Metastatic spine cancer can cause pain and neurological issues, making it challenging to distinguish from spinal compression fractures using magnetic resonance imaging (MRI). To improve diagnostic accuracy, this study developed artificial intelligence (AI) models to differentiate between metastatic spine cancer and spinal compression fractures in MRI images. MRI data from Gyeongsang National University Hospital, collected from January 2019 to April 2022, were processed using Otsu’s binarization and Canny edge detection algorithms. Using these preprocessed datasets, convolutional neural network (CNN) and support vector machine (SVM) models were built. The T1-weighted image-based CNN model demonstrated high sensitivity (1.00) and accuracy (0.98) in identifying metastatic spine cancer, particularly with data processed by Otsu’s binarization and Canny edge detection, achieving exceptional performance in detecting cancerous cases. This approach highlights the potential of preprocessed MRI data for AI-assisted diagnosis, supporting clinical applications in distinguishing metastatic spine cancer from spinal compression fractures.

The purpose of this study was to compare the surface changes and shear bond strength between a resin composite and two zirconia ceramics subjected to sandblasting and forming gas (5% H2 in N2) plasma surface treatment. Two types of zirconia ceramic specimens (3Y-TZP and (Y,Nb)-TZP) were divided into groups based on the following surface treatment methods: polishing (Control), sandblasting (SB), sandblasting and plasma (SB-P), and plasma treatment (P). Subsequently, chemical surface modification was performed using Clearfil SE Bond (Kuraray, Tokyo, Japan), and the Filtek Z-250 (3M, Maplewood, MN, USA) resin composite was applied. Shear bond strengths (SBS) and surface characteristics were determined. Plasma treatment was effective in increasing the wettability. For SBS, there were significant differences among the groups, and the (Y,Nb)-TZP and SB-P groups showed the highest bond strength. Similarly, for the 3Y-TZP specimens, the shear bond strength increased with both plasma and sandblasting treatments, although no statistically significant change was observed. In the P group, both (Y,Nb)-TZP and 3Y-TZP showed a significant decrease in shear bond strength with the resin composite compared to the control group.

In this study we analyze a recent controversy within the biomedical world, concerning the evaluation of safety of certain vaccines. This specific struggle took place among experts: the Danish epidemiologist Peter Gøtzsche on one side and a respected scientific institution, the Cochrane, on the other. However, given its relevance, the consequences of such a conflict invest a much larger spectrum of actors, last but not least the public itself. Our work is aimed at dissecting a specific aspect happening in this complex scenario: strategy. In other words, we want to highlight the value and the impact of strategic decisions when complex issues, as those analyzed, are at stake. In order to address this we have decided to adopt a game-theoretic approach. Our work will be structured as it follows. First, we will introduce the controversy and the two main actors: Peter Gøtzsche and the Cochrane. Second, we will explain why this controversy is important and its value beyond its academic relevance. Third, we will frame the controversy as a game and will provide several models representing different situations, also furnishing an analysis of these distinct scenarios. AIM: To evaluate visual outcomes and changes in fluid after administering monthly anti-vascular endothelial growth factor (VEGF) injections to treat neovascular age-related macular degeneration (nAMD) with subretinal fluid (SRF) and pigment epithelial detachment (PED). METHODS: This prospective study included eyes with nAMD previously treated with as-needed anti-VEGF injections. The patients were treated with six monthly intravitreal injections of ranibizumab. Quantitative volumetric segmentation analyses of the SRF and PED were performed. The main outcome measures included best-corrected visual acuity (BCVA), and SRF and PED volumes. RESULTS: Twenty eyes of 20 patients were included in this study. At the 6-month follow-up, BCVA and PED volume did not change significantly (P=0.110 and 0.999, respectively) but the mean SRF volume decreased from 0.53±0.82 mm3 at baseline to 0.08±0.23 mm3 (P=0.002). The absorption rate of the SRF volume was negatively correlated with the duration of previous anti-VEGF treatment (P=0.029). Seven of the 20 eyes (35%) showed a fluid-free macula and significant improvement in BCVA (P=0.036) by month 6. CONCLUSION: Quantifying the SRF can precisely determine the patient's responsiveness to anti-VEGF treatment of nAMD.

Scientific and philosophical accounts of cognition and perception have traditionally focused on the brain and external sense organs. The extended view of embodied cognition suggests including other parts of the body in these processes. However, one organ has often been overlooked: the gut. Frequently conceptualized as merely a tube for digesting food, there is much more to the gut than meets the eye. Having its own enteric nervous system, sometimes referred to as the “second brain,” the gut is also an immune organ and has a large surface area interacting with gut microbiota. The gut has been shown to play an important role in many physiological processes, and may arguably do so as well in perception and cognition. We argue that proposals of embodied perception and cognition should take into account the role of the “gut complex,” which considers the enteric nervous, endocrine, immune, and microbiota systems as well as gut tissue and mucosal structures. The gut complex is an interface between bodily tissues and the “internalized external environment” of the gut lumen, involved in many aspects of organismic activity beyond food intake. We thus extend current embodiment theories and suggest a more inclusive account of how to “mind the gut” in studying cognitive processes.

Background Chronic subscapularis tendon tear (SBT) is a degenerative disease and a common pathologic cause of shoulder pain. Several potential risk factors for chronic SBT have been reported. Although metabolic abnormalities are common risk factors for degenerative disease, their potential etiological roles in chronic SBT remains unclear. The purpose of this study was to investigate potential risk factors for chronic SBT, with particular attention to metabolic factors. Methods This study evaluated single shoulders of 939 rural residents. Each subject undertook a questionnaire, physical examinations, blood tests, and simple radiographs and magnetic resonance imaging (MRI) evaluations of bilateral shoulders. In the framework of symbiotic associations, the so-called “holobiontic” perspective has increasingly emerged, according to which animals and plants should no longer be considered as autonomous entities, delimited by clear boundaries, but should rather be seen as functional units, consisting of inter-relational networks of different species. From this perspective, the functions that were previously ascribed to a single component, must be updated in the light of this relational perspective and rather judged as a product of the functional unit, that is, the holobiont. In a functionalist perspective, the famous conception of the extended mind investigates the hypothesis that the vehicle of cognition can extend not only outside the brain but also beyond the limits of the body, through artificial devices. The present work aims to offer a variant of the extended theory of mind not only as a theoretical possibility but also in accordance with some recent developments in biological and biomedical research. It will therefore be argued that the activity of the human microbiota (bacteria living in associations with the human species) plays a functional role in the regulation of our cognitive processes suggesting that the microbiota constitutes a functional extension of the cognitive system previously described as “human”. The revision and extension of the biological individual in favor of the holobiont as a functional unit (therefore the true creator of cognitive functions) also leads to the need to update the boundaries of the individual as a cognitive agent.

Background Textile-reinforced mortar (TRM) is an innovative strategy for the reduction of the seismic vulnerability of existing masonry buildings consisting in the application on the masonry surface, of a mortar coating with fiber-based grids or textiles embedded. The paper presents the calibration and application of a simplified modelling approach, based on multi-layered elements, for the simulation of existing masonry elements and structures strengthened through TRM. Methods The strengthened masonry is modelled by using 20-nodes brick elements formed by a stacking sequence of layers representing the different material components (the masonry, the mortar coating and the embedded reinforcement). The nonlinear behavior of the materials is considered and calibrated on the basis of experimental characterization tests on individual components available in the literature. The simplified assumption of perfect bond among layers is considered. Results Non-linear static analyses are performed on samples of increasing complexity: elementary panels, structural elements (piers and spandrels) and a pilot building. The results of some tests on TRM strengthened masonry, available in the literature, are considered to assess the model reliability in terms of capacity curves and collapse mode. The model is capable of detecting the typical failure mechanism of both unstrengthened and TRM strengthened masonry, namely the diagonal cracking, the in-plane bending and the out-of-plane bending and is able to detect the activation also of mixed failure modes, that often occur in actual configurations. Conclusions Given the coarse mesh size and the smear plasticization assumption, the model is not suitable for the rigorous reproduction of individual cracks but represents a good compromise between the goal to grasp the structural performances at the wide scale, including failure modes, and the analysis optimization.

IntroductionSuicide is an important public health problem. Well-established risk factors of suicide include depression, family history of mental disorders, substance problem, chronic physical illness, and others. Sensory impairment, especially visual impairment (VI), has a critical impact on both mental and physical health. However, the association between VI and risk of suicide has not been thoroughly investigated and remains controversial. Our aim is to systematically review and meta-analyze the current evidence on the association between VI and risk of suicide and to evaluate the direction and magnitude of the association.Methods and analysisWe aim to search PubMed, EMBASE and the Cochrane Library to identify all population-based studies on the association between VI and risk of suicide. Two reviewers will independently conduct study selection, data extraction and risk of bias (ROB) assessment. The Newcastle-Ottawa scale will be applied to evaluate the methodologic quality of the included studies for ROB assessment. The primary outcome measure will be the relative risk (RR) of suicide, and the secondary outcome measures will be the risks of suicidal ideation (SI) and suicide attempt (SA). Estimates of risk with 95% confidence intervals (CIs) for suicide, SI and SA, respectively, will be calculated and summarized. We will perform random-effects meta-analyses to combine the pooled effects. Meta-regression will be applied to investigate the effects of multiple factors across studies. Subgroup and sensitivity analyses will be conducted for screening of any potential sources of heterogeneity. Publication bias will be evaluated by funnel plot and Begg and Mazumdar correlation testing. The body of evidence will be assessed using the Grading of Recommendations, Assessment, Development and Evaluation (GRADE) approach.ConclusionThis article presents a study protocol for investigating the association between VI and risk of suicide. The findings of this study will contribute to our current knowledge of the impact of VI as a risk factor of suicide. In addition, meta-regression and subgroup analyses will provide further insights to factors affecting the association between VI and suicide risk.Trial registrationSystematic review registration: PROSPERO CRD 42022325106.

The COVID-19 pandemic is not just a medical and epidemiological problem. In fact, its impact concerns numerous aspects of human life (such as social and the political-economic dimension). This review aims at highlighting some crucial and neglected aspects of the pandemic in order to include them into a more general framework for the understanding of the phenomenon. Accordingly, it is structured as follows. First, after e brief recap of COVID-19 onset, it is argued the so-called proximate causes of the pandemic, i.e., the mechanisms by which viruses infect their hosts and the patterns of spread of the resulting pathologies, are not enough for a more adequate understanding of it. Second, it is shown how possible solutions to the risk of an upcoming pandemic involve studying the ultimate causes of this phenomenon. This means understanding not only how COVID-19 has become a global issue but also why it was possible for this to happen. Next, it is argued that is urgent to go to the root of the possible conditions: thus looking at the ecological dimension of diseases, the role of microorganisms in evolution, up to rethinking the organization of health systems. Third, to keep these very different perspectives together entails the study of COVID-19 from the point of view of the relationships between biological entities in a purely systemic dimension. Fourth, special attention is given to the symbiotic perspective offered by the study of the microbiota. It is argued how this perspective on microbiota provides an innovative interpretative lens with which to analyze various aspects (from the immunological to the ecosystemic one) of the pandemic. In conclusion, it is claimed that this field of study could perhaps offer not only elements that will be useful to make the treatment and containment strategies of the pandemic effective in its mechanisms, but also may suggest innovative elements for the solutions about the deep reasons that have made COVID-19 a global issue.
