Abstract

We present a new framework to study the time variation of fundamental constants in a model-independent way. Model independence implies more free parameters than assumed in previous studies. Using data from atomic clocks based on ^87 Sr, ^171 Yb ^+ and ^133 Cs, we set bounds on parameters controlling the variation of the fine-structure constant, α , and the electron-to-proton mass ratio, µ . We consider variations on timescales ranging from a minute to almost a day. In addition, we use our results to derive some of the tightest limits to date on the parameter space of models of ultralight dark matter and axion-like particles.

Abstract

Even partway through an IVF cycle, at the point when a woman’s eggs have been collected, it is hard to provide reliable answers to the common question of ‘Am I likely to have a good embryo to transfer?’ Sometimes, it only takes one good egg to be successful. However, doctors and patients are acutely aware that low egg numbers, older age and having conditions such as endometriosis can stack the odds against success. We have developed a model to try and answer this question for those patients who wish for more information to help guide their expectations after egg collection. A new tool is presented to predict whether a woman having IVF treatment will have a good enough embryo either to transfer on day 5 or freeze. It was built using information from all 2015 to 2016 UK cycles and predicts using age, number of eggs collected and cause of subfertility.

Abstract

The Laser Interferometer Space Antenna Pathfinder (LPF) main observable, labelled∆g, is the differential force per unit mass acting on the two test masses in free fall condition, after the contribution of all non-gravitational forces have been compensated. At low frequencies the differential force is compensated by an applied electrostatic actuation force,which then must be subtracted from the measured acceleration to obtain∆g. Any inaccuracy in the actuation force contaminates the residual acceleration. This study investigates the accuracy of the electrostatic actuation system and its impact on the LPF main observable. It is shown that the inaccuracy is mainly caused by the rounding errors in the waveform processing and also by the random error caused by the analog to digital converter (ADC) random noise in the control loop. Both errors are one order of magnitude smaller than the resolution of the commanded voltages. We developed a simulator based on the LPF design to compute the close-to-reality actuation voltages and consequently, the resulting actuation forces. The simulator is applied in post-processing the LPF data.

Abstract

The global carbon sequestration and avoided emissions potentially achieved via blue carbon is high (∼3% of annual global greenhouse gas emissions); however, it is limited by multidisciplinary and interacting uncertainties spanning the social, governance, financial, and technological dimensions. We compiled a transdisciplinary team of experts to elucidate these challenges and identify a way forward. Key actions to enhance blue carbon as a natural climate solution include improving policy and legal arrangements to ensure equitable sharing of benefits; improving stewardship by incorporating indigenous knowledge and values; clarifying property rights; improving financial approaches and accounting tools to incorporate co-benefits; developing technological solutions for measuring blue carbon sequestration at low cost; and resolving knowledge gaps regarding blue carbon cycles. Implementing these actions and operationalizing blue carbon will achieve measurable changes to atmospheric greenhouse gas concentrations, provide multiple co-benefits, and address national obligations associated with international agreements.

Abstract

STUDY QUESTION Is it possible to develop a patient smartphone application for medically assisted reproduction (MAR) that is acceptable to patients and fertility staff? SUMMARY ANSWER Staff and patients responded positively to the MediEmo smartphone application, perceiving it to be acceptable and feasible to implement in a busy clinic. WHAT IS KNOWN ALREADY Digital tools are increasingly popular to provide practical, administrative and psychological support alongside medical treatments. Apps and other digital tools have been developed for use alongside MAR but there is very limited research on the development or acceptability and feasibility of these tools. STUDY DESIGN, SIZE, DURATION Mixed methods research. This article outlines the development phase of the MediEmo smartphone app, which was guided by the Medical Research Council development framework for complex interventions. The resulting MediEmo app was then implemented into a single centre for MAR in the UK, acceptability evaluated and feasibility explored among 1106 potential participants undertaking IVF cycles. PARTICIPANTS/MATERIALS, SETTING, METHODS Consultation and data collection took part at a single mid-sized urban fertility clinic. Development of the MediEmo smartphone application took place during 2013 to 2017. Implementation of the MediEmo took place from June 2017 to September 2020. The MediEmo app comprises three functions (six features) namely medication management (medication timeline, messaging), mood management (emotional tracking, coping support) and functional support (frequently asked questions, symptom checker). Data on age, fertility diagnosis, anti-Müllerian hormone level were collected about the users of the MediEmo in addition to MediEmo usage data and attitudes towards the MediEmo smartphone application. MAIN RESULTS AND THE ROLE OF CHANCE Informed by the developmental process described, MediEmo is an app combining patient medication diary management and ease of integration into clinic systems with emotional support, emotional tracking and data capture. This study demonstrates acceptability and feasibility of MediEmo, with good uptake (79.8%), mood data sensitivity and reliability and positive feedback. LIMITATIONS, REASONS FOR CAUTION Single centre, small number of users in questionnaire studies. WIDER IMPLICATIONS OF THE FINDINGS The findings suggest smartphone apps can contribute to fertility care and that patient engagement is high. Evaluation of any apps introduced into clinical pathways should be encouraged to promote development of the most useful digital tools for fertility patients. Abstract

In Ontario, bariatric surgery is publicly funded and is performed only in accredited tertiary care hospitals. The purpose of our study was to report on the safety and outcomes of performing bariatric surgery at an ambulatory site of a tertiary care hospital in southern Ontario.We conducted a retrospective cohort study of all adult (age ≥ 18 yr) patients who underwent primary laparoscopic Roux-en-Y gastric bypass (LRYGB) or laparoscopic sleeve gastrectomy (LSG) at the ambulatory site of our tertiary care hospital between September 2016 and August 2018. The 2 sites are 1.4 km apart. Patient demographic characteristics, duration of surgery, intraoperative and 90-day postoperative complications, number of transfers and readmission to the tertiary care hospital, and emergency department visits were collected.A total of 314 patients (285 women [90.8%] and 29 men [9.2%] with a mean age of 41.8 yr [standard deviation (SD) 8.9 yr]) underwent surgery: LRYGB in 295 cases (93.9%) and LSG in 19 (6.0%). The mean body mass index was 45.3 (SD 5.1), the median American Society of Anesthesiologists score was 3 (range 2-4), and the median Edmonton Obesity Staging System score was 2 (range 0-4). The mean operative time was 119.8 (SD 23.1) minutes for LRYGB and 96.2 (SD 22.0) minutes for LSG, and the mean length of stay was 2.1 (SD 0.6) days and 2.1 (SD 0.2) days, respectively. Thirteen patients (4.1%) required transfer to the tertiary care hospital for a postoperative complication. Of 312 patients, 29 (9.3%) presented to emergency department within 90 days after surgery, and 8 (2.6%) required readmission to hospital; no deaths were reported.The findings suggest that LRYGB and LSG can be performed safely at an ambulatory site of a tertiary care hospital. However, caution should be exercised in performing these procedures at an ambulatory site without a tertiary care hospital affiliation, as patients may require urgent transfer for a serious postoperative complication.

Abstract

We review the recent experimental clarification of the fracture path in Liquid Metal Embrittlement with austenitic and martensitic steels. Using state of the art characterization tools (Focused Ion Beam and Transmission Electron Microscopy) a clear understanding of crack path is emerging for these systems where a classical fractographic analysis fails to provide useful information. The main finding is that most of the cracking process takes place at grain boundaries, lath or mechanical twin boundaries while cleavage or plastic flow localization is rarely the observed fracture mode. Based on these experimental insights, we sketch an on-going modeling strategy for LME crack initiation and propagation at mesoscopic scale. At the microstructural scale, crystal plasticity constitutive equations are used to model the plastic deformation in metals and alloys. The microstructure used is either extracted from experimental measurements by 3D-EBSD (Electron Back Scattering Diffraction) or simulated starting from a Voronoï approach. The presence of a crackwithin the polycrystalline aggregate is taken into account in order to study the surrounding plastic dissipation and the crack path. One key piece of information that can be extracted is the typical order of magnitude of the stress-strain state at GB in order to constrain crack initiation models. The challenges of building predictive LME cracking models are outlined.

Abstract

Non-recurrent short-term variations of the galactic cosmic-ray (GCR) flux above 70 MeV n(exp −1) were observed between 2016 February 18 and 2017 July 3 on board the European Space Agency LISA Pathfinder (LPF) mission orbiting around the Lagrange point L1 at 1.5 × 10 (exp 6) km from Earth. The energy dependence of three Forbush decreases is studied and reported here. A comparison of these observations with others carried out in space down to the energy of a few tens of MeV n(exp −1) shows that the same GCR flux parameterization applies to events of different intensity during the main phase. FD observations in L1 with LPF and geomagnetic storm occurrence are also presented. Finally, the characteristics of GCR flux non-recurrent variations (peaks and depressions) of duration <2 days and their association with interplanetary structures are investigated. It is found that, most likely, plasma compression regions between subsequent corotating high-speed streams cause peaks, while heliospheric current sheet crossing causes the majority of the depressions.
