1. Trends in large joint total arthroplasty in patients with rheumatic diseases: an experience of a specialized department of traumatology and orthopedics

Abstract

Total knee and hip arthroplasty (TKA and THA respectively) is frequently used in severe structural changes in patients with osteoarthritis (OA) and rheumatoid arthritis (RA).Objective: to evaluate the dynamics of the number of TKA of THA in patients with OA and RA treated in the Department of Traumatology and Orthopedics of V.A. Nasonova Research Institute of Rheumatology.Material and methods. The analysis of 11-year period (from 2012 to 2022) of surgical activity of the Department of Traumatology and Orthopedics of V.A. Nasonova Research Institute of Rheumatology.Results and discussion. Within the mentioned period, 2955 TKA and THA were performed in 1590 patients with OA and 1365 with RA. From 2012 to 2018, the number of surgeries in patients with OA and RA was approximately the same, but since 2019 there has been a significant increase in the number of total arthroplasties (TA) in OA. The median delta (Δ) of the number of TKA and THA performed annually over 11 years was 13.5 [-17.5; 75.7] in patients with OA and 2.5 [-10.25; 25.0] in patients with RA (p=0.032).Conclusion. In the last 11 years there was no significant dynamics in the number of TKA and THA in patients with RA. At the same time, there is a statistically significant increase in the number of TA in patients with OA, which is in line with the global trend.

Abstract

Avascular necrosis (AN) of bone tissue is a common pathology that affects people of any age, more often young and able-bodied. The disease leads to rapid destruction of the subchondral bone and collapse, followed by the development of secondary osteoarthritis (OA) of the affected joint.The purpose of this review article is to present the accumulated knowledge about the prevalence of AN, the most commonly affected joints, risk factors and pathogenesis of the disease. Since most of the world’s literature sources present knowledge about the individual parts and facts that make up the pathogenesis of AN, this article analyzes all known paths of the development of the disease from the onset of ischemia to collapse and the development of secondary OA and the pathogenesis is presented in chronological order. Based on the results of the article, a definition of the term AN was proposed, and the stages of the disease within the pathogenesis, the most promising for conservative methods of treatment, were identified.

Abstract

The sorbent BAKS-001 on the basis of nanoparticles of carbon and silicon dioxide was synthesized. To increase the sorption properties of the composite, additional components were added to the formulation of the starting material. The sorbent BAKS-001 was obtained by combining an anhydrous precipitate of silica with coal BAU, dehydrated at 150°C and ground at colloid mill, together with silver nanoparticles obtained by treatment of glucose with an ammoniacal solution of silver hydroxide in a solution of starch. The obtained clay mixture was granulated, dried and calcined at 750-800°C. New sorbents of BAKS-002 were obtained by introducing iron sulfate and BAKS-003 into the composition of the base composite by sulfonation with concentrated sulfuric acid. The sorbents were studied by the method of electron microscopy and the stalagmantry method of analysis. The two-step character of isotherms of sorption of butanol due to the presence in the structure of composites of two parts with different degrees of dispersion was established. Abstract

The works of Prof. Speransky school gave new understanding of etiology, pathogenesis, clinic and therapy of pathological processes. These works establish that 1) many and various pathological processes in their origin are entirely due to neural influences and 2) neurotrophic component enters as the main one, in all pathological processes without exception, being from the beginning to the end the factor determining their condition. As a result, new ways of influence on the course of pathological processes are outlined.

Abstract

It is demonstrated that an increasing impact of anthropogenic factors becomes characteristic of the modern sanitary state of water reservoirs in rural areas. Priority indices of water quality in rural water reservoirs are proposed. Proceeding from the data thus obtained there is recommended a set of water-protective measures aimed at raising safety of water management conditions.
