هدفت هذه الدراسة إلى التعرف على مفهوم الدبلوماسية الوقائية ضمن إطار ميثاق الأمم المتحدة، وعرض لأهم تطبيقاتها، والصعوبات التي تعترضها وصولاً إلى تحديد الأسباب التي تؤدي إلى فشل هذه الوسيلة في الكثير من الحالات، وفي سبيل ذلك قامت الباحثة في جمع المعلومات المتعلقة بالدبلوماسية الوقائية كإحدى الوسائل السلمية لحل النزاعات الداخلية منها والدولية، مع اللجوء إلى تحليل الأسباب والصعوبات التي تواجه الأمم المتحدة في تطبيق هذه الوسيلة بغية التوصل إلى مدى فعاليتها في تحقيق السلم والأمن الدوليين، وقد توصلت الدراسة إلى مجموعة من النتائج أهمها أن التطبيق الفعال للدبلوماسية الوقائية يتطلب معرفة دقيقة للحقائق والأسباب التي تؤدي إلى حدوث التوترات والصراعات، بالإضافة إلى أن هنالك مجموعة من الصعوبات التي تعترض تطبيق الدبلوماسية الوقائية بشكل فعال، على الرغم من التحسينات في بعض الأزمات وتخفيف التوترات، إلا أن هناك حاجة للمزيد من الجهود لتحقيق هدف الدبلوماسية الوقائية بشكل أكبر وأكثر فعالية.

Abstract Immune checkpoint inhibitor treatment has the potential to prolong survival in non-small cell lung cancer (NSCLC), however, some of the patients develop resistance following initial response. Here, we analyze the immune phenotype of matching tumor samples from a cohort of NSCLC patients showing good initial response to immune checkpoint inhibitors, followed by acquired resistance at later time points. By using imaging mass cytometry and whole exome and RNA sequencing, we detect two patterns of resistance¨: One group of patients is characterized by reduced numbers of tumor-infiltrating CD8+ T cells and reduced expression of PD-L1 after development of resistance, whereas the other group shows high CD8+ T cell infiltration and high expression of PD-L1 in addition to markedly elevated expression of other immune-inhibitory molecules. In two cases, we detect downregulation of type I and II IFN pathways following progression to resistance, which could lead to an impaired anti-tumor immune response. This study thus captures the development of immune checkpoint inhibitor resistance as it progresses and deepens our mechanistic understanding of immunotherapy response in NSCLC.

العقيدة العسكرية لروسيا الاتحادية هي واحدة من وثائق التخطيط الاستراتيجي الأساسي في روسيا الاتحادية وتشكل نظاماً من الآراء ووجهات النظر المعتمدة رسمياً من قبل الدولة في التحضير للحماية والدفاع المسلح عن روسيا الاتحادية. يهدف هذا البحث إلى دراسة الخطوط العامة لوثائق العقيدة العسكرية الروسية في مرحلة ما بعد الحرب الباردة، والتعرف على أهم تطوراتها، إضافة إلى المقارنة بين هذه الوثائق لمعرفة التغيير الحاصل في مضمونها وتشخيص طبيعة التهديدات العسكرية الفعلية والمحتملة وطبيعة الحرب المستقبلية التي يمكن أن تنخرط فيها روسيا الاتحادية. توصل البحث إلى مجموعة من النتائج أهمها: عبرت وثائق العقيدة العسكرية الروسية المتعددة، منذ انهيار الاتحاد السوفييتي، عن واقع الحكم في روسيا، وعن موقع هذه الدولة في النظام الدولي، وعن نظرتها إلى نفسها وإلى مستقبلها، وإلى حركة الدول الأخرى التي تتنافس معها في الساحة الدولية. من خلال مقارنة وثائق العقيدة العسكرية الروسية الصادرة منذ عام 2000 وصولاً إلى وثيقة 2021، يتضح أن هناك تحولاً في الفكر العسكري لروسيا وهذا التحول تزامن مع تغير النخب الحاكمة منذ مطلع القرن الحادي والعشرين بوصول نخب حاكمة جديدة لها رؤى وتطلعات تقوم على ضرورة استعادة روسيا لمكانتها العالمية. تباينت كل عقيدة عسكرية عن الأخرى من ناحية المضمون، من خلال تحديد مصادر الخطر على الدولة، وأشكال هذه المخاطر والتهديدات، كما من ناحية روحية الاستراتيجية، وما يظهر فيها من مستوى ثقة الدولة بنفسها وبقدرتها على خدمة الأهداف المرسومة في التخطيط الاستراتيجي للأمن القومي. إن القاسم المشترك بين وثائق العقيدة العسكرية الروسية التي تطبقها روسيا منذ 2009 وحتى الآن هو شعورها المتزايد بالخطر الوجودي من توسعة واقتراب حلف الناتو من الأراضي الروسية.

Objective: The purpose of the study was to understand how people with multiple sclerosis experience dual-tasking situations in their everyday lives. Methods: Focus groups involving a total of 11 individuals with multiple sclerosis (eight females and three males) participated in this qualitative inquiry. Participants were asked open-ended questions focused on the nature of and consequences around dual tasking when standing or walking. Reflexive thematic analysis was employed to examine the data. Results: Three themes were generated from the data: (a) Life Is a Dual Task, (b) The Social Divide, and (c) Sacrifices for Stability. Conclusions: This study highlights the significance and impact of dual tasking on the lived experience of adults with multiple sclerosis, furthering the need to more fully examine this phenomenon and potentially improve fall-prevention interventions and facilitate community participation. إن جريمة القرصنة البحرية هي جريمة تهدد البشرية جمعاء, حيث أن البعض يعتقد أنها أسطورة من أساطير الماضي, إلا أن واقعها يثبت عكس ذلك من خلال ارتفاع عمليات القرصنة في الآونة الأخيرة, مما يجعلها من أبرز وأهم مواضيع الساعة ومن أبشع الجرائم التي تهدد سلامة الملاحة البحرية والتجارة الدولية وكذلك حياة الأشخاص والسفن والممتلكات في العديد من دول العالم. حيث أن هذا التهديد الذي يزداد يوماً بعد يوم يستدعي اهتمام القانون الدولي والفقه الدولي من أجل محاربة وقمع هذه الظاهرة وايجاد حلول لمكافحتها.

تهدف هذه الدراسة إلى التعرف على الأسس التي يبني على أساسها مجلس الأمن قراراته والتعمق فيها في حالة حدوث تهديد للسلم والأمن الدوليين، إضافة إلى تقييم الجزاءات الاقتصادية الدولية التي يقرها مجلس الأمن و التدابير الفردية التي تفرضها إحدى الدول الكبرى ، وفي سبيل ذلك فقد قامت الباحثة في جمع المعلومات الخاصة حول مجلس الأمن منذ تأسيس منظمة الأمم المتحدة عقب انهيار عصبة الأمم المتحدة مع اللجوء إلى تحليل الأساس القانوني الذي ارتكز عليه توقيع الجزاءات الاقتصادية الدولية التي تم النص عليها في الفصل السابع من ميثاق الأمم المتحدة للتوصل إلى مدى استقلال الجزاءات الاقتصادية الدولية التي تطبق في حق الدول المخلة لأحكام وقواعد القانون الدولي. وقد توصلت الدراسة إلى مجموعة من النتائج اهمها: إن قرارات مجلس الأمن على الرغم من أن ميثاق الأمم المتحدة قد خوله صلاحيات واسعة في مهمته الأساسية المتمثلة في المحافظة على السلم والأمن الدوليين، قد يشوبها الالتباس والتحيز لصالح الدول الكبرى وفي مقدمتها الولايات المتحدة الأمريكية.

Following the declaration of the COVID-19 pandemic, the restrictions imposed by the Member States (MSs) of the European Union (EU) led the tourism and commercial air transport industry to face serious financial difficulties that required airlines to apply for state support. In March 2020, the European Commission adopted a State Aid Temporary Framework (TF) for the COVID-19 period to simplify the process of granting aid, allowing the MSs rapid economic interventions. Claims regarding the approval of State aid to certain airlines reached the EU General Court (EGC), on the grounds of violation of the fair competition principle. It became clear that the processes enabled by the TF dispensed unequal treatment to airline companies and that the flexibility of the TF created competition imbalances. In spite of these events, little attention has been given to the way its application ignores the requirements of EU State aid control and other procedural justice and fairness principles, by failing to ensure neutrality and equality. This article addresses the matter using a standard legal interpretive approach to explain the imbalances created by the regulation. It presents a systematic review of the current regime, identifies the TF inefficiencies affecting competition principles, and argues for specific adjustments that could enhance its transparency. These contributions are useful to improve the existing framework and help prepare for a better management of future crises.

The aim of the present research was to examine the influence of situational priming of materialistic social models on adolescents' materialistic and non-materialistic life aspirations. Three experimental studies were conducted on students aged 13-16 years. Each study used a different means of cuing materialism (scrambled sentences, questions concerning events, and images with materialistic themes) and tested the effects of four distinct social models (mother, father, peer, and media). Life aspirations were measured with the Aspiration Index. The results indicated that activation of materialistic social models increases the importance that adolescents place on financial success (Studies 1, 2, 3) and image (Study 3) aspirations, but does not generally affect popularity or non-materialistic aspirations (Studies 1, 2, 3). Effects were typically of the same magnitude across the four social models examined. RationaleThe sciatic nerve runs a predictable course combining L4-S3 nerve roots through the true pelvis and under the greater sciatic notch. There are reports of bony protuberances from the sacrum and ilium in cases of spinal dysraphism; however advanced imaging, treatment, or outcomes are not described. There are no cases with associated fibular hemimelia in the current literature.Patient concernsThis is a 4-year-old girl with tethered cord, acetabular dysplasia with hip subluxation, congenital short femur, anterior cruciate ligament (ACL) deficiency, and fibular hemimelia with her sciatic nerve coursing through the ilium.DiagnosisAberrant course of the sciatic nerve through the ilium in the setting of spinal dysraphism.OutcomesThe hip subluxation was treated with a femoral varus derotation osteotomy and Salter osteotomy with transposition of the sciatic nerve into the greater sciatic notch resulting in a stable hip with no sciatic nerve symptoms at last follow-up.LessonsThe combination of spinal dysraphism with acetabular dysplasia should be a warning for anomalous sciatic nerveanatomy, possibly through the ilium. Preoperative imaging (MRI, CT scan) may be obtained and carefully reviewed for the course of the sciatic nerve prior to pelvic or femoral osteotomy. Decompressing the sciatic nerve from the aberrant foramen may be considered as part of the procedure.

The field of medicine is undergoing a fundamental change, transforming towards a modern data-driven patient-oriented approach. This paradigm shift also affects perinatal medicine as predictive algorithms and artificial intelligence are applied to enhance and individualize maternal, neonatal and perinatal care. Here, we introduce a pharmacometrics-based mathematical-statistical computer program (PMX-based algorithm) focusing on hyperbilirubinemia, a medical condition affecting half of all newborns. Independent datasets from two different centers consisting of total serum bilirubin measurements were utilized for model development (342 neonates, 1,478 bilirubin measurements) and validation (1,101 neonates, 3,081 bilirubin measurements), respectively. The mathematical-statistical structure of the PMX-based algorithm is a differential equation in the context of non-linear mixed effects modeling, together with Empirical Bayesian Estimation to predict bilirubin kinetics for a new patient. Several clinically relevant prediction scenarios were validated, i.e., prediction up to 24 h based on one bilirubin measurement, and prediction up to 48 h based on two bilirubin measurements. The PMX-based algorithm can be applied in two different clinical scenarios. First, bilirubin kinetics can be predicted up to 24 h based on one single bilirubin measurement with a median relative (absolute) prediction difference of 8.5% (median absolute prediction difference 17.4 μmol/l), and sensitivity and specificity of 95.7 and 96.3%, respectively. Second, bilirubin kinetics can be predicted up to 48 h based on two bilirubin measurements with a median relative (absolute) prediction difference of 9.2% (median absolute prediction difference 21.5 μmol/l), and sensitivity and specificity of 93.0 and 92.1%, respectively. In contrast to currently available nomogram-based static bilirubin stratification, the PMX-based algorithm presented here is a dynamic approach predicting individual bilirubin kinetics up to 48 h, an intelligent, predictive algorithm that can be incorporated in a clinical decision support tool. Such clinical decision support tools have the potential to benefit perinatal medicine facilitating personalized care of mothers and their born and unborn infants.

خلفية البحث: تم الإبلاغ عن زيادة نسبة العدلات على الخلايا اللمفاوية في العديد من الأمراض الالتهابية ولكن أهميتها السريرية في الداء الرثياني لا تزال غير واضحة. الالتهاب هو الآلية الرئيسية المحددة التي تؤدي إلى الإعاقة وزيادة معدل الوفيات في المرضى الذين يعانون من الداء الرثياني، لذلك يعد تقييم الالتهاب في الداء الرثياني عبر مؤشرات موثوقة أمراً بالغ الأهمية للتنبؤ بالنتائج طويلة المدى لمريض معين. هدف البحث: دراسة العلاقة بين نسبة العدلات على اللمفاويات ودرجة نشاط المرض في الداء الرثياني، والمقارنة بين الحالات النشطة والحالات التي تمر بمرحلة هوادة، كذلك دراسة العلاقة بين هذه النسبة وعدد من العوامل المؤثرة مثل: قيمة الخضاب، عدد الصفيحات، سرعة التثفل، الـبروتين الارتكاسي-C، العامل الرثياني والـ ACPA. مواد وطرائق البحث: تم اجراء دراسة مقطعية مستعرضة شملت 71 مريضاً مشخصاً بالداء الرثياني حسب المعايير الدولية لـ ACR/EULAR لعام 2010، من المقبولين في شعبة أو عيادة أمراض الجهاز الحركي في مستشفى تشرين الجامعي في اللاذقية خلال الفترة الممتدة بين تشرين الثاني 2020 وتشرين الثاني 2021م والمحققين لمعايير الاشتمال في البحث. تم اجراء التحاليل المخبرية التالية لهم: تعداد الدم الكامل CBC، CRP، ESR، RF، ACPA وحساب قيمة DAS-28(CRP) ونسبة الـ NLR لدى هؤلاء المرضى. تم تقسيم هؤلاء المرضى إلى 4 مجموعات للدراسة حسب قيمة الـ DAS-28 كالتالي: (هجوع 7 مرضى، فعالية خفيفة 9 مرضى، فعالية متوسطة 36 مريضاً وفعالية شديدة 19 مريضاً). كما شملت عينة البحث على 71 فرداً من الأصحاء المتطابقين بالعمر والجنس، والذين لا تنطبق عليهم معايير الاستبعاد، وادخالهم كمجموعة شواهد حيث تم اجراء التحاليل المخبرية ذاتها ومقارنة النتائج معها. كما تم استخدام منحنيات (ROC Curve) لمعرفة أفضل نقطة قطع Cut-off لنسبة العدلات على اللمفاويات للتنبؤ بفعالية المرض. النتائج: كانت NLR أعلى بشكل هام إحصائياً عند مرضى الداء الرثياني مقارنة مع مجموعة الشواهد. كان هناك ارتباط إيجابي ذو أهمية إحصائية بين NLR وDAS-28 عند مرضى الداء الرثياني. لم نجد أي ارتباط ذو أهمية إحصائية بين NLR والمشعرات التالية: ESR، CRP، RF، ACPA وWBC. المشعر الالتهابي الأكثر حساسية للتنبؤ بزيادة شدة نشاط المرض هو سرعة التثفل، تلاها الـ CRP ثم نسبة العدلات على اللمفاويات (NLR)، كما تم تحديد أفضل نقطة قطع لنسبة العدلات على اللمفاويات عند 2.53 حيث بلغت الحساسية 71.4% والنوعية 62.4%. الخلاصة: نسبة العدلات على اللمفاويات هي مشعر التهابي متوفر وقليل التكلفة، يرتبط ارتباطاً وثيقاً بالمشعرات الالتهابية الأخرى ومؤشرات نشاط المرض في الداء الرثياني. وبالتالي، يمكن أن تكون الـ NLR أداة منفردة مفيدة لتقييم نشاط المرض في الداء الرثياني.

الهدف: تهدف الدراسة لاستقصاء العلاقة بين مشعرات الصفيحات الدموية وتطور شدة المرض شعاعياً وسريرياً عند مرضى الفصال العظمي البدئي في مفصل الركبة. طرق الدراسة: دراسة تحليلية مقطعية عرضية، شملت عينة البحث 83 مريضاً ( 24ذكراً, 59أنثى) من مرضى الفصال العظمي البدئي في مفصل الركبة، تراوحت أعمار مرضى عينة البحث بين 40 الى 60 سنة وتم أخذ قصة مفصلة عن كل مريض، وإجراء فحص سريري شامل للحركات الفاعلة والمنفعلة للمفاصل، مع التقييم السريري تبعاً للمشعر السريري WOMAC، وتمَّ قياس الوزن و طول لكل مريض مع حساب الـ BMI (kg/m2 )، كما تم إجراء تعداد دم كامل (CBC) وتقييم مشعرات الصفيحات الدموية, وسرعة التثفل ESR))، والبروتين C التفاعلي CRP) )، وإجراء صورة شعاعية بسيطة لمفصلي الركبتين أمامية خلفية وجانبية وتحديد درجة كيلغرن-لورانس K-L))، النتائج: أظهرت النتائج أنَّ حوالي ثلثي عينة الدراسة كانت مع (OA) Osteoarthritis خفيف إلى متوسط الشدة حسب مشعر التقييم السريري WOMAC وأيضاً حسب المشعر الشعاعي K-L. لوحظ أيضاً علاقة ذات دلالة احصائية هامة بين BMI ومشعر WOMAC ومشعر K-L حيث كانت أعلى في مجموعة الشديد لكلا المشعرين. بينت الدراسة وجود فروقات ذات دلالة إحصائية في متوسط حجم الصفيحات ((MPV بين مجموعتي المرضى حسب التقييم الشعاعي والتي كانت أعلى في مجموعة الشديد. كما بينت وجود زيادة هامة احصائياً في MPV بين مجموعتي المرضى حسب تقييمهم الشعاعي لـ K-L في مجموعة الشديد تبعاً لاختلاف مجالات قيم مشعر كتلة الجسم. لم يلاحظ أي علاقة ذات دلالة هامة احصائياً بين مشعرات الصفيحات الدموية الأخرى وشدة الفصال العظمي في مفصل الركبة شعاعياً وسريرياً تبعاً لاختلاف مجالات قيم مشعر كتلة الجسم. الخلاصة: يمكن اعتبا قيمة MPV مشعراً مهماً لتطور الفصال العظمي البدئي في مفصل الركبة شعاعياً.

The present study expands on current understanding of dual-task cognitive-motor interference, by including cortical activation measures to both traditional and ecologically valid dual-task paradigms. Fifteen individuals with multiple sclerosis and 14 control participants underwent mobility testing while wearing functional near-infrared spectroscopy. In the absence of increased prefrontal cortical activation, subjects with multiple sclerosis performed significantly worse on measures of cognition under both single- and dual-task conditions. These findings suggest that persons with multiple sclerosis may be unable to allocate additional cortical resources to cognition under dual-task conditions, leading to significant cognitive-motor interference and decrements in performance.
