BackgroundIn contrast with the setting of acute myocardial infarction, there are limited data regarding the impact of diabetes mellitus on clinical outcomes in contemporary cohorts of patients with chronic coronary syndromes. We aimed to investigate the prevalence and prognostic impact of diabetes according to geographical regions and ethnicity.Methods and resultsCLARIFY is an observational registry of patients with chronic coronary syndromes, enrolled across 45 countries in Europe, Asia, America, Middle East, Australia, and Africa in 2009–2010, and followed up yearly for 5 years. Chronic coronary syndromes were defined by ≥1 of the following criteria: prior myocardial infarction, evidence of coronary stenosis >50%, proven symptomatic myocardial ischaemia, or prior revascularization procedure.Among 32 694 patients, 9502 (29%) had diabetes, with a regional prevalence ranging from below 20% in Northern Europe to ∼60% in the Gulf countries. In a multivariable-adjusted Cox proportional hazards model, diabetes was associated with increased risks for the primary outcome (cardiovascular death, myocardial infarction, or stroke) with an adjusted hazard ratio of 1.28 (95% confidence interval 1.18, 1.39) and for all secondary outcomes (all-cause and cardiovascular mortality, myocardial infarction, stroke, heart failure, and coronary revascularization). Differences on outcomes according to geography and ethnicity were modest.ConclusionIn patients with chronic coronary syndromes, diabetes is independently associated with mortality and cardiovascular events, including heart failure, which is not accounted by demographics, prior medical history, left ventricular ejection fraction, or use of secondary prevention medication. This is observed across multiple geographic regions and ethnicities, despite marked disparities in the prevalence of diabetes.ClinicalTrials identifierISRCTN43070564

BACKGROUND: Few data are available regarding the use of antithrombotic strategies in coronary artery disease patients with atrial fibrillation (AF) in everyday practice. We sought to describe the prevalence of AF and its antithrombotic management in a contemporary population of patients with stable coronary artery disease.METHODS AND FINDINGS: CLARIFY is an international, prospective, longitudinal registry of outpatients with stable coronary artery disease, defined as prior (≥12 months) myocardial infarction, revascularization procedure, coronary stenosis >50%, or chest pain associated with evidence of myocardial ischemia. Overall, 33,428 patients were screened, of whom 32,954 had data available for analysis at baseline; of these 2,229 (6.7%) had a history of AF. Median (interquartile range) CHA2DS2-VASc score was 4 (3, 5). Oral anticoagulation alone was used in 25.7%, antiplatelet therapy alone in 52.8% (single 41.8%, dual 11.0%), and both in 21.5%. OAC use was independently associated with permanent AF (pCONCLUSIONS: In this contemporary cohort of patients with stable coronary artery disease and AF, most of whom are theoretical candidates for anticoagulation, oral anticoagulants were used in only 47.2%. Half of the patients received antiplatelet therapy alone and one-fifth received both antiplatelets and oral anticoagulants. ACC NIM; International audience; SILHI (High Intensity Light Ion Source) is an ECR ion source producing high intensity proton ordeuteron beams at 95 keV. It is now installed in the IPHI site building, on the CEA/Saclay center. IPHI is a frontend demonstrator of high power accelerator. The source regularly delivers more than 130 mA protons in CWmode and already produced more than 170 mA deuterons in pulsed mode at nominal energy. The last beamcharacterisations, including emittance measurements, space charge compensation analysis and diagnosticimprovements, will be reported. Taking into account the SILHI experience, new developments are in progress tobuild and test a 5 mA deuteron source working in CW mode. This new source will also operate at 2.45 GHz andpermanent magnets will provide the magnetic configuration. This source, of which the design will be discussed,will have to fit in with the SPIRAL 2 accelerator developed at GANIL to produce Radioactive Ion Beams. Spontaneous echo contrast in the left atrium in patients with sinus rhythm is unusual. The aim of this study was to assess the mechanism of this phenomenon and any clinical implications. We studied 90 consecutive patients using transesophageal echocardiography. Spontaneous echo contrast was seen within the left atrium in 29 patients, 11 of whom were in sinus rhythm. In patients with spontaneous contrast and sinus rhythm, the underlying diseases were: mitral insufficiency (5), mitral stenosis (1), mitral prostheses (2), congestive cardiomyopathy (1), hypertrophic cardiomyopathy (1), and heart transplantation (1). Spontaneous echo contrast tended to be faint in this group (10/11). The left atrium was enlarged in all, with subjective poor atrial contraction. Furthermore, reduced mitral A wave suggested an impairment of left atrial contractility. Holter monitoring demonstrated paroxysmal atrial fibrillation in five subjects. Two patients had a history of transient ischemic attacks. No atrial thrombus was observed in any patient. The demonstration of spontaneous echo contrast with transesophageal echocardiography does not seem uncommon even in patients in sinus rhythm. This may indicate an increased risk of thromboembolism.

The originality of this case lies in the patient's age, the mode of onset and the biochemical features (i.e. normal mitochondrial enzymatic complexes but very low respiration when using glutamate as a substrate).

The effects of 6 months treatment with Sotalol on the blood pressure, left ventricular mass and function, were studied in patients with essential hypertension and left ventricular hypertrophy. Thirty-three patients (18 men and 15 women aged 53 +/- 11 years) were included initially and 26 were reviewed after 6 months of treatment. The left ventricular mass and function were evaluated by Doppler echocardiography and all recordings were interpreted "blind" at the end of the study by two operators. Treatment led to a significant reduction of the blood pressure (152 +/- 12/94 +/- 11 versus 166 +/- 18/100 +/- 9 mmHg) and of the heart rate (60 +/- 10 versus 76 +/- 12 beats per minute). The left ventricular mass index decreased by 8% (p less than 0.001) due to reduction in wall thickness. Resting left ventricular systolic function was unchanged. Left ventricular filling patterns improved with an increase in the E/A ratio which was reduced at the beginning of the trial.

The authors report the value of Doppler color flow mapping in the diagnosis of late diastolic mitral regurgitation in two patients with severe post-endocarditis aortic regurgitation requiring rapid surgical intervention. Doppler color flow mapping played an essential part in the management of these cases by helping in the diagnosis of late diastolic mitral regurgitation which is known to carry a very poor prognosis in this context.

The diagnosis of thrombosis of a valvular prosthesis is difficult especially in cases of partial thrombosis. The authors report 19 cases of partial thrombosis of mainly (16 cases) mechanical valve prostheses confirmed by radioscopy and treated either by reoperation or fibrinolysis. Mitral Valve Prostheses: 14 cases (6 Björk, 7 SJM, 1 Carbomedics). All but one case showed significant prolongation of the pressure half time compared with the postoperative reference value. In one case, however, the functional parameters of the prosthesis were normal; the diagnosis was made from the finding of a mobile thrombus on the ventricular surface of the prosthesis. Aortic Valve Prostheses: 5 cases (3 Björk, 1 SJM, 1 Duromedics). Continuous wave Doppler showed significant increases in the velocity of transprosthetic blood flow with respect to the postoperative reference values. Accurate analysis of the mobile elements of the prostheses was usually impossible. These results show that transthoracic Doppler echocardiography remains an excellent method of study and surveillance of mechanical valve prostheses but the limitations of the technique should be familiar to all operators.

The diagnosis of partial thrombosis of mitral disc valve prostheses is particularly difficult because the clinical presentation is often atypical and the motion of the two hemi-discs is hard to analyse by transthoracic echocardiography. The authors report three cases of partial thrombosis of mitral double-disc valve prostheses. It was not possible to analyse the motion of the two hemi-disc correctly in any of the three patients. The mitral transprosthetic flow recorded by continuous wave Doppler was abnormal with respect to the reference postoperative recording. In addition, in one case, an abnormal echo was observed floating in the left ventricular inflow tract and was suggestive of a thrombus. Transesophageal echo showed abnormal motion of one of the hemi-discs. In two patients this consisted in immobility of the hemi-disc in the semi-closed position; in the other patient, limited excursion of the hemi-disc was recorded. Abnormal retrograde flow in the left atrium was observed in 2 cases and abnormal echoes on the atrial surface of the valve were recorded in one case. After fibrinolysis the transthoracic and transesophageal Doppler echocardiographic parameters returned to normal. These three cases underline the complementarity of transthoracic and transesophageal echocardiography in the diagnosis of dysfunction of mitral disc valve prostheses.

International audience; In France, the Spiral 2 project dedicated to radioactive beam production is based on a 40 MeV continuous wave (cw) deuteron Linac. This installation will allow extending the variety of accelerated particles to very heavy elements. Such beams will open research domains for the GANIL facility. To inject the requested 5 mA deuteron beam into the Spiral 2 Linac, the performance of the high intensity light ion source (SILHI) allowed us to propose such an electron cyclotron resonance source. SILHI, developed at CEA/Saclay, regularly produces high intensity (over 100 mA) proton or deuteron beams through a $\Phi$ 9 mm aperture. So for this project, the main modifications of the source design concern the permanent magnets which provide the axial magnetic configuration and the $\Phi$ 3 mm aperture. The source produced its first beam (proton) in 2004. This article will report the beam characterization while the source produced $D^+$ beam with intensity as high as 7.0 mA. Recently, this permanent magnet source has been equipped with the $\Phi$ 9 mm plasma electrode. So high intensity proton beams reaching more than 100 mA are now extracted from this source.

Production of radioactive ions has started at GANIL on the SPIRAL facility since 2001 and numerous multicharged radioactive ion beams have been delivered for high energy nuclear experiments. This article makes an overview of the different beams that have been produced. In the mean time, an important R and D research program is continued in oder to produce new species of radioactive elements. A new concept of multicharged radioactive production that couples a monocharged ion source, based on the monolithe concept, to an ecr ion source like nanogan3 is under developments and is described The development of monocharged ion sources with high efficiencies is also motivated by a new big project that is under studies at GANIL : the SPIRAL 2 Project. The goal of this project consists in extending the disponible radioactive ion beams to very heavy elements created with a new method of production : while the spiral 1 facility uses the projectile fragmentation for radioactive nuclei production, the spiral 2 project is based on the fission of a Uranium carbide target induced by a neutron flow created by a high intensity deuton beam. Minimono is a 2.45 GHz electron cyclotron resonance ion source for singly charged ions which uses only permanent magnets. Measurements of ionization efficiencies, maximum currents extracted, and emittances for H[sup +], [sup 3,4]He[sup +], N[sup +], N[sup e]+, Ar[sup +], Kr[sup +], S[sup +], and Si[sup +] were carried out. In the case of buckminster fullerenes, C[sup +, sub 60], C[sup 2+, sub 60], and C[sup 3+, sub 60] ions were extracted. The results obtained, the general mechanical simplicity of this ion source, and its low cost make this source attractive for the production of stable and radioactive ions. [ABSTRACT FROM AUTHOR]

The present system, based on a rectangular-to-coaxial transition equipped with a tuning piston, has been replaced by a direct rectangular-to-circular wave guide to feed the plasma of the source. In France, the Spiral 2 project dedicated to radioactive beam production is based on a 40 MeV continuous wave (cw) deuteron Linac. This installation will allow extending the variety of accelerated particles to very heavy elements. Such beams will open research domains for the GANIL facility. To inject the requested 5 mA deuteron beam into the Spiral 2 Linac, the performance of the high intensity light ion source (SILHI) allowed us to propose such an electron cyclotron resonance source. SILHI, developed at CEA/Saclay, regularly produces high intensity (over 100 mA) proton or deuteron beams through a [uppercase_phi_synonym] 9 mm aperture. So for this project, the main modifications of the source design concern the permanent magnets which provide the axial magnetic configuration and the [uppercase_phi_synonym] 3 mm aperture. The source produced its first beam (proton) in 2004. This article will report the beam characterization while the source produced D
+ beam with intensity as high as 7.0 mA. Recently, this permanent magnet source has been equipped with the [uppercase_phi_synonym] 9 mm plasma electrode. So high intensity proton beams reaching more than 100 mA are now extracted from this source. [ABSTRACT FROM AUTHOR]- Published

The aim of this study was to evaluate the results of transesophageal echocardiography in the diagnosis of abnormal intraatrial echoes detected by transthoracic echocardiography. Patients with active endocarditis, mitral stenosis, and valve prostheses were excluded. The 47 patients (28 women and 19 men) were classified into 4 groups according to the results of transesophageal echocardiography. Group I: normal (7 cases), "phantom echos"; Group II: anatomical variants (9 cases), Chiari apparatus, muscular spur; Group III: pseudo-tumours (7 cases); retro-atrial haematoma, mitral valve prolapse, interatrial septal aneurysm; Group IV: cardiac masses (24 cases). This group comprises: typical myxomas (10 cases), typical thrombi (2 cases), localised atypical masses, relatively immobile and non-prolapsing: 5 myxomas, 1 metastasis, 2 thrombi. The results of this study suggest that transesophageal echocardiography is very useful in diagnosing suspected abnormal intraatrial echos observed on conventional transthoracic examination. However, the nature of the mass may remain obscure.

- Abstract
The effects of 6 months treatment with Sotalol on the blood pressure, left ventricular mass and function, were studied in patients with essential hypertension and left ventricular hypertrophy. Thirty-three patients (18 men and 15 women aged 53 +/- 11 years) were included initially and 26 were reviewed after 6 months of treatment. The left ventricular mass and function were evaluated by Doppler echocardiography and all recordings were interpreted "blind" at the end of the study by two operators. Treatment led to a significant reduction of the blood pressure (152 +/- 12/94 +/- 11 versus 166 +/- 18/100 +/- 9 mmHg) and of the heart rate (60 +/- 10 versus 76 +/- 12 beats per minute). The left ventricular mass index decreased by 8% (p less than 0.001) due to reduction in wall thickness. Resting left ventricular systolic function was unchanged. Left ventricular filling patterns improved with an increase in the E/A ratio which was reduced at the beginning of the trial.

- Abstract
Unlabelled: To better define the different forms of hypertensive heart disease, we performed 2D guided M mode echo recording of the left ventricle (LV) and Doppler assessment of LV filling in 118 newly recognised (less than 1 year) and untreated hypertensive patients (mean age +/- SD: 49 +/- 10 years, 80 males, 38 females). All recordings were read "blindly". 86 patients underwent also 24 hours BP monitoring during daily routine., Main Results: 1) The relative wall thickness (h/r) shows a normal distribution: mean = 0.39; SD = 0.08; 1st quartile = 0.33; 3rd = 0.43; 2) The septal/posterior wall ratio (ST/PWT) shows also a normal distribution: mean = 1.14; SD = 0.24; 1st quartile = 1; 3rd = 1.3; 3) Among patients with LVH (Devereux's criteria, n = 65) the 6 patients with h/r less than or equal to 0.33 are significantly different from the 18 patients with h/r greater than or equal to 0.43 for age (44 +/- 8 vs 51 +/- 10; p less than 0.03), casual (146 +/- 7 vs 166 +/- 13 mmHg; p less than 0.05) and ambulatory SBP (135 +/- 19 vs 146 +/- 16 mmHg; p less than 0.05), peak systolic stress (280 +/- 15 vs 187 +/- 31 Dynes/cm2; p less than 0.001) and E/A ratio (1.17 +/- 0.01 vs 0.94 +/- 0.27; p less than 0.05) but not for ponderal excess. 4) Patients with asymmetric LVH(ST/PWT greater than 1.3 in 22; greater than 1.5 in 10) are significantly different from patients with symmetric LVH for age (54 +/- 10 vs 47 +/- 10; p less than 0.03), diastolic diameter (51 +/- 5 vs 53 +/- 5 mm; p less than 0.02), ST (13.2 +/- 2.9 vs 10 +/- 1.9 mm; p less than 0.001) and PWT (8.9 +/- 1.5 vs 9.7 +/- 1.5 mm; p less than 0.02) but not for BP fractional shortening and LV filling., Conclusion: eccentric LVH may be found in young hypertensive patients with normal systolic function and LV filling despite high parietal stress; asymmetric LVH is more frequent in older patients in which LVH seems to develop rather on the septum than on free wall, independently of BP and without consequences on LV function.

- Abstract
The authors report the value of Doppler color flow mapping in the diagnosis of late diastolic mitral regurgitation in two patients with severe post-endocarditis aortic regurgitation requiring rapid surgical intervention. Doppler color flow mapping played an essential part in the management of these cases by helping in the diagnosis of late diastolic mitral regurgitation which is known to carry a very poor prognosis in this context.
