incidence of arterial hypertension increased with age: from 3.2 to 7.7% in 20-29-year-old males to 34.2-37.2% in 50-59-year olds. Low (less that 50%) awareness of hypertension was identified in hypertensives. A prospective 5-year follow-up and implementation of prophylactic measures at the urban motor enterprises positively affected the incidence of hypertension and the level of hypertension awareness, and morbidity rates.

The difficulties in hypotensive treatment of arterial hypertension (AH) in drivers encouraged the search for effective methods of correction of elevated arterial pressure (AP). During 5 years a group of 631 drivers with different levels of elevated AP was examined. For their treatment a complex of non-pharmacological methods was proposed, i.e., exercise therapy, preshift gymnastics, psychoemotional relief, autotraining, punctate massage, electric sleep, balneotherapy, decoction of sedative and plant diuretics. The dynamics of mean indices of diastolic pressure and high efficiency of using non-pharmacological methods for treating AH in drivers were demonstrated. The absence of adverse and undesirable effects of non-pharmacological AH-control methods enabled one to regard them as an effective means of treatment of the drivers with the initial AH stages.
