1. Metabolic activity of intestinal microflora in newborns with a different mode of delivery

Abstract

The authors studied microbial composition and metabolic activity of the intestinal microbiota in 20 babies born by caesarean section (main group) and 20 babies born vaginally (control group). There were isolated bifidobacteria from meconium at a concentration of 105– 1010 CFU/g by the bacteriological method in 4 (20%) children of the main group and 6 (30%) children in the control group. The gas-liquid chromatography revealed the metabolic activity of various representatives of the intestinal microflora in children of both groups, regardless of the mode of delivery; the indicators of metabolic activity were significantly lower than in the infants. The children in both groups had similar absolute and relative content of short-chain fatty acids in meconium. The data of gas-liquid chromatography indicate the metabolic activity of the meconium microbiota, which is dominated by anaerobic microbial populations and there is a high proportion of proteolytic and conditionally pathogenic flora. The data obtained confirm the process of intrauterine translocation of microbes from mother to child and indicate the importance of the microflora of the future mother in the process of forming the intestinal microbiota of the child.

Abstract

Objective: presenting the latest finding about the process of the formation of the intestinal microbiota of children in the early stages of development and the factors that influence this process.Intestinal microbiota plays a key role in the physiology and maintenance of the homeostasis of the human body. The results of molecular genetic studies show that the formation of the intestinal microbiota begins in utero, and the child receives mother’s microflora throughout pregnancy, childbirth and breastfeeding. The microbial colonization of a child by mother’s and environmental microorganisms is the most intensive in childbirth and the postnatal period. Both intrauterine and neonatal periods are critical stages in the formation of the child's microbioma, which significantly determine the state of their health throughout further life. The composition of the microbiota being formed depends on the gestational age of the child, the mode of delivery, the type of feeding, antibacterial therapy, the sanitary and hygienic environmental conditions, geographical conditions, etc. A deeper understanding of the processes of intestinal microflora formation will allow developing effective methods for the prevention and correction of microecological disorders in the child.

Abstract

The article considers the key problems of the transition of the national health system to a new platform of personalized medicine and, in particular, pediatrics. In the second part, published in this issue, the main mechanisms for building a new model are analyzed. The main prerequisites (financial and economic, fundamental research and applied research), the emergence and construction in the structure of global trends in the development of health care markets, actualized at the end of the last century, are presented. It is indicated that one of the main incentives for the development of personalized medicine is a noticeable increase in individual and social demand for preventive drugs and the corresponding tools. The development of programs for managing their own health is gaining in importance, and new business-development models (including public and private partnerships) are being developed and implemented. The evidence is examined that a progressive scenario for the development of a model of personalized medicine will provide a significant reduction in the cost of medical care. A large section of the article is devoted to the development of the provision that the understanding and awareness of the importance of the protection and management of one's own health by a modern state, medical audience and individual citizen is the most important aspect of the educational and educational activities of a doctor and a nurse. The perspectives of personalized pediatrics as a global instrument for restructuring the entire healthcare system are considered in detail.

Abstract

The article considers the key problems of the transition of the national health-care system to a new platform of personalized medicine and, in particular, pediatrics. The first part, published in this issue, analyzes the most important of the necessary aspects of the infrastructure of the new model. Evidence is given of the extreme urgency of introducing a new model of predictive, preventive and personalized medicine (PPPM). The result of implementation should be breakthrough success in solving many epidemiological, diagnostic, curative, preventive, social and economic problems. It is emphasized that neonatology and pediatrics are the most important link in this paradigm. When considering the potential architectonics of the model, important characteristics of its main segments are revealed. Diagnostic principles (genotyping, targeting, and dynamic screening of biomarkers) and arsenal (genomics, proteomics, metabolomics, mathematical modeling tools, etc.) of personalized medicine are presented. Attention is focused on the need to create information (global, regional and target-specific) banks that are necessary for monitoring individual health. The need to create a new social decision-making mechanism for selecting a preventive protocol that minimizes the risks of the disease or prevents its development is discussed. Four categories of basic programs of medical and social support of persons from the risk category are considered. The necessary conditions for translating these programs into practice are presented. The main tasks and problems of developing the principles for the preparation of preventive-prophylactic and protocols of medical rehabilitation for personalized medicine were discussed.

Abstract

The clinical and social impacts of persistent infections remain relevant. The main aspects of this pediatric problem are the epidemiology and pathogenesis of persistent infections, their clinical manifestations, possibilities for their treatment and prevention, and prediction of the active reproduction of infectious agents. The issues of the human body’s interaction of persistent viruses with viruses that cause acute infectious diseases remain inadequately studied. There is no answer to the question as to the nature of this (synergistic, competitive) interaction or lack thereof. It is unclear how the persistent viruses interact with the host genome. The family and interfamily types of persistent infections, which is of key value in infection prevention in not only a baby, but also in a fetus, have not been adequately explored. This paper focuses on the most important and interesting medical and biological aspects of the problem of persistent infections and gives answers to some of them.

Abstract

Klebsiella is one of the most common causes of neonatal sepsis. Cases caused by extended-spectrum β-lactamase-producing Klebsiella strains are of the most relevance. The infections caused by these bacterial strains are resistant to the majority of cephalosporins commonly used in practice and lead to significant mortality. The development of their clinical forms, including severe ones, is determined by a number of virulence factors that include capsular antigen, lipopolysaccharide, pili, and siderophores. Despite the fact that the probability of sepsis is highest in premature infants, full-term ones are equally at risk in the situations associated by the infection due to extended-spectrum β-lactamase-producing Klebsiella. Our investigation of the specific features of the course of neonatal sepsis caused by Klebsiella established that hemorrhagic syndrome evolving in the presence of thrombocytopenia is the most common condition developing in Klebsiella-induced sepsis that determines the severity of a patient’s condition and affects the outcome of the disease.

Abstract

The paper reviews the data available in the literature on the health status and development of children born after in vitro fertilization (IVF). It analyzes the data of Russian and foreign publications in the period 1995 to 2013 with a predominant focus on large-scale multicenter studies. Results. Suboptimal perinatal outcomes (miscarriage, low birth weight, and their complications) are mainly caused by parental factors, such as infertility, advanced maternal age, and concurrent metabolic, genetic and epigenetic alterations, which affects the quality of gametes and the course of pregnancy and is likely to determine the increased (1,3—2) risk of congenital malformations and the need for their surgical correction in the children born after IVF as compared to those from spontaneous pregnancies. The only proven IVF-associated factor for suboptimal perinatal outcomes is multiple pregnancy. The data on the higher rate of somatic and other diseases in children born after IVF are controversial and unconfirmed by large-scale studies; it is difficult to compare the magnitude of genetic and epigenetic changes due to their rarity. The prediction of the reproductive status and delayed diseases (cancer, metabolic disturbances) in this group of children remains open.

Abstract

This review deals with the current epidemiological features of acute enteric infections in children. It provides the viewpoints of a number of investigators (including the authors of this paper) on the pathogenesis of infectious diarrheas and on the optimization of a set of diagnostic tests aimed to detect the pathogen of community-acquired enteric infections. The matters of pathogenetic therapy and the efficiency of using the drugs traditionally incorporated into a complex of treatment are examined. The correctness of the main points of the classification of these diseases, which is adopted in Russian practice, is discussed.

Abstract

Despite widespread vaccination, pertussis remains an important cause of infant morbidity and mortality. About 16 million people are ill with this disease and approximately 195 thousand children die worldwide every year. However, only 5—10% of all pertussis cases are diagnosed and notified. In spite of wide immunization coverage, there is really a pertussis epidemic in many countries of the world now; moreover, vaccinated children constitute a high proportion of disease cases. Adolescents and adults are a major reservoir for cyclic outbreaks of pertussis; however, they are rarely diagnosed with the disease. The review gives scientists' opinion on the causes of this phenomenon and proposed measures to reduce morbidity in different age groups. The severe and complicated forms of pertussis and fatal outcomes occur in babies during the first months of life and therefore current vaccination strategies should be aimed at preventing the infection just in this age group of children.

We show that triangular families of Boolean functions comprise an exponentially small fraction of proper families of a given order. We prove that if F is a proper family of Boolean functions, then the number of solutions of an equation F(x) = A is even. Finally, we describe a new class of proper families of Boolean functions.

Abstract

This paper presents the results of an investigation into hemoglobin-based protein films that were formed on a liquid surface. X-ray standing wave measurements were performed at the ID 10 beamline of the European Synchrotron Radiation Facility (ESRF) and at the Langmuir station of the Kurchatov Synchrotron Radiation Source. It was found that the ability of the protein to bind metal ions is substantially increased due to the conformational rearrangements of protein macromolecules caused by various damaging effects. The elemental composition of protein preparations, which were isolated from children and adults with chronic metabolic diseases accompanied by endogenous intoxication, was analyzed. The results of the investigations offer evidence that an increase in the ligand-binding properties of the protein molecules, which was observed in model experiments using protein films, is a common trait and corresponds to in vivo processes accompanying metabolic disturbances in the body.

Abstract

Introduction. Organ lesions due to the new coronavirus infection are determined by several factors: direct cytopathic effect of SARS-CoV-2, release of an unbalanced amounts of proinflammatory cytokines leads to systemic damage of the vascular endothelium and increasing hypoxia. These factors in total result in organ dysfunction.Methods and materials. We examined 96 pathological studies’ protocols of patients who died from the PCR-confirmed new coronavirus infection. Histological slides stained with hematoxylin and eosin were also analyzed.Results. Macroscopical and microscopical changes seen mostly in lungs, myocardium and other internal organs were described. We reviewed main causes of death (pulmonary and cardiopulmonary failure, multiple organ dysfunction syndrome) and most common concomitant diseases (diabetes mellitus, essential hypertension, obesity). The morphological features of diffuse alveolar damage in the acute and proliferative phases were described in detail. Taken together with the synchronous involvement of the microvasculature in the pathological process it became the main cause of death. The most severe structural changes in lungs were seen after 20 days of disease. These changes were correlated with dysregenerative metaplastic and dysplastic processes. Processes of disturbed regeneration reflect both the severe viral transformation of the alveolar epithelium and the fact that regeneration developed under mechanical ventilation.

Abstract

Acute respiratory viral infections (ARVI) occupy a leading place in the general structure of bronchopulmonary diseases in children. Their severity is often caused by bronchoobstructive syndrome with severe respiratory insufficiency. A similar clinical picture is given by asthmatic bronchitis, which often resembles bronchoobstructive syndrome [4]. Clinical and laboratory aspects of differential diagnosis of acute respiratory viral infections with bronchoobstructive syndrome and asthmatic bronchitis in young children remain insufficiently studied, while their timely recognition is of extremely great practical importance for rational therapy [6].

Abstract

Intensification of lipid peroxidation processes is of great importance in the violation of functional integrity of tissues of the respiratory tract and increased vascular and tissue permeability in a number of bronchopulmonary diseases. Since respiratory viruses are cytopathogenic agents, their leading role in the development of membrane disorders in acute inflammatory respiratory diseases in young children should be assumed. The works previously carried out in this direction refer only to the complicated forms of respiratory viral infections.

Abstract

To date, the causes of recurrent acute respiratory viral infections (ARI) in young children have not been conclusively established. The aim of this work was to study the features of the clinical course of acute respiratory viral infections and the immune status of young children who often and rarely have these infections. There were 132 acute respiratory viral infections patients at the age of 1 to 3 years under observation. There were 87 (65.9%) from 1 to 2 years old, and 45 (34.1%) from 2 to 3 years old. All patients depending on the frequency of ARI, registered in them during the recent year, were divided into 2 age groups: the 1st group (37 patients) included children ill with ARI from 7 to 10 times a year, and the 2nd group (95 patients) - up to 6 times a year.

Abstract

The aim of this work was to study the parameters of the components of the kinin blood system in children with severe forms of acute respiratory viral infections (ARVI) with neurotoxicosis syndrome. 55 children with ARVI (aged from 1 to 6 months - 14, from 6 months to 1 year - 18, from 1 to 3 years - 11, from 3 to 7 years - 12). 38 patients were admitted in the first three days of illness, 12 - on 4-5 days and 5 - at a later date. 30 children had a severe form of acute respiratory viral infection and 25 - moderate. Adenovirus infection was diagnosed in 14 patients, influenza - in 16, parainfluenza - in 7, MS-viral infection in 5, mixed viral infection - in 13. The control group consisted of 10 apparently healthy children.
