Objective: To update the recommendations of the Spanish Society of Neurology on primary and secondary stroke prevention in patients with arterial hypertension. Development: We proposed several questions to identify practical issues for the management of blood pressure (BP) in stroke prevention, analysing the objectives of blood pressure control, which drugs are most appropriate in primary prevention, when antihypertensive treatment should be started after a stroke, what levels we should aim to achieve, and which drugs are most appropriate in secondary stroke prevention. We conducted a systematic review of the PubMed database and analysed the main clinical trials to address these questions and establish a series of recommendations. Conclusions: In primary stroke prevention, antihypertensive treatment should be started in patients with BP levels > 140/90 mmHg, with a target BP of < 130/80 mmHg. In secondary stroke prevention, we recommend starting antihypertensive treatment after the acute phase (first 24 hours), with a target BP of < 130/80 mmHg. The use of angiotensin-II receptor antagonists or diuretics alone or in combination with angiotensin-converting enzyme inhibitors is preferable.

Abstract: Objective: To update the Spanish Society of Neurology's guidelines for stroke prevention in patients with type 2 diabetes or prediabetes, analysing the available evidence on the effect of metabolic control and the potential benefit of antidiabetic drugs with known vascular benefits in addition to conventional antidiabetic treatments in stroke prevention. Development: PICO-type questions (Patient, Intervention, Comparison, Outcome) were developed to identify practical issues in the management of stroke patients and to establish specific recommendations for each of them. Subsequently, we conducted systematic reviews of the PubMed database and selected those randomised clinical trials evaluating stroke as an independent variable (primary or secondary). Finally, for each of the PICO questions we developed a meta-analysis to support the final recommendations. Conclusions: While there is no evidence that metabolic control reduces the risk of stroke, some families of antidiabetic drugs with vascular benefits have been shown to reduce these effects when added to conventional treatments, both in the field of primary prevention in patients presenting type 2 diabetes and high vascular risk or established atherosclerosis (GLP-1 agonists) and in secondary stroke prevention in patients with type 2 diabetes or prediabetes (pioglitazone).

Objective: To update the Spanish Society of Neurology's guidelines for subarachnoid haemorrhage diagnosis and treatment. Materials and methods: A review and analysis of the existing literature. Recommendations are given based on the level of evidence for each study reviewed. Results: The most common cause of spontaneous subarachnoid haemorrhage (SAH) is cerebral aneurysm rupture. Its estimated incidence in Spain is 9/100, 000 inhabitants/year with a relative frequency of approximately 5% of all strokes. Hypertension and smoking are the main risk factors. Stroke patients require treatment in a specialised centre. Admission to a stroke unit should be considered for SAH patients whose initial clinical condition is good (Grades I or II on the Hunt and Hess scale). We recommend early exclusion of aneurysms from the circulation. The diagnostic study of choice for SAH is brain CT (computed tomography) without contrast. If the test is negative and SAH is still suspected, a lumbar puncture should then be performed. The diagnostic tests recommended in order to determine the source of the haemorrhage are MRI (magnetic resonance imaging) and angiography. Doppler ultrasonography studies are very useful for diagnosing and monitoring vasospasm. Nimodipine is recommended for preventing delayed cerebral ischaemia. Blood pressure treatment and neurovascular intervention may be considered in treating refractory vasospasm. Conclusions: SAH is a severe and complex disease which must be managed in specialised centres by professionals with ample experience in relevant diagnostic and therapeutic processes. Introduction: Update of Acute Ischaemic Stroke Treatment Guidelines of the Spanish Neurological Society based on a critical review of the literature. Recommendations are made based on levels of evidence from published data and studies. Development: Organised systems of care should be implemented to ensure access to the optimal management of all acute stroke patients in stroke units. Standard of care should include treatment of blood pressure (should only be treated if values are over 185/105 mmHg), treatment of hyperglycaemia over 155 mg/dl, and treatment of body temperature with antipyretic drugs if it rises above 37.5 °C. Neurological and systemic complications must be prevented and promptly treated. Decompressive hemicraniectomy should be considered in cases of malignant cerebral oedema. Intravenous thrombolysis with rtPA should be administered within 4.5 hours from symptom onset, except when there are contraindications. Intra-arterial pharmacological thrombolysis can be considered within 6 hours, and mechanical thrombectomy within 8 hours from onset, for anterior circulation strokes, while a wider window of opportunity up to 12–24 hours is feasible for posterior strokes. There is not enough evidence to recommend routine use of the so-called neuroprotective drugs. Anticoagulation should be administered to patients with cerebral vein thrombosis. Rehabilitation should be started as early as possible. Conclusion: Treatment of acute ischaemic stroke includes management of patients in stroke units. Systemic thrombolysis should be considered within 4.5 hours from symptom onset. Intra-arterial approaches with a wider window of opportunity can be an option in certain cases. Protective and restorative therapies are being investigated. Resultados: Se incluyó a 41 pacientes. Abstract: Background and purpose: Endovascular therapies in acute ischaemic stroke may offer benefits to patients that are not eligible for standard use of intravenous tissue activator plasminogen (iv t-PA) or when this is not effective. Our aim is to present the initial experience in with endovascular techniques in the Community of Madrid. Methods: We present data from our registry of acute ischaemic strokes treated with endovascular re-perfusion therapies in five University Hospitals in Madrid (Spain) during the period 2005-2009. We recorded demographic data, vascular risk factors, risk severity with the NIHSS (National Institute of Health Stroke Scale), endovascular techniques, complications and mortality rates. Functional outcome and neurological disability at 90 days was defined by the modified Rankin scale (mRs). Results: A total of 41 patients were treated with endovascular therapies. Mean age was 58.6 ± 19.9, and 56.1% were males. Of those 22 patients had an anterior circulation stroke and 19 had a posterior circulation stroke. Baseline NIHSS score was: median, 17 [range, 2-34]; 7 patients had previously received iv t-PA. The following endovascular techniques were performed: mechanical disruption (26 patients), intra-arterial infusion of t-PA (26 patients), angioplasty and stenting (5 patients), mechanical use of MERCI device (3 patients). Partial or total re-canalization was achieved in 32 patients (78%). Only one patient had a symptomatic cerebral haemorrhage. Three months after stroke, 53.6% of the patients were independent (mRs ≤ 2) and overall mortality rate was 19.5%. Conclusions: Acute ischaemic stroke is a potentially treatable medical emergency within the first hours after the onset of symptoms. Stroke endovascular procedures constitute an alternative for patients with iv t-PA exclusion criteria or when this is not effective. 16. Crafting L2 Multimodal Composing Identities: A Study with Secondary EFL Learners
- Author
-

- Abstract
Identity currently features as a prominent theme in digital multimodal composition (DMC), given the significant impact of DMC on identity expression. However, L2 learners' writer identity development and fluctuation in this type of textual composition needs further exploration in light of the absence of clearly established DMC-specific identity categories and a detailed account of writer identity expansion and restriction processes therein. This study attempts to fill this void by scrutinizing how three 11th graders learning English as a foreign language in Spain crafted their writer identities in English in a 16-week genre-based digital storytelling (DST) intervention that followed the genre-based systemic functional linguistics tradition of L2 writing, and included instruction on attitude (Martin & White, 2005). Qualitative data analysis of multi-source data followed Ivanic's (1998) writer identity framework, attitude categories, and key notions in multimodality, as well as the DST and identity literature. Results indicate students' awareness raising of such categories throughout digital story writing, and general preference for appreciation and judgment in writer identity construction, which was contingent upon certain factors. Students' writer identities and their fluctuation revealed types of specific DMC identity categories, and expansion and restriction processes, which illustrated their multimodal composing identity formation and transformation in digital story creation.

Brassinosteroids (BRs), a new family of plant hormones, have been used in a range of food staples, oil crops, and cereals. However, the scientific literature pertaining to their use in fleshy fruits remains scarce. This review presents, for the first time, the knowledge developed over the last decade on the role of BR preharvest treatments in crop yield and fruit quality properties at harvest and during storage, although information about the effects of BR postharvest treatments is also addressed. This review revealed that 24-epibrassinolide is the most used BR analogue in research experiments, either as a pre- or postharvest treatment, with doses ranging from 0.1 to 15 μM. Additionally, most of the research has been conducted on non-climacteric fruit species. In most of these preharvest treatments, an increase in crop yield has been reported, as well as enhanced anthocyanin concentration in red-coloured fruit. In addition, increases in firmness, total soluble solids, and phenolic content have also been observed. On the other hand, BR postharvest treatments led to the maintenance of these fruit quality properties during storage due to increased antioxidant systems, either enzymatic or non-enzymatic ones. Finally, as future perspectives, it is proposed to extend the research about BR treatments to other climacteric fruits and to deepen the knowledge of how BRs regulate physiological aspects from preharvest to postharvest. Furthermore, it is essential to investigate the role of BRs in the prevention of rot and biotic stress.

In modern orchard systems, the tree canopy is designed to ensure homogeneity in fruit quality. However, even in those crops there are some variables that affect the fruit maturation process and fruit quality properties. The aim of this work was to determine if canopy layer (upper vs. lower), fruit shoot position (grouped vs. individual) and orientation (west vs. east) affect fruit quality attributes of ‘Sanguinelli’ blood oranges. Thus, different quality traits, such as weight, internal colour (IC), external colour (EC), total soluble solids (TSS), titratable acidity (TA) and maturity index (MI) were determined for this purpose. Results showed that fruit weight, internal colour, TA and MI were influenced by the number of fruits per shoot. In this sense, the highest values of weight, IC and MI were found in the grouped fruits, while the highest values in TA were in the individual fruits. Regarding the EC and TSS, they were strongly related to the canopy layer, since the highest values were found in fruit located at the upper parts of the canopy. On the contrary, the orientation did not have a significant effect on fruit quality properties. Therefore, consistent differences in quality traits of ‘Sanguinelli’ blood oranges fruits were observed depending on canopy layer and number of fruits per shoot.

22. Researching Identity and L2 Pragmatics in Digital Stories: A Relational Account
- Author

García-Pastor, María Dolores
- Abstract
This study explores college EFL learners' construction of identity through the analysis of their pragmatic choices in digital stories, in which they narrated their relationship with another person they had helped in the past. More specifically, such choices were examined following Relational Dialectics Theory in learners' enactments of "connection" with and "autonomy" from this person. A specific view of identity in language education, the notion of "relational work" in (im)politeness research, and a social semiotic framework were also employed in data analysis. Learners' pragmatic choices ranged from the selection of the topic of their narratives according to types of social bonds, to the use of specific semiotic resources to build identities in conflict episodes of their stories (i.e., positive identities for themselves and positive and negative identities for their relational partners). The construction of these identities paralleled relational parties' convergent and divergent moves towards connection and autonomy, revealing their relational work. Learners used different semiotic resources in resolution episodes, which enabled them to craft positive identities for themselves as experts, teachers, and learners as well as position their relational partner as a competent agent and shape the connection-autonomy dialectic as "superiority-equality".

25. Learner Identity in EFL: An Analysis of Digital Texts of Identity in Higher Education
- Author
-
García-Pastor, María Dolores
- Abstract
This study explores learner identity (LI) in digital texts of identity (DTI) produced by college learners in English as a foreign language (EFL). In so doing, it aims to shed light on learners' connections of their learning experiences across time and settings, and their impact on the various learner identities that form their LI. It also intends to elucidate how learners construct powerful learner identities in and through their digital discourses. To this end, 51 DTI were collected and scrutinized, following studies on LI, critical and poststructuralist discourse analysis, "thematic" and "dialogic/performative" analysis within narrative research, and Kress and van Leeuwen's (2006) framework for the analysis of images. Learners established connections between their family and daily life spaces, school, and the foreign language community that account for, and shape their construction of their identities as learners in general, and language learners in particular. Students also empowered themselves by identifying with an intercultural speaker, using authority claims, and, in general, enhancing their authoritativeness in and through their texts. These findings underscore the potential of DTI to reinforce learners' identities, and create more equitable learning spaces.

Diabetic nephropathy (DN) is the leading cause of chronic kidney disease. Although hyperglycaemia has been determined as the most important risk factor, hypoxia also plays a relevant role in the development of this disease. In this work, a comprehensive metabolomic study of the response of HK-2 cells, a human cell line derived from normal proximal tubular epithelial cells, to hyperglycemic, hypoxic diabetic-like milieu has been performed. Cells simultaneously exposed to high glucose (25 mM) and hypoxia (1% O2) were compared to cells in control conditions (5.5 mM glucose/18.6% O2) at 48 h. The combination of advanced metabolomic platforms (GC-TOF MS, HILIC- and CSH-QExactive MS/MS), freely available metabolite annotation tools, novel databases and libraries, and stringent cut-off filters allowed the annotation of 733 metabolites intracellularly and 290 compounds in the extracellular medium. Advanced bioinformatics and statistical tools demonstrated that several pathways were significantly altered, including carbohydrate and pentose phosphate pathways, as well as arginine and proline metabolism. Other affected metabolites were found in purine and lipid metabolism, the protection against the osmotic stress and the prevention of the activation of the β-oxidation pathway. Overall, the effects of the combined exposure of HK-cells to high glucose and hypoxia are reasonably compatible with previous in vivo works.

Yellow pitahaya (Selenicereus megalanthus Haw.) is an exotic fruit with great potential for exportation in Ecuador. The research was carried out with the objective of evaluating the fruit growth and ripening as affected by four elicitors: salicylic acid (SA), methyl salicylate (MeSa), methyl jasmonate (MeJa), and oxalic acid (OA), all of them at 1, 5, and 10 mM concentration, compared with untreated plants (control). For each elicitor, nine plants were selected, and on each plant, three fruits were marked to follow up the growth by measuring polar and equatorial diameters. At harvest, yield (kg plant−1 and number of fruits plant−1), fruit weight, percentage of pulp and skin, total soluble solids (TSS), titratable acidity (TA), and firmness were determined. Treated plants enhanced fruit size, crop yield, and fruit weight compared with control fruits, although results depended on the elicitor tested and applied doses. The highest and lowest TSS were found in 10 mM MeSa and 5 mM MeJa-treated fruit, respectively, while the highest TA content was shown in 5 mM SA. Firmness was only enhanced in MeJa-treated fruits. Overall, results suggest that preharvest use of elicitors could modulate the pitahaya ripening and could improve quality attributes at harvest.

PITAHAYAS ,TROPICAL crops ,FRUIT ripening ,CULTIVARS ,BIOACTIVE compounds - Abstract
Pitahaya or dragon fruit is an exotic fruit native to Mesoamerica and is cultivated in several regions of the world. In recent years, pitahaya has become increasingly in demand, firstly, for its good nutritional and organoleptic qualities and, secondly, for its richness in antioxidants and bioactive compounds. Spain has opted for new tropical crops, and among them, pitahaya is one of the most planted in recent years. Most of the investigations on pitahaya were conducted with red cultivars, while the research on yellow pitahaya (Selenicereus megalanthus Haw.) is very scarce. In this review, the current knowledge about types of pitahayas, the fruit growth and ripening, the quality attributes, the postharvest storage, the use of by-products, and the health attributes were covered. HDL cholesterol , *GENETIC variation , *EXERCISE therapy , *BODY mass index , *INSULIN resistance , *BODY composition , *EXERCISE intensity - Abstract
Background/Objectives: Previous studies suggest that there is a genetically determined component of fat oxidation at rest and during exercise. To date, the FTO gene has been proposed as a candidate gene to affect fat oxidation during exercise because of the association of the "at-risk" A allele with different obesity-related factors such as increased body fat, higher appetite and elevated insulin and triglyceride levels. The A allele of the FTO gene may also be linked to obesity through a reduced capacity for fat oxidation during exercise, a topic that remains largely underexplored in the current literature. The aim of this study was to analyze the association between the FTO rs9939609 polymorphism with the rate of fat oxidation during exercise and metabolic syndrome criteria in healthy participants. Methods: A total of 80 healthy participants (41 men and 39 women) underwent comprehensive assessments, including measurements of anthropometric variables, blood pressure and blood measures of fasting glucose, triglycerides, low- and high-density lipoprotein cholesterol (LDL-c and HDL-c), insulin, interleukin-6 (IL-6) and C-reactive protein (CRP) concentrations. Additionally, the Homeostatic Model Assessment (HOMA-IR) was used to evaluate insulin resistance. Peak oxygen uptake (VO2peak) and maximal fat oxidation rate (MFO) were also measured during an incremental cycling test. FTO rs9939609 genotyping (TT, AT, AA) was performed using genomic DNA samples obtained from a buccal swab and measured with PCR. Results: There were 32 participants (40.0%) with the TT genotype; 31 (38.8%) with the AT genotype; and 17 (21.2%) with the AA genotype. Age, body characteristics, VO2peak, blood pressure and blood variables were similar across all three genotypes. However, serum insulin concentration and HOMA-IR were associated with the FTO rs9939609 genotype with higher values in AA with respect to AT and TT participants (p < 0.050). Still, MFO was similar in TT, AT and AA participants (0.35 ± 0.13, 0.37 ± 0.11, 0.33 ± 0.11 g/min, p = 0.702). In the dominant model, there was no statistical difference between TT and A allele carriers. However, the recessive model revealed that AA participants had higher values of body mass, body mass index, blood insulin concentration and HOMA-IR than T allele carriers (p < 0.050), with no differences in MFO. Conclusions: In our sample of healthy individuals, the FTO rs9939609 polymorphism was associated with several phenotypes associated with obesity and insulin resistance, particularly under the AA vs. T allele/recessive model. However, the FTO rs9939609 polymorphism was not associated with MFO during exercise as fat oxidation was similar across genotypes. This suggests that reduced fat oxidation during exercise is unlikely to be a cause of the obesogenic influence of the FTO AA genotype. Clinically, these findings suggest that the obesogenic effects of the FTO AA genotype are unlikely driven by impaired fat oxidation during exercise. Instead, attention should focus on mechanisms like appetite regulation and energy intake. Moreover, exercise interventions may still effectively mitigate obesity risk, as AA individuals retain normal fat oxidation capacity during exercise. [ABSTRACT FROM AUTHOR]

Polyamines (PAs) are low-molecular-weight compounds that contain amino groups. PAs are present in a variety of organisms, including plants, animals and microorganisms. In plants, the main PAs are putrescine (PUT), spermidine (SPD) and spermine (SPM). They play crucial physiological roles in plant development, including flowering, fruit set, growth, ripening and metabolic processes. In addition, PAs are components of the diet and have a role in health and disease. Furthermore, PAs have been demonstrated to help overcome the negative effects of adverse environmental factors of both biotic and abiotic stresses. Thus, the main objective of this review was to examine the recent literature regarding the mentioned effects of PAs apart from the impact of preharvest PAs treatments, applied at different stages of fruit development, on fresh fruit crop yield and fruit quality properties at harvest, and in their maintenance during storage, with a special emphasis on the fruit content in bioactive compounds with antioxidant activity. Moreover, this review addressed the impact of PAs on other physiological processes affecting crop yield such as flowering and fruit set. [ABSTRACT FROM AUTHOR]

Abdominal wall repair in adults with bladder exstrophy is challenging. We present a case of a 46-year-old woman with bladder exstrophy presenting with a large midline incisional hernia associated with a 13-cm hypoplasia of both pubic rami that precluded fixation of any abdominal mesh. A two-stage approach was adopted. First, a free vascularized osteocutaneous fibula flap was used to reconstruct the pelvic ring. After complete bone union 18 months later, a mesh was anchored to the fibula flap to restore the abdominal wall competence. After 2 years of follow-up, no hernia recurrence was observed, and the patient reported improved quality of life and self-esteem. This novel technique may provide long-term stability and good functional outcomes for reconstruction of the abdominal wall in selected adults with bladder exstrophy. [ABSTRACT FROM AUTHOR]

The evolution of the crystallographic texture after severe plastic deformation (SPD) of the aluminum alloy AA7075, commonly used in the aeronautical and automotive industries, depends on the parameters of the applied deformation process. In this paper, a combination between experimental ECAP processing and numerical simulation using the visco-plastic self-consistent methodology (VPSC) was carried out. The limitations in the homogeneity of the mechanical properties and texture of the parts processed via ECAP can be improved by an adequate choice of the processing route. According to the literature, the most effective route to increase the properties of this material is the Bc route. However, due to the two-fold symmetry along the extrusion axis, the Bc route cannot be used in the components under study. Because of this, it was decided to study C and modified C routes. The simulation results showed the characteristic fibers of the ECAP process measured through X-ray diffraction. The texture analysis shows that the most effective route to obtain a more homogenous shear deformation and therefore reduce the grain size is the Bc route, followed by the modified C route and finally the C route.
