1. The long term prognostic value of peak left atrial strain in cardiac transplant patients

Abstract

Funding Acknowledgements Type of funding sources: None. Background Left atrial (LA) dysfunction is associated with poorer outcomes in many disease processes. Left atrial strain (LAS) is a novel two-dimensional (2D) quantitative analysis of LA function. Cardiac transplantation directly involves the LA during implantation of the donor heart. Traditional echocardiographic indices after transplantation have demonstrated value in correlating with acute cellular rejection (ACR), morbidity and mortality over short- and long-term follow-up. The prognostic value of LA strain has not been previously investigated in this cohort. Purpose We hypothesized that incrementally impaired LA strain in post cardiac transplant patients with varying degrees of ACR may be prognostic of poorer outcomes on long term follow-up. Methods 87 Heart transplant patients, assessed between 2009 and 2015, underwent transthoracic echocardiography and endomyocardial biopsy. 2D strain analysis on the LV and LA were performed along with traditional echocardiographic parameters. Patients were grouped according to peak LAS (PALS) tertiles and rejection burden history was assessed and grouped according to ACR burden at a median of 12 (±5.4) months post transplantation. The primary endpoint was all-cause mortality at follow-up. Results 12 patients met the primary endpoint over a median follow-up of 66 ± 51 months. The mean LA PALS was significantly different across the tertiles (lowest tertile 12.29 ± 2.5% vs middle tertile 17.89 ± 1.1% vs highest tertile 24.54 ± 4.2%; p Conclusions Lower LA PALS is significantly associated with poorer long-term outcomes in cardiac transplant patients with ACR. 2. Automatic alignment of multiple magnets into Halbach cylinders

Abstract

Halbach cylinders have found various applications for their ability to produce strong and homogenous magnetostatic fields. Contrary to their conventional manual fabrication, we introduce a novel approach to automatically align multiple permanent magnets into a Halbach cylinder. The approach uses the magnetic field distribution from a diametrically magnetized cylindrical magnet to simultaneously align multiple magnets. The extent to which the automatic assembly can approximate a Halbach cylinder was analyzed using 3D Finite Element Modeling. Prototypes were built that demonstrated automatic alignment of eight magnets into Halbach cylinders. Automatic alignment eliminates the complexity of manually aligning Halbach cylinders.

Abstract

It is well known that applying an electric field to a flame can affect its propagation speed, stability, and combustion chemistry. External electrodes, arc discharges, plasma jets, and corona discharges have been employed to allow combustible gas mixtures to operate outside their flammability limits or to increase combustion speed. Previously reported experiments have involved silent electrical discharges applied to propagating flames. These demonstrated that the flame propagation velocity can be increased when the discharge is applied to the unburned gas mixture upstream of a flame. In contrast, the work reported here used a coaxial-cylinder, nonthermal, silent discharge plasma reactor to activate a propane gas stream before it was mixed with air and ignited. With the plasma, the physical appearance of the flame changes (increased stability) and substantial changes in mass spectrometer peaks are observed, indicating that the combustion process is enhanced with the application of the plasma.

Percutaneous transluminal angioplasty ± stenting is a well established treatment of focal aorto‐iliac occlusion/stenosis and clinically significant proximal renal artery stenosis. (Surgery remains the treatment of choice in mesenteric ischaemia, although interventional techniques may challenge this as techniques improve.)•. Exciting developments in endovascular techniques for aortic aneurysm repair (EVAR) are taking place, to provide a less invasive procedure in patients with appropriate anatomical prerequisites.•. Inferior vena cava (IVC) filters are becoming an established prophylactic treatment in selected patients with recurrent pulmonary embolism. Stents are also being used successfully for IVC compression syndromes.

Abstract

Axial current discharges were used to implode spherical aluminum shells with thickness tapered to match the external magnetic pressure. These implosions were used to compress hydrogen plasmas with an initial pressure $g100\mathrm{bar}$ and an initial temperature $g1\mathrm{eV}$. The implosion and compressed plasma effect on a central target were observed radiographically. The target's radius vs time indicates that the plasma pressure reaches $\ensuremath{\sim}1\mathrm{Mbar}$ prior to liner-target contact.

Abstract

Atlas is a facility being designed at Los Alamos National Laboratory (LANL) to perform high-energy-density experiments in support of weapon physics and basic research programs. It is designed to be an international user facility, providing experimental opportunities to researchers from national laboratories and academic institutions. For hydrodynamic experiments, it will be capable of achieving a pressure exceeding 30 Mbar in a several cubic centimeter volume. With the development of a suitable opening switch, it will be capable of producing more than 3 MJ of soft X-rays. The capacitor bank design consists of a 36 MJ array of 240 kV Marx modules. The system is designed to deliver a peak current of 45-50 MA with a 4-5-/spl mu/s rise time. The Marx modules are designed to be reconfigured to a 480-kV configuration for opening switch development. The capacitor bank is resistively damped to limit fault currents and capacitor voltage reversal. An experimental program for testing and certifying prototype components is currently under way. The capacitor bank design contains 300 closing switches. These switches are a modified version of a railgap switch originally designed for the DNA-ACE machines. Because of the large number of switches in the system, individual switch prefire rates must be less than 10/sup -4/ to protect the expensive target assemblies. Experiments are under way to determine if the switch-prefire probability can be reduced with rapid capacitor charging.

Abstract

Non tropical, non hypereosinophilic endo myocardial fibrosis has been reported in literature and is a rare entity. Cardiac Sarcoidosis manifesting as non-tropical, non hypereosinophilic endomyocardial fibrosis is unknown, though it classically affects the left ventricle and is not associated with specific risk factors. We describe an atypical presentation of sarcoidosis as non-tropical non hypereosinophilic endomyocardial fibrosis in a middle aged female who presented to us with refractory heart failure.

Abstract

Echocardiography is the mainstay of cardiovascular diagnostics, and is the most performed test for the evaluation of cardiac function. Critical and costly management decisions are based on quantification of left ventricular volumes and ejection fraction. Recent advances in echocardiography, such as microsphere contrast echocardiography for left ventricular opacification and perfusion imaging, three-dimensional transthoracic and trans-oesophageal imaging, strain and tissue Doppler imaging, all contribute to improving accuracy and reproducibility of these important measurements. Such techniques are now routinely available on standard echocardiography equipment in Australian centres for daily use. Hand-carried ultrasound devices have been developed, which are portable, are affordable and offer increased availability of echocardiography to the wider community. Clinicians should be actively encouraged to adopt these technologies to improve the diagnostic quality and reproducability of echocardiography for our patients. This article provides an overview of important recent advances in echocardiographic imaging with an emphasis on their role in clinical practice today.

The majority of transient ischemic attacks (TIAs) and ischaemic strokes are caused by thromboembolism from a stenosed large extracranial artery or occlusion of a large intracranial artery (atherothrombotic stroke), occlusion of a small intracranial artery (lacunar stroke), or embolism from the heart (cardioembolic stroke) [74]. In the remaining patients, the origin of the infarction is not established by investigation (TIA or ischaemic stroke of indeterminate aetiology), or less common causes for ischaemia or infarction are identified (e.g., sickle cell disease or vasculitis).

Abstract

We present a patient with an arteriovenous fistula of the peroneal artery acquired after a left dome tibial osteotomy with midshaft fibular osteotomy. He had subsequently had a total knee replacement on that side. The arteriovenous malformation was only diagnosed when he represented with symptoms and signs of venous hypertension with sterile recurrent haemarthroses in the left knee. Percutaneous obliteration of the fistula, by a combination of coil embolisation and balloon occlusion, cured the symptoms.

Abstract

Efforts have been undertaken to determine the one-dimensional density profile of liner-driven cylindrical targets from analysis of the transverse radiographs of liner-driven implosion experiments at Pegasus II. The technique reported here uses forward modeling to determine the density of the different materials in a cylindrically symmetric target after liner impact. The material density was determined by performing a least-squares fit of a model function in the cylindrical geometry to match the horizontal line-out of the radiographic intensity data. Estimates of the effective X-ray spectrum are required for this analysis because of the strong energy dependence of the X-ray interaction cross sections with the different materials in the experiment. The implosions described in this report maintain good cylindrical symmetry. The analysis is performed with inexpensive, commercially available software. In this work, the density profiles are determined for two separate experiments, designated DH-5 and LLNL-3.

Abstract

Pegasus-II is a pulsed power facility at Los Alamos National Laboratory which is used to conduct a variety of experiments in the high energy density regime, with applications to the physics of nuclear weapons as well as basic science. The chief mission of the facility is the systematic investigation of hydrodynamic physics issues through the use of a magnetically-driven, cylindrical imploding liner. At 4.3-MJ of stored energy, Pegasus-II is one of the largest capacitor-bank facilities in the world. Peak currents as high as 12 MA have been produced with a quarter-cycle time of 6-8 /spl mu/s. The active portion of the standardized aluminum liner is a 3.2-g right hollow cylinder (4.8-cm outer diameter, 2-cm high, and 0.04-cm wall thickness) designed such that the inner surface of the liner remains at solid aluminum density during the course of the experiment. The run-in time for a typical experiment is /spl sim/10 /spl mu/s. For some experiments, a diagnostic package is placed inside the liner to analyze physical processes associated with multi-microsecond convergent implosions of macroscopic solid shells. These campaigns include the study of instability growth rates, hydrodynamic bounce and mix, and mechanical heating associated with high strain and strain rate. Other experimental studies, in particular those examining the shock production of ejecta and shock-driven hydrodynamic vortex formation and compression, have involved the use of an internal target package.

We have used the LANL PEGASUS Z-pinch facility to drive a thin cylindrically-convergent Al liner to /spl sim/3 km/s to launch /spl sim/30 GPa shocks in a 3-mm thick 10-mm-i.d. aluminum cylinder whose interior is filled with 1 atm Xe gas. The subsequent material motion of the metal and gas is diagnosed with both radial and axial flash X-rays and with optical framing cameras Instabilities are seeded by implanting wires of assorted higher density metal parallel to the cylinder axis. We have done two shots, varying the target from Al 1100-O to Al 6061-T6 to explore the effect of changing material strength. The images show the spallation failure of the metal-gas interface on shock release and the effect of the seeded instabilities.

Abstract

Using pulsed power to implode a liner onto a target can produce high shock pressures for many interesting application experiments. With the Pegasus II facility in Los Alamos, a detailed theoretical analysis has indicated that the highest attainable pressure is around 2 Mbar for a best designed aluminum liner. Recently, an interesting composite liner design has been proposed which can boost the shock pressure performance by a factor of 4 over the aluminum liner. This liner design was adopted in the first megabar (Megabar-I) liner experiment carried out on Pegasus last year to verify the design concept and to compare the effect of Rayleigh-Taylor instabilities on liner integrity with the code simulations. The authors present briefly the physical explanation why the composite liner provides the best shock pressure performance. The theoretical modeling and performance of Megabar-I liner are discussed. Also presented are the first experimental results and the liner design modification for their next experiment.

The Atlas capacitor bank design consists of a 36 MJ array of 240 kV Marx modules. The system is designed to deliver a peak current of 40-50 MA with a 4-5 /spl mu/s risetime. The Marx modules, are designed to be reconfigured to a 480 kV configuration, if needed, for opening switch development. The bank is resistively damped to limit fault currents and capacitor voltage reversal. The system is configured for very low-inductance operation to rapidly implode heavy liner loads. An experimental program for testing and certifying prototype components is underway. For many applications the Atlas liner will be a nominal 70 g aluminum cylinder. Using composite inner layers and a variety of interior target designs, a wide variety of experiments' in /spl sim/cm/sup 3/ volumes may be performed. These include shock compression experiments up to /spl sim/3 TPa (30 Mbar), quasi-adiabatic compressions up to 6-fold compression and pressures above 10 TPa, hydrodynamic instability studies in nonlinear and turbulent regimes over multi-cm propagation lengths, experiments with dense plasmas in the so-called high-gamma regime, studies of materials response at very high strains and strain rates, and materials studies in ultrahigh magnetic fields (above 10/sup 3/ T).

Abstract

When a shock wave interacts at the surface of a metal sample "ejected matter" (ejecta) can be emitted from the surface at velocities larger than the sample velocity. The mass, size, shape, and velocity of ejecta varies depending on the initial shock conditions and the target's material properties. In order to understand this phenomena, diagnostics have been developed and implemented at the Pegasus Pulsed Power Facility (PPPF) located at Los Alamos National Laboratory (LANL). The facility provides both radial and axial access for making measurements. There exist optical, laser, and X-Ray paths for performing measurements on the target assembly located near the center of the machine. The facility can provide many mega-amps of current which are transported to a 5.0-cm diameter, 2.0-cm high aluminum cylinder. The current and associated magnetic field set up forces which implode the aluminum cylinder radially inward. As the aluminum cylinder reaches the appropriate velocity, it impacts a target cylinder. Due to this impact, a shock wave is set up in the target and eventually interacts at the inner surface of the target cylinder where ejecta are produced. A 1.5-cm diameter collimator cylinder located in the target cylinder is used to control the number of ejecta particles that arrive at the center region where ejecta measurements are made. Two diagnostic techniques for characterizing ejecta, inline Fraunhofer holography and visible shadowgraphy are detailed in this report.

Atlas is a facility being designed at Los Alamos National Laboratory (LANL) to perform high energy-density experiments in support of weapon-physics and basic-research programs. It is designed to be an international user facility, providing experimental opportunities to researchers from national laboratories and academic institutions. For hydrodynamic experiments, it will be capable of achieving pressures exceeding 20-Mbar in a several cm/sup 3/ volume. With the development of a suitable opening switch, it will also be capable of producing soft X-rays. The 36 MJ capacitor bank will consist of 240 kV Marx modules arranged around a central target chamber. The Marx modules will be discharged through vertical triplate transmission lines to a parallel plate collector inside the target chamber. The capacitor bank is designed to deliver a peak current of 45 to 50 MA with a 4- to 5-/spl mu/s risetime. The Marx modules are designed to be reconfigured to a 480 kV configuration for opening switch development. Predicted performance with a typical load is presented. Descriptions of the major subsystems are also presented.

Atlas will be a high-energy (36 MJ stored), high-power (/spl sim/10 TW) pulsed power driver for high energy-density experiments, with an emphasis on hydrodynamics. Scheduled for completion in late 1999, Atlas is designed to produce currents in the 40-50 MA range with a quarter-cycle time of 4-5 /spl mu/s. It will drive implosions of heavy liners (typically 50 g) with implosion velocities exceeding 20 mm//spl mu/s. Under these conditions, very high pressures and magnetic fields are produced. Shock pressures in the 50 Mbar range and pressures exceeding 10 Mbar in an adiabatic compression will be possible. By performing flux compression of a seed field, axial magnetic fields in the 2000 T range may be achieved. A variety of concepts have been identified for the first experimental campaigns on Atlas. Experimental configurations, associated physics issues, and diagnostic strategies are all under investigation as the design of the Atlas facility proceeds. Near-term proof-of-principle experiments employing the smaller Pegasus II capacitor bank have been identified, and several of these experiments have now been performed. This paper discusses a number of recent Pegasus II experiments and identifies several areas of research presently planned on Atlas.

We have used 12 megamp, 5 megajoule axial discharges to electromagnetically implode tapered thickness spherical aluminum shells, achieving peak implosion velocities above 20 km/sec inner surface, 10 km/sec thickness averaged. The shell thickness was proportional to the inverse of the square of the cylindrical radius. This causes the ratio of magnetic pressure to shell areal mass density (and spherical acceleration) to be independent of polar angle, so that the spherical shape is nominally maintained during the implosion. We have used these implosions to compress hot hydrogen plasmas with initial pressure about 100 atm and initial temperature above 1 eV. The hot hydrogen plasmas were injected beforehand using 1 megamp, 100 kilojoule range co-axial gun discharges through a circular array of vanes to strip away magnetic field. The imploding shell and the compressed hot hydrogen working fluid's effect on a diagnostic compression target were observed with radiography.

Pulsed power driven cylindrical shock physics experiments are being performed at the PEGASUS facility at Los Alamos National Laboratory. A time dependent, axial X-ray imaging capability forms a subset of the measurements to quantify material behavior during shock propagation. 20 ns pulsed W target X-ray sources with about 10 mR at 1 m form the backlighter. Inorganic scintillators generate time dependent, visible images, which are relayed to a shuttered, microchannel plate intensifier imaging system coupled to electronic video readouts. The dynamic range, sensitivity, scene contrast, and system spatial resolution are optimized to specific experiments via optimization of fluor response to both X-ray energy and light output time response. In addition, a series of calibration data are taken to permit characterizing density information through postevent image processing. This dataset includes flat field and step wedge images. The flat field is particularly important due to the relatively large spatial variation in the X-ray dose resulting from the close proximity (60 cm) of the source to the imager.

Summary form only given. A soft X-ray tube built and developed by one of the authors (DP) is a standard radiographic diagnostic for imploding liner experiments conducted at Los Alamos. For particular experiments one would like to increase the X-ray dose and simultaneously decrease the X-ray spot size generated by this tube. We are using both particle-in-cell and Monte Carlo models to determine those design changes to the diode and/or driving voltage that can produce the desired characteristics. To evaluate the X-ray dose, the spectral features of the radiation, and spot size that are generated from electrons striking a cone-shaped tungsten anode target, we are using the Monte-Carlo transport code, MCNPE. A range of electron energies and incident angles is modeled to try to optimize the X-ray characteristics.

Abstract

Pulsed power systems have been used in the past to drive solid liner implosions for a variety of applications. In combination with a variety of target configurations, solid liner drivers can be used to compress working fluids, produce shock waves and study material properties in convergent geometry. The utility of such a driver depends in part on how well-characterized the drive conditions are. This, in part, requires a pulsed power system with a well-characterized current waveform and well-understood electrical parameters. At Los Alamos, the authors have developed a capacitively driven, inductive store pulsed power machine, Pegasus, which meets these needs. They have also developed an extensive suite of diagnostics which are capable of characterizing the performance of the system and of the imploding liners. Pegasus consists of a 4.3 MJ capacitor bank, with a capacitance of 850 /spl mu/f fired with a typical initial bank voltage of 90 kV or less. The bank resistance is about 0.5 m/spl Omega/, and bank plus power flow channel has a total inductance of about 24 nH. In this paper, the authors consider the theory and modeling of the first precision solid liner driver fielded on the Pegasus pulsed power facility.

Abstract

The Los Alamos High Energy Density Physics program uses capacitively driven low voltage, inductive-storage pulse power (including the 4.3 MJ Pegasus II capacitor bank facility) to implode cylindrical targets for hydrodynamics experiments. Once a precision driver liner was characterized an experimental series characterizing the aluminum target dynamics was performed. The target was developed for shock-induced quasi-particle ejecta experiments including holography. The concept for the liner shock experiment is that the driver liner is used to impact the target liner which then accelerates toward a collimator with a slit in it. A shock wave is set up in the target liner and as the shock emerges from the back side of the target liner, ejecta are generated. By taking a laser hologram the particle distribution of the ejecta are hoped to be determined. The goal for the second experimental series was to characterize the target dynamics and not to measure and generate the ejecta. Only the results from the third shot, Pegasus II-26 fired April 26th, 1994, from the series are discussed in detail. The second experimental series successfully characterized the target dynamics necessary to move forward towards our planned quasi-ejecta experiments.

The pulsed power Pegasus facility at Los Alamos National Laboratory has been configured to drive cylindrically symmetric shocks into a load region containing multiple density regions. A series of experiments are being performed to permit the development of diagnostic techniques to characterize the spatial distribution and temporal dependence of the cylindrical shocks generated in a 4.5 cm diameter/spl times/2 cm thick "target" region. The loads utilized in the preliminary experiments contain multiple regions. Optically transmissive regions are being diagnosed with long pulse, 694 nm laser backlighting. Framing cameras and fast optically shuttered cameras are being developed for high resolution image capture. Pulsed X-ray sources permit characterization of material boundary regions through differential X-ray absorption in a radiographic geometry. The desire for multiple visible and X-ray images requires fast time response, electronic imaging techniques. Typical shock pressures and velocities of 300 Kbar and 5 mm//spl mu/s coupled with a desired spatial resolution of better than 5lp/mm dictate temporal resolution requirements on the order of 50 ns. The current status of the diagnostic development is described as well as possible directions for future improvements.

Abstract

Flash X-ray units are used to diagnose pulsed power driven experiments on the Pegasus machine at Los Alamos. Several unique designs of Marx powered flash X-ray units have been developed to meet the requirements of the Pegasus experiments. All of these units are compact, battery powered, fiber optically controlled, and EMP shielded. Some of these units are operated with a windowless X-ray tube in the Pegasus machine vacuum tank thereby making the full bremsstrahlung spectrum available for both hard and soft X-ray images. Other units obtain multiple X-ray flashes that are almost collinear by employing an X-ray tube configuration which allows closely spaced X-ray emitting anodes. These units all emit a 10 ns FWHM X-ray pulse. Their Marx banks store from 12 to 100 Joules of electrical energy. The X-ray output ranges from 20 to 100 mR at 3 m with endpoint energies from 100 to 500 keV.

Abstract

An in-line holographic imaging system has been developed for hydrodynamic experiments at the Pegasus facility located at Los Alamos National Laboratory. Holography offers the unique capability to record distributions of particles over a three dimensional volume. The system to be discussed is used to measure particle distributions of ejecta emitted after a cylindrical aluminum liner (5.0 cm in diameter, 2.0 cm high) impacts a target (3.0 cm in diameter). The ejecta emerge from the target traveling up to 7 mm//spl mu/s and moves toward the axial center of the system where the holographic imaging is performed. In-line holography is particularly suited for the Pegasus pulsed power facility where the geometry restrictions make off axis holography impractical. In order to record the fast moving particles a frequency-doubled Nd:YAG laser system has been implemented which produces a 80 ps 20 millijoule pulse at 532 nm. An optical relay system composed of a Fourier optical lens pair has been developed which is placed 4.0 cm from the center of the region of interest. This relay lens pair forms an intermediate image 32 cm from the object plane and the hologram is placed 4 cm downstream from the intermediate image. The holographic system and resolution capability is discussed.

Our aim was to reduce motion artefact during radiographic angiography and MRI of the lower limbs. The legs were immobilized using a bead bag with a filtered closeable valve. Air was removed by suction and the bag moulded into a rigid restraint. The bead bag is an effective, well tolerated and easily used immobilization device that is suitable for use in radiological imaging.

We introduce a permanent magnet assembly that can be configured to obtain uniform, gradient, or tunable field distribution. The design is composed of a single ring shaped permanent magnet and a concentric ferromagnetic shim. Magnetic field is configured by changing the shape of the air gap inside the ring magnet. Circular cross-section produces up to 0.54 T uniform field, whereas rectangular or triangular cross-sections result in gradient magnetic field distributions. Tunable field from a given ring magnet is obtained by changing the thickness of the ferromagnetic shim or the spacing between the shim and the permanent magnet.

Abstract

A detailed neurological investigation of patients with Kallmann’s syndrome (KS) has been performed in an attempt to relate phenotypic characterization with genotype. Twenty-seven subjects with KS were studied (including 12 males with X-linked disease and 3 females). Six male and 2 female normosmics with isolated GnRH deficiency, 1 male with KS variant, and 1 obligate female carrier were also imaged. Evidence for X-linked disease was derived both from analysis of pedigree and by mutation analysis at the KAL locus. The female carrier and all 8 normosmics had normal olfactory bulbs and sulci, as did 3 male KS. The study, therefore, confirms the value of magnetic resonance imaging in the diagnosis of KS, but suggests that the technique is not sufficiently sensitive to differentiate KS from the normosmic form of GnRH deficiency in all cases. Phenotypic characterization of KS was more effectively achieved by accurate estimation of olfactory status. Three new mutations at the KAL locus were identified, 2 single exon deletions and 1 point mutation. In 2 pedigrees with clear Xlinked inheritance, no coding sequence mutations were detected; it may be that these harbor mutations of pKAL, the recently characterized .5’-promoter region. No clear relationship could be established between specific phenotypic anomalies and particular KAL mutations. Involuntary, mirror movements of the upper limbs were present in 10 of 12 cases of X-linked KS, but in none of the other subjects. Although this phenomenon has been ascribed to an abnormality of the corpus callosum, in the present study magnetic resonance imaging demonstrated no quantitative or qualitative morphological anomalies of this structure. We present a patient with an arteriovenous fistula of the peroneal artery acquired after a left dome tibial osteotomy with midshaft fibular osteotomy. He had subsequently had a total knee replacement on that side. The arteriovenous malformation was only diagnosed when he represented with symptoms and signs of venous hypertension with sterile recurrent haemarthroses in the left knee. Percutaneous obliteration of the fistula, by a combination of coil embolisation and balloon occlusion, cured the symptoms.

A patient with chronic inflammatory demyelinating polyneuropathy (CIDP) established by biopsy developed cauda equina symptoms due to swelling of the nerve roots in the lumbar spinal canal. Magnetic resonance imaging of the lumbar spine showed profoundly thickened nerve roots from the level of the conus medullaris, filling the caudal thecal sac. Immunosuppressant treatment produced partial clinical and radiological resolution. This case shows that spinal compressive syndromes may occur in acquired hypertrophic neuropathies as well as in hereditary motor and sensory neuropathy and expands the range of the clinical presentation of CIDP.

A 63-year-old gentleman was admitted to hospital complaining of progressive neck swelling, with bruising and increasing difficulty in swallowing. These symptoms were preceded by a severe bout of coughing. The night prior to admission he had eaten a meal of haddock, had woken well but then experienced the bout of coughing. He had a past medical history of polycythaemia which was treated with busulphan and regular venesection. Examination revealed a distressed patient with massive pulsatile swelling on the right side of his neck. The overlying skin was bruised. His blood pressure measured 150/80.
